Resumen del trabajo presentado al 8th Forum on New Materials - CIMTEC, celebrado en Perugia (Italia) del 4 al 14 de junio de 2018., Conductive polymers are very attractive for biomedical applications. Their responsiveness to electrical stimulation found application in nerve regeneration strategies, enhanced neuronal growth or tissue regeneration. Often their broad use is hampered by low biocompatibility,lack of recognition elements for endogenous components, thus alternative strategies may rely on improvement of fibrillar proteins properties in situ. In this direction, the fluorophore dithienothiophene-S,S-dioxide (DTTO) is able to spontaneously enter human fibroblasts and become incorporated into collagen quaternary structures giving rise to fluorescent and conducting fibrils. In order to translate to an in vivo system, we employed the freshwater Hydra vulgaris, which structural simplicity resembles a living tissue, without organs. By challenging polyps with DTTO we demonstrated the incorporation of this thiophene into supramolecular structures giving rise to microfibrils. Electronic force microscopy showed electrical conductivity and circular dichroism confirmed the presence of proteins within the fibrils. Besides the possibility of engineering endogenous components with synthetic moieties, this approach opens the path to a sort of ‘live animal factory’ for the production of innovative biomaterials.

Multianalyte biosensor patch based on polymeric microneedles (MNs) have been fabricated by photolithography of poly (ethylene glycol) diacrylate (PEGDA) and have been dynamically characterized by means of quartz crystal microbalance and electrochemical measurements. The resulting flexible device acts as multianalyte working electrodes detecting glucose and lactic acid into interstitial liquid by means of redox reaction with glucose oxidase (GOx) and lactose oxidase (LOx) enzymes. Sensitivities of the order of nA mM−1 in the mM range are revealed in both case, after five minutes of swelling time and ten second of interaction.

In the last years, in order to achieve active tuning of photonic crystals devices, the possibility to use liquid crystal inside photonic crystals has been explored. On this line of argument, in this paper, we numerically investigate a tunable T-shaped waveguide diplexer, based on a two-dimensional square lattice photonic crystal composed of silicon rods in a liquid crystals. We prove that complete splitting of the entire input wavelengths range in two sub-ranges symmetrical with respect to the middle (switching) wavelength, and propagating in right and left arms respectively, can be achieved. Moreover, changing the refractive index of liquid crystals by electro-optical effect, a tuning of switching wavelength of about 60 nm can be obtained.

Microneedles are largely applied in biomedicine, both in diagnostics and therapeutics, since they can be considered the perfect interface between patient and sensing/dispensing devices. In this paper, we present an electro-chemical biosensor based on polymeric microneedles array for the measure of glucose level in interstitial fluids. The microneedles array has been fabricated by photolithography of a polymer, PolyEthylene Glycol DiAcrylate hydrogel, mixed with an enzyme, Glucose oxidase, and a redox mediator, the Vynilferrocene. The microneedles array acts as a working electrode when gold plated by sputtering and their tips have been etched to create a sensitive area. The redox reaction with glucose creates a charge transfer, enhanced by the redox mediator, resulting in a current proportional to the glucose concentration.

Microneedles have been widely applied in biomedicine as tool both in diagnostics and therapeutics. In fact, microneedles are the perfect and painless interface between the derma and a device. Using microneedles as interface and a porous Silicon membrane as reservoir and optical controller, it is possible to realize a smart device for drug delivery. In this paper, we present the fabrication and the results for a patch device loaded with fluorescein molecules, as proof of concept.

Significance:Continuous external tissue expansion (CETE) is a versatile tool in soft tissue injury management, and could be an addition to the traditional reconstructive ladder.Recent Advances:This critical review discusses the principles and application of CETE, covering a company-sponsored consensus meeting on this emerging technology and highlighting the DermaClose®(Synovis Micro Companies Alliance, Inc., Birmingham, AL) device's unique approach to soft tissue injury management. There is clinical evidence to support the use of CETE in the management of a number of wound types, including fasciotomy, trauma, amputation, and flap donor sites. The device can be applied to open wounds, potentially avoiding the need for a skin graft or other more complex or invasive reconstruction options. DermaClose applies constant tension without restricting blood flow and does not require repeated tightening.Critical Issues:CETE is becoming more widely used by surgeons of different specialties, and numerous reports describing its efficacy and safety in wound management have been published. Surgeons using CETE must follow the correct technique and select patients carefully to achieve optimal outcomes. However, there is no single source of information or consensus recommendations regarding CETE application.Future Directions:Prospective evidence on the efficacy and safety of CETE in clinical practice is required to communicate the best techniques and share important experiences. This will help to solidify its place in the reconstructive ladder as a valuable additional option for surgeons.

In this work we have explored the photoluminescence signal emitted by graphene oxide (GO) nanosheets infiltrated in silanized porous silicon (PSi) matrix. A strong enhancement of the PL emitted from GO by a factor of 2.5 with respect to GO on crystalline silicon was successfully demonstrated. In addition, a weak wavelength modulation of GO photoluminescence emission was observed, resulting in very attractive aspect for its exploitation in innovative optoelectronic devices and high sensible fluorescent sensors.

Microneedles are newly developed biomedical devices, whose advantages are mainly in the non-invasiveness, discretion and versatility of use both as diagnostics and as therapeutics tool. In fact, they can be used both for drugs delivery in the interstitial fluids and for the analysis of the interstitial fluid. In this work we present the preliminary results for two devices based on micro needles in PolyEthylene (Glycol). The first for the drugs delivery includes a membrane whose optical reflected wavelength is related to the concentration of drug. Here, we present our preliminary result in diffusion of drugs between the membrane and the microneedles. The second device is gold coated and it works as electrode for the electrochemical detection of species in the interstitial fluid. A preliminary result in detection of glucose will be shown.

In this work, we present a patented procedure that generates three dimensional (3D) image of a microscopic object with high definition, obtained by merging Scanning Electron Microscopy (SEM) and Digital Holography Microscopy (DHM) data. A high spatial resolution (up to r z =λ/20, r yx ≅100nm) 3D model of the object is generated starting from two digital images, acquired separately, following standard characterization procedures. Then, the two set of data are combined using an original mathematical algorithm, which has been implemented with a commercially available software. The developed methodology is of broad interest and has been applied to elements of biological interest, in order to prove its features.

Diatoms are monocellular algae responsible of 20-25% of the global oxygen produced by photosynthetic processes. The protoplasm of every single cell is enclosed in an external wall made of porous hydrogenated silica, the frustule. In recent times, many effects related to photonic properties of diatom frustules have been discovered and exploited in applications: light confinement induced by multiple diffraction, frustule photoluminescence applied to chemical and biochemical sensing, photonic-crystal-like behavior of valves and girdles. In present work we show how several techniques (e.g. digital holography) allowed us to retrieve information on light manipulation by diatom single valves in terms of amplitude, phase and polarization, both in air and in a cytoplasmatic environment. Photonic crystal metamaterial can exhibit negative index properties and this behaviour is well described by a resonator model. In this work, we present the experimental evidence that a Lorentz resonator correctly reconstruct data obtained with a negative refracting Photonic Crystal (PhC) by using a standard optical technique, such as ellipsometry. In particular we show that, in the frequency range in which the effective refractive index, neff, is equal to -1, the incident light couples efficiently to the guided modes in the top surface layer of the PhC metamaterial. These modes resemble surface plasmon polariton resonances. In add we present measurements by using standard technique of prism coupling evanescent wave. Once again the presence of localized plasmon-like modes at the surface of a silicon two-dimensional photonic crystal slab is demonstrated. Also in this case, in analogy with surface plasmons supported in metals in a photonic crystal metamaterial, the electromagnetic surface waves arise from a negative effective permittivity. These results opens new strategies in light control at the nanoscale, allowing on chip light manipulation in a wide frequency range and avoiding the intrinsic limits of plasmonic structures due to absorption losses in metals. The synthesis of metallic plasmonic nanoparticles (NPs) faces challenges in stability and reproducibility, especially with silver. Here, we present a protocol for tunable synthesis of spherical silver NPs (AgNPs) with stable optical properties. We describe steps for preparing solutions, morphological characterization of AgNPs by transmission electron microscopy, and testing stability. AgNPs exhibit enduring stability and compatibility with various pH values. Moreover, they can be functionalized for optical biosensing applications, offering versatility in nanomaterial applications.

In a quasi-zero-average-refractive-index (QZAI) metamaterial, the light scattered out is extremely directive (Delta theta(out) = 0.06 degrees), in despite of a divergent source at near infrared wavelength (lambda=1.55 mm). In this paper we discuss and experimentally demonstrate that this is possible when the light is coupled with diffraction order of a grating with alternating complementary media. The experimental data prove with a high degree of accuracy also the strong vertical confinement of the beam even in the air region of the metamaterial, where any simple vertical confinement mechanism is absent. This extremely sensitive device works on a large contact area and open news perspective to integrated spectroscopy.

In this work a complete and dedicated technology platform for all-organic electronic applications with micron and sub-micron feature-size is proposed. Both the organic technology and the design tools have been developed by considering organic materials to be deposited by solution techniques. The complete process flow has been defined in order to realize multilayered functional structures as all-organic active and passive devices. Feature sizes ranging from 50 nm up to 10 mum can be achieved according to the adopted solution processing techniques. The technology platform comprises also the CAD tools required for the design and realization of the all-organic devices.

Today, nanophotonics still lacks components for modulation that can be easily implementable in existing silicon-on-insulator (SOI) technology. Chalcogenide phase change materials (PCMs) are promising candidates for tuning in the near infrared: at the nanoscale, thin layers can provide enough contrast to control the optical response of a nanostructure. Moreover, all-dielectric metamaterials allow for resonant behavior without having ohmic losses in the telecom range. Here, a novel hybridization of a SOI-based metamaterial with PCM GeTe is experimentally investigated. A metamaterial based on Si nanorods, covered by a thin layer of GeTe, is designed and fabricated. Switching GeTe from amorphous to crystalline leads to a rather high resonance-governed reflection contrast at 1.55 μm. Additional confocal Raman imaging is done to differentiate the crystallized zones of the metamaterials’ unit cell. The findings are in good agreement with numerical analysis and show good perspectives of all-dielectric tunable near-infrared nanophotonics.

Operative management of complex ventral hernia still remains a significant challenge for surgeons. Closure of large defects in the unprepared abdomen has serious pathophysiological consequences due to chronic contraction and retraction of the lateral abdominal wall muscles. We report outcomes of 56 consecutive patients who had preoperative Botulinum toxin A (BTA) abdominal wall relaxation facilitating closure and repair. This was a prospective observational study of 56 patients who underwent ultrasound-guided BTA into the lateral abdominal oblique muscles prior to elective ventral hernia repair between November 2012 and January 2017. Serial non-contrast abdominal CT imaging was performed to evaluate changes in lateral oblique muscle length and thickness. All hernias were repaired laparoscopically, or laparoscopic-open-laparoscopic (LOL) using intraperitoneal onlay mesh. 56 patients received BTA injections at predetermined sites to the lateral oblique muscles, which were well tolerated. Mean patient age was 59.7 years, and mean BMI was 30.9 kg/m2(range 21.8–54.0). Maximum defect size was 24 × 27 cm. A subset of 18 patients underwent preoperative pneumoperitoneum as an adjunct procedure. A comparison of pre-BTA to post-BTA imaging demonstrated an increase in mean lateral abdominal wall length from 16.1 cm to 20.1 cm per side, a mean gain of 4.0 cm/side (range 1.0–11.7 cm/side) (p< 0.0001). This corresponds to an unstretchedmean length gain of 8.0 cm of the lateral abdominal wall. Laparoscopic/LOL primary closure was achieved in all cases, with no clinical evidence of raised intra-abdominal pressures. One patient presented with a new fascial defect 26 months post-operative. Preoperative BTA to the lateral abdominal wall muscles is a safe and effective technique for the preparation of patients prior to operative management of complex ventral hernias. BTA temporary flaccid paralysis relaxes, elongates and thins the chronically contracted abdominal musculature. This in turn reduces lateral traction forces facilitating laparoscopic repair and fascial closure of large defects under minimal tension.

Although 90% of women at risk for unintended pregnancy in the United States use contraception, <1% of these women use the intrauterine contraceptive device. The mechanism of action of intrauterine contraceptive devices has been controversial, but several studies suggest that interference with sperm migration or function and with fertilization may be the most likely mechanisms. More important, there is lack of compelling evidence that the intrauterine contraceptive device acts as an abortifacient. The risk for pelvic inflammatory disease among users now appears to be extremely low, primarily as a result of better selection of candidates. A levonorgestrel-releasing intrauterine contraceptive device may offer some new therapeutic approaches for the treatment of certain gynecologic disorders. Although women who are not at risk for pelvic inflammatory disease or sexually transmitted diseases are appropriate candidates for the intrauterine contraceptive device, it appears that use can be expanded to selected nulliparous women and women with certain medical conditions. - Abstract
Insulin is a common treatment option for many patients with type 2 diabetes, and is generally used late in the natural history of the disease. Its injectable delivery mode, propensity for weight gain and hypoglycaemia, and the paucity of trials assessing the risk-to-safety ratio of early insulin use are major shortcomings associated with its use in patients with type 2 diabetes. Development of new insulins—such as insulin analogues, including long-acting and short-acting insulins—now provide alternative treatment options to human insulin. These novel insulin formulations and innovative insulin delivery methods, such as oral or inhaled insulin, have been developed with the aim to reduce insulin-associated hypoglycaemia, lower intraindividual pharmacokinetic and pharmacodynamic variability, and improve imitation of physiological insulin release. Availability of newer glucose-lowering drugs (such as glucagon-like peptide-1 receptor agonists, dipeptidyl peptidase-4 inhibitors, and sodium-glucose co-transporter-2 inhibitors) also offers the opportunity for combination treatment; the results of the first trials in this area of research suggest that such treatment might lead to use of reduced insulin doses, less weight gain, and fewer hypoglycaemic episodes than insulin treatment alone. These and future developments will hopefully offer better opportunities for individualisation of insulin treatment for patients with type 2 diabetes.

Context:The mechanisms responsible for contribution of variants in the gene TFC7L2to the risk for type 2 diabetes (T2DM) remains far from being completely understood, and available studies have generated nonunivocal results.Objective:We investigated the postprandial glucose metabolism in subjects at risk for T2DM carrying the TCF7L2risk allele.Design, Setting, and Participants:Twenty-three subjects carrying the risk-conferring TCF7L2 genotypes (11 TT and 12 CT at rs7901346) and 13 subjects with wild-type genotype (CC) underwent a standard mixed-meal test (MMT) in combination with stable isotope tracers.Outcome Measurements:We evaluated endogenous and exogenous glucose fluxes and hormonal responses.Results:Fasting plasma glucose, insulin, C-peptide, glycated hemoglobin, endogenous glucose production, and plasma glucose clearance were similar in the three groups, whereas plasma glucagon levels were lower in both CT and TT than in CC (64 ± 20, 63 ± 18 and 90 ± 29 pg/mL, respectively; both P= .01). In response to the MMT, TT subjects had lower plasma glucose levels than CC subjects [mean area under the time-concentration curve (AUC) 6.1 ± 3.9 vs 7.1 ± 12.0 mmol/L, P= .04] and lower insulin secretion rate (mean AUC 385 ± 95 vs 530 ± 159 pmol/m2· min, P= .02). Initial (0–60 min) rate of appearance (Ra) of oral glucose was lower in TT compared with CT/CC (AUC 2.7 ± 1.1 vs 3.8 ± 1.2 μmol/kg · min, P= .02) with no difference among the three groups in endogenous glucose production. The AUC0–60minfor Ra of exogenous glucose (Raex) was positively correlated with the plasma glucose AUC0–60min. Total RaexAUC0–120minwas correlated with total AUC0–120minof plasma glucose (r = 0.45, P< .01). Plasma glucagon-like peptide-1 and glucose-dependent insulinotropic peptide levels in response to the MMT were not affected by genotype.Conclusions:In subjects at risk for T2DM, the TCF7L2 polymorphisms were associated with reduced Raexinto systemic circulation, causing reduced postprandial blood glucose increase and, in turn, lower insulin secretion rate with no impairment in β-cell function. The reduced Raexis likely due to greater glucose retention in the splanchnic area.

In the narrative framework of the Decameron, banquets take on stylized forms as if they followed a codified ritual: there are intervals of music playing, singing and dancing. On the other hand, the tales offer a variety of eating modes, associated to various private and public situations. We have food consumption as a high-class show. a frugal meal, a necessary event, an imaginary scene (Cockaigne), and a medicine for good health. Moreover, food is occasionally related to sexual and even sadistic scenes. Each of these situations has its own stylistic, pragmatic and textual features, which are analyzed in the paper also through a comparison of Boccaccio's writing with other late Latin and Romance narrative texts (i.e. Coena Cipriani, Marguerite's Navarra Heptameron, French fabliaux and Italian tales by Sercambi and Saechetti). - Abstract
Graphene oxide (GO) is a photoluminescent material whose application in optoelectronics has been strongly limited due to its poor emission intensity. In this work, a GO–porous silicon (GO–PSi) hybrid structure is realized in order to investigate the emission properties of GO infiltrated into a porous matrix. GO–PSi is characterized by Fourier transform infrared spectroscopy, spectroscopic reflectometry, and steady-state photoluminescence. A photoluminescence enhancement by a factor of 2.5 with respect to GO deposited on a flat silicon surface is demonstrated. Photoluminescence measurements also show a modulation of the emitted signal; this effect is attributed to the interference phenomena occurring inside the PSi monolayer.

The vast majority of Alzheimers disease (AD) are late-onset forms (LOAD) likely due to the contribution of genetic, environmental, and stochastic factors, superimposed on a physiologically age-related decline of neuronal functions. Increasing evidence indicates epigenetic modifications in LOAD brains, and many of the environmental factors associated with AD risk, such as heavy metals and dietary factors, are able to modify the epigenome. There is also indication that environmentally-induced early life modifications of the genome during embryogenesis and brain development could contribute to the development of the disease later in life. DNA methyltransferase 3b (DNMT3b) is an enzyme involved in de novo methylation of the genome during embryogenesis, expressed in progenitor cells during neurogenesis. In the present study we evaluated two functional DNMT3B promoter polymorphisms, namely -149 C>T (rs2424913) and - 579 G>T (rs1569686), as candidate LOAD risk factors. Our analysis of 376 Italian LOAD patients and 308 matched controls revealed no difference in allele frequencies between the case an the control group (OR 1.10 (0.88-1.39) for rs2424913, and OR 1.02 (0.81-1.28) for rs1569686). Also the genotype distributions of both polymorphisms were closely similar between groups, and no significant effect on disease age at onset was observed. Overall, present results do not support a major role for rs2424913 or rs1569686 in LOAD pathogenesis.

Tyrosine kinase receptors have been shown to play an important role in epithelial thyroid tumor growth and angiogenesis. Thyroid cancers commonly present oncogene mutations involved in MAPK kinase pathway like BRAF and RET; they are also frequently dependent on VEGF stimuli. Preliminary clinical experiences suggest a promising role of sunitinib (a tyrosine kinase inhibitor) for the treatment of advanced thyroid cancers. This review deals with the available data on the effect of sunitinib in the treatment of metastatic, radioiodine refractory thyroid cancers. We also report our experience with the off-label use of sunitinib in such patients.

- Abstract
In a quasi- zero-average-refractive-index (QZAI) metamaterial, the light scattered out is extremely directive (out = 0.06°), in despite of a divergent source at near infrared wavelength (=1.55 mm). In this paper we discuss and experimentally demonstrate that this is possible when the light is coupled with diffraction order of a grating with alternating complementary media. The experimental data prove with a high degree of accuracy also the strong vertical confinement of the beam even in the air region of the metamaterial, where any simple vertical confinement mechanism is absent. This extremely sensitive device works on a large contact area and open news perspective to integrated spectroscopy.

93. Body Perfusion during Adult Cardiopulmonary Bypass is Improved by Pulsatile flow with Intra-Aortic Balloon Pump

Purpose To evaluate if the use of an intra-aortic balloon pump (IABP) during cardioplegic arrest improves body perfusion.Methods 158 coronary artery bypass graft (CABG) patients were randomized to linear cardiopulmonary bypass (CPB) (n=71, Group A) or automatic 80 bpm intra-aortic ballon pump (IABP) induced pulsatile CPB (n=87, Group B). We evaluated hemodynamic response by Swan-Ganz catheter, inflammation by cytokines, coagulation and fibrinolysis, transaminase, bilirubin, amylase, lactate and renal function (estimated glomerular filtration rate (eGFR), creatinine, and incidence of renal insufficiency and failure).Results IABP induced Surplus Hemodynamic Energy was 15.8±4.9 mmHg, with higher mean arterial pressure during cross-clamping (p=0.001), and lower indexed systemic vascular resistances during cross-clamping (p=0.001) and CPB discontinuation (p=0.034). IL-2 and IL-6 were lower, while IL-10 proved higher in Group B (p<0.05). Group B showed lower chest drainage (p<0.05), transfusions (p<0.05), INR (p<0.05), and AT-III (p=0.001), together with higher platelets, aPTT (p<0.05), fibrinogen (p<0.05) and D-dimer (p<0.05). Transaminases, bilirubin, amylase, lactate were lower in Group B (p<0.05); eGFR was better in Group B from ITU-arrival to 48 hours, both in preoperative kidney disease Stages 1–2 (p<0.03) and Stage 3 (p<0.05), resulting in lower creatinine from ITU-arrival to 48 hours (p<0.03). Incidence of renal insufficiency (p=0.004) and need for renal replacement therapy (p=0.044) was lower in Group B Stage 3. Group B PaO2/FiO2 and lung compliance improved from aortic declamping to the first day (p<0.003) with shorter intubation time (p=0.01).Conclusion Pulsatile flow by IABP improves whole-body perfusion during CPB.

Endothelial dysfunction represents an important pathway thereby cardiovascular risk factors promote the development and progression of atherosclerosis. Hypothyroidism is associated with an increased cardiovascular risk, and the assessment of endothelium function is recognised an effective tool for the detection of evidence of preclinical cardiovascular alterations. Both vascular smooth muscle cells and endothelium play pivotal roles in modulating vascular tone and both are potential targets of thyroid hormone action. The pathogenesis of the association between endothelial dysfunction and hypothyroidism is complex and still not well established. The presence of traditional and emerging risk factors may contribute to the development of endothelium impairment, generating a chronic state of injury that triggers abnormal endothelial response. Levothyroxine replacement therapy is currently used for restoring euthyroidism and improving cardiovascular risk of hypothyroid patients. The decision to treat patients with subclinical hypothyroidism should depend on the presence of risk factors, rather than on a TSH threshold. However, the actual effectiveness of thyroid hormone substitution in reducing the risk of cardiovascular events, especially in subclinically hypothyroid patients, remains to be elucidated. Large multicenter, placebo-controlled prospective trials are necessary to address the issue. The article also discusses recent patents in this field.

Two N-salicylidene-N′-acylhydrazines tridentate ligands containing heterocycles were prepared and utilized for the synthesis of mononuclear acentric complexes of CuII and PdII and pyridine serves as an additional ligand (MLxPy, M = metal, Lx = ligand). Characterization also includes single-crystal X-ray diffraction analysis. These complexesshow high second-order nonlinear optical activity (μβ = 620 × 10–48esu for both CuLIIPy and PdLIIPy and μβ = 1600 × 10–48 and 1400 × 10–48 esu for CuLIPy and PdLIPy, respectively, at incident wavelength of 1907 nm). The properties of polymers (PMLx) obtained by grafting poly(4-vinylpyridine) with fragments of the complexes are also reported. The polymers exhibit good thermal stability, high glass-transition temperatures and the absence of crystallinity. The second-order nonlinear optical (NLO) properties of thin, transparent poled films, prepared by spin coating and thermal corona poling, were investigated for some of the polymers. The value of the d33 parameter was measured at 1064 and 1500 nm (i.e., with and without resonance contribute). In the latter case, the d33 values are 21 and 12 pm/V for PCuLII and PPdLII, respectively. A high time stability of the NLO properties of these materials was found. (© Wiley-VCH Verlag GmbH & Co. The aim of the present study was to analyze heart function in subclinical hyperthyroidism (sHT) in otherwise healthy subjects by new methods using intramyocardial ultrasonic techniques. Twenty-four newly diagnosed and untreated sHT patients (20 women, 4 men; mean age: 42±4 yr) and 24 sex- and age-matched healthy volunteers were studied. All subjects were submitted to conventional 2D color-Doppler echocardiography, pulsed wave tissue Doppler imaging (PWTDI) for the analysis of diastolic function, color Doppler myocardial imaging (CDMI) for the analysis of regional strain and strain rate (SR) expression of regional myocardial deformability, and to integrated backscatter (IBS) for the evaluation of intrinsic contractility and tissue characterization. Regional myocardial systolic strain findings were significantly higher in sHT patients when compared with controls (p<0.001). Considering diastolic SR, the early phase of diastolic SR was compromised in sHT subjects as compared with controls (p<0.001). Cyclic variation index (CVI), expression of intrinsic contractility, was significantly higher in sHT subjects in comparison with controls (p<0.0001). IBS values were comparable between the 2 study groups. In conclusion, the present study suggests that in patients with sHT early systolic hyperdeformability and hypercontractility are present, together with impairment of both active and passive phases of diastole. On the contrary, no left ventricular hypertrophy or other structural alterations are documented.

The aim of the present study was to evaluate cardiac function and texture in patients with subclinical hypothyroidism (sHT) both by conventional and new ultrasonic intramyocardial tissue techniques. sHT was characterized by normal serum free tetraiodotironine and free triiodotironine levels and slightly increased serum TSH level. Twenty-four patients affected by sHT and 24 sex- and age-matched healthy volunteers were studied. The results of the present study were: (a) the detection in sHT subjects of a lower cyclic variation index (CVI) indicating an altered myocardial intrinsic contractility; (b) a higher ultrasonic myocardial reflectivity indicating an altered myocardial texture; (c) the detection of lower systolic strain and strain-rate indicating an alteration of myocardial regional deformability; (d) an initial impairment of left ventricular diastolic function indicated by a decrease of peak E mitral flow velocity and an increase of peak A mitral flow velocity. All parameters studied with conventional 2D-echo in sHT patients were comparable with controls, except for a mild alteration in diastolic function. A significant correlation among systo-diastolic modifications detected by CDMI and IBS and serum TSH levels were found. The CVI at septum, the PWDTI S-peak wave and the systolic strain at septum were inversely related to the serum TSH levels. In conclusion, the new intramyocardial ultrasonic techniques confirm and extend the previous knowledge on the effect of the sHT on the heart, allowing the detection of early ultrastructural and regional functional systolic and diastolic abnormalities.

- Abstract
We investigate the tunability of a T-shaped waveguiding diplexer, based on a two-dimensional square lattice photonic crystal (PhC) composed of silicon rods in air and in liquid crystals (LCs), respectively. For both devices we prove that a complete splitting of the entire input wavelength range into right and left arms respectively can be achieved. Moreover, a tuning of the switching wavelength can be obtained by changing the refractive index of silicon by the thermo-optic effect for PhCs in air, and the refractive index of liquid crystals by the electro-optic effect for PhCs in LCs.

In this paper the relationship between maximum uptake of 131I, fractional reduction of thyroid volume and outcome of Graves' disease is discussed.

- Abstract
Substantial reductions in thyroid volume (up to 7080%) after radioiodine therapy of Graves' hyperthyroidism are common and have been reported in the literature. A relationship between thyroid volume reduction and outcome of 131I therapy of Graves' disease has been reported by some authors. This important result could be used to decide individually the optimal radioiodine activity A0 (MBq) to administer to the patient, but a predictive model relating the change in gland volume to A0 is required. Recently, a mathematical model of thyroid mass reduction during the clearance phase (3035 days) after 131I administration to patients with Graves' disease has been published and used as the basis for prescribing the therapeutic thyroid absorbed dose. It is well known that the thyroid volume reduction goes on until 1 year after therapy. In this paper, a mathematical model to predict the final mass of Graves' diseased thyroids submitted to 131I therapy is presented. This model represents a tentative explanation of what occurs macroscopically after the end of the clearance phase of radioiodine in the gland (the so-called second-order effects). It is shown that the final thyroid mass depends on its basal mass, on the radiation dose absorbed by the gland and on a constant value α typical of thyroid tissue. α has been evaluated based on a set of measurements made in 15 reference patients affected by Graves' disease and submitted to 131I therapy. A predictive equation for the calculation of the final mass of thyroid is presented. It is based on macroscopic parameters measurable after a diagnostic 131I capsule administration (0.371.85 MBq), before giving the therapy. The final mass calculated using this equation is compared to the final mass of thyroid measured 1 year after therapy administration in 22 Graves' diseased patients. The final masses calculated and measured 1 year after therapy are in fairly good agreement (R = 0.81). The possibility, for the physician, to decide a therapeutic activity based on the desired decrease of thyroid mass instead of on a fixed thyroid absorbed dose could be a new opportunity to cure Graves' disease.
