52. Risk factors associated with road traffic injuries at the prone-areas in Kampala city: a retrospective cross-sectional study

Abstract

Background: Road traffic injuries (RTIs) pose a disproportionate public health burden in the low and middle-income countries (LMICs) like Uganda, with 85% of all the fatalities and 90% of all disability-adjusted life years lost reported worldwide. Of all RTIs which are recorded in Uganda, 50% of cases happen in Kampala —the capital city of Uganda and the nearby cities. Identifying the RTI prone-areas and their associated risk factors can help to inform road safety and prevention measures aimed at reducing RTIs, particularly in emerging cities such as Kampala. Methods: This study was based on a retrospective cross-sectional design to analyze a five year (2011 – 2015) traffic crash data of the Uganda Police Force. Results: Accordingly, 60 RTI prone-areas were identified to exist across the Kampala. They were ranked as low and high risk areas; 41 and 19, respectively and with the majority of the latter based in the main city center. The bivariate analysis showed a significant association between identified prone-areas and population flow (OR: 4.89, P–value: 0.01) and traffic flow time (OR: 9.06, P–value: 0.01). On the other hand, the multivariate regression analysis only showed traffic flow time as the significant predictor (OR: 6.27, P–value: 0.02) at identified RTI prone-areas. Conclusions: The measures devised to mitigate RTI in an emerging city like Kampala should study thoroughly the patterns of traffic and population flow to help to optimize the use of available resources for effective road safety planning, injury prevention and sustainable transport systems.

53. A Comparison of Student Feedback Obtained through Paper-Based and Web-Based Surveys of Faculty Teaching

Abstract

Many colleges have either begun or are contemplating using the web-based survey to gather student feedback on faculty teaching. Unlike the paper-based method, the web-based approach gives every student in the class the opportunity to provide feedback. Hence, the populations that participate in the web-based and paper-based methods may be quite different, and so may be the feedback. This paper compares the results of student feedback gathered through paper-based and web-based approaches. The results provide information to faculty and administrators on any differences they may expect as they make the transition from a paper-based to a web-based survey of faculty teaching.

Abstract

In special circumstances such as the one presented by the coronavirus universities must convert their face-to-face courses to online. This conversion is challenging for all universities, but extremely challenging for universities that offer all their courses face-to-face. This paper presents the major activities that a quick conversion from face-to-face courses to online courses should consider. This paper shows that higher education institutions can acquire the basic technology and be quickly trained to convert their face-to-face courses to online while trying to achieve the quality that is in line with the quality level defined in their organization's strategy. Abstract

Background: Stakeholders are responsible for managing the risks of disasters. Hence, appropriate, collaborative, timely interactions of involved organizations, and having a collective view of these interactions, have an important influence on the operation of the whole system. This study was aimed at social network analysis (SNA) for the implementation of the Sendai framework for disaster risk reduction in Iran. Methods: SNA was used in this study. A review of literature on disaster risk management (DRM) plus snowball sampling technique identified a list of 85 stakeholders. Delphi method among purposefully selected experts was used to score the relationship between the stakeholders. Louvain method, along with the modularity optimization method, was applied to identify groups of stakeholders with greater interactions. Centrality measurements were used to define organizations with key-roles in the network. Results: The density of this network was 0.75, which showed that not all the stakeholders were connected. The National Disaster Management Organization and Civil Defense Organization showed higher influences considering their responsibilities. A total of 3 clusters of stakeholders with specific duties that mostly interact with each other and have some interaction with other groups were recognized. Conclusion: Understanding the pre-disaster interaction of the network and the strengths and weaknesses of the interactions among stakeholders could help improve DRM. [ABSTRACT FROM AUTHOR]

Abstract

Resilient hospitals are essential to reduce the human impact of disasters. The evidence on the disaster impact on hospitals is limited, and research on hospital resilience is mostly from pre-disaster phases, and does not portray post-disaster realities. This dissertation investigates the impact of a high-magnitude earthquake on a tertiary hospital in Nepal and explores resilience mechanisms, using a combination of quantitative and qualitative methodologies. The key findings are: i) the earthquake produced resource-intensive conditions that required lengthy in-hospital treatment; ii) the hospital’s function was affected and the provision of elective care was disrupted; iii) the hospital’s resilience was highly influenced by emerging adaptations and on resilience of individual staff, which depended of meaningfulness, sense of safety, and sense of belonging. Abstract

Eiders have long been recognised as among the most vulnerable people in disaster events. This paper reports a qualitative study of the self-perceived needs of older people in the aftermath of the Barn earthquake in Iran in 2003. A total of 56 people aged from 65 to 88 years were recruited to the study using purposive sampling, including 29 men and 27 women. Six focus group discussions and ten semi-structured individual interviews were conducted. Each focus group involved six to ten people from the cities of Bam and Baravat and their rural suburbs. Content analysis was used to analyse the transcribed data. The analysis identified four major themes among the informants' concerns: inappropriate service delivery, affronts to dignity, feeling insecure and emotional distress. A disaster-prone country like Iran needs to be appropriately prepared with culturally sensitive plans to meet the needs of those who suffer from their effects, not least older people. Emergency relief managers should note that for many older people in a disaster zone, customary forms of relief are neither required nor appropriate, and that their distinctive immediate and long-term needs should be assessed and met. Relief agencies need to be trained to be age-sensitive and should mainstream older people's rights in the planning and implementation of both the response and recovery phases of assistance. Abstract

Introduction: In the aftermath of mass casualty incidents (MCIs), many decisions need to be made in a fast and influential manner in a high pressure environment to distribute the limited resources among the numerous demands. This study was planned to rank the criteria influencing distribution of casualties following trauma-related MCI. Methods: This study utilized a modified Delphi methodology, concentrating on extracted criteria attained from preceding systematic literature reviews. The 114 extracted criteria were classified into eight sections including space, staff, equipment, system and structures, triage, treatment, transport, and uncategorized criteria and were imported into an online survey tool. In the first round, experts were asked to rank each criterion on a five-point Likert scale. The second round incorporated feedbacks from the first round, stating percent and median scores from the panel as a whole. Experts were then called upon to reassess their initial opinions regarding uncertain remarks from the first round, and once again prioritize the presented criteria. Results: Fifty-seven criteria were regarded as relevant to the following sections: space: 70% (7/10); staff: 44% (4/9); system / structure: 80% (4/5); equipment: 39.1% (9/23); treatment; 66.7% (6/9); triage: 73.7% (14/19); transport: 38.7% (12/31) and other sections: 12.5% (1/8). The first round achieved nearly 98% (n=48) response rate. Of the 114 criteria given to the experts, 68 (almost 60%) were approved. The highest percentage of approval belonged to the system and structures sections (4/5=80%). The response rate for the second round was about 86% (n=42). A consensus could be reached about nearly 84% (57) of the 68 criteria presented to experts. Conclusion: “Casualty Level of Triage on the Scene” and “Number of Available Ambulances” were the two criteria that obtained the highest level of consensus. On the other hand, “gender of casualty”, “Number of Non-Medical staff in each Hospital” and “Desire to transport family members together” got lowest level of consensus. This sorted list could be used as a catalogue for developing a decision support system or tool for distribution of victims following mass casualty incidents.

Abstract

Recent advances in approaches and production technologies for the production of goods and services have made just-in-time (JIT) a strong alternative for use in intermittent and small batch production systems, especially when time-based competition is the norm and a low inventory is a must. However, the conventional JIT system is designed for mass production with a stable master production schedule. This paper suggests supplementing the information provided by production kanbans with information about customer waiting lines to be used by operators to schedule production in each workstation of intermittent and small batch production systems. This paper uses simulation to analyze the effect of four scheduling policy variables--number of kanbans, length of the withdrawal cycle, information about customer waiting lines, and priority rules on two performance measures--customer wait-time and inventory. The results show that using information about customer waiting lines reduces customer wait-time by about 30% while also reducing inventory by about 2%. In addition, the effect of information about customer waiting lines overshadows the effect of priority rules on customer wait-time and inventory. Abstract

Being the second-largest country in the Middle East, Iran has a long history of civilisation during which several dynasties have been overthrown and established and health-related structures have been reorganised. Iran has had the replacement of traditional practices with modern medical treatments, emergence of multiple pioneer scientists and physicians with great contributions to the advancement of science, environmental and ecological changes in addition to large-scale natural disasters, epidemics of multiple communicable diseases, and the shift towards non-communicable diseases in recent decades. Given the lessons learnt from political instabilities in the past centuries and the approaches undertaken to overcome health challenges at the time, Iran has emerged as it is today. Iran is now a country with a population exceeding 80 million, mainly inhabiting urban regions, and has an increasing burden of non-communicable diseases, including cardiovascular diseases, hypertension, diabetes, malignancies, mental disorders, substance abuse, and road injuries.
