The safe storage of spent nuclear fuel (SNF) in dry cask storage systems (DCSSs) is critical to the nuclear fuel cycle and the future of nuclear energy. A critical component of DCSSs is the SNF canister. The canister is a sealed stainless-steel structure, which is first vacuum dried and then backfilled with helium. The structural deterioration within a canister can be monitored through its internal gas properties. This monitoring serves as the driving force behind the non-invasive ultrasonic sensing approach in this paper. A major challenge in collecting gas-borne signals using ultrasonic sensing is the impedance mismatch between the stainless-steel canister and the helium gas inside. Only a small fraction of the ultrasonic signal makes its way from the transmitter to the receiver through the gas medium. In this paper, experimental studies on a partial full-scale canister mock-up were carried out to capture the gas-borne signals. Damping materials were applied on the outside, and blocking and unblocking tests were conducted to identify the gas-borne signal. The research results showed that the excitation frequency played an important role in maximizing the gas-borne signals. The gas-borne signal was successfully detected at around the theoretical time-of-flight (TOF) at 225 kHz. A high signal-to-noise ratio (SNR) was achieved in the measurements. Next, acoustic impedance matching (AIM) layers were added, and it was found that the gas signal energy was improved by 160.4% compared with that of no AIM layers. Subsequently, the relative humidity (RH) level and temperature of the gas were varied to simulate abnormal internal conditions of the canister. The non-invasive testing system demonstrated reliability and sensitivity in detecting gas temperature and RH variations. Theoretical calculations demonstrated the potential for detecting low-level xenon and air within an actual SNF canister filled with helium. Last, an active noise cancellation (ANC) method, previously developed by the authors, was verified on the canister mock-up for the first time. This study aimed to determine the influence of musculoskeletal ultrasound (MSKUS) curriculum on applicants during the residency-selection process. A survey of 666 applicants for the Johns Hopkins University, Mayo Clinic, and Harvard/Spaulding Rehabilitation Physical Medicine and Rehabilitation programs was conducted in June 2020. A total of 180 respondents scored the influence of a MSKUS curriculum on their decision making for residency selection. In addition, applicants were asked to rank specific areas of physical medicine and rehabilitation that influenced their decision making. Participants most commonly included MSKUS in their top three areas of interest when constructing their rank order list. When asked whether MSKUS presence within a program had an effect during the interview-selection process, 71% responded with "very important" or "absolutely essential" (P < 0.001). For 74% of applicants, exposure to MSKUS in residency was an important factor when creating their rank order list (P < 0.001). More than 92% of applicants stated that they are "likely" or "very likely" to use MSKUS in their future practice and 83% would recommend a program with MSKUS to future candidates (P < 0.001). Based on these results, a large percentage of physical medicine and rehabilitation applicants intend on using MSKUS in their future practice. Ultrasonic sensing is a non-invasive technique for monitoring impurity gas composition in various industrial applications where safety and regulatory compliance are crucial. In this study, ultrasonic sensing and neural networks were used to analyze impurity gases (i.e., air and argon) in helium. An experimental platform was established to acquire ultrasonic data. In the forward problem, an artificial neural network (ANN) model was used to forecast the response and time-of-flight (TOF) based on the excitation, and argon and air concentrations. The inverse problem was solved using a convolutional neural network (CNN) to predict the argon and air concentrations given the ultrasonic response and excitation. The results showed that the ANN accurately predicted the ultrasonic response and the change in TOF with concentration. As the air concentration was increased from 0 to 9.8%, the TOF sensitivity to detect argon decreased by 39.8% and 16.1% from ANN and sound speed theory, respectively. The CNN demonstrated high accuracy in predicting concentrations for inputs in the testing dataset. The application of the trained CNN indicated that it over-predicts air concentration while under-predicting the argon concentration. To improve accuracy, the predicted air and argon concentrations should be corrected by -0.992% and 1.027% bias, respectively. [ABSTRACT FROM AUTHOR]

Ultrasonic wave propagation may be used in non-invasive sensing to identify the properties of a fluid in a container. An ultrasonic excitation can be coupled into the liquid or the gas through the walls of the container and detected by a receiver on the other side. However, in addition to this fluid path, another signal will also propagate through the structure and is referred as noise in this study. For most materials, this structural noise will be of higher amplitude compared to the fluid signal due to the large acoustic impedance difference between the solid container and the fluid. In this paper, an active noise cancellation (ANC) method is developed and demonstrated on an experimental setup. It was shown that this method cancels more than 90% of the noise signal in a given time window of interest. A sequential version of this cancellation method is also proposed, and it was shown that its performance improves as the number of canceling excitations is increased. The performance was found to be independent of the location of canceling transducers and position of the silence window but dependent on the width of the silence window. As an example, the ANC method was applied to noise cancellation of a signal travelling through water in a stainless-steel container. The signal-to-noise ratio (SNR) of the water signal in the window of interest was increased by 148% from 13.98 dB to 34.68 dB by using the proposed ANC method. [ABSTRACT FROM AUTHOR]

Nutrient export of nitrogen and phosphorus from row crop agriculture in the Upper US Midwest is a threat to the structure and function of aquatic systems. To meet Environmental Protection Agency (EPA) nutrient reduction goals, the Upper US Midwest needs to implement strategies to reduce nutrient export from agriculture. Studies demonstrate the potential of cover crops to reduce the export of nitrate-nitrogen from the Upper US Midwest. We investigated the impact of the economically viable winter cash cover crop pennycress (Thlaspi arvense) on soil porewater nutrients and soil nutrients and characteristics. We used nine replicated 0.8 ha plots (n = 3 per treatment) at a production scale research farm over 4 years with pennycress and fertilized pennycress (56 kg ha -1 of urea) treatments compared to a fallow reference. Over the study period, soil porewater nitrate-nitrogen was reduced by 53% in pennycress plots and 34% in fertilized pennycress plots relative to the fallow reference at a depth of 45 cm. Early season establishment was crucial in providing nutrient reduction potential. In 2021, poor pennycress establishment resulted in porewater nitrate-nitrogen concentrations 141% higher than in 2022 with excellent pennycress establishment. Following pennycress termination, soil nitrate-nitrogen was reduced by 24% in pennycress and 26% in fertilized pennycress compared to the fallow reference in the top 30 cm of soil. Following 4 years of pennycress planting, nitrate-nitrogen concentrations were significantly reduced with no broad effect on soil characteristics. Abstract: Ankle sprains are the most common lower extremity injury in physically active individuals. These injuries are classified as lateral, medial, and/or syndesmotic. Treatment may include functional rehabilitation, bracing, weight-bearing restriction, medications, injections, and surgery. While most sprains heal rapidly, permanent disability and pain may arise. Diagnostic ultrasound has been demonstrated to be accurate in diagnosing ligamentous injuries, but it is often excluded from management algorithms that rely on physical examination alone to diagnose significant injuries. This article proposes a comprehensive, evidence-based diagnostic ankle ultrasound protocol to implement in conjunction with thorough history and physical examination. Background: Total mesorectal excision is the standard surgical procedure for rectal cancer treatment. Several studies have shown a close correlation between the prognosis of patients with rectal cancer and the completeness of the mesorectal specimen., Objective: To assess the correlation between macroscopic assessment of mesorectal excision and long-term oncological outcomes., Design: Retrospective analysis of an Institutional Review Board-approved database., Settings: Tertiary referral center., Patients: Patients with rectal cancer who were operated on between March 2016 and October 2019 were classified into 3 groups based on the mesorectal specimen quality: complete, near complete, and incomplete. Only patients with a follow-up of ≥2 years and without signs of preoperative distant disease were included., Main Outcome Measures: Relationship between total mesorectal excision and local and distant recurrence rates in patients with rectal cancer., Results: A total of 124 patients (35.5% females) were included in the analysis, with a mean age of 58.1 (SD 12) years and a mean BMI of 26.4 (SD 4.59) kg/m². Neoadjuvant chemoradiation was administered to 71% of patients, whereas 13.7% received total neoadjuvant therapy. Restorative procedures were performed in 107 patients (86.3%), whereas 17 patients (13.7%) underwent abdominoperineal resection. The majority of mesorectal excision specimens (87.09%) were complete or near complete. Local recurrence rates were 6.3% (1/16) in the incomplete and 7.4% (8/108) in the complete/near complete group ( p = 0.86). Metachronous distant metastases occurred in 6 patients (37.5%) in the incomplete group and in 24 patients (22.2%) in the complete/near complete group (p = 0.18). Thus, specimen quality did not appear to impact disease-free survival., Limitations: Retrospective, single-center study with relatively short follow-up., Conclusions: In the era of a multidisciplinary approach and extensive use of neoadjuvant therapy, macroscopic completeness of total mesorectal excision may not be as valuable a prognosticator as in the past. Abstract: Magnetic resonance imaging is the current gold standard imaging modality for diagnosing anterior cruciate ligament (ACL) tears. However, ultrasound has grown in popularity for detecting ACL injuries because of its low cost, portability, and dynamic assessment capabilities. Recent studies demonstrate high sensitivity and specificity for diagnosing isolated ACL tears via ultrasound, but tremendous heterogeneity remains for optimal technique including patient positioning, transducer placement, and dynamic versus static ultrasound usage. As ultrasound becomes ubiquitous in clinics, training rooms, and on the sidelines, identifying objective and sensitive measurements to appropriately screen athletes for significant knee injuries is imperative. This article aims to review the current role of diagnostic ultrasound in ACL injuries and propose a standardized version of the Prone Lachman with Ultrasound test, which is an objective, reliable, and easily reproducible technique to evaluate ACL competency. Purpose: To describe the pharmacokinetics (PK) of ciprofloxacin 0.3 % and fluocinolone acetonide 0.025 % otic solution (CIPRO+FLUO), ciprofloxacin 0.3 % otic solution alone (CIPRO), and fluocinolone acetonide 0.025 % otic solution alone (FLUO) administered into the middle ears of pediatric patients with Acute Otitis Media with Tympanostomy Tubes (AOMT)., Materials and Methods: We performed a PK analysis of patients who participated in two multicenter, randomized, double-blind AOMT clinical trials (SALVAT studies CIFLOTIII/10IA02 and CIFLOTIII/10IA04). Each patient received 0.25 mL of CIPRO+FLUO, CIPRO, or FLUO twice a day instilled into the ear canal(s) for 7 days to treat AOMT. Blood samples of patients with unilateral AOMT were collected before the administration of the first dose of study medication at Visit 1 (day 1) and within 1-2 h after the last dose on day 7. Blood samples were analyzed to detect ciprofloxacin and fluocinolone acetonide concentrations using two validated liquid chromatography-tandem mass spectrometry (LC-MS-MS) methods, with the lower limit of quantification for ciprofloxacin and fluocinolone acetonide in plasma samples being 1 ng/mL. Thirty randomly selected patients between 10 months and 10 years of age (mean age, 4.4 years) were included in the study. Background: COVID-19 has significantly impacted healthcare worldwide. Lack of screening and limited access to healthcare has delayed diagnosis and treatment of various malignancies. The purpose of this study was to determine the effect of the first year of the COVID-19 pandemic on sphincter-preserving surgery in patients with rectal cancer., Methods: This was a single-center retrospective study of patients undergoing surgery for newly diagnosed rectal cancer. Patients operated on during the first year of the COVID-19 pandemic (March 2020-February 2021) comprised the study group (COVID-19 era), while patients operated on prior to the pandemic (March 2016-February 2020) served as the control group (pre-COVID-19)., Results: This study included 234 patients diagnosed with rectal cancer; 180 (77%) patients in the pre-COVID-19 group and 54 patients (23%) in the COVID-19-era group. There were no differences between the groups in terms of mean patient age, sex, or body mass index. The COVID-19-era group presented with a significantly higher rate of locally advanced disease (stage T3/T4 79% vs 58%; P = .02) and metastatic disease (9% vs 3%; P = .05). The COVID-19-era group also had a much higher percentage of patients treated with total neoadjuvant therapy (52% vs 15%; P = .001) and showed a significantly lower rate of sphincter-preserving surgery (73% vs 86%; P = .028). Time from diagnosis to surgery in this group was also significantly longer (median 272 vs 146 days; P < .0001)., Conclusion: Patients undergoing surgery for rectal cancer during the first year of the COVID-19 pandemic presented later and at a more advanced stage.
