- Abstract
Background and Objectives Midwives, as members of the health team, have an important role in providing health services at three levels of the care system, especially the health center, family, and community. Self-efficacy of clinical performance is an essential variable in providing quality services. The present study aimed to compare the self-efficacy of clinical performance of midwives and non-midwife healthcare workers working in Ahvaz health centers in Iran. Subjects and Methods This cross-sectional descriptive-analytical study was carried out on 115 healthcare workers (including 67midwives and 48 non-midwives) working in health centers in Ahvaz, Iran. Participants were selected based on the inclusion and exclusion criteria and completed the self-efficacy clinical performance questionnaire. Finally, the results were analyzed in two groups of midwives and non-midwife healthcare workers using an independent t-test or Mann-Whitney test and Chi-square test with the help of SPSS software (version 22). Results According to the analysis, midwives had the highest score of clinical self-efficacy in prenatal care, while non-midwives had the highest score in the field of children. In addition, in midwives compared to non-midwives, the self-efficacy score of clinical performance in all areas except children was significantly different, and in all areas, the mean score obtained by midwives was higher than that of non-midwives (P<0.001). Conclusion Given the higher rate of clinical performance in midwives compared to the non-midwives group, as healthcare providers in all areas of care provided to healthcare clients, health policymakers should consider this capability of midwives in planning related to health promotion. Aim: To identify the extent and type of evidence exploring the workplace experiences of nurses and midwives with disabilities. Design: Scoping Review. Data Sources: Four electronic databases were systematically searched in February 2022 to identify studies discussing the experiences of nurses and midwives with disabilities, published between 2012 and 2022. Google Scholar and Theses Global were also searched. Review Methods: All search results, irrespective of their format, were uploaded to Covidence to assist with the selection of evidence. Studies discussing the experiences of nurses and midwives with disabilities were included. Two reviewers conducted screening and data extraction independently, and a third reviewer resolved conflicts. Results: Searches identified 130 studies. After screening for title and abstract, full‐text review identified 23 studies for data extraction and analysis. Data were summarized and presented in tabular form under two categories (1) workplace experiences and (2) impact in the workplace. Results were presented in a descriptive, narrative form with accompanying tables. Conclusion: The nurse or midwife can be impacted significantly by disability. Currently, little is known about the workplace experiences of nurses and midwives with disabilities. Therefore, a better understanding of the experiences of the nurse and midwives with disabilities is important to increase diversity, equity, accessibility, and inclusion. Impact: Shortage of nurses and midwives worldwide highlights the need to understand why nurses and midwives with disabilities decide to leave the profession. Further research exploring the experiences of nurses or midwives with disabilities will help identify support requirements, develop frameworks for reasonable adjustment accommodations, and assist with future workforce planning. Introduction: International educational programs build cultural humility and safety skills in nursing and midwifery students; however, long-term outcomes of these programs are unclear. The purpose of this study was to explore the impact of international educational programs on nurses' and midwives' future professional practice. Method: Using grounded theory informed by Charmaz, 13 general nurses, two mental health nurses, three midwives, and four dual-qualified nurse/midwives across eight different countries were interviewed. Three categories evolved from the analysis. This article reports on the category Recognizing and adapting to cultural differences. Findings: Participants developed cultural safety and awareness from participation in programs extending into future practice. Experiencing and adapting to cultural similarities and differences, they developed culturally congruent practices many years after program completion. Discussion: International programs contributed to participants' professional practice. Positive and ongoing influences are important for employers to promote patient safety and culturally congruent quality care. 107. 'Do we need doulas...?' – Perspectives of maternity care managers on the role of doulas in Poland.

Aim: The purpose of this study was to analyse perinatal care managers' perspectives on the role of doulas in Poland and to consider how managers' perspectives might affect the opportunities for doulas to practice in individual hospitals. Design: This is a qualitative descriptive study. Method: The total of 17 hospitals was selected for the study. Semi‐structured interviews were conducted with 11 manager staff members. Results: Three groups of facilities were identified: '0' (n = 6) – refused to give interviews, 'A' (n = 8) – marginal experience in working with doulas, and 'B' (n = 3) – regular experience in working with doulas. The hospitals from Group A showed indifference towards working with doulas. Group B declared a positive attitude towards such cooperation. Attitudes towards doulas vary among executives and are related to the frequency of doula‐assisted births. Our results indicate factors that may influence the attitude of medical staff towards doulas and which may contribute to improve future initiatives meant to facilitate collaboration between midwives and doulas. 108. Job adjustment predictive factors of healthcare midwives in health system reform in Iran.

Background: Possessing sensitive and multiple responsibilities in the country's health system, particularly after the implementation of the health reform in Iran, midwives must be able to optimally perform their duties in their new job as healthcare providers. This study aimed to identify the factors that predict job adjustment for Iranian midwives working in healthcare. Methods: In this cross-sectional study, 310 midwives were recruited from 209 health centers in the Iranian province of West Azerbaijan using the census method and asked to complete research questionnaires. Data were collected using job adjustment, job satisfaction, and organizational commitment scales. SPSS version 25 was used to perform ANOVA and calculate multiple linear regression coefficients for data analysis. In addition, the AMOS software was employed for path analysis and the identification of predictive variables. Results: The mean age of the participants was 37.67 ± 7.1 years. Most participants (35.5%) were interested in their occupation as a midwife, and 27.1% were very interest. They had a moderate to strong tendency (76.1%) to remain in their new profession. In addition, 58.1% of participants experienced moderate job adjustment. For healthcare midwives, "desire to remain in the midwifery profession" and "organizational commitment" were significant predictors of job adjustment. "Desire to remain in the midwifery profession" directly affected midwives' job adjustment, while "interest in the new profession" had an indirect effect. Furthermore, "adequacy of income to expenses," "job satisfaction," and "organizational commitment" through the mediating role of "desire to remain in the profession" can, directly and indirectly, influence their job adjustment. Conclusion: To better prepare midwives for their role as healthcare providers, organizational managers should focus their efforts and plan primarily on providing incentives to increase the longevity of staying in the profession of midwifery increase job adjustment, job satisfaction, and organizational commitment, thereby improving the quality-of-service delivery. 109. The pandemic experiences of Ontario perinatal providers: a qualitative study.

Background: The COVID-19 pandemic has produced widespread disruptions for healthcare systems across Canada. Perinatal care in Ontario, Canada was subject to province-wide public health restrictions, reallocation of hospital beds and human health resources. To better understand the impacts of the pandemic on Ontario perinatal care, this study explored the perspectives of perinatal care providers about their clinical COVID-19 pandemic experiences. Methods: Semi-structured key informant virtual interviews were conducted between August 2021 and January 2022 with 15 Ontario-based perinatal care providers. Recorded interviews were transcribed, and thematic content analysis used to identify major themes and subthemes. Results: Participants were mainly women, practicing in Eastern and Central Ontario as health providers (obstetricians, nurses, midwives), allied regulated health professionals (social worker, massage therapist), and perinatal support workers (doula, lactation consultant). Major themes and subthemes were identified inductively as follows: (1) Impacts of COVID-19 on providers (psychosocial stress, healthcare system barriers, healthcare system opportunities); (2) Perceived impacts of COVID-19 on pregnant people (psychosocial stress, amplification of existing healthcare barriers, influences on reproductive decision making; minor theme- social and emotional support roles); (3) Vaccine discourse (provider empathy, vaccines and patient family dynamics, minor themes- patient vaccine hesitancy, COVID-19 misinformation); and (4) Virtual pregnancy care (benefits, disadvantages, adaptation of standard care practices). Conclusions: Perinatal care providers reported significant stress and uncertainty caused by the COVID-19 pandemic and evolving hospital protocols. Providers perceived that their patients were distressed by both the pandemic and related reductions in pregnancy healthcare services including hospital limits to support companion(s). Although virtual pregnancy care impaired patient-provider rapport, most providers believed that the workflow efficiencies and patient convenience of virtual care is beneficial to perinatal healthcare. 110. Two Pregnancy Care Models in Poland—A Descriptive–Comparative Study.

Care for a pregnant woman can take various organizational forms. The World Health Organization (WHO) recommends leaders increase research into health systems. The aim of this is to manage the healthcare system in such a way as to provide beneficiaries with access to high-quality services with limited financial resources. The study presented in this paper was conducted using a diagnostic survey on a group of 1642 Polish women in the traditional model of care (TM) and 3216 women in the Coordinated Care for Pregnant Women Program (CCP). Two research tools were used in this study. The first is a survey prepared by the National Health Fund, the second is an author's survey. The results indicate that most women (85%) receiving care under the CCP received effective pain management, compared to 67% under the traditional care model (p < 0.001). In the CCP, women were significantly more likely to receive midwife assistance in infant care (90%) than women in the traditional care model (60%) (p < 0.001). Significantly more CCP patients want to return to the same hospital for their subsequent childbirth (74%) than women in the traditional model of care (43%) (p < 0.001). In patients' opinion, the new CCP model is superior in meeting their needs and providing higher-quality services. However, educating women that an obstetrician-gynecologist and a midwife can manage their pregnancy is still necessary. In addition to the CCP program, special attention should be paid to available pain management during childbirth since the use of analgesia is still insufficient in Polish hospitals. Spirituality is an integral part of holistic care, but studies have revealed that the spiritual dimension of care is largely neglected in healthcare settings. This study evaluated midwives' perceptions of spirituality and spiritual care. The study is a cross-sectional design and was conducted from April 1 to June 30, 2018 in Aydın, Turkey. The research sample of the study contacted 101 midwives. The Spirituality and Spiritual Care Rating Scale were used to collect data. BACKGROUND: Breastfeeding is an important milestone in human life. The decision of a mother to breastfeed her child depends on the guidance, education and support she will recieve before and immediately after the parturition period and during the first weeks during breastfeeding. However, the efforts of professionals to support breastfeeding can be undermined by a number of factors, such as hospital practice, mother itself and social barriers. AIM: This review identify factors affecting breast-feeding mothers of twin infants, to reinforce and encourage them to breast-feed exclusively. RESULTS: Mothers who give birth to more than one infant often have difficulties in starting breastfeeding during their hospitalization and appropriate interventions and support are required. In addition, the birth of twin babies is physically and mentally more difficult compared to single birth and this is a factor against the onset of breastfeeding. CONCLUSION: Therefore, systematic support and training by midwives will ensure that the mother starts breastfeeding and sufficient milk production. Introduction and aim. In extraordinary situations, sexual/reproductive health services are very important for the health of the whole society, especially women and children. The aim of this study is to assess the impact of training provided on sexual health/reproductive health during extraordinary situations on the knowledge levels of nurses and midwives. Material and methods. The research was conducted using a single-group pre-test-post-test follow-up test measurement quasi-experimental design. It was completed between November 2021-June 2022 with 140 participants working in a province in Turkey. The data were collected with the "Descriptive Information Form" and the "Sexual Health and Reproductive Health Knowledge Assessment Form for Extraordinary Situations". Training was given with the Sexual Health/Reproductive Health Training Booklet for Extraordinary Situations. Results. It was found that the participants scored 20.82±4.47 on the Sexual Health and Reproductive Health Knowledge Assessment Form for Extraordinary Situations in the pre-test, 27.63±2.67 in the post-test, and 27.07±3.46 in the follow-up test. As a result of the training, it was determined that the difference between the scores they got from the Sexual Health and Reproductive Health Knowledge Assessment Form for Extraordinary Situations was due to the pre-test (p<0.05). Conclusion. In the study, the participants scored above the average in the pre-test, while they achieved significantly higher scores in the post-test and follow-up test. 114. Effect of simulation-based training workshop on obstetric emergency team collaboration and communication: a mixed study

Aims and objectivesTo explore the effects of simulation-based midwife training workshops and determine whether such a program can improve team collaboration and communication.BackgroundSimulation training improves communication, team cooperation, critical thinking, and situational awareness.DesignThis mixed study was conducted September 15–18, 2021.MethodsParticipants included 23 obstetricians and midwives who completed 2 days of simulation training, including communication, skills, teamwork, single technical operation, and scene running. The Clinical Teamwork Scale was used before and after the comparison, and the data were analyzed using a phenomenological analytic process.ResultsThe total team cooperation, transparent thinking, closed-loop communication, overall decision-making, clear responsibility, and leadership scores of the trainees were significantly higher after than before the training. The experience of attending a simulated training workshop can be divided into two themes: innovative ways of offering training and active learning. Three key themes emerged from each category: education combined with recreation; full participation in interactions; and teamwork and communication. (1) application of knowledge (2) dissemination, and (3) sublimation of knowledge.ConclusionThis study’s findings indicated a good experience and higher team cooperation score among midwives participating in simulation-based training in China, the value of our work is to show that the researched teaching methods, although published in other contexts, are also valuable in the Chinese context, suggesting that they will pass on the methods and concepts of the simulated training to others and change the current status of classroom teaching, which is its most meaningful practical training effect.Relevance to clinical practiceThese results imply that simulation-based midwife training for obstetric emergencies is required to improve the comprehensive ability of midwives to address obstetric emergencies, thereby improving maternal clinical outcomes.No patient or public contributionNeither patients nor the public were involved in this study, and the midwives and obstetricians voluntarily participated.

115. Examining the Challenges of Using Accompanying Midwife from the Perspective of Midwives and Pregnant Women Referring to Hospitals in Shahrekord

Background: The presence of accompanying midwives (doulas) in the delivery room always entails many challenges. This study was carried out to determine the challenges of attendance with training from the point of view of midwives and expectant mothers in selected hospitals of Chaharmahal and Bakhtiari province, Iran. Methods: The present study was a descriptive study on 70 midwives working in the delivery block, 120 pregnant mothers, and 20 trained attendants. The sampling of midwives and doulas was performed using the census method, and the sampling of pregnant mothers was conducted using the convenience method. The study tool included questionnaires made by the researcher about demographics and socioeconomics, therapeutics, and psychological challenges for the group of mothers and midwives. The face validity and content validity of the questionnaires were confirmed by midwifery professors and reproductive health specialists, and their reliability was confirmed by internal consistency. The data were analyzed using descriptive statistics with SPSS software (version 21). Results: In this study, 70 midwives working in selected hospitals of Chaharmahal and Bakhtiari province, 120 pregnant mothers, and 20 trained attendants were examined. Most of the pregnant women (n = 39, 32.5%) mentioned that the most important motivation for requesting a doula during pregnancy and labor was to overcome the fear of childbirth. Most of them (n = 101, 84.9%) also stated that they used doula services at the right time. Meanwhile, 54 women (45%) were familiar with doula services during pregnancy, and most of them (n= 67, 56.3%) stated that doula services can be useful and effective in all three stages of pregnancy, labor, and delivery. Conclusions: There are dissatisfactions and challenges regarding the support of doulas during pregnancy and childbirth. Therefore, it is recommended to provide the necessary training to healthcare providers, especially obstetricians, midwives, and mothers, regarding the importance and place of doula’s supportive care in this era. Additionally, doula activities should be integrated and promoted in the care of the health team.

Objective: This study aimed to assess the core competence of midwives in township hospitals through a self-assessment questionnaire. The relationship between professional identity and core competence and the factors influencing midwives’ core competence was also investigated. Method: Convenience sampling was conducted in 77 township hospitals in Ganzhou, Jiangxi Province, China, with 150 participants. The questionnaires were distributed online in November 2021. We conducted a descriptive data analysis, a correlation analysis of the two variables of professional identity and core competencies, and multivariate linear regression to analyse the influencing factors, including the sociodemographic information, the Midwife Core Competence Scale, and the Nurses’ Professional Identity Scale scores. Results: The mean score for the core competence was 206.43 (±37.45) out of 270. The highest score was for pregnancy care (3.97 ± 0.70) and the lowest was for newborn care (3.72 ± 0.78). Early detection of obstetric risk is the main competence of midwives to anticipate the mother’s inability to adapt during pregnancy. The early detection competence of midwives determines the success of obstetric risk management, preventing pain and even death of mothers and babies. This study aimed to analyze the effect of obstetric risk early detection training on improving the competence of midwives in basic health care facilities. The study employed a quasi-experimental pre-posttest design with a control group. The samples were selected based on inclusion criteria, encompassing independent practice midwives and public health center midwives. The study comprised 27 midwives in the treatment group and 27 in the control group. The determination of and control groups was conducted through simple random sampling. 118. Adherence to the national guidance on foods and drinks to limit or avoid during pregnancy in England: the PEAR Study

Abstract Objective: The National Health Service (NHS) England website provides guidance on foods/drinks to avoid or limit during pregnancy because of microbiological, toxicological or teratogenic hazards. The aims were to determine adherence and whether demographic characteristics were associated with adherence. Design: Cross-sectional study. Setting: Online survey of postpartum women resident in England during pregnancy. Participants: Recently, postpartum women resident in England during their pregnancy (n 598; median age 33 (IQR 30–36) years) completed an online questionnaire (April–November 2022). Questions included those on consumption of twenty-one food/drink items that the NHS advises pregnant women to avoid/limit. The study is part of the Pregnancy, the Environment And nutRition (PEAR) Study. Summary statistics were used to determine proportions adhering to the guidance. Adjusted logistic regression was used to model the associations of adherence with demographic characteristics. Results: Adherence was generally high (>90 % for eight of ten food/drink items to be avoided). However, among pre-pregnancy consumers, several items were not completely avoided, for example, 81 % (128/158) for game meat/gamebirds, 37 % (176/478) for cured meats and 17 % (81/467) for soft cheeses. Greater educational attainment (e.g. caffeinated soft drinks OR 2·25 (95 % CI 1·28, 3·94)), greater maternal age (e.g. oily fish 1·64 (1·05, 2·56)) and lower parity (e.g. caffeinated coffee 0.28 (0.11, 0.69)) were the most usual characteristics associated with adherence. Conclusion: Evidence of concerning levels of non-adherence for some food/drink items suggests a case for more education on some of the guidance, particularly for women with lower educational attainment, greater parity and greater maternal age. Further research on barriers to the implementation of the guidance is needed.

Background: Contribution of nursing is crucial for Universal Health Coverage and achieving Sustainable Development Goal 3. Tracer studies help evaluate accountability and demonstrate the impact of nursing educational institutions in a highly competitive job market. The study aimed at assessing the employability of nursing graduates, and perceived quality of the nursing program they pursued in a public tertiary institution in Ghana. Method: The study employed a descriptive cross-sectional survey. One hundred and ten (110) nursing graduates responded to a web-based survey. The data were gathered using a customized questionnaire based on the Tarlac State University Graduates Tracer Questionnaire. Data were collected via google forms, and analyzed using descriptive and inferential statistics. Results: Majority of the participants were females (n = 64, 58.2 %). Most of the participants had completed a Bachelor’s degree at the tertiary institution (n = 101, 91.8 %). A significant proportion of the participants were gainfully employed during the study period (n = 102, 92.7 %) with majority of them working in the public sector (n = 81, 79.4 %). The quality of the diploma or degree program obtained was rated good in most of the areas by over 50 % of the participants. There was no statistically significant association between the highest educational qualification of participants and their perceptions on the quality of the nursing program. Conclusion: There is the need to conduct this kind of tracer study on a regular basis to keep track of graduate’s progress and obtain feedback on the quality of programs being offered so as to meet the ever-changing needs of society.

Aim: The study aimed to describe the various ways in which Italian midwives reconcile their profession's philosophical views about labor with the use of epidural analgesia in clinical practice. Design: Interpretive description. Methods: A purposeful sample of 41 midwives and 12 midwifery students participated in the study. Information about their perceptions and experiences of the use of epidural analgesia and how this practice aligns with their professional values were explored through in-depth interviews (n = 10 focus groups; n = 1 one-to-one interview). The constant comparative method was used to analyze the data. Results: With respect to aligning midwifery care with the administration of epidural analgesia, two midwifery positions were identified: the midwife that acted "in harmony" and the "disoriented midwife", defined by three dimensions: 1) the midwife's conception of her professional role; 2) the woman's attitude towards labor and; 3) the midwife's relationship and comfort with other professionals in the labor room. Conclusion: The results of this research highlight how difficult it can be for a midwife to reconcile a specific philosophical view of labor with the use of epidural analgesia. These findings can provide useful insights to help midwives in the challenging task of combining epidural analgesia with their philosophical view of labor to offer a better birth experience to women.

Care for a pregnant woman can take various organizational forms. The World Health Organization (WHO) recommends leaders increase research into health systems. The aim of this is to manage the healthcare system in such a way as to provide beneficiaries with access to high-quality services with limited financial resources. The study presented in this paper was conducted using a diagnostic survey on a group of 1642 Polish women in the traditional model of care (TM) and 3216 women in the Coordinated Care for Pregnant Women Program (CCP). Two research tools were used in this study. The first is a survey prepared by the National Health Fund, the second is an author’s survey. The results indicate that most women (85%) receiving care under the CCP received effective pain management, compared to 67% under the traditional care model (p < 0.001). In the CCP, women were significantly more likely to receive midwife assistance in infant care (90%) than women in the traditional care model (60%) (p < 0.001). Significantly more CCP patients want to return to the same hospital for their subsequent childbirth (74%) than women in the traditional model of care (43%) (p < 0.001). In patients’ opinion, the new CCP model is superior in meeting their needs and providing higher-quality services. However, educating women that an obstetrician-gynecologist and a midwife can manage their pregnancy is still necessary. In addition to the CCP program, special attention should be paid to available pain management during childbirth since the use of analgesia is still insufficient in Polish hospitals.

125. A virtual simulation-based training program on birthing positions: a randomized controlled trial

Abstract Background Restricting parturient women in healthcare facilities from choosing positions that provide the greatest comfort and benefit during labor is a global barrier. Several complex factors, including caregiver preference and medical intervention, shape the limitation. Therefore, a practical need exists to train midwives on the knowledge and skills to change this condition. Methods The study used a parallel, single-blind, randomized controlled trial at a provincial maternity and child health hospital in Fujian, China, from June 1 to December 31, 2019. The midwives in a birth suite were selected and randomly enrolled in a one-month simulation-based hybrid training or face-to-face teaching in September 2019. The four-level Kirkpatrick’s model, including reaction, learning, behavior, and results, was used to evaluate training effects before and after the program. Data were analyzed with SPSS 25.0 using Student’s t-test, Spearman’s correlation test, Mann–Whitney U test, Wilcoxon signed-rank test, and chi-square test analysis of variance. The significance level was set at p 0.05). The virtual simulation group demonstrated an increase in non-supine birth rate (p = 0.030) and a decrease in perineal incision rate among primiparas compared to pre-intervention (p = 0.035). Moreover, knowledge performance was associated with the duration of virtual simulation (r = 0.664, p = 0.001). Conclusions Virtual simulation is a fascinating innovation that enables midwives to develop birthing positions without practicing on real pregnant women and is one solution to achieve work competency within a shortened training period.

Abstract Background Woman-centered care (WCC) is the cornerstone of the midwifery profession. However, no study has been conducted on WCC provided by Iranian midwives and its associated factors. Thus, this study aimed to determine WCC and factors associated with midwives’ WCC for midwives working in urban health centers and public and private hospitals in Tabriz, Iran. Methods This cross-sectional study was the first part (i.e., the quantitative phase) of a sequential explanatory mixed-method study conducted on 575 midwives working in urban health centers and public and private hospitals in Tabriz-Iran from November 2022 to January 2023. The required data was collected by distributing a socio-demographic and job characteristics questionnaire and woman-centered care scale-midwife self-report (WCCS-MSR). To determine the factors associated with WCC, an independent t-test or one-way analysis of variance (ANOVA) was used in bivariate analysis, and a general linear model (GLM) was employed in multivariate analysis to control possible confounding variables. Results The statistical population consisted of 575 midwives, with a response rate of 88.2%. According to the GLM, the total mean WCCS-MSR score of single [β (95% CI) 23.02 (7.94 to 38.10)] and married [β (95% CI) 21.28 (6.83 to 35.72)] midwives was significantly higher than that of divorced midwives after adjusting their demographic and job characteristics. Amaç: Araştırma kadın doğum ve jinekoloji kliniklerinde çalışan hemşire ve ebelerin pandemi dönemindeki algılarını, deneyimlerini ve bu deneyimi etkileyen faktörleri belirlemek amacıyla gerçekleştirilmiştir. Yöntem: Bu araştırma nitel yöntemle yapılmıştır. Çalışmanın, katılımcı grubunu bir ilde iki hastanenin kadın doğum ve jinekoloji kliniklerinde görev yapan 20 hemşire ve ebe oluşturmuştur. Veriler Eylül- Kasım 2020 tarihleri arasında kişisel bilgi formu ve yarı yapılandırılmış görüşme formu ile yüz-yüze ve derinlemesine bireysel görüşme yöntemi ile toplanmıştır. Verilerin değerlendirilmesi için içerik analizi kullanılmıştır. Bulgular: Katılımcıların yaş ortalaması 35,30 ±6,62 yıl, %80 (n=16)’i ebe ve %20 (n=4)’si ise hemşiredir. Araştırmada “pandemiyi algılama”, “pandemide bakım hizmetlerinde yaşanan değişimler”, “güçlükler” ve “kazanımlar” temaları ortaya çıkmıştır. Katılımcıların Covid 19’un kendilerine ve yakın aile üyelerine bulaşması ile ilgili endişe ve stres yaşadıkları, çalışma koşulları ve yönetimsel nedenlere bağlı yorgunluk, değersizlik ve öfke hissettikleri belirlenmiştir. Bu süreçte hemşire ve ebelerin izlem ve takiplerinin, hasta eğitimlerinin farklılaştığı görülmüştür. Pandemide hemşire ve ebelerin günlük yaşam aktiviteleri, yönetimsel, kişisel koruyucu ekipman kullanımına bağlı güçlükler yaşadıkları bununla beraber krizi yönetebilme becerisinden dolayı mesleki benlik saygılarının güçlendiği belirlenmiştir. Sonuç: Pandeminin aşılama öncesi döneminde gebe, yenidoğan ve jinekoloji hastasına yönelik bakım hizmetlerinde değişimler gerçekleşmiştir. Background & aim: Midwives can participate in empowering women by improving women's health and supporting them through different life stages. Despite the emphasis on the role of midwives in empowering women, the strategies through which midwives can fulfill this role are not fully investigated. The aim of this study was to review the role of midwifes in women's empowerment.Methods: In this narrative review, which was conducted based on the Scale for Assessing Narrative Review Articles (SANRA), databases including ISI, Scopus, PubMed, Cochrane library, PsycInfo, SID, Magiran and IranMedex were searched by two researchers without time limit up to September 2022, using keywords of "female empowerment", "empowerment", "midwife", "midwifery" and their Persian equivalents. All the original articles focused on midwives’ role in women's empowerment were included. Articles without access to full-text, conference papers, short communications and editorials were excluded. Qualitative synthesis was conducted on 27 articles included in the study.Results: Two main categories including "reproductive and sexual health" and "gender-based discrimination and violence" were identified. The first category included midwives' role in pregnancy, childbirth, postpartum, breastfeeding and family planning services. The second category involved midwives role to empower women in confronting domestic violence and harmful cultural practices.Conclusion: Despite midwives success in empowering women, they face many obstacles, both within and outside the health system, especially in combatting against discrimination and gender-based violence, which need to be addressed. To empower women, it is crucial that governments, communities and international organizations pay more attention to the role of midwives in women’s empowerment.

129. Motherhood Role from a Postpartum Perspective: Effects Reflected by High-Risk and Normal Pregnancies

High-risk pregnancies not only heighten concerns about the baby’s health but also have the potential to impact the mother–infant relationship by shifting the mother’s focus towards her own health needs. This study aims to delve into the intricacies of motherhood roles among women experiencing high-risk pregnancies compared to those with uncomplicated pregnancies, aiming to shed light on the disparities between the two groups. The participants of this descriptive, comparative, and correlational study consisted of literate mothers admitted to a hospital in Eastern Turkey, diagnosed with high-risk pregnancies, and with no prior history of psychological support. Due to an inability to reach the entire population, an unknown sampling method was employed for sampling calculation. The sample comprised 133 mothers with high-risk pregnancies and an equal number of healthy mothers, totaling 266 participants. Data were collected using the “Maternal Introduction Form” and the “Maternal Role Gaining Scale”, and analyses were conducted using the SPSS Statistical Programme. Given the non-normal distribution of the variables, nonparametric tests were applied post reliability analysis. There is a statistically significant difference (p < 0.05) in the scores of maternal attitude and anxiety, as well as maternal role and the Maternal Role Gaining Scale, based on various demographic factors such as marriage duration, spouse’s education, family economic status, pre-pregnancy health issues, medication use, hospitalization status and reason, assigned sex of the baby and desired gender, total pregnancies, mode of delivery, postpartum difficulties, support for baby care, feeding method, high-risk pregnancy diagnosis, and week of diagnosis. A Bonferroni corrected analysis also revealed significant differences between mothers with and without high-risk pregnancies.

(1) Background: The transition to twin parenthood is a demanding challenge with a higher risk of maternal and fetal complications during pregnancy and a postpartum period that involves caring for more than one newborn at the same time with similar and simultaneous needs. (2) Aim: To find out about parents’ needs when experiencing the transition to twin parenthood and to describe the intervention of their specialized nursing support network. (3) Methodology: A descriptive exploratory study, based on a proper non-probabilistic sample of 15 nurses and 55 couples who are parents of twins, using two online questionnaires publicized on social networks. (4) Results: The couple’s needs were identified through knowledge of their experiences and difficulties during pregnancy and after the twin birth. Couples’ and nurses’ perceptions differed on the identified needs. The specialized nursing support network focuses its intervention on providing informative guidance on twin pregnancy and postpartum period, health education, group sharing experiences, home visits, planning, and including a family support network in the management of twin care and the creation of a daily routine. (5) Conclusions: There is a need to implement a program focused on the needs of parents of twins, promoting realistic expectations for the birth and parenting of twins, preparing parents, improving their well-being, and creating a specialized nursing support network available to this population.

131. The Associations of Perceived Self-Efficacy with Emotional Intelligence, Personality, Resilience, and Attitudes Towards Death among Midwives

Midwives’ self-efficacy can significantly affect the provided care and, therefore, maternal and neonatal outcomes. The aim of the present study was to investigate associations of perceived self-efficacy with emotional intelligence, personality, resilience, and attitudes towards death among midwives in Greece. From 2020 to 2022, a total of 348 midwives were recruited in this descriptive cross-sectional study. The participants were employed as independent professionals, in public hospitals or regional health authorities. Data collection involved five research instruments: the General Self-Efficacy Scale (GSES), the Trait Emotional Intelligence Questionnaire-Short Form (TEIQue-SF), the Eysenck Personality Questionnaire (EPQ), the Connor-Davidson Resilience scale (CD-RISC), and the Death Attitude Profile-Revised (DAP-R) scale. The mean score for the GSES was 29.1 (SD = 4.2), suggesting a moderately elevated level of self-efficacy among midwives. The results revealed that higher scores on the GSES were significantly associated with higher scores on the Extraversion subscale (p < 0.001) and lower scores on the Neuroticism (p < 0.001) and Lie (p = 0.002) subscales of the EPQ. Additionally, high self-efficacy was significantly correlated with high emotional intelligence (p < 0.001), high neutral acceptance of death (p = 0.009), and high resilience (p < 0.001). These findings highlight the relationship between the self-efficacy of Greek midwives and various psychological factors, as well as the multifaceted nature of self-efficacy and its importance for midwives’ psychological well-being and professional functioning.

Aim: This descriptive qualitative study provides information on research dealing with women's opinions, experiences, and attitudes toward menopause. Design: A descriptive qualitative study. Methods: Individual semi-structured in-depth interviews were used for qualitative research into this topic. The research group consisted of 17 women aged 40 to 55 years. Thematic data analysis was used to analyze the data obtained. Results: Based on the thematic analysis, the following categories were created: 1) Association with the period of menopause; 2) Experiencing menopause; and 3) Education. The results showed that women viewed menopause as a transitional phase of life. The main symptoms included hot flashes, night sweats, disturbed sleep, mood swings, and weight gain. Prior to the study, participants were not interested in additional information on the issue. Conclusion: As estrogen levels decline later in life, women transition from the reproductive to the post-reproductive phase of their lives. For women to experience healthy menopause, they must understand the process before they begin to experience the consequences of estrogen deficiency.

Background and Objective: One of the important problems of midwifery is high job stress and its consequences. Having the ability to manage stress is one of the basic requirements of this profession that can reduce midwifery mistakes in the workplace. Although job stress exists in all jobs, midwives face higher levels of stress due to the nature of their job, and this issue can lead to a decrease in service quality. The present study was conducted to determine the relationship of stress management with the level of perceived job stress and the quality of services provided by midwives working in the maternity department of hospitals in Hamedan province, Iran. Materials and Methods: The statistical population of this descriptive cross-sectional study consisted of 210 midwives working in hospitals with obstetrics and gynecology departments in Hamedan province. The random sampling method was used to select the samples. The required data for this study were collected using a demographic information form, Cobin's Stress Management Questionnaire, Hospital Stress Scale, and Maternity Care Quality in Maternity Unit Scale. Data were analyzed in Stata-13 software using Kolmogorov-Smirnov statistical tests and multivariate linear regression. The significance level was considered less than 0.05. Results: The findings showed that 81.4% of midwives had a high level of stress management ability. Moreover, 65.7% of midwives perceived moderate-level job stress, and 79% of midwives provided good quality services in the delivery room. Based on the results, was a moderate and inverse relationship between stress management and perceived job stress (beta=-0.47), and a moderate and direct relationship between stress management and the quality of provided services (beta=0.56). It was also found that perceived job stress had a weak and inverse relationship with the quality of provided services (beta=-0.25). Conclusion: The results revealed that having the ability to manage stress and control job stress could improve the quality of services provided by midwives. It is necessary for midwife managers to improve the process of providing quality services by holding stress management workshops.

Background: The incidence of Section Caesarea (SC) based on the results of the 2018, states that the highest prevalence is in the DKI Jakarta area with a percentage reached 31.3% while the Papua region percentage is only 6.7%. It is known that the causes of birth with CS include, a narrow pelvis, disproportion of the fetal head, malposition of the fetus, and disorders of the descent of the fetal head which often cause complaints of symphysis pubis dysfunction (SPD). Every pregnant woman needs to empower herself to increase her self-confidence in facing the pregnancy process until before delivery. Several studies have stated that practicing yoga during pregnancy can reduce pregnancy complaints. During the Covid-19 pandemic, pregnant women were limited to doing activities outside the home. In this case, independent yoga media is needed that is safe, easy to understand, and as a guide for pregnant women, both in the form of videos, flashcards, spin cards, and picture cards of practical yoga movements.Objectives: To find out the effectiveness of yoga movements in dealing with complaints of symphysis pubis dysfunction in pregnancy.Methods: This research method uses quasi-experimental research with a one-group pre-test design which reveals a causal relationship involving one group of subjects. Respondents consisted of 25 pregnant women entering the third trimester of pregnancy with the inclusion criteria of pregnant women experiencing dysfunction of the symphysis pubis before the intervention. The interventions that will be carried out are measuring pain scales, printing yoga movement cards, teaching, and doing yoga 4 times in 1 month.Results: It is known that the number of respondents who did yoga before the intervention was mostly in the mild pain category (36%) and moderate (28%). After the intervention, the majority of respondents were in the no-pain category (44%). Respondents with severe pain before intervention (16%) decreased to (8%) post-intervention. The results of statistical tests using the Wilcoxon test obtained p = 0.036 where this value is below 0.05, which means that there is an effect of yoga movements in reducing complaints of symphysis pubis dysfunction in pregnancy.Conclusions: From the results of the research that has been done, it can be concluded that there is an influence of yoga movements in reducing complaints of symphysis pubis dysfunction in pregnancy. There were significant changes in pregnant women before and after the intervention. Therefore pregnant women are encouraged to do yoga regularly to reduce complaints of pregnancy discomfort which can interfere with the delivery process.

Background & aim: Job satisfaction of midwives plays an important role in the efficiency and performance of health centers and the quality of midwifery services. This study reviewed job satisfaction and its related factors among midwives working in health systems in Iran.Methods: In this systematic review, the databases of Web of Science, Scopus, PubMed, PsycINFO, Embase, Google Scholar and Magiran, were searched between 2000 and 2021 using keywords of midwife, job satisfaction, occupational stress, and Iran. The research steps were based on PRISMA. For quality assessment, the articles were evaluated according to the inclusion criteria by two researchers with the Newcastle-Ottawa scale. Results: A total of 23 articles including 3,352 midwives working in the health and medical centers were reviewed. The level of job satisfaction was moderate, and the job satisfaction of midwives working in hospitals was lower than that of those working in health centers. The highest level of satisfaction was related to the job positions and relationships with colleagues, and the lowest level was in relation to the salary and job benefits. Marital status, age, income, salary, organizational position, education, employment status, interest in the field of education, and workplace had a relationship with increasing job satisfaction. Tensions in the workplace environment was associated with decreasing job satisfaction.Conclusion: It is necessary to provide conditions to improve job satisfaction of midwives in order to identify and removing barriers and improving professional belonging to promote occupational health and optimize the quality of providing midwifery services.

137. The mediating effect of psychological empowerment on the relationship between work environment and clinical decision-making among midwives: a multicentre cross-sectional study

Abstract Background Clinical decision-making is considered an essential behaviour in clinical practice. However, no research has been done to examine the associations among midwives’ clinical decision-making, work environment and psychological empowerment. Thus, this study aimed to determine the influence of work environment on midwives’ clinical decision-making and confirm the mediating role of psychological empowerment. Method This study was designed as a multicentre cross-sectional study, and included 602 registered midwives from 25 public hospitals in China. A sociodemographic questionnaire, Work Environment Scale, Psychological Empowerment Scale and Clinical decision-making Scale were applied. A structural equation model was conducted to estimate the hypothesis model of the clinical decision-making among midwives and explore the potential mediating mechanism of midwives’ clinical decision-making. This model was employed maximum likelihood estimation method and bootstrapping to examine the statistical significance. Results The mean score of clinical decision-making among midwives was 143.03 ± 14.22, at an intermediate level. The data of this hypothesis model fitted well, and the results showed that work environment positively affected psychological empowerment, which in turn positively affected clinical decision-making; psychological empowerment partly mediated the relationship between work environment and clinical decision-making among midwives. Conclusions Midwives’ clinical decision-making could be promoted directly or indirectly by providing a healthy work environment and improving psychological empowerment. It is essential for hospital managers to pay attention to the assessment of the midwives’ work environment and actively improve it, such as establishing a supportive, fair and just workplace, and maintaining effective communication with midwives. Furthermore, managers can also promote midwives’ clinical decision-making behaviour by enhancing their psychological empowerment via enhancing job autonomy.

This study explores vaccine hesitancy among nursing and midwifery undergraduate students in Switzerland—a cohort crucial to public health given their future roles as healthcare professionals—with a particular emphasis on the HPV vaccine, which exhibits lower confidence levels compared to other vaccines.MethodsThis study will employ an online questionnaire distributed to nursing and midwifery undergraduate students from various healthcare universities. The questionnaire will collect data on vaccine hesitancy (general confidence in vaccines and specifically in the HPV vaccine), HPV vaccine coverage, socio-demographics, likelihood to recommend vaccines to patients, perception of vaccination education and interest in complementary medicine.ConclusionThe study’s findings will contribute to our understanding of vaccine hesitancy among nursing and midwifery undergraduate students, providing insights that can inform targeted interventions and education strategies to bolster vaccine confidence among future healthcare professionals, thereby enhancing public health efforts.

140. A survey of bloodborne occupational exposure protection behavior among qualified Chinese midwives: A cross-sectional study

Background: Bloodborne occupational exposure is a major public health concern of the China Health Commission, especially among midwives who are at high risk among healthcare workers. Knowledge of occupational exposure and appropriate protective behaviors play important roles in reducing occupational exposure. The purpose of this study was to understand the knowledge and level of protection against bloodborne occupational exposure among midwives in China. Methods: This was a multi-center, cross-sectional study. Midwives from hospitals that are members of Infectious Disease Nursing Committee of Chinese Nursing Association were selected as survey participants from February 2019 to February 2022 using a judged sampling method. Data were collected by using a self-developed questionnaire for Chinese midwives to report their current knowledge and behavior related to bloodborne occupational exposure protection. Results: A total of 2850 questionnaires were distributed and 2742 valid questionnaires were obtained, resulting in an effective rate of 96.21 %. Midwives scored 2742 (6.495 ± 1.529) points for their knowledge about bloodborne occupational exposure protection, with the level and type of hospital being independent factors affecting the midwives' knowledge of bloodborne occupational exposure protection (χ2 = 27.284, P = 0.038; χ2 = 28.374, P = 0.000). Of the midwives, 1460 were qualified for bloodborne occupational exposure protection behavior, with a qualified rate of 53.25 %. Working years (χ2 = 9.372, P = 0.002) and working hours (χ2 = 13.933, P = 0.000) were also the independent factors for bloodborne occupational exposure protective behavior in midwives. Conclusion: Chinese midwives possess relatively good knowledge of bloodborne exposure protection against bloodborne infectious diseases, but their behavioral level is not optimistic. Improvements to both knowledge and behavioral level of bloodborne occupational exposure protection are necessary.

ABSTRACT Objectives: to evaluate the outcomes of Interval Copper Intrauterine Device (IUD) insertion performed by certified midwives and obstetric nurse practitioners at a Peri-Hospital Birth Center. Methods: a cross-sectional study was conducted involving 75 women who underwent IUD insertion between January 2018 and February 2020. Data collection was carried out using medical records and telephone interviews. Results: no instances of uterine perforation were observed. Expulsion rates of the devices were 1.3% within 30 to 45 days of use and 5.3% within the first year of use. The follow-up removal rate was 4.0%. The average pain score reported was 4.2 (SD = 3.3). Among those who continued using the device, 93.1% expressed satisfaction. Conclusions: the findings demonstrate that IUD insertion by certified midwives and obstetric nurse practitioners is a safe procedure, yielding outcomes comparable to those reported in the existing literature.

Background: Restricting parturient women in healthcare facilities from choosing positions that provide the greatest comfort and benefit during labor is a global barrier. Several complex factors, including caregiver preference and medical intervention, shape the limitation. Therefore, a practical need exists to train midwives on the knowledge and skills to change this condition. Methods: The study used a parallel, single-blind, randomized controlled trial at a provincial maternity and child health hospital in Fujian, China, from June 1 to December 31, 2019. The midwives in a birth suite were selected and randomly enrolled in a one-month simulation-based hybrid training or face-to-face teaching in September 2019. The four-level Kirkpatrick's model, including reaction, learning, behavior, and results, was used to evaluate training effects before and after the program. Data were analyzed with SPSS 25.0 using Student's t-test, Spearman's correlation test, Mann–Whitney U test, Wilcoxon signed-rank test, and chi-square test analysis of variance. The significance level was set at p < 0.05. Results: Forty-two midwives were initially randomized to either the virtual simulation group or the face-to-face group. One midwife was excluded from the analysis due to intervention discontinuation, resulting in a final analysis of 41 midwives (n1 = 21, n2 = 20). Post-intervention, the virtual simulation group exhibited higher satisfaction and learning effects compared to the face-to-face group, while the rate of perineal incision in primiparas was lower (p<0.05). No significant changes or differences were observed in self-rated behavior between the two groups (p>0.05). The virtual simulation group demonstrated an increase in non-supine birth rate (p = 0.030) and a decrease in perineal incision rate among primiparas compared to pre-intervention (p = 0.035). Moreover, knowledge performance was associated with the duration of virtual simulation (r = 0.664, p = 0.001). Conclusions: Virtual simulation is a fascinating innovation that enables midwives to develop birthing positions without practicing on real pregnant women and is one solution to achieve work competency within a shortened training period. Background: Woman-centered care (WCC) is the cornerstone of the midwifery profession. However, no study has been conducted on WCC provided by Iranian midwives and its associated factors. Thus, this study aimed to determine WCC and factors associated with midwives' WCC for midwives working in urban health centers and public and private hospitals in Tabriz, Iran. Methods: This cross-sectional study was the first part (i.e., the quantitative phase) of a sequential explanatory mixed-method study conducted on 575 midwives working in urban health centers and public and private hospitals in Tabriz-Iran from November 2022 to January 2023. The required data was collected by distributing a socio-demographic and job characteristics questionnaire and woman-centered care scale-midwife self-report (WCCS-MSR). To determine the factors associated with WCC, an independent t-test or one-way analysis of variance (ANOVA) was used in bivariate analysis, and a general linear model (GLM) was employed in multivariate analysis to control possible confounding variables. Results: The statistical population consisted of 575 midwives, with a response rate of 88.2%. According to the GLM, the total mean WCCS-MSR score of single [β (95% CI) 23.02 (7.94 to 38.10)] and married [β (95% CI) 21.28 (6.83 to 35.72)] midwives was significantly higher than that of divorced midwives after adjusting their demographic and job characteristics. Also, the total mean WCCS-MSR score of midwives with sufficient income was significantly higher than those with insufficient income [β (95% CI) 8.94 (0.12 to 17.77). In addition, the total mean WCCS-MSR score of midwives with < 5 years of work experience [β (95% CI) − 7.87 (− 14.79 to − 0.94)], and midwives with official-experimental employment status [β (95% CI) − 17.99 (− 30.95 to − 5.02)], was significantly lower than those with more than 5 years of work experience and contractual employment status. Conclusions: The findings indicate that marital status, level of income, years of practice, and employment status were significantly related to WCC provided by midwives. Focusing only on the midwifery community is insufficient to ensure the improved quality of WCC. However, arrangements should be made at three levels, including policy-makers, managers, and health care provider (midwives). Plain Language Summary: As the cornerstone of the midwifery profession, WCC represents a universal, integrated, and synonymous concept with practice, which implies focusing on women as individuals. This cross-sectional study determined WCC and its associated factors of midwives working in urban health centers and public and private hospitals in Tabriz, Iran. A sample size of 575 midwives was used to determine the factors associated with WCC. The required data was collected by distributing a socio-demographic and job characteristics questionnaire and woman-centered care scale-midwife self-report (WCCS-MSR). A generalized linear model (GLM) was used to determine the factors associated with WCC. Midwives obtained the highest total mean score in the Works in Partnership with the Woman (WP-W) subscale and the lowest total mean score in the Ensures Midwifery Philosophy Underpins Practice within the Context of the Maternity Service (EMPUP-MS) subscale in the obtainable scores, which ranged from 0 to 100. Our study found that the total mean WCCS-MSR score of single and married midwives was significantly higher than that of divorced midwives. Also, the total mean WCCS-MSR score of midwives with sufficient income was significantly higher than those with insufficient income. In addition, the total mean WCCS-MSR score of midwives with < 5 years of work experience, and midwives with official-experimental employment status, was significantly lower than those with more than 5 years of work experience and contractual employment status. The findings indicated that marital status, level of income, years of practice, and employment status were significantly related to WCC provided by midwives. Therefore, to improve the quality of WCC, identifying the mentioned factors will help policy-makers to consider facilitating measures, providing practical solutions, and designing future interventions. The aim was to determine the experiences of Turkish midwives and nurses when caring for families with perinatal loss. It was a descriptive and qualitative study. One-to-one interviews were conducted with ten midwives and five nurses using a semi-structured interview form. It was found that the nurses and midwives experienced pain and grief during caring for women who had perinatal losses, and they tried to relieve these feelings by using positive thinking and prayer. The nurses and midwives stated that they were unsure how to approach women or family experiencing perinatal loss, and they needed mentoring and training. The results showed that it would be beneficial to plan training for nurses and midwives and to conduct mentoring for those experiencing difficulties. For nurses and midwives frequently facing perinatal loss, there will be benefit in planning grief care training at regular intervals and mentoring for those experiencing difficulties. Background/Aims: The Indonesian family planning programme promotes contraceptive use to control population density. Counselling is widely used to improve contraceptive use. The aim of this study was to explore women's use of contraceptives before and after counselling. Methods: This cross-sectional study collected data from 10 790 women across 10 districts/cities in East Java. The data obtained were analysed using descriptive statistical analysis and paired t-tests. Results: After counselling, use of contraceptives increased significantly (P<0.001), from 75.8% using contraceptives before counselling to 99.98% using them after counselling. Use of the contraceptive pill notably increased from 16.7% to 40.9%. Conclusions: Short-term contraceptives were most popular with women in this population. Background/Aims: Midwives are vital healthcare professionals that are autonomous and passionate specialists on women, pregnancy, birthing and families. Staying up to date with contemporary issues enhances their ability to provide high-quality evidence-based care. One aspect of maternal care that is changing is the use of gender-neutral language. The aim of this review was to examine the use of gender-neutral language in maternity settings and collate the literature to expose any knowledge gaps. Methods: A narrative literature review was conducted, using primary research and literature reviews from the Cochrane Library, CINAHL and MEDLINE databases. A total of 106 articles were included. Results: Data analysis yielded four themes: etymology and the origins of language in maternity, the effects of language in maternity, attitudes, and social media and language. The literature identified the perspective of the lesbian, gay, bisexual, transgender, intersex, queer or questioning, asexual and other sexually or gender diverse population who have accessed maternity and childcare services. Not all literature was maternity specific, but provided insight into how to improve maternity services. Conclusions: It is recommended that education be provided for healthcare professionals when working with the LGBTIQA+ community. Updating paperwork and policies at a structural level will have a vast impact holistically on LGBTIQA+ maternity consumers. [ABSTRACT FROM AUTHOR]

The item pool of the scale was created by the researchers with the literature as twenty-three items in a five-point Likert type and submitted to the opinions of twelve specialists. Results: The three-factor scale structure designed by making explanatory and confirmatory factor analysis was confirmed with the validity and reliability. According to the analysis results; the scale consisted of three sub-dimensions (managerial and personal; physical and occupational; organizational and environmental) and 15 items. It was founded that the content validity index of the scale items was 0.468-0.851; factor loads were 0.510-0.871; item-total score correlations were 0.467-0.768, and the Cronbach's alpha reliability coefficient of the sub-dimensions ranged between 0.703-0.876 and it was 0.912 for the whole scale. Conclusion: As a result of the validity and reliability analyzes of the Attitude Scale towards Innovative Behaviors of Nurses and Midwives, it was founded that it can be used as a valid and reliable scale. Childbirth is a physiological process and usually occurs vaginally and lasts from 4 to 12 hours. There are absolute and relative indications for cesarean section. The object of this study is not the indications for operational birth, but the cases in which they are missing, but women prefer to give birth with surgery. Globally, there is an increase in the percentage of Caesareans performed for no objective reasons compared to vaginal births. This negative trend is observed in a number of countries with relatively high birth rates such as Brazil, Turkey. In EU countries, caesarean sections are within 15-30% of the total number of births. In Bulgaria, almost half of the births are carried out by cesarean section and very few of them are by absolute indications of this. By 2017 the percentage of operations is 45%, and in 2021 it is already 49.95%. The World Health Organization recommends that about 10-15-19% of women give birth surgically. All this data tells us that many of the sections in our country and around the world are unnecessarily done without having an absolute or relative indications for that.Objektive: to examine the opinion and attitudes of non-borne, pregnant, parturient women how they prefer to give birth if they do not have absolute or relative indications for that and what is the involvement of the midwife in forming their opinion.Materials and methods: questing of respondents, observation, interview Results and discussion: There were 984 respondents. The questing of respondents was held from February to 15.04.2023 in Women's Consultations, Departments in Hospitals, Social Networks. More than 70% of women are in favor of a normal vaginal birth, 23% would choose an operative birth without objective prerequisites. When asked how they gave birth, more than half had the answer was operatively 488 and 465 vaginally. The biggest influence on the decision to give birth has the - obstetrician-gynecologists -leading The Consulting Cabinet for Maternal Care/CCMC/ – 86.45% . Very little is the influence of the midwife or a close health care specialist. Respondents' answers indicate that they would contact a midwife working in the Women's Health Kabinet about the choice of birth giving.Positivly answered 864 women and only 147 answered negatively.Conclusion:. The study fulfilled its purpose and showed what is the role of the midwife in making a decision about the woman's mode of delivery, when there are no absolute and relative prerequisites for Caesarean Section at this time. It made it possible, based on the answers, to give some recommendations for increasing the role of the midwife as an active participant in decision-making about the mode of birth. Teenage pregnancy is something that plays a role in the medical and social health concerns in Bulgaria as well as abroad. Our country holds a regrettable place in the championship regarding the "children giving birth to children" phenomenon. Getting pregnant and giving birth before the age of 20 is referred by the medical term 'adolescent pregnancy' Adolescence is a period in which hormonal, physiological and mental changes are observed in the girl's body. A crucial question is whether the adverse outcomes observed among mothers and children of teenage mothers are causally related to maternal age or are the result of something else. The goal of the current overview is to make a comprehensive review and analysis of the peculiarities of the medical and social characteristics of the adolescent pregnancy, their sexual and reproductive behavior in the specialized scientific literature at hand. A total of 318 articles by Bulgarian and foreign authors were analyzed. The search was limited by articles that are completed and comprehensive and are written in English, Bulgarian and Russian and published in peer-reviewed scientific journals. The results of the literature review show that there is a significant social risk for girls which give birth at the extremely early age of 15 or under. Getting pregnant and giving birth as a teenager carries a variety of risks both in terms of the fetal development as well as the well-being and maternal health of the mother. Adolescent pregnancy is accompanied by a large number of complications - hyperemesis, preeclampsia and eclampsia, gestational hypertension, gestosis, spontaneous abortions, iron deficiency anemia, etc. This requires specific integrated actions across of multitude of sectors. The intervention of the midwife and the women's consultation plays an essential role in the prevention of early pregnancy and the maternal and child health in adolescent age. [ABSTRACT FROM AUTHOR]

150. Skills and knowledge of midwives at free-standing birth centres and home birth: A meta-ethnography.

- Abstract
When midwives offer birth assistance at home birth and free-standing birth centres, they must adapt their skill set. Currently, there are no comprehensive insights on the skills and knowledge that midwives need to work in those settings. Midwifery care at home birth and in free-standing birth centres requires context specific skills, including the ability to offer low-intervention care for women who choose physiological birth in these settings. To synthesise existing qualitative research that describes the skills and knowledge of certified midwives at home births and free-standing birth centres. We conducted a systematic review that included searches on 5 databases, author runs, citation tracking, journal searches, and reference checking. Meta-ethnographic techniques of reciprocal translation were used to interpret the data set, and a line of argument synthesis was developed. The search identified 13 papers, twelve papers from seven countries, and one paper that included five Nordic countries. Three overarching themes and seven sub-themes were developed: 'Building trustworthy connections,' 'Midwife as instrument,' and 'Creating an environment conducive to birth.' The findings highlight that midwives integrated their sensorial experiences with their clinical knowledge of anatomy and physiology to care for women at home birth and in free-standing birth centres. The interactive relationship between midwives and women is at the core of creating an environment that supports physiological birth while integrating the lived experience of labouring women.
