Electromagnetic phenomena due to plasma current disruptions are the cause for the main mechanical operation loads over the ITER equatorial level port plug structures. This paper presents a detailed finite element simulation and analysis of the transient electromagnetic effects of three different plasma current disruption cases over three designs of diagnostic shielding module (DSM) structure. The DSMs are contained into and supported by the generic equatorial port plug (GEPP) analyzed structure. The three plasma disruption cases studied were: major disruption upwards linear decay in 36 ms (MD UP LIN36), vertical displacements events, upwards and downwards linear decay in 36 ms (VDE UP LIN36 and VDE DW LIN36). A detailed analysis for GEPP structure and three DSM-first wall (FW) designs (horizontal and vertical drawers and monoblock) is also presented in order to extract the Eddy current distribution on these devices and thus the resultant electromagnetic forces and moments acting on them.

The port-based ITER diagnostic systems are housed primarily in two locations, the equatorial and upper port plugs. The port plug structure provides confinement function, maintains ultra-high vacuum quality and the first confinement barrier for radioactive materials at the ports. The port plug structure design, from the ITER International Organisation (IO), is cooled and heated by pressurized water which flows through a series of gun-drilled water channels and water pipes. The cooling function is required to remove nuclear heating due to radiation during operation of ITER, while the heating function is intended to heat up uniformly the machine during baking condition. The work presented provides coupled thermo-hydraulic analysis and optimization of a Generic Equatorial Port Plug (GEPP) structure cooling and heating system. The optimization performed includes positioning, minimization of number and size of gun drilled channels, complying with the flow and functional requirements during operating and baking conditions.

This paper presents a detailed finite element simulation of the welds of the prototype Vessel Advanced Technology Segment (VATS) by electron-beam welding. The flexible support housings are reinforcing cylinders of the pressure boundary of the ITER vacuum vessel. They connect inner and outer shells. Eight different simulation sequences were carried out to explain the different mechanisms that drive the distortion process during welding and to lead to an optimum sequence which minimizes the final distortions. The simulations were used to guide the manufacture of the final sequence of the VATS.

In this paper a comparison between the results obtained using multi pass welding finite element (FE) simulation and the contour method was made to evaluate the accuracy in residual stresses simulated for plates with different thicknesses. The contour method has been used to measure the residual stresses in multi pass butt-welded plates. Two 316 austenitic stainless steel multi pass Metal Inert Gas (MIG) butt-welded plates of 10 mm thickness were cut using wire Electric Discharge Machining (EDM). The measurements of the cross-section were made with a coordinate measuring machine (CMM) and the points obtained were used to calculate the residual stresses by mean of static analysis of finite elements. A multi pass welding FE simulation of the two plates was made to obtain the residual stresses after time cooling. The simulated results are generally in good agreement with the experimental measurements. Other plates of 25 mm thickness and the same material were multi pass MIG butt-welded to evaluate the behavior with different thicknesses. In this case the number of passes was 11. The same method was applied to obtain the residual stresses. A comparison between different thicknesses was made. The residual stresses validation will allow the finite element simulation to be used for the later simulation of residual stresses relaxation.

This paper presents a detailed finite element simulation of the longitudinal rib welds of Vessel Advanced Technology Segment (VATS) by e-beam welding. Nine different simulation sequences were carried out to explain the different mechanisms that drive the distortions process during welding and to lead to an optimum sequence which minimizes the final distortions. The simulations were used to guide the manufacture of the final sequence of the VATS. Distortion measurements taken after welding compared very well with the simulated results.

[EN] We show a unified method of proving the existence of C(1)-Frechet smooth and Lipschitz mappings which are surjective or whose range of the derivative contains the whole dual unit ball. The planned in situ maintenance tasks in the ITER port interspace are fundamental to ensure the operation of equipment to control, evaluate and optimize the plasma performance during the entire facility lifetime. They are subject to a limit of shutdown dose rates (SDDR) of 100 µSv h−1 after 106 s of cooling time, which is nowadays a design driver for the port plugs as well as the application of ALARA. Three conceptual shielding proposals outside the ITER ports are studied in this work to support the achievement of this objective. Considered one by one, they offer reductions ranging from 25% to 50%, which are rather significant. This paper shows that, by combining these shields, the SDDR as low as 57Δ µSv h−1 can be achieved with a local approach considering only radiation from one port (no cross-talk form neighboring ports). The locally evaluated SDDR are well below the limit which is an essential pre-requisite for achieving 100µSv h−1 in a global analysis including all contributions. Further studies will have to deal with a realistic port plug design and the cross-talks from neighbour ports.

In this paper, an Electron Beam (EB) welded mock-up is used for the calibration of the finite element method welding simulation developed. Once the method is validated, a new simulation welding sequence is proposed with the objective to reduce the mock-up final distortions. The final distortions should be reduced from 7 mm to 1 mm. Distortion measures and final deformed shapes are compared between two simulation scenarios to establish the effects of the new provisions assumed. The new simulation scenario includes a balanced longitudinal weld sequence in order to minimize the effects of the accumulative angular distortion process. The power of the FE method developed is demonstrated and will allow the optimization of the welding sequences to carry out previous simulation in FEM in future applications of ITER.

The ITER VV Sectors have to be manufactured to extremely tight tolerances, without the production of a prototype to establish the welding distortions. It is thus necessary to develop a (computationally) successful methodology for modelling weld process in order to predict welding distortion and residual stresses. Further, in order to optimise the manufacture sequence and method, the model should be able to rapidly assess the effect of using different sequences. Once the optimal sequence has been decided, then the method has also to be able to accurately predict the final shape.

[EN] Given a Banach space (X,k · k), we study the connection between uniformly convex functions f : X ¿ R bounded above by k · kp , and the existence of norms on X with moduli of convexity of power type. In particular, we show that there exists a uniformly convex function f : X ¿ R bounded above by k · k2 if and only if X admits an equivalent norm with modulus of convexity of power type 2., We thank Professor C. A sequence {e i }i = 1 ∞ in a Banach space X is called a Schauder basis for X if for each x ∈ X there is a unique sequence of scalars {α i }i = 1 ∞ such that \(x =\sum _{ i=1}^{\infty }\alpha _{i}e_{i}\). If the convergence of this series is unconditional for all x ∈ X (i.e., any rearrangement of it converges), we say that the Schauder basis is unconditional . This is equivalent to say that under any permutation \(\pi: \mathbb{N} \rightarrow \mathbb{N}\), the sequence {eπ(i)}i = 1 ∞ is again a basis of X.

The following is the definition of a type of Schauder basis that works also for nonseparable spaces. It is due to P. Enflo and H. P. Rosenthal in [EnRo73].

In this chapter we review some problems on smoothness, rotundity, and its connection to the structure of spaces. We recommend, for example, [BenLin00, DeGoZi93, Fa97, FHHMZ11, HMVZ08], and the recent book [HaJo14] for this area.

A subset K of a Banach space X is said to be a Chebyshev set if every point in X has a unique nearest point in K. In such a case, the mapping that to x ∈ X associates the point in K at minimum distance is called the metric projection.

The aim of the study is to identify the construct and criterion validity of the nursing diagnosis label Sedentary Lifestyle.A cross-sectional study in a nursing consultation in primary health care was conducted. Participants were all people that was attended for one year over 50 who voluntarily wish to participate (n=85) in the study. Objective weekly physical activity was measured in METs with an Accelerometer, objective measure of performance was measured by gait speed EPESE Battery (both measures that were used as the gold standard), and physical activity questionnaires (RAPA), the COOP-WONCA physical fitness chart. Spearman correlation coefficients, mean comparison tests and analysis of sensitivity and specificity were used as statistical analysis.The diagnosis "Sedentary Lifestyle" showed a positive correlation between its manifestations and physical activity measured in METs (r=0.39) and EPESE gait speed (r=0.35). The diagnosis showed a sensitivity of 85.1% and a specificity of 65.2% and showed ability to discriminate active people from those that are not using METs as a measure of physical activity (t=-4.4).The diagnosis "Sedentary Lifestyle" shows criterion and construct validity.El objetivo del estudio es identificar la validez de criterio y constructo de del diagnóstico de enefermería estilo de vida sedentario.Se realizó un estudio transversal en una consulta de enfermería de atención primaria. Participaron todas las personas mayores de 50 años atendidas durante un año que voluntariamente desearon participar (n=85). Se midió la actividad física objetiva en METs semanales mediante acelerómetro, medida objetiva de desempeño mediante test de velocidad de marcha de la batería EPESE (ambas medidas que se utilizaron como criterio), y cuestionarios de actividad física (RAPA), desempeño (COOP-WONCA). Como análisis estadístico se utilizaron coeficientes de correlación de Spearman, pruebas de comparación de medias y análisis de sensibilidad y especificidad.El diagnóstico EVD mostró una correlación positiva en sus manifestaciones con la medida de actividad física en METs (r=0,39)y con la velocidad de marcha EPESE (r=0,35). El diagnóstico mostró una sensibilidad del 85,1% y una especificidad del 65,2% y mostró capacidad para distinguir las personas activas de las que no lo son según la actividad física medida en METs (t=-4,4).El diagnóstico EVD muestra validez de criterio y de constructo.

Development of the port-based diagnostic and service system integration in the Tokamak has been a challenging task for ITER (Nuclear Facility INB-174) engineering. Port integration is demanding more work on ex-vessel to identify electromagnetic thermal and nuclear environment in interspace and port cell regions. Protecting the diagnostic equipment in the interspace & port cell regions poses considerable engineering constraints. This paper will focus on the normal and accidental environmental conditions such as seismic, magnetic field, radiation, pressure, temperature etc., with particular interest on US Domestic Agencies (USDA) based diagnostic systems.

Diagnostics play a very important role in the modern Tokamak where optimum performance is essential. To achieve this, the device must be equipped with reliable and robust sensors and instrumentation that allow the operation envelope to be fully explored. Development of these diagnostics to maintain this reliability is necessary. Further to the development, the systems must be integrated in a way that maintains their performance while simultaneously satisfying the key requirements needed for safety and tokamak operation. ITER will have 50 diagnostics; almost all of which are utilized primarily for the real-time operation of the tokamak. While there is still much work to do, to date, significant progress has been made in the development of these systems. The work load for the developments is shared across all the ITER partners. This paper focuses on the challenges for the integration of the systems.

Eddy current induced electromagnetic (EM) loads during plasma events are the design driver for ITER port plug (PP) structure, diagnostic first wall (DFW), shield module (DSM) and diagnostic system supported by the PP structure. Generic models using commercial software OPERA, MAXWELL and ANSYS are developed and benchmarks are performed for global EM analysis to obtain port-specific design driving EM loads. The 20 degree vessel sector models of the upper and equatorial PP structure take the same ITER TF, CS and PF coil and plasma current (15 MA baseline plasma scenario) as input, then solve for eddy current induced on all passive structural components for various DINA disruption cases. The worst load case can be exponential decay or linear decay of plasma current depending on the dimension and location of diagnostic components inside the PPs. Static and transient magnetic fields from generic models are mapped onto an excel datasheet to establish component design load specification. Three levels of modeling effort are suggested. The global model analysis is used to validate the impact of component design to the PP global system response as well as to study its component dynamic effect as a result of the disruption loads on the PP structure. The local sub-model analysis can be used to extract more accurate EM loads on diagnostics and the excel datasheet of static and transient field maps are used as the initial design load specification for in-port components.

Bonet and Cascales [Non-complete Mackey topologies on Banach spaces, Bulletin of the Australian Mathematical Society, 81, 3 (2010), 409-413], answering a question of M. Kunze and W. Arendt, gave an example of a norming norm-closed subspace N of the dual of a Banach space X such that mu(X, N) is not complete,where mu(X, N) denotes the Mackey topology associated with the dual pair aEuroX, NaEuro parts per thousand. We prove in this note that we can decide on the completeness or incompleteness of topologies of this form in a quite general context, thus providing large classes of counterexamples to the aforesaid question. Moreover, our examples use subspaces N of X* that contain a predual P of X (if exists), showing that the phenomenon of noncompleteness that Kunze and Arendt were looking for is not only relatively common but illustrated by "well-located" subspaces of the dual. PURPOSE: Customized laser surgery attempts to correct higher order aberrations, as well as defocus and astigmatism. The success of such a procedure depends on using a laser beam that is small enough to produce fine ablation profiles needed to correct higher order aberrations. METHODS: Wave aberrations were obtained from a population of 109 normal eyes and 4 keratoconic eyes using a Shack-Hartmann wavefront sensor. We considered a theoretical customized ablation in each eye, performed with beams of 0.5, 1.0, 1.5, and 2.0 mm in diameter. We then calculated the residual aberrations remaining in the eye for the different beam sizes. Retinal image quality was estimated by means of the modulation transfer function (MTF), computed from the residual aberrations. Fourier analysis was used to study spatial filtering of each beam size. RESULTS: The laser beam acts like a spatial filter, smoothing the finest features in the ablation profile. The quality of the correction declines steadily when the beam size increases. A beam of 2 mm is capable of correcting defocus and astigmatism. Beam diameters of 1 mm or less may effectively correct aberrations up to fifth order. CONCLUSION: Large diameter laser beams decrease the ability to correct higher order aberrations. A top-hat laser beam of 1 mm (Gaussian with FWHM of 0.76 mm) is small enough to produce a customized ablation for typical human eyes. [J Refract Surg 2003;19:15-23]

[EN] Morris (1983) proved that every separable Banach space $X$ that contains an isomorphic copy of $c_0$ has an equivalent strictly convex norm such that all points of its unit sphere $S_X$ are unpreserved extreme, i.e., they are no longer extreme points of $B_{X^{**}}$. We use a result of Hájek (1995) to prove that any separable infinite-dimensional polyhedral Banach space has an equivalent $C^{\infty}$-smooth and strictly convex norm with the same property as in Morris' result. All around the ITER vacuum vessel, forty-four ports will provide access to the vacuum vessel for remote handling operations, diagnostic systems, heating, and vacuum systems: 18 upper ports, 17 equatorial ports, and 9 lower ports. Among the lower ports, three of them will be used for the remote handling installation of the ITER divertor. Once the divertor is in place, these ports will host various diagnostic systems mounted in the so-called diagnostic racks. The diagnostic racks must allow the support and cooling of the diagnostics, extraction of the required diagnostic signals, and providing access and maintainability while minimizing the leakage of radiation toward the back of the port where the humans are allowed to enter. A fully integrated inner rack, carrying the near plasma diagnostic components, will be an stainless steel structure, 4.2 m long, with a maximum weight of 10 t. This structure brings water for cooling and baking at maximum temperature of 240 °C and provides connection with gas, vacuum and electric services. Additional racks (placed away from plasma and not requiring cooling) may be required for the support of some particular diagnostic components. The diagnostics racks and its associated ex vessel structures, which are in its conceptual design phase, are being designed to survive the lifetime of ITER of 20 years. This paper presents the current state of development including interfaces, diagnostic integration, operation and maintenance, shielding requirements, remote handling, loads cases and discussion of the main challenges coming from the severe environment and engineering requirements.

A collaborative study was carried out to determine the repeatability (error in each laboratory) and reproducibility (error between the different laboratories) of the fatty acid analysis of Iberian pig fat by gas chromatography. To do this, a comparison was made, in accordance with the ISO 5725-2 procedure, of the weight percentage (%) of the main fatty acids (C16:0, C18:0, C18:1 and C18:2) in 60 samples, determined by six laboratories. The results obtained for the relative standard deviation of the repeatability (0.56, 0.49, 0.29, and 0.69%) and for the relative standard deviation of the reproducibility (3.04, 3.54, 1.78 and 2.86%) show that the accuracy obtained is adequate for this analysis method, and in the same order or even higher than that obtained by other authors in similar samples. The differences between the results obtained on using two extraction methods of liquid fat from subcutaneous adipose tissue were recorded with no significant differences being observed between them.

Tubularized incised plate uretroplasty (TIPU) technique for hypospadias repair or Snodgrass procedure has become increasingly popular since its description in 1994. The elasticity of the neouretra is reduced and several studies have pointed out that flow parameters of the patients operated on this procedure show some grade of asymptomatic functional obstruction, although there are reports on the improvement of these parameters on the mid term. We evaluated the functional outcome in the form of urinary flow in asymptomatic children following uncomplicated TIPU.We reviewed the urine flow rate of asymptomatic toilet trained children who underwent TIPU at our institution between 2005 and 2012. Uroflowmetries were performed in a serial fashion, during the follow up visits at the first months after the repair and yearly afterwards. Unfavourable values were plateau or interrupted curves and peak flow below the 5th percentile of a validated Nomogram for children (Gutiérrez-Segura). Statistical work up: SPSS 15.0.85 patients were eligible. The mean age at surgery was 2.7 years. Median follow up was 29 months (6-82 months). Hypospadias was distal penile in 76 (89.5%) and mid penile in 9 (10.6%). We obtained 131 uroflowmetries (1.54 per patient) at a mean age of 5.1 years (2.5-8). 66.7% of the peak flow values and 55% of the average flow values were below the 5th percentile of the Nomogram. Flow curve was normal (bell- shaped) in 35 charts (26.5%), irregular in 38 (28.8%) and plateau in 56 (42.4%). Peak flow values improved in the second year after surgery (compared to the first year values): 7 ml/sgvs 8.09 ml/sg (p=0.07). The difference was significative in low volumes (100 ml) (6.3 vs 7.8 ml/sg, p=0.04). Bell-shaped curve rate also grew: 21% (6/29) vs 28% (29/74), p0.05. We constructed a specific Nomogram for this population.Our data confirm that many asymptomatic patients who undergo TIPU repair present altered flow patterns. Spontaneous partial improvement is the norm after the first year of surgery.

An extreme point of the closed unit ball of a Banach space is said to be preserved if it is extreme of the closed unit ball of the bidual space; otherwise it is called unpreserved. The beginning of the present work takes the form of a survey on this topic, presenting some elementary facta about those concepts—usually with new proofs—and discussing in particular Katznelson’s solution to a Phelps’ question on preserved extreme points, not available, to our knowledge, in the literature. In a second part, some new results are presented. Since some of them depend on the concept of polyhedrality, we first review several results on this topic. Then we present Godun renorming theorem for the class of nonreflexive Banach spaces, and Morris renorming result—with a new proof—on separable Banach spaces containing a copy of \(c_0\). We show that, under some extra conditions—polyhedrality—a similar renorming, this time adding smoothness, can be defined ensuring strict convexity with all points in the unit sphere unpreserved extreme. We finalize this work by presenting what—to our knowledge—is the first nonseparable result of this kind for the natural class of the weakly compactly generated Banach spaces.

Mesenteric cysts (MC) are benign cystic tumors that grow within mesentery or omentum tissue. We have reviewed the cases of MC reported and operated on in our centre.Retrospective review of clinical records of MC cases during the period 2002-2012 RESULTS: A total of 7 patients were found. Mean age was 5.3 years (range 3-11). Abdominal ultrasound was the diagnostic tool in all cases, except for one, which was diagnosed during laparotomy. All presented abdominal pain, 5 (71.4%) vomiting, 4 (57%) gross abdominal distension, 3 (42.8%) fever, and none presented complete abdominal obstruction, although 2 patients (28.6%) had slight sub-occlusion symptoms. All MC were pedicled or sesil, except for our last case, which extended into the retroperitoneum. All specimens were reported as Limphatic Malformation. None recurred.MC in children are mostly Lymphatic Malformations of mesentery or omentum origin, and clinical presentation varies from chronic abdominal pain to sudden-onset peritonitis or volvulus. About 50-60% require intestinal resection and anastomosis.

Ureteral valves are a rare malformation, with an incidence of 1 in 5000-8000 live births. Urological malformations are associated with 50% of cases according to the current literature. We report 4 cases treated in our hospital from 2004 to 2012. Three of the patients had renal dysgenesis, one case associated with complete urethral duplication, and another case associated with a ureteral atresia.

This paper is devoted to showing that Asplund operators with range in a uniform Banach algebra have the Bishop¿Phelps¿Bollobas property, i.e., they are approximated by norm attaining Asplund operators at the same time that a point where the approximated operator almost attains its norm is approximated by a point at which the approximating operator attains it. To prove this result we use the weak*-to-norm fragmentability of weak*-compact subsets of the dual of Asplund spaces and we need to observe a Urysohn type result producing peak complex-valued functions in uniform algebras that are small outside a given open set and whose image is inside a Stolz region., This research was partially supported by MEC and FEDER projects MTM2008-05396 and MTM2011-25377. We further develop a classical geometric construction of V. Klee and show, typically, that if X is a nonreflexive Banach space with separable dual, then X admits an equivalent norm vertical bar . vertical bar which is Frechet differentiable, locally uniformly rotund, its dual norm vertical bar . vertical bar* is uniformly Gateaux differentiable, the weak* and the norm topologies coincide on the sphere of (X*, vertical bar . vertical bar*) and, yet, vertical bar . vertical bar* is not rotund. This paper describes the changes made to existing version of the Structural Design Criteria for In-vessel Components (SDC-IC) within the ITER project, as a result of the revision and update process carried out recently. Several ITER components, referred to as In-vessel Components, are located inside the ITER Vacuum Vessel: (a) Blanket System: shields the Vessel and Magnets from heat and neutron fluxes; (b) Divertor: extracts heat, helium ash and impurities from the plasma; (c) Fuelling: gas injection system to introduce fuel into the Vacuum Vessel; (d) Ion Cyclotron Heating & Current Drive System: transfers energy to the plasma by electromagnetic radiation; (e) Electron Cyclotron Heating & Current Drive System: uses radio waves to heat to the plasma; (f) Neutral Beam Heating & Current Drive System: accelerates Deuterium particles into the plasma; (g) Lower Hybrid Heating & Current Drive System: drives electric current into the plasma; (h) Diagnostics: measurement systems to control plasma performance, and further understand plasma physics; (i) Test Blankets: demonstrate techniques for ensuring tritium production within the tokamak. ITER In-vessel Components will be subjected to special operating and environmental conditions (neutron radiation, high heat fluxes, electromagnetic forces, etc.). The effects of irradiation on them, including embrittlement, swelling and creep, are not addressed in the existing commercial codes. These conditions are different from conditions in fission reactors and create challenging issues related to the design of these components. For this reason the Structural Design Criteria for ITER In-vessel Components (SDC-IC) [1] was developed for design purposes. SDC-IC was based mainly on the RCC-MR [2] code, and included rules for assessment of effect of neutron irradiation. In 2008 some issues were identified: (1) Some parts had not been fully prepared to cover all needed areas for design; (2) Some important topics needed to be improved; (3) New editions of codes on pressure equipment had been published; (4) No manufacturing rules were included, so consistency between manufacturing rules to be used and design rules in SDC-IC needed to be demonstrated; (5) Compliance with the ESP (French Decree concerning the Pressure Equipment Directive 97/23/EC for non-nuclear pressure vessels) [3] and ESPN (French Order applicable for pressure vessels intended for nuclear facilities) [4] needed to be addressed. The work carried out for Fusion For Energy (European Union’s Joint Undertaking for ITER) is: (a) Modification of design rules, incorporating rules from recently developed codes, and development of specific design rules to cover ITER specific issues and operational conditions; (b) Demonstration of consistency between design rules in SDC-IC and european standards used for manufacturing, in particular EN 13445 [5]; identifying areas where consistency is not provided; (c) Assessment of the compliance with the Essential Safety Requirements of the French Regulations (ESP and ESPN).

Cholelithiasis in children is an infrequent disease whose incidence has increased over the last few decades. It has its own typical characteristics, different from those present in adults. Laparoscopic cholecystectomy has become the treatment of choice for symptomatic cases, still its connection with right-sided colon cancer and other diseases associated with the procedure has led to recommend it with some reservation. Though several authors adopted cholecystotomy as a therapeutic option in recent years, its use remains controversial. Our indication criteria for this technique are presented herewith, as well as long- and medium-term results (mean follow-up of 10 years) of our series, previously reported in 2003, and a new case added later in 2006.Since 2003, a prospective study of 15 patients from our previous series and a new patient added afterwards is being carried out, including: 1) Medical history checking for symptoms connected with biliary conditions. 2) DIAGNOSTIC TESTS: laboratory tests and ultrasonography.Out of our 16 patients: A girl relapsed a few months later and underwent cholecystectomy, but whether it was a true relapse or a missed gallstone was not established. A patient with a history of recurrent abdominal pain has shown no improvement in his symptoms even though his tests are normal. Two patients have been lost for our study, but showed normal tests for 5 and 7 years respectively. Our 11 remaining patients show no symptoms as well as normal laboratory and imaging tests. A new patient joined our study in 2006, showing normal tests to date and no postsurgical complications.Based on our experience, cholecystotomy has proved to be a complication-free, safe and easy procedure with a virtually nonexistent relapse rate in the long and medium term for patients in the pediatric age group presenting with single or double cholelithiasis, unchanged for 1-2 years with no size increase, no apparent etiology, no complications and a normal bile duct.

To be hospitalized is a highly distressing event for children. At present, a resort used in Spain and other countries to reduce children's anxiety in the health context are hospital's clown. We studied the effect of the hospital's clowns about the anxiety in children that going to be operated.We recruited 60 children aged 6 to 10 years scheduled to undergo elective surgery. 30 children would have clowns before the surgery (case group) and 30 would not have them (control group). In the case group, two clowns performed for children. We measured the anxiety with several scales (STAIC, CCPH, faces scale), after the performance and until 7 days after the surgery.The outcomes show both groups a tendency to increase anxiety but the children of the case group showed less increase at the anxiety's score. In the control group is showed that the children are more alterated at seven days from the discharge.Children that receive the clown's care, have tendency to be less distressing and with less fear that another ones, measurement by STAIC and faces scale, and these results are maintained seven days after the discharge.

We report a case of complete urinary tract duplication with true diphallia associated to intestinal and neural anomalies. Complete penile duplication with hypospadias and bifidum scrotum were showed. Moreover, he had got anorrectal disease (anterior anus) and neural tube defects (myelomeningocele). Radiological and functional studies were performed and complete duplication lower urinary tract with coordinate miction were found. Combined surgical approach were used: perineal to remove lateralized and hypospadic penile and abdominal for cystoplasty. We report a case due to the extremely low prevalence. Only 15 cases have been described in the literature.

The primary vesicoureteral reflux (PVUR) is the most common urologic pathology during childhood and affects from 1 to 3% of newborn. It causes acute pyelonephritis (APN) and renal damage in addition to hospital visits with high economic and social costs. Nowadays the endoscopic treatment (ETR) seems to be the most suitable one for the reflux, due to its lower biological cost and its good results. We report our experience in the endoscopic treatment of the primary vesicoureteral reflux (ETR) in Murcia Pediatric Hospital from 1998 to 2007.We present a retrospective study describing 989 patients (1498 ureters) treated by means of ETR. All patients were treated with chemoprophylaxis since the moment of the diagnosis. ETR was fulfilled by the surgeon in an ambulatory way and with Sevofluorano. Deflux, Macroplastique and Coaptite were used. Age, stade, number of ETR, material, chemoprophylaxis, ureterocistoneostomy and ETR costs were evaluated.One-thousand four-hundred and ninty-eight ureteral units, corresponding to 989 patients, (51,2% males), at ages from 4 months to 21-years-old, have been treated by means of ETR. The average age was 4-years-old. Grade III reflux was the most common of all, with 801 ureters affected. Deflux was used in 777 patients, Macroplastique in 203 and Coaptite in 9. No problems in early post-operation happened and the average length was shorter than 2 hours. Forty-eight hours after, 7 patients had to be treated due to APN. Four patients showed late litiasis related to ETR. Two suffered litotomy. The 89.9% of the patients were cured at first ETR. The 11.2% needed a second ETR, with 2.12% of failure (21 patients). Six cured with the third injection and 2 needed a fourth ETR. Thirteen children were treated with ureterocistoneostomy. Radiological correction have not relation with PVUR level and does not show significant differences. Failure regarding the used material was around 9% for Deflux, 11.8% for Macroplastique and 66.6% for Coaptite. The cost of ETR was 1.400 euros, the cost of ureterocistoneostomy 4.822 euros and of the chemoprophylaxis 4.158,7 euros per year.From our experience, ETR is a method as safe as surgery and with less morbility. It is, furthermore, much more predictable than chemoprophylaxis and implies lower economical, biological and social costs than those other methods. In case of failure, ETR does not prevent any other kind of treatment. We regard ETR as a first order therapeutic gesture when dealing with PVUR.

The utility of the nonsurgical correction of congenital auricular deformities by ear molding and splinting has been previously established. Occasionally, its application cannot be easy, and the later collaboration of the parents is necessary. We report a new method of splinting that simplifies the procedure.prospective, case series.Twenty ears in 15 patients between 7 and 60 days of age (average 22 days) were treated. They were 12 prominent ears, 4 Stahl's ears, 2 lop ears, 1 Crinkled ear, and 1 case of increase of antihelix folder. Cotton impregnated with 2-Octyl-Cyanoacrylate is placed as splint for 6 weeks.The bilateral application lasted less than 5 minutes, end there were no spills to the external auditory canal. The splint was given off to the 2 weeks, and a second procedure was necessary in all the cases. There were no dermatitis or skin ulcers. The treatment was successful in 11 cases, partial improvement in 3, poor correction in 4, and recurrence in 2.The splint therapy is an easy nonsurgical method for the treatment of congenital auricular deformities that applied during the first weeks of life provides good aesthetic results in more than 50% of the patients.

In adult patients, it has been reported that prominent ears can be treated by mattress sutures placed through the anterior aspect of the ear, with a minimally invasive technique. In order to verify the utility of this method in children, we used this technique in our patients.Twenty-two prominent ears were treated in 12 patients between 8 and 11 years of ages. All were operated under general anesthesia. The anterior surface of the auricular cartilage is scored with a rasp. The antihelix shape is obtained, and mattress sutures are placed through the anterior surface of the ear.A good aesthetic result was obtained in all the cases. Superficial skin necrosis happened in three. Partial loss of correction in the superior pole was noted in four ears. In 6, some knots were visible through the skin.This repair is a simple technique for the treatment of the prominent ears in children, of good aesthetic results, that it avoids the necessity of dorsal skin resection and produces folds of antihelix more naturally curved. In children, to avoid the exteriorization of the suture material, the knots must be placed in the posterior surface of de auricular cartilage.

Gastrointestinal duplication is an infrequent congenital abnormality. Colonic and tubular duplications are the least common. In neonates there are few described cases. It's difficult to identify the duplication preoperatively and a standard surgical approach has not been developped. We report a neonate with bleeding of the digestive tube who presented a colonic duplication who required multiple diagnostic test before the diagnosis was made for colonoscopy. Laparotomy showed a total tubular duplication of the right, transverse and left colon with proximal and distal communication. Exclusion of the duplicated segment was performed, but three months after the operation, subocclusion due to stenosis of the colonic distal loop was developed. This finding was confirmed by a second colonoscopy and septum was partially removed. The study implies that CD, though uncommon, should be included in the differential diagnosis of rectal bleeding and the importance of the colonoscopy in the diagnosis and treatment of this malformation.

ITER ECE diagnostic [1] needs not only to meet measurement requirements, but also to withstand various loads, such as electromagnetic, mechanical, neutronic and thermal, and to be protected from stray ECH radiation at 170 GHz and other millimeter wave emission, like Collective Thomson scattering which is planned to operate at 60 GHz. Same or similar loads will be applied to other millimetre-wave diagnostics [2], located both in-vessel and in-port plugs. These loads must be taken into account throughout the design phases of the ECE and other microwave diagnostics to ensure their structural integrity and maintainability. The integration of microwave diagnostics with other ITER systems is another challenging activity which is currently ongoing through port integration and in-vessel integration work. Port Integration has to address the maintenance and the safety aspects of diagnostics, too. Engineering solutions which are being developed to support and to operate ITER ECE diagnostic, whilst complying with safety and maintenance requirements, are discussed in this paper.

The use of a correcting element to compensate for higher-order aberrations in an optical system often requires accurate alignment of the correcting element. This is not always possible, as in the case of a contact lens on the eye. We propose a method consisting of partial correction of every aberration term to minimize the average variance of the residual wave-front aberration produced by Gaussian decentrations (translations and rotations). Analytical expressions to estimate the fraction of every aberration term that should be corrected for a given amount of decentration are derived. To demonstrate the application of this method, three examples are used to compare performance with total and with partial correction. The partial correction is more robust and always yields some benefit regardless of the amount of decentration.

Results from experimental animals have suggested that the retina can determine the amount and the sign of defocus at each location in the visual field to control the growth of the underlying sclera.
