Histochemical technique using autoradiographic procedure was presented which can be used to demonstrate non-labeled mercury in paraffin-embedded tissues of the central nervous system and other organs. Compared to other histochemical methods, this technique allows the use of formalin-embedded, stored, serial paraffin sections and appears more sensitive for the demonstration of intracellular mercury in Minamata disease (methylmercury poisoning). The technique forms reaction products of high quality which can easily be seen in cell bodies and does not disturb the structure of tissue cells during preparation.

胃癌患者54例をDisease free group (II群) 21例, Localized group (III群) 11例, Regional group (IV群) 7例, Metastatic group (V群) 14例に分け, 良性疾患22例 (I群) とともにリンパ球subsets (OKT3, OKT4, OKT8, OKIal, Leu7) を検討した. 1) 比率ではII, IV群でOKT4+細胞が42.7±9.67%, 40.9±7.37%と低く, OKIal+細胞がV群で23.1±12.3%と高い以外, 差が認められなかった. 2) 絶対数ではリンパ球減少に伴ない, Leu7+細胞以外の各分画がIV, V群で減少していた. 特にOKT3+, OKT4+, OKT8+細胞数減少が著明であった. 以上より, 進行胃癌患者ではリンパ球subsetsの比率の異常よりむしろ絶対数減少 (特にT細胞系) が著明で, これが細胞性免疫能低下に関与していることが示唆された.

胃癌に対する胃全摘兼摘脾を施行した4例に対し, 平均21%重量の脾自家移植により脾温存を試み以下の成績を得た. 1) 脾自家移植にともなう合併症は認められなかった. 2) 術後の脾シンチで全例に生着を確認し, 移植脾による網内系貧食能が認められた. 3) 術後血清IgMの低下は認められず, 縫合不全例では, 著しい上昇を認めた. 4) 術後血小板は一過性に増加したが, その後術前値に復した. 以上本法による胃癌手術は, 癌の根治切除の原則を満たしつつ脾機能を温存するという安全で有用な術式と考えられる.

458. Effects of Theo-Esberiven on the coronary circulation and myocardial metabolisms in dogs

In isolated dog heart, Theo-Esberiven 0.1 mg, which contains 12 mg proxyphylline, 2.5 mg rutin and 5 mg Melilotus extract, and proxyphylline (12 mg) increased coronary blood flow (CBF) and was associated with increased heart rate (HR) and myocardial contractile force (MCF). The effect of Theo-Esberiven on CBF was about 1.7 times higher than that of proxyphylline. Theo-Esberiven did not significantly affect myocardial oxygen consumption (QO2) and redox potential (deltaEh), while proxyphylline aggravated myocardial metabolisms, as determined from these parameters. Esberiven 0.1 ml, which contains 2.5 mg rutin and 5 mg Melilotus extract, slightly but signficantly increased CBF and decreased QO2 and deltaEh without changing HR and MCF. In situ, intra-coronary injection of either Theo-Esberiven or proxyphylline resulted in a dose-dependent increase in left circumflex coronary flow (LCCF). The effect of Esberiven on LCCF was much less and was slight at the higher doses. Intravenous injection of Theo-Esberiven (0.1 ml/kg) increased LCCF. Besides this change, marked fall in blood pressure and tachycardia were induced by both Theo-Esberiven and proxyphylline without the change in dP/dtmax. Esberiven by the same route led to the decrease in blood pressure associated with HR and dP/dtmax. These results indicate that Theo-Esberiven may be appropriately prescribed for ischemic heart diseases.

An enzyme-linked immunosorbent assay for human epidermal growth factor (hEGF) by the sandwich method with the use of orthophenylene diamine (OPDA) for the substrate of the enzyme reaction was developed. This assay showed high sensitivity, and there was no problem with the recovery rates even without any pretreatment of samples. The changes of hEGF levels in plasma and serum depend on the condition of blood storage from sampling until separation of the cell content. The levels of hEGF markedly increased with time in serum, especially when stored at room temperature but less so in plasma. Changes of hEGF levels in plasma were negligible for up to 3 h if the blood was stored at 4° C until separation.

Strains of methicillin- and cephem-resistant Staphylococcus aureus (MCRSA) were recovered from 25 patients on the ward of the second department of surgery of Ehime University Hospital, from 3/1/86 to 9/30/87.21 patients were children and four were adults. The most common diseases in these children were congenital heart disease and congenital biliary atresia. All of the adult patients were old and compromised hosts. Several routes of spread among there patients were suspected, because each strain was of a different coagulase type and phage group. Susceptibility to antibiotics of the coagulase-positive strains was different from that of the coagulase-negative strains. Chlorhexidin was effective for killing these strains. As the number of patients infectied or colonized with MCRSA is expected to increase gradually in the future, control of hospital infection and choice of more effective antibioticus are important.

Chronic chemical denervation of the sympathetic nervous system with 6-OH dopamine resulted in an augmentation of the coronary blood flow (CBF) increase inherent in the canine heart-lung preparation, but not in an augmented accumulation of 6-keto PGF1α in the circulating blood. In denervated HLP, 6-keto PGF1α was lower and the vasodilator response to PGF2 larger at the start of the experiments. It was concluded that exaggerated increase in CBF was due to a potentiation of the vasodilator response to PGI2.

A 56-year-old man died of widespread metastases of malignant fibrous histiocytoma (MFH) primarily developed in the left lower gingiva after a rapid clinical course is reported. Primary and metastatic tumors showed light and electron microscopic, histochemical and immunohistochemical characteristics of MFH together with cytoplasmic hyaline globules and stromal osteoids. Hyaline globules revealed periodic acid-Schiff positive and diastase resistant reactions but not positive with any histochemical and immunohistochemical reactions. Osteoids were partially calcified and rimmed by osteoblastic tumor cells. The present case of MFH may provide difficulties in the differential diagnosis from some osteoid-forming sarcomas in soft tissues with relation to the histogenesis.

A rare case of adult duodenal obstruction caused by a web is reported.The patient, a 30-year-old man, was admitted to this hospital with the chief complaints of abdominal distension and vomiting. Examination by an upper GI series and gastro-duodenoscopy led to the diagnosis of duodenal web. Laparotomy was performed and a complete circular web was found in close contact with the papilla of Vater. The duodenal web was resected and the postoperative course was uneventful.The duodenal web developed from incomplete recanalization of duodenum in the embryonic stage. Intraluminal duodenal diverticulum also results from the same process. Resection of the web is the best therapeutic procedure for this anomaly.

Benzoyl isocyanates react with ethyl (dimethylsulfuranilidene) acetate, dimethyl sulfonium phenacylide and dimethyloxosulfonium methylide to give the corresponding stable benzoylcarbamoyl- sulfonium ylides which on pyrolysis, are converted into oxazoles. However, with dimethylsulfonium phenacylide, thioacyl isocyanate affords the thiazole derivative. In connection with the formation of oxazoles, the reaction of benzoyl isocyanates with some diazoalkanes was also investigated: the products are not 1,2,3-triazolones as reported by Neidlein, but instead benzoylcarbamoyldiazo compounds which were thermally decomposed to oxazoles.

OBJECTIVE:To test the validity of diffusion tensor imaging (DTI) measures of tissue injury by examining such measures in a white matter structure with well-defined function, the medial longitudinal fasciculus (MLF). Injury to the MLF underlies internuclear ophthalmoparesis (INO). METHODS:40 MS patients with chronic INO and 15 healthy controls were examined under an IRB-approved protocol. Tissue integrity of the MLF was characterized by DTI parameters: longitudinal diffusivity (LD), transverse diffusivity (TD), mean diffusivity (MD) and fractional anisotropy (FA). Severity of INO was quantified by infrared oculography to measure versional disconjugacy index (VDI). RESULTS:LD was significantly lower in patients than in controls in the medulla-pons region of the MLF (p < 0.03). FA was also lower in patients in the same region (p < 0.0004). LD of the medulla-pons region correlated with VDI (R = -0.28, p < 0.05) as did FA in the midbrain section (R = 0.31, p < 0.02). CONCLUSIONS:This study demonstrates that DTI measures of brain tissue injury can detect injury to a functionally relevant white matter pathway, and that such measures correlate with clinically accepted evaluation indices for INO. The results validate DTI as a useful imaging measure of tissue integrity.

The reacion of L-alanine (L-ala) with Fe(NO3)2·9H2O in the presence of H5dhpta (1,3-diamino-2-hydroxypropane-N,N,N′,N′-tetraacetic acid) and base (NaOH) affords the novel tetranuclear complex Na[Fe4(dhpta)2(O)(OH)(L-ala)2]·6H2O having L-alaninato bridges and an (Fe4(O2H))9+ core whose crystal structure and magnetic properties were determined.

Condensations of tris(2-aminoethyl)amine (tren) with acetylacetone (acac) give two different types of complexes, depending upon the coexisting metal ions; one contains two tridentate ligands which have β-ketoimine moieties at all primary amino groups of tren, and the other has a pentadentate ligand having a β-ketoimine moiety at one of the three amino groups of tren. The Sr2+, Ca2+, Mg2+, and Mn2+ ion give the former type complexes and Cu+, Co3+, and Zn2+ give the latter. The crystal structures of all the complexes have been characterized by X-ray analyses. Photochemical reactions of the binuclear palladium(0) complex [Pd2(dppm)3] with organic halides yield palladium(I) dihalogeno complexes with irradiation of visible light. Photooxidation of alcohols was observed for ethanol and 2-propanol with the palladium complex. Photophysical and quenching properties of a chiral [Pt(BINAP)2] complex (BINAP = RR- or SS-2,2′-bis(diphenylphosphino)-1,1′-binaphthyl) were studied. The emitting MLCT excited state has a long lifetime. Photochemical reactions of [Pt(BINAP)2] with organic halides cause halogen abstraction. Atom and electron transfer may be candidates for the initial stage of the photoreaction. The mechanism of the photoreaction is discussed with the quenching data.

Immobilized DNA membranes prepared by binding between DNA and poly(γ-methyl-l-glutamate) membranes having a cis-diaminedichloro platinum group-generated oscillation of the membrane potential under a concentration gradient of several salts at frequencies between 0 and 5 Hertz at pH 7.0, although nonmodified poly(γ-methyl-l-glutamate) membranes showed a constant potential. The amplitude of oscillation depended on the length of the joint segments between the DNA and poly(γ-methyl-l-glutamate) membranes; the immobilized DNA membranes modified with diaminopropane and diaminobutane showed a high oscillation amplitude (e.g., 4−5 mV). It was also influenced by the salts used, and the highest oscillation amplitude in the membrane potential across the immobilized DNA membranes modified with diaminobutane was observed when NaCl was used as the salt solution. Thyrotoxicosis with diffuse thyroid disease can be caused by Graves' disease (GD) or destructive thyroiditis (DT). Optimal treatment of the underlying condition requires a prompt and accurate method for the diagnosis of thyrotoxicosis. This study evaluated measurement of the mean peak systolic velocity of the superior thyroid artery (STA-PSV) by ultrasonography in detecting thyrotoxicosis in Japanese patients. We recruited 44 patients with untreated GD, 13 with DT, 55 with treated GD, and 49 subjects without thyroid disease. Blood samples were taken to analyze thyroid function and STA-PSV was measured by ultrasonography. The mean STA-PSV was the highest in the untreated GD group, followed by treated GD patients and then those with DT. Receiver operating characteristic curves of the STA-PSV values demonstrated that the area under the curve required discriminating untreated GD from DT was 0.941. The optimal sensitivity and specificity were 83.7% and 92.3%, respectively, using 45 cm/sec as the cutoff value. In conclusion measurement of STA-PSV by ultrasonography is useful for the diagnosis of thyrotoxicosis in Japanese patients.

482. Urinary excretion of thyroid hormone in CKD patients: a proof-of-concept of nephrogenic hypothyroidism.

Purpose: Patients with chronic kidney disease (CKD) complicated by hypothyroidism exhibit a higher prevalence of urine protein than that in the general population. This study was aimed at investigating thyroid hormones and thyroid hormone-binding proteins excreted in urine to elucidate the urine protein-associated underlying mechanisms of hypothyroidism. Methods: Between November 2016 and August 2018, thyroid function (serum free T3 [sFT3], free T4 [sFT4], and thyroid-stimulating hormone [sTSH]), kidney function (estimated glomerular filtration rate [eGFR]), thyroid antibodies and albumin (Alb) were evaluated in 99 Japanese CKD patients with proteinuria at our outpatient clinic. A urine examination was also performed to assess the following parameters: total T3, total T4, TSH, Alb, preAlb, thyroid-binding globulin, and protein. Results: The median patient age at study recruitment was 60 years; 50 patients (50.5%) were male. The median eGFR and Alb level were 20.3 ml/min/1.73 m2 and 3.8 g/dL, respectively. 21 patients (21.2%) were diagnosed with nephrotic syndrome (NS). The median sFT3, sFT4, and sTSH levels were within normal limits. Approximately 70% of the patients had thyroid dysfunction and 51.5% had overt or subclinical hypothyroidism without predominantly antibody positive. Regarding NS and non-NS patients, age and Alb were significantly different between these groups, while sex and eGFR were not significant, but the urinary T4 and TSH levels were higher in the NS group; thus, more severe hypothyroid. Conclusion: We found a significant association between hypothyroidism and NS regardless of sex and antibodies. Urinary loss of thyroid hormones must be a factor influencing hypothyroidism independent of autoimmunity. [ABSTRACT FROM AUTHOR]
