Abstract

The frequent association between coronavirus disease 2019 (COVID-19) and olfactory dysfunction is creating an unprecedented demand for a treatment of the olfactory loss. Systemic corticosteroids have been considered as a therapeutic option. However, based on current literature, we call for caution using these treatments in early COVID-19-related olfactory dysfunction because: (1) evidence supporting their usefulness is weak; (2) the rate of spontaneous recovery of COVID-19-related olfactory dysfunction is high; and (3) corticosteroids have well-known potential adverse effects. We encourage randomized placebo-controlled trials investigating the efficacy of systemic steroids in this indication and strongly emphasize to initially consider smell training, which is supported by a robust evidence base and has no known side effects.

Abstract

Respiratory tract viruses are the second most common cause of olfactory dysfunction. As we learn more about the effects of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), with the recognition that olfactory dysfunction is a key symptom of this disease process, there is a greater need than ever for evidence-based management of postinfectious olfactory dysfunction (PIOD). Our aim was to provide an evidence-based practical guide to the management of PIOD (including post-coronavirus 2019 cases) for both primary care practitioners and hospital specialists. A systematic review of the treatment options available for the management of PIOD was performed. The written systematic review was then circulated among the members of the Clinical Olfactory Working Group for their perusal before roundtable expert discussion of the treatment options. The group also undertook a survey to determine their current clinical practice with regard to treatment of PIOD. The search resulted in 467 citations, of which 107 articles were fully reviewed and analyzed for eligibility; 40 citations fulfilled the inclusion criteria, 11 of which were randomized controlled trials. In total, 15 of the articles specifically looked at PIOD whereas the other 25 included other etiologies for olfactory dysfunction. The Clinical Olfactory Working Group members made an overwhelming recommendation for olfactory training; none recommended monocycline antibiotics. The diagnostic role of oral steroids was discussed; some group members were in favor of vitamin A drops. Further research is needed to confirm the place of other therapeutic options.

53. COVID-19: Recovery from Chemosensory Dysfunction. A Multicentre study on Smell and Taste.

Abstract

With the COVID-19 pandemic, chemosensory dysfunction are among the most prevalent symptoms. Most reports are subjective evaluations, which have been suggested to be unreliable. The objective is to test chemosensory dysfunction and recovery based on extensive psychophysical tests in COVID-19 during the course of the disease. Prospective cohort study. A total of 111 patients from four centers participated in the study. All tested positive for SARS-COV-2 with RT-PCR. They were tested within 3 days of diagnosis and 28 to 169 days after infection. Testing included extensive olfactory testing with the Sniffin' Sticks test for threshold, discrimination and identification abilities, and with the Taste Sprays and Taste Strips for gustatory function for quasi-threshold and taste identification abilities. There was a significant difference in olfactory function during and after infection. During infection 21% were anosmic, 49% hyposmic, and 30% normosmic. After infection only 1% were anosmic, 26% hyposmic, and 73% normosmic. For gustatory function, there was a difference for all taste qualities, but significantly in sour, bitter, and total score. Twenty-six percent had gustatory dysfunction during infection and 6.5% had gustatory dysfunction after infection. Combining all tests 22% had combined olfactory and gustatory dysfunction during infection. After infection no patients had combined dysfunction. Chemosensory dysfunction is very common in COVID-19, either as isolated smell or taste dysfunction or a combined dysfunction. Most people regain their chemosensory function within the first 28 days, but a quarter of the patients show persisting dysfunction, which should be referred to specialist smell and taste clinics for rehabilitation of chemosensory function. 3 Laryngoscope, 131:1095-1100, 2021.

Abstract

Adequate nasal breathing is indispensable for athletes, and nasal symptoms have been shown to interfere with their subjective feeling of comfortable breathing and quality of life. Nasal symptoms are caused by either structural abnormalities or mucosal pathology. Structural pathologies are managed differently from mucosal disease, and therefore, adequate diagnosis is of utmost importance in athletes in order to choose the correct treatment option for the individual. Literature suggests that nasal symptoms are more prevalent in athletes compared to the general population and certain sports environments might even trigger the development of symptoms. Given the high demands of respiratory function in athletes, insight into triggering factors is of high importance for disease prevention. Also, it has been suggested that athletes are more neglectful to their symptoms and hence remain undertreated, meaning that special attention should be paid to education of athletes and their caregivers. This review aims at giving an overview of nasal physiology in exercise as well as the possible types of nasal pathology. Additionally, diagnostic and treatment options are discussed and we focus on unmet needs for the management and prevention of these symptoms in athletes within the concept of precision medicine.

Abstract

Invasive fungal sinusitis can lead to dramatic complications in immunocompromised patients and requires prompt diagnosis. Here we report three cases with ophthalmological complications secondary to invasive fungal sinusitis in immunocompromised patients. From an ophthalmological point of view, these cases illustrate different clinical presentations, evolutions, complications, treatments, prognoses, and highlight different pathophysiological mechanisms. Diagnoses were delayed in all cases. In none of the cases did patients recover better vision than counting fingers at 24 months follow up, and two patients died. This case series highlights key points useful for quickly recognising this highly morbid infection in immunocompromised patients.

In a preregistered, cross-sectional study, we investigated whether olfactory loss is a reliable predictor of COVID-19 using a crowdsourced questionnaire in 23 languages to assess symptoms in individuals self-reporting recent respiratory illness. We quantified changes in chemosensory abilities during the course of the respiratory illness using 0–100 visual analog scales (VAS) for participants reporting a positive (C19+; n = 4148) or negative (C19−; n = 546) COVID-19 laboratory test outcome. Logistic regression models identified univariate and multivariate predictors of COVID-19 status and post-COVID-19 olfactory recovery. Both C19+ and C19− groups exhibited smell loss, but it was significantly larger in C19+ participants (mean ± SD, C19+: −82.5 ± 27.2 points; C19−: −59.8 ± 37.7). Smell loss during illness was the best predictor of COVID-19 in both univariate and multivariate models (ROC AUC = 0.72). Additional variables provide negligible model improvement. VAS ratings of smell loss were more predictive than binary chemosensory yes/no-questions or other cardinal symptoms (e.g., fever). Olfactory recovery within 40 days of respiratory symptom onset was reported for ~50% of participants and was best predicted by time since respiratory symptom onset. We find that quantified smell loss is the best predictor of COVID-19 amongst those with symptoms of respiratory illness. To aid clinicians and contact tracers in identifying individuals with a high likelihood of having COVID-19, we propose a novel 0–10 scale to screen for recent olfactory loss, the ODoR-19. We find that numeric ratings ≤2 indicate high odds of symptomatic COVID-19 (4 < OR < 10). Once independently validated, this tool could be deployed when viral lab tests are impractical or unavailable.

Abstract

Le génome humain contient plus de huit cents gènes de récepteurs olfactifs et 25 gènes derécepteurs au goût amer, qui sont exprimés dans de nombreux tissus. Très peu de données existentsur leur présence au niveau des muqueuses nasosinusiennes humaines. Les récepteurs olfactifs ontprobablement des fonctions en dehors de l’olfaction. Il est donc nécessaire d’établir lesquels d’entreeux participent à l’olfaction en les recherchant au sein de la muqueuse olfactive. Par ailleurs, lesrécepteurs au goût amer participent potentiellement à d’autres processus physiologiquesindépendants de la gustation. Au niveau du nez et des sinus, ces récepteurs pourraient êtreimpliqués dans l’immunité innée nasale.La première partie de ce travail s’est intéressée à l’expression des gènes des récepteurs olfactifs dansla muqueuse olfactive humaine. Des prélèvements de muqueuse olfactive entière ont pu êtreréalisés lors de 26 autopsies, puis analysés par les techniques de RT-qPCR. Nous mettons en évidenceque 273 gènes sont exprimés en moyenne. Un groupe de 90 gènes est exprimé chez tous les sujets,un autre groupe de 140 gènes est exprimé chez plus de la moitié des sujets et enfin un derniergroupe de 125 gènes est exprimé chez moins de la moitié des sujets. Cette variabilitéinterindividuelle d’expression des gènes de récepteurs olfactifs pourrait donc intervenir dans lavariabilité des performances olfactives entre individus. Nous avons également réalisé une deuxièmesérie de six prélèvements dans le but de préciser si une différence d’expression existait entre lamuqueuse olfactive antérieure et postérieure. Nos résultats montrent une expression semblableentre ces deux parties.La deuxième partie de ce travail a été consacrée aux récepteurs au goût amer. Abstract

COVID-19 has heterogeneous manifestations, though one of the most common symptoms is a sudden loss of smell (anosmia or hyposmia). We investigated whether olfactory loss is a reliable predictor of COVID-19. Abstract

Recent anecdotal and scientific reports have provided evidence of a link between COVID-19 and chemosensory impairments, such as anosmia. However, these reports have downplayed or failed to distinguish potential effects on taste, ignored chemesthesis, and generally lacked quantitative measurements. Here, we report the development, implementation, and initial results of a multilingual, international questionnaire to assess self-reported quantity and quality of perception in 3 distinct chemosensory modalities (smell, taste, and chemesthesis) before and during COVID-19. In the first 11 days after questionnaire launch, 4039 participants (2913 women, 1118 men, and 8 others, aged 19-79) reported a COVID-19 diagnosis either via laboratory tests or clinical assessment. Importantly, smell, taste, and chemesthetic function were each significantly reduced compared to their status before the disease. Difference scores (maximum possible change ±100) revealed a mean reduction of smell (−79.7 ± 28.7, mean ± standard deviation), taste (−69.0 ± 32.6), and chemesthetic (−37.3 ± 36.2) function during COVID-19. Qualitative changes in olfactory ability (parosmia and phantosmia) were relatively rare and correlated with smell loss. Importantly, perceived nasal obstruction did not account for smell loss. Furthermore, chemosensory impairments were similar between participants in the laboratory test and clinical assessment groups. These results show that COVID-19-associated chemosensory impairment is not limited to smell but also affects taste and chemesthesis. The multimodal impact of COVID-19 and the lack of perceived nasal obstruction suggest that severe acute respiratory syndrome coronavirus strain 2 (SARS-CoV-2) infection may disrupt sensory-neural mechanisms.

Abstract

Recent anecdotal and scientific reports have provided evidence of a link between COVID-19 and chemosensory impairments, such as anosmia. However, these reports have downplayed or failed to distinguish potential effects on taste, ignored chemesthesis, and generally lacked quantitative measurements. Here, we report the development, implementation, and initial results of a multilingual, international questionnaire to assess self-reported quantity and quality of perception in 3 distinct chemosensory modalities (smell, taste, and chemesthesis) before and during COVID-19. In the first 11 days after questionnaire launch, 4039 participants (2913 women, 1118 men, and 8 others, aged 19-79) reported a COVID-19 diagnosis either via laboratory tests or clinical assessment. Importantly, smell, taste, and chemesthetic function were each significantly reduced compared to their status before the disease. Difference scores (maximum possible change ±100) revealed a mean reduction of smell (-79.7 ± 28.7, mean ± standard deviation), taste (-69.0 ± 32.6), and chemesthetic (-37.3 ± 36.2) function during COVID-19. Qualitative changes in olfactory ability (parosmia and phantosmia) were relatively rare and correlated with smell loss. Importantly, perceived nasal obstruction did not account for smell loss. Furthermore, chemosensory impairments were similar between participants in the laboratory test and clinical assessment groups. These results show that COVID-19-associated chemosensory impairment is not limited to smell but also affects taste and chemesthesis. The multimodal impact of COVID-19 and the lack of perceived nasal obstruction suggest that severe acute respiratory syndrome coronavirus strain 2 (SARS-CoV-2) infection may disrupt sensory-neural mechanisms.

Abstract

Objective: Olfactory disorders may be associated with different etiologies, including upper respiratory infections, sinonasal conditions, head injuries, exposure to toxins, and congenital anosmia. This study aimed to evaluate the prevalence of different etiologies for olfactory dysfunction as observed in a sub-Saharan African population. Methods: This descriptive cross-sectional study was conducted in a series of 116 consecutive patients with an olfactory disorder who lived in the city of Bukavu in the South Kivu province of the Democratic Republic of the Congo. The study was conducted from June 1, 2016 to May 30, 2017. We used the Sniffin’ Sticks test, adapting the identification (I) test to our population but retaining the standard threshold (T) and discrimination (D) tests. The patients were classed as anosmic if their composite T + D + I score was <16, hyposmic if it was 16–30, and normosmic if it was >30. Informed consent was obtained in accordance with the Declaration of Helsinki II. We calculated proportions for each olfactory disorder. Results: Median age (minimum–maximum) in our 116 patients was 42.5 (18–83) years. Women made up 60% of our sample. It was observed that 70.7% of patients had anosmia and 29.3% hyposmia. In descending order, the main causes were upper respiratory infections (49.1%), congenital causes (34.5%), nasal polyps (6%), nose and/or sinus surgery (3.4%), head injuries (2.6%), metabolic causes (2.6%), and occupational exposure to toxins (1.7%). Hyposmia predominated when the cause was upper respiratory infection, whereas anosmia did when the cause was congenital and noninfectious. Conclusion: In our study population, upper respiratory infections were the main cause of dysosmia. The anosmic cases we observed tended to be congenital in nature, suggesting that there existed other etiological factors that require further investigation.

Abstract

The current COVID-19 or Sars-CoV-2 pandemic increased awareness of hyposmia or anosmia, as this can be an accompanying symptom. In mild cases, anosmia without rhinorrhea can be the only presenting symptom of this infection. Timely identification can lead to early detection of otherwise asymptomatic carriers. History taking and essential clinical assessment with appropriate protective measures can be performed in patients in whom COVID-19 is suspected. Patients with anosmia without nasal obstruction should be considered COVID-19 suspect and this should initiate testing or self-isolation. As for treatment of hyposmia or anosmia, the authors do not advise treatment with systemic corticosteroids in patients with COVID-19. Based on expert opinion, nasal corticosteroids can be considered, with a preference for spray formulation. Patients who were already using topical or inhalation corticosteroids for proven pre-existing disease (such as asthma and/or allergy) should be advised to continue their maintenance therapy. ENT (Ear Nose Throat) focus on hyposmia and anosmia should be continued, to gain additional knowledge of the disease mechanisms of COVID-19 and improve follow-up, not only on the pneumological aspects but also to evaluate the impact on quality of life of potentially long-term side effects caused by anosmia.

Abstract

Invasive fungal sinusitis can lead to dramatic complications in immunocompromised patients and requires prompt diagnosis. Here we report three cases with ophthalmological complications secondary to invasive fungal sinusitis in immunocompromised patients. From an ophthalmological point of view, these cases illustrate different clinical presentations, evolutions, complications, treatments, prognoses, and highlight different pathophysiological mechanisms. Diagnoses were delayed in all cases. In none of the cases did patients recover better vision than counting fingers at 24 months follow up, and two patients died. This case series highlights key points useful for quickly recognising this highly morbid infection in immunocompromised patients. [ABSTRACT FROM AUTHOR]

The topic of human adult neural plasticity and neurogenesis is of great interest for medical and scientific community, but it is also largely debated. In the last years, an increasing interest has been paid to the olfactory system, and particularly to the plasticity of the olfactory bulb (OB). While the molecular/cellular mechanisms underlying OB plasticity remain a matter of debate, measurements of the OB using magnetic resonance imaging clearly indicate that it is a highly plastic structure. Abstract

Background: The aim of this study was to develop a new psychophysical test to assess intranasal trigeminal chemosensory function. Methodology: The test is similar to the Sniffin’ Sticks test, but using pens impregnated with substances preferentially activating trigeminal afferents. Our test comprises detection threshold, discrimination, identification and lateralization tasks. In a first study, we evaluated healthy controls. In a second study, we evaluated the potential usefulness of this test in patients with rhinological conditions. Results: Study 1: 86 controls were included. Threshold, identification and lateralization performance decreased with age. Test-retest reliability was similar to that of olfactory tests. Study 2: results of the controls group were compared to those of 59 patients (14 allergic rhinitis, 11 chronic rhinosinusitis with nasal polyps (CRSwNP), 9 without nasal polyps (CRSsNP), and 25 with an olfactory disorder (OD)). Controls had 1) lower detection thresholds compared to CRSwNP, CRSsNP and OD, 2) better discrimination and identification scores compared to OD, and 3) better lateralization scores compared to CRSwNP and CRSsNP. Conclusions: Our test allows to identify age-related changes in trigeminal chemosensory function. Trigeminal function seems to be differently affected in different pathologies. Further studies are necessary to validate our results and evaluate the impact of olfactory co-activation on the observed results

Abstract

Background: Olfactory dysfunction is deemed to be a significant contributor to poor quality of life in chronic rhinosinusitis (CRS). Objective: Firstly, to assess the effectiveness of three modalities of corticosteroids administration in patients with CRS. Secondly, to compare modalities between themselves. Subjects and methods: A cohort of 30 patients with CRS (aged from 18 to72 years) were recruited and randomized in three groups depending on the route of corticosteroids administration. Corticosteroids were administrated during 16 days by oral (Medrol®, 32mg/8d - 16mg/4d - 8mg/4d), nasal spray (Rhinocort®, 2 x 2 x 64Fg/nostril) or nebulized (Pulmicort®, 2 x 1mg/4mL) (Sonic nebulizer, AOHBOX NL11SN, DTF Medical) route. Olfactory function was assessed using orthonasal (Sniffing stick test with the threshold-discrimination-identification score)(TDI) and retronasal psychophysical olfactory (odors identification) (RETRO) tests at inclusion and after the treatment. Results: TDI and RETRO were similar between three groups at baseline. TDI improved by 5.5, 5.8 and -1.1 for sonic nebulization, oral and nasal spray groups respectively. This improvement was significantly different between modalities (p = 0.010) and only clinically relevant for oral and nebulized administration. It was similar between oral and nebulized administration but significantly higher than nasal spray administration. RETRO improved by 1.1, 4.2 and 0.7 for sonic nebulization, oral and nasal spray groups respectively (p = 0.231) Conclusion: Effectiveness of nebulized and oral administration is demonstrated on orthonasal olfactory. The benefit is better than with nasal spray.
