Abstract

Mechanical ventilation combined with sedation is widely used in the intensive care unit (ICU). However, this intervention is not without consequence on the patient. ICU patients can, in fact, remember perceptions that occurred during their mechanical ventilation—a phenomenon known as explicit recall. This phenomenon is not well defined, and no common terminology exists in the medical and nursing literature, where a variety of concepts are used interchangeably to describe the same experience. The goal of this concept analysis was to address the conceptual vagueness that surrounds explicit recall. Using Rodgers’ evolutionary approach, a total of 68 articles were analyzed to identify the concept’s antecedents, attributes, and consequences. The findings revealed that the explicit recall concept is perceptive, interpretative, subjective, dynamic, and temporal. It occurs following treatment that requires general anesthesia or sedation. It is also shaped by the modalities of anesthesia and sedation, as well as individual characteristics. Abstract

Public health experts in the US and Canada are increasingly mobilizing molecular-based surveillance techniques in seeking to identify and control HIV. Both countries are also leading in the world to criminalize HIV exposure, transmission and non-disclosure, as well as having pervasive ongoing criminalization regimes toward drug use, sex work and migration. Molecular surveillance aims to rapidly identify 'risk network' clusters of people normally out of reach to public health (e.g. drug taking sexual networks of gay men, rural people living in poverty), in the near real-time, where transmissions are taking place, and to then intervene with enhanced public health approaches. This commentary critically interrogates the seemingly benign new technology that is being presented by leading public health authorities evacuated of social and political context. Abstract

The concept of whistleblowing, which began to emerge in the 1970s, has gained significant traction over time and across disciplines, including law, management, public administration, sociology, psychology, and health sciences. Interestingly, nurses and nursing students account for the majority of the participants in studies pertaining to whistleblowing. Nursing research conducted in the past two decades provide a good foundation on which to build a better understanding of the context in which whistleblowing takes place, the process of whistleblowing itself, and the repercussions experienced by whistleblowers, but major conceptual gaps remain. In fact, limited attention has been given to the conceptual underpinnings and the use of the concept of whistleblowing in nursing. The goal of the present conceptual analysis was to start addressing this gap and raise some critical questions about the future application of this concept in nursing, including potential opportunities and limitations. Our analysis allowed us to identify a number of antecedents, attributes, and consequences of whistleblowing in nursing. It also revealed three areas needing more attention: the concept itself, organizational culture, and research into the complexities of whistleblowing. [ABSTRACT FROM AUTHOR]

126. Femmes vivant avec le VIHsida et lipodystrophie: Étude du processus de transformations corporelles

Abstract

À l'instar de la majorité des pays à revenu élevé, le Canada dispose, depuis 1996, de régimes pharmacologiques pour traiter les personnes vivant avec le VIH/SIDA. Parmi les réactions néfastes de la thérapie antirétrovirale, le syndrome de la lipodystrophie est une condition débilitante qui afflige les personnes vivant avec le VIH/sida et ce, tant sur le plan physique que psychosocial. Conscient de l'ampleur de cette problèmatique qui, à ce jour, ne semble pas avoir été étudiée en contexte québécois, cette recherche qualitative décrit le processus de transformations corporelles auquel font face les femmes atteintes de lipodystrophie et explore les répercussions psychosociales de cette condition qui se manifeste progressivement au cours du traitement. Un devis de recherche qualitatif, de type descriptif et exploratoire, convenait tout à fait aux objectifs de cette recherche. Compte tenu de la problématique de recherche et des objectifs de recherche, la théorisation ancrée a été privilégiée pour orienter ce projet au plan methodologique. La collecte des données s'est deroulée sur une période de 3 mois et a été réalisée par le biais d'entrevues semi-dirigées avec 19 femmes vivant avec le VIH/SIDA. En résumé, les résultats de recherche indiquent que le processus de transformations corporelles engendre une profonde détresse chez les femmes vivant avec le VIH/SIDA. Les résultats de l'étude montrent, d'une part, la détresse (physique et mentale) engendrée par le corps lipodystophie et attestent, d'autre part, des relations de pouvoir avec le complexe médico-pharmaceutique. Au terme de cette recherche les résultats montrent que: le corps est un objet de pouvoir au coeur de ce complexe; que les tensions relatives au traitement amplifient la détresse des femmes vivant avec le VIH/SIDA; que la visibilité du corps entraîne la stigmatisation des femmes vivant avec le VIH/SIDA; et que la lipodystrophie n'est pas la transformation banale de caractéristiques physiques, mais plutôt la reconfiguration définitive de la personne en entier.
