Introduction La sous-utilisation des moniteurs de la curarisation a amene une reflexion sur la simplification de leur utilisation [1] . Le but de cette etude est de comparer les performances a l’induction et a la recuperation de deux acceleromyographes : le TOF Watch-SX™ qui doit etre calibre a la mise en route pour determiner l’intensite supramaximale de stimulation et le TofScan™ dont l’intensite est fixee arbitrairement et donc de mise en œuvre plus rapide. Materiel et methodes Dans cette etude pilote, prospective et observationnelle, etaient inclus des patients adultes necessitant une anesthesie generale avec intubation orotracheale et curarisation, pour l’intubation plus ou moins pour la chirurgie, par atracurium ou rocuronium, entre juin et octobre 2013. Les donnees recueillies etaient : lors de la curarisation, les delais respectifs des deux appareils pour obtenir un rapport T4/T1 a 70 %, 50 %, 10 %, a 10 %, 25 %, 40 %, 70 % et 90 %. Ces delais etaient compares par un test de Student pour series appariees et presentees avec un graphique de Bland et Altman [2] avec des limites d’agrement a plus ou moins deux ecart-types. Resultats Sur 32 patients etudies, l’intensite supramaximale de stimulation avec le TOF Watch est 55 ± 8 mA (vs fixe a 50 mA avec le TOF scan). Le delai de curarisation complete (0 reponse) n’etait pas significativement different (biais a –26 s) mais avec une large dispersion (limites d’agrement entre –172et + 119 secondes) ( Fig. 1 ). Le delai de decurarisation (T4/T1 > 90 %) etait similaire (biais a 0) mais avec une dispersion de –4 a + 4 minutes. Lors de la decurarisation, la difference est maximum en debut de decurarisation (biais de 5 minutes a 2 ou 3 reponses) pour se reduire progressivement au dela ( Fig. 2 ). Discussion Lors de la curarisation, on observe une concordance mediocre entre les 2 appareils avec un differentiel de delai eleve (jusqu’a 3 minutes pour certains patients) pouvant impacter le moment de l’intubation et ses conditions. De meme, la variabilite des delais en debut de decurarisation peut influencer de facon plus ou moins appropriee les decisions de reinjection pendant la chirurgie ou d’antagonisation. L’absence de difference significative entre les 2 appareils en fin de decurarisation peut apparaitre comme rassurante en influencant peu la decision d’extubation ou le diagnostic de curarisation residuelle. L’absence de biais lors de la decurarisation est en accord avec la litterature [3] . Neanmoins l’importance de la dispersion justifie des etudes ulterieures pour en preciser les causes.

Introduction L’utilisation d’un monitorage par l’index bispectral (BIS) permet d’optimiser l’administration des agents anesthesiques en integrant les variations interindividuelles de la relation dose–concentration–effet. Des valeurs de BIS comprises entre 40 et 60 correspondent a une profondeur d’anesthesie adequate. L’existence d’un rapport de suppression (RS) positif signe une anesthesie profonde quelle que soit la valeur de BIS [1] . Dans certaines etudes, le temps passe avec un BIS [2] . L’objectif principal de notre travail etait d’analyser les valeurs de BIS recueillies dans des centres differents pour evaluer les pratiques professionnelles et l’observance des consignes cibles. Materiel et methodes Analyse retrospective des donnees de BIS enregistrees consecutivement par BIS Vista dans 5 centres hospitaliers francais (un centre de lutte contre le cancer et 4 CHU) (A, B, C, D, E). Seules les valeurs de BIS enregistrees avec un index de qualite du signal > 50 ont ete analysees. Les donnees des patients et du type d’anesthesie generale n’etaient pas renseignees. Resultats Les valeurs rapportees dans le Tableau 1 sont exprimees en moyenne ± ecart-type. Le pourcentage de temps passe a des valeurs de 40 0 (A = 16 ± 42 ; B = 11 ± 24 ; C = 10 ± 16 ; D = 51 ± 51 ; E = 29 ± 59 p = 0,0001), de RS > 10 pendant plus d’1 minute et de BIS 60 etaient associees a un signal electromyographique > 45. En poolant les donnees de tous les centres, le BIS moyen est de 47 ± 11 (moyenne mini 31, moyenne maxi 70), le pourcentage de temps passe dans l’objectif 40–60 de 50 ± 30 % et le pourcentage de temps passe a une valeur de BIS Discussion Nos resultats montrent qu’en pratique clinique, seulement 50 % du temps d’anesthesie dans des centres pourtant consideres comme experts, est passe dans la cible theorique de BIS. Le nombre et la duree des episodes d’anesthesie trop profonde sont aussi eleves. Cette analyse retrospective observationnelle ne mentionne pas les donnees du patient et la conduite de l’anesthesie par le praticien. Il existe probablement une grande heterogeneite dans l’interpretation des valeurs affichees et des corrections appliquees comme en temoigne la difference entre les centres. Ces resultats incitent a realiser une etude multicentrique colligeant les donnees du patient, de l’anesthesie et du BIS pour connaitre les facteurs a leur origine, optimiser l’utilisation de ce monitorage et la prise en charge anesthesique des patients.

Le traitement des tumeurs endocrines etant souvent chirurgical, la prise en charge anesthesique s’impose. Elle comporte des points communs a toutes les anesthesies et des elements specifiques a certaines tumeurs. Dans ce chapitre, la prise en charge anesthesique de l’ensemble des tumeurs endocrines est abordee. Ce sujet, pourtant fondamental, n’est que tres rarement aborde dans la litterature et les colloques dedies a cette thematique. Son importance, la frequence des formes multiples de tumeurs endocrines au sein des syndromes de predisposition, et la valeur de modele de l’analyse des donnees sur le spectre complet des tumeurs endocrines justifient l’elargissement de ce chapitre a l’ensemble des tumeurs endocrines.

Anesthesia is a combination of unconsciousness, amnesia, and analgesia, expressed in sleeping patients by limited reaction to noxious stimulations. It is achieved by several classes of drugs, acting mainly on central nervous system. Compared to other therapeutic families, the anesthetic drugs, administered by intravenous or pulmonary route, are quickly distributed in the blood and induce in a few minutes effects that are fully reversible within minutes or hours. These effects change in parallel with the concentration of the drug, and the concentration time course of the drug follows with a reasonable precision mathematical models based on the Fick principle.Therefore, understanding concentration time course allows adjusting the dosing delivery scheme in order to control the effects. The purpose of this short review is to describe the basis of pharmacokinetics and modeling, the concentration-effects relationship, and drug interactions modeling to offer to anesthesiologists and non-anesthesiologists an overview of the rules to follow to optimize anesthetic drug delivery.

Goal of the study To assess the benefit of pressure support ventilation during fibreoptic intubation performed under propofol anaesthesia in patients having an anticipated difficult intubation. Procedures Thirty-two patients with ENT cancer, and having at least two criteria for anticipated difficult intubation were prospectively included. All patients received topical lidocaine 2% and propofol by plasma target control infusion (initial target concentration 3 μg ml−1, then adjusted to maintain loss of consciousness without apnoea). They were randomly assigned between two groups: spontaneous breathing (SB) or pressure support ventilation (with a support level set at 10 cm H2O) both using Fio2=1. Conditions for fibreoptic intubation, respiratory parameters (pulse oxymetry, ventilatory frequency, tidal volume and P E ′ CO 2 after intubation) and haemodynamic parameters were recorded. Results Patient characteristic data and intubation conditions were similar between both groups. All patients had a successful fibreoptic intubation and none needed a rescue procedure because of desaturation. In spite of a longer duration of intubation, P E ′ CO 2 after intubation was lower and tidal volume during intubation was higher with pressure support ventilation than in SB patients [38.1 (4.2) vs 42.3 (4.7) mm Hg and 371 (139) vs 165 (98) ml, respectively]. Desaturation episodes were observed in two SB patients conversely to no episode during pressure support ventilation, probably because of the higher minute ventilation. Conclusion Pressure support represents a useful method to improve ventilation during fibreoptic intubation under propofol anaesthesia in patients with an anticipated difficult intubation.

Peritoneal carcinomatosis (PC), which has hitherto been regarded as a lethal entity, can now be cured with surgery (treating macroscopic tumor seeding) combined with hyperthermic intraperitoneal chemotherapy (HIPEC) (treating residual microscopic disease). The purpose of this study was to analyze the morbidity and mortality of a particular approach associating optimal (R0-R1) cytoreduction, optimal HIPEC combining oxaliplatin and irinotecan, and an optimal homogeneous intraperitoneal temperature of 43 degrees C.A total of 106 consecutive patients were included in this prospective phase 2 study. After complete resection of the PC, HIPEC was performed by the Coliseum technique with oxaliplatin (360 mg/m2) combined with irinotecan (360 mg/m2) in 2 L/m2 of 5% dextrose, over 30 minutes at a real intraperitoneal temperature of 43 degrees C. During the hour preceding HIPEC, patients received 5-fluorouracil (400 mg/m2) and leucovorin (20 mg/m2) intravenously, resulting in tritherapy.Postoperative mortality and morbidity rates were 4% and 66%, respectively. The most frequent complications were digestive fistula (24%), lung infection (16%), and severe hematological toxicity (11%). Statistical correlation was evidenced between morbidity and the carcinomatosis score (P = .0008), the number of resected organs (P = .0001), the duration of surgery (P = .0001), and blood loss (P = .0001).This new approach, optimized in three respects (complete cytoreduction, combination oxaliplatin with irinotecan, and high temperature) has resulted in a relatively high but acceptable incidence of adverse events considering the expected advantage for survival.

The phytopathogenic fungi Phytophthora cryptogea and Phytophthora capsici cause systemic leaf necrosis on their non-host tobacco; in culture they release proteins, called cryptogein and capsicein, which elicit similar necrosis. In addition, both proteins protect tobacco against invasion by the pathogen Phytophthora nicotianac, the agent of the tobacco black shank, that is unable to produce such an elicitor. Cryptogein causes visible leaf necrosis starting at about 1 microgram/plant, whereas 50-fold as much capsicein is required for the same reaction. Capsicein induces protection even in near absence of leaf necrosis. The activities of both elicitors are eliminated upon pronase digestion. They are proteins of similar Mr (respectively 10,323 and 10,155) and their complete amino acid sequences were determined. They consist of 98 residues, with some internal repetitions of hexapeptides and heptapeptides. 85% identity was observed between both sequences: only two short terminal regions are heterologous, while the central core is entirely conserved. Secondary structure predictions, hydropathy and flexibility profiles differ only around position 15 and at the C-terminus; these modifications could play a role in the modulation of their biological activities. After a search of the sequence data bases, they appear to be novel proteins.
