51. Optimal Tracking Control for Underactuated Airship

Abstract

Background: A non-linear mathematical model of underactuated airship is derived in this paper based on Euler-Newton approach. The model is linearized with small disturbance theory, producing a linear time varying (LTV)model. The LTV model is utilized to design a linear quadratic tracking (LQT) controller. Two scenarios of LQT are presented in this work according to assumed costates transformations to compute the LQT control law. Results: The LTV model is verified by comparing its output response with the result of the nonlinear model for a given input signal. It shows an acceptable error margin. The verified LTV model is used in designing the LQT controller. The controller is designed to minimize the error between the output and required states response with acceptable control signals using a weighted cost function. Two LQT controllers are presented in this work based on two different transformations used in solving the differential Riccati equation (DRE). These controllers are tested by a sample trajectory to deduce the characteristics of each assumption. Finally, a hybrid LQT controller is used and tested on circular, helical, and bowed trajectories. Conclusion: The first assumption of costates transformation has a good tracking performance, but it is sensitive to the change of trajectory profile. Whereas, the second one overcomes this problem due to considering the trajectory dynamics. Therefore, the first assumption is performed across the whole trajectory tracking except for parts of trajectory profile changes where the second assumption is applied.

52. The Use of Prognostic Models in Allogeneic Transplants: A Perspective Guide for Clinicians and Investigators

Abstract

Allogeneic hematopoietic cell transplantation (HCT) can cure many hematologic diseases, but it carries the potential risks of increased morbidity and mortality rates. Prognostic evaluation is a scientific entity at the core of care for potential recipients of HCT. It improves the decision-making process of transplant versus not and of which transplant strategy and allows for future trials targeting patients' intolerances to transplant; hence it ultimately improves transplant outcomes. Prognostic models are key for appropriate actuarial outcome estimates, which has been frequently shown to be better than physicians' subjective estimates. To make the most accurate prognostic evaluation for HCT, one should rely on more than one prognostic model. For relapse and relapse-related mortality risks, the refined disease risk index is currently the most informative model. It can be supplemented by disease-specific models that consider genetic mutations as predictors in addition to information on measurable residual disease. For non-relapse mortality and HCT-related morbidity risks, the HCT-comorbidity index and Karnofsky performance status have proven to be the most reliable and most accepted by physicians. These can be supplemented by gait speed as a measure of frailty. Some other global prognostic models might add additional prognostic information. Physicians' educated perceptions can then put all this information in context taking into consideration conditioning regimen and donor choices. The future of transplantation mandates 1) clinical investigators specifically trained in prognostication, 2) increased reliance on geriatric assessment, 3) the use of novel biomarkers such as genetic variants, and 4) successfully applying novel statistical methods such as machine-learning.

54. Effect of the COVID-19 Pandemic on the Rate of Interventional Pain Management Therapies. Could the Application of Personal Protective Equipment Help?

Abstract

Chronic pain symptoms are distressing conditions that necessitate regular visits to pain therapists and may require interventions, however, the COVID-19 pandemic has caused patients and their therapists to limit both visits and interventions with the transition to telehealth, with little or no preparation or training. This has resulted in the extensive use of over-the counter analgesia and corticosteroids.Our study aimed to evaluate the effect of the COVID-19 pandemic on the rates of counseling and interventional pain management therapies (IPMT), and determine the effects of implementing an infection control program (ICP) and mandating personal protective equipment (PPE) on these rates.Prospective multicenter survey, based on an online self-assessed questionnaire.Departments of Anesthesia, Pain, Intensive Care Unit, Physical Medicine, Rheumatology, and Rehabilitation at Egyptian University hospitals.A self-assessed questionnaire was uploaded on Google forms and links were sent to enrolled therapists with an identification number to allow self-administration and privacy. Feedback was analyzed by 2 authors who were blinded to the identity of the responders.A total of 57.9% of responders increased their patients' contact by phone and video conference. Within 1-4 months after the outbreak began, 59% stopped in-person contact and 38.2% stopped their IPM practice. Prescriptions of analgesics and oral steroids increased by about 50%. The majority of responders complained of a shortage of ventilation appliances in their workplaces. About 50% of them always use ICP, 85% use surgical masks, 61% use gloves, and 45% wear gowns when meeting with patients. After the application of PPE, 45.5% of responders increased their consultation rate and 40% increased their rate of IPMT.This study is limited to being a national study, and so lacked comparative data.The COVID-19 outbreak seriously affected the rates of in-person consultations and IPMT for patients with chronic pain and increased the rates of consumption of analgesia and oral steroids. Most responders reported a shortage of PPE especially ventilation appliances in workplaces. A high percentage of responders lack interest in ICP and PPE, despite the positive effects of its application on consultation and IPMT rates.

Abstract

Background The African Surgical OutcomeS-2 (ASOS-2) trial tested an enhanced postoperative surveillance intervention to reduce postoperative mortality in Africa. We undertook a concurrent evaluation to understand the process of intervention delivery. Methods Mixed-methods process evaluation, including field notes, interviews, and post-trial questionnaire responses. Qualitative analysis used the framework method with subsequent creation of comparative case studies, grouping hospitals by intervention fidelity. A post-trial questionnaire was developed using initial qualitative analyses. Categorical variables were summarised as count (%) and continuous variables as median (inter-quartile range [IQR]). Odds ratios (OR) were used to rank influences by impact on fidelity. Results The dataset included eight in-depth case studies, and 96 questionnaire responses (response rate 67%) plus intervention fidelity data for each trial site. Overall, 57% (n=55/96) of hospitals achieved intervention delivery using an inclusive definition of fidelity. Delivery of the ASOS-2 interventions and data collection presented a significant burden to the investigators, outstripping limited resources. The influences most associated with fidelity were: surgical staff enthusiasm for the trial (OR=3.0; 95% confidence interval [CI], 1.3–7.0); nursing management support of the trial (OR=2.6; 95% CI, 1.1–6.5); performance of a dummy run (OR=2.6; 95% CI, 1.1–6.1); nursing colleagues seeing the value of the intervention(s) (OR=2.1; 95% CI, 0.9–5.7); and site investigators' belief in the effectiveness of the intervention (OR=3.2; 95% CI, 1.2–9.4). Conclusions ASOS-2 has proved that coordinated interventional research across Africa is possible, but delivering the ASOS-2 interventions was a major challenge for many investigators. Future improvement science efforts must include better planning for intervention delivery, additional support to investigators, and promotion of strong inter-professional teamwork. Abstract

To assess the pattern of internet usage among preparatory school students in Beni-Suef Governorate, Egypt, and the Al Qassim Region, Saudi Arabia, and identify its associated factors. A cross-sectional study using a multistage proportionate sampling technique was conducted from October to December 2019 for 1537 preparatory school students from both countries; 772 from Beni-Suef Governorate, Egypt, and 765 students from the Al Qassim Region, Saudi Arabia. The Young’s Internet Addiction Test (YIAT) was used. Mean age of Egyptian and Saudi participants were 12.82 ± 0.81 and 13.8 ± 1.16 years, respectively. Mild to moderate IA was reported in 42.5% & 42.9%, among Egyptian students compared to 51.6% & 24.3% for Saudis, while severe IA was reported in 4.0% & 3.7% among Egyptian and Saudis, respectively. Internet searches were conducted by 43.5% among Egyptian students compared to online communication among Saudis (52.9%). Facebook was used by 42.0% of Egyptian students, while 52.4% Saudi students used Snap chat. In both samples, there was a statistically significant difference between internet addiction and gender, academic year, level of academic achievement, and purpose of internet use (p ≤ 0.05). Excessive use of the internet is a common finding among Egyptian and Saudi preparatory student internet users.

58. An 8-year pragmatic observation evaluation of the benefits of allogeneic HCT in older and medically infirm AML patients

Abstract

We designed a prospective, observational study enrolling patients presenting for treatment of AML at 13 institutions to analyze associations between hematopoietic cell transplantation (HCT) and survival, quality of life (QOL), function and geriatric health, in 6 groups: 1) the entire cohort, 2) ≥65 years old, 3) high comorbidity burden, 4) intermediate cytogenetic-risk, 5) adverse cytogenetic-risk, and 6) first complete remission with or without measurable residual disease. Patient health and preferences were assessed eight times across 2 years. Time-dependent regression models were used. Among 692 evaluable patients, 46% received HCT with 2-year survival of 58%. In unadjusted models, HCT was associated with reduced risks of mortality in the entire group and most of the subgroups. However, after accounting for covariates associated with increased mortality (age, comorbidity-burden, disease risks, frailty, impaired QOL, depression, and impaired function), the associations between HCT and longer survival disappeared in all groups. While function, social life, performance status, and depressive symptoms were better for those selected for HCT compared to those who were not, these health advantages were lost after receiving HCT. Recipients and non-recipients of HCT similarly ranked and expected cure as main goal of therapy, while physicians expected more cure for the formers. Accounting for health impairments negate survival benefits from HCT for AML, suggesting that the unadjusted observed benefit is due mostly to selection of the healthier candidates. Considering patients' overall expectations of cure but also the QOL burdens of HCT motivate the need for randomized trials to identify the best candidates for HCT.

Hence, the study was aimed to evaluate the effect of instructional guidelines on women's awareness regarding nutrition after breast cancer surgery. Subjects and method: Design: A quasi-experimental research design was used with a pre-and post-test. Setting: the research was conducted at the inpatient and outpatient clinic of Fayoum Oncology Center. Subjects: A purposive sample of 130 women was included. Three tools were used: Tool (I) women' structured interviewing questionnaire, it included two parts: (a) demographic characteristics; (b) women' medical data, Tool (II) women' knowledge regarding nutrition sheet, and Tool (III) women' practice regarding nutrition sheet (pre/post). Results: More than half of women were having a family history of cancer. The majority of women reported that the main source of information regarding nutritional knowledge after breast cancer surgery was doctors. There was a positive correlation (P=0.005) between women's knowledge scores and their practice post-one-month nutritional instructional guidelines implementation. There were marked improvements in women's knowledge and practices regarding nutrition after breast cancer surgery post instructional guidelines implementation (P=0.005). Conclusion: The instructional guidelines implementation regarding nutrition had a positive effect on improving women's knowledge and practices after breast cancer surgery. Recommendations: The instructional guidelines should be conducted regularly for women after breast cancer surgery to discuss and teach them different aspects about nutrition and replication of the current study with a larger sample

Abstract

This paper discusses how much food is wasted in Egypt, and how can we prevent these losses considering wheat crop case study. The main objective is to define and reduce the amount of food losses in Egypt. Official data and a field questionnaire designed to serve the objectives of the analysis showed that the average water losses due to wheat wastage in Egypt (2012–2016) is 6.0 billion m3 from total wheat available for consumption in Egypt. The amount of water losses is considerable. Also, the relationship of the demographic variables of the sample of the study with the presence or absence of loss of wheat bread indicate that there is a statistically significant relation between the demographic variables studied, and the wheat wastage in Egypt. Findings also show that with careful trade and agricultural policy changes, both wasted economic valuable crop and water resources could be better utilized.

Abstract

Summary Background Risk of mortality following surgery in patients across Africa is twice as high as the global average. Most of these deaths occur on hospital wards after the surgery itself. We aimed to assess whether enhanced postoperative surveillance of adult surgical patients at high risk of postoperative morbidity or mortality in Africa could reduce 30-day in-hospital mortality. Methods We did a two-arm, open-label, cluster-randomised trial of hospitals (clusters) across Africa. Hospitals were eligible if they provided surgery with an overnight postoperative admission. Hospitals were randomly assigned through minimisation in recruitment blocks (1:1) to provide patients with either a package of enhanced postoperative surveillance interventions (admitting the patient to higher care ward, increasing the frequency of postoperative nursing observations, assigning the patient to a bed in view of the nursing station, allowing family members to stay in the ward, and placing a postoperative surveillance guide at the bedside) for those at high risk (ie, with African Surgical Outcomes Study Surgical Risk Calculator scores ≥10) and usual care for those at low risk (intervention group), or for all patients to receive usual postoperative care (control group). Health-care providers and participants were not masked, but data assessors were. The primary outcome was 30-day in-hospital mortality of patients at low and high risk, measured at the participant level. All analyses were done as allocated (by cluster) in all patients with available data. This trial is registered with ClinicalTrials.gov , NCT03853824 . Findings Between May 3, 2019, and July 27, 2020, 594 eligible hospitals indicated a desire to participate across 33 African countries; 332 (56%) were able to recruit participants and were included in analyses. We allocated 160 hospitals (13 275 patients) to provide enhanced postoperative surveillance and 172 hospitals (15 617 patients) to provide standard care. The mean age of participants was 37·1 years (SD 15·5) and 20 039 (69·4%) of 28 892 patients were women. 30-day in-hospital mortality occurred in 169 (1·3%) of 12 970 patients with mortality data in the intervention group and in 193 (1·3%) of 15 242 patients with mortality data in the control group (relative risk 0·96, 95% CI 0·69–1·33; p=0·79). 45 (0·2%) of 22 031 patients at low risk and 309 (5·6%) of 5500 patients at high risk died. No harms associated with either intervention were reported. Interpretation This intervention package did not decrease 30-day in-hospital mortality among surgical patients in Africa at high risk of postoperative morbidity or mortality. Further research is needed to develop interventions that prevent death from surgical complications in resource-limited hospitals across Africa. Abstract

Objective: This study aimed to verify the relationships between voice, and behavior in children and adolescents with and without dysphonia based on parents’ reports and perceptual voice analysis by Phoniatricans.Study Design: This is a case control study.Patients and Methods: The study involved 51 of dysphonic school-age children and adolescents and 62 vocally normal controls aged from 6-18 years. Participants were assessed with auditory perceptual voice analysis, clinical laryngoscopic examination (CLE). Parents of all participants completed the Child Behavior Checklist for ages 6–18 years (CBCL)..Results: Children and adolescents with dysphonia scored higher on Internalizing, Externalizing, Anxiety/depression, Withdrawal/depression, Somatic complaints, Social problems, Attention problems, Rule-breaking, and Aggressive behavior. Children and adolescents with vocal problem scored lower on Activities, Social, School, and thought problems. Correlation analysis between dysphonia severity and CBCL scores demonstrated significant positive correlation in the following scales (Internalizing scale r=0.549; P=0.001, externalizing scale (r=0.370; P= 0.001 and total Behavior scales (r= 0.581; P=0.00), school scale score (r= 0.288; P=0.002), and total Competence indicators (r= 0.230; P= 0.014), withdrawal/depression, social problems, rule-breaking, and aggressive behavior .Conclusion: Vocal fold nodules may be a risk for behavior problems due to the higher scores on behavior problems scales in children and adolescents, especially internalization and externalization aspects and significant positive correlation found between dysphonia severity and CBCL scores. Early interventions of these problems are needed to prevent the persistence of such problems, and reduce its negative impact.

Abstract

Background: Numerous options for hialtal hernia repair exist, including primary simple suture of the crura alone, primary repair reinforcement with synthetic mesh, which may cause erosions and dysphagia, or biologic mesh, which is costly and does not reduce long-term recurrence rates. The purpose of the presentwork is to determine the short-term effectiveness of laparoscopic paraesophageal hiatal hernia (PEH) repair with falciform ligament reinforcement. Materials and Methods: This is a prospective study of laparoscopic repair of paraesophageal hiatal herni with a falciform ligament reinforcement. Symptoms severity and frquency were recorded Preoperatively and at 6 months follow-up. patients with neither recurrent hiatal herni nor prior gastric surgery patients were included in the study. Results: Twenty-six individuals with mean age 37.12 (±9.24) years were enrolled, and all participants had postoperative evaluation with an average follow-up of 20.6 ±1.2 months. The mean severity of symptoms reduced from 15.69 ± 2.35 to 10.85 ± 1.69, The mean frequency of symptoms reduced from 23.81 ± 3.62 to 3.69 ± 2.72 and the mean total symptom score decreased from 39.5 ± 4.02to 14.54 ± 3.65, all of them were statistically significant, (p<0.0001). Only one case had recurrences during follow up and were detected on radiological assessment. The case refused reoperation. Summary: Laparoscopic hiatal hernia repair with a falciform ligament reinforcement is a promising approach with acceptable recurrence rate. Abstract

Background:Dietary supplement use received wide attention and interest throughout the world, particularly in Gulf countries, because of advanced economic and industrial growth.Objective:The present study aimed to determine the prevalence and correlates of dietary supplement use among medical students at Jouf University, Saudi Arabia.Methods:For this cross-sectional study, an anonymous self-administered structured questionnaire was distributed among medical students at Jouf University, Saudi Arabia. Out of the 381 medical students, 366 answered the questionnaire with a response rate of 96%. Analysis of data was done using the Statistical Package for Social Science (SPSS) program, version 24. Statistical significance was considered at P values Results:Regarding the prevalence of dietary supplement use, 36.3% (n=133) and 29.5% (n=108) of the respondents were previous and current users of dietary supplements, respectively. Multivitamins and vitamin D were the most prevalent dietary supplements used by the participants. The students reported that the internet was the main source of knowledge concerning dietary supplements. The significant predictors of dietary supplement use in the logistic regression analysis were: age >21 years (odds ratio (OR): 3.26; confidence interval (CI): 1.67-10.53), females sex (odds ratio (OR): 2.23; confidence interval (CI): 2.34-6.84), and being in the third academic year or more (odds ratio (OR): 2.58; confidence interval (CI): 1.82-5.37). The most reported reasons for utilizing dietary supplements were nutritional supplementation and health promotion. Nearly three-quarters of the students agreed that dietary supplementation is good for health and recommended others to use them after doctor recommendation.Conclusion:Dietary supplement use was remarkably high among medical students at Jouf University, Saudi Arabia. Consumption of dietary supplements increased significantly among older students, females, and those in the third academic year or more. This study recommends other studies involving students from non-health domain colleges to detect if there is a difference in the prevalence of dietary supplement use within this population.

Abstract

Background: Acridocarpus orientalis (AO) is a medicinal herb indigenous to tropical and subtropical Africa, Arabian Peninsula, and New Caledonia with reported anti-inflammatory and antioxidant properties. Objective: To determine whether AO has any beneficial effects on diabetes-induced metabolic parameters in rats. Materials and Methods: Diabetes mellitus was induced in male Wistar rats by streptozotocin. Diabetic rats were treated with three doses of AO extract (50, 100, and 200 mg/kg BW) for 30 days. Kidney, liver, and pancreatic tissue samples were processed for histopathology to determine the effect of AO on the cells of these organs. The effect of AO on pancreatic islet cells and serum insulin levels was also examined using immunohistochemistry and enzyme-linked immunosorbent assay techniques, respectively. Results: AO (100 mg/kg BW) caused a marked reduction in blood glucose levels in diabetic rats compared to diabetic control on day 10 of the study. Moreover, AO (200 mg/kg BW) increased the number of insulin-positive cells with a concomitant reduction in the number of glucagon-immunoreactive cells in pancreatic islets. AO (100 mg/kg) also increased the serum level of superoxide dismutase significantly. Although the administration of AO was able to significantly decrease the diabetes-associated increases in serum creatinine and bilirubin levels, it had no effect on blood urea nitrogen, serum aspartate, or alanine aminotransferase levels. Histopathological examination showed that AO has no toxic effect on the structure of the pancreas, liver, and kidney. Conclusion: Our findings showed that AO could alleviate some complications of diabetes mellitus.

Abstract

Background Overdose of paracetamol affects liver cells leading to cell death. This is done through hepatic necrosis, which is characterized by a rupture of the plasma membrane. A variety of investigations has been conducted using herbal extracts to assess liver toxicity induced by paracetamol. Here we examined herbal plant species, Acridocarpus orientalis (A. orientalis), ethanolic extract to study its potential to protect against hepatotoxicity in mice. Serum alanine transaminase (ALT), aspartate transaminase (AST) and reduced glutathione (GSH) enzymes were estimated for all mice groups. A phytochemical screening was also conducted. Results Phytochemical screening showed that A. orientalis contains flavonoids, tannins, carbohydrates and phenolics derivatives. A. orientalis pretreatment showed significant reduction of liver marker enzymes ALT and AST in almost all concentrations. Furthermore, serum GSH levels were significantly increased in A. orientalis pretreated mice groups. In addition, a reduction in liver weights in pretreated mice with A. orientalis showed significant weight loss at dose 250 mg/kg BW (P A. orientalis pretreated mice. Conclusion pretreatment with A. orientalis protects mice against heplatotoxic adverse effects of paracetamol as indicated by reduction of serum ALT and AST enzymes, with elevation of GSH antioxidant protective enzyme, which may have contributed to the main hepatoprotective action of A. orientalis.

Abstract

The characteristics of Ni-Cr porous alloys containing 0.5 wt%Cr up to 12 wt%Cr were determined. The alloys were prepared from a mix of Ni-particles with filamentary shape and Cr-particles with irregular shape which then mixed with 0.5 wt% paraffin wax as a binder. The samples were formed by compaction and then sintered at 1200 °C for 1 h. The microstructure of samples was found to consist of a Ni-Cr solid solution matrix with X-ray energy dispersive analysis of Cr% less than 0.33 wt%. The matrix microstructure which composed of equiaxed grains (20-60 μm) surrounded high Cr content islands which also contained γ phase (Ni2.88 Cr1.22). The apparent density of the alloys was found to decrease with Cr content from 8.32 g/cm3 for samples containing 0.5 wt%Cr to 7.26 g/cm3 for samples containing 12 wt%Cr. Therefore, the estimated porosity was found to increase from 6.4% to 16.6% with increasing Cr content from 0.5 wt% to 12 wt%. The microhardness of the samples varied from 153.6 to 284.2 VHN for the equiaxed grains and from 119.6 to 240.6 VHN for the island areas.

Abstract

Background: Hysterectomy is one of the most common surgeries in women worldwide. It is applied for the treatment of various problems, such as pelvic pain, menstrual problems, tumors, and other related diseases. Objective: The aim of this work was to preservation of ovarian function as long as possible to decrease manifestation of menopause in hysterectomized patients. Patients and Methods: The study was case control study included 58 patients attendant in outpatient clinic of Obstetrics and Gynecology department, Zagazig University Hospital, and Banha Teaching Hospital during the period from April 2017 to October 2018. All patients were scheduled to total abdominal hysterectomy without oophorectomy due to benign uterine disease. Patients was classified into two groups randomly: Group 1: included odd number of patients 29 was subjected to total abdominal hysterectomy with bilateral complete excision of the tubes. Group II: included an even numbers of patients (n =29) for whom the classical approach of hysterectomy was performed. Results: There was no significant difference between groups regarding Operation, time Hospital stay and Bleeding. Also regarding number of antral follicle post operatively. While these are significant differences between Ovary size distributions between studied groups at different times. Conclusions: It could be concluded that salpingostomy with abdominal hysterectomy is a safe and convenient treatment that does not have a deleterious effect on ovarian reserve.

Abstract

Background and Objectives Hepatitis C Virus (HCV) infection is a major global health challenge. Chronic hepatitis C viral infection is associated with wide metabolic disarrangements. HCV interacts with lipid metabolism leading to steatosis, causing wide adipocytokines changes and impairs glucose metabolism leading to increased prevalence of insulin resistance (IR) and type 2 diabetes. Several oral anti-HCV drugs (direct acting antivirals; DAAs) have been developed over the last several years. Now, HCV can be eliminated from the infected host within 12 weeks of DAA combination therapy. As, there is limited data on the effects of DAA therapy on metabolic profiles, lipid profile and adiponectin level. Hence, this study was carried out to evaluate the Impact of Direct Acting Antivirals therapy on metabolic profiles and adiponectin serum level in Egyptian patients with chronic hepatitis C infection. Methods This study was performed on 100 patients who have chronic infection with hepatitis C, patients were divided into two groups, Group I was 50 patients were treated with (Sofosbuvir/Daclatasvir/Ribavirin) for 3 months. Group II was 50 patients were treated with (Ombitasvir, Paritaprevir, Ritonavir/Ribavirin) for 3 months. With follow up changes were occurred in Fasting Lipid profile (total cholesterol, triglyceride, HDL and LDL), metabolic profile (fasting blood sugar, Fasting Insulin, HOMA-IR and HBA1C) and Serum adiponectin level before and after end of treatment. Results One hundred patients met inclusion criteria were reviewed, (56%) were males in group I while (46%) were males in group I, Mean age in group I was 47.4 ± 5.3 years while in group II was 43.2 ± 6.6 years, statistical analysis of data showed significant difference in the lipid profile in group I before and after treatment .As, we found significant reduction in serum triglycerides after treatment (113.2 ± 22.9 mg/dL vs 105.6 ± 23.2 mg/dL, P Conclusion Eradication of chronic hepatitis C viral infection with DAAs may affect the lipid and the metabolic profile of patients after the end of treatment according to the type of DAAs used in the treatment and according to the stage of the liver disease of patients. But not affect the serum Adiponectin level.

Abstract

Background The propeller perforator flap is a variant of perforator flaps in which complete skeletonization of the perforator with venae commitantes is done to transfer the islanded flap on its pedicle with up to 180 degrees of twisting that predispose to its gradual occlusion by time. These flaps have been recently advocated for lower extremity reconstruction. However, reconstruction of the lower extremity especially in the distal third of the leg quite often involves multistage procedures that might necessitate flap re-elevation and little knowledge is available about postoperative patency of these flaps. Aim of the work The aim of this study is to assess the postoperative patency and flow pattern in twisted vascular pedicle of propeller perforator flaps used in lower extremity reconstruction. Materials and methods A Prospective study was conducted on 15 patients 18 years or older of both genders undergoing lower extremity soft tissue reconstruction by propeller perforator flaps rotated from 90 to 180 degrees at Ain Shams University Hospitals and Nasser Institute between 1st February and 31th December 2019. With exclusion of smoking, peripheral vascular diseases and diabetes mellitus, each patient was evaluated preoperatively in regards to age, gender, comorbidities, bacterial contamination or infection, cause of tissue loss. The flap size, source vessel, arc of rotation, donor site closure was analyzed. The flow pattern was evaluated by handheld Doppler device and used as a control for the study. Postoperatively, the same handheld Doppler device was used to localize the vascular pedicle, evaluate its patency and assess the flow pattern at a minimum follow up period of 3 months postoperatively in all patients. Results The mean age of the fifteen patients included was 35.3 years old. Six patients lost tissue was caused by unstable scar, while post-traumatic in the remaining. Preoperatively, four patients had osteomyelitis and four had wound infection. The rest of patients did not report any infection. Eight out of fifteen patients used perforators from peroneal vessel source, while seven patients used posterior tibial artery source. Forty percent of included participants had a 180 degrees arc of rotation, while 26.7% of participants had an arc rotation angle of 120 degrees, and 33.3% had 90 degrees. Five patients had early post-operative congestion that improved by conservative methods without de-rotation of the flap, one patient had skin graft loss over the donor site of the flap, and another patient suffered in addition to skin graft loss distal congestion leading to superficial skin necrosis. Eight patients had no complications. All patients had patent vessels with biphasic flow pattern postoperatively at a minimum of 3 months postoperatively. Conclusion The perforator propeller flaps are safe, reliable procedures and are considered as an ideal option in reconstructing small-medium defects of the middle and distal third of the leg providing similar skin texture with low rate of vascular obstruction. The present study documented patent vascular pedicle of propeller perforator flaps at three months postoperatively.

Abstract

Background: The Surgical staging of endometrial cancer includes a hysterectomy, bilateral salpingo-oophorectomy, and estimation of regional LN involvement, specially the pelvic and aortic lymph node. Some advised that lymphadenectomy explains the risk of recurrence and helps clinicians make the decision to give adjuvant treatment to high-risk patients. While, some suggested that, complete pelvic and para-aortic lymphadenectomy itself is associated with major comorbidities. Sentinel lymphadenectomy has been suggested in the staging process of the diseases. The aim of this study was to compare the role SLN mapping over total lymphadenectomy for detection of disease metastasis in cases of cancer of the endometrium. Method: 30 patients that were diagnosed of having endometrial cancer based on biopsy were included in this study and were divided in to two equal groups and mapping of sentinel lymph nodes was done to 15 patients and all patients had undergone lymphadenectomy. Results: In this study The mean age of patients was 55 years (range 45-74) for group 1 and was 56ys (range 43-75) for group2. Also the increase in weight as BMI, Median (Range) was 35 (26-45) for group 1 and for group2 was 34 (28-50)... And shown in our study there is no significant difference between both groups regarding BMI, age, clinical, family history and preoperative pathological results. we approved that proved that SLN biopsy has relatively equal values like lymphadenectomy in staging of endometrial cancer with sensitivity 80% and specificity 90%. 97. GENETIC ANALYSIS OF WHEAT TOLERANCE TO DROUGHT UNDER DIFFERENT CLIMATIC ZONES.

Abstract

Nine bread wheat (Triticum aestivum L.) genotypes with different characteristics were crossed in a half-diallel model in two different climatic zones in north and south Egypt (Kafrelsheikh and Toshka) during the two successive winter seasons 2017/18, 2018/19. Genotypes and their F1-hybrids were evaluated under standard irrigation and drought stress conditions, to estimate heterosis of certain crosses for drought tolerance, and to pick out crosses useful in breeding programs for drought tolerance. The findings showed that substantial mean genotypes were found for all traits under various irrigation treatments as well as in the combined study. Means of all studied traits were decreased relative to nonstress environments. Highly significant differences were observed for general and specific combining ability for the studied traits under both irrigation treatments and in the combined analysis. The GCA/SCA ratio ranged from 0.31 in Kafrelsheikh and 0.80 in Toshka for No. of spike plant-1 to 0.10 in Kafrelsheikh and 0.70 in Toshka for grain yield/plant, indicating that non-additive gene effects were more important than additive ones in the inheritance of all studied traits. The best general combiners were P3, P4 and P8 for earliness in both locations and P9 under Toshka conditions for grain yield/plant. The cross combination (P5 × P6) showed strong SCA effects for 1000-grain weight and (P4 × P7) days to heading. If crosses showing a high specific combination capability requires only one good general combiner, such combination will release desirable transgressive segregates providing that the additive genetic system present in a good combination and complementary epistatic effects present in the crosses will function in the same direction in order to minimize undesirable plant characteristics and to maximize the characters of the crosses. Therefore, several of the aforementioned crosses may be of primary importance in breeding programs under drought stress conditions. Thus, it may be suggested to carry on selection for water stress tolerance in Toshka to avoid effects of rain water and shallow water table in Kafrelsheikh zone. [ABSTRACT FROM AUTHOR]

Abstract

Introduction: After laparoscopic cholecystectomy (LC), pain is still a significant concern leading to extended hospital stays or readmissions. A standardized strategy is needed to offer effective pain relief postoperatively. The pain in the early postoperative period is mainly due to elimination of intraperitoneal surface tension. The aim of this study is to evaluate the restoration of intraabdominal surface tension and the use of bupivacaine‐soaked tachosil to control parietal abdominal pain at the port sites to optimize postoperative pain management. Patients and methods: Between March 2020 to December 2021, 816 patients undergoing LC were randomized into two groups after exclusion of 12 patients: Group A—interventional contained 402 patients; Group B—control contained 402 patients. Data to be compared were made in terms of operative time, shoulder pain, upper abdominal pain, and number of analgesic doses and hospital stay. Pain intensity was assessed by using the visual analog scale. Results: There was no significant variation in the demographic data between the two groups. There was significant statistical difference between Groups (A) and (B) regarding severity of shoulder pain and port site pain and number of analgesic doses and hospital stay in favor of Group (A). The results were evaluated within 95% confidence intervals and significance was determined as P <.05. Conclusion: The restoration of intraabdominal surface tension by absorbing as much CO2 as possible at the end of laparoscopic cholecystectomy via the epigastric port route, as well as the use of bupivacaine‐soaked tachosil to control parietal abdominal pain at the port sites; both steps significantly improved postoperative pain management, reduced the number of analgesic doses, and decreased the length of hospital stay. Abstract

The aim of this study was to evaluate the antiangiogenic and immunomodulatory potential of sulfated polysaccharides from the marine algae Macrocystis integrifolia characterized by FTIR. The cytotoxicity of sulfated polysaccharides was evaluated using the 3-(4,5-dimethylthiazol-2-yl) -2,5-diphenyltetrazolium bromide (MTT) assay. Antiangiogenic activity was evaluated using the chicken chorioallantoic membrane (CAM) assay. Immunomodulatory activity was determined on macrophage functionality and allergic response. The results showed that sulfated polysaccharides significantly decreased angiogenesis in chicken chorioallantoic membranes (p < 0.05). Likewise, they inhibited in vivo chemotaxis and in vitro phagocytosis, the transcription process of genes that code the enzymes cyclooxygenase-1 (COX-1), cyclooxygenase-2 (COX-2) and nitric oxide synthase-2 (NOS-2) and the nuclear factor kappa-light chain enhancer of activated B cells (NF-κB), showing immunomodulatory properties on the allergic response, as well as an in vivo inhibitory effect in the ovalbumin-induced inflammatory allergy model (OVA) and inhibited lymphocyte proliferation specific to the OVA antigen in immunized mice. Finally, these compounds inhibited the histamine-induced skin reaction in rats, the production of immunoglobulin E (IgE) in mice, and the passive response to skin anaphylaxis in rats. Therefore, the results of this research showed the potential of these compounds to be a promising source for the development of antiangiogenic and immunomodulatory drugs.
