Background: People with Huntington's disease (HD) have been observed to have lower rates of cancers. Objective: To investigate the relationship between age of onset of HD, CAG repeat length, and cancer diagnosis. Methods: Data were obtained from the European Huntington's disease network REGISTRY study for 6540 subjects. Population cancer incidence was ascertained from the GLOBOCAN database to obtain standardised incidence ratios of cancers in the REGISTRY subjects. Results: 173/6528 HD REGISTRY subjects had had a cancer diagnosis. The age-standardised incidence rate of all cancers in the REGISTRY HD population was 0.26 (CI 0.22-0.30). Individual cancers showed a lower age-standardised incidence rate compared with the control population with prostate and colorectal cancers showing the lowest rates. There was no effect of CAG length on the likelihood of cancer, but a cancer diagnosis within the last year was associated with a greatly increased rate of HD onset (Hazard Ratio 18.94, p < 0.001). Conclusions: Cancer is less common than expected in the HD population, confirming previous reports. However, this does not appear to be related to CAG length in HTT. A recent diagnosis of cancer increases the risk of HD onset at any age, likely due to increased investigation following a cancer diagnosis.

- Abstract
Patients with schizophrenia can have difficulty recognizing emotion, and the impact of this difficulty on social functioning has been widely reported. However, earlier studies did not thoroughly explore how this deficit may vary according to emotion intensity, or how it may differ among individuals and across cultures. In the present study, our aim was to identify possible deficits in facial emotion recognition across a wide range of emotions of different intensities among patients with schizophrenia from the Democratic Republic of Congo (DRC). Thirty stable patients with schizophrenia and 30 healthy controls matched for age and level of education were evaluated using a validated and integrative facial emotion recognition test (TREF). A total recognition score and an intensity threshold were obtained for each emotion. Patients with schizophrenia had emotion recognition deficits, particularly for negative emotions. These deficits were correlated to the severity of negative symptoms. Patients showed no threshold deficit at the group level, but analysis of individual profiles showed marked heterogeneity across patients for the intensity of the emotion decoding deficit. Our study confirms the existence of deficits in emotion recognition for negative emotions in patients with schizophrenia, generalizes it to DRC patients, and underlines considerable heterogeneity among patients.

Abstract Background This study aimed to document the outcome dimensions that physicians see as important in defining cure from depression. The study also aimed to analyse physicians' attitudes about depression and to find out whether they affect their prescribing practices and/or the outcome dimensions that they view as important in defining cure. Methods A 51-item questionnaire based on six validated scales was used to rate the importance of several depression outcome dimensions. Physicians' attitudes about depression were also assessed using the Depression Attitude Scale. Overall, 369 Belgian physicians (264 general practitioners [GPs]; 105 psychiatrists) participated in the DEsCRIBE™ survey. Results GPs and psychiatrists strongly agreed that functioning and depressive symptomatology were most important in defining cure; anxious and somatic symptomatology was least important. GPs and psychiatrists differed in their attitudes about depression (p <0.001). Logistic regression revealed that the attitudes of GPs - but not psychiatrists - were significantly associated with their rates of antidepressant prescription (p < 0.001) and that certain attitudes predicted which outcome dimensions were seen as important in defining cure. Conclusions Belgian GPs and psychiatrists strongly agreed on which criteria were important in defining cure from depression but differed in their attitudes about depression. The outcome dimensions that were considered important in defining cure were influenced by physicians' attitudes - this was more pronounced in GPs than in psychiatrists.

- Abstract
Background: Psychiatric side effects of mefloquine, an antimalarial drug, are well known (agitation, depression and suicidal ideation, psychosis with paranoia and hallucinations). Case Presentation: We report the case of a 30-year-old man without psychiatric antecedents, who developed acute psychosis and depression with suicidal ideation after five weeks of treatment with mefloquine. The symptomatology decreased rapidly a few days later, under antipsychotic drug (olanzapine) and benzodiazepines (lorazepam) and the treatment doses were diminished after one week. However, three weeks later, the same symptomatology reappeared and the treatment doses of both psychotropic drugs had to be increased once again. The antidepressant escitalopram was also introduced. Three months were necessary to definitively suppress the psychotropic drugs. Plasma mefloquine clearance was monitored by a validated liquid-chromatography massspectrometric method and confirmed the very long halflife of the drug, which is estimated at 14.5 days. Conclusions: The pathophysiological mechanisms of these psychiatric side effects probably involve a toxic limbic encephalopathy. Mefloquine is highly lipophilic and may accumulate in the limbic system. In conclusion, due to the very long half-life of mefloquine, psychiatric symptoms can persist for weeks, necessitating an appropriate psychotropic treatment. Chronic symptoms resulting from permanent neurotoxicity may also occur.

Au départ de l’observation de la prise en charge hospitalière d’une patiente victime de violences sexuelles au Sud-Kivu et souffrant de SSPT, les auteurs tentent de démontrer l’importance d’inclure les spécificités culturelles et socio-politiques ainsi que la nature des violences subies dans la compréhension de la situation. Les patients souffrant de SSPT dans ce type de contexte ne répondant qu’imparfaitement aux traitements de références, il est primordial de réfléchir à une prise en charge adaptée aux spécificités de leur situation. Partant des concepts de déshumanisation et de résilience, les auteurs insistent sur la nécessité de respecter le mode d’expression de la souffrance chez un patient souffrant de SSPT et sur l’importance d’une mise en place d’un réseau d’aide à la résilience à court et long terme.

- Abstract
Objective: Huntington's disease (HD) is characterized by motor and cognitive impairments including memory, executive, and attentional functions. However, because earlier studies relied on multidetermined attentional tasks, uncertainty still abounds regarding the differential deficit across attentional subcomponents. Likewise, the evolution of these deficits during the successive stages of HD remains unclear. The present study simultaneously explored 3 distinct networks of attention (alerting, orienting, executive conflict) in preclinical and clinical HD. Method: Thirty-eight HD patients (18 preclinical) and 38 matched healthy controls completed the attention network test, an integrated and theoretically grounded task assessing the integrity of 3 attentional networks. Results: Preclinical HD was not characterized by any attentional deficit compared to controls. Conversely, clinical HD was associated with a differential deficit across the 3 attentional networks under investigation, showing preserved performance for alerting and orienting networks but massive and specific impairment for the executive conflict network. This indexes an impaired use of executive control to resolve the conflict between task-relevant stimuli and interfering task-irrelevant ones. Conclusion: Clinical HD does not lead to a global attentional deficit but rather to a specific impairment for the executive control of attention. Moreover, the absence of attentional deficits in preclinical HD suggests that these deficits are absent at the initial stages of the disease. In view of their impact on everyday life, attentional deficits should be considered in clinical contexts. Therapeutic programs improving the executive control of attention by neuropsychology and neuromodulation should be promoted

- Abstract
OBJECTIVE: Repetitive thoughts can be divided in two modes: abstract/analytic (decontextualized and dysfunctional) and concrete/experiential (problem-focused and adaptive). They constitute a transdiagnostic process involved in many psychopathological states but have received little attention in schizophrenia, as earlier studies only indexed increased ruminations (related to dysfunctional repetitive thoughts) without jointly exploring both modes. This study explored the two repetitive thinking modes, beyond ruminations, to determine their imbalance in schizophrenia. METHODS: Thirty stabilized patients with schizophrenia and 30 matched controls completed the Repetitive Response Scale and the Mini Cambridge-Exeter Repetitive Thought Scale, both measuring repetitive thinking modes. Complementary measures related to schizophrenic symptomatology, depression and anxiety were also conducted. RESULTS: Compared to controls, patients with schizophrenia presented an imbalance between repetitive thinking modes, with increased abstract/analytic and reduced concrete/experiential thoughts, even after controlling for comorbidities. Schizophrenia is associated with stronger dysfunctional repetitive thoughts (i.e. abstract thinking) and impaired ability to efficiently use repetitive thinking for current problem-solving (i.e. concrete thinking). CONCLUSION: This imbalance confirms the double-faced nature of repetitive thinking modes, whose influence on schizophrenia's symptomatology should be further investigated. The present results also claim for evaluating these processes in clinical settings and for rehabilitating the balance between opposite repetitive thinking modes.

- Abstract
Schizophrenia is associated with a strong deficit in the decoding of emotional facial expression (EFE). Nevertheless, it is still unclear whether this deficit is specific for emotions or due to a more general impairment for any type of facial processing. This study was designed to clarify this issue. Thirty patients suffering from schizophrenia and 30 matched healthy controls performed several tasks evaluating the recognition of both changeable (i.e. eyes orientation and emotions) and stable (i.e. gender, age) facial characteristics. Accuracy and reaction times were recorded. Schizophrenic patients presented a performance deficit (accuracy and reaction times) in the perception of both changeable and stable aspects of faces, without any specific deficit for emotional decoding. Our results demonstrate a generalized face recognition deficit in schizophrenic patients, probably caused by a perceptual deficit in basic visual processing. It seems that the deficit in the decoding of emotional facial expression (EFE) is not a specific deficit of emotion processing, but is at least partly related to a generalized perceptual deficit in lower-level perceptual processing, occurring before the stage of emotion processing, and underlying more complex cognitive dysfunctions. These findings should encourage future investigations to explore the neurophysiologic background of these generalized perceptual deficits, and stimulate a clinical approach focusing on more basic visual processing.

Introduction: Emotional decoding impairments have been largely demonstrated in schizophrenia for facial and prosodic stimuli, when presented separately. Nevertheless, the exploration of crossmodal integration has been far less considered, despite its omnipresence in daily social interactions. Moreover, the role played by basic visuo-motor impairments in unimodal and crossmodal decoding remains unexplored. Methods: Thirty-two patients were compared with 32 matched controls in an emotional decoding task including unimodal (visual and auditory) and crossmodal (congruent and incongruent) conditions. A control perceptive task was also conducted to take potential low-level perceptual deficits into account. Results: Schizoprenic patients presented lower performance and higher reaction times for both unimodal tasks (visual and auditory) and crossmodal conditions. Moreover, reaction times for the visuo-perceptive task were also significantly longer for patients compared to controls. Conclusions: The consistency of the results across unimodal and crossmodal tasks suggests a globalised emotional impairment in schizophrenia, independent of the sensorial modality and crossmodal nature of the stimuli. Centrally, given the results in the visuo-perceptive task, the impairments observed for emotional recognition appears at least partly explained by primary cognitive deficits, namely reduced processing speed.

Au départ de l’observation de la prise en charge hospitalière d’une patiente victime de violences sexuelles au Sud-Kivu et souffrant de SSPT, les auteurs tentent de démontrer l’importance d’inclure les spécificités culturelles et socio-politiques ainsi que la nature des violences subies dans la compréhension de la situation. Les patients souffrant de SSPT dans ce type de contexte ne répondant qu’imparfaitement aux traitements de références, il est primordial de réfléchir à une prise en charge adaptée aux spécificités de leur situation. Partant des concepts de déshumanisation et de résilience, les auteurs insistent sur la nécessité de respecter le mode d’expression de la souffrance chez un patient souffrant de SSPT et sur l’importance d’une mise en place d’un réseau d’aide à la résilience à court et long terme.

Objective: Huntington’s disease (HD) is characterized by motor and cognitive impairments including memory, executive, and attentional functions. However, because earlier studies relied on multidetermined attentional tasks, uncertainty still abounds regarding the differential deficit across attentional subcomponents. Likewise, the evolution of these deficits during the successive stages of HD remains unclear. The present study simultaneously explored 3 distinct networks of attention (alerting, orienting, executive conflict) in preclinical and clinical HD. Method: Thirty-eight HD patients (18 preclinical) and 38 matched healthy controls completed the attention network test, an integrated and theoretically grounded task assessing the integrity of 3 attentional networks. Results: Preclinical HD was not characterized by any attentional deficit compared to controls. Conversely, clinical HD was associated with a differential deficit across the 3 attentional networks under investigation, showing preserved performance for alerting and orienting networks but massive and specific impairment for the executive conflict network. This indexes an impaired use of executive control to resolve the conflict between task-relevant stimuli and interfering task-irrelevant ones. Conclusion: Clinical HD does not lead to a global attentional deficit but rather to a specific impairment for the executive control of attention. Moreover, the absence of attentional deficits in preclinical HD suggests that these deficits are absent at the initial stages of the disease. In view of their impact on everyday life, attentional deficits should be considered in clinical contexts. Therapeutic programs improving the executive control of attention by neuropsychology and neuromodulation should be promoted.

- Abstract
Huntington’s disease (HD) is centrally characterized by motor, neurocognitive and psychiatric symptoms, but impaired emotional decoding abilities have also been reported. However, more complex affective abilities are still to be explored, and particularly empathy, which is essential for social relations and is impaired in various psychiatric conditions. This study evaluates empathic abilities and social skills in pre-clinical and clinical HD, and explores the distinction between two empathy sub-components (emotional-cognitive). Thirty-six HD patients (17 pre-clinical) and 36 matched controls filled in the Empathy Quotient Scale, while controlling for psychopathological comorbidities. At the clinical stage of HD, no global empathy impairment was observed but rather a specific deficit for the cognitive sub-component, while emotional empathy was preserved. A deficit was also observed for social skills. Pre-clinical HD was not associated with any empathy deficit. Emotional deficits in clinical HD are thus not limited to basic emotion decoding but extend towards complex interpersonal abilities. The dissociation between impaired cognitive and preserved emotional empathy in clinical HD reinforces the proposal that empathy subtypes are sustained by distinct processes. Finally, these results underline the extent of distinct affective and social impairments in HD and the need to grasp them in clinical contexts.

Introduction: L'objectif général poursuivi dans cette étude est de décrire le tableau clinique de la dépression post-accident vasculaire cérébral (DPAVC) à Kinshasa. Méthodes: Il s'agit d'une étude transversale descriptive portant sur 56 patients suivis pour hémiplégie post-accident vasculaire cérébral au Centre de réhabilitation pour personnes handicapées de Kinshasa (CRPHK) du 1er au 31 août 2011. Résultats: Au Patient Health Questionnaire (PHQ9), 21. 40 % des patients présentaient une dépression modérée à sévère. A l'échelle de Rankin, 63. 8% des patients étaient capables de marcher sans aide. L'apathie modérée à sévère était présente chez 44. 64%; à l'échelle de sévérité de la fatigue, le score de 12. 7 % des sujets était compatible avec un état dépressif. La dépression était associée à la sévérité des troubles neurologiques, à l'incapacité évaluée avec l'échelle de Rankin, à l'apathie et à une appréciation mauvaise de son propre état de santé par le patient. Conclusion: La DPAVC est fréquente à Kinshasa parmi les patients en réhabilitation. La fréquence observée est comparable à celles retrouvées dans des travaux antérieurs dans le monde et aux rares travaux publiés en Afrique. La DPAVC est associée de manière significative à la sévérité des troubles neurologiques, au degré d'handicap, à la fatigue, à l'apathie et à la mauvaise perception de son état de santé.

Il est bien connu que le viol a un potentiel important d’induction d’état de stress post-traumatique. Un des aspects insuffisamment exploré, est la place de la culture dans la psychopathologie du trauma. Nous montrons dans cet article toute l’importance de prendre en compte des interprétations culturelles dans la clinique des victimes de viol au Sud-Kivu (République Démocratique du Congo). Une relation sexuelle en dehors du couple sera considérée comme mortifère. Elle transforme la victime du viol en poison. Ainsi tout relation sexuelle ultérieure avec le conjoint sera redoutée, car potentiellement fatale pour ce dernier. Ce nouveau regard sur les manifestations traumatiques pourra être utile pour penser nos dispositifs de soins., [Theoretical and clinical approach to trauma in cases concerning rape in Sud-Kivu (Democratic Republic of Congo, DRC)] It is well known that rape has significant potential for inducing post-traumatic stress disorder. One aspect that has been insufficiently explored in the past is the following: What is the place of culture in the psychopathology of trauma? This article highlights the relevance of taking into account cultural interpretations in the management of rape victims in Sud-Kivu (Democratic Republic of Congo). A sexual relationship outside the couple is considered as mortiferous for it transforms the victim of rape into poison. Any further sexual relationship with the partner will thus be dreaded, since it is potentially fatal for the latter. This new insight into traumatic manifestations of rape victims may be useful for our patient care planning.

Disturbed processing of emotional faces and voices is typically observed in schizophrenia. This deficit leads to impaired social cognition and interactions. In this study, we investigated whether impaired processing of emotions also affects musical stimuli, which are widely present in daily life and known for their emotional impact. Thirty schizophrenic patients and 30 matched healthy controls evaluated the emotional content of musical, vocal and facial stimuli. Schizophrenic patients are less accurate than healthy controls in recognizing emotion in music, voices and faces. Our results confirm impaired recognition of emotion in voice and face stimuli in schizophrenic patients and extend this observation to the recognition of emotion in musical stimuli. © 2015 Elsevier Ireland Ltd.

Background: Cognitive impairments have been extensively reported for patients suffering from schizophrenia but a lot of questions persist about these troubles. Several domains of cognitive functions are impaired which result in daily life difficulties. Many arguments tend to show that children suffering from psychotic spectrum disorders present cognitive impairments also but things are not clearly defined. The aim of this review is to compare the cognitive impairments of children suffering from psychotic symptoms and adults suffering from schizophrenia in order to improve the understanding of the two clinical affections. A better understanding of the disease is necessary to improve the health care which currently give poor results. Subjects and methods: Besides the Diagnostic and Statistical Manual (DSM-5) and the French reference books, the present research has been conducted using PubMed, Medline, PsycINFO, PsycARTICLES and ScienceDirect. Literature about cognitive impairments of adults with schizophrenia and children with psychotic spectrum disorders is assessed and results are compared. Results: Both children and adults suffer from cognitive impairments: language, memories, executive and motor functions and IQ are impaired. An important heterogeneity in the troubles has been observed and a poor knowledge in terms of onset time, evolution, intensity, impact on everyday life and therapeutic interest has been reported. Conclusion: Adults with schizophrenia and children suffering from psychotic spectrum disorders have similar cognitive impairments in many domains of cognition. The similarities in term of cognitive impairments highlighted in the present work should support new studies in order to adapt to children the current cognitive therapies which are mostly provided to adults. As the cognitive impairments are heterogeneous, each patient should receive a tailored neurocognitive exam which takes into account its abilities and weaknesses.

Stigma is the most powerful obstacle to the development of mental health care. Numerous activities aiming to reduce the stigma of mental illness and the consequent negative discrimination of the mentally ill and their families have been conducted in Europe. Descriptions of many of these activities are not easily available, either because there are no publications that describe them, or because descriptions exist only in local languages. This supplement aims to help in overcoming this imbalance by providing a description of anti-stigma activities in 14 countries in Europe regardless of the language in which they were published and regardless whether they were previously published. The review was undertaken by experts who were invited to describe anti-stigma activities in the countries in which they reside. It was suggested that they use all the available evidence and that they consult others in their country to obtain a description of anti-stigma activities that is as complete as possible. The anti-stigma activities undertaken in the countries involved are presented in a tabular form. The texts contributed by the authors focus on their perception of the stigma of mental illness and of activities undertaken to combat it in their country. Although much has been done against the stigmatization and discrimination of the mentally ill, fighting stigma remains an essential task for mental health programs and for society. The descriptions summarized in this volume might serve as an inspiration for anti-stigma work and as an indication of potential collaborators in anti-stigma programs.

- Abstract
Introduction: L'objectif général poursuivi dans cette étude est de décrire le tableau clinique de la dépression post-accident vasculaire cérébral (DPAVC) à Kinshasa. Méthodes: Il s'agit d'une étude transversale descriptive portant sur 56 patients suivis pour hémiplégie post-accident vasculaire cérébral au Centre de réhabilitation pour personnes handicapées de Kinshasa (CRPHK) du 1er au 31 août 2011. Résultats: Au Patient Health Questionnaire (PHQ9), 21. 40 % des patients présentaient une dépression modérée à sévère. A l'échelle de Rankin, 63. 8% des patients étaient capables de marcher sans aide. L'apathie modérée à sévère était présente chez 44. 64%; à l'échelle de sévérité de la fatigue, le score de 12. 7 % des sujets était compatible avec un état dépressif. La dépression était associée à la sévérité des troubles neurologiques, à l'incapacité évaluée avec l'échelle de Rankin, à l'apathie et à une appréciation mauvaise de son propre état de santé par le patient. Conclusion: La DPAVC est fréquente à Kinshasa parmi les patients en réhabilitation. La fréquence observée est comparable à celles retrouvées dans des travaux antérieurs dans le monde et aux rares travaux publiés en Afrique. La DPAVC est associée de manière significative à la sévérité des troubles neurologiques, au degré d'handicap, à la fatigue, à l'apathie et à la mauvaise perception de son état de santé.

- Abstract
Objectifs : Décrire l’évolution du profil clinique de la DPAVC sur une période d’un an et déterminer les facteurs associés à l’évolution de la DPAVC sur une période d’un an. Méthodes : Il s’agit d’une étude longitudinale portant sur 30 patients suivis pour hémiplégie post-accident vasculaire cérébral au Centre de réhabilitation pour personnes handicapées de Kinshasa (CRPHK). La sévérité de la dépression a été évaluée avec le « Patient Health Questionnaire » en abrégé PHQ9. Le protocole de l’étude a été administré à chaque patient à deux reprises avec une année d’intervalle. Résultats : L’âge moyen des patients était de 55,87±12,67ans. Soixante-dix pour cent des patients étaient des hommes. Il n’a pas été noté de différence statistiquement significative entre les deux évaluations pour l’état neurologique, l’état de santé perçu et les résultats du test de l’attention. Le pourcentage des patients présentant un syndrome dépressif était de 26,67 % en 2011 et de 20 % en 2012. Les scores aux échelles de l’invalidité et de l’apathie ont été améliorés de manière significative. La moyenne observée pour l’invalidité est passée de 2,77±1,19 à 2,46±2,19 (p=0,002). De 66,7 % en 2011, la proportion des patients capables de marcher sans aide est passée à 93,3 % en 2012 (p=0,03). Par ailleurs, la proportion des patients apathiques est passée de 43,3 % à 13,3 % (p=0,01). L’âge supérieur ou égal à 65ans, le sexe féminin, les troubles de sommeil post-AVC et l’apathie sont restés associés à la DPAVC entre les deux évaluations, avec une augmentation de la force de l’association pour l’apathie. Conclusions : La fréquence de la DPAVC chez les patients de Kinshasa est élevée et reste stable dans le temps. L’invalidité est la caractéristique clinique qui a évolué le plus favorablement. L’association avec l’apathie, présente au début de l’étude s’est renforcée dans le temps.

- Abstract
Ces dernières décennies, des progrès importants ont été accomplis dans la compréhension du phénomène de créativité, grâce au développement des neurosciences. L’envie créatrice semble s’accompagner d’une augmentation du tonus dopaminergique. C’est peut-être la raison pour laquelle de nombreux artistes utilisent ou ont utilisé des drogues psychédéliques. Ces dernières peuvent occasionner une augmentation de leur créativité subjective, une diminution de l’inhibition et de l’anxiété, une meilleure concentration, une fluence accrue et une flexibilité accrue des idées, une « empathie » envers les objets et envers les personnes, une association accrue d’idées différentes, une accessibilité accrue aux idées pré- et inconscientes et une habilité accrue à visualiser la solution complète. Par ailleurs, la relation entre la créativité et l’existence de pathologies mentales est très ancienne. En particulier, la relation entre créativité et bipolarité est bien documentée. Plusieurs pistes tant biologiques que cognitives peuvent en rendre compte. Cependant, le produit artistique ne peut se limiter au contenant cérébral. Le produit artistique exprime toujours également un contenu singulier, modelé par l’histoire et la culture singulières de l’artiste
