Rationale: Patients with diabetes represent almost 20% of all ICU admissions and might respond differently to high-dose early active mobilization. Objectives: To assess whether diabetes modified the relationship between the dose of early mobilization on clinical outcomes in the TEAM trial. Methods: All TEAM trial patients were included. The primary outcome was days alive and out of the hospital at Day 180. Secondary outcomes included 180-day mortality and long-term functional outcomes at Day 180. Logistic and median regression models were used to explore the effect of high-dose early mobilization on outcomes by diabetes status. Measurements and Main Results: All 741 patients from the original trial were included. Of these, 159 patients (21.4%) had diabetes. Patients with diabetes had fewer days alive and out of the hospital at Day 180 (124 [0–153] vs. 147 [82–164]; P = 0.013) and higher 180-day mortality (30% vs. 18%; P = 0.044). In patients receiving high-dose early mobilization, the number of days alive and out of the hospital at Day 180 was 73.0 (0.0–144.5) in patients with diabetes and 146.5 (95.8–163.0) in patients without diabetes (P value for interaction = 0.108). However, in patients with diabetes, high-dose early mobilization increased the odds of mortality at 180 days (adjusted odds ratio, 3.47; 95% confidence interval, 1.67–7.61; P value for interaction = 0.001). Conclusions: In this secondary analysis of the TEAM trial, in patients with diabetes, a high-dose early mobilization strategy did not significantly decrease the number of days alive and out of the hospital at Day 180, but it increased 180-day mortality. - Abstract
Background: Early mobilisation (EM) is an intervention that may improve the outcome of critically ill patients. There is limited data on EM in COVID-19 patients and its use during the first pandemic wave. Methods: This is a pre-planned subanalysis of the ESICM UNITE-COVID, an international multicenter observational study involving critically ill COVID-19 patients in the ICU between February 15th and May 15th, 2020. We analysed variables associated with the initiation of EM (within 72 h of ICU admission) and explored the impact of EM on mortality, ICU and hospital length of stay, as well as discharge location. Statistical analyses were done using (generalised) linear mixed-effect models and ANOVAs. Results: Mobilisation data from 4190 patients from 280 ICUs in 45 countries were analysed. 1114 (26.6%) of these patients received mobilisation within 72 h after ICU admission; 3076 (73.4%) did not. In our analysis of factors associated with EM, mechanical ventilation at admission (OR 0.29; 95% CI 0.25, 0.35; p = 0.001), higher age (OR 0.99; 95% CI 0.98, 1.00; p ≤ 0.001), pre-existing asthma (OR 0.84; 95% CI 0.73, 0.98; p = 0.028), and pre-existing kidney disease (OR 0.84; 95% CI 0.71, 0.99; p = 0.036) were negatively associated with the initiation of EM. EM was associated with a higher chance of being discharged home (OR 1.31; 95% CI 1.08, 1.58; p = 0.007) but was not associated with length of stay in ICU (adj. difference 0.91 days; 95% CI − 0.47, 1.37, p = 0.34) and hospital (adj. difference 1.4 days; 95% CI − 0.62, 2.35, p = 0.24) or mortality (OR 0.88; 95% CI 0.7, 1.09, p = 0.24) when adjusted for covariates. Conclusions: Our findings demonstrate that a quarter of COVID-19 patients received EM. There was no association found between EM in COVID-19 patients' ICU and hospital length of stay or mortality. Purpose Early mobilization within 72 h of intensive care unit (ICU) admission improves functional status at hospital discharge. We aimed to assess the effectiveness of early, goal-directed mobilization in critically ill patients across a broad spectrum of initial consciousness levels. Methods Post hoc analysis of the international, randomized, controlled, outcome-assessor blinded SOMS trial conducted 2011-2015. Randomization was stratified according to the immediate post-injury Glasgow Coma Scale (GCS) ([less than or equal to] 8 or > 8). Patients received either SOMS-guided mobility treatment with a facilitator or standard care. We used general linear models to test the hypothesis that immediate post-randomization GCS modulates the intervention effects on functional independence at hospital discharge. Results Two hundred patients were included in the intention-to-treat analysis. The significant effect of early, goal-directed mobilization was consistent across levels of GCS without evidence of effect modification, for the primary outcome functional independence at hospital discharge (p = 0.53 for interaction), as well as average achieved mobility level during ICU stay (mean achieved SOMS level) and functional status at hospital discharge measured with the functional independence measure. In patients with low GCS, delay to first mobilization therapy was longer (0.7 ± 0.2 days vs. 0.2 ± 0.1 days, p = 0.008), but early, goal-directed mobilization compared with standard care significantly increased functional independence at hospital discharge in this subgroup of patients with immediate post-randomization GCS [less than or equal to] 8 (OR 3.67; 95% CI 1.02-13.14; p = 0.046). Bei gebesserten Überlebensraten von Intensivpatienten werden deren Langzeitergebnisse zunehmend wichtig. Der demographische Wandel zeigt sich auch in der Population dieser Intensivpatienten. Ein wichtiger Begriff, um eine vulnerable Patientenpopulation zu charakterisieren ist die Frailty. Der funktionelle Status nach einem Intensivaufenthalt zeigte sich in der Vergangenheit als wichtiger prognostischer Faktor. Zu klären sind der Einfluss der Frailty auf Funktionalität und weitere Outcomes., Improved survival rates of critically ill patients emphasize the importance of patient outcomes. Demographic change leads to a more vulnerable patient cohort. Frailty therefore is an important way of describing these more vulnerable patients. Functionality on the other hand was already shown to be an important prognostic factor in patient outcomes. The impact of frailty on functionality and other patient outcomes is aim of this study.

Mechanische Thoraxkompressionsgeräte werden häufig bei der kardiopulmonalen Reanimation (CPR) eingesetzt, aber noch nicht für den regelmäßigen Gebrauch empfohlen. In der COVME-Studie wurden drei Beatmungsmodi bei Patienten untersucht, die mit diesen Geräten behandelt wurden. Die Ergebnisse zeigten ein deutlich geringeres Tidalvolumen in der CPAP-Gruppe und ein höheres Atemminutenvolumen (AMV) in der BIPAP- und VCV-Gruppe. Alle Gruppen hatten ein angemessenes AMV während der HLW. Es scheint ratsam, BIPAP zu verwenden, um das Risiko einer Totraumbeatmung zu vermeiden., Mechanical chest compression devices are frequently used in cardiopulmonary resuscitation (CPR), but not yet recommended for regular use. The COVME Study investigated three ventilation modes on patients undergoing CPR with those devices. Results showed a significantly lower tidal volume in the CPAP group and higher respiratory minute volume (RMV) in BIPAP and VCV group. All groups had an adequate RMV during CPR. It seems advisable to use BIPAP to avoid risk of dead space ventilation.

Im klinischen Alltag und Forschung ist die Fremdeinschätzung durch Angehörige ein wichtiges Mittel zur Erfassung der Funktionalität und Lebensqualität. Bei Patient*innen mit Aufenthalt auf der Intensivstation werden die Angehörigen über den prämorbiden Zustand der Patient*innen zur Einschätzung des Outcomes und Ermittlung des Therapieziels befragt. Es stellt sich die Frage, welchen Einfluss ein postoperativer Intensivaufenthalt auf die Übereinstimmung zwischen Selbst- und Fremdeinschätzung hat., In everyday clinical practice and studies proxies’ assessment is important to obtain data of the functionality and health related quality of life (HRQOL) of patients. With patients admitted to Intensive Care Units (ICUs), proxies are asked about the premorbid state of the patients to determine the outcome and objective of therapy. It was asked whether a postoperative ICU stay has an influence on the reliability of proxies’ assessment in terms of functionality and HRQOL of patients.

- Abstract
Background: CONCISE is an internationally agreed minimum set of outcomes for use in nutritional and metabolic clinical research in critically ill adults. Clinicians and researchers need to be aware of the clinimetric properties of these instruments and understand any limitations to ensure valid and reliable research. This systematic review and meta-analysis were undertaken to evaluate the clinimetric properties of the measurement instruments identified in CONCISE. Methods: Four electronic databases were searched from inception to December 2022 (MEDLINE via Ovid, EMBASE via Ovid, CINAHL via Healthcare Databases Advanced Search, CENTRAL via Cochrane). Studies were included if they examined at least one clinimetric property of a CONCISE measurement instrument or recognised variation in adults ≥ 18 years with critical illness or recovering from critical illness in any language. The COnsensus-based Standards for the selection of health Measurement INstruments (COSMIN) checklist for systematic reviews of Patient-Reported Outcome Measures was used. The Preferred Reporting Items for Systematic Reviews and Meta-Analyses were used in line with COSMIN guidance. The COSMIN checklist was used to evaluate the risk of bias and the quality of clinimetric properties. Overall certainty of the evidence was rated using a modified Grading of Recommendations, Assessment, Development and Evaluation approach. Narrative synthesis was performed and where possible, meta-analysis was conducted. Results: A total of 4316 studies were screened. Forty-seven were included in the review, reporting data for 12308 participants. The Short Form-36 Questionnaire (Physical Component Score and Physical Functioning), sit-to-stand test, 6-m walk test and Barthel Index had the strongest clinimetric properties and certainty of evidence. The Short Physical Performance Battery, Katz Index and handgrip strength had less favourable results. Fried et al. 2001 define frailty as a distinct clinical syndrome that is reversible and in which patients experience one or more of the following: unwanted weight loss, self-reported fatigue, low physical activity, slow walking speed and reduced grip strength [1]. Individuals with frailty syndrome are at increased risk for postoperative complications, which may be associated with mid- and long-term limitations [2, 3]. There is evidence that prehabilitation can positively influence negative outcomes after surgery in individuals with (pre-)frailty [4, 5]. Prehabilitation is usually a multimodal intervention that can include the same components as rehabilitation, e. g. physiotherapy, psychotherapy, occupational therapy, and medical training therapy, as well as speech therapy, nutritional and social counselling [5, 6]. There is only limited data on the views of the patients, who have undergone prehabilitation before elective surgery [7]. However, the knowledge of the facilitators and barriers to the implementation of prehabilitation is important in order to adequately consider the affected person's perspective in the development and implementation of prehabilitation programmes. Therefore, in the present study, qualitative interviews will be conducted with participants from the intervention group of a project called “Prehabilitation of elderly frail or pre-frail patients prior to elective surgery” (PRAEP-GO) and its scientific support program “Analysis of frailty syndrome within the framework of the innovation fund project PRAEP-GO” (ANA-PRAEP-GO). PRAEP-GO is a multicenter, parallel group, randomized controlled clinical trial lead by Prof Dr Claudia Spies (ClinicalTrials.gov: NCT04418271) and presents a new form of integrated care based on §140a SGB V. Its aim is to evaluate the (cost-)effectiveness of prehabilitation prior to elective surgery in (pre-)frail elderly persons [8]. Because PRAEP-GO is limited to evaluating clinical issues only, ANA-PRAEP-GO is its scientific support program to be able to research further pathophysiological, clinical, psychosocial and work-organizational connections (ClinicalTrials.gov: NCT04880824). The aim of the present qualitative study is to identify barriers and facilitators to the implementation of the prehabilitation program from the point of view of the participants of ANA-PRAEP-GO. The qualitative interviews and their analysis will be performed by Ms Carina Pfab as part of her master’s thesis at the Berlin School of Public Health under the supervision of the investigators Dr Tanja Rombey and Dr Jörn Kiselev. Background and goal of studyCardiopulmonary resuscitation (CPR) in prehospital care is a major reason for emergency medical service (EMS) dispatches. CPR outcome depends on various factors, such as bystander CPR and initial heart rhythm. Our aim was to investigate whether short-term outcomes such as the return of spontaneous circulation (ROSC) and hospital admission with spontaneous circulation differ depending on the location of the out-of-hospital cardiac arrest (OHCA). In addition, we assessed further aspects of CPR performance.Materials and methodsIn this monocentric retrospective study, protocols of a prehospital physician-staffed EMS located in Munich, Germany, were evaluated using the Mann–Whitney U-test, chi-square test, and a multifactor logistic regression model.Results and discussionOf the 12,073 cases between 1 January 2014 and 31 December 2017, 723 EMS responses with OHCA were analyzed. In 393 of these cases, CPR was performed. The incidence of ROSC did not differ between public and non-public spaces (p = 0.4), but patients with OHCA in public spaces were more often admitted to the hospital with spontaneous circulation (p = 0.011). Shockable initial rhythm was not different between locations (p = 0.2), but defibrillation was performed significantly more often in public places (p p p = 0.006).ConclusionThe location of OHCA did not seem to affect the incidence of ROSC, although patients in public spaces had a higher chance to be admitted to the hospital with spontaneous circulation. Shockable initial heart rhythm, defibrillation, and the start of resuscitative efforts by an emergency physician were associated with higher chances of hospital admission with spontaneous circulation. Bystander CPR and bystander use of automated external defibrillators were low overall, emphasizing the importance of bystander education and training in order to enhance the chain of survival.
