Amaç: Çalışmada esmololün intraoperatif anestezik- postoperatif analjezikgereksinimi, postoperatif kortizol veprolaktin düzeylerine etkisini araştırmayı amaçladık. Gereç veYöntem: Çalışmaya 60 hasta dahil edilmiştir.Çalışma grupları şu şekilde dizayn edildi; 1: Esmolol infüzyonuna propofol veremifentanil eklendi, 2: Sadece propofol ve remifentanil, 3: Esmololinfüzyonuna desfluran ve remifentanil eklendi, 4: Sadece desfluran veremifentanil kullanıldı. Ameliyat öncesi ve sonrası kortizol ve prolaktinseviyeleri ölçüldü.Bulgular: Analjezik gereksinimi tüm gruplar içinde grup 1'de anlamlı olarakdüşüktü ve grup 4 ile karşılaştırıldığında grup 3'te düşük bulundu. Kalp hızıesmolol gruplarında (grup 1 ve 3) kontrol gruplarına göre anlamlı olarak dahadüşüktü. Prolaktin düzeyleri bütün gruplarda preoperatif değerlere görepostoperatif dönemde anlamlı yüksek bulundu. Ameliyat öncesi seviyelere ilekarşılaştırıldığında postoperatif kortizol düzeylerinde grup 1de anlamlı birazalma gözlendi.Sonuç: Laparoskopik kolesistektomi hastalarında anesteziyönetiminde adjuvan esmolol kullanımı herhangi bir hemodinamik instabiliteyeneden olmadan anestezik - analjezik gereksinimini azaltıp postoperatif kortizolyanıtı baskılarken ameliyat sonrası prolaktin yanıtı üzerinde önemli bir etkisiyoktur., Purpose: We aimed to analyze effects of esmolol on intraoperativeanesthetic-postoperative analgesic requirements, postroperative cortisol andprolactin levels. Material andMethods: Sixty patients have been included. Studygroups were as follows; 1: Esmolol infusion was added to propofol andremifentanil, 2: Only propofol andremifentanil, 3: Esmolol infusion was added to desflurane and remifentanil, 4:Only desflurane and remifentanil was used. Preoperative and postoperativecortisol and prolactin levels were measured. Results: Analgesic requirements were significantly lower in group 1 and waslower in group 3 compared to group 4 (p: 0.005). Heart rates were significantlylower in esmolol groups (group 1 and 3) compared to their controls (p: 0.001) .Prolactin levels significantly increased postoperatively in all groups comparedto preoperative levels (p: 0.005 – 0.001). There was no significant differencebetween study groups in means of postoperative prolactin (p: 0.478). Weobserved a significant decrease in cortisol levels postoperatively in group 1compared to preoperative levels (p: 0.037). Conclusion: Using adjuvant esmolol during anestheticmaintenance of laparoscopic cholecystectomy patients decreases anesthetic –analgesic requirements without causing any hemodynamic instability, andsupresses postoperative cortisol response but does not have any significanteffect on postoperative prolactin response.

Uvod. Evocirani potencijali predstavljaju promene u električnoj aktivnosti nervnog sistema izazvane nekim stimulusom. Koriste se za dijagnozu patoloških lezija n. trigeminusa i za praćenje efikasnosti različitih tretmana trigeminalne neurlagije. Takođe su vredan istraživački alat za otkrivanje fundamentalnih aspekata senzorne fiziologije. Dokazani su efekti širokog spektra opioidnih i neopioidnih analgetika na komponente evociranih potencijala u eksperimentalnom modelu bola. Isto tako evocirani potencijali su korisno neurofiziološko sredstvo za proučavanje emocija povezanih sa bolom. Poznato je da endogeni faktori kao što su stepen uznemirenosti osobe, stres, značenje i značaj stimulusa takođe određuju evocirane potencijale. Veza kasnih komponenti evociranih potencijala sa senzacijom bola je samo indirektna i predstavlja odraz emocionalnih procesa izazvanih primarnom senzacijom bola. Stres je faktor koji izaziva i održava intenzitet bola. Psihološki stres može proizvesti efekte u različitim fiziološkim sistemima koji su slični onima nastalim fizičkim uzrokom zbog aktiviranja dva sistema stresnog odgovora: simpatiko-adrenomedularnog sistema i hipotalamo-hopofizno-adrenalne osovine. Hronično povišen nivo glukokortikoida ugrožava sinaptičku plastičnost i kogniciju, smanjuje neurogenezu i gustinu kičmene moždine, dovodi do morfolških oštećenja, kao npr. demijelinizacije i atrofije dendrita, kao i funkcionalnih poremećaja, tipa hipersenzitivnosti neurona. Bol je složeno iskustvo koje jednako podrazumeva nocicepciju kao i emocionalni i kognitivni aspekt koji mogu menjati njegovu percepciju. Bol nije jednostavna posledica aktivacije receptora za bol, već složen subjektivni doživljaj, na koga umnogome utiču psihološki, socijalni, emocionalni i mnogi spoljni faktori. Cilj istraživanja. Osnovni cilj istraživanja je da se registrovanjem evociranih potencijala nastalih stimulacijom zubne pulpe uoče promene u prenošenju nervnih signala kod ispitanika pod stresom i pacijenata sa trigeminalnom neuralgijom, kao i pod dejstvom analgetika, anksiolitika i placeba. Metod istraživanja. Istraživanje je sprovedeno na klinici za Oralnu hirurgiju Instituta za Stomatologiju Stomatološkog fakulteta u Pančevu, posle dobijanja saglasnosti Etičkog odbora ustanove. U istraživanju je učestvovalo 120 ispitanika, 59 muškog pola i 61 ženskog pola, podeljenih u šest grupa. Najveći deo uzorka formiran je od studenat, dobrovoljca, Stomatološkog fakuleta u Pančevu. Pacijenti sa trigeminalnom neuralgijom dobijeni su sa Neurološkog odeljenja Opšte bolnice Pančevo, kao i sa Klinike za Maksilofacijalnu hirurgiju Instituta za Stomatologiju Stomatološkog fakulteta u Pančevu. Grupa ispitanika pod stresom bila je formirana iz zdrave mlade populacije na osnovu rezultata Skale depresivnosti, anksioznosti i stresa. Nakon potpisivanja informisanog pristanka za učešće u istraživanju ispitanicima je uzeta anamneza i izvršen neurološki i stomatološki pregled, a zatim su sami popunjavali Skalu depresivnosti, anksioznosti i stresa, jedan od najpoznatijih instrumenata, konstruisan sa namerom da u sebi objedini procenu tri neprijatna emocionalna stanja: depresivnosti, anksioznosti i stresa. Pre otpočinjanja registrovanja evociranih potencijala svim ispitanicima je izmeren arterijski pritisak i puls i određen prag percepcije bola ispitivačem vitaliteta pulpe stimulacijom gornjih centralnih skutića strujom niskog intenziteta, koji će biti stimulisani i u cilju registrovanja evociranih potencijala. Posle postizanja praga percepcije bola ispitanici su davali subjektivan iskaz o jačini stimulusa na osnovu Wong-Bakerove vizuelno deskriptivne skale. Sve navedeno je ponovljeno i nakon registrovanja evociranih potencijala. Standardizovanje uslova i parametara stimulacije i registrovanja evociranih potencijala vršeno je na ispitanicima kontrolne grupe promenom jačine impulsa, perioda ponavljanja impulsa, broja usrednjenih uzoraka, pojačenja impulsa, propusne moći gornjeg filtera i dužine snimka. Na svakom ispitaniku je urađeno pet snimanja. Kod pacijenata sa neuralgijom trigeminalnog nerva snimanje je izvođeno na oba gornja centralna sekutića. Grupama ispitanika koje su dobijale Brufen, Bromazepam ili placebo dva puta je vršeno snimanje evociranih potencijala, prvi put pre primene leka, a drugi put 45 minuta nakon primene leka. Nivo salivarne α-amilaze određivan je upotrebom aparata Nipro Salivary Amylase Monitor. Pljuvačku za određivanje nivoa kortizola sakupljali smo pomoću Salivette vaterolni. Nakon centrifugiranja iz supernatanta je određivana koncentracija kortizola ELISA imunoesej testom-Eclia. Rezultati istraživanja. Dobijeni parametri stimulacije i registrovanja su: konstantna jačina struje od 1 mA, trajanje impulsa 1 ms, period ponavljanja impulsa 1 u sekundi, broj usrednjenih uzoraka 2, pojačanje impulsa 10 µV, propusna moć gornjeg filtera 70 Hz, propusna moć donjeg filtera 0,5 Hz, dužina snimka u trajanju od 300 ms. Određene su standardne vrednosti perioda latence i amplitude talasa evociranih potencijala: N60, P100, N145 i P195 za osobe muškog pola i N55, P95, N145 i P195 za osobe ženskog pola. Ovariohisterektomi yapılan köpeklerde cerrahi yaklaşımın postoperatif ve oksidatif stres ile ağrı üzerine etkisiSunulan çalışmada, operasyon süresi ve ensizyon uzunluğu gibi farklı cerrahi yaklaşımların postoperatif ve oksidatif stres ile ağrı üzerine etkilerinin araştırılması amaçlandı. Çalışmada, kliniğimize ovariohisterektomi (OHE) amacıyla getirilen yaşları 1-3 arasında olan ve ortalama 25 ± 2.1 kg canlı ağırlığa sahip farklı ırklardan toplam 24 adet dişi köpek kullanıldı. Kontrol grubunu (n=12) tecrübeli bir hekim tarafından operasyonu yapılan köpekler oluştururken, deneme grubunu (n=12) intörn öğrenciler tarafından operasyonu gerçekleştirilen köpekler oluşturdu. İlk kan örneği operasyondan 24 saat önce (T-24) alındı. Operasyonlar genel anestezi altında gerçekleştirilirken premedikasyon sırasında (T) da kan örnekleri alındı. Aim: Immaturity of epithelial sodium channels (ENaC), which are affected by adrenocorticotropin hormone (ACTH) and cortisol, is believed to be the major underlying cause of transient tachypnea of the newborn. The aim of this study is to investigate the differences in ACTH and cortisol levels and the expression of lung ENaC α subunit of the babies in relation to mode of delivery.Methods: The study was planned as prospective cohort study. Eighteen women who underwent elective caesarean section (C/S) and fifteen women who admitted to hospital for normal spontaneous delivery (NSD) and their term 33 newborn infants were included in the study. Blood samples for ACTH and cortisol levels were collected from the mothers prior to birth and the newborn infants 1 hour after birth. Nasal mucosa species of the infants were performed to assess ENaC α subunit levels. Kadın sağlığını olumsuz olarak etkileyen aile içi şiddet olgusu, gebelik döneminde de devam edebilmektedir. Ülkemizde yapılan birçok çalışmada gebelerin şiddet açısından gebelik süreci boyunca sadece bir kez değerlendirildiği, özellikle tüm gebelik sürecini kapsayacak şekilde yapılan, perinatal sonuçları ve doğum sonrası süreci de değerlendiren çalışma sayısının az olduğu görülmektedir. Bu çalışmada, gebelikte aile içi şiddetin kortizol hormon salınımına, erken doğuma, düşük doğum ağırlığına ve emzirme durumuna olan etkisini belirlemek amaçlanmıştır. Araştırmanın evrenini Gaziantep Halide Alevli Aile Sağlığı Merkezi'nde takipli yaklaşık 412 gebe oluşturmuştur. Ancak kayıpların olabileceği tahmin edilerek 255 gebeye ulaşılmıştır. Veri toplama araçları araştırmacı tarafından geliştirilen soru formu, KYAİŞBÖ (Kadına Yönelik Aile İçi Şiddeti Belirleme Ölçeği) ve Emzirme Öz-Yeterlilik Ölçeği'nden oluşmaktadır. Çalışmamızda gebelere üç izlem yapılmıştır. Her izlemde ve yenidoğandan da kortizol hormon düzeyi için tükürük örneği alınmıştır. A simple, fast and sensitive electrochemical method for the direct determination of cortisol in commercial pharmaceutical product and human urine was developed. The electrochemical reduction and quantitative analysis of cortisol were performed in Britton Robinson Buffer Solution (BRBS, pH 2.0) with a bare glassy carbon electrode (GCE) using cyclic voltammetry (CV) and square wave voltammetry (SWV). The compound showed one irreversible and adsorption-controlled reduction peak in CV. When SWSV mode is used, the compound yielded a well-defined voltammetric response at about -1.14 V (vs. Ag/AgCl) in BRBS at pH 2.0. Experimental results revealed an excellent linear correlation between the peak current and the concentration in the range of 1-50 µg mL-1 (2.8×10-6 -1.4×10-4 mol L-1 ). Detection and quantification limits were found to be 0.069 µg mL-1 (1.9×10-7 mol L-1 ) and 0.229 µg mL-1 (6.32×10-7 mol L1 ), respectively. Amaç: Psikososyal faktörler periodontal hastalık için önemli bir risk faktörü olarak tanımlanır, konak direncini değiştirerek ve endokrin dengesizliklere neden olarak periodontal dokuları etkileyebilir. At present study, there wasaimed to evaluate the relationship between perceivedstress scale and stress hormone level and periodontalstatus.Material and Methods: There was included the totalof 72 individuals, 42 subjects with gingivitis and 30periodontal healthy subjects in this study. Clinical andradiological periodontal evaluation of all participantswere performed. Stress levels were assessed usingsalivary cortisol level and Cohen's Perceived StressScale (PSS-10) scores. Salivary cortisol level was measuredusing by ELISA method. The data were analyzedstatistically according to p 0.05 significant levels.Results: When the sociodemographic data werecompared, no statistical difference was found betweenthe groups (p 0.05). Salivary cortisol level and PSS10scores in periodontal healthy subjects were lowerthan subjects with gingivitis (p 0.05). There was apositive correlation between salivary cortisol level andperceived stress scale scores and periodontal clinicalparameters.Conclusion: There were higher levels of stress inindividuals with gingivitis. According to the presentstudy might mention that there is a positiverelationship between stress and periodontal disease.However, the further investigations are needed toexplain the relationship between stress andperiodontal disease.

Proximal femoral fractures are the most common cause of emergency admission to hospital with high postoperative morbidity. The hypothesis was that a single shot of intrathecal dexamethasone and levobupivacaine in anaesthesia for surgical correction of proximal femoral fracture in elderly patients reduces surgical stress with better quality hospitalisation. The study included sixty elderly patients with proximal femoral fracture, ASA status 2 and 3, randomised into two groups. The study group of thirty patients received 8 mg of dexamethasone and 12,5 mg of levobupivacaine 0,5 % intrathecally, DLSA group, and the parallel group of thirty patients received 12,5 mg of levobupivacaine 0,5 % intrathecally, LSA group. Plasma cortisol and glucose concentrations were assessed before and after anaesthesia, pain intensity was evaluated using Visual Analogue Scale score and hospitalisation was analysed. Results showed decreased cortisol concentrations, longer analgesia duration and shorter hospitalisation in the DLSA group. Glucose concentrations did not differ significantly between the patients in either group. Enlightening the study results collected, single shot of intrathecal administration of dexamethasone in spinal anaesthesia for surgical treatment of proximal femoral fractures reduces the stress response by decreasing plasma cortisol concentrations prolonging analgesia with better rehabilitation possibilities, hence shortening hospitalisation which explains this pattern of anaesthesia., Proksimalni prijelomi bedrene kosti su najčešći uzrok prijma u bolnicu s visokim poslijeoperacijskim morbiditetom. Rasvjetljavanjem skupljenih rezultata, jedna intratekalna injekcija deksametazona u spinalnoj anesteziji za kiruršku korekciju prijeloma proksimalnog dijela bedrene kosti snižava stresni odgovor smanjenjem plazmatske koncentracije kortizola s duljim analgetskim učinkom te boljim mogućnostima rehabilitacije, skraćujući time hospitalizaciju što opravdava ovaj anesteziološki obrazac kao metodu izbora.

Hipoteza je da jedna intratekalna doza deksametazona s levobupivakainom za kiruršku korekciju prijeloma proksimalnog dijela bedrene kosti smanjuje kirurški stres sa kvalitetnijim boravkom u bolnici. U istraživanje je bilo uključeno šezdeset pacijenata starije dobi s prijelomom proksimalnog dijela bedrene kosti, ASA status 2 i 3, te su bili randomizirani u dvije skupine. Trideset pacijenata je primilo 8 mg deksametazona i 12, 5 mg 0, 5% levobupivakaina intratekalno, DLSA skupina, trideset pacijenata u usporednoj skupini je primilo 12, 5 mg 0, 5% levobupivakaina intratekalno, LSA skupina ; tijekom anestezije za kiruršku korekciju. Plazmatske koncentracije kortizola i glukoze mjerene su prije i poslije kirurškog zahvata, intenzitet boli je određivan zbrojem VAS, te je analizirano trajanje hospitalizacije. Rezultati istraživanja su prikazali smanjene koncentracije kortizola, dulje trajanje analgezije te kraće trajanje hospitalizacije u DLSA skupini u usporedbi sa LSA skupinom. Koncentracije serumske glukoze nisu se značajnije razlikovale između skupina. Rasvjetljavanjem skupljenih rezultata, jedna intratekalna injekcija deksametazona u spinalnoj anesteziji za kiruršku korekciju prijeloma proksimalnog dijela bedrene kosti snižava stresni odgovor smanjenjem plazmatske koncentracije kortizola s duljim analgetskim učinkom te boljim mogućnostima rehabilitacije, skraćujući time hospitalizaciju što opravdava ovaj anesteziološki obrazac kao metodu izbora.

Son zamanlarda, D vitamini yetersizliği ve / veya eksikliği salgın durumuna ulaşmış olup erişkinlerde düşük kemik mineral yoğunluğu, kas zayıflığı, kanser ve obezite gibi çeşitli sağlık sorunlarıyla ilişkilendirilmiştir. Kortizol, TSH, T4 ve testosteron gibi bazı hormonların da atletik performans ve vücut kompozisyonu üzerinde önemli etkileri olduğu bilinmektedir. Bu çalışmanın amacı, genç milli atletlerde plazma 25-hidroksivitamin D [25(OH)D3] ile hormon düzeyleri, kas kuvveti, antropometrik parametreler ve beslenme profili arasındaki ilişkiyi incelemekti. Çalışmaya Türkiye atletizm milli takımında olan, atlama branşı ile ilgilenen 20 erkek ve 6 kadın sporcu (15-25 yaş) dahil edildi. Katılımcıların atletik performansları, drop sıçrama, dikey sıçrama, squat sıçrama, durarak uzun atlama ve serbest kol sıçrama gibi çeşitli atlama testleri kullanılarak değerlendirildi. Anaerobik güç (watt) her katılımcı için Lewis nomogramı kullanılarak hesaplandı. Her katılımcının vücut kompozisyonu, skinfold kaliper kullanılarak deri kıvrım kalınlıkları ile hesaplandı. Deri kıvrım kalınlığı ölçümü standart protokollere göre yedi bölgeden (göğüs, karın, triseps, suprailiak, midaksiller, subskapula ve uyluk) yapıldı. Vücut yağ yüzdesi Jackson / Pollock 7 yöntemine göre hesaplandı. Plazma 25(OH)D3 ve diğer hormon seviyeleri, kit protokolu talimatlarına göre ELISA kitleri kullanılarak tespit edildi. Ayrıca sporcular yaşam tarzları, beslenme profilleri ve güneşe maruz kalma süreleri ile ilgili ayrıntılı bir anket doldurdu. İstatistiksel analizler SPSS 18.0 kullanılarak yapıldı. Bağımlı ve bağımsız değişkenler arasındaki ilişkiyi analiz etmek için Spearman'ın korelasyon testi kullanıldı ve p değeri ≤ 0,05 istatistiksel olarak anlamlı kabul edildi. Çalışmada elde ettiğimiz sonuçlara göre, D vitamini yetersizliği ve / veya eksikliği oranı kadınlarda %66, erkeklerde %44 idi. Plazma 25(OH)D3 seviyeleri ile performans ölçümleri arasında anlamlı bir ilişki bulunamadı. Bununla birlikte, plazma 25(OH)D3 seviyeleri ile antropometrik parametreler (göğüs, karın, triceps ve suprailiak deri kıvrım kalınlığı) ve kadınlarda vücut yağ yüzdesi (p, Vitamin D, a vital nutrient, exerts important physiological effects on bone mineralization, muscle function, cellular growth, immune response and glucose metabolism. Recently, vitamin D insufficiency and/or deficiency has reached epidemic proportions and has been linked to several health problems such as low bone mineral density, muscle weakness, cancer and obesity in adults. Some hormones such as cortisol, TSH, T4 and testosterone have also important effects on athletic performance and body composition. The objective of the present study was to examine the relationship between plasma 25-hydroxyvitamin D [25(OH)D3] and hormone levels, muscle strength, anthropometric parameters and nutritional profile in national young athletes. Twenty male and six female athletes (15-25 years) performing jumping events in Turkish national team were included in the study. Athletic performances of the participants were evaluated by using several jump tests such as drop jump, vertical jump, squat jump, standing long jump, and free arm jump. Anaerobic power (watt) was calculated by using Lewis nomogram for each participant. Body composition of each participant was estimated from skinfold thickness using a skinfold caliper. The skinfold measurement was performed at seven sites (chest, abdominal, triceps, suprailiac, midaxillary, subscapular and thigh) according to standard protocols. Body fat percentage was calculated according to Jackson/Pollock 7 method. Plasma 25(OH)D3 and other hormone levels were detected by using ELISA kits according to the manufacturer's instructions. Also, athletes were completed a detailed questionnaire relating to their lifestyle, nutritional profile and sun exposure practices. Statistical analysis was performed using SPSS 18.0. Spearman’s correlation test was used to analyze the relationship between the dependent and independent variables. A p value ≤ 0.05 was considered statistically significant. According to our results, the rate of vitamin D insufficiency and/or deficiency was 66% in females and 45% in males. No significant relationship was found between the plasma 25(OH)D3 levels and performance measures. V raziskavi smo ugotavljali intenzivnost in trajanje izločanja stresnega hormona kortizola v slini konj, ob tem pa smo spremljali srčni utrip. Obe metodi veljata za neinvazivni. Delo s konji je potekalo od meseca februarja do konca maja 2018 na kmetiji Kolarič v Obrežu. V poskus smo vključili dva mlada in neujahana konja iz istega hleva – 3 leta staro žrebico slovenske hladnokrvne pasme ter 3 leta starega kastrata slovenske toplokrvne pasme. Sam poskus smo razdelili na predpreizkus in tri faze. V predpreizkusu je potekala habituacija, tako sta se konja navadila na merilno napravo ter sam postopek poskusa. V prvi fazi smo opravili le z lonžiranjem s hlevsko uzdo, v drugi fazi smo uvedli jahalno uzdo z navadno mehko brzdo, v tretji fazi pa še podsedelnico ter sedlo. Lonžiranje, ki je skupno trajalo 30 minut, smo izvajali na prostem, v ograjenem prostoru s travnato površino. Vzorci sline so v času posamezne vaje bili odvzeti 5-krat. Višje vrednosti srčnega utripa so se pojavljale pri hladnokrvnem konju ter so znašale 64,76 min-1, pri toplokrvnem konju 62,46 min-1. Večje razlike smo opazili pri izmerjenih vrednostih izločenega kortizola. Pri toplokrvnem konju so med izvajanjem vaje v prvi fazi znašale 0,105 µg/dl, v drugi 0,111 µg/dl, ter v tretji fazi 0,142 µg/dl. Pri hladnokrvnem pa so bile vrednosti med izvajanjem vaje v prvi fazi 0,076 µg/dl, v drugi 0,114 µg/dl ter v tretji fazi 0,122 µg/dl. The secretion intensity and the duration of the stress hormone cortisol in horse saliva, as well as their heart rate were measured. Both methods are considered non-invasive. The research lasted from February to the end of May 2018, on the Kolarič farm in Obrež. For the experiment we chose two young and untrained horses from the same barn, a 3 years old filly who is a Slovenian cold-blooded breed and a 3 years old castrated Slovenian warm-blooded breed. We divided the experiment into a pre-test and three phases. In the pre-test the horses were exposed to the procedure so that they could get used to the measuring equipment and then the procedure of the test. In the first phase we only did longeing with the barn rein. In the second phase we introduced a riding bridle with an ordinary soft bit and in the final third phase, a saddle pad and the saddle longeing which lasted 30 minutes. The research had taken place outdoors, in a fenced grassy area. Saliva samples from the horses were taken 5 times. A higher heart rate was recorded with the cold-blooded horse, but when the results we calculated, it showed a smaller difference. For the cold-blooded horse, the heart rate was 65 beats per min., and the warm blooded had a heart rate of 62 beats per minute. Bigger differences were recorded in the cortisol secretion. During the exercise, the warm-blooded horse had 0,105 µg/dl in the first phase, 0,111 µg/dl in the second, and 0,142 µg/dl in the third phase. The cold-blooded horse had 0,076 µg/dl in the first phase, 0,114 µg/dl in the second, and 0,122 µg/dl in the third phase.

Namen diplomskega dela je bil raziskati, kako sistem reje in način odvzema mleka vplivata na koncentracijo oksitocina in kortizola v kozjem mleku. V raziskavo smo vključili 20 koz slovenske srnaste pasme, ki jih redijo v PRC Logatec, kjer je poskus potekal. Vzeli smo 10 koz iz ekološke in 10 iz konvencionalne reje. Posamezna koza je bila vključena v poskus štiri zaporedne tedne. Vsak teden smo odvzeli po dva vzorca mleka – med molžo in med sesanjem, tako, da smo ob zaključku poskusa imeli po 8 vzorcev mleka na kozo, 4 vzorce odvzete med molžo in 4 med sesanjem. Vsebnost kortizola in oksitocina v vzorcih mleka so izmerili na Veterinarski fakulteti v Ljubljani. Rezultati so pokazali, da so koze iz ekološke reje »srečnejše«, saj so v povprečju imele višje koncentracije oksitocina in nižje koncentracije kortizola v mleku, kar pomeni, da je ekološka reja za koze manj stresna kot konvencionalna. V nadaljevanju smo ugotovili, da predstavlja molža za koze večji stres kot sesanje, saj so bile vrednosti kortizola med sesanjem značilno nižje kot med molžo. Proti pričakovanjem pa je bila med molžo, v povprečju, tudi višja raven oksitocina v mleku, kar pa bi lahko pripisali enemu izmed glavnih sprožilcev »sreče« pri domačih živalih - krmi, ki je naznanjala začetek molže. Tudi kozliči koz, katerih raven oksitocina v mleku je bila višja in katerih raven kortizola je bila nižja, so priraščali hitreje – te rezultate smo dobili s podatki dnevnih prirastov, ki smo jih izračunali iz vmesnih tehtanj. Ugotovili smo tudi, da na raven oksitocina in kortizola v mleku statistično značilno vpliva tudi število somatskih celic v mleku, zaporedna laktacija in število kozličev na kozo. The aim of B. Sc Thesis was to investigate how the farming method and the method of taking milk from the Alpine goats, affects the milk oxytocin and cortisol concentrations. There were 20 Alpine goats included in our study, from which there were 10 from organic and 10 from conventional farming. The goats were located at PRC (Pedagogical Research Centre) Logatec. Each goat participated in the study for 4 consecutive weeks each week we took two samples from each individual. We therefore had 8 samples per goat– 4 taken during milking and 4 during suckling. The measurements were taken at the Veterinary Faculty in Ljubljana. Our results showed that goats from the organic farming are »happier« and under less stress, based on a higher oxytocin concentrations and lower cortisol concentrations, compared to the conventional farming. Milking caused higher stress response in goats than suckling with cortisol concentrations being significantly higher. In contrary to our expectations, the oxytocin concentrations during milking were higher than during suckling. This phenomenon could be explained with the »main source of happiness« in domestic animals – food, which may represent the trigger before milking started. The kids of goats, which had higher levels of oxytocin and lower levels of cortisol grew faster than kids of goats with reverse values – calculated from daily gain, which was calculated from the intermediate weighing. There were additional factors that influenced the milk hormonal concentrations, such as: the number of somatic cells, the successive lactation and the number of kids per goat.

Cilj ove doktorske disertacije je bio da se ispita uticaj upotrebe antiektoparazitika na sprečavanje nastanka hroničnog stresa, kroz određivanje koncentracije kortizola i kortikosterona u krvi, dlaci i mleku, kao i na proizvodne sposobnosti životinja, kroz određivanje količine proizvedenog mleka, sastava mleka i broja somatskih ćelija u mleku. Pre izvođenja samog ogleda utvrđene su bazalne vrednosti kortizola u krvi, dlaci i mleku kod holštajn rase goveda, uz ispitivanje uticaja laktacije i rase na koncentracije ovog hormona. Za utvrđivanje bazalnih vrednosti kortizola u krvi i dlaci odabrano je 25 jedinki holštajn rase (13 krava i 12 junica) i 24 jedinke buša rase (13 krava i 11 junica). Kod istih krava određena je i bazalna vrednost kortizola u mleku, dok je kod junica određena i razlika u koncentraciji kortizola u opranoj i neopranoj dlaci. Kod dodatnih 12 krava holštajn rase i istih junica holštajn rase utvrđena je razlika između konentracije kortizola u beloj i crnoj dlaci. Za ispitivanje vrednosti indikatora stresa u uslovima tretmana antiektoparaziticima odabrano je 26 krava holštajn rase. Ogled je izveden tokom letnjeg perioda 2014. godine. Krave su na početku ogleda podeljene u dve grupe, netretirana (n=13) i tretirana (n=13), ali su zbog pojave mastitisa iz tretirane grupe isključene 3 krave, a iz netretirane 1 krava. Pre izvođenja ogleda posmatranjem i daljom identifikacijom je utvrđeno značajno prisustvo ektoparazita, štalske muve (Stomoxys calcitrans), na kravama. Odabrana grupa krava je tretirana antiektoparazitikom na bazi ciflutrina 0., 28. i 56. dana ogleda. Od svih krava su tokom trajanja ogleda uzimani uzorci krvi, mleka i dlake 0., 21., 42., 63. i 84. dana ogleda (periodi P0, P1, P2, P3 i P4), za određivanje koncentracije kortizola i kortikosterona kao i uzorci mleka za određivanje hemijskog sastava i broja somatskih ćelija u mleku. Takođe, u uzorcima krvi određena je koncentracija glukoze, ukupnih proteina, albumina, globulina, uree, ukupnog bilirubina i aktivnost enzima kreatin kinaze i alkalne fosfataze. Hipoteza: Očna hipertenzija i glaukom kao komplikacija debljine - Kortizol je rješenje patofiziološke slagalice?

The aim of the current study was to evaluate the level of saliva cortisol and heart rate in lactating goats depending on different environmental condition, from early spring to the summer heat in the mountainous region of Croatia with a moderate climate zone (average winter air temperature from -2°C to -4°C and summer below 20°C). Ten lactating goats (6 Saanen and 4 Alpine) kept confined, occasionally tethered, were included in the study. The barn was wooden, thermally insulated, divided into five equal pens divided by wooden partitions. Each pen housed two goats on deep hay bedding and ensuring sufficient space allowance per animal. Saliva sampling was carried out in early April when goats were kept exclusively in the barn and occasionally tethered, at the end of May when they spent part of the day grazing freely, and the end of July when they spent only the night in barn. Saliva samples of saliva were taken at 11 a.m. using special equipment and frozen until analysis, and heart rate was determined by palpation of a. femoralis. Samples were analysed by the enzyme immunoassay (EIA). Following statistical analysis of the results, no statistically significant differences in cortisol levels and heart rate were observed during the sampling period. Since the hypothalamus- pituitary-adrenal gland is extremely susceptible to environmental changes, cortisol level is considered to be a credible indicator of the ability of animals to adapt to changing environmental conditions. Based on the obtained results, it is possible to conclude that the investigated goats were kept in conditions and in a way that was not stressful to them and are almost ideal for achieving their optimal production performance, and goats adapted very well to the environmental changes during different seasons., Cilj ovog istraživanja bio je procijeniti razinu kortizola u slini mliječnih koza te vrijednosti b la ovisno o različitim uvjetima držanja, od ranog proljeća do vrhunca ljetnih vrućina u unutrašnjem planinskom području Hrvatske s umjerenom kontinentalnom klimom (zimi se prosječne temperature zraka kreću od -2 °C do -4 °C, a ljeti ispod 20 °C). Istraživanjem je bilo obuhvaćeno deset mliječnih koza u laktaciji (6 sanskih i 4 alpskih) koje su cijelu zimu provele zatvorene u oboru i povremeno na vezu. Obor je bio drveni, toplinski izoliran, podijeljen drvenom pregradom u 5 jednakih odjeljaka. U svakom su odjeljku bile smještene po dvije koze na dubokoj stelji od sijena i na površini više no dostatnoj po grlu. Uzorkovanje sline i utvrđivanje vrijednosti b la provodilo se početkom travnja kada su koze držane isključivo u zatvorenom i povremeno na vezu, krajem svibnja kada su dio dana provodile slobodne na ispaši i krajem srpnja kada su u oboru jedino provodile noć. Uzorci sline korištenjem posebnog pribora uzimani su ujutro u 11 sati, i smrznuti do izvođenja pretrage. Vrijednosti b la utvrđivane su palpacijom a. femoralis. Uzorci sline analizirani su imunoenzimnim postupkom (EIA). Nakon statističke obrade rezultata utvrđeno je da nema statistički značajne razlike u razini kortizola i vrijednosti b la tijekom različitih mjeseci uzorkovanja. S obzirom da je os hipotalamus-hipofiza-nadbubrežna žlijezda iznimno osjetljiva na promjene u okolišu, razina kortizola uzima se kao vjerodostojan pokazatelj sposobnosti prilagodbe životinje na promijenjene okolišne uvjete. Na temelju dobivenih rezultata moguće je zaključiti da su istraživane koze držane u uvjetima i na način koji za njih ne predstavljaju stres te su gotovo savršeni za postizanje njihova optimalnog proizvodnog potencijala, kao i da su se na promjene u okolišu tijekom različitih godišnjih doba vrlo dobro prilagođavale.

Over the past three decades, it has been confirmed that the alarming increase in number of obese patients is related to the sleep duration and its quality. Neurohumoral response to short sleep duration and poor sleep quality leads to decreased levels of melatonin, leptin and orexin, asto increased levels of cortisol, ghrelin and neuropeptide Y. Such an inaccurate regulation contributes both to excessive intake of energy-dense diet and to the reduction of energy expenditure during physical activity. The bachelor thesis summarizes recent information about the relationship of sleep and obesity, focusing on hormones and peptides involved in the regulation of energy balance processes, including the posssibility to use their mechanisms for weight control.

Before the beginning of the experiment, the basal values of blood, hair and milk cortisol concentrations were established for Holstein breed, including examination of effects of lactation and breed on concentration of this hormone. Twenty five Holstein breed animals (13 cows and 12 heifers) and 24 Busha breed animals (13 cows and 11 heifers) were chosen for determination of blood and hair basal cortisol concentrations. Bu çalışma, östrus senkronizasyonunusağlamak amacıyla kullanılan CIDR uygulamasının, Kıvırcık ve Karacabey Merinosukoyunlarında uygulama esnasında bir stres kaynağı olup olmadığının belirlenmesiamacıyla yürütülmüştür. Bu amaçla koyunlara CIDR aparatlarının takıldığı 1. gün,7. gün ve çıkarıldığı 14. gün alınan kan örneklerinde T3 (triiodothyronine), T4(thyroxine) ve kortizol hormonu seviyeleri ölçülmüştür. Bu çalışmada 40 başKıvırcık ve 40 baş Karacabey Merinosu olmak üzere toplam 80 baş koyunkullanılmıştır. Koyunların boyun toplardamarından her üç dönemde 10 ml kanEDTA’lı tüplere alınarak ticari RIA kitleri ile T3, T4 ve kortizol konsantrasyonlarıbelirlenmiştir. Araştırma; miyofasiyal ağrı sendromu (MAS) olan bireylerde tetik nokta enjeksiyonu sırasında uygulanan inhaler aromaterapinin ağrı, anksiyete, konfor ve kortizol düzeyine etkisini incelemek amacıyla plasebo-randomize kontrollü olarak yapılmıştır. Araştırmanın örneklemini; Eylül 2017-Mart 2018 tarihleri arasında Ege Üniversitesi Algoloji Polikliniği'ne başvuran MAS tanısı almış, ilk kez tetik nokta enjeksiyonu yapılacak ve örnekleme alınma kriterlerine uyan 22 aromaterapi, 22 plasebo ve 22 kontrol olmak üzere 66 hasta oluşturmuştur. Miyofasiyal ağrı sendromu olan bireylere tetik nokta enjeksiyonu sırasında; aromaterapi grubuna difüzör ile lavanta yağı, plasebo grubuna kokusuz bebe yağı inhalasyonu uygulanmıştır. Kontrol grubuna rutin tedavi dışında herhangi bir uygulama yapılmamıştır. Araştırma verileri; Birey Tanılama Formu, Görsel Analog Ölçek (Visuel Analog Scale-VAS), Durumluk Kaygı Envanteri, Distress Termometresi, Genel Konfor Ölçeği, Hasta ve Hekim Memnuniyet Skalası ve kortizol için tükürük örneği alınarak yüz yüze görüşme tekniği ile toplanmıştır. Bu olumlu etkiler göz önüne alındığında bağımsız bir hemşirelik müdahalesi olarak kullanılabileceği düşünümektedir., The study was conducted as a placebo-randomized controlled trial to examine the effects of inhaler aromatherapy administered during trigger point injection on pain, anxiety, comfort and cortisol levels in patients with myofascial pain syndrome (MPS). The sample of the research; Between September 2017 and March 2018, MAS who had applied to the Ege University Algology Polyclinic was diagnosed and 66 patients, including 22 aromatherapy, 22 placebo and 22 controls, were selected for the first injection of the trigger point. During injection of the trigger point for individuals with myofascial pain syndrome; the aromatherapy group with diffuser and lavender oil, odorless baby oil inhalation was applied to the placebo group. The control group was not administered any other routine. Research data; Personal Identification Form, Visual Analogue Scale (VAS), State Anxiety Inventory, Distress Thermometer, General Comfort Scale, Patient and Physician Satisfaction Scale and saliva sample for cortisol were collected by face to face interview technique. In the analysis of the data; Statistical Package for Social Science (SPSS) package program, analysis of cortisol values was obtained by using Graphpad Prism v7 program. In the evaluation of the data, the normal distribution suitability of numerical data was evaluated by shapiro-wilk test, descriptive statistics were shown by number and percentage, mean and standard deviation of measurement data. Independent samples were t test, one way analysis of variance, Pearson correlation analysis, chi-square, Kruskal Wallis, Wilcoxon, Friedman and Mann Whitney U tests. The results were evaluated at 95% confidence interval, p 0.05). In addition, the median value of the aromatherapy group was significantly lower after the distress thermometer treatment, while the placebo and control group increased significantly. Compared to the aromatherapy, placebo and control groups, the final total average in the aromatherapy group was significantly higher when compared to the general comfort scale, but significantly lower in the placebo and control groups (p 0.05). In the aromatherapy group, respiratory rate, heart rate and diastolic blood pressures were significantly lower than those of the control group (p 0.05). However systolic blood pressure of the placebo control group after treatment were significantly higher (p 0.05). As a result of this study; it is observed that aromatherapy administered during trigger point injection in patients with myofascial pain syndrome can decrease pain intensity, anxiety and stress level, increase comfort level and patient satisfaction. Considering these positive effects, it can be used as an independent nursing intervention.

Steroidlerin tayini genel olarak immunoassay yöntemler ve kromatografik yöntemlerle yapılmaktadır. Söz konusu yöntemler zaman alıcı ön işlemler ve pahalı ekipmanlar içerir. Ancak mikrobiyal biyosensörler ve moleküler damgalanmış polimer (MIP) tabanlı elektrokimyasal sensörlerin kullanılmasıyla hızlı, duyarlı, ucuz ve seçimli steroid tayini yapılabilir. Bu tez çalışmasında kortizol-kortizon ve prednisolona yönelik MIP sensörleri, kolesterole yönelik ise mikrobiyal biyosensör sistemi geliştirilmiştir. MIP sensörleri altın nanopartikül kaplanmış grafit elektrot yüzeyinde pirolün kalıp molekül varlığında elektropolimerizasyonu yöntemi ile hazırlanmıştır. Sensör cevapları Diferansiyel Puls (DP) yöntemi ile Ferri/Ferro çözeltisinden izlenmiştir. Kortizon-kortizol ve prednisolona yönelik geliştirilen MIP sensörlerinde kalıp molekül/monomer oranı, inkübasyon ve ekstraksiyon süresi optimizasyonları yapılmıştır. Kortizon-MIP sensörünün doğrusal tayin aralığı 1-100 μM, tayin limiti 0,25 μM olarak bulunurken Prednisolon-MIP sensörünün doğrusal tayin aralığı 1-75 μM, tayin limiti ise 0,35 μM olarak bulunmuştur. Pseudomonas sp. ve katalaz enzimi çok duvarlı karbon nanotüp (MWCNT)/iyonik sıvı (IL)/karbon pasta elektrot (CPE) içerisine immobilize edilmiştir. Biyosensörün immobilizasyon aşamaları CV ve impedans spektroskopisi ile karakterize edilmiştir. Ölçümlerde mediyatör sistemi olarak hidrokinon kullanılmıştır. Such methods include time consuming preprocessing and expensive equipment. However, using microbial biosensors and MIP based electrochemical sensors, rapid, sensitive, inexpensive and selective steroid detection can be performed. In this thesis, MIP sensors for cortisol-cortisone and prednisolone and microbial biosensor system for cholesterol have been developed. MIP sensors were prepared by electropolymerization method in the presence of pyrole and template molecule on gold nanoparticle coated graphite electrode surface. Sensor responses were monitored by Ferri / Ferro solution with Differential Pulse (DP) method. Template molecule / monomer ratio, incubation and extraction duration optimizations were performed in the MIP sensors that developed for cortisone-cortisol and prednisolone. The linear detection range of the Cortisone-MIP sensor was found to be 1-100 μM and the detection limit 0.25 μM, while the linear detection range of the Prednisolon-MIP sensor was found to be 1-75 μM and the detection limit 0.35 μM. In the reproducubility experiments average value (x), standard deviation (±) and variation coefficient (CV %) values for the 50 M cortisone (n = 10) by the Cortisone-MIP sensor were calculated to be 48.10 μM, ± 2.42 M and 5.04%, respectively, and for the prednisolone (n = 10) were found as 48.69 μM, ± 1.06 μM and 2.189% respectively. MIP sensors have been used to test substrate specificity, interferenceeffect, and storage stability. In addition, the developed MIP sensors were tested for cortisol-cortisone and prednisolone in lyophilized urine and serum samples. Within the microbial biosensor development step, cholesterol metabolism induced and organic phase tolerant Pseudomonas sp. and catalase enzyme were used as a biocomponent of biosensor. Pseudomonas sp. and the catalase enzyme were immobilized into multi-wall carbon nanotube (MWCNT) / ionic liquid (IL) / carbon paste electrode (CPE). Immobilization steps of the biosensor were characterized by CV and impedance spectroscopy. Hydroquinone was used as the mediator system in the measurements. The amperometric working potential was found to be -0.2 V. Optimization and characterization studies were also carried out by amperometric method. The optimization of working conditions of microbial biosensor were carried out and optimum temperature and pH were found to be 35 °C and 7.0 (Phosphate buffer, 50 mM). The effect of amounts of bacteria on the biosensor responses was investigated. The linear range of this microbial biosensor that developed for the determination of cholesterol was found to be 5-600 μM and the detection limit was found as 1.52 μM. The effect of steroid compounds such as cortisone, cortisol, testosterone, β-sitosterol and estradiol, and the presence of endogenous analytes such as glucose and ascorbic acid on the biosensor response was investigated in substrate specificity and interference effect experiments. After optimization and characterization studies of the microbial biosensor, the biosensor response to cholesterol in saturated cyclohegzan was investigated and also responses obtained were compared with response obtained by water phase. Cholesterol determination was carried out both in cyclohegzan and in water phase by developed microbial biosensor.

It has been established that rubber mats improve lying comfort in stall-housed gilts at low ambient temperatures. The present study investigated whether rubber mats influence stress levels in gilts by improved lying comfort. The study was performed in a commercial pig farm service unit, during the 28-day breeding cycles in autumn and winter. During both cycles, the control and experimental groups of gilts (9 gilts each) were housed in gestation stalls with a concrete slatted floor; in the experimental groups, the floor was additionally coated with adjusted 2-cm thick rubber mat. Stress level was assessed by determination of cortisol concentrations in gilt serum on days 1, 8, 15 and 28 of each cycle, when the postural behaviour of gilts was observed. Proksimalni prijelomi bedrene kosti (PPBK) najčešći su uzrok prijma u bolnicu za kiruršku rekonstrukciju kuka s visokim poslijeoperacijskim morbiditetom. Hipoteza je da jedna intratekalna doza deksametazona u spinalnoj anesteziji s levobupivakainom snižava poslijeoperacijsku bol i kognitivne promjene svijesti. U istraživanje je uključeno 60 pacijenata s PPBK, ASA statusa 2 i 3 randomiziranih u dvije skupine: DLSA skupina je primila 8 mg deksametazona tijekom uvođenja spinalne anestezije sa 12.5 mg 0.5%-tnog levobupivakaina. LSA skupina primila je samo levobupivakain. Poslijeoperacijska kognitivna promjena svijesti procijenjivana je CAM-ICU bodovnom ljestvicom, intenzitet boli zbrojem VAS te su mjerene vrijednosti kortizola i glukoze u krvi prije i poslije operacije. Kirurška korekcija poslije 48 sati od prijma učinjena je kod 63% pacijenata. U skupini LSA koristio se efedrin (P=0,044). U skupini DLSA zabilježeno je kraće trajanje hospitalizacije (P=0.045), kraće vrijeme od operacije do otpusta (P=0,044), manji udio POKD i delirija (P=0,043), kraći nastup bloka(P=0,005), dulje trajanje analgezije (P, Proximal femur fractures are the most common cause of emergency admission to hospital with high postoperative morbidity. The hypothesis is that one single shot of dexamethasone with levobupivacaine in spinal anesthesia reduces postoperative pain and cognitive disturbances. The study included 60 patients with proximal femur fracture, ASA status 2 and 3 who were randomized into two groups. DLSA group received 8 mg of dexamethasone and 12.5 mg of 0.5% levobupivacaine intrathecally. LSA group received only levobupivacaine. The postoperative cognitive disturbance was evaluated with CAM-ICU score scale, the pain intensity was measured with VAS scale and cortisol and glucose samples were taken before and after surgery. Surgical correction was made in 63% of patients in 48 hours after admission. Ephedrine was used in LSA group, P=0,044. 1 Abstract Depressive disorders are one of the three most frequent diseases causing disability of everyday life of humans. Its occurrence in the population is rapidly increasing. Etiology of depression is unclear, and the treatment usually only ameliorates its symptoms. In patients, there were identified signs not only of chronic stress, which has been associated with depression for quite a long time, but also signs of chronic inflammation in the body. This has led to focusing on proinflammatory cytokines and their connection to chronic stress and depressive symptomatology. We are also interested in the causal link between pro-inflammatory markers and stress that has not yet been unequivocally clarified. The aim of this study is to combine the knowledge about the influence of chronic stress on the development of depressive disorder gained from animal and human models. Additionally, to combine the knowledge of the effect of specific proinflammatory cytokines on the development of the depressive disorder and the change in brain structures morphology which may underlie the symptoms of this disease.

U fiziološkom odgovoru na stres i kontroli stresnog odgovora uglavnom sudjeluju dvije komponente neuroendokrinog sustava, simpatičko-medularna (SAM) os, koja regulira prvi odgovor na stres i os hipotalamus-adenohipofiza-kora nadbubrežne žlijezde (HPAC), čija se aktivacija uočava 15 do 20 minuta nakon stresnog podražaja. Cilj ovog istraživanja bilo je ispitati bazalnu, fiziološku aktivnost SAM i HPAC osi te njihovu povezanost s nizom kovarijabli (spol, tip škole, turnus pohađanja nastave, ritam spavanja i budnosti te demografska, akademska i društvena obilježja) u zdravih učenika koji pohađaju završne razrede srednjih škola. Specifični ciljevi uključivali su ispitivanje koncentracije kortizola i aktivnosti alfa-amilaze kao salivarnih biljega odgovora na stres te istraživanje njihove povezanosti sa subjektivnim procjenama razine stresa i sa strategijama suočavanja sa stresom, kao i s polimorfizmima jednog nukleotida (SNP) gena za mineralokortikoidni i glukokortikoidni receptor (rs5522, rs6189, rs6190) te za vezni protein 5 za FK506 (rs1360780). U istraživanje su bila uključena 903 maturanta gimnazija i strukovnih škola iz četiri najveća grada u Hrvatskoj (Zagreb, Split, Rijeka i Osijek). Uzorci sline prikupljeni kod kuće, tijekom jednog radnog dana u tri vremenske točke (po buđenju, 30 do 45 minuta nakon buđenja i neposredno prije lijeganja), uzeti su za analizu koncentracija kortizola dok se aktivnost alfa-amilaze mjerila samo u jednom uzorku, neposredno po buđenju. Kako bi se ispitao cirkadijalni ritam lučenja kortizola analizirani su i indeksi lučenja kortizola (CAR, DCD, AUCG). Učenice su imale statistički značajno veće koncentracije kortizola u jutarnjim satima, dok se koncentracije kortizola neposredno prije lijeganja nisu razlikovale među spolovima. Učenice imaju različite obrasce priprema za izazove koji ih očekuju tijekom nadolazećeg dana u odnosu na učenike, imaju značajno viši CAR, izraženiji DCD, veći AUCG u odnosu na učenike koji su imali statistički značajno veću aktivnost alfa-amilaze. Turnus pohađanja nastave, koji svakako posljedično utječe na ritam spavanja i trajanje budnosti, pokazao se kao prediktor lučenja kortizola. Statistički značajne razlike većine varijabli kortizola potvrđene su između učenika koji pohađaju različite turnuse nastave (SCC30-45, CAR, DCD i AUCG). Učenici koji su se budili ranije i imali duže trajanje budnosti imali su veći CAR, niži DCD i veći AUCG. Nisu potvrđene statistički značajne razlike niti jedne ispitivane varijable u učenika koji pohađaju različite tipove škola (gimnazije vs. strukovne škole). Također, statistički značajne razlike između učenika i učenica bile su uočene i u subjektivnoj procjeni stresa, suočavanju te njihovoj povezanosti s koncentracijama i indeksima kortizola. Zabrinutost za budućnost su oba spola procijenila kao najstresniju domenu života, dok su učenice imale statistički značajno veću razinu ukupnog stresa u odnosu na učenike te su više koristile aktivno suočavanje sa stresom. U ovom su istraživanju pronađene statistički značajne razlike povezanosti alela G polimorfizma rs 5522 s višom koncentracijom kortizola po buđenju (P=0,036), nižim CAR-om (P=0,021), dok je povezanost s DCD-om bila granična (P=0,050). Može se pretpostaviti kako učenici s dokazanim alelom G imaju niži CAR te zbog toga pokazuju lošiju pripremljenost za stresne izazove u nadolazećem danu, unatoč nešto izraženijim vrijednostima DCD-a. Ovo istraživanje ukazuje na potrebu definiranja protokola prikupljanja uzoraka sline u nekliničkim uvjetima i metodoloških smjernica za populacijska istraživanja, ponajprije zbog postojanja niza kovarijabli koje utječu na mjerenje koncentracije kortizola i aktivnosti alfa-amilaze. Dobiveni rezultati doprinose saznanjima o spolnim razlikama u funkciji HPAC osi, koje bi se trebale primijeniti i tijekom provedbe preventivnih mjera u adolescenata te ranijeg otkrivanja skupina koje su posebno osjetljive na stresne poticaje. In the stress response, the SAM axis is first activated. About fifteen to twenty minutes following the SAM activation, the HPA axis is activated. The aim of the study was to examine basal, physiologically activity of the SAM and HPAC axis activity and to determine associations of various covariates (gender, school type, school shift, sleep-wake rhythm, demographic, academic and life style characteristics) with altered daily salivary cortisol profiles in healthy students attending finishing grades of secondary schools. Specific aims of the study were to analyse concentration of cortisol and activity of alpha- amylase, as salivary markers of stress reaction, to determine association of salivary markers of stress with stress perception and coping, and to determine associations of salivary markers of stress with single nucleotide polymorphisms (SNP) in the genes for the mineralocorticoid and glucocorticoid receptors (rs5522, rs6189, rs6190) and FK506 binding protein 5 (rs1360780). The study included 903 secondary school students enrolled in the finishing classes of gymnasiums and vocational schools from the four largest cities in Croatia (Zagreb, Split, Rijeka and Osijek). The saliva samples were collected at students’ homes, over the course of one weekday. Salivary cortisol was sampled at three time points: at awakening, 30 to 45 after wakening and at bedtime. Salivary alpha-amylase was sampled at awakening. In order to analyse the circadian rhythm of salivary cortisol secretion, three indexes were analysed (CAR, DCD and AUCG).Females had higher morning concentrations of salivary cortisol than males, whereas bedtime cortisol concentrations were not different in females and males. Females had different salivary cortisol profile and different mechanisms for preparing for the anticipated stress of the upcoming day, higher CAR, steeper DCD and larger AUCG than males. On the other side, males had higher activity of salivary alpha-amylase. School shift, and therefore the sleep-wake up rhythm, was an important predictor of the secretion of salivary cortisol. There were statistically significant differences in two school shifts with respect to the majority of salivary cortisol measures (SCC30-45, CAR, DCD and AUCG). Students who woke-up earlier and were longer awake had larger CAR, flatter DCD and larger AUCG. Students from two types of schools (gymnasiums vs. vocational schools) did not differ in any of the measured salivary variables. Furthermore, female and male students differed significantly in stress perception, coping and associations between stress perception, coping and salivary cortisol concentrations and indexes. Future was rated as the most stressful problem domain in both sexes. The perception of total stress was higher in females than males, and females used more active coping than males. Finally, we found statistically significant differences in associations between G allele of rs 5522 polymorphism and higher cortisol concentration at awakening (P=0.036), lower CAR (P=0.021) and higher DCD (P=0.050), but at borderline. It can be hypothesized that the G allele is associated with the lower CAR and therefore, decreased ability of preparing for the anticipated stress (despite borderline association with the steeper DCD). This study points to the need for defining the protocol for salivary sampling in non-clinical conditions and developing methodological guidelines for population-based research, primarily due to the number of co-variables that affect the concentration of salivary cortisol and the activity of salivary alpha-amylase. Obtained results contribute to better understanding of sex differences in the functioning of the HPA axis, which should be considered in the prevention of various psychopathological conditions in adolescence and used for earlier identification of vulnerable groups at high risk of stress-related health problems.

Introduction: Pineal region is a deep-seated part of the brain surrounded by highly eloquent structures. Differential diagnosis of space-occupying lesions in this region encompasses pineal gland cysts, pineal gland tumours, metastases, germ cell tumours, meningiomas, gliomas, hemangioblastomas and neuroectodermal tumours. In this thesis, I focused mainly on patients with pineal cysts, which is a benign affection of the human pineal gland on the borderline between pathology and normality. The clinical management of patients with a pineal cyst remains controversial, especially when patients present with non-specific symptoms. A melatonin secretion in patients with a pineal cyst before and after a pineal cyst resection has not been studied yet and the effect of surgery on human metabolism is unknown. Materials and Methods: We performed a prospective study between 2000 and 2016. All patients with a pineal cyst larger than 7 mm were included. Epidemiological data, presenting symptoms, surgical results and radiographic and clinical follow-up were documented. Araştırma, koroner anjiyografi (KAG) ve perkütan transluminal koroner anjiyoplasti (PTKA) işlemi öncesi uygulanan refleksolojinin hastaların fizyolojik, psikolojik ve bazı biyokimyasal parametreler üzerine olan etkisini belirlemek amacıyla randomize kontrollü olarak gerçekleştirilmiştir. Araştırmanın evrenini, Aralık 2015- Ağustos 2016 tarihleri arasında Ege Üniversitesi Tıp Fakültesi Hastanesi Kardiyoloji Kliniğine başvuran, ilk kez KAG ve PTKA yapılacak olan, örneklemini ise evren içerisinden basit ve tabakalı randomizasyon yöntemi ile seçilen, örnekleme alınma ölçütlerine uygun ve araştırmaya katılmayı kabul eden 112 hasta (30 KAG refleksoloji, 30 KAG kontrol, 26 PTKA refleksoloji, 26 PTKA kontrol) oluşturmuştur. Araştırmada veri toplama aracı olarak; KAG ve PTKA'dan 90 dakika önce Birey Tanılama Formu, Durumluk ve Sürekli Kaygı Ölçeği, Görsel Analog Skala, Distress Termometresi, D Tipi Kişilik Ölçeği uygulanmış ve laboratuvar örnekleri alınmıştır. Bu işlemlerin ardından uygulama grubundaki hastalara her iki ayağa toplam 10 dakika genel ayak masajı ve 30 dakika ayak refleksolojisi uygulanmış, kontrol grubundakilere uygulanmamıştır. Refleksoloji uygulamasından 30 dakika sonra ölçekler tekrar uygulanmış ve laboratuvar örnekleri alınmıştır. KAG ve PTKA sonrası ölçekler tekrar uygulanmıştır. Verilerin değerlendirmesinde numerik verilerin normal dağılıma uygunluğu shapiro-wilk testi ile değerlendirilmiş; mann-whitney u testi, friedman varyans testi, wilcoxon işaret testi ve ki-kare testi kullanılmıştır. KAG ve PTKA sonrası yapılan ölçümlerde KAG ve PTKA refleksoloji grubunda Durumluk Kaygı Ölçeği, ağrı, sistolik kan basıncı, solunum ve kalp hızı/dakika düzeyinde anlamlı bir azalma saptanırken stres düzeyinde KAG refleksoloji ve PTKA kontrol grubunda, diyastolik kan basıncı düzeyinde ise KAG refleksoloji grubunda anlamlı bir azalma saptanmıştır (p, This randomly controlled study was conducted to determine the effect of reflexology implemented before coronary angiography (CAG) and percutaneous transluminal coronary angioplasty (PTCA) on physiological, psychological and some biochemical parameters of patients. The study population consisted of patients who presented to the Cardiology Clinic of the Ege University Medical Faculty Hospital between December 2015 and August 2016 for the first time to undergo CAG and PTCA. Of these patients, 112 (30 CAG reflexology, 30 CAG control, 26 PTCA reflexology and 26 PTCA control patients) selected using the simple and stratified randomization method comprised the study sample. Data collection tools used in the study were the Personal Information Form, State and Trait Anxiety Scale, Visual Analogue Scale, Distress Thermometer, Type D Personality Scale. The tools were applied 90 minutes before CAG and PTCA, and laboratory samples were taken. Following these procedures, the patients in the treatment group had general foot massage for 10 minutes and foot reflexology for 30 minutes for both feet. The patients in the control group did not have any massage. The tools were re-administered 30 minutes after reflexology, and laboratory samples were taken. The tools were administered after CAG and PTCA again. In the analysis of the data, whether the numerical data had normal distribution was checked with the Shapiro-Wilk Test. In addition, The Mann-Whitney U Test, Friedman variance test, Wilcoxon sign test and chi-square test were used. There was no statistically significant difference between the CAG reflexology and control, and PTCA reflexology and control groups before reflexology in terms of their scores for the D Type Personality Scale and State Anxiety Scale, and their stress, pain, life signs and cortisol levels (p> 0.05). In the measurements conducted after 30 minutes of reflexology, a significant decrease was observed in the state anxiety scale, distress, respiration and heart rate/minute values in the CAG and PTCA reflexology groups (p 0.05).
