Blocking T cell activation in organ transplantation is important to prevent rejection. Here the authors show that unconventional monocyte-derived host dendritic cells enter allogeneic grafts to amplify the T cell response outside lymph nodes.

A novel simple, direct and selective gas chromatography–mass spectrometry (GC/MS) procedure was developed for the determination of the antihistamine drug dimenhydrinate (DMH) in presence of six of its related substances and potential impurities, namely, diphenylmethane, diphenylmethanol, benzophenone, orphenadrine, caffeine and 8-chlorocaffeine. The method involved resolution of the underivatized compounds using a trifluoropropylmethyl polysiloxane (Rtx-200) capillary column and the mass spectrometric detection was carried out in the electron-impact (EI) mode. Excellent baseline separation of DMH and the cited related substances was achieved in less than 15 min. Quantification of the parent drug DMH was based on measuring its peak area. The reliability and analytical performance of the proposed method were validated with respect to linearity, range, precision, accuracy, specificity, robustness, detection and quantification limits. Calibration curve of DMH was linear over the range 50–500 μg/mL with determination coefficient (R2) = 0.9982. The proposed method was successfully applied for the assay of DMH in tablets dosage form with recoveries >96.80%.

A field experiment is carried out during June to November, 2014 to study the impact of chemical fertilizers with biofertilizer (EM1) on the yield of rice and its components (Oryza sativa L.), cv.Yasamin. The experiment is done using Completely Randomized Block Design with three replicates. There are four treatments of mixed fertilizers, urea and biofertilizer (EM1) that is used in this study, T1 (recommended dose of chemical fertilizer for rice), T2 (biofertilizer + 1/4 recommended dose of chemical fertilizer for rice), T3 (1/2 recommended dose of chemical fertilizer for rice) and T4 (biofertilizer + whole recommended dose of chemical fertilizer for rice). Significantly, increases are observed in seeds per panicle, 150.9, number of tillers.m-2 462 and grain yield, 5.31 Mg. ha-1 in T4 treatment). Using biofertilizer (EM1) reduces spikelet sterility to 4%.

This paper aims to propose methods to mitigate the risks of flash flood events in arid rural communities with poor infrastructure. A flash flood management case study was conducted at Afouna Village in Egypt, which is characterized by an arid climate and faced a devasting flash flood in 2015. First, the flash flood was modelled and it was found that it corresponds to a 100 year return period flood that led to an almost 13 million m3 total runoff volume. A structural protection approach, using an artificial infiltration pond, was applied to mitigate the flooding risks through water harvesting and recharging the groundwater of the Moghra aquifer. In this study, a novel approach was proposed, which is substituting the low permeability silty sand (2.0 × 10−4 m/s) in the pond area with a high permeability one (9.6 × 10−3 m/s), which will enhance water harvesting and reduce direct evaporation. Modern techniques of hydrological modelling were utilized in order to achieve the optimal use, and harvesting, of flash flood water.

Early management of vascular injury, starting at the field, is imperative for survival no less than any operative maneuver. Contemporary prehospital management of vascular trauma, including appropriate fluid and volume infusion, tourniquets, and hemostatic agents, has reversed the historically known limb hemorrhage as a leading cause of death. In this context, damage control (DC) surgery has evolved to DC resuscitation (DCR) as an overarching concept that draws together preoperative and operative interventions aiming at rapidly reducing bleeding from vascular disruption, optimizing oxygenation, and clinical outcomes. This review addresses contemporary DCR techniques from the prehospital to the surgical setting, focusing on civilian vascular injuries.

Experience with temporary intravascular shunts (TIVS) for vessel injury comes from the military sector and while the indications might be clear in geographically isolated and under resourced war zones, this may be an uncommon scenario in civilian trauma. Data supporting TIVS use in civilian trauma have been extrapolated from the military literature where it demonstrated improved life and limb salvage. Few non-comparative studies from the civilian literature have also revealed similar favorable outcomes. Still, TIVS placement in civilian vascular injuries is uncommon and by some debatable given the absence of clear indications for placement, the potential for TIVS-related complications, the widespread resources for immediate and definitive vascular repair, and the need for curtailing costs and optimizing resources. This article reviews the current evidence and the role of TIVS in contemporary civilian trauma management.

Cytokines play major roles in tissue destruction/repair. The present study investigates proliferative and osteogenic differentiation potentials of gingival mesenchymal stem/progenitor cells (G-MSCs), influenced by IL-1/TNF-α inflammatory/anti-inflammatory conditions. Human G-MSCs were isolated, characterized, and cultured in basic medium (control group, M1), in basic medium with IL-1β, TNF-α, and IFN-γ (inflammatory group, M2) and with IL-1ra/TNF-αi added to M2 (anti-inflammatory group, M3). MTT tests at days 1, 3, and 7 and CFU assay at day 12 were conducted. Osteogenic differentiation was analyzed by bone-specific transcription factors (RUNX2), alkaline phosphatase (ALP), type I collagen (Col-I), osteopontin (OPN), and osteonectin (ON) expression at days 1, 3, 7, and 14 and Alizarin red staining at day 14. At day 3, the control group showed the highest cell numbers. At day 7, cell numbers in inflammatory and anti-inflammatory group outnumbered the control group. At day 12, CFUs decreased in the inflammatory and anti-inflammatory groups, with altered cellular morphology. The anti-inflammatory group demonstrated elevated bone-specific transcription factors at 14 days. After 14 days of osteogenic induction, calcified nodules in the anti-inflammatory group were higher compared to control and inflammatory groups. For regeneration, initial inflammatory stimuli appear essential for G-MSCs’ proliferation. With inflammatory persistence, this positive effect perishes and is followed by a short-term stimulatory one on osteogenesis. At this stage, selective anti-inflammatory intervention could boost G-MSCs’ differentiation.

Abdominal pain is one of the most common reasons for outpatient and emergency department visits. We present one such case of early closure in a 32-year-old male with recurrent abdominal pain who was diagnosed with irritable bowel syndrome (IBS). Family history was suspicious for hereditary angioedema (HAE). The HAE workup came back positive, and the patient was started on prophylactic therapy, which led to an improvement in symptoms and quality of life. The purpose of this case is to create awareness among physicians to test for HAE in patients diagnosed with IBS who, based on their history or physical examination, have clinical suspicion for HAE.

Egypt has been experiencing challenging economic, social and political disturbance during the end of the twentieth century and towards the beginning of the twenty-first. With its fast expanding population, high consumption rate and economic deficit, the Egyptian community continues to experience a low quality of living. These issues contributed greatly to the uprising, following the Tunisian model, in January, 2011. However, the political change that followed did not help a lot, as development strategies are still far from being sustainable. In the beginning of the millennium, Egypt experienced an 8.6% energy deficit, followed by a growing energy crisis [1]. Knowing that almost 50% of the energy produced in Egypt is consumed inside buildings [2], environmentally conscious architecture can be a very powerful tool in development plans, as it can reasonably reduce the consumption of energy and other resources and enhance the quality of living for people. This work analyzes the supposed role of environmentally conscious architecture and, hence, evaluates its importance as a major component of national development plans in the near future. The analysis encompasses a review of the status quo in terms of consumption patterns and required services and, then, compares it with the case in which environmentally conscious architecture is adopted.

The human gingiva, characterized by its outstanding scarless wound healing properties, is a unique tissue and a pivotal component of the periodontal apparatus, investing and surrounding the teeth in their sockets in the alveolar bone. In the last years gingival mesenchymal stem/progenitor cells (G-MSCs), with promising regenerative and immunomodulatory properties, have been isolated and characterized from the gingival lamina propria. These cells, in contrast to other mesenchymal stem/progenitor cell sources, are abundant, readily accessible, and easily obtainable via minimally invasive cell isolation techniques. The present review summarizes the current scientific evidence on G-MSCs’ isolation, their characterization, the investigated subpopulations, the generated induced pluripotent stem cells- (iPSC-) like G-MSCs, their regenerative properties, and current approaches for G-MSCs’ delivery. The review further demonstrates their immunomodulatory properties, the transplantation preconditioning attempts via multiple biomolecules to enhance their attributes, and the experimental therapeutic applications conducted to treat multiple diseases in experimental animal models in vivo. G-MSCs show remarkable tissue reparative/regenerative potential, noteworthy immunomodulatory properties, and primary experimental therapeutic applications of G-MSCs are very promising, pointing at future biologically based therapeutic techniques, being potentially superior to conventional clinical treatment modalities.

Purpose. To evaluate accuracy of various Keratoconus (KC) screening indices, in relation to Topographic Keratoconus (TKC) grading. Setting. Al Watany Eye Hospital, Cairo, Egypt. Methods. Data of 103 normal (group 1) and 73 KC eyes (group 2), imaged by Pentacam (branded as Allegro Oculyzer), were analysed. Group 2 was divided into 2a: 14 eyes (TKC = 1, early KC), 2b: 25 eyes (TKC = 1 to 2 or 2, moderate KC), and 2c: 34 eyes (TKC = 2 to 3 up to 4, severe KC). Participants were followed up for six years to confirm diagnosis. Area under the receiver operating characteristic curve (AUROC) was calculated for evaluated curvature, elevation, and pachymetry indices with various reference shapes at different diameters. Results. When comparing normal to KC eyes, ten indices had significantly higher AUROC. Only five of them had significantly higher AUROC in early KC compared to normal corneas: Pachymetry Progression Index- (PPI-) Maximum (Max), Ambrósio’s Relational Thickness- (ART-) Max, PPI-Max minus PPI-Minimum (Min), central corneal thickness (CCT), and diagonal decentration of thinnest point from the apex (AUROC = 0.690, 0.690, 0.687, 0.683, and 0.674, resp.). Conclusion. Generally, ten pachymetry and elevation-based indices had significantly higher AUROC. Five indices had statistically significant high AUROC when comparing early KC to normal corneas.

Background. Many older adults know about the health benefits of an active lifestyle, but, frequently, pain prevents them from engaging in physical activity. The majority of older adults experience pain, a complex experience that can vary across time and is shaped by sociocultural factors like gender. Objectives. To describe the time-varying associations between daily pain and physical activity and to explore differences in these associations between women and men. Methods. One hundred and twenty-eight community-dwelling older adults aged 65 years and older were asked to report their pain levels three times daily over a 10-day period and wear an accelerometer to objectively capture their daily physical activity (step counts and minutes of moderate to vigorous physical activity). Results. Increased daily step counts and minutes of moderate to vigorous physical activity were associated with increased daily pain, especially among women. Confirming past literature and contrasting findings for daily pain reports, overall pain levels across the study period were negatively associated with minutes of moderate to vigorous physical activity. Conclusions. Findings highlight that pain is significantly associated with physical activity in old age. The nature of this association depends on the time scale that is considered and differs between women and men.

Adult stem or programmable cells hold great promise in diseases in which damaged or nonfunctional cells need to be replaced. We have recently demonstrated that peripheral blood monocytes can be differentiated in vitro into cells resembling specialized cell types like hepatocytes and pancreatic beta cells. During phenotypic conversion, the monocytes downregulate monocyte/macrophage differentiation markers, being indicative of partial dedifferentiation, and are partially reprogrammed to acquire a state of plasticity along with expression of various markers of pluripotency and resumption of mitosis. Upregulation of stem cell markers and mitotic activity in the cultures was shown to be controlled by autocrine production/secretion of activin A and transforming growth factor-beta (TGF-β). These reprogrammed monocyte derivatives were termed “programmable cells of monocytic origin” (PCMO). Current efforts focus on establishing culture conditions that increase both the plasticity and proliferation potential of PCMO in order to be able to generate large amounts of blood-derived cells suitable for both autologous and allogeneic therapies.

Hybrid and online synchronous delivery of courses is a time-demanding approach to teaching and learning systems that is designed to engage students in investigations of authentic concepts or problems without coming to the pre-assigned classrooms two or three times a week. This study presents perceptions and attitudes of students that attended a hybrid course in environmental engineering that suddenly converted to an online synchronous delivery due to COVID-19. It also presents an assessment of the hybrid and online synchronous delivery approach on the final exam scores as well as the final grades of the same course. The course, 'Introduction to Environmental Engineering', was developed as an online course for Civil and Environmental Engineering program students and delivered with hybrid and online synchronous options due to COVID-19 pandemic for several semesters to test the concept. In the hybrid delivery set up, all the quizzes and homework assignments were online while the midterm and final exams were in-class. For Spring 2020 the final exam was online and for Summer 2020 both the midterm and final exams were online due to COVID-19 adjustment. At the very end of the semesters, an online anonymous survey was conducted with five questions to understand the students' perception and attitude on exam taking options and learning environment. Students' perceptions and attitudes about online synchronous delivery approach compared to hybrid delivery approach, as well as the learning outcomes, appeared at face value not to be favorable. However, statistical analysis revealed that differences between online synchronous delivery and the Pre-COVID-19 hybrid delivery were not significant, indicating that, at least for these engineering students, online synchronous delivery is a justifiable option.

The study aimed to examine school drug education and agile leadership and lawlessness factors in children involving adults. This report led to a study on child narcotics crimes. 17 district court decisions from 2017-2020 are the unit of analysis, using the Question Nvivo-12 plus software process. The aim of using Nvivo-12 plus is to analyze data with a data coding model. In addition, the writers also interviewed main witnesses, including judges who prosecuted child narcotics cases. . The data analysis procedure includes a validity test using triangulation of sources, time, and place. Grouping of data in the form of images and tables that have been processed for analysis with three data analysis techniques through percentages, making explanations, and comparing with theories or previous research results. According to the findings of the study, drugs education for youngsters involved in narcotics cases is critical. Furthermore, economic constraints, unsatisfactory home conditions, invites from acquaintances, and the influence of social media are all variables that lead to children committing narcotics offenses. In addition to these factors, the authors also found something new that makes children entangled in narcotics crimes, namely the involvement of adults who use children in the narcotics trade. For these factors, agile leadership is need, especially adult leaders with sensitivity, leadership unity, and quality of optimizing resources to protect children from the effects of drug abuse and the need to build particular educational institutions for children involved in crime. So children are given adequate education and skills, so they are not entangled again in drug cases.

n interactive algorithm to restoration of noisy image is presented in this paper. This algorithm composites of two filters working together with scalar recursive Kalman filter at same time. The first one is the arithmetic median filter with a suitable window length, while the second one is the mean filter with only five neighboring point, the output of these two filter are entered to a scalar Kalman filter, which process the image with raster scan. The suggested scheme is able to reduce the mixed noise(Additive Gaussian and Impulse) from the gray scale images. The output of the scalar Kalman filter is used to adjust (modify) the median and mean filters. Different examples are given, which illustrate the performance of the proposed algorithm comparing with the single mean, single Kalman, single median filter, and median- rational hybrid filter (MRHF) using two bidirectional median sub-filters.
