Dr. Robert Alan Hewko, a clinical professor in the University of British Columbia's Department of Psychiatry and former medical lead for the consultation-liaison service at Vancouver General and UBC Hospitals, passed away in December 2023. He was known for his dedication to clinical service, education, and leadership in medical psychiatry. Dr. Hewko focused on various clinical problems, such as delirium, pain, addiction, and pseudoseizures, and was praised for his ability to listen and help patients. He also played a significant role in teaching and spreading international standards of care in British Columbia. Abstract

Background Giving children the best start in life is critical for their future health and wellbeing. Political devolution in the UK provides a natural experiment to explore how public health systems contribute to children's early developmental outcomes across four countries. Method A systematic literature review and input from a stakeholder group was used to develop a public health systems framework. This framework then informed analysis of public health policy approaches to early child development. Results A total of 118 studies met the inclusion criteria. All national policies championed a 'prevention approach' to early child development. Political factors shaped divergence, with variation in national conceptualizations of child development ('preparing for life' versus 'preparing for school') and pre-school provision ('universal entitlement' or 'earned benefit'). Poverty and resourcing were identified as key system factors that influenced outcomes. Scotland and Wales have enacted distinctive legislation focusing on wider determinants. However, this is limited by the extent of devolved powers. Conclusion The systems framework clarifies policy complexity relating to early child development. The divergence of child development policies in the four countries and, particularly, the explicit recognition in Scottish and Welsh policy of wider determinants, creates scope for this topic to be a tracer area to compare UK public health systems longer term. Abstract

It has been argued that transition points in life, such as the approach towards, and early years of retirement present key opportunities for interventions to improve the health of the population. Research has also highlighted inequalities in health status in the retired population and in response to interventions which should be addressed. We aimed to conduct a systematic review to synthesise international evidence on the types and effectiveness of interventions to increase physical activity among people around the time of retirement. A systematic review of literature was carried out between February 2014 and April 2015. Searches were not limited by language or location, but were restricted by date to studies published from 1990 onwards. Methods for identification of relevant studies included electronic database searching, reference list checking, and citation searching. Systematic search of the literature identified 104 papers which described study populations as being older adults. However, we found only one paper which specifically referred to their participants as being around the time of retirement. The intervention approaches for older adults encompassed: training of health care professionals; counselling and advice giving; group sessions; individual training sessions; in-home exercise programmes; in-home computer-delivered programmes; in-home telephone support; in-home diet and exercise programmes; and community-wide initiatives. The majority of papers reported some intervention effect, with evidence of positive outcomes for all types of programmes. A wide range of different measures were used to evaluate effectiveness, many were self-reported and few studies included evaluation of sedentary time. While the retirement transition is considered a significant point of life change, little research has been conducted to assess whether physical activity interventions at this time may be effective in promoting or maintaining activity, or reducing health inequalities

Abstract

Purpose:Identify location and intensity of discomfort experienced by healthy participants wearing cervical orthoses. Method:Convenience sample of 34 healthy participants woreStro II, Philadelphia, Headmaster,andAspenVista®cervical orthoses for four-hour periods. Participants reported discomfort level (scale 0–6) and location. Results:Participants reported mean discomfort for all orthoses over the four-hour test between ‘a little discomfort’ and ‘very uncomfortable’ (mean discomfort score = 1.64, SD = 1.50). Seven participants prematurely stopped tests due to pain and six reported maximum discomfort scores. Significant linear increase in discomfort with duration of wear was found for all orthoses. Significantly less discomfort was reported with Stro II than Headmaster and Philadelphia. Age correlated with greater perceived discomfort. Orthoses differed in the location discomfort was experienced. Conclusion:Existing cervical orthoses cause discomfort influenced by design and duration of wear with orthoses’ design the more significant factor. This work informed the design of a new orthosis and future orthoses developments. Practitioner Summary:The purpose of this study was to gain greater knowledge about the discomfort caused by wearing of existing neck orthoses in order to inform the design and development of a new neck orthosis. 147. Medicine, metaphors and metaphysics: An interdisciplinary analysis of the ethical, medical and sociocultural questions raised by therapeutic equivalence

Abstract

The practice of medicine is the latter-day solution to that intensely human and social predicament we call illness. Medical discourse, however, increasingly tends towards guidelines, protocols, cost considerations and other institutionally-derived issues. This dissertation examines a single concept, therapeutic equivalence, and utilizes it as a metaphor for this focal shift, arguing that this reduced perspective not only ignores the considerable socio-cultural context in which illness takes place, but adversely affects the paradigms and practice of medicine - as well as research, policy and clinical care. Therapeutic equivalence is the basis for a health (pharmaceutical) policy usually called reference-based pricing, used in many jurisdictions and institutions around the world (such as New Zealand's Pharmac, the BC Reference Drug Program and the majority of American HMO's), in which pharmacoeconomic analyses determine the most costeffective drug(s) within a certain class of drugs in order to restrict general access. Using the well-studied BC reference drug program (RDP) as its primary example, this work examines the regulatory and evidentiary framework of the term 'equivalence', analyzes the medical research on therapeutic equivalence and delves into the deeper socio-cultural and epistemological questions the term raises to demonstrate how institutional and statistical interpretations of pathology now dominate medical discourse. The many uncertainties, ambiguities and variations inherent to physiology, pharmacodynamics and pharn~acokinetics are thus ignored; risk is minimized and subjective states and individual narratives of illness, largely disregarded. Moving from drug classificationsldefinitions and the conceptual underpinnings of medical research to the increased convergence of corporate and research interests, this work examines the limitations of ontological disease classifications which assume knowledge is static and questions the current emphasis on biomarkers and numeric results (e.g., blood pressure or cholesterol readings). This work argues that such classification systems are limiting and frame illness in reductionist ways - and have ethical, iatrogenic, medical, social and personal consequences. Broader and more nuanced communications, with greater patient input, are called for.
