Abstract

We report a case of adrenal metastasis from colorectal cancer in a 54-year-old woman. Nine months after resection for advanced rectal carcinoma, a computed tomography scan revealed bilateral adrenal metastasis. The level of serum carcinoembryonic antigen was normal. A bilateral adrenalectomy was performed after chemotherapy. Histopathological examination showed adenocarcinoma, compatible with metastasis from the rectal cancer. Adrenal metastasis should be considered in the patients’ follow-up for colorectal cancer.

104. Métastases osseuses des carcinomes différenciés de la thyroïde : expérience du service de médecine nucléaire du CHU Ibn Rochd de Casablanca

Abstract

Objectifs Cette etude a pour objectif de rapporter les caracteristiques epidemiologiques et cliniques des metastases osseuses chez les patients atteints de carcinomes differencies de la thyroide (CDT). Patients et methodes L’etude a analyse retrospectivement tous les cas de CDT presentant une extension osseuse confirmee histologiquement et adresses pour iratherapie au service de medecine nucleaire du CHU Ibn Rochd de Casablanca entre janvier 2005 et decembre 2013. Resultats Parmi les 3077 patients admis pour CDT au cours de la meme periode, 32 presentaient une atteinte osseuse (soit 1,04 %). L’âge moyen etait de 59 ans (± 13), le sexe feminin etait nettement predominant (sex-ratio [H/F] = 0,14). Les metastases osseuses etaient revelatrices dans 34,4 % des cas. Il s’agissait de carcinome vesiculaire dans 50 % des cas, de carcinome papillaire dans 34,4 % des cas et plus rarement de carcinome vesiculaire variante trabeculaire (12,5 %) ou medullaire (3,1 %). Les metastases osseuses etaient isolees dans 53 % des cas, associees a des metastases pulmonaires dans 34,5 % des cas ou a une extension ganglionnaire dans 9,5 % des cas. Les lesions etaient le plus souvent multiples (44 %) ; unique au niveau du bassin (19 %), des cotes (15,5 %), du crâne (12,5 %), des membres (6 %) ou du rachis (3 %). Tous les patients ont ete traites par iratherapie (3 cures en moyenne [(± 1,3]) ; 35,7 % avaient beneficie d’une resection de metastases et 31 % de radiotherapie sur le site atteint. Les patient etait suivis sur une duree de 6 ans (± 2) ; 55 % des patients presentaient une maladie persistante, 16 % etaient en remission, 14 % etaient perdus de vue, 9 % presentaient une aggravation et 6 % de deces etaient recenses. Conclusions La presence de metastases osseuse modifie la prise en charge des CDT, impliquant le plus souvent des therapeutiques lourdes et un suivi prolonge.

113. Traitement par irathérapie d’un carcinome papillaire de la thyroïde chez un patient en insuffisance rénale chronique terminale : première expérience au Maroc

Abstract

Le but de ce travail est de rapporter le cas d’un patient âge de 38 ans, presentant une insuffisance renale chronique terminale en hemodialyse periodique depuis quatre ans. Le bilan pre-greffe a revele un carcinome papillaire de la thyroide multifocal avec metastases ganglionnaires. Apres la chirurgie, la realisation d’une iratherapie pour l’ablation des residus thyroidiens a necessite une coordination etroite entre differents departements (nephrologie, medecine nucleaire et radioprotection). Etant donnee la quantite importante de dialysat generee par hemodialyse (120 litres/jour), le passage en dialyse peritoneale continue ambulatoire (DPCA) a ete decide afin de limiter l’irradiation du personnel soignant et diminuer la quantite de dechet radioactif a gerer. Le dosage de la thyroglobuline en sevrage etait de 79 ng/mL. Nous avons administre au patient 1850 MBq d’iode 131, soit 30 % de la dose requise habituellement devant un carcinome classe haut risque, permettant d’assurer a la fois un effet therapeutique et un sejour limite en milieu hospitalier. L’hospitalisation s’est deroulee sur cinq jours. La mesure de l’activite du liquide de DP a revele une diminution de la radioactivite de 6 % en moyenne (± 2) lors de chaque echange. La scintigraphie corps entier post-iratherapie a rapporte une bonne fixation du radiopharmaceutique par trois residus thyroidiens sans fixation anormale sur le reste du corps. A la sortie du patient, le debit a 1 metre etait a 2,5 μsv/h et en regard de l’abdomen a 28 μsv/h. Au cours du suivi, nous avons note une diminution progressive du taux de thyroglobuline et des anticorps antithyroglobuline passant, respectivement, de 22 ng/mL et 27 UI/mL a 6 mois a 2 ng/mL et 24 UI/mL a 9 mois.

Les metastases ganglionnaires comptent parmi les facteurs pronostiques les plus importants dans les carcinomes differencies de la thyroide (CDT), leur presence augmente la probabilite de recidive locoregionale. Objectifs Cette etude a pour objectif de mettre en evidence les principaux facteurs predictifs de metastases ganglionnaires chez les patients atteints de CDT dans notre contexte. Patients et methodes L’etude a selectionne retrospectivement tous les cas de CDT presentant une extension ganglionnaire confirmee histologiquement et adresses pour iratherapie au service de medecine nucleaire du CHU Ibn Rochd de Casablanca durant l’annee 2012. Une analyse univariee et multivariee a ete realisee comparant les donnees epidemiologiques, cliniques et histologiques de patients avec metastases ganglionnaires (groupe I) avec celles des patients sans metastases ganglionnaires (groupe II). Resultats Sur les 434 patients presentant un CDT, 33 patients presentaient une atteinte ganglionnaire confirmee histologiquement (soit 7 %). L’âge moyen etait de 48 ans (± 16), le sex-ratio (H/F) etait egal a 0,2. La taille moyenne de la tumeur etait de 33 mm (± 2). Les tumeurs etaient classees T1 dans 53,6 % des cas, T2 dans 17,9 % des cas et T3 dans 28,6 % des cas. Parmi les patients presentant une atteinte ganglionnaire, 18,2 % etaient de sexe masculin ( p = 0,05), 15,2 % presentaient un envahissement regional perithyroidien ( p = 0,001), 54,5 % des emboles vasculaires ( p p Conclusions Ces donnees semblent en accord avec les resultats rapportes par la litterature, un nombre de patients plus important permettra de conforter ces resultats.

Abstract

Les cancers de primitifs inconnus (CPI) sont definis par la decouverte d’une ou plusieurs metastases prouvees histologiquement dont le primitif n’a pu etre identifie a la suite d’un bilan complet. Ils posent le probleme de diagnostic etiologique. Objectifs L’objectif de cette etude est de rapporter l’experience du service de medecine nucleaire du CHU Ibn Rochd de Casablanca en termes d’exploration des cancers de primitifs inconnus par la TEP/TDM au 18FDG. Patients et methodes Vingt patients presentant des metastases prouvees histologiquement de primitifs inconnus ont ete inclus dans l’etude. Tous les patients avaient beneficie d’un bilan diagnostique qui est revenu negatif, avant le recours a la TEP/TDM au 18FDG. Resultats La TEP/TDM au 18FDG a permis d’orienter vers le primitif chez onze des vingt patients explores. Les localisations reperees etaient les poumons dans trois cas, le tube digestif dans trois cas, la peau dans deux cas, la prostate dans deux cas et le cavum dans un cas. Chez neuf patients, l’examen a permis d’identifier des lesions hypermetaboliques multiviscerales evocatrices de metastases (dont huit avec atteinte multiviscerale) non suspectees auparavant. Conclusions La TEP/TDM permet a la fois de detecter le primitif apres un bilan conventionnel negatif et de realiser le bilan d’extension de la maladie. Ces resultats meritent d’etre confirmes dans une etude prospective, notamment pour juger de l’impact des resultats de la TEP/TDM sur la survie des patients.

Abstract

Introduction: Single nucleotide polymorphisms (SNPs) in DNA repair genes are mainly correlated with the response to radiotherapy in nasopharyngeal cancer (NPC). In NPC patients, previous research has studied the association between X-ray repair cross-complementing group 1 and 3 (XRCC1 and XRCC3) polymorphisms and radio-therapeutic response. The objective of our study was to test the association between XRCC1 Arg399Gln and XRCC3 Thr241Met polymorphisms and the response to radiotherapy in the NPC Moroccan population. Material and methods: A total of 100 patients with NPC were genotyped for polymorphisms in XRCC1 and XRCC3 genes. Results: The results revealed that the genotypes and alleles of both SNPs did not show any significant association with clinical stages (for XRCC1 Arg-399Gln: p [genotype] = 0.559; p [allele] = 0.440) and (for XRCC3 Thr241Met: p [genotype] = 0.638; p [allele] = 0.567). Moreover, in the study of the association between the polymorphisms and radiotherapy, the response to radiation therapy between genotypes and alleles was not statistically significant (for XRCC1 Arg399Gln p [genotype] = 0.583; p [allele] = 0.459) and (for XRCC3 Thr241Met p [genotype] = 0.660; p [allele] = 0.590). Conclusions: The present study suggests that XRCC1 Arg399Gln polymorphism does not have any impact on the radio-therapeutic response in Moroccan NPC patients whereas XRCC3 Thr241Met polymorphism may act as a prognostic indicator for NPC patients treated with radiotherapy. Abstract

Background: The identification of effective prognosis biomarkers for nasopharyngeal carcinoma (NPC) is crucial to improve treatment and patient outcomes. In the present study, we have attempted to evaluate the correlation between pre-treatment plasmatic Epstein-Barr virus (EBV) DNA load and the conventional prognostic factors in Moroccan patients with NPC. Methods: The present study was conducted on 121 histologically confirmed NPC patients, recruited from January 2017 to December 2018. Circulating levels of EBV DNA were measured before therapy initiation using real-time quantitative PCR. Results: Overall, undifferentiated non-keratinizingcarcinoma type was the most common histological type (90.1 %), and 61.8 % of patients were diagnosed at an advanced disease stage (IV). Results of pre-treatment plasma EBV load showed that 90.9 % of patients had detectable EBV DNA, with a median plasmatic viral load of 7710 IU/ml. The correlation between pre-treatment EBV DNA load and the conventional prognostic factors showed a significant association with patients' age (p = 0.01), tumor classification (p = 0.01), lymph node status (p = 0.003), metastasis status (p = 0.00) and overall cancer stage (p = 0.01). Unexpectedly, a significant higher level of pre-treatment EBV DNA was also found in plasma of NPC patients with a family history of cancer (p = 0.04). The risk of NPC mortality in patients with high pretreatment EBVDNA levels was significantly higher than that of those with low pre-treatment plasma EBV-DNA levels (p < 0.05). Furthermore, patients with high pre-treatment EBV-DNA levels (≥ 2000, ≥ 4000) had a significant low overall survival (OS) rates (p < 0.05). Interestingly, lymph node involvement, metastasis status and OS were found to be the most important factors influencing the EBV DNA load in NPC patients. Conclusions: The results of the present study clearly showed a high association between pre-treatment EBV DNA load, the crucial classical prognostic factors (T, N, M and disease stage) of NPC and OS, suggesting that pre-treatment EBV DNA can be a useful prognostic biomarker in clinical decision-making and improving NPC treatment in Morocco. 130. Epidemiological and Histological Characteristics of Differentiated Thyroid Carcinoma: A Case of the Department of Nuclear Medicine at Ibn Rochd Hospital, Casablanca, Morocco.

Abstract

Background: Thyroid carcinoma (TC) represents the most frequent type of endocrine cancers, with its incidence steadily increasing worldwide. Our study aimed to describe the epidemiological and histological characteristics of differentiated thyroid carcinoma (DTC) at the Department of Nuclear Medicine in Ibn Rochd University Hospital, Casablanca, Morocco., Methods: This was a cross-sectional study of DTC cases treated between 2004 and 2012 in the Department of Nuclear Medicine at Ibn Rochd University Hospital. We retrospectively reviewed medical records at this department, focusing on sociodemographic characteristics, such as age, gender, geographic origin, family history of cancer, and clinical information related to tumor features, including histological type, tumor size, and multifocality. The data were statistically analyzed using the jamovi 2.3.17 software (released September 2022, the jamovi project, retrieved from https://www.jamovi.org), considering the characteristics of the variables., Results: The findings revealed that 89% of the patients were females, and 78.7% were under the age of 55, ranging from 14 to 85 years. Married status represented 75.25% of our cases. Personal history of cancer and a family history of thyroid carcinoma were present in 0.9% and 1.17%, respectively. Concerning histological characteristics, the main histological type was papillary thyroid carcinoma (PTC) at 93%. Within these 1,283 cases, the follicular variant was the most frequent (42.89%). In addition, the tumor size was less than 2 cm in 43.80%, and it was encapsulated in 21.60%. Moreover, we staged our data according to the 8th edition of the American Joint Committee on Cancer staging system, revealing that at the time of diagnosis, 94.13% were in stage I., Conclusion: This study provides an overview of the epidemiological and histological characteristics of DTC in Morocco. Abstract

Proposal: A distinct epidemiology, etiology, clinical characteristics, and therapeutic outcomes characterize nasopharyngeal carcinoma (NPC) from other head and neck cancers. An actualized analysis of NPC patients' features enables a global view of NPC management. Accordingly, the current study investigated the epidemiological and clinical characteristics of Moroccan patients with NPC, as well as their 4-years survival outcomes and influencing prognostic factors., Methods: We prospectively analyzed data of 142 histologically confirmed Moroccan patients with NPC between October 2016 and February 2019. Kaplan-Meier and Cox regression analyses were used to assess predictive prognostic factors related to NPC. All analyses were conducted using SPSS version 21 statistical software., Results: In the present study, a net male predominance was found, with a mean age of 44±16.3 years old. Advanced stages of NPC were observed in 64.1% of patients, and 32.4% of patients presented with distant metastasis at diagnosis. The 4-years overall survival, locoregional relapse-free survival, distant metastasis-free survival and progression-free survival were 68.0%, 63.0%, 53.9%, and 39.9%, respectively. Age, N category and distant metastasis were identified as the most important independent prognosis factors for NPC in this cohort (p<0.05)., Conclusion: In conclusion, NPC affects young adults and is frequently diagnosed at advanced disease stages, impacting therefore negatively patients survival; which is in line with data from endemic areas for NPC. The current study clearly highlights that a greater attention should be directed to improving the management of this aggressive malignancy.

Abstract

Objective: Nasopharyngeal carcinoma (NPC) is a severe malignant disease. Despite its low frequency, NPC is very common in North African population. Radiotherapy is the standard therapeutic treatment of NPC. However, radioresistance hampers the success of treatment. At the molecular scale, radioresistance is due to genetic variations involved in DNA repair pathways in NPC patients. Several studies reported that single nucleotide polymorphisms (SNPs) in excision repair cross complementing group 1 (ERCC1) could be associated with radioresistance. In this optic, the present study aimed to evaluate the association between DNA repair gene polymorphisms ERCC1 C8092A and ERCC1 C118T and radiotherapy response of patients with NPC., Methods: A total of 95 patients with confirmed NPC were recruited at the Mohammed VI Center for Cancer Treatment, Casablanca - Morocco between 2016 and 2018. Two single nucleotide polymorphisms in ERCC1 gene were genotyped. Multiple analysis software was used to assess the correlation between these SNPs and radio-therapeutic response., Results: Sequencing of ERCC1 C8092A polymorphism revealed that CC and CA genotypes were found in 51.6% and 45.3% of cases, respectively, whereas the homozygote AA genotype was reported in only 3.1% of cases. For ERCC1 C118T polymorphism, the heterozygote CT genotype was identified in 49.5% of cases. Homozygotes genotypes CC and TT were detected in 17.9% and 32.6% respectively of NPC cases. Of note, no significant association was found between the ERCC1 C8092A polymorphism and response to radiation therapy (p=0.81). Similarly, there was no significant association between the response to radiotherapy and allelic distribution (p=0.56). Likewise, no correlation was observed neither with genotypes (p=0.07) nor with alleles (p=0.09) of ERCC1 C118T polymorphism and response to radiation therapy., Conclusion: Our results clearly showed that ERCC1 C8092A and ERCC1 C118T polymorphisms were not associated with response to radiotherapy in Moroccan NPC patients. Large studies are warranted to confirm the role of these SNPs in therapeutic response of NPC patients.

Abstract

The current study was designed to investigate the changes in the circulating Epstein−Barr virus DNA load (EBV DNA) at various time points before and after treatment and its clinical significance in nasopharyngeal carcinoma (NPC). A total of 142 patients with NPC were prospectively enrolled in this study. The plasma EBV DNA concentration was measured before and after treatment using qPCR. The prognostic values of the EBV DNA load were analyzed using the Kaplan−Meier and Cox regression tests. Following multivariate analysis, our data showed that high pre-EBV DNA loads were associated with significantly poorer distant metastasis free survival (DMFS) and progression free survival (PFS); detectable end-EBV DNA loads were associated with significantly worse loco-regional recurrence free survival (LRRFS) and PFS, and the detecTable 6 months-post-EBV DNA loads were associated with significantly poorer overall survival (OS), DMFS and PFS (p < 0.05). Additionally, combining the pre-EBV DNA load and the stage of the disease, our results showed that patients at stage III-IVA with a low pre-EBV DNA load had similar survival rates as patients at stage II with a low or high pre-EBV DNA load, but had better survival rates than those at stage III-IVA with a high pre-EBV DNA load. Taken together, we showed that the change of the EBV DNA load measured at several time points was more valuable than at any single time point for predicting patients’ survival for NPC. Furthermore, combining the pre-EBV DNA load and the TNM classification could help to formulate an improved prognostic model for this cancer.

Abstract

Objective: The present study aimed to assess the prognostic interest of metabolic and anatomic parameters derived from 2-deoxy-2-[ 18 F]fluoro-D-glucose positron emission tomography/computed tomography ([ 18 F]FDG PET/CT) and head and neck magnetic resonance imaging (HN-MRI) for better management of nasopharyngeal carcinoma (NPC)., Methods: In this study, pre-treatment [ 18 F]FDG PET/CT and HN-MRI parameters of NPC patients diagnosed between January 2017 and December 2018, were prospectively investigated. Correlation between those parameters and 4-year patient's survival outcomes was evaluated using Kaplan-Meier and Cox-regression analyses., Results: Our results revealed a significant association between pre-treatment nodal-maximum standardized uptake value (N-SUV max) and N categories (p = 0.01), between pre-treatment node-to-tumor SUV ratio (NTR) and both tumor size (p = 0.01) and N categories (p = 0.009), as well as between metabolic tumor volume (MTV) and both tumor size and NPC overall stage (p < 0.000). In multivariate analyses, pre-treatment N-SUV max, NTR and MTV were significant independent predictors of overall survival, distant metastasis-free survival, and progression-free survival (PFS) (p < 0.05). N-SUV max and MTV were also found to be significant independent predictors of loco-regional recurrence-free survival (p < 0.05), whereas HN-MRI detection of skull-base bone invasion was an independent factor associated with worse PFS in NPC (p = 0.03)., Conclusions: The present study highlights N-SUV max, NTR and MTV derived from [ 18 F]FDG PET/CT, and skull-base bone invasion defined by HN-MRI, as promising metabolic and anatomic prognosis biomarkers for NPC., (© 2022. Abstract

Iodine-131 ablation therapy for thyroid cancer in the patient on chronic hemodialysis represents a real problem since the main route of elimination of radioiodine is urinary. There is no recommendation on the management of this treatment in the patient on hemodialysis. We report our experience of management of this treatment in a patient aged 38 years, undergoing hemodialysis for chronic renal failure, and who have been indicated the treatment with iodine-131 for papillary thyroid carcinoma high risk. After multidisciplinary discussions (nephrologists and specialists in nuclear medicine and radiation safety), it has been decided to treat the patient with continuous ambulatory peritoneal dialysis therapy (CAPD). Because of the low but continuous elimination of iodine in the case of CAPD, the patient received a reduced ablative (131)I dose of 1850 MBq, which is 30% of the usual dose delivered in subjects with normal renal function. The patient was hospitalized for four days in nuclear medicine unit and the (131)I radioactivity emitted from him was 2.5 μSv/h at one meter at his hospital discharge. In conclusion, CAPD in relay of hemodialysis is a technique of renal replacement therapy that can be suggested to minimize exposure to radioactivity to the patient, his family and the medical staff., (Copyright © 2015 Association Société de néphrologie.
