OBJECTIF : Le but de ce travail est de détecter les facteurs de risque de survenue des infec- tions au cours d’une cure thermale. MÉTHODE : Les facteurs de risques potentiels ont été définis par consensus entre un pneumologue, un infectiologue et un rhumatologue thermal. Les pathologies présumées à risque étaient cancer ou hémopathie évolutifs, greffe d’organe, insuffisance respiratoire chronique, infection à virus de l’immunodéficience humaine. Les traitements potentiellement à risque étaient : méthotrexate ou néflunomide, traitement anti-TNF alpha, corticothérapie générale, traitement immunosuppresseur. Nous avons recruté prospectivement et consécutivement tous les curistes présentant au moins un facteur de risque et noté l’ensemble des événements survenant au cours de leur cure thermale. Les témoins étaient les curistes suivant immédiatement le curiste à risque sur la liste de rendez-vous. Les statistiques ont utilisé le test de Chi2 pour les comparaisons de variables qualitatives et le test U de Mann et Whitney pour les variables numériques. RÉSULTAT : Entre 2002 et 2004, les 10 médecins participant à l’étude ont surveillé 39150 cures sur les 98208 suivis aux Thermes nationaux d’Aix-les-Bains. Ils ont recruté 376 patients à risque et 362 témoins. Il y a eu 31 infections dans le groupe à risque et 20 dans le groupe témoins (7,6 % vs 4,9 %, p=0,14). Les analyses en sous-groupes de traitement ou de pathologie à risque ne montrent pas de différence significative avec les témoins, même si les chiffres sont proches du seuil pour les insuffisants respiratoires (24 % vs 7 % ; n=29, p=0,14). La réalisation d’une cure voies respiratoires (VR) est associée à un risque d’infection significatif chez les sujets à risque (29,1 % vs 0,0 % ; n=24, p=0,01). Le risque d’infection est particulièrement élevé dans le sous-groupe de patients effectuant une cure VR et présentant une insuffisance respiratoire (55 % vs 0 %, n=9 ; p=0,03). CONCLUSION : Les patients ayant une bronchite chronique ou une insuffisance respiratoire chronique, s’ils font une cure ORL associée à la cure rhumatologique ont un risque d’infection supérieur. Ce groupe semble donc présenter un véritable sur-risque d’infection et il faudra bien peser le rapport béné- fice /risque avant de leur proposer un séjour thermal., OBJECTIVE : The purpose of this study is to detect risk factor for incident infections during a spa therapy. METHODS : Potential risk factors were defined by consensus between a pneumologist, a specialist of infectious diseases and a senior rheumatologist practitioner of the spa center. Risk pathologies were cancer, hemopathy, organ graft, chronic bronchitis, respiratory deficiency and VIH infection. Risk treatments were methotrexate, neflunomide, TNF alpha blockers, corticoids, immunosupressors. We recruited prospectively and consecutively all patients treated in the spa center presenting almost one risk factor and note all diseases occurring during the spa stay. Controls were the patients following the risk patients on the appoint- ment list. Statistical anlysis used chi2 fort qualitative variable and U Mann Whitney test for continuous variables. RESULTS : Between 2002 and 2004, the 10 rheumatologist practitioners examined 39150 patients for spa therapy on the 98208 treated in the Aix-les-Bains spa centre. They recruited 376 risk patients and 362 controls. They were 31 infections in the risk group and 20 in the control group (7,6 % vs 4,9 %, p=0,14). Treatment or pathology subgroup analysis detected no significant difference with control even if, results are close of statistical significance for respiratory deficiency and chronic bronchitis (24 % vs 7 % ; n=29, p=0,14). Spa therapy for respiratory tract is associated with a significant increase of risk infection (29,1 % vs 0,0 % ; n=24, p=0,01). It is higher with patient subgroup associating spa for respiratory tract and chronic bronchitis or respiratory deficiency (55 % vs 0 %, n=9 ; p=0,03). CONCLUSION : Patients with chronic bronchitis or respiratory deficiency, if they have a spa therapy for respi- ratory tract have a higher incidence of infection. This group seems to present an over risk of infection and benefit/risk ratio have to be weighted before proposing them a spa stay.

This paper presents initial results from new velocity observations in the eastern part of the equatorial Atlantic Ocean from a moored current-meter array. During the "EQUALANT" program (1999-2000), a mooring array was deployed around the equator near 10 W that recorded one year of measurements at various depths. Horizontal velocities were obtained in the upper 60 m from an upward-looking acoustic Doppler current profiler (ADCP) and at 13 deeper levels from current meters between 745 and 1525 m. To analyze the quasiperiodic variability observed in these records, a wavelet-based technique was used. Quasiperiodic oscillations having periods between 5 and 100 days were separated into four bands: 5-10, 10-20, 20-40, and 40-100 days. The variability shows (i) a strong seasonality (the first half of the series is dominated by larger periods than the second one) and (ii) a strong dependence with depth (some oscillations are present in the entire water column while others are only present at certain depths). For the oscillations that are present in the entire water column the origin of the forcing can be traced to the surface, while for the others the question of their origin remains open. Phase shifts at different depths generate vertical shears in the horizontal velocity component with relatively short vertical scales. This is especially visible in long-duration events (>100 days) of the zonal velocity component. Comparison with a simultaneous lowered acoustic Doppler current profiler (LADCP) section suggests that some of these flows may be identified with equatorial deep jets. A striking feature is a strong vertical shear lasting about 7 months between 745 and 1000 m. These deep current-meter observations would then imply a few months of duration for the jets in this region.

Temperature, salinity, velocity, and wind from a mooring at 0 degrees N, 23 degrees W are used along with satellite data for sea surface temperature and sea level to examine the contribution of tropical instability waves ( TIWs) to the energy and heat balance of the equatorial Atlantic mixed layer. The TIWs appear as periodic 20 - 30 day fluctuations of currents, temperature, and salinity, which intensify beginning in June and peak in late boreal summer. The intensification occurs in phase with strengthening of the southeasterly trade winds and the seasonal appearance of the equatorial tongue of cold mixed layer temperatures. In 2002 these waves, which warm the mixed layer by 0.35 degrees C during summer months, are maintained by both barotropic and baroclinic conversions that are of comparable size. Salinity fluctuations, previously neglected, increase the magnitude of baroclinic energy conversion.

Nous présentons des résultats d'expériences d'assimilation séquentielle de données altimétriques dans un modèle linéaire de l'océan Atlantique tropical. Nous assimilons des champs d'anomalies du niveau de la mer issus des mesures altimétriques du satellite Geosat. En dépit des faibles amplitudes des anomalies, nous observons des améliorations très nettes des simulations du modèle grâce aux assimilations. Nous montrons de ce fait que les données altimétriques peuvent se révéler très utiles pour améliorer les simulations des modèles en utilisant des techniques d'assimilation adaptées. (Résumé d'auteur)
