Abstract

El presente trabajo tiene como objetivo realizar un análisis de la Economía Circular y la sostenibilidad ambiental en la recuperación de los emprendimientos del Cantón Jipijapa teniendo en cuenta las tecnologías agiles. Los emprendimientos en estudio se encuentran desprovistos de capacitaciones y conocimientos para su recuperación hacia la transición de un modelo de producción y consumo circular sostenible. Para dar cumplimiento a dicho objetivo, se realizó un análisis documental, en la consulta de la literatura especializada de las temáticas afines con los estudios de otros autores y con la constatación de las principales problemáticas que presentan los emprendedores para el desarrollo y la transformación, que evoluciona con el apoyo de las tecnologías agiles para el logro de la eficiencia de los sistemas productivos. Dicha situación es constatada a través de encuestas dirigidas a una muestra de 164 persona con la que se obtuvo conocimiento útil para el desarrollo sostenible ambiental en función de la recuperación transformadora a seguir y unas entrevistas con preguntas estructuradas a los representantes de los emprendimientos del cantón de Jipijapa para el conocimiento de los mecanismos que se aplican para el desarrollo sostenible ambiental que se lleva a cabo, para la recuperación transformadora con énfasis en la Economía Circular. Se concluyó demostrando poco conocimiento sobre el tema, escaza capacitación a los emprendedores para el desarrollo sostenible ambiental y desconocimiento de las ventajas y buenas prácticas que ofrece la Economía Circular.

Abstract

Measuring the potential effects of future climate changes on the spatio-temporal variance of optimal conditions for seasonal species is a key conservation issue. This study assesses the impact of climate change on the spatial and temporal patterns of optimal conditions for activity in two burrowing frogs, Smilisca fodiens and S. dentata. Ecological Niche Modeling was used to implement niche seasonality models, with calibration performed during the peak activity (July). These models were then transferred to current and future conditions for the remainder of the year, predicting future scenarios up to 2070 with an intermediate trajectory greenhouse gas concentration of 4.5 W/m2. Climate change transferability was assessed for four potential scenarios: 1) high precipitation and low temperature, 2) high precipitation and high temperature, 3) low precipitation and low temperature, and 4) low precipitation and high temperature. We examined the impact across future projected areas and analyzed geographic change trends based on latitude, longitude, and elevation. For both species, the best scenario would involve increased precipitation in the future. However, the worst-case would be a combination of reduced precipitation and higher temperatures. Due to large area loss, northern populations of S. fodiens may be highly vulnerable. Concerning S. dentata, the outlook is worrisome, with all known populations experiencing losses in most months. Area gains may not help either species since they tend to occur at elevations above their known ranges. Using a seasonal approach in spatio-temporal analysis enhances comprehension of the behavioral adaptations of seasonal species and their vulnerability to current and future climatic variations

Abstract

Introduction: Robotic transanal surgery (RTS) is the analog of TAMIS and represents a new focus for the advancement of transanal platforms robotic transanal surgery is sometimes referred to as robotic TAMIS. Though limited to only a few centers world-wide, experience with robotic transanal surgery has been encouraging. Most research with robotic transanal surgery has concentrated on local excision of rectal neoplasia, although more complex procedures such as transanal proctectomy are possible using the robotic approach. This article reports the surgical technique of R-TAMIS performed in the Brazilian National Cancer Institute (INCA, Rio de Janeiro). Methods: 71-year-old, female with cardiologic disease (heart failure), with a 1.5 cm rectal neoplasm at 4 cm from the anal verge in the right anterolateral position. Biopsy revealed neuroendocrine tumor. A compete colonoscopy revealed no evidence of synchronous lesions. Work-up included 3D endorectal ultrasonography and magnetic resonance imaging, which demonstrated the lesion to be uT1uN0. The patient was counseled about surgical options − local excision versus low anterior resection. Due to the cardiologic condition, the patient was elected to proceed with local excision with robotic transanal surgery. Conclusion: Robotic TAMIS is a safe and effective operative procedure for high selected cases of rectal neoplasm. It enhances surgeon ergonomics and facilitates tumor removal and suture in the transanal approach. RESUMO Introdução: A cirurgia transanal robótica (RTS,Robotic Transanal Surgery) é análoga da TAMIS (cirurgia minimamente invasiva transanal) e representa um novo foco para o avanço das plataformas transanais. A cirurgia transanal robótica é algumas vezes chamada de TAMIS robótica. Embora limitada a apenas alguns centros em todo o mundo, a experiência com a cirurgia transanal robótica tem sido encorajadora. A maioria das pesquisas com cirurgia transanal robótica tem se concentrado na excisão local da neoplasia retal, embora procedimentos mais complexos, como a proctectomia transanal, sejam possíveis utilizando a abordagem robótica. Este artigo relata a técnica cirúrgica de R-TAMIS realizada no Instituto Nacional do Câncer (INCA, Rio de Janeiro). Métodos: paciente de 71 anos, sexo feminino, com doença cardiológica (insuficiência cardíaca), com neoplasia retal de 1,5 cm a 4 cm da borda anal em posição anterolateral direita. A biópsia revelou tumor neuroendócrino. Uma colonoscopia completa não revelou evidências de lesões sincrônicas. A investigação incluiu ultrassonografia endorretal em 3D e ressonância magnética, que demonstrou que a lesão era uT1uN0. A paciente foi aconselhada sobre as opções cirúrgicas – excisão localversus ressecção anterior baixa. Devido à condição cardiológica, optou-se por proceder à excisão local com cirurgia transanal robótica. Conclusão: A TAMIS robótica é um procedimento cirúrgico seguro e eficaz para casos altamente selecionados de neoplasia retal. Ela melhora a ergonomia do cirurgião e facilita a remoção do tumor e a sutura na abordagem transanal.

Abstract

Este livro é um convite a uma viagem emocionante aos caminhos das pesquisas etnográficas, ao encontro dos prazeres e desprazeres da vivência no mundo do trabalho sexual masculino nos grandes centros urbanos, questões vivenciadas e pensadas no campo da sexualidade, em especial no que se refere às transformações e significados da prostituição masculina nas ruas e nos meios virtuais na cidade de Aracaju, bem como remeta-os à análise sobre algumas categorias socioantropológicas, como: estigmatizações, sexualidades, territórios marginais, meios virtuais, desejos, masculinidades e marginalização dos garotos de programa, desmistificando olhares normatizadores da vida sexual para além de uma visão reduzida e dissonante da realidade contemporânea. Algumas perspectivas reforçam essas transformações. A emergência de novas formas de configuração do trabalho sexual, tais como: outras áreas da cidade sendo apropriadas para a prática; anúncios em revistas e jornais e atendimentos em domicílio. Nesse sentido, a virtualização do trabalho sexual masculino contribuiu e contribui para a adoção de novas formas de negócios sexuais, em especial por meio dos sites de encontros e relações sexuais. Esse é o ponto principal do livro em questão, entendendo como esses espaços se tornam territórios de vivência da sexualidade e onde interações sexuais ocorrem, visando compreender, bem como chamar a atenção de outros olhares que são lançados a esses espaços. Sabe-se então que os territórios sexuais são ambientes propícios à vivência extrema e sem norma da sexualidade, dos devaneios mais sublimes e reais da pessoa humana, da solidão, dos desejos, das emoções dos amores e desamores. Nessas diversas perspectivas faço um convite a todos, todas e todes a virem comigo fazer essa viagem até o extremo das sexualidades.

Abstract

This document presents a project for the design of a hangar for aircraft maintenance at Reus Airport. Two main objectives are established: --On the one hand, define the feasibility of the proposal, taking into account a 25-year horizon in which a traffic forecast and a profitability analysis are developed. Finally, the proposal is complemented with the case study of the Ryanair hangar located at Sevilla Airport, which can facilitate decision-making in some further sections of the project. --On the other hand, obtain a structural design proposal that can really adapt to the airport environment, considering all possible factors for the calculus and needs of the infrastructure. To carry out the first point, a brief study of the current situation of the airport was executed. Later on, the airport's available growth capacity was analyzed and the expected annual operations and passenger flow were projected. In addition, in the economic analysis, following the forecasts analyzed above and after obtaining costs, revenues and profits, the aim is to delimit the profitability indicators (NPV, IRR and Payback) in order to be able to conclude the viability of the project. As for the structural design, a solution has been chosen using a metallic structure, porch type with trusses and sandwich panel enclosures. In addition, the foundations of the structure are calculated and a slab is proposed for the paving of the hangar. Prior to the design and calculation of the structure, the needs and design requirements of the hangar are introduced, analyzing factors such as the geotechnics of the terrain, the restrictions marked by aeronautical easements or the type of aircraft maintenance foreseen. Finally, different hangar facilities are designed to make the project more plausible and to limit the values of the final budget presented as much as possible. 145. Sudden infant death syndrome in the Federal District, Brazil: is there evidence?

Abstract

OBJECTIVES: The syndrome of sudden infant death (SIDS) affects children aged between one month and one year of age. Although it has long been reported, there is still no known cause. Although it has long been reported, there is still no known cause. The present work aims to raise the profile and prevalence of SIDS in the Federal District, Brazil. METHODS: Descriptive, transversal and retrospective study relating to deaths, beyond autopsies and the demographics of children between 28 days and one year of age was performed at the Institute of Forensic Medicine of the Federal District between 2003 and 2009. RESULTS: The number of infant deaths registered during the study period was 431. Of these, seven (1.62% of total) died while sleeping and four met the conceptual criteria. The mean age was 102.28 (±11.28) days. However, none was diagnosed as SIDS and theirs causes were analyzed. CONCLUSION: There was no report of SIDS in the Federal District between 2003-2009, contrary it was reported in South/South-Eastern of Brazil whose incidence varied between 0.13 and 1/1,000 live births. This finding suggests underdiagnosis and indicates the lack of a SIDS specific protocol for post-mortem identification. This study shows the need to implement an investigative protocol for SIDS, knowledge, and dissemination of the syndrome among health professionals and the community and shows the importance of awareness campaigns and prevention of this phenomenon.

Resumen: Introducción: Recientemente se han desarrollado dializadores de helixona de nueva generación como parte del esfuerzo continuo para mejorar la biocompatibilidad del dializador y evitar reacciones adversas a los dializadores sintéticos. Este estudio tuvo como objetivo evaluar el rendimiento y la pérdida de albúmina de esta nueva serie de dializadores en hemodiafiltración y compararla con dializadores de helixona de la generación anterior. Material y métodos: Se realizó un estudio prospectivo en 19 pacientes. Cada paciente recibió ocho sesiones de diálisis con los mismos parámetros de diálisis de rutina; solo varió el dializador: FX60 CorDiax, FX CorAL 60, FX600 CorDiax, FX CorAL 600, FX80 CorDiax, FX CorAL 80, FX800 CorDiax y FX CorAL 800. Los índices de reducción (RR) de urea, creatinina, β2-microglobulina, mioglobina, cadenas ligeras kappa (κFLC), prolactina, α1-microglobulina, α1-glicoproteína ácida, cadenas ligeras lambda (λFLC) y albúmina se compararon intraindividualmente. También se midió la pérdida de albúmina del dializado. Resultados: Todos los tratamientos fueron bien tolerados. La media de líquido de reposición osciló entre 31 y 34 litros. La comparación de los tratamientos de diálisis no mostró diferencias entre moléculas pequeñas e incluso del tamaño de las β2-microglobulinas. Se encontraron pocas diferencias entre los RR de mioglobina, κFLC, prolactina, α1-microglobulina y λFLC. Las pérdidas de albúmina en el dializado fueron similares, con menos de 2,5 gramos por sesión. El dializador FX80 CorDiax mostró puntuaciones de eliminación global ligeramente más altas que el resto de los evaluados, a excepción del FX CorAL 800. Conclusión: La nueva generación de dializadores de helixona, actualizada para minimizar el riesgo de reacciones adversas, ha mantenido la eficacia y la pérdida de albúmina conseguidas por la generación de helixona previa. Abstract: Introduction: New generation helixone dialyzers has recently been developed as part of the ongoing effort to improve dialyzer hemocompatibility and avoid adverse reactions to synthetic dialyzers. This study aimed to assess the performance and albumin loss of this new dialyzer series in hemodiafiltration and compare it with the previous generation helixone series. Material and methods: A prospective study was conducted in 19 patients. Each patient underwent eight dialysis sessions with the same routine dialysis parameters; only the dialyzer varied: FX60 CorDiax, FX CorAL 60, FX600 CorDiax, FX CorAL 600, FX80 CorDiax, FX CorAL 80, FX800 CorDiax, and FX CorAL 800. The reduction ratios (RR) of urea, creatinine, β2-microglobulin, myoglobin, kappa-free immunoglobulin light chains (κFLC), prolactin, α1-microglobulin, α1-acid glycoprotein, lambda immunoglobulin light chains (λFLC), and albumin were compared intra-individually. Dialysate albumin loss was also measured. Results: All treatments were well tolerated. The mean amount of replacement fluid ranged from 31 to 34 litres. Comparison of dialysis treatments showed no differences between small molecules and even up to those the size of β2-microglobulins. Little differences were found between myoglobin, κFLC, prolactin, α1-microglobulin, and λFLC RRs, and only FX80 CorDiax was slightly superior to the others. Mean dialysate albumin losses were similar, with less than 2.5 grams lost in each dialyzer. The FX80 CorDiax showed slightly higher global removal scores than the other dialyzers evaluated, except for FX CorAL 800. Resumen: Introducción: Las alteraciones de la concentración plasmática de sodio (Nap, expresado en mEq/l) son frecuentes en los pacientes en hemodiálisis (HD). Los monitores de HD tienen la capacidad de estimar la Nap mediante un algoritmo interno a partir de las medidas de la dialisancia iónica. En el presente trabajo se estudia la correlación entre la Nap estimada por el monitor de diálisis y la medida en el laboratorio de bioquímica de nuestro centro. Material y métodos: Estudio observacional prospectivo y unicéntrico en los pacientes en programa crónico de HD con el monitor 6008 CAREsystem y prescripción estándar de sodio (138 mEq/l) y bicarbonato (32 mmol/l). De cada paciente se extrajeron muestras de sangre venosa antes y después de la sesión para asegurar validez inter e intraindividual. Se analizó la Nap en el laboratorio mediante potenciometría indirecta y simultáneamente se registraba la estimada por el monitor de HD al inicio y al terminar la sesión. Para el análisis estadístico se realizó un diagrama de dispersión y se calculó el cociente de correlación de Spearman. Además, se representaron las diferencias entre métodos mediante diagramas de Bland-Altman. Abstract

Introduction: Changes in plasma sodium concentration (pNa, expressed in mEq/L) are common in hemodialysis (HD) patients. Hemodialysis monitors can estimate pNa by using an internal algorithm based on ion dialysance measurements. The present study studies the accuracy of the correlation between the pNa estimated by the dialysis monitor and that measured by the biochemistry laboratory at our center. Material and methods: A single-centre prospective observational study in patients on a chronic HD program with the 6008 CAREsystem monitor and standard sodium (138 mmol/L) and bicarbonate (32 mmol/L) prescriptions. Venous blood samples were drawn from each patient before and after each HD session to ensure inter- and intra-individual validity. The pNa was measured in the biochemistry laboratory using indirect potentiometry and simultaneously the estimated pNa by the HD monitor was recorded at the beginning and at the end of the HD session. For statistical analysis, a scatterplot was made, and Spearman’s correlation quotient was calculated. In addition, the differences between both methods were represented as Bland-Altman diagrams. Abstract

Introduction: New generation helixone dialyzers has recently been developed as part of the ongoing effort to improve dialyzer hemocompatibility and avoid adverse reactions to synthetic dialyzers. This study aimed to assess the performance and albumin loss of this new dialyzer series in hemodiafiltration and compare it with the previous generation helixone series. Material and methods: A prospective study was conducted in 19 patients. Each patient underwent eight dialysis sessions with the same routine dialysis parameters; only the dialyzer varied: FX60 CorDiax, FX CorAL 60, FX600 CorDiax, FX CorAL 600, FX80 CorDiax, FX CorAL 80, FX800 CorDiax, and FX CorAL 800. The reduction ratios (RR) of urea, creatinine, ß2-microglobulin, myoglobin, kappa-free immunoglobulin light chains (κFLC), prolactin, α1-microglobulin, α1-acid glycoprotein, lambda immunoglobulin light chains (λFLC), and albumin were compared intra-individually. Dialysate albumin loss was also measured. Results: All treatments were well tolerated. The mean amount of replacement fluid ranged from 31 to 34 L. Comparison of dialysis treatments showed no differences between small molecules and even up to those the size of β2-microglobulins. Little differences were found between myoglobin, κFLC, prolactin, α1-microglobulin, and λFLC RRs, and only FX80 CorDiax was slightly superior to the others. Mean dialysate albumin losses were similar, with less than 2.5 g lost in each dialyzer. Resumen: Introducción: Recientemente se han desarrollado dializadores de helixona de nueva generación como parte del esfuerzo continuo para mejorar la biocompatibilidad del dializador y evitar reacciones adversas a los dializadores sintéticos. Este estudio tuvo como objetivo evaluar el rendimiento y la pérdida de albúmina de esta nueva serie de dializadores en hemodiafiltración y compararla con dializadores de helixona de la generación anterior. Material y métodos: Se realizó un estudio prospectivo en 19 pacientes. Cada paciente recibió ocho sesiones de diálisis con los mismos parámetros de diálisis de rutina; sólo varió el dializador: FX60 CorDiax, FX CorAL 60, FX600 CorDiax, FX CorAL 600, FX80 CorDiax, FX CorAL 80, FX800 CorDiax y FX CorAL 800. Los índices de reducción (RR) de urea, creatinina, ß2-microglobulina, mioglobina, cadenas ligeras kappa (κFLC), prolactina, α1-microglobulina, α1-glicoproteína ácida, cadenas ligeras lambda (λFLC) y albúmina se compararon intraindividualmente. También se midió la pérdida de albúmina del dializado. Resultados: Todos los tratamientos fueron bien tolerados. La media de líquido de reposición osciló entre 31 y 34 litros. La comparación de los tratamientos de diálisis no mostró diferencias entre moléculas pequeñas e incluso del tamaño de las β2-microglobulinas. Se encontraron pocas diferencias entre los RR de mioglobina, κFLC, prolactina, α1-microglobulina y λFLC. Las pérdidas de albúmina en el dializado fueron similares, con menos de 2,5 gramos por sesión. El dializador FX80 CorDiax mostró puntuaciones de eliminación global ligeramente más altas que el resto de los evaluados, a excepción del FX CorAL 800. Conclusión: La nueva generación de dializadores de helixona, actualizado para minimizar el riesgo de reacciones adversas, ha mantenido la eficacia y la pérdida de albúmina conseguidas por la generación de helixona previa.
