Purpose: To compare clinical outcomes and institutional costs of elective laparoscopic and open incisional hernia mesh repairs and to identify independent predictors of prolonged operative time and hospital length of stay (LOS).Methods: Retrospective observational cohort study on 269 consecutive patients who underwent elective incisional hernia mesh repair, laparoscopic group (N = 94) and open group (N = 175), between May 2004 and July 2014.Results: Operative time was shorter in the laparoscopic versus open group (p < 0.0001). Perioperative morbidity and mortality were similar in the two groups. Patients in the laparoscopic group were discharged a median of 2 days earlier (p < 0.0001). At a median follow-up over 50 months, no difference in hernia recurrence was detected between the groups. In laparoscopic group total institutional costs were lower (p = 0.02). At Cox regression analysis adjusted for potential confounders, large wall defect (W3) and higher operative risk (ASA score 3-4) were associated with prolonged operative time, while midline hernia site was associated with increased hospital LOS. Open surgical approach was associated with prolongation of both operative time and LOS.Conclusions: Laparoscopic approach may be considered safely to all patients for incisional hernia repair, regardless of patients' characteristics (age, gender, BMI, ASA score, comorbidities) and size of the wall defect (W2-3), with the advantage of shorter operating time and hospital LOS that yields reduced total institutional costs. Patients with higher ASA score and large hernia defects are at risk of prolonged operative time, while an open approach is associated with longer duration of surgical operation and hospital LOS. As a result of our work on theLycopodiaceaeaccount for the Critical Flora of Italy, a new, synonymic inventory of Italian taxa is presented, based on herbarium studies and critical re-examination of extant literature. Three genera with eight species are accepted:Huperzia(1 species),Lycopodiella(1), andLycopodium(6). An additional, ninth species (Lycopodiella cernua) is listed as a casual alien. The genusDiphasiastrum, accepted in several recent works, is treated at sectional level withinLycopodium(three species and their three hybrids).Lycopodium lagopus( = L. clavatumsubsp.monostachyon) is a recent addition to the Italian flora. Images of five types and Internet URI of other six types are supplied. Dot distribution maps and a dichotomous key to Italian species are provided. Most of the taxa are currently considered unworthy of recognition, of which 16 belong toD. asperasensu lato. The work on the Isoetaceae account for the Critical Flora of Italy has resulted in a new, synonymic inventory of ItalianIsoetestaxa, based on herbarium studies, observation by scanning electron microscopy and critical re-examination of the previously published works. Ten species are listed. The namesI. longissimaandI. gymnocarpaare restored as correct, to replaceI. velataandI. subinermis, respectively. Abstract: Aim of the present work is the analysis (through the study of enzyme polymorphism) of Sicilian and African (Tunisian) populations of Ambrosina bassii, a small perennial endemic to the Central-Western Mediterranean basin, in order to verify if the complex geological history of this part of the Mediterranean area left its mark in the present-day genetic structure of this taxon. Starch gel allozyme electrophoresis of seven putative loci of A. bassii was employed to estimate genetic diversity, genetic structure and gene flow. Populations from Sicily, Tunisia and Sardinia (as outgroup) were sampled. Results show that Sicily populations have 4 private alleles, Sardinia 3, Tunisia just one. One allele is private to both Sardinia and Tunisia, another one to Sardinia and Sicily. Even if there are no alleles private to Sicily and Tunisia, “cluster analysis” (based on Nei''s genetic distances), “non-metric multidimensional scaling” (computed on the basis of a matrix with F ST values between populations) and Bayesian analysis point out a clear isolation of Sardinian populations, and a greater similarity between Sicilian and Tunisian populations compared to Sardinian ones. The strong genetic affinity between populations from Sicily and Tunisia, considering the very low dispersal ability of the species, gives evidence of a recent continuity between the populations as well as between the two areas. Considering also the estimates of divergence times, a post-Messinian terrestrial connection between the two landmasses can be hypothesized. [Copyright &y& Elsevier]

The article discusses the anatomical, morphological and ecological characteristics of a new fern species called Isoëtaceae todaroana which was found in western Sicily, Italy. It mentions two major differences of the species from other known fern species such as the presence of just two leaf chambers and the different shapes of its scales which feature one central spine-like lobe and two lateral rounded lobes. It adds that the species is dedicated to Sicilian botanist Agostino Todaro.

Mediterranean wetlands are severely affected by habitat degradation and related loss of biodiversity. In this scenario, the wide number of artificial farm ponds can play a significant role in the biodiversity conservation of aquatic flora. In the present contribution we show the preliminary results of a study on Mediterranean farm ponds of north-western Sicily (Italy), aimed to investigating the environmental factors linked to the occurrence of submerged macrophytes (vascular plants and charophytes). We studied the aquatic flora of 30 ponds and determined the chemical and isotopic composition of their water bodies on a subset of the most representative 10 sites. Results show that (1) farm ponds host few but interesting species, such as Potamogeton pusillus considered threatened at regional level; (2) Chara vulgaris, C. globularis and P. pusillus behave as disturbance-tolerant species, occurring both in nitrates-poor and nitrates-rich waters, whereas Stuckenia pectinata and Zannichellia palustris occur only in nitrates-poor waters. Although farm ponds are artificial and relatively poor habitats, these environments seem to be important for the aquatic flora and for the conservation of the local biodiversity, and can give useful information for the use of macrophytes as bioindicators in the Mediterranean area. Cytisus aeolicus is a narrow endemic species restricted to the Aeolian archipelago (SE Tyrrhenian Sea, Italy) and it is one of the most evolutionarily isolated plants in the Mediterranean flora. Historical and literature data suggest that both metapopulations and isolated individuals of C. aeolicus are gradually shrinking. Field investigations and drone images demonstrate that the C. aeolicus metapopulation from Stromboli experienced a strikingly fast increase during the last decades. As of 2019, more than 7000 ± 3000 mature individuals occur on Stromboli, i.e. 14 to 20 times more than those counted during the last census, 25 years ago. The diachronic analysis of aerial photos concerning last 80 years and the analysis of the growth rings of some selected plants pointed out that the surface occupied, the demographic structure and the distribution pattern of the subpopulations of Stromboli has been highly fluctuating during last decades. Moreover, data issuing from field observations in permanent plots placed in a transect between two isolated mature individuals showed that, under natural conditions, the germination rate of the seedlings of C. aeolicus can be very high and their establishment rate may exceed 40%. By contrast, seedlings mortality is subject to strong annual fluctuations. Additionally, the pollen morphology of the Strombolian metapopulation of this rare and isolated species is studied here for the first time. Contrary to what is stated in recent literature, the C. aeolicus metapopulation from Stromboli is healthy and very dynamic, albeit frequently damaged by the volcanic activity. Regular and repeated field surveys carried out during 3 years (2017–2019) allowed improving our knowledge on the life cycle of C. aeolicus and a new extinction risk assessment of the species, according to IUCN criteria, is presented. A new species of Brassica sect. Brassica is described here from Sicily (Italy), which is known to be one of the centers of the diversification of wild taxa of this group. The new species (named Brassica tardarae) is restricted to the carbonate cliffs in the Tardara Gorges between Menfi and Sambuca di Sicilia (Agrigento province), an area with a peculiar geological history and where another strictly endemic species was recently described. The morphological relationships between the new species and other similar taxa are discussed, and an analytical key to the Sicilian taxa belonging to the genus Brassica sect. Abstract: The effects of habitat fragmentation on the genetic structure of Ambrosina bassii are analyzed. The species, whose reproductive biology is mostly unknown, is the only representative of its genus and tribe and it is endemic to the central Mediterranean area. The selected study area was the island of Sicily, in which wild populations show a wide morphological variability and ecological amplitude. Patterns of within- and among-population genetic diversity in eleven Sicilian populations, occurring in six disjunct areas, were examined by means of allozyme electrophoresis. High levels of genetic diversity were found as shown by the mean expected heterozygosity (He =0.263), the percentage of polymorphic loci (P95 =65.3), the mean number of alleles per locus (A=2.0). Genetic differentiation between populations was relatively low (mean FST =0.091 and Nm=1.98). A very weak correlation exists between genetic distances and geographic distances between populations. Despite its restricted and fragmented geographical range, A. bassii showed (i) high levels of genetic diversity, mainly within populations; (ii) no genetic differentiation between populations and morphotypes. Investing in Primary Health Care (PHC) is crucial for the future of Public Health (PH), although recent studies highlighted the lack of training in PHC within Italian Residency Schools. This article intends to show the experience and impact of a training course focused on standardizing the knowledge of medical residents in Hygiene and preventive medicine regarding PHC. The strength of the intervention was to demonstrate the feasibility of a unique and homogeneous educational path throughout the national territory. From the results of the evaluation questionnaires, a strong approval of the training experience emerged, with a significant improvement in the perceived knowledge of the subject.

Introduction: The Health District (HD) is a critical component of Italy's National Health Service, responsible for ensuring Primary Health Care (PHC) services in response to community health needs. The Italian government established a national strategic reform program, the National Recovery and Resilience Plan (PNRR), starting in 2022, with a series of health interventions to reorganize the PHC setting, the main reform being the Ministerial Decree 77/2022 (DM77). Our study aimed to provide a description of socio-demographic data and to assess the correlation between HDs, in order to suggest health intervention priorities in PHC reforms., Materials and Methods: We conducted our analysis using a cross-sectional record linkage of data from multiple sources to compare organizational and socio-demographic variables. A dataset was created with each of the 21 Italian Regions' HDs data of population, land area, mean age, ageing index, old-age dependency ratio, birth rate and death rate. The Inland Areas Project data was integrated for a socio-economic perspective., Results: Our study identified comparable groups of HDs, considering demographical, socio-economic and geographical aspects. The study provides a baseline understanding of the Italian situation prior to the implementation of DM77. Background: Biofeedback is the most widespread rehabilitative therapy for the treatment of anismus after failed conservative treatment. Osteopathy represents an alternative therapy for constipation. The aim of this study was to evaluate short- and long-term results of osteopathic treatment as compared to biofeedback in patients with dyssynergic defecation., Methods: This was a prospective cohort pilot study on 30 patients with dyssynergic defecation enrolled at the Colorectal Clinic of the University Hospital of Ferrara, Italy, from May 2015 to May 2016 and followed until May 2020. Dyssynergic defecation was defined as the inappropriate contraction of the pelvic floor or less than 20% relaxation of basal resting sphincter pressure (on anal manometry) with adequate propulsive forces during attempted defecation. Dyssynergic patients were divide into 2 treatment groups: 15 patients had osteopathy and 15 patients had biofeedback. Before and 3 months after rehabilitation treatment, all patients had anorectal manometry, defecography, and ultrasound, and were evaluated with the Cleveland Clinic Florida (CCF) constipation score, obstructed defecation syndrome (ODS) score, Colo-rectal-anal Distress Inventory (CRADI-8), Colo-rectal-anal Impact Questionnaire (CRAIQ-7), and the Brusciano Score (BS). To evaluate the efficacy of osteopathy and biofeedback in the long-term, all patients completed the above-mentioned questionnaires 5 years later via a telephone interview., Results: The two treatments were similarly effective in the short term with reduction in questionnaires scores, and increase in the percentage of anal sphincter release at straining at anorectal manometry in both groups. The ODS score was significantly reduced in biofeedback group (p = 0.021). The 3-month post-treatment BS was lower in the osteopathy group, but this just failed to reach statistical significance (p = 0.050). Periodic rehabilitation reinforcements were provided. The CCF constipation score decreased significantly in the osteopathy group (p = 0.023) after 5 years., Conclusions: Osteopathy is a promising treatment for dyssynergic defecation, and it can be associated with biofeedback.

Background: The objective of this study is to report an 18-year single-center experience in the surgical and endovascular treatment of arterial complications due to self-injection in drug abuser patients., Methods: This retrospective single-center study was conducted analyzing a prospectively collected database including all endovascular or surgical procedures performed from January 2007 to December 2019 for any arterial complication due to self-injection in drug abuser patient. Collected data were patient demographic and comorbidity, site and type of arterial lesion (pseudoaneurysm [PA], arteriovenous fistula [AVF]), signs of systemic or local infection, and procedural data (endovascular/surgical treatment). End points were rate of postoperative complications, reintervention rate, limb salvage, and patients' early and long-term survival., Results: In 11 patients (median age 36 years, range 27-47; male 73%), 13 arterial lesions were treated: 10 (77%) PA, 2 (15%) PA associated with AVF, and 1 (8%) isolated AVF. Arterial lesion involved common femoral artery in 5 (38%), superficial femoral artery in 4 (31%), profunda femoral artery in 1 (8%), brachial artery in 2 (15%), and subclavian artery in 1 (8%). Signs of infections were present in 9 of the 13 cases (69%). The treatment was surgical in 11 (85%) cases: 7 interposition graft (6 great saphenous vein, 1 arterial cryopreserved homograft), 2 direct reconstruction, 1 patch plasty with pericardium bovine patch, and 1 arterial ligation. Endovascular treatment was performed in 2 cases: 1 noninfected PA of the superficial femoral artery, and 1 55-mm PA of the postvertebral segment of the right subclavian artery with clinical sign of hemodynamic instability. At 1 month, postoperative complication rate was 8% (one lower limb claudication after superficial femoral artery ligation). Reintervention rate was 8% (interposition graft rupture for repeated self-injections). Limb salvage and patient survival were both 100%. Median follow-up was 5 years (range 1 month to 11.3 years); surgical group: median 8.2 years (range 2 months to 11.3 years); endovascular group: median 3.5 months (range 1-6). During follow-up, neither complications nor reinterventions occurred, and limb salvage was 100% for both groups. At 2, 4, and 6 years, overall estimated patient survival was 91%, 81%, and 81%, respectively, with no procedure-related death., Conclusions: After surgical or endovascular management of arterial lesions due to self-injection in drug abuser patients, complications occur mainly in the postoperative period. During follow-up, the surgical procedures have low rate of complications, reinterventions, and procedure-related mortality, whereas for the endovascular treatment the mid-term outcomes remain unknown., (Copyright © 2020 Elsevier Inc.
