Abstract

Resume Dans le cadre d'une septicemie a Salmonelles, le diagnostic d'abces de rate est exceptionnel. La Salmonella dublin s'est revelee dans notre etude, tres pathogene. La clinique ne pouvait permettre d'evoquer le diagnostic d'abces splenique, puisqu'il est commun, en de telles circonstances, de palper une splenomegalie, parfois sensible. La scintigraphie n'est pas d'un grand secours puisqu'elle ne permet pas de trancher entre un simple infarctus splenique et sa complication qu'est l'abces. par contre, il faut insister sur ce procede elegant, rapide indolore et sans danger, qu'est l'echotomographie en de tels cas et notre observation en donne une belle illustration.

Abstract

Resume Chez un homme de 19 ans, un syndrome douloureux aigu de l'hypochondre gauche, au 4 jour de l'eruption d'une varicelle typique, fait porter le diagnostic de rupture de rate spontanee, ce qui est confirme par l'intervention chirurgicale. L'aspect microscopique de la rate est celui d'une infection virale non specifique ; il n'est pas vu d'inclusion virale intra-cellulaire ; la dilatation importante des sinus veineux a pu jouer un role particulier dans la rupture sans qu'un trouble hemodynamique precis ait pu etre identifie. L'etude des trois autres cas de la litterature, tous observes chez des adultes, laisse penser que le mecanisme d'une telle rupture est d'habitude lie a des zones de necrose avec hemorragie entrainant des hematomes sous-capsulaires. Dans notre cas particulier, il n'y avait pas de trouble de l'hemostase.

106. Fuel Alcohol Production from Agricultural Lignocellulosic Feedstocks

Abstract

A two-stage, low-temperature, ambient pressure, acid hydrolysis process that utilizes separate unit operations to convert hemicellulose and cellulose in agricultural residues and crops to fermentable sugars is being developed and tested by the Tennessee Valley Authority and the U.S. Department of Energy (DOE) in Muscle Shoals, Alabama. In laboratory and bench-scale evaluations, the conversion and recovery efficiencies of sugars from hemicellulose and cellulose fractions of corn stover were greater than 90%percnt;. Sugar product concentrations of more than 10%percnt; glucose and 10%percnt; xylose were achieved. In these tests, furfural and hydroxymethyl furfural levels in the sugar solutions never exceeded 0.02gll00 mL, which is far below the levels shown to inhibit fermentation. Based on the results of the bench-scale tests, TVA built a one-of-a-kind acid hydrolysis experimental plant to demonstrate the concepts of low-temperature acid hydrolysis on a much larger scale. The experimental plant was coupled to an existing 10 gal/h fermentation and distillation facility. Plant tests using corn stover have been conducted for more than a year and conversion efficiences have equaled those achieved in the laboratory. Laboratory tests to determine the potential for low-temperature acid hydrolysis of other feedstocks-including red clover, alfalfa, kobe lespedeza, winter rape, and rye grass-are being conducted. Where applicable, process modifications to include extraction before or after hydrolysis also are being studied. This paper describes the experimental plant and process, results obtained in the plant, results of alternative feedstocks testing in the laboratory, and a plan for an integrated system that will produce other fuels, feed, and food from crops grown on marginal land.

110. Evolution of Titre of Serum IgG in Newborn

Abstract

The rythm of decline of maternally acquired IgG levels during the neonatal period differs widely from one subject to another. It does not always obey a simple exponential law, and calculations of half-life during the first 2 weeks of life are not very valid up till now. A sudden fall is often observed between the 1st and 4th day, particularly when the initial concentration is high (it represents 25% of the level on an average). The rythm slows down after the fourth or the eighth day. Under normal conditions, the newborn organism rarely synthesizes IgG in quantities that can be measured by ordinary techniques. However, in various pathological conditions an appreciable rise in IgG level may be seen, very probably connected with active formation.

Abstract

Transtracheal brushing-washing of distal pulmonary lesions was performed in 20 patients. Specimens were obtained by brushing and washing through a catheter placed in the pulmonary lesion with the aid of a guide. In nine patients pathogenic organisms were isolated. In the other 11 patients infectious studies of the specimens were negative, five of the patients (true-negatives) showing no infection and six (false-negatives) showing infection. Serious complications were uncommon, but one patient, who had diffuse intravascular coagulation, died. This procedure seems to represent an advance over transtracheal aspiration and is an effective alternative to such invasive methods are transthoracic lung aspiration and pulmonary biopsy.

Neisseria elongata subsp. nitroreducens, an usual commensal of the nasopharynx, can cause serious infections, usually endocarditis. A case of Neisseria elongata endocarditis complicating mitral valve prolapse in a patient allergic to penicillin is reported. Abstract

Sixty patients, for whom a diagnosis of giant cell arteritis (GCA) was clinically suspected, underwent Doppler study as a method of guiding temporal artery biopsy. In group A (23 patients with normal Doppler results), only one had a positive histological lesion. In group B (14 patients with discrete Doppler abnormalities), none had inflammatory lesions. Twelve of the 23 patients in group C (stenosis or thrombosis) had a biopsy result corresponding to GCA. Forty-three percent of the patients in group C had degenerative arterial lesions leading to a decrease in blood flow. There is a small probability, according to this preliminary study, of diagnosing GCA by artery biopsy even when findings from the ultrasonic examination are normal or only slightly irregular. However, a negative Doppler result should in no case exclude a biopsy if the clinical picture is suggestive. ( JAMA 1982;248:2158-2159)

A 30-year-old patient presenting with pain and fever was shown to have an abnormal liver parenchymal pattern on both ultrasound and CT, the changes of which suggested a malignant process. The appearance, however, was later shown to be due to infestation with Toxocara, an appearance which has not been reported previously.
