Background: The S2PLIT‐UG score has been recently published as a risk stratification tool for 1‐year all‐cause mortality among patients discharged after an acute decompensated heart failure (ADHF) event. This score stratifies ADHF patients into low, intermediate and high‐risk categories. It is calculated by combining 6 variables collected at admission including estimated glomerular filtration rate, uric acid, left ventricular ejection fraction, sodium, systolic blood pressure and the history of heart failure‐ related hospitalizations. The study aimed to determine if patients identified as high‐risk by the S2PLIT‐UG score have higher circulating levels of biomarkers associated with poor prognosis such as soluble suppressor of tumorigenicity 2 (ssT2), high‐ sensitivity cardiac troponin I (hs‐cTnI) and N‐ terminal pro b‐type natriuretic peptide (NT‐ proBNP). A secondary aim was to examine correlations of the S2PLIT‐UG score with the aforementioned biomarkers. Methods: A new validation cohort consisting of 96 patients hospitalized for ADHF and without acute coronary syndrome as an underlying culprit were consecutively included in the study during 2018‐ 2019. All patients underwent standard transthoracic echocardiography, laboratory analyses of peripheral blood, and had their S2PLIT‐UG score calculated with a high‐risk score being defined as having ≥4 points. Results: One‐ quarter of patients (25%, N=24) in analyzed cohort were identified as a high‐risk while 75% of patients (N=72) were non‐high risk according to the S2PLIT‐UG stratification system. Out of those designated as non‐high risk, vast majority were low risk (70.8%, N=51) and 29.2% (N=21) were intermediate risk. High risk group did not significantly differ from non‐high risk group of patients in terms of baseline characteristics including age (p=0.161), body mass index (p=0.437), sex distribution (p=0.637), smoking (p=0.626), dyslipidemia (p=0.898), diabetes mellitus (p=0.286) and presence of atrial fibrillation (p=0.288). Introduction: The Heart Failure (HF) surveys and registries have been developed with the intention to characterize this patient population and to improve diagnosis, treatment, and prognosis according to established guidelines of the European Society of Cardiology (ESC). We aimed to provide an overview of past, current, and future European HF registries and to integrate clinical information obtained from these registries. Patients and Methods: The data of EuroHeart Failure Survey (EHFS), ESC-HF Pilot Survey (ESC-HF Pilot), ESC HF Long-Term Registry (ESC- HF-LT-R) and Croatian Heart Failure Registry (CRO-HF-R) were analyzed. Results: The results from EHFS published more than a decade ago suggested that specific clinical investigations for patients with suspected HF such as echocardiography were not performed as frequently as recommended by the ESC. Several years later, the ESC-HF Pilot Survey that encompassed 136 cardiology centers from 12 European countries showed that ischemic etiology of HF was reported in about half of the patients while acute decompensation of HF was the most common clinical profile of acute HF. More recently, results from ESC-HF-LT-R showed that the presence of diabetes markedly increased the risk of 1-year adverse events in HF outpatients and that diabetes treatment was suboptimal. Likewise, the chronic obstructive pulmonary disease frequently coexists in HF and is associated with an increase in all-cause and HF-related hospitalizations during the 1-year follow-up. Data acquired from the CRO-HF-R revealed that disease presentation in HF might differ between women and men. For instance, women had a significantly higher proportion of HF with preserved left ventricular ejection fraction and had higher lipid and uric acid levels compared to men, while men had significantly lower hemoglobin levels and reduced left ventricular ejection fraction compared to women. Finally, latest established version of European HF registry, HF III Registry will continue to gather relevant information about this patient population in modern clinical practice. Conclusion: Registries are an important source of information about characteristics, diagnosis, treatment, and prognosis of HF patients and as such will continue to provide relevant clinical information in the future.

Cilj: Provedena je opservacijska studija, koja je uključivala bolesnike koji su liječeni zbog hiperlipidemije u središnjoj/istočnoj Europi i Izraelu, sa svrhom procjene trenutnog liječenja povišenih vrijednosti LDL kolesterola (LDL-C). Ovdje predstavljamo podatke iz hrvatske populacije. Bolesnici i metode: U studiju su uključeni punoljetni bolesnici, koji su primali terapiju za snižavanje lipida (LLT) i kontrolirali su se kod specijalista (kardiologa/dijabetologa) ili liječnika obiteljske medicine u Hrvatskoj. Podaci su prikupljeni retrospektivno tijekom 12 mjeseci iz bolesničke medicinske dokumentacije. Bolesnici su podijeljeni s obzirom na kardiovaskularni rizik u dvije grupe: grupu s vrlo visokim rizikom (VHR) prema europskim smjernicama ili ekstremno visokim rizikom (EHR) prema nedavnim kriterijima američke udruge kliničkih endokrinologa (AACE). Rezultati: U okviru 4 centra uključeno je 89 bolesnika (svi bolesnici su dobivali terapiju za snižavanje lipida kao mjeru sekundarne prevencije: VHR 41/EHR 48)(Tablica 1). Svi su primali statine, kao monoterapiju: 38 VHR (92, 7%)/40 EHR (83, 3%) ili kombinaciju statina s fibratima (VHR 4, 9%/EHR 14, 6%) ili ezetimibom (VHR 2, 4%/EHR 2, 1%). Oko 70% bolesnika u obje podskupine liječeno je visokim dozama statina (atorvastatin 40-80 mg ili rosuvastatin 20-40 mg/dan). Tijekom opservacije u VHR grupi medijan vrijednosti LDL-C (Q1 ; Q3) iznosio je 2, 5 (2, 0 ; 3, 8) mmol/L u prvoj i 1, 9 (1, 6 ; 2, 4) mmol/L u zadnjoj posjeti. U grupi bolesnika s EHR odgovarajuće vrijednosti LDL-C (Q1 ; Q3) bile su 2, 4 (1, 7 ; 3, 7) mmol/L i 2, 1 (1, 5 ; 3, 1) mmol/L. Background: Pregnancy in heart transplant (HT) recipients with episodes of rejection or positive donor specific antibodies (DSA) is discouraged by all recommendations due to higher risk of complications. Although there is paucity of data for DSA trend during pregnancy in HT recipients, there is evidence of improvement of symptoms during pregnancy in some autoimmune diseases
1-3 . Case report: 23-year-old female who underwent HT due to post-myocarditis cardiomyopathy in June 2014, reported unplanned pregnancy in June 2022. She had a history of acute cellular (3R/3B) and humoral rejection (DSA major histocompatibility complex (HLA) class I, specificity A11, A30, B13, B35 and HLA class II, specificity DR3, DR15, DR51, DR52, DQ2, mean fluorescent intensity (MFI) 1000-13 300) in August 2017 which required mechanical circulatory support due to severe heart failure with multiorgan damage. She underwent pulse corticosteroid treatment, and 12 cycles of plasmapheresis with administration of rituximab, intravenous immunoglobulins, and thymoglobulin. After that she had persistent positive DSA, predominantly DQ2 (MFI up to 12200), and because of that was on quadruple immunosuppression (tacrolimus, mycophenolate (MMF), everolimus, prednisone) from October 2019 to July 2020 when MMF was discontinued due to gastrointestinal side effects. She had one more cellular rejection episode (2R/3A) in March 2022 which was treated with pulse corticosteroids. During pregnancy she was on tacrolimus, everolimus, and prednisone combination with closely monitoring of immunosuppressants concentrations. Echocardiography controls showed normal left ventricle function and mildly reduced right ventricle function, NT-proBNP was slightly elevated (493-823 ng/L) and DSA were in a downward trend which is shown in Figure 1. At 32+3 weeks of pregnancy, she was hospitalized due to early labor. The following day male baby was delivered by Caesarean section due to pathological cardiotocography (Apgar score 8/9). After delivery DSA remained weakly positive. Conclusion: Although pregnancy in HT recipients with history of rejection and positive DSA is discouraged, positive outcome is possible with close monitoring of multidisciplinary team. Introduction: Pulmonary vein isolation (PVI) is an established procedure to treat atrial fibrillation (Afib) but it increases periinterventional thromboembolic burden¹ . Literature provides ambiguous results about ablation effects on different hemostatic markers. The aim of this study was to assess the difference between two used PVI methods (cryoablation and radiofrequency ablation (RFA)) in postinterventional platelet reactivity (PR). Patients and Methods: We analyzed PR in 168 consecutive patients undergoing PVI due to Afib in our institution using Multiplate function analyzer. Blood samples for PR measurements were drawn prior to the procedure and the on the following morning. In total, 123 and 45 patients underwent cryoablation and RFA, respectively. There was no difference in demographics and baseline platelet parameters between the groups (Table 1). ASPItest, ADPtest and TRAPtest were used as assays for the quantitative in vitro determination of PR triggered by arachidonic acid, adenosine diphosphate and thrombin receptor activating peptide-6, respectively. Introduction: The USPSTF has recommended the use of preventive aspirin in all subjects aged 55-79 years without known heart disease when the potential benefit of a reduction in myocardial infarction outweighs the potential harm of bleeding. However, some studies have suggested that for women, the risks of aspirin may outweigh the benefit. Hypothesis: To investigate whether the risk of initial presentation of coronary heart disease with STEMI is lower in aspirin users and, if so, to assess the role of age and concomitant medications and procedures in the reduction of risk. Methods: We analyzed clinical characteristics and outcomes of patients without preexisting CVD that were enrolled in 41 hospitals referring data to the ISACS-TC registry (NCT01218776) from January 2010 to January 2018. Primary end points included rates of STEMI at clinical presentation and adjusted 30-day mortality. For the safety analysis, hemorrhagic events, consisting of gastrointestinal bleeding, hemorrhagic stroke, and bleeding from any other sites, were also analyzed. Estimates were obtained using logistic regression models. Results: There were 1098 subjects with a mean (SD) age of 66 (11.3) years in the aspirin group and 8978 subjects with a mean (SD) age of 60.3 (12.0) years in the no-aspirin group included in the study. After multivariable adjustment for age, sex, kidney function and risk factors, aspirin therapy reduced clinical presentation with STEMI in the protocol cohort (OR, 0.52; 95% CI, 0.46-0.60) with no heterogeneity of efficacy in subgroup analyses stratified by each of these factors (all interaction P>0.05). Similar treatment effects with aspirin therapy were found when concomitant medications were included as dummy variables in sequential logistic analyses (OR, 0.62 for statins, 0.59 for ACE inhibitors and 0.61 for beta-blockers) (Figure 1.). Bleeding events occurred in 8 patients (1.4%) in the aspirin group and 88 (2.4%) in the no-aspirin group (P=0.12), and the incidence of hemorrhagic stroke was not different between groups (P=0.28). Conclusions: Aspirin reduces the risk for STEMI without increasing risk for gastrointestinal bleeding in women and men in a primary prevention setting. These data support the routine use of aspirin as recommended by the USPSTF.

Uvod: Utjecaj koronarnih intervencija na kratkoročno i dugoročno preživljavanje bolesnika s akutnim infark-tom miokarda bez elevacije ST- segmenta (NSTEMI) nije se pokazao jednoznačnim.1 Cilj ovog rada je usporediti unutarbolničke te ishode nakon jedne godine u bolesnika sa NSTEMI i učinjenom perkutanom koronarnom interven-cijom (PCI) u prvih 24 sata (PCI ≤ 24h), s PCI nakon (PCI > 24 h) te onih s optimalnom medikamentnom terapijom (OMT).Metode: U razdoblju od siječnja 2012. do listopada 2017. u Hrvatski ogranak registra ISACS-CT (NCT01218776) uključeno je 1.898 bolesnika, od kojih je 36% imalo NSTEMI (n = 685), a njih je 675 bilo liječeno jednom od tri istraživane terapije. Jednogodišnje praćenje bilo je dostupno u 217 (32%) bolesnika.Rezultati: 57% bolesnika sačinjava PCI ≤ 24 h skupinu (n = 386), 14% (n = 95), (PCI > 24 h), a 29% (n = 194) OMT skupinu. Bolesnici na OMT-u su bili značajno stariji, opterećeniji ko-morbiditetima, hospitalizirani dulje i s duljim odmakom od početka simptoma te s nižom vrijednosti ejekcijske frakcije (EF) pri otpustu (tablica 1). Unutarbolnički mor-talitet nije se razlikovao u tri skupine (PCI ≤ 24 h ; PCI > 24 h ; OMT - 2, 5% ; 3, 5% ; 4, 6% ; p = 0, 220). Bolesnici s PCI ≤ 24 h imali su najveću učestalost reintervencije primjenom PCI (PCI ≤ 24 h ; PCI > 24 h ; OMT: 15% ; 3% ; 0% ; p = 0, 008). Nije bilo značajne razlike u ostalim značajnim kardiovaskularnim događajima - hospitalizacije zbog akutnog infarkta miok-arda s elevacijom ST-segmenta (p = 0, 998), ponovnog NSTEMI-ja (p = 0, 728), aortokoronarnog premoštenja (p = 0, 094), srčanog zatajivanja (p = 0, 554) niti moždanog udara (p = 0, 309). U bolesnika s dostupnim jednogodišnjim praćenjem (slika 1) vidljiva je značajna razlika u preživljavanju (PCI ≤ 24 h ; PCI > 24 h ; OMT: 3, 3% ; 8, 3% ; 19, 4% ; p = 0, 005). Univarijatna regresija pokazala je da PCI ≤ 24 h značajno smanjuje jednogodišnju smrtnost u odnosu na OMT (OR 0, 14, 95% CI 0, 04-0, 51, p = 0, 03), no učinak se gubi nakon podešavanja za dob, EF te kroničnu bubrežnu bolest (KBB), (p = 0, 077), a u navedenom modelu su EF i KBB (OR 0, 95, 95% CI 0, 90-0, 99, p = 0, 049 i OR 6, 31, 95% CI 1, 67-23, 8, p = 0, 007) jedini značajni čimbenici koji utječu na preživljavanje.Zaključak: Postoje značajne razlike bolesnika sa NSTEMI liječenih PCI ≤ 24 h, PCI > 24 h i OMT. Modalitet liječenja nema utjecaja na unutarbolničko preživljavanje, a OMT skupina bolesnika imala je znatno lošije jednogodišnje preživljavanje zbog prediktora jednogodišnje smrt-nosti – EF i KBB

Background and aim: Women have poorer outcomes in acute coronary syndrome (ACS) due to older age, comorbidities, atypical presentation and delay in admission. The aim is to study gender differences in PCI outcomes in the Croatian branch of the ISACS-CT registry. Methods: From January 2012 to February 2017, 1808 patients were enrolled in the Croatian branch of the ISACS-CT, 845 (46%) presenting with STEMI. PCI of the culprit lesion only was performed in 70% (n=594) of the STEMI patients, treatment of two lesions in one procedure in 12% (n=97), and more than two lesions in 2% (n=19). 131. Osobitosti liječenja arterijske hipertenzije bolesnika sa zatajivanjem srca

Arterijska hipertenzija (AH) najznačajniji je promjenjivi čimbenik rizika za razvoj zatajivanja srca. Za pravilno liječenje AH-a u bolesnika sa zatajivanjem srca potrebno je utvrditi koji je tip zatajivanja prisutan. Dobro su etablirane smjernice za liječenje AH-a u bolesnika sa zatajivanjem srca sa smanjenom istisnom frakcijom: antihipertenzivni bi režim tih bolesnika trebao uključivati ACE-inhibitor ili blokator receptora angiotenzina, β-blokator i antagonist mineralokortikoidnih receptora. Te lijekove koji ujedno modificiraju tijek bolesti (zatajivanja srca) važno je titrirati do maksimalnih podnošljivih doza. Ako unatoč tomu perzistiraju simptomi srčanoga zatajivanja, valjalo bi umjesto ACE-inhibitora uvesti kombinaciju blokatora receptora angiotenzina i inhibitora neprilizina. Optimalnom farmakoterapijom, odnosno intenzivnim liječenjem AH-a nastoji se smanjiti simptome i rizik za hospitalizaciju bolesnika zbog srčanoga zatajivanja, usporiti ili obrnuti proces patološkoga remodeliranja miokarda i smanjiti smrtnost.

Ukupno smo analizirali 16 ponovljenih procedura, od čega je 7 bilo nakon prve procedure kriobalonom (CB grupa) te 9 nakon ablacije RF energijom (RF grupa). Većina bolesnika bili su muškarci (75%) prosječne dobi 62 ± 7, 8 godina. Uglavnom su bolovali od paroksizmalne (75%), a ostatak je imao perzistentnu FA. Srednja veličina lijevog atrija bila je 44, 6 ± 4, 8 mm, a srednja ejekcijska frakcija lijeve klijetke iznosila je 61, 3 ± 7, 4%. Nije bilo značajnih razlika između osnovnih karakteristika promatranih grupa.

Učinjena je r etrospektivna a naliza svih VA ablacija od početka programa u Kliničkom bolničkom centru Zagreb. Sve procedure provedene su elektrofiziološkoj (EP) sali uz lokanu anesteziju, dok je manjina (13, 1%) je bila podvrgnuta općoj anesteziji. Za 3D navigaciju korišten je CARTO 3 sustav, a u zadnje dvije godine korišteni su multipolarni kateteri za „high density“ mapiranje.U kupno j e učinjeno 3 8 a blacija VA u 3 6 b olesnika (31 muškarac, dob 59, 1±12, 3 godina), od čega 55, 2% u ishemijskoj BS, a 44, 7% u neishemijskoj BS. 3 bolesnika bolovalo je od BBRVT (bundle branch reentrant ventricular tachycardia). Prosječna LVED bila je 36 ± 11, 8% uz LVIDd od 63, 4 ± 10, 5 mm. Prosječno trajanje procedure bilo je 297, 1±186, 6 min uz dijaskopiju od 24, 6 ± 18, 1 min

134. Clinical presentation of heart failure in female patients-results from CRO-HF Registry

Introduction: Heart failure (HF) could be different in a specific population. The aim of the study was to analyze the clinical presentation of HF in female patients according to results from CRO-HF Registry. Methods and Results: We analysed 2203 hospital HF patients (data based on CRO-HF Registry): 1175 (53.3%) males (m) and 1028 (46.7%) females (f) ; median age 76 y. One third patients suffered of acute HF. Preserved left ventricular function (LVEF = 50%) was recorded in 37.8% patients. Females had frequently preserved LVEF ( = 50%) (f- 49.3%, m-29.3%, p < 0.001) and males reduced LVEF ( < 50%) (m- 70.7%, f-50.7%). History of hypertension (AH) had 67.5% patients, diabetes 34.4%, myocardial infarction 22.7%, renal failure 19.2%, COPD 17.3%, stroke 16.5% and atrial fibrillation 53.7% patients. In males, comparative to females, important disease was COPD (m-19.7%, f-14.7%, p = 0.009), renal failure (m-23%, f- 14.8%, p < 0.001), diabetes (m-36.1%, 32.6%, P = 0.08), stroke (m-17.1%, f- 15.9%, P = 0.547) and smoking habit (m-14.8%, f-6.4%, p= < 0.001). The frequently precipitating factors of HF were: arrhytmiae (m-56.1, f-58.3), valvular diseases (m- 67.3%, f-67%) and infections (m- 18.7, f-20.4%). Important "trigger" in females was hypertension (f-58.7%, m-52.7%, P = 0.009), arrhytmiae (f- 58.3%, m-56.1%), infections (f- 20.4%, m- 18.7%), and acute coronary syndrome (ACS) in males (m- 22.1%, f-17%, P = 0.010). Females had often atrial arrhytmiae (f-92.2%, m-87.6, P = 0.010). Lower values of haemoglobin were recorded in 51.9% patients, higher creatinine had 46.8%, ALT 29.8%, cholesterol 32.7%, triglycerides 31.9%, uric acid 79.3%, and hyperglycaemia 99.8% patients. In females, we recorded higher values of ALT (f-33%, m-27%, P = 0.012, C = 0.066), cholesterol (f- 36.8%, m- 29.1%, P = 0.009), triglycerides (f- 36.1%, m- 28.3%, P = 0.014), uric acid (f-82.9%, m-76.4%, P =0.007), and males lower haemoglobin levels (m-58%, f-44.8%, p = 0.001). Hospital mortality was lower in males than in females (m- 12.6%, f- 14.4%). There were no significant differences in HF treatment according to gender. Conclusions: Clinical presentation of HF could be different in female patients, according to CRO-HF Registry. Females have frequently preserved LVEF, higher values of ALT, cholesterol, triglycerides, and uric acid, and males have lower values of LVEF and hemoglobin. Hypertension is an important "trigger" of HF in females and ACS in males. HF treatment should be adapted to clinical presentations of HF according to gender.

135. Učinkovitost i sigurnost individualizirane primjene antagonista P2Y12 receptora temeljem agregometrije u odnosu na fiksne doze u bolesnika nakon akutnog infarkta miokarda s elevacijom ST- segmenta

Uvod: Postoji niz studija koje su ukazale na postojanje povišene ostatne trombocitne aktivnosti (mjerene testovima trombocitne agregacije) u pojedinih bolesnika liječenih klopi-dogrelom ovisno o stupnju biotransformacije lijeka.1-3 Također je pokazano kako se povišenjem doze klopidogrela može nadvladati navedena povišena aktivnost, a time ujedno i utjecati na kliničke ishode liječenja. Ovdje donosimo rezultate skupine bolesnika koji su dovršili jednogodišnje praćenje, a koji su uvršteni u randomiziranu kliničku studiju ispitivanja učinkovitosti i sigurnosti individualizirane primjene antago-nista P2Y12 receptora temeljem nalaza testova agregometrije u odnosu na fiksne doze tikagrelora u bolesnika nakon akut-nog infarkta miokarda s elevacijom ST- segmenta (STEMI).Bolesnici i metode: U analizu je uključeno 51 uzastopnih bolesnika (9 žena, srednja dob 58, 9 godina) nakon preboljelog STEMI-a. Svi su bolesnici tijekom prvih mjesec dana liječeni standardnom dozom tikagrelora uz acetilsalicilatnu kiselinu, a potom su randomizirani u dvije skupine. Prva skupina je nastavila postojeću antiagregacijsku terapiju tijekom godine dana, a u drugoj je skupini tikagrelor zamijenjen klopidogre-lom uz individualnu titraciju doze ovisno o nalazima agrego-metrije mjerenih Multiplate® analizatorom. Primarni ishod studije bila je incidencija velikih kardiovaskularnih događaja (kardiovaskularna smrt, infarkt miokarda, moždani udar i ponovna revaskularizacija), a sekundarni učestalost velikih i malih krvarenja definiranih BARC klasifikacijom.Rezultati: Tijekom praćenja nije zabilježen niti jedan prima-rni ishod kao niti jedan slučaj velikog krvarenja. Zabilježena je statistički značajno manja incidencija malih krvarenja u skupini bolesnika liječenih individualiziranom primjenom P2Y12 antagonista (p=0, 027) što odgovara 4, 8 puta većoj šansi razvoja krvarenja u bolesnika liječenih fiksnom do-zom tikagrelora u odnosu na bolesnike liječene individuali-ziranim pristupom (OR 4, 8 ; 95% CI 1.118 to 20.611 ; p=0, 035).Zaključak: Rezultati ukazuju kako bi individualizirana primje-na P2Y12 antagonista mogla predstavljati jednako učinkovitu, a sigurniju opciju liječenja bolesnika nakon preboljelog STE-MI-a u odnosu na terapiju fiksnim dozama tikagrelora.

Uvod: Žene s akutnim infarktom miokarda s elevaci- jom ST-segmenta (STEMI) imaju lošije 30-dnevno preživljenje od muškaraca.1 Cilj je istražiti spolne razlike u unutarbolničkoj smrtnosti i smrtnosti nakon jedne go-dine praćenja u Hrvatskom ogranku ISACS-CT registra (NCT01218776).Bolesnici i metode: U razdoblju od siječnja 2012. do lis- topada 2017., uključeno je 1898 bolesnika: 46% (n=881) sa STEMI-jem, 36% (n=685) s akutnim infarktom miokarda bez elevacije ST-segmenta (NSTEMI) te 18% (n=332) s nestabilnom anginom (NA). Praćenje je provedeno na 33% (n=630) bolesnika, od kojih su 44% (n=275) STEMI, 34% (n=217) NSTEMI, te 22% (n=138) s NA.Rezultati: Pri prijemu žene su bile starije, s više komorbid- iteta te hospitalizirane s duljim vremenskim odmakom od početka simptoma (slika 1). Nije bilo spolnih razlika u padu ejekcijske frakcije (EF) < 40% tijekom inicijalne hospitalizacije. Žene sa STEMI-jem imale su znatno lošije unutarbolničke ishode (tablica 1). Uvod: S obzirom na starenje populacije u Europi, bit će sve više bolesnika starijih od 75 godina hospitaliziranih zbog akutnog koronarnog sindroma bez elevacije ST-seg-menta (NSTE-ACS). Unatoč navedenome, postoje samo ograničeni podaci o ishodima liječenja u ovih bolesnika, bilo u opservacijskim studijama, bilo u randomiziranim kliničkim istraživanjima.1 Htjeli smo istražiti može li rana (unutar 24 sata od hospitalizacije) perkutana ko-ronarna intervencija (PCI) poboljšati ishode u bolesnika starijih od 75 godina. 138. Changes in platelet reactivity during TAVI

Although, recent studies described changes in platelet reactivity in days following transcatheter aortic valve implantation (TAVI), precise time course of these changes has not been investigated. 26 consecutive patients with severe and symptomatic aortic stenosis, undergoing TAVI procedure in our institution were enrolled in research. Patients? perioperative clinical characteristics were collected from medical records. All patients who did not have chronic therapy with aspirin and clopidogrel, received loading dose of aspirin (300 mg) and clopidogrel (300 mg) one day before the procedure, followed by their standard maintenance doses (aspirin 100 mg, clopidogrel 75 mg) for the next three months. Patients? platelet reactivity was measured in five time points: just before induction of anesthesia (T1), after heparin administration (T2), 10 minutes after valve implantation (T3), 10 minutes after additional interventional procedures (if necessary) or 20 minutes after valve implantation (T4) and three days after the procedure (T5). Platelet reactivity was measured using impedance aggregometer (Multiplate® analyzer, Roche, Munich, Germany). It was measured in response to three platelet aggregation agonists: arachidonic acid (ASPItest), ADP (ADPtest) and thrombin receptor activating peptide-6 (TRAPtest). Results are presented in Units (U), which is calculated from the height and slope of aggregation curve and represents best overall measure of platelet reactivity. During TAVI procedure, all patients received unfractionated heparin at does 50-70 IU/kg with a target activated clotting time of 250-300 seconds. Mean patient age was 82.3 years with majority patients being male 57.5% (N=15). Mean valve area prior to procedure was 0.71±0.19 and mean transvalvular gradient 45.35±11.14 mmHg. All patients underwent successful TAVI procedure using either self-expandable (N=14, 53.8 %) or balloon-expandable valve (N=12, 46.2 %). Balloon dilatation following initial implantation was performed in 9 cases (34.6 %). Mean post-implantation gradient was 11.75±7.6 mmHg. Significant PVR, defined as more then mild, developed in 7 cases (26.9 %). Mean platelet reactivity before TAVI (T1) was 24.3±22.6 for ASPItest, 47.2±26.7 for ADPtest and 93.1±31.4 for TRAPtest. There was no significant difference between basal measurements and after administering unfractionated heparin (T2). Uvod: Važnost primjene statina u akutnom koronarnom sindromu (AKS) dobro je definirana, no optimalan vre-menski interval, posebice u prvih 24 sata nakon AKS-a, do sada nije u potpunosti istražen.1, 2 Cilj: Prikupljanje podataka o unutarbolničkoj smrtnosti i smrtnosti nakon 1- godišnjeg praćenja u Hrvatskom ogranku ISACS-CT (International Registry of Acute Coronary Syndromes in Transitional Countries) registra. Bolesnici i metode: Provedena je retrospektivna opser-vacijska studija u razdoblju od siječnja 2012. do listopada 2017. godine, u koju je uključeno 1.898 bolesnika: 46, 4% (n = 881) bolesnika s akutnim infarktom miokarda s elevaci- jom ST-segmenta (STEMI), 36, 1% (n = 685) s akutnim in-farktom miokarda bez elevacije ST- segmenta (NSTEMI) te 17, 5% (n = 332) bolesnika s nestabilnom anginom pektoris (NA). Praćenje je provedeno na 33% (n = 630) uzorka, od kojih su 43, 7% (n = 275) STEMI, 34, 4% (n = 217) NSTEMI te 21, 9% (n = 138) bolesnici s NA. Rezultati: Ispitivanu skupinu predstavljali su bolesnici u kojih su statini primijenjeni u prvih 24 sata nakon AKS-a (n = 1.734, 93% ; 24 bolesnika nedostaje), dok su bolesnici koji su liječeni statinima nakon 24h te oni koji nisu liječeni statinima tijekom hospitalizacije, pred-stavljali kontrolnu skupinu. Između definiranih skupina nije bilo značajne razlike u dobi, spolu, indeksu tjelesne mase, ejekcijskoj frakciji lijeve klijetke (LVEF) izmjer-enoj tijekom hospitalizacije, anamnestičkim podacima o pušenju, šećernoj bolesti, arterijskoj hipertenziji ili kroničnom zatajivanju srca, tijekom unutarbolničkog i 1-godišnjeg praćenja (tablica 1). Ukupna unutarbolnička smrtnost iznosila je 4%, slična smrtnosti u skupini bole- snika liječenih statinima unutar prva 24h nakon AKS (3%), dok je unutarbolnička smrtnost u kontrolnoj skupini iznosila 18% (p < 0.001). Rizik za unutarbolničku smrtnost bio je značajno veći u kontrolnoj skupini bolesnika (OR 7.32, 95% CI 4.37-12.27, p < 0.001), dok se rizik za pojavu primarnog ishoda nakon 1-godine praćenja nije značajno razlikovao između podskupina u univarijabilnoj regresijskoj analizi. Starija dob (OR 1.1, 95% CI 1.06-1.20, p < 0.001), veća razina kreatinina (OR 1.0, 95% CI 1.005-1.012, p < 0.001), niža LVEF (OR 0.90, 95% CI 0.86-0.94, p < 0.001) te izos-tanak rane primjene statina (OR 3.6, 95% CI 1.0-13.10, p = 0.05) povezani su sa značajno višim rizikom za unutarbolničku smrtnost u multivarijabilnom regresijskom modelu (tablica 2).Zaključak: Primjena statina u prvih 24 sata nakon AKS-a značajno sma-njuje rizik za unutarbolničku smrtnost, no pozitivan utjecaj rane primjene statina nije značajan nakon jedne godine praćenja.

Uvod: Trombociti ( Trc) s u p osrednici u s vim procesima ključnim za promjene cirkulacije u plućnoj hipertenziji (PH): vazokonstrikciji, upali, zgrušavanju i remodeliranju krvožilja. Niz istraživanja je potvrdio važnost uloge Trc u patogenezi primarne PH.1-3 M eđutim, i ako j e r iječ o mnogo učestalijoj dijagnozi, patogeneza sekundarne PH posljedične bolestima srca je nedovoljno istražena. Cilj ovog istraživanja je ukazati na važnost Trc u nastanku PH uzrokovane kroničnim sistoličkim popuštanjem srca (CSHF). Bolesnici i metode: Aktivnost Trc je mjerena u 160 bolesnika (57±10 godina ; 65% muškaraca) s CSHF, hospitaliziranim u Kliničkom bolničkom centru Zagreb od listopada 2011. do listopada 2016. godine. Svi bolesnici su bili kandidati za napredne metode liječenja, zbog čega im je učinjena desnostrana kateterizacija srca. Po završetku invazivnog mjerenja hemodinamskih parametara (uključujući tlak u plućnoj arteriji- PAP i test vazoreaktivnosti alprostadilom), uzimao se manji uzorak krvi iz plućne arterije. Multiplate uređajem se odredila agregabilnost Trc inducirana acetil-salicilnom kiselinom (ASPI), adenozin difosfatom (ADP), kolagenom (COL) ili trombin receptor aktivirajućim peptidom-6 (TRAP). Rezultati: Ishemijska (ICMP) i dilatacijska (DCMP) kardiomiopatija su bile najčešći uzroci CSHF (45% vs 43%). Očekivano, svim bolesnicima je izmjeren snižen srčani indeks te je ustanovljena PH uzrokovana povišenim tlakom punjenja lijeve klijetke (PCWP) (tablica 1). Bazične vrijednosti testova agregacije su pokazale reduciranu aktivnost Trc, uz značajno niži ASPI u bolesnika s ICMP (p < 0, 001, zbog liječenja acetilsalicilatnom kiselinom). Vrijednosti COL testa su značajno korelirale s PAP i PCWP (slika 1). Test vazoreaktivnosti je učinjen u 26% bolesnika te je bio pozitivan u 53% slučajeva (tablica 2). Promjene u PAP i PCWP su značajno korelirale s promjenama u ASPI i ADP testovima (slika 2). ZAKLJUČAK: Uloga Trc se doima važnom u patogenezi sekundarne PH uzrokovane CSHF. Dijagnoza protutijelima-posredovanog odbacivanja srčanog presatka (antibody-mediated rejection, AMR) temelji se na imunopatološkom nalazu uz potporu kliničkom slikom i dokazom donor-specifičnih protutijela (DSA). AMR je kontinuum od asimptomatske pojave DSA, preko supkliničkog odlaganja komplementa bez histoloških promjena do konačnog popuštanja presatka. Čini se da i supklinička AMR nosi lošiju prognozu. DSA su pokazatelj aloimunizacije i povezana su s popuštanjem, odbacivanjem i vaskulopatijom presatka. Iako nije jasna važnost izoliranog porasta DSA u ranom ili kasnom poslijetransplantacijskom periodu, njihovo liječenje dolazi u obzir.1, 2 Retrospektivno smo od 2012. godine analizirali 193 transplantirana bolesnika, kada smo postupno uveli imunopatološku analizu bioptata i određivanje DSA u serumu. Kombinirajući različite kriterije (patološki, klinički i serološki, tj. pozitivna DSA), AMR smo dijagnosticirali u 12 bolesnika (6, 2%). Četvrtina bolesnika se prezentirala kardiogenim šokom. Kombinacija patološkog i kliničkog kriterija bila je pozitivna u 17%, patološkog i serološkog kriterija u 25%, kliničkog i serološkog u 25%, dok je 33% bolesnika imalo pozitivna sva tri kriterija. Medijan vremena od transplantacije do AMR je iznosio 2, 63 godine (0, 7-5, 9). Medijan dobi bolesnika s AMR je bio 35 (17-62), a 75% su bili muškarci. Svi bolesnici s AMR su bili DSA pozitivni, osim dvoje u kojih se DSA nisu mjerila. 70% ih je imalo klasu I, a 30% obje klase anti-HLA DSA. Najčešći terapijski pristup uključivao je: steroidni puls (92%), plazmaferezu (75%), intravenski imunoglobulin (58%) i rituksimab (58%). Antitimocitni globulin, kao i bortezomib smo dali samo po jednom bolesniku. ECMO (izvantjelesna membranska oksigenacija) je postavljen u bolesnika u kardiogenom šoku. Jednogodišnje preživljenje iznosilo je 83%. Jedan je bolesnik uspješno retransplantiran nakon desenzitizacije. Od 193 bolesnika, DSA smo testirali u njih 97. Pozitivnih je bilo 24%, od kojih je 17% bilo pozitivno na klasu I, 65% na klasu II i 17% na obje klase) (slika 1). Premda učestalost AMR u literaturi varira zbog različitih dijagnostičkih kriterija i razlika u načinu probira, naš rezultat je usporediv. Nažalost niti dijagnoza niti terapija AMR nisu dobro standardizirane. Potrebna su veća multicentrična klinička istraživanja da ispitaju različite dijagnostičke i terapijske pristupe.

Prikaz slučaja: 18-godišnjakinja je hospitalizirana zbog popuštanja srca tri godine nakon transplantacije srca. Na ehokardiografiji su obje klijetke bile izrazito zadebljanih stijenki i reducirane sistoličke funkcije (ejekcijska frakcija lijeve klijetke, LVEF 30%). Nakon hitne biopsije endomiokarda (Bx) primjeni se pulsna terapija steroidima. Zbog razvoja kardiogenog šoka morali smo postaviti vensko-arterijski ECMO (izvantjelesna membranska oksigenacija). Nalaz Bx pokazao je miješani tip akutnog odbacivanja presatka, tj. protutijelima posredovano odbacivanje stupnja pAMR 1 (I+) i stanično- posredovano odbacivanje stupnja 3R. Luminex® je potvrdio postojanje brojnih donor specifičnih protutijela (DSA) na HLA antigene klase I (A11, A30, B13, B35) i klase II (DR3, DR15, DR51, DQ2, DPA1*02) s maksimalnim MFI 13000 za anti-DQ2. Odmah je započeto liječenje plazmaferezom, intravenskim imunoglobulinima (IVIg) i antitimocitnim globulinom (ATG). Četvrtog dana na ehokardiografiji su obje klijetke bile normalne debljine stijenki i bolje sistoličke funkcije (LVEF 40%) te je bolesnica uspješno odvojena od ECMO. Krajem drugog tjedna primijenjen je rituksimab. Nakon dva tjedna na kontrolnoj Bx nije bilo stanično-posredovanog odbacivanja, dok je imunohistokemijski nalaz ostao pozitivan. Na koronarografiji su prikazane normalne arterije. Provedeno je još pet ciklusa plazmafereze i ponovljeno liječenje IVIg-om nakon čega je kontrolna ehokardiografija pokazala lijevu klijetku normalne veličine i funkcije (LVEF 60%) te desnu klijetku normalne veličine i blago reducirane funkcije. Kontrolna Bx ovaj put nije našla znakova niti stanično- niti protutijelima posredovanog odbacivanja. Sedam tjedana od početka liječenja došlo je do negativizacije svih DSA klase I i klase II, osim anti-DQ2 (MFI 6100) (slika 1). Bolesnica je nakon 12 mjeseci stabilno, bez znakova odbacivanja i pogoršanja funkcije presatka. Zaključak: Ovaj prikaz ukazuje na važnost primjene mehaničke cirkulacijske potpore u najtežim oblicima popuštanja srca u transplantiranih bolesnika čime „kupujemo“ vrijeme za čekanje na rezultate pretraga i učinak terapije (engl. bridge-to-decision, bridge-to-recovery). Kombinacijom steroida, plazmafereze, IVIg, ATG i rituksimaba djelovali smo na kompleksne imunološke mehanizme miješanoga, tj. stanično- i protutijelima-posredovanog akutnog odbacivanja srčanog presatka te, u konačnici, omogućili ne samo preživljenje, već i potpuni oporavak bolesnice

Uvod: Vaskulopatija presatka (CAV) je kronična komplikacija transplantacije srca (HTx) koja predstavlja izazov u liječenju zbog difuznog uzorka zahvaćanja koronarnih arterija. Teži stupnjevi bolesti nisu prikladni za revaskularizaciju, zbog čega retransplantacija često ostaje jedina mogućnost liječenja.1-3 Bolesnici i metode: Od ukupno 176 bolesnika u praćenju nakon HTx, učinjene u razdoblju između 2001. i 2015. godine, 129 bolesnika je podvrgnuto barem jednoj koronarografiji kojom je u 45 bolesnika dokazan CAV (te dodatno u dva bolesnika na temelju kliničkog i obdukcijskog nalaza). Prosječna dob iznosila je 51, 6 ± 12, 6 godina, 78 % bolesnika su bili muškog spola, a prosječno vrijeme praćenja 3 godine (IQR 2-6 godina). Pojavnost CAV-a evaluirana je koronarografijom i analizirana s obzirom na trajanje vremena ishemije (VI), rane koncentracije visokoosjetljivog troponina T (hsTnT) te stupnja staničnog odbacivanja (SO). Rezultati: Rane vrijednosti hsTnT (unutar 3 mjeseca poslije HTx) značajno su više kod produljenog VI (p = 0, 040), no nemaju prediktivni značaj za pojavnost CAV-a (p = 0, 529)niti učestalije SO. VI presatka ne korelira s učestalošću značajnog SO. Bolesnici s težim stupnjem CAV-a imali su značajno kraće preživljavanje od onih bez CAV-a ili s blažim/umjerenim oblicima bolesti (p = 0, 016) (slika 1). Učinjena je retrospektivna analiza podataka prikupljenih od začetka provođenja krioablacije FA u Kliničkom bolničkom centru Zagreb. Na početku programa ablacije FA redovito je korištena 3D rotacijska angiografija, no kasnije je prije proceduralno oslikavanje LA u potpunosti napušteno. Za ablaciju je korišten kriobalon od 28 mm putem jedne transeptalne punkcije te je provođena strategija jednostrukog zamrzavanja od 180 sekundi. Uvod: Unatoč mjerama intenzivnog l iječenja smrtnost je u starijih bolesnika s akutnom kardiološkom bolesti značajno visoka.1 C ilj o ve s tudije j e p roučiti č imbenike koji utječu na unutarbolničku smrtnost bolesnika starijih od 80 godina koji su hospitalizirani u intenzivnoj kardiološkoj jedinici. Bolesnici i metode: A nalizirali s mo p odatke b olesnika starijih od 80 godina koji su hospitalizirani u Jedinici intenzivne kardiološke skrbi Kliničkog bolničkog centra Zagreb u razdoblju od 1. siječnja 2015. do 21. prosinca 2017. godine. Rezultati: Ukupno je hospitalizirano 243 bolesnika starijih od 80 godina (13, 39% od ukupnog broja hospitaliziranih u tom razdoblju), a od toga je bilo 146 žena (60, 1%). Najčešća vodeća dijagnoza je bila akutni infarkt miokarda (n=107 ; 44%). Ostale karakteristike pacijenata su prikazane u tablici 1. Unutarbolnička smrtnost je iznosila 21, 4%. Rezultati ukazuju kako su primarna dijagnoza kod prijema, težina prezentacije vodeće bolesti i uporaba invazivnijih načina liječenja glavni predskazatelji smrtnosti starijih bolesnika u intenzivnoj kardiološkoj jedinici (tablica 2). Zaključak: Udio bolesnika starijih od 80 godina u ukupnom broju hospitaliziranih pacijenata u intenzivnim jedinicama nije zanemariv. Primjena metoda intenzivnog liječenja u bolesnika visoke životne dobi zahtijeva individualnu procjenu o samoj svrsishodnosti i ciljevima liječenja. Potreban je razvoj adekvatnih, po mogućnosti lokalno primjenjivih, modela za procjenu stanja ove osjetljive i značajne skupine bolesnika kako bi se racionalizirali resursi i optimiziralo liječenje teških akutnih kardioloških bolesti u osoba visoke životne dobi.
