102. The Improving Community-Based Instruction Project: Final Report.

- Abstract
The final report describes activities of the Improving Community-Based Instruction Project, which conducted research to improve the effectiveness and efficiency of training in community settings with individuals with severe disabilities. Three areas of research were undertaken: (1) studies of the influence of the location of instruction on the acquisition and generalization of community activities; (2) studies on strategies for establishing reliable stimulus control of student responses in community settings; and (3) studies on the impact of various strategies for building performance of complex chains of behavior. The project also developed procedural manuals to assist practitioners in developing community-based and classroom-based instructional programs. The report lists project objectives and briefly reports project activities and accomplishments. Among conclusions are the need for more comparative research on effectiveness of commonly recommended instructional strategies; examination of strategies leading to the adoption of validated techniques by practitioners; and research on instructional strategies to enhance maintenance of performance in community settings. Much of the document consists of attachments including the specific reports for studies in the three designated research areas, the two procedural manuals, and a program analysis form. 103. Teachers' Career Stages and Availability and Appropriateness of Incentives in Teaching.

Evidence on teachers' careers suggests that there are identifiable career stages through which teachers progress. Inherent in these career stages is the need for differentiated teacher incentives and development programs. The premise that the stages of development are important in planning effective professional incentives and development programs is well accepted. A model describing the dynamics of teachers' career cycles offers a view of career progression which reflects influences from environmental factors, both personal and organizational. The components of the career cycle are: (1) preservice, (2) induction, (3) competency building, (4) enthusiastic and growing, (5) career frustration, (6) stable and stagnant, (7) career wind-down, and (8) career exit. Appropriate incentives for each of these career stages are discussed. (JD)

- Abstract
Described and evaluated are the 21 programs funded by Title VI for eight handicapping categories comprising 1,024 children at all educational levels in Oregon during 1972-1973. Explained are criteria such as adequacy of documented needs established by the state Board of Education for funding projects and training procedures for utilization of a Third Party Evaluation Model. The following are among results discussed: number of projects per category such as eight projects for the emotionally disturbed (ED), five projects for the speech and language impaired, and two projects each for the educable mentally retarded (EMR) and preschool children; costs; kinds of services such as a classroom utilizing behavior modification to offset undesirable behaviors learned at home; and conclusions stressing success of resource rooms for ED and EMR students, regular class placement, preschool programs, parent training programs, paraprofessionals for language remediation. Recommendations emphasized the need for further data on effects of integration, and encouragement of proposals featuring vocational educational skills. Given for each project are the title, location (a map is included), population served, funding allocation, dates, background information, objectives and evaluation plan, methodology, the Third Party Evaluator's comments, and pertinent tables or forms. 106. Impact 8 of the Title VI Programs in the State of Oregon: September 1, 1973-August 1974.

- Abstract
Nineteen projects that served seven different classes of handicapped were funded for 1973-74 in Oregon under Title VI of the Elementary and Secondary Education Act of 1965. Six projects were funded for the emotionally disturbed, four projects for the hearing impaired, two projects for the developmentally disabled, two projects for the visually impaired, and one each for the trainable mentally retarded and the preschool handicapped. The projects served 1,650 children at a cost of $247,045. A careful examination of the 19 projects funded supported the following conclusions: that resource rooms demonstrated themselves to be efficient ways to serve emotionally disturbed children; that, given appropriate training and technical assistance, classroom teachers could cope with behavior problems in the classroom; that the use of volunteer aides with both speech and visually impaired children in areas where teachers were working on an itinerant basis continued to demonstrate itself to be an effective technique; and that a project serving developmentally disabled high school youth demonstrated that work experience skills could be taught effectively to such children. - Abstract
Summarized are results of a survey to determine the impact of 133 projects funded through Elementary and Secondary Education Act Title VI-B funds on handicapped children in Oregon by examining the number of children being served, the number of staff, and the number of dollars being expended. Significant findings are listed such as that deaf, blind, and trainable mentally retarded populations would not have been served if Title VI-B had not been available between 1968 and 1970, and that 87 percent of projects funded for an academic year were still in operation compared to only 44 percent of projects funded for the summer only. Among the 11 recommendations are that Title VI-B monies be awarded as seed money only and that the new special education service priorities be established every 3 years. General information includes report purpose, a summary of Title VI provisions, definitions of handicapping conditions, a description of eligible agencies, program purpose, program guidelines, evaluation model, and methodology. Seventeen tables are provided giving statistics such as the number and percentage of children served by handicapping condition. Five special projects are individually described including "A Study of the Extreme Learning Problem Program in Oregon." Appended are the questionnaire used in the survey and the cover letter. (DB)

111. Transition Programs for Students with Moderate/Severe Disabilities.

This text is intended to provide new and practicing professionals with an introduction to the strategies necessary to support the transition of students with moderate and severe disabilities from school to community life. It offers recommendations for designing and implementing middle school, high school, and post-high school programs, addressing the full range of curricular and instructional issues at each of these levels. The book's five sections address: foundations of transition programs, curriculum and individualized planning, instruction and personal supports, preparation for home and community life, and preparation for work. The 13 chapters discuss the following topics: (1) expected outcomes and emerging values; (2) the role of secondary education in transition; (3) curriculum; (4) developing Individualized Education Program Plans; (5) participation in content-area classes; (6) teaching in natural settings; (7) developing leisure and recreational options; (8) preparation for life at home and in the community; (9) self-management; (10) employment training; (11) job placement; (12) administrative issues; and (13) interagency collaboration. Chapters are organized around focus questions, have illustrative case examples, and provide extensive references. Background Randomised control trials and genetic analyses have demonstrated that vitamin D or 25-hydroxyvitamin D (25[OH]D) levels may not play a causal role in the development of cardiovascular disease. However, it is unclear if 25(OH)D is causally associated with cause-specific vascular disease and lipids. Therefore, we examined the causal association of 25(OH)D with myocardial infarction, stroke, ischaemic heart disease, ischaemic stroke, subarachnoid haemorrhage, intracerebral haemorrhage, and lipid levels among both Chinese and Europeans. Methods We used a Mendelian randomisation (MR) design in the China Kadoorie Biobank, the Copenhagen City Heart Study, and the Copenhagen General Population Study. The 25(OH)D-related genetic variants in the CYP2R1 and DCHR7 genes were genotyped in 99,012 Chinese adults and 106,911 Danish adults. Results In Chinese adults, plasma 25(OH)D levels were not significantly associated with cause-specific vascular disease or mortality, with the exception of intracerebral haemorrhage (HR, 1.09 [95% CI, 1.01,1.18] per 25 nmol/L higher plasma 25(OH)D). In Europeans, plasma 25(OH)D levels were inversely associated with all types of vascular diseases and mortality. However, MR analysis did not demonstrate causal associations of genetically increased 25(OH)D levels with cause-specific vascular diseases, or mortality in both Chinese and European adults. In addition, each 25 nmol/L higher 25(OH)D was observationally associated with lower total cholesterol and low-density lipoprotein cholesterol levels, but higher high-density lipoprotein cholesterol levels. Likewise, MR analysis showed that 25(OH)D levels were not causally associated with lipids in both Chinese and European adults after Bonferroni correction. Conclusions We found no evidence to support that genetically increased 25(OH)D was associated with a lower risk of ischaemic stroke, intracerebral haemorrhage, subarachnoid haemorrhage, and lipid levels in both Chinese and European adults. These results suggest that the inverse associations of vitamin D with vascular disease could be the result of confounding. This study with 39 secondary students with severe disabilities integrated into regular programs in either home schools or cluster programs found that the proportion of students residing outside the normal school boundaries and the percent of students classified as Severely Multiply Handicapped were both negatively associated with both in-school and afterschool integration. Rupture of an intracranial aneurysm leads to subarachnoid hemorrhage, a severe type of stroke. To discover new risk loci and the genetic architecture of intracranial aneurysms, we performed a cross-ancestry, genome-wide association study in 10,754 cases and 306,882 controls of European and East Asian ancestry. We discovered 17 risk loci, 11 of which are new. We reveal a polygenic architecture and explain over half of the disease heritability. We show a high genetic correlation between ruptured and unruptured intracranial aneurysms. We also find a suggestive role for endothelial cells by using gene mapping and heritability enrichment. Drug-target enrichment shows pleiotropy between intracranial aneurysms and antiepileptic and sex hormone drugs, providing insights into intracranial aneurysm pathophysiology. Finally, genetic risks for smoking and high blood pressure, the two main clinical risk factors, play important roles in intracranial aneurysm risk, and drive most of the genetic correlation between intracranial aneurysms and other cerebrovascular traits.

- Abstract
Background There is heterogeneity in the names and anatomical descriptions of regional anesthetic techniques. This may have adverse consequences on education, research, and implementation into clinical practice. We aimed to produce standardized nomenclature for abdominal wall, paraspinal, and chest wall regional anesthetic techniques. Methods We conducted an international consensus study involving experts using a three-round Delphi method to produce a list of names and corresponding descriptions of anatomical targets. After long-list formulation by a Steering Committee, the first and second rounds involved anonymous electronic voting and commenting, with the third round involving a virtual round table discussion aiming to achieve consensus on items that had yet to achieve it. Novel names were presented where required for anatomical clarity and harmonization. Strong consensus was defined as ≥75% agreement and weak consensus as 50% to 74% agreement. Results Sixty expert Collaborators participated in this study. After three rounds and clarification, harmonization, and introduction of novel nomenclature, strong consensus was achieved for the names of 16 block names and weak consensus for four names. For anatomical descriptions, strong consensus was achieved for 19 blocks and weak consensus was achieved for one approach. Several areas requiring further research were identified. Conclusions Harmonization and standardization of nomenclature may improve education, research, and ultimately patient care. We present the first international consensus on nomenclature and anatomical descriptions of blocks of the abdominal wall, chest wall, and paraspinal blocks. We recommend using the consensus results in academic and clinical practice.
