Item does not contain fulltext Antiangiogenic therapy targeting vascular endothelial growth factor (VEGF) or its receptor (VEGFR) has proven its effect in the treatment of several types of cancer, including renal cell carcinoma (RCC). However, treatment can be accompanied by notable adverse effects. Mild proteinuria and hypertension are often seen, but sometimes nephrotic range proteinuria and/or renal insufficiency develop. In recent years insight into the toxic effects of anti-VEGF therapy in the kidney has increased. A few biopsies have been done and thrombotic microangiopathy is reported in the majority of cases. However, other patterns of kidney injury have been described as illustrated by the case of a 62-year-old patient who presented two years after initiation of the VEGFR inhibitor cediranib with a nephrotic syndrome and acute renal failure. Kidney biopsy disclosed focal segmental glomerulosclerosis (FS GS) and interstitial nephritis. Partial remission was achieved after stopping the cediranib and a short course of prednisone. We review the different forms of kidney injury that could be caused by anti-VEGF therapy. 01 augustus 2012

- Abstract
We have developed an artificial neural network prediction model for end-stage kidney disease (ESKD) in patients with primary immunoglobulin A nephropathy (IgAN) using a retrospective cohort of 948 patients with IgAN. Our tool is based on a two-step procedure of a classifier model that predicts ESKD, and a regression model that predicts development of ESKD over time. The classifier model showed a performance value of 0.82 (area under the receiver operating characteristic curve) in patients with a follow-up of five years, which improved to 0.89 at the ten-year follow-up. Both models had a higher recall rate, which indicated the practicality of the tool. The regression model showed a mean absolute error of 1.78 years and a root mean square error of 2.15 years. Testing in an independent cohort of 167patients with IgAN found successful results for 91% of the patients. Comparison of our system with other mathematical models showed the highest discriminant Harrell C index at five- and ten-years follow-up (81% and 86%, respectively), paralleling the lowest Akaike information criterion values (355.01 and 269.56, respectively). Moreover, our system was the best calibrated model indicating that the predicted and observed outcome probabilities did not significantly differ. Finally, the dynamic discrimination indexes of our artificial neural network, expressed as the weighted average of time-dependent areas under the curve calculated at one and two years, were 0.80 and 0.79, respectively. Similar results were observed over a 25-year follow-up period. Thus, our tool identified individuals who were at a high risk of developing ESKD due to IgAN and predicted the time-to-event endpoint. Accurate prediction is an important step toward introduction of a therapeutic strategy for improving clinical outcomes.

Item does not contain fulltext Focal segmental glomerulosclerosis (FSGS) is one the most important causes of the nephrotic syndrome in adult patients. FSGS is not a disease entity. The identification of underlying causes of FSGS (secondary FSGS) has increased our insight into the pathogenesis of FSGS. Moreover, differentiating between primary (idiopathic) and secondary forms of FSGS is important to allow appropriate treatment. Recently a new pathological classification of FSGS was proposed, expanding FSGS to include nonsclerotic lesions. In this review we discuss the current diagnostic and therapeutic options in patients with FSGS.

Background: Tonsillectomy has been considered a treatment for IgA nephropathy (IgAN). It is aimed at removing a source of pathogens, reducing mucosa-associated lymphoid tissue and decreasing polymeric IgA synthesis. However, its beneficial effect is still controversial. In Asia, favorable outcomes have been claimed mostly in association with corticosteroids. In Europe, small, single-center uncontrolled studies have failed to show benefits. Methods: The European validation study of the Oxford classification of IgAN (VALIGA) collected data from 1,147 patients with IgAN over a follow-up of 4.7 years. We investigated the outcome of progression to end-stage renal disease (ESRD) and/or 50% loss of estimated glomerular filtration rate (eGFR) and the annual loss of eGFR in 61 patients who had had tonsillectomy. Results: Using the propensity score, which is a logistic regression model, we paired 41 patients with tonsillectomy and 41 without tonsillectomy with similar risk of progression (gender, age, race, mean blood pressure, proteinuria, eGFR at renal biopsy, previous treatments and Oxford MEST scores). No significant difference was found in the outcome. Moreover, we performed an additional propensity score pairing 17 patients who underwent tonsillectomy after the diagnosis of IgAN and 51 without tonsillectomy with similar risk of progression at renal biopsy and subsequent treatments. No significant difference was found in changes in proteinuria, or in the renal end point of 50% reduction in GFR and/or ESRD, or in the annual loss of eGFR. Conclusion: In the large VALIGA cohort of European subjects with IgAN, no significant correlation was found between tonsillectomy and renal function decline.

Contains fulltext : 174822.pdf (Publisher’s version ) (Open Access), In normal glomeruli, parietal epithelial cells (PECs) line the inside of Bowman's capsule and form an inconspicuous sheet of flat epithelial cells in continuity with the proximal tubular epithelial cells (PTECs) at the urinary pole and with the podocytes at the vascular pole. PECs, PTECs and podocytes have a common mesenchymal origin and are the result of divergent differentiation during embryogenesis. Podocytes and PTECs are highly differentiated cells with well-established functions pertaining to the maintenance of the filtration barrier and transport, respectively. For PECs, no specific function other than a structural one has been known until recently. Possible important functions for PECs in the fate of the glomerulus in glomerular disease have now become apparent: (1) PECs may be involved in the replacement of lost podocytes; (2) PECs form the basis of extracapillary proliferative lesions and subsequent sclerosis in glomerular disease. In addition to the acknowledgement that PECs are crucial in glomerular disease, knowledge has been gained regarding the molecular processes driving the phenotypic changes and behavior of PECs. Understanding these molecular processes is important for the development of specific therapeutic approaches aimed at either stimulation of the regenerative function of PECs or inhibition of the pro-sclerotic action of PECs. In this review, we discuss recent advances pertaining to the role of PECs in glomerular regeneration and disease and address the major molecular processes involved.

De Autoriteit Financiële Markten (AFM), toezichthouder op de financiële sector, heeft als speerpunt dat financiële organisaties het klantbelang centraal (KBC) stellen, om duurzaam financieel welzijn te creëren. KBC is een uiting van integer gedrag. In het huidige onderzoek is, onder adviseurs en bemiddelaars in de financiële sector, onderzocht of een ethische organisatiecultuur samenhangt met KBC, betrokkenheid en bevlogenheid. Daarbij is gekeken naar de mediërende rol van ethische spanning in het verband tussen ethische organisatiecultuur en KBC en is gekeken naar de modererende rol van morele identiteit op alle drie de verbanden. Hiervoor is een online vragenlijst uitgezet onder medewerkers van verschillende organisaties van adviseurs en bemiddelaars (N = 182). In lijn met de verwachtingen hangt ethische organisatiecultuur samen met KBC, betrokkenheid en bevlogenheid. Ten tweede is niet gebleken dat het verband tussen ethische organisatiecultuur en KBC wordt gemedieerd door ethische spanning. Ten derde is niet gebleken dat morele identiteit de verschillende verbanden modereert, hoewel morele identiteit wel samenhangt met meer KBC en betrokkenheid. Dit biedt steun voor zowel het clean barrels als good apples perspectief. Op basis van dit onderzoek wordt aangeraden om aan te sturen op een ethische organisatiecultuur door zowel toezichthouder als organisatie zelf om KBC, betrokkenheid en bevlogenheid te bevorderen.

- Abstract
Evidence from the Oxford IgA nephropathy (IgAN) cohort supports the clinical value of subclassifying focal segmental glomerulosclerosis lesions (S1). Using the larger Validation in IgA (VALIGA) study cohort, we investigated the association between podocytopathic changes and higher proteinuria, kidney outcome and response to immunosuppressive therapy. All biopsies were evaluated for glomeruli with segmental capillary occlusion by matrix (“not otherwise specified”, NOS lesion), simple capsular adhesion without capillary occlusion (Adh), tip lesions, and podocyte hypertrophy (PH). S1 required a NOS lesion and/or Adh. A Chi-Squared Automatic Interaction Detection method was used to identify subgroups of FSGS lesions associated with distinctive proteinuria at biopsy and assessed survival from a combined event (kidney failure or 50% decline in estimated glomerular filtration rate). Finally, we evaluated within each subgroup if immunosuppression was associated with a favorable outcome using propensity analysis. In 1147 patients, S1 was found in 70% of biopsies. Subclassification found NOS lesions in 44%, Adh in 59%, PH in 13%, and tip lesions in 3%, with much overlap. Four subgroups were identified with progressively higher proteinuria: from lowest, S1 without NOS, S1 with NOS but without Adh/PH, to highest, S1 with NOS and Adh but without PH, and S1 with NOS and PH. These four subgroups showed progressively worse kidney survival. Immunosuppression was associated with a better outcome only in the two highest proteinuria subgroups. Propensity analysis in these two groups, adjusted for clinical and pathological findings, found a significant time-dependent hazard of combined outcome with corticosteroids. Podocyte hypertrophy and glomeruli with simple adhesions appeared to reflect active lesions associated with a response to corticosteroids, while other S1 lesions defined chronicity. Thus, our findings support subclassifying S1 lesions in IgAN.

We describe a 43-year-old woman who presented with a nephrotic syndrome. Renal biopsy revealed a membranous pattern of glomerular injury. In electron microscopy the subepithelial deposits were comprised of 40 nm wide tubular structures, confirming ITG . During follow-up the patient developed a remission of proteinuria with only supportive treatment.

Item does not contain fulltext, We evaluated the efficacy and safety of rituximab as induction therapy in renal transplant patients. In a double-blind, placebo-controlled study, 280 adult renal transplant patients were randomized between a single dose of rituximab (375 mg/m(2)) or placebo during transplant surgery. Patients were stratified according to panel-reactive antibody (PRA) value and rank number of transplantation. Maintenance immunosuppression consisted of tacrolimus, mycophenolate mofetil and steroids. The primary endpoint was the incidence of biopsy proven acute rejection (BPAR) within 6 months after transplantation. The incidence of BPAR was comparable between rituximab-treated (23/138, 16.7%) and placebo-treated patients (30/142, 21.2%, p = 0.25). Immunologically high-risk patients (PRA >6% or re-transplant) not receiving rituximab had a significantly higher incidence of rejection (13/34, 38.2%) compared to other treatment groups (rituximab-treated immunologically high-risk patients, and rituximab- or placebo-treated immunologically low-risk (PRA = 6% or first transplant) patients (17.9%, 16.4% and 15.7%, p = 0.004). Neutropenia (<1.5 x 10(9) /L) occurred more frequently in rituximab-treated patients (24.3% vs. 2.2%, p < 0.001). After 24 months, the cumulative incidence of infections and malignancies was comparable. A single dose of rituximab as induction therapy did not reduce the overall incidence of BPAR, but might be beneficial in immunologically high-risk patients. Treatment with rituximab was safe.

Deze studie onderzocht de relatie tussen het boundary management gedrag van studenten, overall balans, tevredenheid met het privéleven en zelfgerapporteerde studieprestatie. Er werd bekeken of studie-privé conflict en privé-studie deze relaties medieerden. Daarnaast werd er iets nieuws aan de boundary management literatuur toegevoegd door te kijken naar de modererende rol van voorkeur voor een bepaalde boundary management stijl. Een dagboekstudie van vijf dagen werd afgenomen onder 128 studenten van de Universiteit Utrecht. De studenten vulden van tevoren ook een demografische vragenlijst in. Om de hypotheses te toetsen zijn multipele regressie analyses uitgevoerd. Consistent met mijn verwachtingen werd gevonden dat conflict de relatie van boundary management en de uitkomstvariabelen medieerde, en wel zo dat integratiever boundary management gedrag leidde tot meer conflict, wat leidde tot minder ervaren overall balans en minder tevredenheid met het privéleven. Anders dan voorspeld is er geen evidentie gevonden voor de moderatie van voorkeurstijl. Het hebben van een voorkeur voor een bepaalde stijl had geen invloed op de relatie met conflict. Exploratieve analyses suggereren dat boundary management gedrag geen constant construct is, maar fluctueert van dag tot dag, wat aanleiding geeft tot vervolgonderzoek. Deze resultaten suggereren dat universiteiten hun studenten effectiever kunnen laten studeren door hen te trainen in het effectief toepassen van boundary management.

Het Nieuwe Werken wordt steeds belangrijker voor de herstelbehoefte en de werk-privé balans van individuen. Er wordt echter geen onderscheid gemaakt in wat het effect is van Het Nieuwe Werken op de herstelbehoefte van individuele medewerkers. Tot nu toe werd er vanuit gegaan dat deze relatie voor iedereen hetzelfde werkt. In het huidige onderzoek zijn daarom naast de Herstelbehoefte en de Werk-Privé balans ook Boundary Management, Self-efficacy en Persoonlijkheid meegenomen. Voor dit onderzoek zijn 133 participanten gebruikt van een woningbouwvereniging waar net Het Nieuwe Werken was ingevoerd. De participanten zijn via de email benaderd met de vraag of ze mee wilden doen aan het onderzoek. De belangrijkste bevinden waren dat Self-efficacy, Consciëntieusheid en Vriendelijkheid de relatie tussen Het Nieuwe Werken en Herstelbehoefte modereren. Het blijkt dat de relatie tussen de mate van Het Nieuwe Werken en de Werk-Privé Balans niet via de Herstelbehoefte loopt. Een mogelijke verklaring is dat deze steekproef vooral binnen de traditionele uren (van 9 tot 5) werkte en vooral op kantoor en niet thuis of ergens anders.

The method uses technologies developed for microsystems and has been proven to release small form factor (< 1 mm) and ultrathin solar cells (1.15 μm thick for GaAs, and down to 14 μm in crystalline silicon) with demonstrated functionality. The 14 μm thick c-Si solar cells have achieved efficiencies in excess of 10%. The scale of these cells provide significant benefits for low, medium, and high concentration solar power. These benefits include simplified thermal management, better optical efficiency of the concentrating system, and low-cost module assembly. Due to these advantages over larger scale cells, our economic models predict that this technology could provide $0.50/Wpeak module manufacturing costs with additional savings in the balance of system costs relative to current PV system technologies.

Routine chemical monitoring gives insight in the presence of contaminants in surface waters, but not in their joint ecological effects. Therefore ecological water quality is assessed with bioassays. Recently, a new bioassay using the chydorid Chydorus sphaericus has been developed. Working with smaller volumes, materials and being less time consuming than the traditional Daphnia magna test regarding the culture and experimental design, the 'Chydotox-test' shows a comparable sensitivity. The new Chydotox-test is a promising alternative for the existing Daphnia sp. acute immobilisation test (OECD 1984).

Treatment with cyclophosphamide and prednisolone resulted in the disappearance of signs and symptoms of systemic inflammation. However, renal function deteriorated. Renal biopsy showed evidence of continuing capillary necrosis. Renal function improved with added plasmapheresis treatment. This case report illustrates that in patients with vasculitis necrotizing glomerulonephritis may remain active despite immunosuppressive therapy, even in the absence of extrarenal disease activity.

Item does not contain fulltext We report a 62-year-old man with documented type 2 diabetes mellitus and hypertension, who presented with a rapid deterioration in renal function. The sudden decrease in renal function in this well-controlled diabetic patient prompted us to consider a nondiabetic and nonhypertensive cause. The urinary sediment showed a glomerular haematuria suggestive of glomerulonephritis. A diagnosis of fibrillary glomerulonephritis was made on renal biopsy. Fibrillary glomerulonephritis is a rarely diagnosed disease with clinical manifestations such as proteinuria, microscopic haematuria, nephrotic syndrome and impairment of renal function. A diagnosis of fibrillary glomerulonephritis can only be made by electronmicroscopy of the renal tissue. In this case report the spectrum of this disease is reviewed.

However, FSGS is not a single disease entity. Our aim was to describe the clinical course in initially untreated patients with recently diagnosed idiopathic FSGS. METHODS: This was a retrospective study of patients with a diagnosis of FSGS by histology, who fulfilled the following criteria: proteinuria >3.5 g/day, normal renal function, duration of proteinuria or hypertension of less than one year, normal-sized kidneys, no underlying renal disease, and a negative family history. Renal biopsies were reviewed without knowledge of the clinical course. RESULTS: Twenty patients (13 male, 7 female) fulfilled the study criteria. Median age was 49.3 (range 21.8 to 73.0) years, serum creatinine 90 +/- 20 micromol/l, proteinuria 10.0 +/- 5.5 g/day and serum albumin 24 +/- 6 g/l. After a median follow-up of 9.4 (2.1-18.6) years, 13 patients (65%) were in remission of proteinuria. Renal function deterioration occurred in seven patients, and prompted treatment in four of them. The ten-year death-censored renal survival was 89%. Renal function deterioration and remission rate could be predicted by selectivity index, serum albumin at three months after renal biopsy and the percentage of glomeruli with segmental sclerosis. CONCLUSION: Focal glomerulosclerosis is not a single disease. Case definition using strict clinical criteria identifies a subgroup of patients with idiopathic FSGS who have a good prognosis. In the majority of these patients immunosuppressive therapy is not warranted.

Item does not contain fulltext BACKGROUND: The reported outcome of renal transplantation in patients with the hemolytic uremic syndrome (HUS) varies greatly, probably related to the diverse causes of HUS. In this single-center retrospective study, we have analyzed the recurrence rate, the incidence of acute rejections, and graft survival in patients suffering from adult-onset and childhood-onset HUS. METHODS: The medical records of 35 patients with end-stage renal disease caused by HUS, who received 50 renal allografts, were reviewed. A definite recurrence of HUS was diagnosed if both clinical and histologic signs of thrombotic microangiopathy (TMA) were present in the absence of any endovasculitis. If there were signs of mild endovasculitis, a probable recurrence was diagnosed. RESULTS: After first renal transplantation, 0 definite and 1 (6%) probable recurrence occurred in 18 patients with childhood-onset HUS, as opposed to 7 (41%) definite and 3 (18%) probable recurrences in 17 adult-onset HUS patients (odds ratio [OR], 13.4; 95% confidence interval [CI], 1.7-105.7). In the latter patients, early use of cyclosporine A increased the risk for recurrence. The incidence of acute rejections was increased compared with matched controls (OR, 1.52; 95% CI, 1.05-2.19 for adult-onset HUS and OR, 1.88; 95% CI, 1.34-2.62 for childhood-onset HUS). One-year graft survival in adult-onset HUS was poor (29%), whereas 1-year graft survival in childhood-onset HUS was comparable to matched controls. CONCLUSIONS: In adult-onset HUS, the recurrence rate and the incidence of acute rejections are high, resulting in a detrimental graft survival. In childhood-onset HUS, the recurrence rate is low, but the posttransplantation course is complicated by an increased incidence of acute rejections.

We describe a 30-year-old male patient with nephrotic range proteinuria. Kidney biopsy demonstrated a membranous nephropathy with predominant staining for C1q. Under conservative therapy the outcome was favorable. We suggest that this case represents another variant of C1q nephropathy, thus broadening the spectrum of the disease.

In het huidige onderzoek is een analyse gedaan van het motivationele proces volgens het Job-Demands Resources model. Verwacht werd dat energiebronnen positief zouden samenhangen met werkbevlogenheid. Ook werd verwacht dat persoonlijke hulpbronnen een positief verband zouden vertonen met bevlogenheid. Daarnaast werd verondersteld dat persoonlijke hulpbronnen een versterkend effect zouden hebben op de relatie tussen energiebronnen en bevlogenheid. Onder werknemers van Randstad Uitzendbureau (n = 112) is een digitale vragenlijst afgenomen. De resultaten tonen aan dat de energiebron ontplooiingsmogelijkheden positief samenhangt met bevlogenheid. Ook vertonen de persoonlijke hulpbronnen hoop en weerbaarheid een positieve relatie met bevlogenheid. Tot slot is voorzichtig aangetoond dat de persoonlijke hulpbron optimisme een versterkend effect heeft op het verband tussen de energiebron ontplooiingsmogelijkheden en bevlogenheid. In een organisatie met ontplooiingsmogelijkheden en werknemers die hoog scoren op hoop, weerbaarheid en optimisme werken bevlogen werknemers. Dit zijn tevreden werknemers die optimaal presteren.

In het huidige onderzoek is onderzocht welke rol persoonlijke hulpbronnen (persoonlijkheid en vaardigheden) innemen in het uitputtingsproces van het Job Demands-Resources model. Verwacht werd dat de werkgerelateerde taakeisen (werkdruk, mentale belasting, emotionele belasting, taakonduidelijkheid en administratieve verplichtingen) en persoonlijke hulpbronnen (hoop, optimisme, weerbaarheid, eigen-effectiviteit, beheersingsoriëntatie, flexibiliteit, assertiviteit, pro-activiteit, zelfsturing en work-life crafting) emotionele uitputting voorspellen. Daarnaast werd er verwacht dat de persoonlijke hulpbronnen een modererende rol innemen in de relatie tussen taakeisen en burnout. De hypothesen zijn getoetst onder 228 medewerkers van een Nederlandse zorginstelling. Uit de resultaten blijkt dat mentale belasting, emotionele belasting, taakonduidelijkheid, hoop, optimisme en flexibiliteit de belangrijkste voorspellers zijn van emotionele uitputting in de gezondheidszorg. Flexibiliteit blijkt een modererende rol te hebben in de relatie tussen mentale belasting en emotionele uitputting. Deze studie toont aan dat persoonlijke hulpbronnen een significante bijdrage leveren in de voorspelling van burnout in het Job Demands-Resources model. Op basis van dit onderzoek kunnen interventies ontwikkeld worden, gericht op het verminderen van vermoeidheidsklachten in de gezondheidszorg. Verder onderzoek is nodig om na te gaan of de gevonden voorspellers inderdaad de belangrijkste taakeisen en persoonlijke hulpbronnen zijn in de gehele gezondheidszorg.

In dit onderzoek is getoetst of werkgerelateerde en persoonlijke hulpbronnen de relatie tussen transformationeel leiderschap en de bevlogenheid van werknemers uit generatie Y bemiddelde. In dit onderzoek werd verwacht van wel. Het belang van het onderzoek wordt aangestipt. Het doelt voornamelijk op het feit dat de behoeften van deze generatie, die net op de arbeidsmarkt verschijnt, nog vrij onbekend zijn. Werknemers die geboren zijn tussen 1980 en 1995 vormen werknemers uit generatie Y. De kennis is zowel relevant voor organisaties als voor werknemers zelf. Uit eerder onderzoek blijkt dat generatie Y behoeften te heeft als autonomie, ontwikkelingsmogelijkheden, proactiviteit en selfefficacy. Verondersteld werd dat als er in deze behoeften wordt voorzien, deze constructen als hulpbronnen kunnen functioneren die de positieve relatie tussen transformationeel leiderschap en bevlogenheid van werknemers uit generatie Y bemiddelden. Data is verzameld met behulp van een online survey onderzoek waaraan 145 werknemers uit generatie Y hebben meegedaan. Er zijn drie hulpbronnen die een significante mediator bleken in de relatie tussen transformationeel leiderschap en bevlogenheid van werknemers uit generatie Y, namelijk autonomie, mogelijkheden tot zelfontwikkeling en proactiviteit. Selfefficacy bleek geen mediator in deze relatie. Met deze resultaten kunnen organisaties hun beleidvoering aanpassen om werknemers bevlogen te maken of te houden, door er expliciet op te letten dat de werknemer uit generatie Y in haar behoeften wordt voorzien.

93. Variable heavy chain gene analysis of follicular lymphomas: correlation between heavy chain isotype expression and somatic mutation load

- Abstract
The expansion of follicular lymphomas (FLs) resembles, both morphologically and functionally, normal germinal center B-cell growth. The tumor cells proliferate in networks of follicular dendritic cells and are believed to be capable of somatic hypermutation and isotype switching. To investigate the relation between somatic mutation and heavy chain isotype expression, we analyzed the variable heavy (V(H)) chain genes of 30 FL samples of different isotypes. The V(H) genes of the FLs were heavily mutated (29.3 mutations on average). In addition, isotype-switched lymphomas contained more somatic mutations than immunoglobulin M-positive lymphomas (33.8 mutations per V(H) gene versus 23.0, respectively). In all but one of the FLs, the ratios of replacement versus silent mutations in the framework regions were low, independent of the absolute number of somatic mutations and the level of intraclonal variation. Analysis of relapse samples of 4 FLs showed no obvious increase in somatic mutation load in most FLs and a decrease in intraclonal variation in time. In 3 of 4 cases, we obtained evidence for selection of certain subclones, rather than clonal evolution. Our findings question if intraclonal variation is always a reflection of ongoing somatic hypermutation. This may have implications for the concept of antigen-driven lymphomagenesis. - Abstract
Time-resolved photoluminescence measurements reveal a minority carrier lifetime of greater than 412 ns at 77K under low excitation for a long-wavelength infrared InAs/InAs0.72Sb0.28 type-II superlattice (T2SL). This lifetime represents an order-of-magnitude increase in the minority carrier lifetime over previously reported lifetimes in long-wavelength infrared InAs/Ga1-xInxSb T2SLs. The considerably longer lifetime is attributed to a reduction of non-radiative recombination centers with the removal of Ga from the superlattice structure. This lifetime improvement may enable background limited T2SL long-wavelength infrared photodetectors at higher operating temperatures., Published in the Journal of Applied Physics Letters, v99 n25 Article 251110 p1-3, 2011. Item does not contain fulltext, BACKGROUND: The variable course of immunoglobulin A nephropathy (IgAN) warrants accurate tools for the prediction of progression. Urinary kidney injury molecule-1 (KIM-1) and neutrophil gelatinase-associated lipocalin (NGAL) are markers for the detection of early tubular damage caused by various renal conditions. We evaluated the prognostic value of these markers in patients with IgAN. METHODS: We included patients (n = 65, 72% male, age 43 +/- 13 years) with biopsy-proven IgAN, who were evaluated for proteinuria. Urinary KIM-1 and NGAL were measured by enzyme-linked immunosorbent assay. We analysed data using Cox regression for the outcome end-stage renal disease (ESRD). RESULTS: Median serum creatinine was 142 mumol/L and proteinuria 2.2 g/day. During follow-up (median 75 months), 23 patients (35%) developed ESRD. In patients with IgAN median urinary KIM-1 excretion was 1.7 ng/min and NGAL excretion was 47 ng/min, both significantly higher than in healthy controls. KIM-1 and NGAL were correlated with proteinuria (r = 0.40 and 0.34, respectively, P < 0.01) and each other (r = 0.53, P < 0.01) but not with estimated glomerular filtration rate (eGFR). Interestingly, KIM-1 was not significantly correlated with the excretion of alpha(1)-microglobulin (alpha(1)m) and beta(2)-microglobulin (beta(2)m), known markers of tubular injury. Univariate analysis showed that baseline serum creatinine and urinary excretion of total protein, alpha(1)m, beta(2)m, immunoglobulin G, KIM-1 and NGAL were significantly associated with ESRD. By multivariate analysis, serum creatinine and KIM-1 excretion proved to be significant independent predictors of ESRD. CONCLUSION: KIM-1 and NGAL excretion are increased in patients with IgAN and correlate with proteinuria but not with eGFR. Baseline serum creatinine and urinary KIM-1, but not proteinuria, are independent predictors of ESRD.

Item does not contain fulltext, A subset of ciliopathies, including Sensenbrenner, Jeune, and short-rib polydactyly syndromes are characterized by skeletal anomalies accompanied by multiorgan defects such as chronic renal failure and retinitis pigmentosa. Through exome sequencing we identified compound heterozygous mutations in WDR19 in a Norwegian family with Sensenbrenner syndrome. In a Dutch family with the clinically overlapping Jeune syndrome, a homozygous missense mutation in the same gene was found. Both families displayed a nephronophthisis-like nephropathy. Independently, we also identified compound heterozygous WDR19 mutations by exome sequencing in a Moroccan family with isolated nephronophthisis. WDR19 encodes IFT144, a member of the intraflagellar transport (IFT) complex A that drives retrograde ciliary transport. We show that IFT144 is absent from the cilia of fibroblasts from one of the Sensenbrenner patients and that ciliary abundance and morphology is perturbed, demonstrating the ciliary pathogenesis. Our results suggest that isolated nephronophthisis, Jeune, and Sensenbrenner syndromes are clinically overlapping disorders that can result from a similar molecular cause.

- Abstract
Het doel van dit onderzoek was eventuele verschillen in kaart te brengen tussen generaties in a) hun streven naar het bereiken van een balans tussen werk en privé en b) hun ervaringen van conflict en facilitatie tussen werk en privé. Daarnaast worden de eventuele effecten hiervan op werkmotivatie, werktevredenheid en levenstevredenheid onderzocht. Niet in overeenstemming met de hypothesen is aangetoond dat de jongste generatie op de werkvloer (Generatie Y) minder streeft naar werk-privé balans in vergelijking tot de twee oudere generaties op de werkvloer (Generatie X en Generatie Baby Boomers). Tevens in tegenstelling met de hypothesen is er geen verschil tussen de generaties gevonden in hun ervaringen van WP conflict, PW conflict, WP facilitatie en PW facilitatie. In overeenstemming met de hypothesen is wel gebleken dat PW conflict negatief correleert met motivatie en werktevredenheid en dat WP conflict van negatieve invloed kan zijn op levenstevredenheid.

RIVM rapport:Het RIVM heeft geïnventariseerd welke effecten zogeheten exoten kunnen hebben op de flora en fauna van de Nederlandse zoete oppervlaktewateren. Exoten zijn niet-inheemse planten en dieren die door menselijk handelen in Nederland zijn terechtgekomen. Het merendeel is niet schadelijk, maar sommige kunnen een bedreiging vormen voor inheemse soorten. Zo kunnen inheemse soorten in de knel komen of verdwijnen door de komst van exoten. Hoewel deze verstorende invloed wordt erkend, wordt deze te weinig in ogenschouw genomen bij maatregelen die vanuit de Kaderrichtlijn Water (KRW) zijn ingesteld om de flora en fauna te verbeteren In het onderzoek staat eveneens beschreven welk internationaal en nationaal beleid is vastgesteld om schade door exoten te voorkomen en ze zonodig te bestrijden. Internationaal is er al veel beleid opgesteld, bijvoorbeeld om de biodiversiteit te beschermen of om plagen in de land- en tuinbouw te voorkomen. Vaak is dat beleid niet bindend. Nationaal is ook beleid vastgesteld. Doorgaans is dat beleid ingegeven door deelbelangen, bijvoorbeeld om de biodiversiteit te beschermen of om plagen in de land- en tuinbouw te voorkomen. Het RIVM beveelt daarom aan de regie te verbeteren op het signaleren en zonodig bestrijden van exoten. Dat kan bijvoorbeeld door deze bij één coordinerende instantie te leggen., RIVM made an inventory of impacts of invasive species on plant and animal life of the Netherlands. Invasive species are non-indigenous species which entered Netherlands by human influence. Most invasive species are not harmful, but some of them may threaten indigenous species. At worst indigenous species may get extinct. Although the disturbing effects of invasive species are acknowledged, they do not get enough attention when measures are taken to improve ecological water quality for the Water Framework Directive. RIVM also described which international and national policy is in place to prevent damage by invasive species and combat them when necessary. There are several international regulations, e.g. to protect biodiversity or to combat agricultural pests. These regulations are often not binding. There are also national regulations. In general these regulations are dealing with sectoral issues e.g. to protect biodiversity or to combat agricultural pests. RIVM recommends to improve coordination of monitoring invasive species and combatting them when necessary. This might be done by appointing one coordinating institution.

Het behouden van en investeren in talentvolle werknemers is tegenwoordig een belangrijk aandachtspunt van organisaties. Talenten kunnen competitief voordeel opleveren en inzicht in de behoefte van talenten kan in de toekomst voor succes zorgen. Deze studie geeft inzicht in de factoren die een rol spelen in de uitputting en motivatie van talenten en hoe talenten de aangeboden talentenprogramma’s ervaren. De subpopulatie ‘talenten’ is nog nauwelijks onderzocht en is daarom aan de hand van het bekende Job demands Resources Model van Schaufeli & Bakker (2004; 2007) bekeken. Het JD-R model is gerepliceerd op deze specifieke groep werknemers en er zijn twee extra factoren aan toegevoegd. De extra factoren zijn vriendelijkheid en labelvoordeel. Vriendelijkheid is een persoonlijkheidsdimensie, bij een hoge score op deze factor werd een sterkere relatie verwacht tussen taakeisen en emotionele uitputting. Labelvoordeel is een nieuwe variabele en omvat de visie die talenten hebben ten opzichte van het talentenprogramma. Een hoge score betekent dat zij het als toegevoegde waarde zien en als unieke kans voor zelfontplooiing. Een lage score toont dat zij negatieve percepties ervaren die met name zijn gericht op de hoge druk en verwachtingen van de omgeving. Van de factor labelvoordeel werd de invloed bekeken op het motivationele proces. De respondenten (N = 147) zijn talentvolle werknemers van een internationale organisatie. Zij zijn op basis van objectieve criteria geselecteerd door de organisatie en er is kwantitatief onderzoek verricht door middel van een online vragenlijst. Uit de resultaten is gebleken dat de twee processen van het JD-R model grotendeels worden ondersteund, echter blijkt dat deze twee processen niet voldoende zijn om de uitputting en motivatie van talenten volledig te dekken. Er zijn crossrelaties gevonden, die in vervolgonderzoek meegenomen moeten worden. Het meest opmerkelijke resultaat is de bufferende werking die een hoge score op labelvoordeel lijkt te

Het doel van het onderzoek is om inzicht te krijgen in motivationele processen tijdens organisatieverandering. Gebaseerd op het JD-R model wordt een positieve samenhang verwacht tussen persoonlijke en werkhulpbronnen, werkbeleving en organisatie uitkomsten. Verder wordt gekeken naar de modererende rol van persoonlijke hulpbronnen op de relatie tussen werkhulpbronnen en werkbeleving en de medierende rol van werkbeleving op de relatie tussen werkhulpbronnen en gedrag van werknemers. 98 werknemers uit verschillende sectoren die te maken hadden met verschillende organisatieveranderingen vulden de online vragenlijst in. Regressieanalyse liet zien dat de werkhulpbron open-systeem-waarden cultuur positief samenhangt met bevlogenheid en bevlogenheid positief samenhangt met team veranderprestatie. De voornaamste conclusie is dat organisatiecultuur een belangrijke voorspeller is voor bevlogenheid, wat positief samenhangt met team veranderprestatie. Dit biedt aanknopingspunten voor vervolgonderzoek naar motivationele processen binnen organisatieverandering. De verwachte moderatie en mediatie effecten kunnen niet bevestigd worden.
