myositi - Abstract
Idiopathic inflammatory myopathies (IIMs) encompass a heterogeneous group of rare autoimmune diseases characterised by muscle weakness and inflammation, but in antisynthetase syndrome arthritis and interstitial lung disease are more frequent and often inaugurate the disease. Clinical practice guidelines (CPGs) have been proposed for IIMs, but they are sparse and heterogeneous. This work aimed at identifying: i) current available CPGs for IIMs, ii) patients ' and clinicians' unmet needs not covered by CPGs. It has been performed in the framework of the European Reference Network on rare and complex connective tissue and musculoskeletal diseases (ReCONNET), a network of centre of expertise and patients funded by the European Union's Health Programme. Fourteen original CPGs were identified, notably recommending that: i) extra-muscular involvements should be assessed; ii) corticosteroids and methotrexate or azathioprine are first-line therapies of IIMs. ii) IVIG is a treatment of resistant-DM that may be also used in other resistant-IIMs; iii) physical therapy and sun protection (in DM patients) are part of the treatment; v) tumour screening for patients with DM include imaging of chest, abdomen, pelvis and breast (in woman) along with colonoscopy (in patients over 50 years); vi) disease activity and damages should be monitor using standardised and validated tools. Yet, only half of these CPGs were evidence-based. Crucial unmet needs were identified both by patients and clinicians. In particular, there was a lack of large multidisciplinary working group and of patients ' preferences. The following fields were not or inappropriately targeted: diagnosis; management of extra-muscular involvements other than skin; co-morbidities and severe manifestations.

- Abstract
Anti-Jo-1 is the most frequently detectable antibody in the antisynthetase syndrome (ASSD), an autoimmune disease characterized by the occurrence of arthritis, myositis, and interstitial lung disease (ILD). Recently, we organized an international collaborative group called American and European NEtwork of Antisynthetase Syndrome (AENEAS) for the study of this rare and fascinating disease. The group collected and published one of the largest series of ASSD patients ever described and with one of the longer follow-up ever reported. The number of participating centers is steadily increasing, as well as the available cohort. In the first paper, we showed that arthritis, myositis, and ILD may be frequently the only feature at disease onset, raising problems to reach a correct diagnosis of this syndrome. Nevertheless, we first observed that the ex novo appearance of further manifestations is common during the follow-up, strengthening the importance of a correct diagnosis. In our cohort, the 24 % of the 243 patients up to now collected had isolated arthritis as a presenting feature. These patients represent the most intriguing group in terms of differential diagnosis and clinical time course. Furthermore, data on this aspect are scanty, the reason that lead us to evaluate these aspects in our cohort of patients, reviewing also available literature. In fact, the most relevant aspect is that ASSD is rarely suspected in this setting of patients, in particular in case of poliarticular involvement, positive rheumatoid factor (RF), or anti-cyclic citrullinated peptide antibodies (ACPA) or evidence of joint erosions at plain radiographs. These findings were not rare in our cohort, and they have been also described in other series. Furthermore, manifestations such as Raynaud’s phenomenon, mechanic’s hands, and fever that may lead to the suspect of ASSD are observed only in a third of cases. - Abstract
Objective Arthritis, myositis and interstitial lung disease (ILD) constitute the classic clinical triad of anti-synthetase syndrome (ASSD). These patients experience other accompanying features, such as Raynaud's phenomenon, fever or mechanic's hands. Most ASSD patients develop the complete triad during the follow-up. In the present study we aimed to determine whether the subsequent appearance of accompanying features may suggest the development of triad findings lacking at the onset in anti-Jo1 positive ASSD patients. Methods Anti-Jo1 positive patients presenting with incomplete ASSD (no > 2 classic triad features) were assessed. Clinical characteristics and clusters of disease manifestations were retrospectively collected and analyzed in a large international multicenter cohort of ASSD patients. Results 165 patients (123 women) with incomplete ASSD were identified. Ninety-five patients (57.5%) developed new classic triad manifestations after 15 months median (IQR 9–51) and 40 (24%) developed new accompanying features after 19 months median (IQR 6–56) from disease onset. During the follow-up, the ex-novo occurrence of triad features was observed in 32 out of 40 patients (80%) with new accompanying findings and in 63 out of 125 patients (50.5%) without new accompanying findings (p = 0.002). In patients with at least one new accompanying feature the odds ratio for the occurrence of new triad manifestations was 3.94 with respect to patients not developing ex-novo accompanying findings (95% CI 1.68–9.21, p = 0.002). Conclusion Anti-Jo1 ASSD patients with incomplete forms at disease onset are at high risk for the subsequent occurrence of lacking classic triad findings. Although all ASSD patients should be carefully assessed for the occurrence of new triad features, a closer follow-up should be considered in the subgroup of patients developing ex novo accompanying findings. - Abstract
Antisynthetase syndrome (ASSD) is a rare clinical condition that is characterized by the occurrence of a classic clinical triad, encompassing myositis, arthritis, and interstitial lung disease (ILD), along with specific autoantibodies that are addressed to different aminoacyl tRNA synthetases (ARS). Until now, it has been unknown whether the presence of a different ARS might affect the clinical presentation, evolution, and outcome of ASSD. In this study, we retrospectively recorded the time of onset, characteristics, clustering of triad findings, and survival of 828 ASSD patients (593 anti-Jo1, 95 anti-PL7, 84 anti-PL12, 38 anti-EJ, and 18 anti-OJ), referring to AENEAS (American and European NEtwork of Antisynthetase Syndrome) collaborative group's cohort. Comparisons were performed first between all ARS cases and then, in the case of significance, while using anti-Jo1 positive patients as the reference group. The characteristics of triad findings were similar and the onset mainly began with a single triad finding in all groups despite some differences in overall prevalence. The "ex-novo" occurrence of triad findings was only reduced in the anti-PL12-positive cohort, however, it occurred in a clinically relevant percentage of patients (30%). Moreover, survival was not influenced by the underlying anti-aminoacyl tRNA synthetase antibodies' positivity, which confirmed that antisynthetase syndrome is a heterogeneous condition and that antibody specificity only partially influences the clinical presentation and evolution of this condition.

Il saggio ha come oggetto la produzione di albi illustrati in Italia nel trentennio che va dalla data fatidica del 1987 – individuata da Antonio Faeti come uno spartiacque fondamentale nell'evoluzione delle letteratura per l'infanzia in Italia – e il 2017. La riflessione muove dalla metamorfosi della figura dell'illustratore di libri per bambini nel corso del Novecento, la cui professionalità ha conosciuto radicali cambiamenti nel segno di una complessità e di un riconoscimento sociale che negli ultimi decenni sono diventati sempre più manifesti, e propone una sintetica mappatura di quello che è successo nel trentennio preso in esame: con un'attenzione specifica alle tipologie testuali implicate (fiction e non fiction picturebooks), alle forme della rappresentazione (dal realismo mimetico alle forme della stilizzazione) e agli strumenti di lavoro adoperati (da quelli tradizionali come l'acquarello e le tempere ai nuovi mezzi digitali).

Il contributo si concentra sull’opera letteraria dell’artista australiano Shaun Tan, che nel corso degli ultimi vent’anni ha progettato, scritto e illustrato numerosi libri per bambini (e non solo), confrontandosi con una molteplicità di forme testuali e tecniche espressive e riuscendo a imporsi a livello internazionale quale ineludibile punto di riferimento. Sebbene tanto varî negli stili espressivi adottati, i suoi libri sono legati tra loro da sottili fili rossi che rimandano a due principi fondamentali: l’idea che la letteratura possa essere luogo di meraviglia e di riflessione (territorio dell’incertezza interpretativa e dunque significativa occasione di educazione all’ascolto) e l’idea che la testualità, anche nei libri per bambini, vada liberata dalle pastoie delle convenzioni e dell’omologazione, rappresentando un’occasione unica di sperimentazione di forme e di intrecci tra linguaggi e tecniche. All’interno di tale visione dell’arte del racconto è centrale, per Tan, il concetto di periferia, da intendersi sia come spazio reale, teatro di molte delle storie, sia come spazio concettuale e simbolico, al tempo stesso luogo di germinazione delle idee e territorio ideale di incontro con l’alterità e il mistero: ed è proprio in questo luogo liminare, di confine, analogo ai boschi della tradizione fiabesca, che il discorso critico trova il suo punto focale, arrivando a toccare la grande questione che in maniera più o meno esplicita o trasfigurata è sempre presente nei lavori di Tan: la questione dell’appartenenza, cui è strettamente legato il tema dell’identità e che proprio nella periferia – intesa anche e proprio come spazio concettuale – trova il suo punto di scaturigine.

The 2014–2015 Holuhraun eruption in Iceland, emitted ∼11 Tg of SO2 into the troposphere over 6 months, and caused one of the most intense and widespread volcanogenic air pollution events in centuries. This study provides a number of source terms for characterisation of plumes in large fissure eruptions, in Iceland and elsewhere. We characterised the chemistry of aerosol particle matter (PM) and gas in the Holuhraun plume, and its evolution as the plume dispersed, both via measurements and modelling. The plume was sampled at the eruptive vent, and in two populated areas in Iceland. The plume caused repeated air pollution events, exceeding hourly air quality standards (350 μg/m3) for SO2 on 88 occasions in Reykjahlíð town (100 km distance), and 34 occasions in Reykjavík capital area (250 km distance). Average daily concentration of volcanogenic PM sulphate exceeded 5 μg/m3 on 30 days in Reykjavík capital area, which is the maximum concentration measured during non-eruptive background interval. There are currently no established air quality standards for sulphate. Combining the results from direct sampling and dispersion modelling, we identified two types of plume impacting the downwind populated areas. The first type was characterised by high concentrations of both SO2 and S-bearing PM, with a high Sgas/SPM mass ratio (SO2(g)/SO4 2− (PM) > 10). The second type had a low Sgas/SPM ratio (1 and PM2.5), sulphate (on average ∼90% of the PM mass) and various trace species, including heavy metals. The fine size of the volcanic PM mass (75–80% in PM2.5), and the high environmental lability of its chemical components have potential adverse implications for environmental and health impacts. However, only the dispersion of volcanic SO2 was forecast in public warnings and operationally monitored during the eruption. We make a recommendation that sulphur gas-to-aerosol conversion processes, and a sufficiently large model domain to contain the transport of a tropospheric plume on the timescale of days be utilized for public health and environmental impact forecasting in future eruptions in Iceland and elsewhere in the world.

Anti Jo-1 antibodies are the main markers of the antisynthetase syndrome (ASSD), an autoimmune disease clinically characterized by the occurrence of arthritis, myositis, and interstitial lung disease (ILD). These manifestations usually co-occur (for practical purpose complete forms) in the same patient, but cases with only 1 or 2 of these findings (for practical purpose incomplete forms) have been described. In incomplete forms, the ex novo occurrence of further manifestations is possible, although with frequencies and timing not still defined. The aim of this international, multicenter, retrospective study was to characterize the clinical time course of anti Jo-1 positive ASSD in a large cohort of patients. Included patients should be anti Jo-1 positive and with at least 1 feature between arthritis, myositis, and ILD. We evaluated the differences between complete and incomplete forms, timing of clinical picture appearance and analyzed factors predicting the appearance of further manifestations in incomplete ASSD. Finally, we collected 225 patients (58 males and 167 females) with a median follow-up of 80 months. At the onset, complete ASSD were 44 and incomplete 181. Patients with incomplete ASSD had frequently only 1 of the classic triad findings (110 cases), in particular, isolated arthritis in 54 cases, isolated myositis in 28 cases, and isolated ILD in 28 cases. At the end of follow-up, complete ASSD were 113, incomplete 112. Only 5 patients had an isolated arthritis, only 5 an isolated myositis, and 15 an isolated ILD. During the follow-up, 108 patients with incomplete forms developed further manifestations. Single main feature onset was the main risk factor for the ex novo appearance of further manifestation. ILD was the prevalent ex novo manifestation (74 cases). In conclusion, ASSD is a condition that should be carefully considered in all patients presenting with arthritis, myositis, and ILD, even when isolated. The ex novo appearance of further manifestations in patients with incomplete forms is common, thus indicating the need for an adequate clinical and instrumental follow-up. Furthermore, the study clearly suggested that in ASSD multidisciplinary approach involving Rheumatology, Neurology, Pneumology, and Internal Medicine specialists is mandatory. CA extern

- Abstract
Objective To evaluate if the timing of appearance with respect to disease onset may influence the arthritis presentation pattern in antisynthetase syndrome (ASSD). Methods The patients were selected from a retrospective large international cohort of ASSD patients regularly followed-up in centres referring to AENEAS collaborative group. Patients were eligible if they had an antisynthetase antibody testing positive in at least two determinations along with arthritis occurring either at ASSD onset (Group 1) or during the course of the disease (Group 2). Results 445 (70%; 334 females, 110 males, 1 transsexual) out of the 636 ASSD we collected had arthritis, in the majority of cases (367, 83%) from disease onset (Group 1). Patients belonging to Group 1 with respect to Group 2 had an arthritis more commonly polyarticular and symmetrical (p=0.015), IgM-Rheumatoid factor positive (p=0.035), erosions at hands and feet plain x-rays (p=0.036) and more commonly satisfying the 1987 revised classification criteria for rheumatoid arthritis (RA) (p=0.004). Features such as Raynaud's phenomenon, mechanic's hands and fever (e.g. accompanying findings) were more frequently reported in Group 2 (p=0.005). Conclusion In ASSD, the timing of appearance with respect to disease onset influences arthritis characteristics. In particular, RA features are more common when arthritis occurs from ASSD onset, suggesting an overlap between RA and ASSD in these patients. When arthritis appears during the follow-up, it is very close to a connective tissue disease-related arthritis. Also, the different prevalence of accompanying features between these two groups is in line with this possibility

- Abstract
Anti Jo-1 antibodies are the main markers of the anti-synthetase syndrome (ASSD), an autoimmune disease clinically characterized by the occurrence of arthritis, myositis, and interstitial lung disease (ILD). These manifestations usually co-occur (for practical purpose complete forms) in the same patient, but cases with only 1 or 2 of these findings (for practical purpose incomplete forms) have been described. In incomplete forms, the ex novo occurrence of further manifestations is possible, although with frequencies and timing not still defined. The aim of this international, multicenter, retrospective study was to characterize the clinical time course of anti Jo-1 positive ASSD in a large cohort of patients. Included patients should be anti Jo-1 positive and with at least 1 feature between arthritis, myositis, and ILD. We evaluated the differences between complete and incomplete forms, timing of clinical picture appearance and analyzed factors predicting the appearance of further manifestations in incomplete ASSD. Finally, we collected 225 patients (58 males and 167 females) with a median follow-up of 80 months. At the onset, complete ASSD were 44 and incomplete 181. Patients with incomplete ASSD had frequently only 1 of the classic triad findings (110 cases), in particular, isolated arthritis in 54 cases, isolated myositis in 28 cases, and isolated ILD in 28 cases. At the end of follow-up, complete ASSD were 113, incomplete 112. Only 5 patients had an isolated arthritis, only 5 an isolated myositis, and 15 an isolated ILD. During the follow-up, 108 patients with incomplete forms developed further manifestations. Single main feature onset was the main risk factor for the ex novo appearance of further manifestation. ILD was the prevalent ex novo manifestation (74 cases). In conclusion, ASSD is a condition that should be carefully considered in all patients presenting with arthritis, myositis, and ILD, even when isolated. The ex novo appearance of further manifestati

In un’epoca in cui domina la logica del profitto e dell’immediatamente spendibile, il racconto, la letteratura sembrano appartenere inevitabilmente all’ambito dell’inutilità. La fiaba, con le grandi domande che da millenni essa pone all’umanità, costituisce invece l’assolutamente utile serbatoio di immaginario e di costruzione identitaria che alimenta il bisogno di meraviglioso proprio di ogni essere umano.

Il punto di partenza delle considerazioni oggetto di questo lavoro, è quello di considerare l'idea di "impertinenza" come uno strumento educativo capace di sviluppare un pensiero critico, divergente, aperto al possibile e nel credere che questo lo si possa trovare anche nelle illustrazioni dei libri per bambini. Analizzeremo in particolare le opere di due famosi illustratori contemporanei, Roberto Innocenti e Suzy Lee, evidenziando i caratteri di "impertinenza" dei loro libri. The starting point for the considerations that are the subject of this paper, is to consider the idea of “impertinenza” as an educational device capable of develop-ing a critical thinking, divergent, open to possible and in believing that it can also be find in the illustrations of books for children. We will analyze in particular the works of two famous contemporary illustrators, Roberto Innocenti and Suzy Lee, highlighting in their books the features of “impertinenza”.

Se la letteratura per l’infanzia, a partire dal XVIII secolo, si è fondata sulla trasmissione intenzionale di un messaggio che spesso diventava precetto, norma cui conformarsi, i bambini e le bambine, i ragazzi e le ragazze hanno sempre, magari di nascosto, letto anche altro. I giovani lettori sono andati a cercare quei libri, talvolta «difficili», che in alcune parti andavano oltre i loro limiti ma che proprio per questo apparivano più vivi e si rivelavano da subito capaci di traghettarli oltre la soglia, alla conquista del proprio sé, della propria identità, sorta di talismano nel cammino di formazione di ognuno. Il presente contributo analizza le immagini d'infanzia e il complesso sistema di norme che la letteratura a lei destinata lascia emergere storicamente: dall'immagine del bambino modello, ubbidiente e conformato a quella del monello ribelle e trasgressivo che mette in risalto i comportamenti contraddittori di quegli adulti che si vorrebbero portatori del giusto comportamento.

Nella storia della Letteratura per l'infanzia, l'albo illustrato ha ormai conquistato un posto di rilievo; esso, per sua natura, è caratterizzato dalla contaminazione di linguaggi diversi e dalla polisemia che nasce dal fruttuoso incontro tra testo e immagine, produttore di sempre nuovi testi. Il "picture book" si pone al servizio di un processo in cui la costruzione di una competenza testuale, fondata principalmente sul pensiero logico, deve procedere insieme alla stimolazione e all'utilizzo del pensiero analogico metaforico e combinatorio. I "silent book" dell'illustratrice sudcoreana Suzy Lee, che costituiscono una sorta di trilogia ("L'onda", "Ombra" e "Mirror"), si propongono quale terreno di interessante e prolifica analisi del meccanismo polialfabetico dell'albo illustrato, con il loro valicare i limiti anche fisici posti dal libro volgendo lo sguardo del lettore verso una nuova prospettiva.

Objective To describe demographic, clinical and laboratory features of systemic sclerosis sine scleroderma (ssSSc) in a large multicentre systemic sclerosis (SSc) cohort.Methods Data involving 1808 SSc patients from Italian Systemic sclerosis PRogression INvestiGation registry were collected. The ssSSc was defined by the absence of any cutaneous sclerosis and/or puffy fingers. Clinical and serological features of ssSSc were compared with limited cutaneous (lcSSc) and diffuse cutaneous (dcSSc) subsets.Results Among patients with SSc, only 61 (3.4%) were classified as having ssSSc (F/M=19/1). The effusive six months long 2014-2015 Bárðarbunga eruption (31 August-27 February) was the largest in Iceland for more than 200 years, producing 1.6 ± 0.3 km3 of lava. The total SO2 emission was 11 ± 5 Mt, more than the amount emitted from Europe in 2011. The ground level concentration of SO2 exceeded the 350 µg m−3 hourly average health limit over much of Iceland for days to weeks. Anomalously high SO2 concentrations were also measured at several locations in Europe in September. The lowest pH of fresh snowmelt at the eruption site was 3.3, and 3.2 in precipitation 105 km away from the source. Elevated dissolved H2SO4, HCl, HF, and metal concentrations were measured in snow and precipitation. Environmental pressures from the eruption and impacts on populated areas were reduced by its remoteness, timing, and the weather. Alla lettura del racconto collodiano condotto dell'altra autrice del volume, si affianca quella del Pinocchio “immaginato” attraverso le illustrazioni e del burattino in fuga tra metamorfosi e avventura (Susanna Barsotti). Le Avventure possono essere considerate forse un libro evocativo e suggestivo più che descrittivo e dunque difficile da illustrare senza deludere il lettore; al tempo stesso si presentano come costantemente aperte a tante rappresentazioni cosicché la mancanza di descrizione precisa offre alla fantasia qualsiasi via d’uscita e, negli esiti migliori, continua ad alimentarla. Infine il percorso dell’avventura intesa come prova delle cose volontariamente cercata, il curioso e insopprimibile bisogno di sperimentare tutto il reale, l'impulso irrefrenabile a trasformare il sentito dire e l'immaginato in vita vissuta, pagata di persona, una sorta di quest in chiave comica che costituisce l'elemento essenziale della Weltanschauung di Pinocchio.

This chapter took the floor, starting from the idea that today more than ever the book with illustrations is a very important text source and a privileged device, preferred and liked as the mere reading. The book is often a bearer of polysemic expression, although sometimes transmitted through various means of expression (written text and pictures). Hereof starts also the analysis of some of the major works of one of the most important Italian illustrators of books for adolescents and not only for them, because Roberto Innochenti examines also himself, and also the matter to what extent have his illustrations an indirect influence on literature for children.

La riflessione proposta con questo contributo, muove da una suggestione suscitata dalla lezione di Jean-Luc Nancy, laddove si è parlato di piacere del rapporto, di soggetto e alterità; da qui, appunto si parte, appunto, per allontanarsene eandare a toccare brevemente la relazione tra il piacere di narrare e di narrarsi implicito nel racconto fiabesco e il valore che questo assume nella costruzione identitaria. Anche nelle fiabe, infatti, si innescano meccanismi di costruzione identitaria. Attraverso l’avventura lungo un percorso “problematico” in cui il protagonista, insieme al lettore-ascoltatore, si smarrisce, si perde, per poi ritrovarsi, dopo aver superato numerosi ostacoli che impongono un cambiamento, il racconto fiabesco, offrendo un ricco patrimonio di immagini, simboli, figure cui attingere, impone una riflessione su noi stessi e sulla nostra esistenza.
