Uber is a regulatory entrepreneur: a company that seeks to change the law in its favor. In the United States, Uber's regulatory entrepreneurship changed the American taxi industry and the laws that regulate it. But the company's venture into France presented unique challenges. This Comment examines Uber's regulatory entrepreneurship in France and analyzes the factors that hindered Uber's quest to revolutionize the French taxi industry. It discusses the tension between Uber and the French government and the policies behind both sides' goals. It also seeks a compromise between Uber and the French regulators: one that acknowledges the issues with both Uber's aggressive business model and France's heavy government regulations. Introduction: L’HAS recommande un dépistage régulier des complications du diabète. D’après l’enquête ENTRED 2007, seuls 28% des patients ont bénéficié du dosage de la microalbuminurie, 50% d’une consultation ophtalmologique, 61% d’une gradation podologique. Matériels et méthodes: Dans le cadre du programme DIABSAT, un véhicule muni d’une antenne satellite et équipé d’un rétinographe non mydriatique, d’un appareil de mesure de la microalbuminurie, d’un monofilament, d’un stétho-doppler, d’un enregistreur des pressions plantaires a sillonné trois départements de la région Midi-Pyrénées. Une infirmière a réalisé auprès des diabétiques n’ayant pas un suivi régulier les examens. Ces données transmises par satellite sont interprétées à distance par des ophtalmologistes, diabétologues et pédicure-podologues référents et les comptes-rendus sont envoyés par courrier aux patients, à leurs médecins traitants et diabétologues. Résultats: En moyenne, 5 (+/– 3) patients sont dépistés par journée. L’âge moyen des patients est de 70 ans (+/– 10). 59.2% sont des hommes. 95.6% ont un DT2 avec une ancienneté moyenne de 10,9 ans (+/– 9.7). 100% des patients sont suivis par leur médecin généraliste, 22.2% par un diabétologue. La mesure des IPS a été réalisée chez 92.1% des patients (15.7% pathologiques); le dépistage de la neuropathie des membres inférieurs chez 95.2% des patients (15.3% pathologiques); l’enregistrement des pressions plantaires chez 92.3% des patients. 16.5% des personnes dépistées peuvent bénéficier d’un remboursement des soins podologiques (10,6% : grade 2; 5.9% : grade 3). La microalbuminurie a été réalisée sur 61.3% des patients (25.3% pathologiques) et la rétinographie chez 50,1% des patients (18.4% de rétinopathies diabétiques, 26.9% de maculopathies ou autres anomalies). Conclusion: Ces résultats montrent qu’il existe un réel besoin de dépistage dans des zones isolées pour améliorer le suivi du diabète selon les recommandations de l’HAS, optimiser l’action des réseaux de soins et tenter de réduire le nombre de déplacements onéreux. Objectif: Comme une étude pilote avait précédemment montré la présence d’une population de Staphylococcus aureus colonisante appartenant aux complexes clonaux CC5 et CC8 sur les plaies du pied chez le diabétique, le but de la présente étude a été d’évaluer la prévalence de ces souches en France. Patients et méthodes: Les patients adultes diabétiques (type 1 et 2), présentant une plaie du pied (grades 1–4; SPILF 2007) avec S. aureus comme seul pathogène ont été inclus du 1/04/08 au 30/06/10 dans 12 CHU français. Tous les patients avec plaie de grade 1 (non infectée) ont eu une consultation de suivi dans le mois suivant au cours de laquelle un 2e prélèvement a été effectué. L’origine clonale des souches a été détectée par biopuces à ADN (StaphyType96®, Alere). Résultats: 195 patients ont été inclus (71% d’hommes, 75 plaies de grade 1) et 223 souches de S. aureus ont été isolées dont 35 méthicillino-résistantes lors de la consultation inaugurale ou de suivi. À l’inclusion, 44 (59%) souches isolées de plaies de grade 1 appartenaient aux clones CC5/CC8 contre 6 (5%) dans les plaies infectées (grades 2–4) (p<0,001). Lors du suivi des plaies de grade 1, a) 57 (76%) ont cicatrisé ou évolué favorablement et 49/57 (86%) présentaient une souche appartenant aux clones CC5/CC8; b) 18 (24%) se sont aggravées et aucune ne présentait une souche des clones CC5/CC8. Conclusion: Cette étude démontre l’existence en France d’une population de S. aureus colonisante présente sur les plaies du pied chez le diabétique. L’identification de ces souches permet de distinguer les colonisations (grade 1) des infections (grades 2–4), prédire le devenir de la plaie et devrait contribuer à un meilleur usage des antibiotiques dans la prise en charge des plaies du pied chez le diabétique. The Diabsat programme, coordinated by the Midi-Pyrénées diabetes network (Diamip), uses new technologies to enable patients to update the tests for chronic complications of their diabetes. In the long run, this favours their health prognosis and their quality of life.

Objective: To discover the incidence of pain in patients with acute or chronic wounds of various causes during dressing removal, and the effect of switching to a non-adherent dressing., Method: A total of 656 primary care physicians reported the relevant details of all acute or chronic wounds observed during routine visits throughout the study period. The pain experienced during dressing changes was systematically evaluated. In patients with moderate to severe pain, a more extensive evaluation was performed and they were invited to complete a self-evaluation questionnaire. If the patients were seen at a subsequent visit, a new evaluation was performed., Results: In total 5850 patients were seen: 2914 with acute wounds and 2936 with chronic wounds. During dressing changes, a similar number of patients with acute and chronic wounds reported 'moderate to severe' pain during the medical screening visit (79.9% and 79.7%) and 'very severe' pain in their self-evaluation questionnaire completed at home (47% and 59% respectively). Dressing removal was most painful when there was adherence to the wound bed. Switching to a new, non-adherent dressing reduced pain during dressing changes in 88% of patients with chronic wounds and 95% of patients with acute wounds., Conclusion: This study demonstrates that similar problems with patient acceptability arise irrespective of wound aetiology. Pain is a major problem and is most often related to dressing selection. Selecting a suitable, non-adherent dressing improves patient acceptability.

Diabetic foot ulcers are a frequent and severe complication of diabetes mellitus. A multidisciplinary approach (in-patient and out-patient care) improves prognosis and reduces the amputation rate. Risk factors are well defined and easily identified. High-risk patients (sensory loss, vascular disease, previous ulcer, foot deformities) have to be detected. In France, comprehensive foot-care programs, including education and regular foot examination, must be developed. The reduction of the economical and human burden needs the urgent formation of more multidisciplinary teams (to take care of patients properly and very early), foot care networks, and prevention programs.

To assess the cost-effectiveness of Dermagraft(R) (human dermal replacement) in the treatment of the diabetic foot ulcer, compared to standard treatment. A Markov model was developed, to simulate, over a 52-week period, the health status of a cohort of 100 patients with a diabetic foot ulcer treated either with conventional therapy or with Dermagraft(R). The considered health states were: healed, same site recurrence, unhealed not infected, cellulitis, osteomyelitis, amputation and death. Each week, the patient may progress among states according to a set of transition probabilities directly derived from the original clinical trial conducted in the USA. The cost of each health state was estimated by a Delphi panel of French diabetologists (direct costs only, valuated from a societal perspective). A sensitivity analysis was performed. The total number of healed ulcers included first ulcers healed (76.38% for Dermagraft(R) vs. 69.35% for standard treatment; median time to heal is 14-15 weeks for Dermagraft(R) compared with 28-29 weeks for standard treatment) plus recurrences which are subsequently healed within the 52-week period (14.29 for Dermagraft(R) vs. 25.09 for standard treatment; median time to heal is 3-4 weeks for Dermagraft(R) compared with 5-6 weeks for standard treatment). The average expected cost per treated patient (C/E) using standard treatment for the considered 52-week period is 47,418 FF vs. 54,384 FF for Dermagraft(R) (including 18,200 FF for Dermagraft(R) acquisition and 36,184 FF for standard treatment). Because Dermagraft(R) heals more ulcers within 52 weeks, the average cost per healed ulcer is lower (53,522 FF vs. 56,687 FF for standard treatment). The incremental cost-effectiveness ratio of Dermagraft(R) (DeltaC/DeltaE) equals 38,784 FF, indicating the extra investment that the decision-maker has to accept for an additional ulcer healed with Dermagraft(R) compared with conventional treatment.
