Highlights • Dose distributions for prostate SBRT were comparable between MRI-linac and conventional plans., Background and purpose Recently, intermediate and high-risk prostate cancer patients have been treated in a multicenter phase II trial with extremely hypofractionated prostate radiotherapy (hypo-FLAME trial). The purpose of the current study was to investigate whether a 1.5 T magnetic resonance imaging guided linear accelerator (MRI-linac) could achieve complex dose distributions of a quality similar to conventional linac state-of-the-art prostate treatments. Materials and methods The clinically delivered treatment plans of 20 hypo-FLAME patients (volumetric modulated arc therapy, 10 MV, 5 mm leaf width) were included. Prescribed dose to the prostate was 5 × 7 Gy, with a focal tumor boost up to 5 × 10 Gy. MRI-linac treatment plans (intensity modulated radiotherapy, 7 MV, 7 mm leaf width, fixed collimator angle and 1.5 T magnetic field) were calculated. Dose distributions were compared. Results In both conventional and MRI-linac treatment plans, the V35Gy to the whole prostate was >99% in all patients. Mean dose to the gross tumor volume was 45 Gy for conventional and 44 Gy for MRI-linac plans, respectively. Organ at risk doses were met in the majority of plans, except for a rectal V35Gy constraint, which was exceeded in one patient, by 1 cc, for both modalities. The bladder V32Gy and V28Gy constraints were exceeded in two and one patient respectively, for both modalities. Conclusion Planning of stereotactic radiotherapy with focal ablative boosting in prostate cancer on a high field MRI-linac is feasible with the current MRI-linac properties, without deterioration of plan quality compared to conventional treatments. more...

Stereotactic body radiotherapy (SBRT) for prostate cancer (PCa) is gaining interest by the recent publication of the first phase III trials on prostate SBRT and the promising results of many other phase II trials. Before long term results became available, the major concern for implementing SBRT in PCa in daily clinical practice was the potential risk of late genitourinary (GU) and gastrointestinal (GI) toxicity. A number of recently published trials, including late outcome and toxicity data, contributed to the growing evidence for implementation of SBRT for PCa in daily clinical practice. However, there exists substantial variability in delivering SBRT for PCa. The aim of this topical review is to present a number of prospective trials and retrospective analyses of SBRT in the treatment of PCa. We focus on the treatment strategies and techniques used in these trials. In addition, recent literature on a simultaneous integrated boost to the tumor lesion, which could create an additional value in the SBRT treatment of PCa, was described. Translation is an energy-intensive process and tightly regulated. Generally, translation is initiated in a cap-dependent manner. Under stress conditions, typically found within the tumor microenvironment in association with e.g. nutrient deprivation or hypoxia, cap-dependent translation decreases, and alternative modes of translation initiation become more important. Specifically, internal ribosome entry sites (IRES) facilitate translation of specific mRNAs under otherwise translation-inhibitory conditions. This mechanism is controlled by IRES trans-acting factors (ITAF), i.e. by RNA-binding proteins, which interact with and determine the activity of selected IRESs. We aimed at characterizing the translational regulation of the IL-33 decoy receptor sST2, which was enhanced by fibroblast growth factor 2 (FGF2). We identified and verified an IRES within the 5'UTR of sST2. Furthermore, we found that MEK/ERK signaling contributes to FGF2-induced, sST2-IRES activation and translation. Determination of the sST2-5'UTR structure by in-line probing followed by deletion analyses identified 23 nucleotides within the sST2-5'UTR to be required for optimal IRES activity. Finally, we show that the RNA-binding protein heterogeneous ribonucleoprotein A1 (hnRNP A1) binds to the sST2-5'UTR, acts as an ITAF, and thus controls the activity of the sST2-IRES and consequently sST2 translation. Specifically, FGF2 enhances nuclear-cytoplasmic translocation of hnRNP A1, which requires intact MEK/ERK activity. Background & Aim Traumatic events leading to focal lesions in articular cartilage are an important cause of joint functionality deterioration, leading to physical inactivity and osteoarthritis. Although restorative and regenerative strategies utilizing different scaffolds seeded with chondrocytes or progenitor cells have been tested with partial success. Their indication is limited for small lesions and their mechanical insufficiency of the fibrocartilage can result in their degeneration overtime, requiring patients to be re-operated within two years. Treatment modalities that favor the application of rapidly-crosslinking biopolymeric solutions to reconstruct cartilage lesions via an arthroscopic approach are highly desirable to reduce risk, surgery time and post-operative recovery times. The desired features of the alternative materials for lesion restoration should match the mechanical properties of cartilage, mimic its mechanical behavior and exhibit satisfactory chondro-inductive capacity. Methods, Results & Conclusion In this work, a new methacrylamide-functionalized gelatin extracted from a cold-adapted species was evaluated as the principal biopolymer for a photo-curable and injectable formulation for cartilage restoration. Unlike using mammalian gelatin, the cold-adapted nature allowed the formulation of highly concentrated biopolymeric solutions (>35 % W/V), while maintaining arthroscopic extrudability and avoiding sol-gel transition at room temperature. The dense and interconnected network of biopolymers after photo-crosslinking showed similar stiffness to human cartilage (∼ 1 MPa), whereas the chondrocyte-laden biopolymeric network was capable to induce formation of cartilage tissue in vitro, presenting similar values to human cartilage in glycosaminoglycan (GAG) (∼ 2 µg/mg), mRNA collagen(Col)2A/Col1A mRNA ratio (∼7) and similar distribution of GAG and Col staining across the scaffold. The development of new application fields of microelectromechanical systems (MEMS) devices often implies a high degree of complexity and the integration of several functionalities within these devices. In this paper, thermoelectric planar thin-film microcoolers become more and more important due to their cooling and temperature stabilization ability, respectively, in MEMS devices. This paper reports on the investigation of the design, manufacturing, and characterization of a membrane-integrated thin-film thermoelectric cooling arrangement for active temperature control and precise local cooling of the sensitive region in a thin-film dew-point sensor. The sensor concept is based on the combination of a thermal sensor heater and a planar thin-film Peltier cooler, which are all arranged on a freestanding and thermally insulated membrane. To obtain a high performance concerning the maximum temperature decrease of the active-cooled membrane, a highly efficient thermoelectric materials combination of Sb and Bi0.87Sb0.13 was used for the fabrication of the in-plane Peltier configuration. For the first sensor setup, a temperature decrease of 10.6 K was achieved under atmospheric conditions at 293 K. Reactive oxygen species (ROS) can cause cellular damage and are thought to promote cancer-development. Nevertheless, under physiological conditions, all cells constantly produce ROS, either as chemical by-products or for signaling purpose. During differentiation cells induce the NADPH oxidase Nox4, which constitutively produces low amounts of H2O2. We infer that this constitutive H2O2 is unlikely to be carcinogenic and may rather maintain basal activity of cellular surveillance systems.Utilizing two different murine tumor models we demonstrate that Nox4 prevents malignant transformation and facilitated the recognition of DNA-damage. Upon DNA-damage repair is initiated as consequence of phosphorylation of H2AX (γH2AX). Repair only occurs if nuclear activity of the γH2AX-dephosphorylating phosphatase PP2A is kept sufficiently low, a task fulfilled by Nox4: Nox4 continuously oxidizes AKT, and once oxidized AKT captures PP2A in the cytosol. Absence of Nox4 facilitates nuclear PP2A translocation and dephosphorylation of γH2AX. Simultaneously the proportion of active, phosphorylated AKT is increased. Thus, DNA-damage is not recognized and the increase in AKT activity promotes proliferation. The combination of both events resulted in genomic instability and tumor initiation.With the identification of the first cancer-protective source of reactive oxygen species, Nox4, the paradigm of reactive-oxygen species-induced initiation of malignancies should be revised.SignificanceThe stereotype of ROS produced by NADPH oxidases as cause of malignant diseases persists generalized since decades. Deregulation of the phosphatidylinositol 3-kinase (PI3K)-Akt-mammalian target of rapamycin (mTOR)-70kDa ribosomal protein S6 kinase 1 (p70(S6K)) pathway is commonly observed in many tumors. This pathway controls proliferation, survival, and translation, and its overactivation is associated with poor prognosis for tumor-associated survival. Current efforts focus on the development of novel inhibitors of this pathway. In a cell-based high-throughput screening assay of 15,272 pure natural compounds, we identified pomiferin triacetate as a potent stabilizer of the tumor suppressor programmed cell death 4 (Pdcd4). Mechanistically, pomiferin triacetate appeared as a general inhibitor of the PI3K-Akt-mTOR-p70(S6K) cascade. Interference with this pathway occurred downstream of Akt but upstream of p70(S6K). Specifically, mTOR kinase emerged as the molecular target of pomiferin triacetate, with similar activities against mTOR complexes 1 and 2. In an in vitro mTOR kinase assay pomiferin triacetate dose-dependently inhibited mTOR with an IC50 of 6.2 μM. Molecular docking studies supported the interaction of the inhibitor with the catalytic site of mTOR. Importantly, pomiferin triacetate appeared to be highly selective for mTOR compared to a panel of 17 lipid and 50 protein kinases tested. As a consequence of the mTOR inhibition, pomiferin triacetate efficiently attenuated translation. In summary, pomiferin triacetate emerged as a novel and highly specific mTOR inhibitor with strong translation inhibitory effects. We aimed to develop national reference values for birth weight, length, head circumference, and weight for length for newborn triplets based on data from the German perinatal survey of 2007-2011.Perinatal survey data of 3,690 newborn triplets from all the states of Germany were kindly provided to us by the AQUA Institute in Göttingen, Germany. Data of 3,567 newborn triplets were included in the analyses. Sex-specific percentile values were calculated using cumulative frequencies. Percentile values at birth were computed for the 3rd, 10th, 25th, 50th, 75th, 90th, and 97th percentiles for 21-36 completed weeks of gestation.We present the first German reference values (tables and curves) for the anthropometric dimensions of triplet neonates and compare selected birth weight and length percentiles of triplets (after 32 and 34 completed weeks of gestation) to those of singletons and twins. The differences in the 50th birth weight percentiles between singletons and triplets after 32 completed weeks of gestation were 180 g for girls and 210 g for boys; after 34 weeks of gestation the differences were 320 and 325 g, respectively. The differences between twins and triplets after 32 weeks of gestation were 100 g for girls and 120 g for boys; after 34 weeks of gestation they were 130 and 135 g, respectively. GILZ (glucocorticoid-induced leucine zipper) is inducible by glucocorticoids and plays a key role in their mode of action. GILZ attenuates inflammation mainly by inhibition of NF-κB and mitogen-activated protein kinase activation but does not seem to be involved in the severe side effects observed after glucocorticoid treatment. Therefore, GILZ might be a promising target for new therapeutic approaches. The present work focuses on the natural product curcumin, which has previously been reported to inhibit NF-κB. GILZ was inducible by curcumin in macrophage cell lines, primary human monocyte-derived macrophages, and murine bone marrow-derived macrophages. The up-regulation of GILZ was neither associated with glucocorticoid receptor activation nor with transcriptional induction or mRNA or protein stabilization but was a result of enhanced translation. Because the GILZ 3'-UTR contains AU-rich elements (AREs), we analyzed the role of the mRNA-binding protein HuR, which has been shown to promote the translation of ARE-containing mRNAs. Our results suggest that curcumin treatment induces HuR expression. An RNA immunoprecipitation assay confirmed that HuR can bind GILZ mRNA. In accordance, HuR overexpression led to increased GILZ protein levels but had no effect on GILZ mRNA expression. Our data employing siRNA in LPS-activated RAW264.7 macrophages show that curcumin facilitates its anti-inflammatory action by induction of GILZ in macrophages. Experiments with LPS-activated bone marrow-derived macrophages from wild-type and GILZ knock-out mice demonstrated that curcumin inhibits the activity of inflammatory regulators, such as NF-κB or ERK, and subsequent TNF-α production via GILZ. The processing of polymers necessitates the use of corrosion and wear resistant tool materials being in direct contact with the feedstock material. Corrosion resistant cold work tool steels, the so called plastic mold steels, are successfully applied here, offering both a good wear and corrosion resistance. The lifetime of this tool depends on the applied heat treatment but also the processing route has a distinct effect on the resulting properties. In this work, different powder metallurgical routes like hot isostatic pressing, build-up welding (plasma transfer arc (PTA)) and thermal spraying (high velocity oxy fuel (HVOF) and atmospheric plasma spraying (APS)) were applied to produce coatings on low-alloyed construction steel. Coatings are compared in relation to the changes in microstructure and the feasibility of an adequate heat treatment. This paper discusses strategies to maximize wear resistance in dependence of heat treatment and the microstructural changes arising from the processing. Die Verarbeitung von Polymeren erfordert den Einsatz von korrosions- und verschleisbestandigen Werkzeugstahlen. Korrosionsbestandige Kaltarbeitsstahle, die sogenannten Kunststoffformenstahle, werden in diesem Bereich erfolgreich eingesetzt, da diese sowohl eine gute Korrosions-, als auch eine gute Verschleisbestandigkeit bieten. Die Standzeit dieser Stahle hangt zum einen von der durchgefuhrten Warmebehandlung ab. Zum anderen ubt aber auch der Herstellungsprozess einen masgeblichen Einfluss auf die resultierenden Eigenschaften aus. In dieser Arbeit wurden die pulvermetallurgischen Fertigungsrouten Heis-Isostatisches-Pressen, PPA-Schweisen und das thermische Spritzen (HVOF, APS) verwendet, um eine Beschichtung auf einem niedrig legierten Baustahl zu erzeugen. Die Beschichtungen wurden dabei in Hinblick auf die Anderungen der Mikrostruktur durch die Herstellung sowie der Warmebehandlung untersucht. Die idiopathische periphere Fazialisparese in der Schwangerschaft oder postpartal ist ein seltenes Krankheitsbild. Die Angaben zu Haufigkeit, Prognose und Risikofaktoren sind widerspruchlich. Wir fuhrten eine retrospektive Analyse von schwangeren Patientinnen oder Wochnerinnen mit peripherer Fazialisparese durch, die in der Frauen- und HNO-Klinik behandelten wurden. Untersucht wurde der Zeitpunkt des Auftretens, der Schweregrad, die Therapie und das Ergebnis im Zeitraum von 1999 bis 2010. Im Studienzeitraum wurden insgesamt 9 Patientinnen aufgrund einer peripheren Fazialisparese behandelt. Die Inzidenz der schwangerschaftsassoziierten Fazialisparese lag in unserem Kollektiv bei 56 auf 100.000 Geburten. In einem Fall bestand eine Praeklampsie. Die Therapie erfolgte mit Kortikosteroiden und bis 2005 mit Pentoxifyllin. Alle Paresen heilten zufriedenstellend (House-Brackman 1–2) aus. Die Haufigkeit der mit einer Schwangerschaft assoziierten oder postpartalen idiopathischen peripheren Fazialisparese erscheint im Vergleich mit der internationalen Literatur im untersuchten Krankengut nicht erhoht. Eine fruhzeitige Therapie mit Kortikosteroiden in Absprache mit den behandelnden Frauenarzten ist auch bei schwangeren oder postpartalen Patientinnen indiziert, um ein optimales Therapieergebnis zu sichern. more...

Background Carpal tunnel syndrome is a commonly encountered entrapment disorder resulting from mechanical insult to the median nerve. Magnetic resonance imaging (MRI)-based investigations have documented typical locations of the median nerve within the carpal tunnel; however, it is unclear whether those locations are consistent within an individual on different days. Methods To determine the day-to-day variability of nerve location, 3.0 T MRI scans were acquired from six normal volunteers over multiple sessions on three different days. Half of the scans were acquired with the wrist in neutral flexion and the fingers extended, and the other half were acquired with the wrist in 35° of flexion and the fingers flexed. Prior to half of the scans (in both poses), subjects performed a preconditioning routine consisting of specified hand activities and several repetitions of wrist flexion/extension. The shape, orientation, location, and location radius of variability of the median nerve and three selected flexor tendons were determined for each subject and compared between days. Findings Two of the six subjects had substantial variability in nerve location when the wrist was in neutral, and four of the subjects had high variability in nerve position when the wrist was flexed. Nerve variability was typically larger than tendon variability. The preconditioning routine did not decrease nerve or tendon location variability in either the neutral or the flexed wrist positions. Interpretation The high mobility and potential for large variability in median nerve location within the carpal tunnel needs to be borne in mind when interpreting MR images of nerve location. more...

Both the median nerve and its surrounding soft tissue structures are well visualized on magnetic resonance (MR) images of the wrist and hand. Addressing nerve damage from impingement of flexor digitorum tendons co-occupying the tunnel is attractive, but to date has been restricted by a lack of means for making individual identifications of the respective tendons. In this image analysis work, we have developed a region-growing method to positively identify each individual digital flexor tendon within the carpal tunnel by tracking it from a more distal MR section where the respective tendon identities are unambiguous. Illustratively, the new method was applied to MRI scans from four different subjects in a variety of hand poses. Conventional shape measures yielded less discriminatory information than did evaluations of individual tendon location and arrangement. Le fumage - particulierement le fumage de cigarettes -constitue le moyen le plus courant de consommation du tabac dans le monde entier. Il est generalement admis que le fumage entraine des risques pour la sante. Les produits de combustions et de pyrolyse du tabac generes au cours du fumage sont consideres comme etre responsables des effets nuisibles. Les produits de tabac sans fumee (a mâcher ou a priser), ce qui constitue un autre moyen tres repandu de la consommation du tabac, ne sont ni brules ni pyrolyses. Par consequent, il existe un debat de plus en plus intense sur le role potentiel que jouent des produits de tabac sans fumee pour la reduction des effets nuisibles de la consommation du tabac. Un bref apercu est donne sur les differents types de tabacs sans fumee consommes dans le monde entier. Les produits disponibles sur le marche sont fortement differents dans la composition et les modes d'utilisation. Les produits sans fumee disponibles en Europe et l'Amerique du Nord sont clairement a distingues des produits vendus en Asie, en Afrique et en Amerique du sud. Les tabacs sans fumee consommes en Europe et en Amerique du Nord sont examines ici concernant leur composition chimique et leurs effets toxicologiques. Les composants d'interet particulier, present dans le tabac sans fumee, particulierement les N-nitrosamines specifiques du tabac (TSNA) sont examines plus en detail. Un resume sur l'evidence epidemiologique des effets sanitaires allegues du tabac sans fumee est presente. Un apercu de la litterature est donne et examine concernant les maladies de la bouche non neoplastiques, les differentes formes du cancer, les maladies du systeme circulatoire, plusieurs autres maladies et la grossesse. Alors que beaucoup des etudes epidemiologiques souffrent de faiblesses et de donnees contradictoires, il semble etre evident que la consommation du tabac sans fumee est moins nuisible a la sante que le fumage. En effet, pour les marques commerciales modernes europeennes de tabac a sucer, comme le tabac a sucer suedois, soumis a des normes strictes de qualite, des donnees suggerent que les risques sanitaires soient, s'il y en a, tres reduits. Les discussions en cours traitent de l'influence potentielle du tabac sans fumee sur le comportement de fumage (commencement ou cessation) et des maladies specifiques du tabac, la Suede servant comme un exemple. 79. The Memory Screening Outreach Program: Findings from a Large Community-Based Sample of Middle-Aged and Older Adults

Few papers have been published concerning the efficacy of community-based memory and dementia screening programs. This article examines the descriptive, clinical, and outcome characteristics of participants attending the Memory Screening Outreach Program (MSOP). The program provided free screenings of participants' short-term memory processes, neurocognitive complaints, and depressive and psychiatric symptomatology. Screening results were subsequently forwarded to participants and their designated healthcare providers (HCPs), and approximately 8 weeks later, participants who received follow-up recommendations were mailed a survey assessing screening-related outcomes. One thousand community-based persons aged 44 to 91 participated in the program at 16 screening sites. Results indicated that 44.3% of the MSOP participants received follow-up recommendations secondary to age-inappropriate memory impairments, depressive or psychiatric symptomatology, significant neurocognitive complaints, need for respite, or a combination thereof. Memory impairments and depressive or psychiatric symptomatology contributed, at least in part, to the recommendations of 24.0% and 30.9% of the sample, respectively. The prevalence of impaired Wechsler Memory Scale-III subtest performances ranged from 20.5% to 5.4%, and 28.7% of the participants exhibited elevated depression inventory scores. Of the participants who returned follow-up surveys, 49.5% visited their primary HCPs regarding their screening results; of the participants who had not followed up, 36.8% indicated plans to do so. Of the participants who followed up, contributing conditions and diagnoses were identified in 50.9%, medication or supplement recommendations or regimen changes were provided to 47.1%, and 17.0% were referred to healthcare specialists. Overall, the MSOP proved efficacious in identifying age-inappropriate memory impairments, notable depressive and psychiatric symptomatology, and significant neurocognitive complaints in an appreciable portion of a large, community-based, sample of middle-aged and older adults, which, in turn, contributed to the identification and treatment of a diversity of conditions at follow-up. more...

Physicists have argued that periodic orbit bunching leads to universal spectral fluctuations for chaotic quantum systems. To establish a more detailed mathematical understanding of this fact, it is first necessary to look more closely at the classical side of the problem and determine orbit pairs consisting of orbits which have similar actions. In this paper we specialize to the geodesic flow on compact factors of the hyperbolic plane as a classical chaotic system. We prove the existence of a periodic partner orbit for a given periodic orbit which has a small-angle self-crossing in configuration space which is a `2-encounter'; such configurations are called `Sieber-Richter pairs' in the physics literature. Aims: To evaluate the performance of bacterial culture of placental swabs for the diagnosis of intrauterine infections in preterm and term infants. Methods: A total of 1438 placentas were examined after preterm and term deliveries and the results were compared with neonatal surface swabs and blood cultures, with respect to the diagnosis of perinatal infection. Swabs were inserted between the cotyledones into the space between placenta and amnion, or obtained from the membranes after separation of the amnion from the chorion. Results: Sixty-one of 1438 (4.2%) cultures of placental swabs were positive including 21 Bacteroides species, 13 Eschericia coli (E. coli), 11 group B streptococcus (GBS), 4 Enterococcus faecalis, and 12 other species. Seventeen of the 61 neonates with positive placental cultures showed clinical signs of infection or sepsis (5 E. coli, 6 GBS, 2 Enterococcus faecalis, 1 Klebsiella pneumoniae, 1 Staphylococcus aureus, 1 Candida albicans, 1 Gardnerella vaginalis), but only 4 of these neonates had a positive blood culture. 123 of 1438 (8.6%) placental swabs were contaminated. Sensitivity and positive predictive value of placental swabs for the diagnosis of neonatal infection were 62.5% and 38.5% for E. coli and 60% and 54.5% for GBS, respectively. Conclusion: Placental swabs appear to be helpful for the retrospective identification of the causative bacteria in case of chorioamnionitis with or without neonatal infection. Fur eine effektive Durchfuhrung besteht aber die Notwendigkeit einer professionellen, interdisziplinaren Kooperation. Ernahrungsteams sollten sich aus diesem Grund aus Arzten, Diatassistenten, Pflegepersonal, Ernahrungswissenschaftlern, Apothekern und auch Psychologen zusammensetzen. Durch die zusatzliche Anwendung von verhaltenstherapeutischen Masnahmen, durch Kommunikations-, Attributions- und Problemlosetraining erhoht sich die Wahrscheinlichkeit einer Verhaltensanderung und somit die Effektivitat der Therapie. Damit kommt es zu einem Ruckgang an Komplikationen, Morbiditat und Mortalitat. Krankenhausaufenthalte konnen verkurzt und Kosten gesenkt werden. Nutritional therapy and consultation is a job involving communication, education, and psychology, whose aims are defined through medicine and nutrition science. For effective implementation, however, professional, interdisciplinary cooperation is required. For this reason „nutrition teams” should be comprised of physicians, dieticians, nursing staff, nutritionists, apothecaries, and also psychologists. Through the additional use of behavioural therapy techniques, such as communication, attribution and problem solving training, the probability of an attitude change and consequently the effectiveness of the therapy, can be increased. Prevention campaigns and new antiretroviral therapies caused a decline in HIV-infections as well as mortality from HIV/AIDS in industrialised countries. Despite this development AIDS is one of the ten mean causes of death worldwide, with 3,1 million deaths in 2002. Hence there is an urgent need for prevention and information campaigns, which ideally should start in early childhood or at school age. It is well known that preventive strategies start at a time when the risk or the possibility of a risky behaviour does not yet exist. A WHO-report in 1993 showed, that sex education often comes too late, as sexual activity of adolescents is already on its way. Persons who are already sexually active can be influenced to have fewer sexual partners and to increase safer sex methods. There is no evidence for causing a higher risk by sex education of adolescents. In Austrian schools numerous experts and organisations cooperate in the sphere of sex education and STD-prevention. School children in Vienna are confronted with this topic by the schools they attend according to the Sex Education-Act of 1970 on a voluntary basis. Various examples of effective teaching models prove intensive efforts and successful interdisciplinary cooperation. This study analyses common risk-reduction-methods and describes the Austrian way of HIV/AIDS/STD prevention in school children, although there is no obligation for sex education in schools, according to the School Teaching-Act of 1986, which leads to better knowledge and subsequent risk behaviour of adolescents in the same geographical area. more...

Recurrent urinary tract infections (RUTI) are very common in women and substantially affect the quality of life of these patients. RUTI is strongly associated with predisposing risk factors occurring especially in elderly women, including functional or anatomical changes such as mucosal atrophy, cystocele, increased post-void residual volume and oliguria. The colonization of the vaginal and periurethral mucosa with virulent, persistent bacteria represents an additional cause for RUTI. The clinical symptoms together with dipstick urine analysis and urine culture lead to the diagnosis. Therapy and prophylaxis of RUTI are based on the elimination of the mentioned risk factors. Long-term or intermittent antibiotic treatment are pertinent therapeutic options. The first choice antibiotic regimen for prophylaxis of RUTI is trimethoprim. Behavioral changes and supportive measures such as acidification of the urine or local hormone therapy also reduce the incidence rates of RUTI. In addition, alternative therapeutic approaches including herbal or bacterial extracts and acupuncture showed promising effects, but need further evaluation. more...

Purpose: Knowledge-based Planning (KBP) founded on prior planning experience and Auto-Planning Engine (APE; commercialized in Pinnacle v9.10 TPS) based on progressive optimization algorithm both aim to eliminate the trial-and-error process in radiotherapy inverse planning. This study investigates the performance of the approaches in a multi-institutional setting to evaluate their functionalities in oropharyngeal cancer and offers suggestions how they can be implemented in the clinic. Methods: Radboud University Medical Center (RUMC) provided 35 oropharyngeal cancer patients (SIB-IMRT with two-dose-level prescription: 68 Gy to PTV68 and 50.3 Gy to PTV50.3) with corresponding comparative APE plans. Johns Hopkins University (JHU) contributed to a three-dose-level (70 Gy 63 Gy and 58.1 Gy) plan library for RUMC's patient KBP generation. MedStar Georgetown University Hospital (MGUH) contributed to a KBP approach employing overlap-volume histogram (OVH-KBP) for generating RUMC's patient KBP plans using JHU's plan library. Since both approaches need their own user-defined parameters as initial inputs the first 10 patients were set aside as training set to finalize them. Meanwhile cross-institutional comparisons and adjustments were implemented for investigating institutions’ protocol discrepancies and the approaches’ user-defined parameters were updated accordingly. The finalized parameters were then applied to the remaining 25 patients for OVH-KBP and APE generation. A Wilcoxon rank-sum test was used for statistical comparison with significance level of p0.2. Conclusions: The study demonstrates that KBP and APE can generate plans of comparable quality in a multi-institutional setting. Variations in clinical protocols can be effectively addressed for cross-institutional adaptations. There are many large-scale problems that require new approaches to computing, such as earth observation, environmental management, biomedicine, industrial and scientific modeling. The CrossGrid project addresses realistic problems in medicine, environmental protection, flood prediction, and physics analysis and is oriented towards specific end-users: Medical doctors, who could obtain new tools to help them to obtain correct diagnoses and to guide them during operations; industries, that could be advised on the best timing for some critical operations involving risk of pollution; flood crisis teams, that could predict the risk of a flood on the basis of historical records and actual hydrological and meteorological data; physicists, who could optimize the analysis of massive volumes of data distributed across countries and continents. Corresponding applications will be based on Grid technology and could be complex and difficult to use: the CrossGrid project aims at developing several tools that will make the Grid more friendly for average users. Portals for specific applications will be designed, that should allow for easy connection to the Grid, create a customized work environment, and provide users with all necessary information to get their job done. more...

A new approximate approach to include metallic loss of planar transmission lines in the finite-difference frequency-domain analysis is presented. From the two-dimensional field behavior in the vicinity and inside of nonideal metallic layers, analytical approximations are obtained and incorporated into the algorithm. This approach leads to considerable savings in computational efforts. The same mesh size as in the lossless case can be used at an acceptable accuracy level and skin depth does not need to be resolved. Furthermore, preprocessing can be automated. The benefits of the new method are demonstrated for typical monolithic microwave integrated circuit coplanar waveguides. more...

We have performed microwave–infrared double resonance measurements on OCS in pure 4He and mixed 4He/3He clusters. In contrast to our previous study on OCS in pure 4He cluster, where an asymmetric inhomogeneous linewidth increasing with rotational quantum number j was observed [S. Grebenev, M. Havenith, F. Madeja, J. P. Toennies, and A. F. Vilesov, J. Chem. Phys. 113, 9060 (2000)], we find the lines in the mixed helium cluster to be much narrower, showing a symmetric, probably homogeneous line width. The spectra indicate a significant difference in the rotational relaxation of OCS in mixed clusters as compared to pure 4He cluster. We have applied the model of Lehmann [K. K. Lehmann, Mol. Phys. 97, 645 (1999)], describing the interaction of a molecule with the helium cluster, and modified it in order to describe particle in a box states of OCS in mixed helium clusters. Alternative translation initiation via internal ribosome entry sites (IRESs) allows for sustained expression of specific proteins under conditions, such as stress, where global translation is inhibited. As cells in tumors are constantly facing stress conditions, e.g. hypoxia or nutrient deprivation, it is not surprising that mRNAs encoding for oncogenes appear to be enriched in IRESs. IRES-dependent translation in eukaryotes commonly involves certain RNA-binding proteins, the so-called IRES trans-acting factors (ITAFs), that can directly interact with the IRESs to facilitate translation initiation. Upstream of N-Ras (UNR) is such an ITAF that can regulate the IRES activity of several cellular and viral IRESs. It is known that its 5' untranslated region (5' UTR) also contains an IRES that is regulated by polypyrimidine-tract binding protein (PTB), heterogeneous nuclear ribonucleoprotein (hnRNP) C1/C2 and UNR itself. Prediction analyses indicated that hnRNPA1 might bind to the 5' UTR of UNR as well. hnRNPA1 was previously already shown to serve as an ITAF in the regulation of the IRES activities of numerous mRNAs including egr2 and sST2. The aims of this project were to determine the impact of hnRNPA1 on the IRES activity of UNR. To test, if hnRNPA1 interacts with the 5' UTR of UNR, as predicted, we employed RNA affinity chromatography. We found that hnRNPA1 indeed strongly bound to the UNR 5' UTR. To assess, if hnRNPA1 consequently affects UNR IRES activity, we overexpressed hnRNPA1. Overexpression of hnRNPA1 markedly increased UNR IRES activity. hnRNPA1 overexpression further induced total UNR protein expression, which indicates that elevated UNR IRES activity results in enhanced translation. With respect to the exact interaction sites, hnRNPA1 binding was predicted to occur at overlapping sites to the previously published PTB binding sites. Deletion of the hnRNPA1 binding sites not only decreased the UNR IRES activity, it further rendered it insensitive to elevated hnRNPA1 levels. Taken together, we found that the 5' UTR of UNR contains specific hnRNPA1 binding sites and that binding of hnRNPA1 at these sites enhances the UNR IRES activity. Our findings therefore add a new piece to the complex puzzle of UNR regulating ITAFs. Having established hnRNPA1 as a new regulator of UNR IRES activity, might further put hnRNPA1 forward as an interesting target to regulate UNR expression. Dietrich Hoffmann passed away on April 20, 2011, at his home in Larchmont, New York. He had suffered from Parkinson's disease for more than 20 years. With Dietrich Hoffmann's death the tobacco community lost one of its most prominent scientists, who was familiar with all areas of tobacco research. His work guided and influenced a whole generation of scientists working in the tobacco industry, universities, regulatory agencies, national governments or international organizations, such as the World Health Organization and the International Agency for Research on Cancer (IARC). It is an obligation of honor for the authors M. Kunze, H. Klus, and the editors of BeiträgezurTabakforschung International publish a short tribute in memory of Dietrich Hoffmann. more...

We report on 2 primigravidae developing pneumomediastinum and subcutaneous emphysema associated with the second stage of labor. Both of the newborns had a comparatively high birth-weight (3,855 g and 4,245 g, respectively). In the first case, the patient felt a sudden chest pain during expulsion followed by dyspnea and swelling of the face. The birth was terminated by vacuum extraction. In the second case, a mild shoulder dystocia occurred, which could be resolved by McRoberts maneuver. The patient exhibited a swollen face and shortness of breath on the first postpartum day. Physical examination revealed crackly skin over the lower face, both sides of the neck and the front of the chest. In both cases, CT scan of the chest revealed pneumomediastinum and a subcutaneous emphysema of the upper thorax, however without showing a pneumothorax. The first patient underwent bronchoscopy and esophagogastroduodenoscopy; there were no abnormalities detected. The pneumomediastinum and subcutaneous emphysema gradually diminished spontaneously in both cases. Appropriate pain management and empirical antibiotics were applied. Pneumomediastinum during labor and birth, typically with subcutaneous emphysema in the face and neck, is a rare condition, but there are reported cases in the literature. Based on the limited available evidence, it seems to have a benign, self-limiting course. A conservative management appears -appropriate and sufficient. more...

We study a problem from nonlinear optics that leads to an integro-differential equation which can be written as the abstract bifurcation problem Au = λLu + ∇φ(u). For such a type of equation, a general theorem on the bifurcation of solutions from the essential spectrum is proved. The hypothesis that φ is non-negative (as is usually assumed) could be omitted and replaced by other conditions that allow us to use the results for the particular example.
