Unintentional injury represents a significant health threat to children, and infancy marks a particularly vulnerable stage. This multi-method study (questionnaire, diary) measured parents' (N = 143) use of three popular home-safety practices (teaching about safety, environment modification to reduce access to hazards, supervision) and child injury rates at two stages of motor development during infancy (sitting, walking). Associations between these three safety practices and parental beliefs (protectiveness needed, perceived benefits of the child experiencing minor injuries) were examined, as was the effectiveness of these three practices to prevent injury. Results revealed that different parental beliefs were associated with implementing different safety strategies at each motor development stage. Strategies were differentially effective depending on mobility status of the infant, with supervision being the only strategy that was effective to prevent injury at both motor development stages. Implications for developing safety messages to promote parents' injury-prevention strategies are discussed.

In this article, we examine the CBC/Radio-Canada series, "Canada: A People's History," for its use of empathy, specifically with regard to its portrayal of Aboriginal people. We call the empathy promoted in the series, emotive empathy, and compare it to the concept of historical empathy constructed by researchers in history education. The emotive empathy employed in this series, while adequate for public audiences, is not sufficient for history classrooms because it lacks a cognitive dimension. We discuss implications for using the series, and by extension, other instructional resources, to promote the development of historical empathy. (Contains 2 figures.)

The article examines why the New Democratic Party (NDP) is an impediment to social progress in Canada. Topics include socialism, social democracy, and the demise of the left; components of the Leaf manifesto issued by a coalition of various groups of environmentalists, Indigenous, labor, faith leaders, and prominent authors that called for a series of significant changes to Canadian society; and the disqualification of Anjali Appadurai's leadership bid for the British Columbia NDP.

This article illuminates major features of high-quality leadership programmes across different education systems. We do so by focusing on capturing commonalities and variations in high-quality pre-service programmes from five differing societies, all of which are high-performing education systems. To this end, we first delineate key profiles of each programme. Based on that, we discuss commonalities and variations in leadership programmes in terms of framework, content and operational features. Reducing flood risk through disaster planning and risk management requires accurate estimates of exposure, damage, casualties, and environmental impacts. Models can provide such information; however, computational or data constraints often lead to the construction of such models by aggregating high‐resolution flood hazard grids to a coarser resolution, the effect of which is poorly understood. Through the application of a novel spatial classification framework, we derive closed‐form solutions for the location (e.g., flood margins) and direction of bias from flood grid aggregation independent of any study region. These solutions show bias of some key metric will always be present in regions with marginal inundation; for example, inundation area will be positively biased when water depth grids are aggregated and volume will be negatively biased when water surface elevation grids are aggregated through averaging. In a separate computational analysis, we employ the same framework to a 2018 flood and successfully reproduce the findings of our study‐region‐independent derivation. Extending the investigation to the exposure of buildings, we find regions with marginal inundation are an order of magnitude more sensitive to aggregation errors, highlighting the importance of understanding such artifacts for flood risk modelers. Of the two aggregation routines considered, averaging water surface elevation grids better preserved flood depths at buildings than averaging of water depth grids. This work provides insight into, and recommendations for, aggregating grids used by flood risk models. To support primary care physicians (PCPs) and their patients with knee osteoarthritis (OA), we created a series of evidence-based whiteboard educational videos for patients with knee OA. In a previous study we piloted these videos with patients to understand their acceptability and impact. The purpose of this study was to explore PCPs' perspectives to understand the utility of novel patient education videos to support management. We conducted a qualitative descriptive study using semi-structured interviews of newly practicing and resident PCPs after watching a series of patient education videos. A thematic analysis approach was used combining both inductive and research question driven coding, category formation, and theme identification. Ten participants were included. Barriers to optimal management were identified including the challenge of patient adherence and access to non-operative treatments. PCPs indicated that the videos would support their management of patients with knee OA by (1) supporting credibility and building trust with their patient, (2) reinforcing patient understanding, and (3) enhancing their own management of patients. Future implementation of these resources with attention to barriers that may limit uptake is necessary and may optimize management of knee OA in primary care. The provision of patient educational materials may support patient buy-in regarding appropriate management, enhance patient understanding, and improve physicians' future practice. Viewing these videos could potentially improve physicians' future resource use, recommendation of non-operative treatments, and the quality of their total knee replacement referrals, which has benefits to both the patient and the healthcare system. Perceived barriers to implementing evidence-based recommendations may pose a challenge for patients and clinicians and should be considered to help facilitate the uptake of educational interventions. Since 1996, we have been working to have Canadian governmental authorities implement health-promoting public policy that would improve the quality and equitable distribution of the social determinants of health, with rather little to show for our efforts. Canada's RADARSAT missions improve the potential to study past flood events; however, existing tools to derive flood depths from this remote-sensing data do not correct for errors, leading to poor estimates. To provide more accurate gridded depth estimates of historical flooding, a new tool is proposed that integrates Height Above Nearest Drainage and Cost Allocation algorithms. This tool is tested against two trusted, hydraulically derived, gridded depths of recent floods in Canada. This validation shows the proposed tool outperforms existing tools and can provide more accurate estimates from minimal data without the need for complex physics-based models or expert judgement. Objective: To translate and validate the Child Three-Factor Eating Questionnaire (CTFEQr17), assessing cognitive restraint (CR), uncontrolled eating (UE) and emotional eating (EE), among French-speaking Canadian young individuals.Design: Phase 1 comprised a translation and the evaluation of the comprehension of the questionnaire. Phase 2 comprised a confirmatory factor analysis (CFA), the evaluation of internal consistency (Cronbach's α), test-retest reliability (intra-class correlation coefficients (ICC)) and construct validity, including correlations among the CTFEQr17 and Eating Attitudes Test (EAT-26), anthropometrics, dietary intake and diet quality.Setting: Primary and secondary schools, Québec City, Canada.Participants: Phases 1 and 2 included 20 (40 % boys, mean age 11·5 (sd 2·4) years) and 145 (48 % boys, mean age 11·0 (sd 1·9) years) participants, respectively.Results: Phase 1 resulted in the questionnaire to be used in Phase 2. In Phase 2, the CFA revealed that the seventeen item, three-factor model (CTFEQr17) provided an excellent fit. Internal consistency was good (Cronbach's α: 0·81-0·90). Test-retest reliability was moderate to good (ICC = 0·59, (95 % CI 0·48, 0·70), ICC = 0·78, (95 % CI 0·70, 0·84), ICC = 0·50, (95 % CI 0·38, 0·62) for CR, UE and EE, respectively). CR correlated with EAT-26 score (r = 0·43, P < 0·0001). UE and EE correlated negatively with BMI Z-scores (r = -0;·26, P = 0·003; r = -0;·19, P = 0·03, respectively). CR correlated with the proportion of energy intake from protein and diet quality (r = 0·18, P = 0·04; r = 0·20, P = 0·02, respectively).Conclusion: The CTFEQr17 is suitable to use among French-speaking Canadian young individuals. Exploitation of mineable oil sands in Canada has thus far produced over 50 km2 of tailings in ponds where the fine solids remain suspended for years, even decades. These ponds represent a major liability and environmental burden for operating companies, with their dewatering critical to their remediation. In this work, we used the chemistry of the tailing pond constituents to design an ionic liquid (IL) for dewatering the mature fine tailings (MFT), taking advantage of the ability to introduce unique and different functionalities into each ion of the IL. We chose a class of protic ILs whose ions will interact with the solids while rejecting water. As an example, octylammonium oleate can be used to remove more than 70% of the water from MFT. The nature of the ILs makes their use amenable to a variety of large‐scale separation techniques allowing engineering versatility in the design of the final process. [ABSTRACT FROM AUTHOR]

Progress in reducing health inequalities through public policy action is difficult in nations identified as liberal welfare states. In Canada, as elsewhere, researchers and advocates provide governing authorities with empirical findings on the sources of health inequalities and document the lived experiences of those encountering these adverse health outcomes with the hope of provoking public policy action. However, critical analysis of the societal structures and processes that make improving the sources of health inequalities difficult—the quality and distribution of living and working conditions, that is the social determinants of health—identifies limitations in these approaches. Within this latter critical tradition, we consider—using household food insecurity in Canada as an illustration—how polemics and anger mobilization, usually absent in health inequalities research and advocacy—could force Canadian governing authorities to reduce health inequalities through public policy action. We explore the potential of using high valence terms such as structural violence, social death and social murder, which make explicit the adverse outcomes of health‐threatening public policy to force government action. This article focuses on Canada's liberal welfare state and the COVID-19 pandemic, offering an overview of some of the unequal health and social effects of the crisis. It argues that the policy response to the pandemic should be situated within a broader pattern of welfare restructuring and organization that serves to instrumentalize economic insecurity and extend labor discipline. Without making firm predictions about the future, we argue that this is likely to reproduce increasingly unequal patterns of welfare access, providing benefits to some constituents while disciplining vulnerable groups to facilitate competitive gains. Cancer screening is an important component of a cancer control strategy. Indigenous people in Canada have higher incidence rates for many types of cancer, including those that can be detected early or prevented through organized screening programs. Increased participation and retention in cancer screening is critical to improved population health outcomes amongst Indigenous people. This rapid review evaluates cancer screening interventions published in the last six years. Included studies demonstrated increased participation in breast, colorectal, or cervical cancer screening programs in Indigenous populations or showed promise of increased participation based on the factors that influence people’s screening practices, such as knowledge, attitude, or intent to screen. The Preferred Reporting Items for Systematic Reviews guided the search strategy. The review identified 85 articles with 12 meeting the specified criteria: seven studies reported an increase in cancer screening participation and five studies reported improved knowledge, attitude, or intent to screen. The use of multiple culturally appropriate strategies in co-designed studies were the most effective. This review will be used to inform First Nations (FN) populations and Screening Programs in Alberta of potential strategies to address disparities identified through a recent data analysis comparing cancer screening and outcomes between FN and non-FN people. Health research in Canada carries a history of exploitation and cultural insensitivity in its approaches, resulting in a deeply rooted mistrust among Indigenous Peoples. Communities are signalling the urgent need for health research to be conducted in a more conscientious way. To address these gaps, our team of three Elders and three researchers co-developed a series of workshops in the province of British Columbia, Canada, in 2019 to increase participants' knowledge and skills for conducting culturally responsive health research. Workshops examined power and privilege; self-awareness/self-reflection; Indigenous and Western worldviews; cultural safety; allyship; and research principles and practices. Activities were experiential and privileged Indigenous knowledges. An Indigenous-informed evaluation captured participants' experiences via online surveys. Participants described workshop learnings as deeply impacting ways they approach research practice. Thematic analysis of participant reflections revealed three overarching themes: bringing together the mind and the heart; self-reflection and initiating change; and understanding cultural safety as a lifelong journey. Lessons learned by facilitators included the importance of applying Indigenous ways of knowing to create safe spaces for healing and learning; empowering participants to critically self-reflect; and rooting the work in ceremony. The ethical responsibility to allow time and space for meaningful dialogue was crucial for aligning with Indigenous protocols of coming together. This project demonstrates that experiential workshops, cofacilitated by Elders and researchers, are an innovative, effective, and Indigenous-centred approach for providing education on how to engage in culturally safe and culturally resonant research. Background: Smoking by cancer patients and survivors causes adverse cancer treatment outcomes, but little information is available about how smoking can affect cancer treatment costs. Methods: We developed a model to estimate attributable cancer treatment failure because of continued smoking after a cancer diagnosis (AFS). Canadian health system data were used to determine the additional treatment cost for afs for the most common cancers in Canada. Results: Of 206,000 patients diagnosed with cancer annually, an estimated 4789 experienced afs. The annual incremental cost associated with treating patients experiencing AFS was estimated at between $198 million and $295 million (2017 Canadian dollars), reflecting an added incremental cost of $4,810-$7,162 per patient who continued to smoke. Analyses according to disease site demonstrated higher incremental costs where the smoking prevalence and the cost of individual second-line cancer treatment were highest. Of breast, prostate, colorectal, and lung cancers, lung cancer was associated with the highest incremental cost for treatment after AFS. Conclusions: The costs associated with AFS in Canada after a cancer diagnosis are considerable. Populations in which the smoking prevalence and treatment costs are high are expected to benefit the most from efforts aimed at increasing smoking cessation capacity for patients newly diagnosed with cancer. Background: Little research has focused on implementation of electronic Patient Reported Outcomes (e-PROs) for meaningful use in patient management in 'real-world' oncology practices. Our quality improvement collaborative used multi-faceted implementation strategies including audit and feedback, disease-site champions and practice coaching, core training of clinicians in a person-centered clinical method for use of e-PROs in shared treatment planning and patient activation, ongoing educational outreach and shared collaborative learnings to facilitate integration of e-PROs data in multi-sites in Ontario and Quebec, Canada for personalized management of generic and targeted symptoms of pain, fatigue, and emotional distress (depression, anxiety). Patients and methods: We used a mixed-methods (qualitative and quantitative data) program evaluation design to assess process/implementation outcomes including e-PROs completion rates, acceptability/use from the perspective of patients/clinicians, and patient experience (surveys, qualitative focus groups). We secondarily explored impact on symptom severity, patient activation and healthcare utilization (Ontario sites only) comparing a pre/post population cohort not exposed/exposed to our implementation intervention using Mann Whitney U tests. We hypothesized that the iPEHOC intervention would result in a reduction in symptom severity, healthcare utilization, and higher patient activation. We also identified key implementation strategies that sites perceived as most valuable to uptake and any barriers. Results: Over 6000 patients completed e-PROs, with sites reaching 51%–95% population completion rates depending on initial readiness. e-PROs were acceptable to patients for communicating symptoms (76%) and by clinicians for treatment planning (80%). Patient experience was better than the provincial average. Compared to the pre-population, we observed a significant reduction in levels of anxiety (p = 0.008), higher levels of patient activation (p = 0.045), and reduced hospitalization rates (12.3% not exposed vs 10.1% exposed, p = 0.034). A pre/post population trend towards significance for reduced emergency department visit rates (14.8% not exposed vs 12.8% exposed, p = 0.081) was also noted. Conclusion: This large-scale pragmatic quality improvement project demonstrates the impact of implementation strategies and a collaborative improvement approach on acceptability of using PROs in clinical practice and their potential for reducing anxiety and healthcare utilization; and improving patient experience and patient activation when implemented in 'real-world' multi-site oncology practices. Previous treatments of Samuel Douglas Huyghue's Argimou: A Legend of the Micmac (1842) have foregrounded the humanitarian theme that surfaces periodically in the novel. This essay explores Argimou as a work of absolution writing – writing that justifies and even furthers the physical marginalization and elimination of Indigenous peoples in their homelands while also relieving future generations of Settlers of any feelings of guilt or complicity that they might otherwise bear. Huyghue's novel models at least three key features of this genre. First, it naturalizes Indigenous genocide by creating a cultural rival who is self-exterminating; second, it undermines treaty; and third, it creates a clear path to historical disavowal, encouraging readers from the colonizing culture to superficially self-identify as social progressives and as good and faithful allies to Indigenous peoples. Total joint arthroplasties (TJA) are successful procedures for the treatment of end-stage hip and knee arthritis. Length of stay in hospitals after these procedures has been steadily decreasing over time, with outpatient procedures (discharge on the same day as surgery) introduced in the US within the last 20 years. Reducing length of stay after TJA can provide cost savings. Centres in Canada have started to utilize outpatient TJA procedures, but we have identified some barriers that may have limited their implementation. We have summarized the current literature for outpatient TJA and discussed potential solutions for the current barriers. [ABSTRACT FROM AUTHOR]

In order to examine the effects of interrogative strategies in promoting children's associative learning, children five and six years of age, as well as sixth-grade children, were first presented with a variety of picture-paired associates and then tested for the ability to memorize them. (MP)

A scoping review of published literature and dialogue with international nurse practitioner educators and researchers revealed the education of nurse practitioner students varied within and between countries. This lack of cohesiveness hinders nurse practitioner role development and practice nationally and internationally. A rapid review of grey literature was conducted on nurse practitioner education standards in six countries (Australia, Canada, Finland, Norway, the Netherlands, and USA). Data were extracted from graduate level nurse practitioner education programs' websites from each country (n = 24). Extracted data were verified for accuracy and completeness with a nurse practitioner educator from each program. Data were analyzed using content analysis. Variations in nurse practitioner education within and between countries were explored by comparing admission criteria, curricular content, clinical requirements, teaching methods, and assignment and evaluative methods. The findings will help inform education programs and further research about nurse practitioner education internationally. [ABSTRACT FROM AUTHOR]

There is little doubt that the implementation of a Basic Income Guarantee (BIG) in Canada and other liberal welfare states would alleviate some of the most egregious examples of absolute poverty that contribute to poor health such as lack of adequate food and shelter and inability to meet basic household and personal needs. BIG would likely improve the health of the most disadvantaged by moving them closer to the relative poverty line. Yet, advocacy for and implementation of BIG carries potential dangers. Since health improves with every step up the income ladder, simply moving people closer to the relative poverty line without providing additional universal benefits and supports common to most other developed nations would limit its health promotion potential. In addition, governing authorities in liberal political economies can use BIG to justify continuing imbalances in economic and political power that skews the distribution of the social determinants of health. In addition, implementation of BIG -- despite its more progressive advocates calls for maintaining or enhancing of existing social programs – can serve as justification for reducing or removing these programs, thereby threatening health. In this paper, we explore how corporate domination of two major disease associations in Canada, Heart and Stroke Canada (HSC) and Diabetes Canada (DC), as manifested in membership of their boards of directors may be acting with biomedical complicity to create hegemonic discourse on the nature of cardiovascular disease (CVD) and type 2 diabetes mellitus (T2DM). This is problematic as the activities that derive from this discourse thwart public policy action to address the primary causes and means of managing chronic disease: Canadians' living and working conditions. Through critical analysis of the membership of HSC and DC boards of directors, we link their corporate and biomedical backgrounds with the limiting of chronic disease association messaging to narrow discredited behavioural approaches. We also draw attention to other means by which the corporate sector is able to shape disease association discourse on the causes and means of managing chronic disease. To rectify this, we call for membership of these boards to include those knowledgeable with broader understandings of health and those most likely to suffer CVD and T2DM: the poor, excluded, and marginalised. Background The human papillomavirus (hpv) is a common sexually transmitted infection and a primary cause of cervical cancer. The Government of Canada has set a target of reaching 90% hpv vaccine coverage among adolescents by 2025. Here, we examine hpv vaccine uptake in school-based immunization programs across Canada and explore how achieving the 90% target could affect the future incidence of cervical cancer, mortality, and health system expenditures in a cohort of Canadian women. Methods Data for hpv vaccine uptake in the most recent reported school year available in each jurisdiction were provided in 2017 by jurisdictional school-based immunization programs and were used to estimate a national weighted average of 67%. The OncoSim microsimulation model (version 2.5) was used to compare 3 different levels of hpv vaccine uptake (0%, 67%, 90%) on health and economic outcomes for a hypothetical cohort of all 5- to 10-yearold girls in Canada in 2015. Results Vaccine uptake for girls in school-based programs varied from 55.0% to 92.0% in the jurisdictions reviewed. The OncoSim model projects that increasing uptake to 90% from 67% would result in a 23% reduction in cervical cancer incidence rates (to 3.1 cases from 4.0 cases per 100,000, averaged across the lifetime of the cohort) and a 23% decline in the average annual mortality rate (to 1.0 deaths from 1.3 deaths per 100,000). Finally, the model projects that the health system will incur a cost of $9 million (1% increase) during the lifetime of the cohort if uptake is increased to 90% from 67%. Costs are discounted (1.5%) and expressed in 2016 Canadian dollars. Costs reflect the payer perspective. Conclusions Our model shows that increasing hpv vaccine uptake to 90% from current levels for girls in schoolbased immunization programs could result in substantial reductions in the future incidence and mortality rates for cervical cancer in Canada. The welfare state literature on developing nations is concerned with how governmental illegitimacy and incompetency are the sources of inequality, exploitation, exclusion, and domination of significant proportions of their citizenry. These dimensions clearly contribute to the problematic health outcomes in these nations. In contrast, developed nations are assumed to grapple with less contentious issues of stratification, decommodification, and the relative role of the state, market, and family in providing economic and social security, also important pathways to health. There is an explicit assumption that governing authorities in developed nations are legitimate and competent such that their citizens are not systematically subjected to inequality, exploitation, exclusion, and domination by elites. In this article, we argue that these concepts should also be the focus of welfare state analysis in developed liberal welfare states such as Canada. Such an analysis would expose how public policy is increasingly being made in the service of powerful economic elites rather than the majority, thereby threatening health. To assess the extent to which Canadian undergraduate baccalaureate nursing programs have incorporated Canadian competencies for Registered Nurses in primary care into their curricula. Canadian competencies for Registered Nurses in primary care have several benefits, including their ability to inform primary care education in undergraduate nursing programs and to assist in building a robust primary care nursing workforce. We conducted a national cross-sectional survey of undergraduate baccalaureate nursing programs (n = 74). The survey was conducted between April-May 2022. We used a modified version of the "Community Health Nurses' Continuing Education Needs Questionnaire". Respondents indicated their level of agreement on a 6-point Likert scale with 47 statements about the integration of the competencies in their program (1 = strongly disagree; 6 = strongly agree). The response rate was 51.4%. The overall mean across the six competency domains was 4.73 (SD 0.30). The mean scores of each domain ranged from 4.23 (SD 1.27) for Quality Assurance, Evaluation and Research to 5.17 (SD 0.95) for Communication. There are gaps in how these competencies are included in undergraduate education programs and opportunities to strengthen education for this growing workforce in Canada. [ABSTRACT FROM AUTHOR]

Background: Health systems worldwide struggle to identify, adopt, and implement in a timely and system-wide manner the best-evidence-informed-policy-level practices. Yet, there is still only limited evidence about individual and institutional best practices for fostering the use of scientific evidence in policy-making processes The present project is the first national-level attempt to (1) map and structurally analyze-quantitatively-health-relevant policy-making networks that connect evidence production, synthesis, interpretation, and use; (2) qualitatively investigate the interaction patterns of a subsample of actors with high centrality metrics within these networks to develop an in-depth understanding of evidence circulation processes; and (3) combine these findings in order to assess a policy network's "absorptive capacity" regarding scientific evidence and integrate them into a conceptually sound and empirically grounded framework.Methods: The project is divided into two research components. The first component is based on quantitative analysis of ties (relationships) that link nodes (participants) in a network. Network data will be collected through a multi-step snowball sampling strategy. Data will be analyzed structurally using social network mapping and analysis methods. The second component is based on qualitative interviews with a subsample of the Web survey participants having central, bridging, or atypical positions in the network. Interviews will focus on the process through which evidence circulates and enters practice. Results from both components will then be integrated through an assessment of the network's and subnetwork's effectiveness in identifying, capturing, interpreting, sharing, reframing, and recodifying scientific evidence in policy-making processes.Discussion: Knowledge developed from this project has the potential both to strengthen the scientific understanding of how policy-level knowledge transfer and exchange functions and to provide significantly improved advice on how to ensure evidence plays a more prominent role in public policies. STUDY DESIGN: Delphi panel study. BACKGROUND: Biomechanical parameters measured during a drop vertical jump task are risk factors for anterior cruciate ligament (ACL) injury and are targeted during rehabilitation after ACL reconstruction. A clinically feasible tool that quantifies observed performance on the drop vertical jump would help inform treatment efforts. The content and scoring of such a tool should be deliberated on by a group of experts throughout its development. OBJECTIVES: To establish consensus on the content and scoring of a clinician-rated drop vertical jump scale (DVJS) for use during rehabilitation after ACL reconstruction. METHODS: Using a modified Delphi process, a panel of experts (researchers and clinicians) on the risk factors, prevention, treatment, and biomechanics of ACL injury anonymously critiqued versions of a DVJS. The DVJS was developed iteratively, based on the feedback from the panel, using Likert scale responses to questions and providing written comments. Three to 5 rounds were planned a priori, with a requirement of 75% agreement on included items after the final round. RESULTS: Twenty of the 31 invited experts (65%) participated. Approximately 93% agreement was achieved after the fourth round. Final items on the scale included the rating of knee valgis collapse (no collapse to extreme collapse) and the presence of other undesirable movements, including lateral trunk lean, insufficient knee flexion, and limb-to-limb asymmetry. CONCLUSION: The Delphi process resulted in a beta version of a DVJS. Expert consensus was achieved on its content and scoring to support further clinical testing of the scale. [ABSTRACT FROM AUTHOR]

Acute care hospitals have a role in managing the health care needs of people affected by cancer when they are at the end of life. However, there is a need to provide end-of-life care in other settings, including at home or in hospice, when such settings are more appropriate. Using data from 9 provinces, we examined indicators that describe the current landscape of acute care hospital use at the end of life for patients who died of cancer in hospital in Canada. Interprovincial variation was observed in acute care hospital deaths, length of stay in hospital, readmission to hospital, and intensive care unit use at the end of life. High rates of acute care hospital use near the end of life might suggest that community and home-based end-of-life care might not be suiting patient needs. [ABSTRACT FROM AUTHOR]

Introduction: The UN Declaration on the Rights of Indigenous Peoples states that indigenous people have a fundamental right to contribute to the management of the resources that support their livelihoods. Salmon are vital to the economy and culture of First Nations in coastal British Columbia, Canada. In this region, traditional systems of management including weirs – fences built across rivers to selectively harvest salmon – supported sustainable fisheries for millennia. In the late-19th century traditional fishing practices were banned as colonial governments consolidated control over salmon. Outcomes: In collaboration with the Heiltsuk First Nation we revived the practice of weir building in the Koeye River. Over the first four years of the project we tagged 1,226 sockeye, and counted 8,036 fish during fall stream walks. We used a mark-recapture model which accounted for both pre-spawn mortality due to variation in temperature, and tag loss, to produce the first mark-resight estimates of sockeye abundance in the watershed (4,600 – 15,000 escapement). Discussion: High river temperatures are associated with increased en route morality in migrating adult sockeye. We estimated pre-spawn mortality ranged from 8 – 72% across the four years of study, highlighting the degree to which climate conditions may dictate future viability in sockeye salmon populations. There are differing interpretations among health researchers about how income inequality leads to health inequalities, but there is consensus that income inequality is cause for concern. Alarms have also been sounded by public policy researchers and institutes, which see income inequality as having a corrosive effect on society by hindering economic growth, dampening political participation, and eroding quality of life. This article examines how various public policy models explain and promote policy change to reduce income inequality. The focus of this article is on income inequality, but its raison d'être is the assumption that income inequality is an important driver of the health inequalities that both diminish and prematurely end life. Research has identified a wide range of factors that influence the health of children and their health as adults. Key among these are early childhood conditions, care, and education, housing security and income security. A number of indicators have been developed to measure the health of children such as the incidences of infant mortality and low birth weight. In international comparisons with other rich countries, Canada ranks poorly. These health outcomes are not inevitable but are shaped by the quality and distribution of a wide range of social determinants. Public policies shape access to and quality of the social determinants of children's health (SDCH). Because the SDCH are amenable to public policy, it is important to understand public policy and the public policy development process in order to promote policy change. Yet few health practitioners who work with children consider or understand how the public policy development process works. This article examines three models of public policy development: pluralism, policy paradigms, and political economy. It will be argued that of the three models, political economy, with its focus on the role of political ideology and power distribution, provides the most compelling explanation of the public policy development process and provides means to bring about policy change to improve the health of children. Chemical enhanced oil recovery techniques have been suggested as efficient alternatives to thermal methods in many thin/small viscous oil reservoirs in western Canada. These acidic oil reservoirs have been screened as good candidates for alkaline (A) and surfactant (S) floods. In this study, sand-pack core flooding experiments and pore-scale microfluidic tests were designed to give some new insights on the mechanisms of AS floods augmented with polymer (P) solutions in viscous oil systems. Viscous oil samples from Luseland field (14,850 mPa s at 25 °C) was used in all the experiments. The most efficient chemical generates ultralow interfacial tension, 0.002 mN/m, producing low viscosity type II (+) emulsion (740 mPa s, which is much lower than the viscosity of originating oil). The two pore volumes injection of this cocktail results in around 20% OOIP incremental oil recovery, which is almost doubled after injecting two pore volumes of extended water. The core effluents were all low viscosity oil in water emulsions, consistent with observations in batch mixing experiments. The incremental oil recovery to ASP flood is almost twice the recovery obtained in the polymer flood with nearly 4.5 times larger pressure build-up across the core. These observations suggest that the main mechanism is emulsification and oil entrainment rather than improvement in sweep efficiency. These speculated mechanisms based on the core scale observations were verified through the results of the microfluidic experiments. ASP solution can efficiently penetrate the residual ganglia or the edge of the water channels from which oil can be efficiently stripped. This oil is easily entrained in the form of low viscosity oil in water emulsions. Our results emphasize the critical role of polymer in the cocktail. In the absence of polymer, AS cannot efficiently penetrate the residual viscous oil leading to only a minuscule improvement in oil recovery. • A successful ASP cocktail has to be screened for each individual oil. • For the specific oil (14,850 mPa.s at 25 °C) the most efficient chemical generates ultralow interfacial tension, 0.002 mN/m. • The most efficient chemical produces low viscosity type II (+) emulsion resulting in 20% OOIP incremental oil recovery after two PVIs which is almost doubled after injecting two PVIs of extended water flooding. • The incremental oil recovery to ASP flood is almost twice the recovery obtained in the polymer flood with almost 4.5 times larger pressure build up across the core. • The main displacement mechanism is emulsification and oil entrainment rather than improvement in sweep efficiency as confirmed by microfluidics tests. • The authors of this manuscript declare that there is all credits were appropriately given in the material disclosed in this manuscript. Background Early implementation of programmatic colorectal cancer (CRC) screening for average-risk individuals 50-74 years of age in Canada has used fecal occult blood tests [FTS (guaiac or immunochemical)] and colonoscopy for follow-up of abnormal FTS. This paper presents results of an evaluation of this CRC screening. Methods Five Canadian provincial programs provided aggregated data for individuals with a first-round FT processed between January 1, 2009, and December 31, 2011. Results The 104,750 people who successfully completed a first round of screening represented 16.1% of those who had access to the programs between January 1, 2009, and December 31, 2011 (mean age: 61.2 years; men: 61.4 years; women: 61.1 years). Of those participants, 4661 had an abnormal FT (4.4%). Uptake of colonoscopy within 180 days after an abnormal FT was 80.5%, ranging from 67.8% to 89.5% by program. The positive predictive value (PPV) for adenoma was 35.9% for guaiac FT and 50.6% for immunochemical FT. Adenoma and CRC detection rates were, respectively, 16.9 and 1.8 per 1000 screened. Of invasive CRCS detected, 64.6% were stage i or n. Conclusions Considering the variation in characteristics and stage of implementation of each provincial program, the collaboration of the provinces leading to this report on the early performance of CRC screening in Canada is a major milestone. Targets are met or nearly met for significant indicators such as PPV for adenoma and cancer detection rate. Participation is expected to increase as programs are fully implemented in the provinces. In this article, the author discusses the landscape-scale patterns of the cyanobacterial toxin microcystin in Canadian lakes discovered by researchers through a regression tree model using the TREES module in the software SYSTAT 13. He mentions that the model helps to determine the relationship between microcystin concentrations, the concentration of total nitrogen (TN) to total phosphorus (TP) and the TN:TP ratio.

Type 2 diabetes (T2DM) is a serious life-threatening chronic disease whose prevalence is especially high among Canadians living in poverty. And these Canadians with T2DM in poverty are especially likely to experience serious consequences of the disease. Of special concern is Statistics Canada reporting an explosive increase in mortality rates from diabetes (of which 90% represent T2DM) in low-income urban neighbourhoods across Canada. We place findings from interviews with 60 Canadians with T2DM who live in poverty within the context of recent shifts in public policy that have affected the distribution of the social determinants of health. Findings of material deprivation among our participants indicate that it is almost impossible for these individuals to acquire the diet necessary to prevent the adverse outcomes associated with T2DM. These findings draw attention to consideration of the important role public policy plays in affecting the situation of people living in poverty who are afflicted with T2DM and other chronic diseases. Background In 2006, the federal government committed funding of $250 million over 5 years for the Canadian Partnership Against Cancer Corporation to begin implementation of the Canadian Strategy for Cancer Control (cscc). The Partnership was established as a not-for-profit corporation designed to work actively with a broad range of stakeholders and organizations that had been engaged in the development of the cscc and with the public more broadly. A policy experiment unto itself, the Partnership was the first disease-based organization funded at the federal level outside of government. It was charged with a mandate to enable transfer of knowledge and to catalyze coordinated and accelerated action across the country to reduce the burden of cancer. Implementation Implementation has involved establishing shared goals, objectives, and plans with participating partners. Knowledge management—incorporating pan- Canadian approaches to the identification of content, processes, technology, and culture change—was used to enable that work across the federated health care delivery system. Evaluation of the organization through independent review, the ability to achieve initiative-level targets by 2012, and progress measured using indicators of system performance was used to examine the effectiveness of the strategy and approach overall. Discussion and Conclusions Evaluation findings support the conclusions that Canada has made progress in achieving immediate outcomes (achievable in <5 years) associated with advancing its cancer control goals and that there is evidence that, with sustained effort, those goals will translate into a long-term (>25 years) impact on cancer. The mechanism of funding the Partnership to develop collaboration among stakeholders in cancer control to achieve coordinated action has been possible and has been enabled through the Partnership's knowledge-to-action mandate. Opportunities are available to further engage and clarify the roles of stakeholders in action, to clearly define outcomes, and to further quantify the economic benefits that have resulted from a coordinated approach. With the ongoing funding commitment to support coordinated action within a federated environment of health care delivery, there is opportunity to reduce the impact that cancer may have in the long term in Canada. Background Despite the positive conclusions of several randomized controlled trials and the publication of national recommendations on colorectal cancer (CRC) screening, uptake remained low. The inauguration of the National Colorectal Cancer Screening Network in 2007, the same year that the first screening program was announced in Canada, provided an opportunity for integrated knowledge translation to accelerate the processes of program implementation and screening uptake. Aim Two primary aims were identified. The first focused on means to monitor the effects of various implementation plans in delivering high-quality population-based CRC screening. The second focused on identifying and addressing knowledge gaps that may impair screening participation. Method The methods used are described in the context of the knowledge-to-action cycle and demonstrate that the initiative itself dictates the point in the cycle at which to start. Results The identified need to monitor various implementation plans resulted in the shared development of a quality determinants document. All programs committed to designing data collection so that core components could be measured and compared; 6 operating programs have conducted the first data collection, which will allow for monitoring and for new knowledge creation as the process develops further. The knowledge gap identification project started with new knowledge creation, which identified a higher-than-expected willingness of Canadians to discuss CRC screening with physicians, but a low level of understanding of screening as a wellness-related behaviour. Knowledge translation interventions have been developed with the stakeholders to address those gaps, and ongoing surveys to be carried out later in 2011 will help to gauge progress in the understanding and acceptance of CRC screening by the population. Conclusions A national network that engaged all programs, policymakers, experts, and lay representatives successfully used knowledge translation principles to enhance the trajectory of CRC screening in Canada.
