Abstract

Tensor effects in shell evolution are studied within the mean-field approach. Particular attention is paid to the analysis of the magic gaps in different regions of the nuclear chart, namely, Z,N=8, 20, and 28. Hartree-Fock calculations with Skyrme and Gogny interactions are performed where the tensor term has a zero and finite range, respectively. Results obtained with and without the tensor component are compared between them and with the experimental data, when available. To complete this analysis, the tensor effect is also investigated within the relativistic Hartree-Fock model, where the exchange of ρ mesons and pions is taken into account. It turns out that the tensor effect in the evolution of the magic gaps can be more easily identified in the cases Z,N=8 and 20, whereas the interpretation of the effect is more complicated for Z or N= 28. Consequently, we indicate the regions defined by the magic numbers 8 and 20 as suitable for fitting the tensor parameters in a mean-field approach: We suggest to include explicitly the data associated to these gap evolutions in the fitting procedures. In general, with the parametrizations used in this work (which have not been fitted on these data), the mean-field results obtained with the tensor contribution do not reproduce the experimental trend, that is, the reduction of the gaps at 8 and 20 that is observed when going toward the drip lines. Since some of the considered nuclei have N=Z, a discussion will be devoted to the interpretation of the experimental data concerning these nuclei and to the Wigner-energy correction.

Abstract

We present a model which describes the properties of odd-even nuclei with one nucleon more, or less, with respect to the magic number. In addition to the effects related to the unpaired nucleon, we consider those produced by the excitation of the closed shell core. By using a single particle basis generated with Hartree-Fock calculations, we describe the polarization of the doubly magic-core with Random Phase Approximation collective wave functions. In every step of the calculation, and for all the nuclei considered, we use the same finite-range nucleon-nucleon interaction. We apply our model to the evaluation of electric quadrupole and magnetic dipole moments of odd-even nuclei around oxygen, calcium, zirconium, tin and lead isotopes. Our Random Phase Approximation description of the polarization of the core improves the agreement with experimental data with respect to the predictions of the independent particle model. We compare our results with those obtained in first-order perturbation theory, with those produced by Hartree-Fock-Bogolioubov calculations and with those generated within the Landau-Migdal theory of finite Fermi systems. The results of our universal, self-consistent, and parameter free approach have the same quality of those obtained with phenomenological approaches where the various terms of the nucleon-nucleon interaction are adapted to reproduce some specific experimental data. Abstract

We study the electromagnetic responses of 4He within the framework of the self-consistent continuum random-phase approximation theory. In this approach, the ground-state properties are described by a Hartree-Fock calculation. The single-particle basis constructed in this manner is used in the calculations of the continuum responses of the system. Finite-range interactions are considered in the calculations. We compare our results with photon-absorption cross sections and electron-scattering quasielastic data. From this comparison, and also from the comparison with the results of microscopic calculations, we deduce that our approach describes well the continuum excitation. [ABSTRACT FROM AUTHOR]

Abstract

Background and aim: The objective of this cross-sectional study was to evaluate the degree of glycaemic control and the frequency of diabetic complications in Italian people with diabetes who were treated with continuous subcutaneous insulin infusion (CSII). Methods and results: Questionnaires investigating the organisation of diabetes care centres, individuals’ clinical and metabolic features and pump technology and its management were sent to adult and paediatric diabetes centres that use CSII for treatment in Italy. Information on standard clinical variables, demographic data and acute and chronic diabetic complications was derived from local clinical management systems. The sample consisted of 6623 people with diabetes, which was obtained from 93 centres. Of them, 98.8% had type 1 diabetes mellitus, 57.2% were female, 64% used a conventional insulin pump and 36% used a sensor-augmented insulin pump. The median glycated haemoglobin (HbA1c) level was 60 mmol/mol (7.6%). The HbA1c target (i.e. 18 years) was achieved in 43.4% of paediatric and 23% of adult participants. Factors such as advanced pump functions, higher rate of sensor use, pregnancy in the year before the study and longer duration of diabetes were associated with lower HbA1c levels. The most common chronic complications occurring in diabetes were retinopathy, microalbuminuria and hypertension. In the year before the study, 5% of participants reported ≥1 episode of severe hypoglycaemic (SH) episodes (SH) and 2.6% reported ≥1 episode of ketoacidosis. Conclusions: Advanced personal skills and use of sensor-based pump are associated with better metabolic control outcomes in Italian people with diabetes who were treated with CSII. The reduction in SH episodes confirms the positive effect of CSII on hypoglycaemia. Clinical trial registration number: NCT 02620917 (ClinicalTrials.gov).

Abstract

We conducted a retrospective survey in pediatric centers belonging to the Italian Society for Pediatric Diabetology and Endocrinology. The following data were collected for all new-onset diabetes patients aged 0–18 years: DKA (pH < 7.30), severe DKA (pH < 7.1), DKA in preschool children, DKA treatment according to ISPAD protocol, type of rehydrating solution used, bicarbonates use, and amount of insulin infused. Records(n=2453)of children with newly diagnosed diabetes were collected from 68/77 centers (87%), 39 of which are tertiary referral centers, the majority of whom (n=1536, 89.4%) were diagnosed in the tertiary referral centers. DKA was observed in 38.5% and severe DKA in 10.3%. Considering preschool children, DKA was observed in 72%, and severe DKA in 16.7%. Cerebral edema following DKA treatment was observed in 5 (0.5%). DKA treatment according to ISPAD guidelines was adopted in 68% of the centers. In the first 2 hours, rehydration was started with normal saline in all centers, but with different amount. Bicarbonate was quite never been used. Insulin was infused starting from third hour at the rate of 0.05–0.1 U/kg/h in 72% of centers. Despite prevention campaign, DKA is still observed in Italian children at onset, with significant variability in DKA treatment, underlying the need to share guidelines among centers.

Abstract

Continuous subcutaneous insulin infusion (CSII) is increasingly being used worldwide, mostly thanks to technical improvements. This study examined the current status of CSII in Italy. Physicians in charge of 272 diabetes centers caring for patients using CSII were sent a questionnaire investigating clinical features, pump technology and management of these patients; a large proportion (217 centers, 79.8%) joined the study. By end-April 2013, data had been collected on 10152 patients treated with CSII; 98.2% had type 1 diabetes, 82.4% were adults, 57% female. Only just over half the centers (59%) managed more than 20 CSII patients each. The distribution of patients varied widely both among and within different regions. The main indication for CSII was the de- sire to improve glycemic control. Dropouts (8.65%) were mainly due to difficulties with pump wearability or non-optimal glycemic control. Among CSII patients 61% used a traditional pump, 39% a sensor augmented pump. Only 68% used the CSII advanced functions and glucose sensors were used twelve days per month on average. Round-the-clock assistance was guaranteed in 81% of centers; a full diabetes team followed patients in only 40% of adult-care centers and 50% of pediatric units. CSII is increasingly used in Italy, by adults and pediatric patients. However, further work is needed to unify treatment strategies throughout the country and to encourage optimal pump use and applications.

Abstract

Aim/hypothesis: To compare the frequency of diabetic ketoacidosis (DKA) at diagnosis of type 1 diabetes in Italy during the COVID-19 pandemic in 2020 with the frequency of DKA during 2017-2019., Methods: Forty-seven pediatric diabetes centers caring for >90% of young people with diabetes in Italy recruited 4,237 newly diagnosed children with type 1 diabetes between 2017 and 2020 in a longitudinal study. Four subperiods in 2020 were defined based on government-imposed containment measures for COVID-19, and the frequencies of DKA and severe DKA compared with the same periods in 2017-2019., Results: Overall, the frequency of DKA increased from 35.7% (95%CI, 33.5-36.9) in 2017-2019 to 39.6% (95%CI, 36.7-42.4) in 2020 (p=0.008), while the frequency of severe DKA increased from 10.4% in 2017-2019 (95%CI, 9.4-11.5) to 14.2% in 2020 (95%CI, 12.3-16.4, p<0.001). DKA and severe DKA increased during the early pandemic period by 10.4% (p=0.004) and 8% (p=0.002), respectively, and the increase continued throughout 2020. Immigrant background increased and high household income decreased the probability of presenting with DKA (OR: 1.55; 95%CI, 1.24-1.94; p<0.001 and OR: 0.60; 95 CI, 0.41-0.88; p=0.010, respectively)., Conclusions/interpretation: There was an increase in the frequency of DKA and severe DKA in children newly diagnosed with type 1 diabetes during the COVID-19 pandemic in 2020, with no apparent association with the severity of COVID-19 infection severity or containment measures. There has been a silent outbreak of DKA in children during the pandemic, and preventive action is required to prevent this phenomenon in the event of further generalized lockdowns or future outbreaks., Competing Interests: No author reported any conflict of interest as regards this study. Abstract

Aims: To assess whether vitamin D levels at birth were associated with risk of having type 1 diabetes up to 10 years of age and the potential modifier effect of ethnic group., Methods: The Piedmont Diabetes Registry and the Newborn Screening Regional data were linked to identify cases (n = 67 incident children aged ≤10 years at diabetes onset, 2002-2012) and up to five controls (n = 236) matched for birthday and ethnic group. Cards with neonatal blood spot were used and 25-hydroxyvitamin D(3) assessed with tandem mass spectroscopy., Results: In conditional logistic regression, OR for unit increment of log vitamin D was 0.78 (95 % CI 0.56-1.10). Vitamin D was significantly lower in migrant than in Italian control newborn babies (p < 0.0001), and interaction between vitamin D and migrant status was statistically significant (p = 0.04). Compared to migrant newborns babies with vitamin D ≥ 2.14 ng/ml, migrants with lower levels had an OR of 14.02 (1.76-111.70), whereas no association was evident in Italians., Conclusions: Our case-control study within the Piedmont Diabetes Registry showed no association between vitamin D levels at birth and risk of having type 1 diabetes up to 10 years of age, apart from the subgroup of migrant babies, which might have clinical implications if confirmed.

Abstract

Mycoplasma pneumoniae is a common intracellular pathogen, which is responsible for infections of the respiratory tract, particularly in patients between 5 and 30 years of age. Nevertheless, there is increasing evidence that Mycoplasma pneumoniae plays a role in determining clinical presentations different from the respiratory ones. Among extra pulmonary complications skin eruptions are more frequent than others, even with severe clinical features such as Stevens Johnson syndrome. It is important to note that dermatological involvement can occur before, during or after the appearance of respiratory symptoms or without them. We report two patients whose onset of symptoms was not a respiratory tract disease, as usual in Mycoplasma pneumoniae infections, but prolonged and high grade fever with a relevant skin involvement pointing out the importance of researching Mycoplasma pneumoniae in the pathogenesis of peculiar clinical features. The first patient is a 4-year-old boy with signs of Stevens Johnson syndrome while the second patient is a 16-year-old girl with red-purple maculae on both legs and arms; in both cases we detected Mycoplasma IgM antibodies as a part of differential diagnosis. We discuss below the immunological mechanism by which Mycoplasma pneumoniae can determine the clinical features shown by our patients.
