Background: Allergen immunotherapy (AIT) represents the only possibility of causal therapy for allergic respiratory diseases. Although the prevailing high prevalence of allergic diseases and restrictions in the daily lives of patients, AIT is offered to a suboptimal number of patients in Germany. Methods: Insured patients with documented allergic respiratory disease of one of the largest statutory health insurances in Germany, ‘DAK-Gesundheit’, were contacted by postal mail and asked to participate in the study. In case of written consent, primary and secondary data of patients were collected and analysed. Patient characteristics, predictors of being offered AIT, predictors of performing AIT and guideline-compliant care were analysed. Results: 2505 subjects were included in the VerSITA study. Allergy to tree pollen and native speaking were identified as predictors, which increase the probability of being offered AIT. The probability was significantly decreased by the characteristics allergic rhinitis only, allergic asthma only, age in years, non-German citizenship, no graduation and lower secondary qualification. Significant positive predictors for an AIT to be actually performed were: Allergy to tree pollen and male sex. Predictors that decrease the likelihood that AIT is performed were: only allergic asthma, current smoker, former smoker, age and non-German citizenship. Furthermore, it was possible to identify characteristics in which guideline-compliant patients differed significantly from the rest of the study population. Conclusions: Based on statutory health insurance data and patient survey data, the VerSITA study provides a broad and in-depth overview of the care situation with regard to AIT in Germany and identifies deficits. in press

Most countries that apply risk-equalization in their health insurance market(s) perform risk-equalization on medical claims but do not include other components of the insurance premium, such as administrative costs. Using fixed effects panel regressions from individual insurers in Australia, Germany, the Netherlands, Switzerland, and the US, we find evidence that health insurers with a high morbidity population on average have higher administrative costs. We argue that administrative costs should also be included in risk-equalization and we show that such equalization results in additional equalization payments nontrivial in size. Using examples from Germany and the US, we show how in practice policymakers can include administrative costs in risk-equalization. Zusammenfassung Hintergrund Psychische Erkrankungen verursachen sowohl individuelle als auch gesellschaftliche Belastungen. Zentrales Ziel der Strukturreform der Psychotherapie-Richtlinie (Inkrafttreten 2017) war die verbesserte Steuerung des Zugangs zur psychotherapeutischen Versorgung. Die hier vorgestellten Fokusgruppen, die im Rahmen des durch den Innovationsfonds geförderten Forschungsprojekts „Evaluation der Psychotherapie-Richtlinie (Eva PT-RL)“ durchgeführt wurden, adressieren die Umsetzung der Ziele der Reform und der einzelnen neu eingeführten Versorgungselemente sowie Hürden bei der Implementierung aus der Perspektive von Leistungserbringer*innen, Patient*innen und Kostenträgern. Methoden Es wurden sechs Fokusgruppen und fünf Einzelinterviews mit Personen der drei genannten Stakeholder-Gruppen durchgeführt. Grundlage war ein an die jeweilige Personengruppe angepasster auf Basis einer strukturierten Literaturrecherche erstellter halbstrukturierter Gesprächsleitfaden. Die Gespräche wurden von einem Moderatorinnenteam per Videokonferenz durchgeführt, aufgezeichnet und transkribiert. Die Auswertung erfolgte über eine qualitative Inhaltsanalyse in Anlehnung an Mayring. Ergebnisse Eine allgemein positive Beurteilung insbesondere hinsichtlich des Erstzugangs zur psychotherapeutischen Versorgung erfuhr die psychotherapeutische Sprechstunde, wobei gleichzeitig ein verzögerter Übergang zur gegebenenfalls folgenden Richtlinien-Psychotherapie insbesondere aufgrund Kapazitätenmangels kritisch diskutiert wurde. Es wurden günstige Effekte der telefonischen Erreichbarkeitszeiten sowie einer verstärkten Vernetzung der psychotherapeutischen Versorgung auch mit anderen psychosozialen Angeboten angesprochen. Die Umsetzung der Akutbehandlung und der Rezidivprophylaxe wurde jedoch eher kritisch diskutiert. Als zentrales Ergebnis zeigt sich, dass der durch die Reform angestrebte Anstoß zum Paradigmenwechsel von Psychotherapeut*innen unterschiedlich wahrgenommen und umgesetzt wird – von einem Fokus auf individuelle Therapieprozesse hin zur Public Mental Health-Perspektive mit einer Verpflichtung zur Versorgungssteuerung. Schlussfolgerung Angesichts der variablen Strategien zur Umsetzung der Strukturreform durch die Psychotherapeut*innen, der Kritik an der Ausgestaltung einzelner Reformelemente und der Diskussion um Verzögerungen beim Zugang zur Richtlinientherapie aus der psychotherapeutischen Sprechstunde besteht weiterer Forschungsbedarf. Die Ergebnisse der Fokusgruppen dienen als Grundlage für die folgenden Projektschritte, u. a. eine Befragung von Psychotherapeut*innen, Patient*innen und hausärztlich tätigen Ärzt*innen.

Zusammenfassung Ziel der Studie Hauptziel der Studie ist der Vergleich der frühesten Erstattungszeitpunkte neuer Untersuchungs- und Behandlungsmethoden (NUB) in der ambulanten GKV-Versorgung und der PKV-Versorgung. Methodik Im Rahmen der Erstattung im GKV System wurde auf die Beschlüsse und weitergehende Dokumentation des G-BA und des Bewertungsausschusses zurückgegriffen. Für die Erstattung im PKV System wurden Beschlüsse der einschlägigen Organe, wie bspw. des Zentralen Konsultationsausschusses für Gebührenordnungsfragen, aber auch Ausarbeitungen der Ärztekammer Nordrhein und des GOÄ-Arbeitskreise des PKV-Verbands herangezogen. Aufgreifkriterium waren einschlägige Beschlüsse des G-BA der Jahre 2010 bis 2019. Ergebnisse Es wurden 17 Einzelbeschlüsse, zwei aus mehreren Einzelbeschlüssen bestehenden Beschlussreihen sowie Regelungen zur Erstattung von Companion Diagnostics einbezogen. Es ergaben sich unterschiedliche Zeitabstände zwischen Verfahrensbeginn und Eingang der neuen Methode in die GKV-Versorgung (14 bis 216 Monate). Gleichzeitig zeigt die Untersuchung, dass bei einem Großteil der betrachteten Methoden die Erstattung in der PKV früher möglich war als in der GKV. Es zeigte sich weiterhin bei fünf Methoden eine potenzielle einzelvertragsbedingte Einschränkung der Erstattung in der PKV, aber in zwei Fällen auch die Erstattung durch die PKV bei für die im Verfahrensergebnis des G-BA ausgeschlossenen Leistungen. Schlussfolgerung Die untersuchten NUB wurden in der Regel früher, teilweise erheblich früher in der PKV erstattet. Die frühere Erstattung durch die PKV impliziert jedoch auch ein Risiko, auch Leistungen zu erstatten, die vom G-BA im weiteren Verlauf als ohne medizinischen Nutzen bewertet werden. Legt man die Bewertung des G-BA als Maßstab zugrunde, könnte hiermit eine Verringerung der Kosteneffektivität der Versorgung und im Einzelfalle ein Patientenschaden durch Über- oder Fehlversorgung entstehen. Es besteht ein Spannungsfeld zwischen frühem Zugang zu NUB (gerade bei sehr langer Dauer des G-BA Verfahrens) und (Kosten-) Effektivität und Sicherung der Versorgungsqualität.

Zusammenfassung Zielsetzung Im Gegensatz zu hämatologischen Neoplasien im Kindesalter gibt es für Erwachsene mit diesen Erkrankungen wenige bis keine einheitlichen Nachsorgemuster und dementsprechend auch keine rational begründeten Nachsorgepläne. Des Weiteren wurden bislang keine Studien zu den mit der Nachsorge einhergehenden Kosten veröffentlicht. Die vorliegende Studie dient zur Erfassung des Ressourcenverbrauchs und der damit einhergehenden Kosten von erwachsenen Patienten. Methodik Die Daten des Ressourcenverbrauchs wurden 2014 retrospektiv für die letzten 12 Monate mittels eines standardisierten Fragebogens erfasst. Die Krankheitskosten (direkte medizinische sowie indirekte Kosten) wurden aus der gesellschaftlichen Perspektive ermittelt. Das Jahr 2014 dient als Basisjahr für die Berechnung der Preise. Zur Ermittlung der indirekten Kosten (Erwerbsminderung, Arbeitsunfähigkeit) wurde der Friktionskostenansatz mit einer Friktionsperiode von 90 Tagen herangezogen. Weiterhin erfolgte ein Vergleich mit Hilfe des Humankapitalansatzes. Ergebnisse Insgesamt nahmen 1531 Patienten an der gesundheitsökonomischen Erhebung teil. 89,4 % der Teilnehmer wiesen im Erfassungszeitraum mindestens einen ambulanten Arztkontakt auf. 235 Teilnehmern (15,3 %) wurde mindestens ein der Nachsorge zuzuordnendes Medikament verabreicht. 9,7 % der Studienteilnehmer gaben keinerlei Inanspruchnahme aufgrund von Nachsorge der Krebserkrankung an. Insgesamt ergaben sich direkte medizinische Kosten von durchschnittlich ca. 3627 € pro Patient. Die indirekten Krankheitskosten beliefen sich unter Verwendung des Friktionskostenansatzes auf ca. 165 € pro Patient. Schlussfolgerung Arzneimittelkosten verursachen mit etwa 60 % den Großteil der direkten Nachsorgekosten bei Patienten mit hämatologischen Neoplasien. Nach der zugrundeliegenden Erkrankung aufgeschlüsselt weisen myeloproliferative Erkrankungen mit einem Anteil von fast 60 % die höchsten Gesamtkosten auf. Unter Verwendung des Friktionskostenansatzes lassen sich die Nachsorgekosten in 95,7 % direkte Kosten und 4,3 % indirekte Kosten unterteilen. 9,7 % der Studienteilnehmer geben keinerlei Inanspruchnahme aufgrund von Nachsorge der Krebserkrankung an und scheinen somit komplett durch das Nachsorgesystem zu fallen.

Hintergrund: Allergische Atemwegserkrankungen haben Einfluss auf die Leistungsfahigkeit und Lebensqualitat der Erkrankten. Die Allergen-Immuntherapie (AIT) stellt den einzigen kausalen Behandlungsansatz mit der Moglichkeit dar, den Krankheitsverlauf positiv zu beeinflussen. Allerdings fehlen fur Deutschland bislang differenzierte Zahlen zur Behandlung geeigneter Patienten. Methoden: Die Versorgungssituation in Deutschland wird mittels einer retrospektiven, querschnittlichen, kohortenbasierten Versorgungsanalyse auf der Basis von Patientenangaben und Routinedaten der gesetzlichen Krankenversicherung (GKV) untersucht. Es werden Merkmale ermittelt, die eine leitliniengerechte Versorgung begunstigen oder verhindern. Daruber hinaus werden Angaben zur Lebensqualitat und zu den Krankheitskosten bei allergischer Rhinitis und allergischem Asthma erhoben und ausgewertet. Ergebnisse: Die Routinedaten liefern umfassende und sektorubergreifende Informationen zu Diagnosen und Inanspruchnahme von Gesundheitsleistungen. Die Patientenangaben bieten Informationen zu Erkrankungsschwere, Therapiehistorie, Lebensqualitat, Inanspruchnahme alternativer Behandlungsmethoden und soziodemografischen Merkmalen. Diskussion: Mit VerSITA wird eine fundierte Grundlage fur zukunftige Forschung und zur Information von Entscheidungstragern geschaffen, um Masnahmen zur Optimierung der Versorgung zu entwickeln. Zusammenfassung Zielsetzung Ziel war es, zu ermitteln, inwieweit „Therapien mit Naturheilmitteln“ in der ärztlichen Aus- und Weiterbildung gelehrt werden, sowie die Umsetzung dieser Therapien in die ärztliche Versorgungpraxis darzustellen. Methodik Es wurde eine strukturierte Recherche sowie eine Befragung von Fachärzten durchgeführt. Ergebnisse Die Therapien sind Teil des Medizinstudiums. Sie sind Inhalt in einer Facharztweiterbildung und sieben Zusatzweiterbildungen.Der überwiegende Teil der befragten Ärzte bietet diese Therapien für ihre Patienten an. Mehr als 65 % gaben an, die Therapien als relevant einzuschätzen. Gründe gegen eine Weiterbildung lagen in deren Kosten/Dauer, Gründe dafür im eigenen Interesse und der großen Nachfrage. Schlussfolgerung Zur Verbesserung der Versorgungsqualität sollte über mehr Transparenz und eine mögliche Umgestaltung der Weiterbildungssysteme diskutiert werden.

Summary Background Allergic respiratory diseases have an impact on the performance and quality of life of the patients. The allergen immunotherapy (AIT) is the only causal treatment approach with the chance to positively influence the course of the disease. However, differentiated figures for the treatment of suitable patients in Germany are still missing. Methods The health care situation in Germany is examined with a retrospective, cross-sectional, cohort-based health care analysis based on patient data and routine data from statutory health insurance (SHI). Characteristics are identified that encourage or prevent guideline-based care. In addition, data on quality of life and costs of illness of allergic rhinitis and allergic asthma are collected and evaluated. Results Routine data provide comprehensive and cross-sectoral information about diagnoses and utilization of health care services. In addition, primary data give information on disease severity, treatment history, quality of life, use of alternative treatment methods, and sociodemographic characteristics of the patients. Conclusion Analysis of the care situation with regard to the specific immunotherapy in cases of allergic respiratory diseases (VerSITA) provides a solid basis for future research and for informing decision-makers in order to develop measures to optimize care.

Objectives The purpose of the prospective clinical and pharmacoeconomic outcomes study of different first-line antiretroviral treatment strategies (PROPHET) was to examine the healthcare costs of human immunodeficiency virus (HIV)–infected persons in Germany treated with different antiretroviral therapy (ART) strategies and to identify variables associated with high costs. Methods The setting was a 24-month prospective multicenter observational cohort study in a German HIV-specialized care setting from 2014 to 2017. A microcosting approach was used for the estimation of healthcare costs. Data were obtained via electronic case report forms. The costs were calculated from both the societal and the statutory health insurance perspective. Regression models were performed that took into consideration the impact of several independent variables. Results Four hundred thirty-four patients from 24 centers throughout Germany were included. Average annual healthcare costs were €20 118 (standard deviation [SD] €6451) per patient from the societal perspective (n = 336) and €17 306 (SD €4106) from the statutory health insurance perspective (n = 292). Expenditures for the ART medication had the highest impact. Total costs declined in the second year of therapy. There was a significant association between the amount of total cost and clinical or therapeutic variables from both perspectives; a diagnosis of acquired immune deficiency syndrome (AIDS) led to higher costs as well as the chosen ART strategy. Age also increased cost from the statutory health insurance perspective. Conclusions The main cost driver of the healthcare costs for HIV-positive patients was antiretroviral drug expenses. Further variables that influenced the costs were identified. The results provide a detailed overview of the resource use of patients in the PROPHET cohort.

The objective of this article is the economic evaluation of the EndoPredict test in the assessment of the indication for chemotherapy in female breast cancer patients with intermediate risk of recurrence. A retrospective cost analysis based on patient files in a German breast center was conducted, including female estrogen receptor positive (ER+) and HER2-negative (HER2-) primary breast cancer patients. The EndoPredict test was performed retrospectively on archived tumor material. Direct medical costs (in- and outpatient treatment, drug prescriptions including chemotherapy and additional medications) were evaluated. Furthermore, hypothetical direct costs were calculated for a scenario where treatment decisions would have considered the results of the EndoPredict test. Eighty-two women [mean age 62 (37–77) years] met the inclusion criteria; half of them received chemotherapy. In total, using the information from the EndoPredict, chemotherapy could have been avoided in 16 patients. Average costs of outpatient chemotherapy were 14,835€/patient. Costs for the EndoPredict were 1811€/patient. Total costs in the hypothetical scenario amounted to 968,273€ (inpatient care: 447,524€; outpatient care including pharmaceuticals: 372,247€; EndoPredict: 148,502€). This retrospective analysis indicates potential cost savings with the application of the EndoPredict in the treatment decision in direct medical costs of about 10.5%, corresponding to 1.384€/test.

Hintergrund und Zielsetzung Kostenberechnungen sind ein wesentlicher Bestandteil gesundheitsökonomischer Evaluationen, weisen jedoch für den deutschen Versorgungskontext teilweise große Unterschiede im methodischen Vorgehen auf. Zielsetzung der vorliegenden Artikelserie ist es, einen konsentierten Vorschlag zu den Vorgehensweisen der Kostenberechnungen in verschiedenen Versorgungssektoren zu präsentieren und mit diesem einführenden Artikel allgemeine, Sektoren-unspezifische Aspekte in der Durchführung von Kostenberechnungen zu beschreiben. Methodik In der Arbeitsgruppe „Standardkosten“ des Ausschusses „Ökonomische Evaluation und Entscheidungsfindung“ haben sich Wissenschaftlerinnen und Wissenschaftler von Universitäten und Entscheidungsträgern im Gesundheitswesen zusammengeschlossen, um die bestehenden Probleme bei Kostenberechnungen im Rahmen gesundheitsökonomischer Forschungen zu diskutieren und ein einheitliches Vorgehen zu erarbeiten. Zur Entwicklung der Empfehlungen fanden drei Arbeitstreffen sowie weitere Telefonkonferenzen statt. Die vorläufigen Ergebnisse wurden auf der 11. Jahrestagung der dggö der Fachöffentlichkeit vorgestellt. Ergebnisse Im vorliegenden ersten Artikel einer zunächst fünf Artikel umfassenden Serie werden grundsätzliche Themen der Perspektivwahl, des Preis-Mengengerüstes, der Studienarten, der Kostenkomponenten, der Datenquellen, der Anpassung von Kostendaten über die Zeit oder über Ländergrenzen hinweg sowie der Umgang mit Unsicherheit behandelt und Empfehlungen zu diesen Themen gegeben. Des Weiteren wird die Gliederung von später folgenden vier Artikeln zu den Berechnungsmethoden in einzelnen Versorgungssektoren beschrieben., Background and aim Costing is a crucial component of health economic evaluations, but there is a wide variation in the approaches of calculating costs in the German health care system. This series of articles aims to propose unified costing methods for the different health care sectors while the current article is addressing overarching problems across all sectors. Methods The working group "Standardized Costing" within the committee "Economic Evaluation" of the German Society of Health Economics (dggo) discussed the current problems in the context of costing for health economic evaluation and developed agreed, unified costing approaches for the different health care sectors. The group consisted of researchers from universities and decision-makers in the German health care system and developed the recommendations at three personal meetings and additional telephone conferences. Preliminary results were presented at the 11th annual meeting of the dggo. Results The current paper presents the recommendations regarding perspective, price-resources framework, study types, cost components, data sources, adjustment of cost data over time or cross-country, and uncertainty in cost calculations. Additionally, the general structure of the four following articles on health care sector-specific costing is presented.

A reduction in sedentary behavior (SB) can contribute to the prevention of chronic diseases. This is the first umbrella review that summarizes the effectiveness and monetary costs of different types of interventions to reduce SB across all age groups and populations in different settings. We comprehensively searched seven databases for systematic reviews and meta-analyses and conducted an umbrella review of the effects of interventions to reduce SB. Additionally, we performed a meta-analysis of primary studies included in the umbrella review. Furthermore, we analyzed health economic aspects of interventions to reduce SB. We included 40 systematic reviews in our umbrella review, with 136 primary studies suitable for further meta-analyses. We found that interventions targeting the physical environment reduce SB most effectively in the majority of populations and settings. Background The amount of empirical research on whether people in fact include health-related changes in leisure time into health state valuations is limited and the results are inconclusive. In this exploratory study, we analyse whether time aspects of diabetes self-care might explain the ratings of the health state (HSR) in addition to the effects of physical and mental health-related quality of life. Methods Using the data from participants with diagnosed type 2 diabetes in the population-based KORA FF4 study (n = 190, 60% Male, mean age 69 ± 10 years), multiple logistic regression models were fitted to explain HSR (good vs. poor) in terms of the SF-12 physical and mental component summary (PCS and MCS) scores, time spent on diabetes self-care and a number of background variables. Results There was no significant association between time spent on diabetes self-care and HSR in models without interaction. Significant interaction term was found between the SF-12 PCS score and time spent on self-care. In models with interaction self-care time has a small, but significant impact on the HSR. In particular, for a PCS score under 40, more time increases the chance to rate the health state as “good”, while for a PCS score above 40 there is a reverse effect. Conclusions The additional impact of self-care time on HSR in our sample is small and seems to interact with physical health-related quality of life. More research is needed on whether inclusion of health-related leisure time changes in the denominator of cost-effectiveness analysis is sufficient.

Einleitung Der interdisziplinäre HLCA-Forschungsverbund untersucht unterschiedliche Dimensionen von Gesundheitskompetenz im Kindes- und Jugendalter. Zu diesen zählen die generische, mentale und digitale Gesundheitskompetenz bei Kinder, Jugendliche, Familien (Seite der Nutzer) und Fachkräfte aus dem Gesundheits-, Sozial-, und Bildungsbereich (Seite der Anbieter) mit einem besonderen Blick auf Strukturen und Verhältnisse. Das Ziel des Verbunds ist es, Gesundheitskompetenz theoretisch-konzeptionell, empirisch, interventionsbezogen sowie bezogen auf die Praxis und Politik zu untersuchen. Methoden Im HLCA-Verbund wurden Modelle für unterschiedliche Altergruppen entwickelt, die Gesundheitskompetenz von Kindern (9-10 Jahre) und Jugendlichen (14-17 Jahren) erhoben, Intervention zur Sträkung der Gesundheitskompetenz von Schüler*innen, Lehrkräften, Eltern und Fachkräften entwickelt, Praktiken in den Lebenswelten von Jugendlichen mit Migrations- und Fluchthintergrund untersucht und Barrieren im Zugang zu und in der Inanspruchnahme von Präventionsmaßnahmen und Interventionen in den Bereichen der psychischen und psychiatrischen Versorgung von Familien und Kindern mit psychisch erkrankten Eltern analysiert. Zudem wurden Netzwerkwerke zur schulischen Gesundheitsförderung und Prävention gebidelt. Die Interventionen werden hinsichtlich gesundheitsökonomischer und Gender-bezogener Aspekte wissenschaftlich begleitet, zudem werden Policy-Forschung und Politikfeldanalysen durchgeführt. Fazit Die Ergebnisse sind im Rahmen der Transferforschung in Empfehlungen für die Wissenschaft, Praxis und Politik übersetzt worden.

Aim Botulinum toxin agents can improve the quality of life of patients with post-stroke upper limb spasticity (ULS) and are recommended by international and German guidelines. However, health-services research indicates an underuse of botulinum toxin agents in this patient group. The study aims to clarify the budget impact of treatment with botulinum toxin agents according to the guidelines for all eligible patients with ULS in Germany compared to usual care. Methods The budgetary impact for the statutory health insurance system was estimated by calculating a Markov cohort model with a timeframe of 5 years comparing three treatment options. Four health states were included. The base-case analysis compares standard doses of two botulinum toxin agents to usual care. The model accounts for direct medical costs. Sensitivity analyses vary doses of botulinum toxin agents and prevalence of spasticity after stroke. Results In the base case, both botulinum toxin agents lead to increased costs compared to usual care. Treatment with Dysport® (cumulative costs for the 5-year period: €280,321,317) is less expensive than the treatment with Xeomin® (€377,511,529). Cumulative costs over 5 years in usual care are €61,306,062. Conclusion The botulinum toxin therapy is associated with additional costs, but an increased use of botulinum toxin would be consistent with guideline-based therapy. In particular, it has to be considered that spasticity as a chronic condition is accompanied by a relevant loss of quality of life. Thus, considering only the therapy costs may not be sufficient for making final therapeutic decisions.

Background Time-saving formats of evidence syntheses have been developed to fulfill healthcare policymakers’ demands for timely evidence-based information. A discrete choice experiment (DCE) with decision-makers and people involved in the preparation of evidence syntheses was undertaken to elicit preferences for methodological shortcuts in the conduct of abbreviated reviews. Methods D-efficient scenarios, each containing 14 pairwise comparisons, were designed for the DCE: the development of an evidence synthesis in 20 working days (scenario 1) and 12 months (scenario 2), respectively. Six attributes (number of databases, number of reviewers during screening, publication period, number of reviewers during data extraction, full-text analysis, types of HTA domains) with 2 to 3 levels each were defined. These were presented to the target population in an online survey. The relative importance of the individual attributes was determined using logistic regression models. Results Scenario 1 was completed by 36 participants and scenario 2 by 26 participants. The linearity assumption was confirmed by the full model. In both scenarios, the linear difference model showed a preference for higher levels for “number of reviewers during data extraction”, followed by “number of reviewers during screening” and “full-text analysis”. Subgroup analyses showed that preferences were influenced by participation in the preparation of evidence syntheses. Conclusion The surveyed persons expressed preferences for quality standards in the process of literature screening and data extraction.

Objective For the assessment of videorasterstereography compared with radiography for monitoring idiopathic scoliosis, new tools given by the INTEGRATE-HTA methodology were included within the frame set by the authority. In contrast to the usual linear approach by covering the single domains' safety and efficacy, costs, and ethical, legal, socio-cultural and organizational issues (ELSOI) separately, the methodology is based on the assumption of interactions of different aspects of the domains. From the very beginning, these interactions are captured systematically using various tools and are repeatedly discussed with stakeholders. Methods Relevant databases were systematically searched for studies and question catalogues were processed in order to be able to search for specific aspects in orienting bibliographic research. An initial logic model was developed and successively expanded. A synoptic table showing multiple mapping of aspects to domains, a complexity checklist, and a semantic complex highlighting the socio-cultural impact of the disease were newly developed. Results Four diagnostic studies with high risk of bias were included. For the clinical outcome, Cobb angle standard deviations up to 8.2 degrees and average measurement differences up to 8.8 degrees were determined. These differences in measurement accuracy correspond to those observed for purely radiographic measurements in other studies. In addition to freedom from radiation as the greatest advantage, there were clear arguments in favor of videorasterstereography when considering ELSOI, for example less harm. Conclusions Using INTEGRATE-HTA contributes to identify interactions between domains and to significantly expand the perspective on a technology. It improves patient-centered understanding and facilitates discussions.

This article is a revised version of our proposal for the establishment of the legal concept of risk-adjusted prevention in the German healthcare system to regulate access to risk-reduction measures for persons at high and moderate genetic cancer risk (Meier et al. Risikoadaptierte Pravention’. Governance Perspective fur Leistungsanspruche bei genetischen (Brustkrebs-)Risiken, Springer, Wiesbaden, 2018). The German context specifics are summarized to enable the source text to be used for other country-specific healthcare systems. Establishing such a legal concept is relevant to all universal and free healthcare systems similar to Germany’s. Disease risks can be determined with increasing precision using bioinformatics and biostatistical innovations (‘big data’), due to the identification of pathogenic germ line mutations in cancer risk genes as well as non-genetic factors and their interactions. These new technologies open up opportunities to adapt therapeutic and preventive measures to the individual risk profile of complex diseases in a way that was previously unknown, enabling not only adequate treatment but in the best case, prevention. Access to risk-reduction measures for carriers of genetic risks is generally not regulated in healthcare systems that guarantee universal and equal access to healthcare benefits. In many countries, including Austria, Denmark, the UK and the US, entitlement to benefits is essentially linked to the treatment of already manifest disease. Issues around claiming benefits for prophylactic measures involve not only evaluation of clinical options (genetic diagnostics, chemoprevention, risk-reduction surgery), but the financial cost and—from a social ethics perspective—the relationship between them. Section 1 of this chapter uses the specific example of hereditary breast cancer to show why from a medical, social-legal, health-economic and socio-ethical perspective, regulated entitlement to benefits is necessary for persons at high and moderate risk of cancer. Section 2 discusses the medical needs of persons with genetic cancer risks and goes on to develop the healthy sick model which is able to integrate the problems of the different disciplines into one scheme and to establish criteria for the legal acknowledgement of persons at high and moderate (breast cancer) risks. In the German context, the social-legal categories of classical therapeutic medicine do not adequately represent preventive measures as a regular service within the healthcare system. We propose risk-adjusted prevention as a new legal concept based on the heuristic healthy sick model. This category can serve as a legal framework for social law regulation in the case of persons with genetic cancer risks. Risk-adjusted prevention can be established in principle in any healthcare system. Criteria are also developed in relation to risk collectives and allocation (Sects. 3, 4, 5).

Zusammenfassung Zielsetzung Die gesetzlichen Regelungen zur Erstattung und Abrechnung von Arzneimitteln sind sehr komplex, sodass die Bestimmung der Arzneimittelkosten im Rahmen von gesundheitsökonomischen Evaluationen oft unterschiedlich gehandhabt wird. Ziel der Arbeit ist es, ein mögliches methodisches Vorgehen zur Kostenbestimmung von Arzneimitteln in Deutschland für gesundheitsökonomische Evaluationen zu beschreiben. Methoden Die Darstellung erfolgt primär aus der Perspektive der gesetzlichen Krankenversicherung und orientiert sich an den allgemeinen Schritten zur Kostenbestimmung: Ressourcenidentifikation, Bestimmung des Mengen- und Preisgerüsts, Aggregation. Dabei sollen neben Besonderheiten im deutschen Kontext auch verschiedene Datenquellen Berücksichtigung finden. Ergebnisse Für die Kostenbestimmung sind insbesondere der Zulassungsstatus und die Erstattungsfähigkeit der Arzneimittel zu berücksichtigen. Zur Erfassung des Ressourcenverbrauchs sind Angaben zur Dosierung (Menge und Häufigkeit) und zur Behandlungsdauer erforderlich. Als eine Grundlage für die Bewertung des Ressourcenverbrauchs gilt der Apothekenabgabepreis und – sofern vorhanden – Festbeträge. Um die Kosten für verschreibungspflichtige Arzneimittel zu berechnen, sind spezifische gesetzliche Regelungen zu Rabatten und Zuzahlungen zu berücksichtigen. Schlussfolgerung Trotz der Bemühungen um eine Standardisierung sind weiterhin Limitationen im Bereich der Kostenermittlung von Arzneimitteln vorhanden. Unsicherheiten ergeben sich insbesondere aufgrund der Nichtberücksichtigung von kassenindividuellen Rabattverträgen, da diese nicht öffentlich zugänglich sind. Auch unter Berücksichtigung der dargelegten Limitationen soll die vorliegende Arbeit einen Beitrag zu einer Verbesserung der Konsistenz der Kostenermittlung und Vergleichbarkeit von Arzneimittelkosten im Rahmen gesundheitsökonomischer Evaluationen leisten und somit den Prozess einer rationalen Entscheidungsfindung befördern.

Zusammenfassung Ziel In Deutschland liegen die jährlichen HIV-Neu-Infektionen bei 3100 (2016) und betreffen v. a. Männer, die Sex mit Männern haben sowie intravenös Drogengebrauchende. Es wird eine Rate von 30 % der HIV-Infektionen angenommen, die erst in einer fortgeschrittenen Phase der Infektion diagnostiziert werden, oft jenseits des optimalen Zeitpunkts des Behandlungsbeginns. Die hohe Rate an Late-Presentern ist ein starkes Argument für eine Stärkung von Beratungs- und HIV-Testangeboten. Ziel dieser Literaturanalyse ist es daher zu beschreiben, welche Zugangsschwellen zu HIV-Testangeboten existieren. Methodik Mit einer Literaturanalyse wurde der aktuelle Erkenntnisstand in Forschung und Praxis darauf hin abgebildet, welche Barrieren Risikogruppen vom HIV-Test abhalten. In die Suche wurden alle Veröffentlichungen zwischen 2010 und 2014 aus Australien, Europa und Nordamerika einbezogen, die sich auf Klienten-initiierte Tests beziehen. Ergebnisse 132 Veröffentlichungen wurden in eine Volltextanalyse einbezogen. Die meisten Veröffentlichungen basieren auf Klientenbefragungen. Die individuelle Risikowahrnehmung beeinflusst die Entscheidung für oder gegen einen HIV-Test maßgeblich. Diese kann über Öffentlichkeitsarbeit und über aufsuchende Angebote beeinflusst werden. In den Strukturen und Abläufen des HIV-Test-Angebotes sind Vertraulichkeit und Anonymität an jeder Stelle zentral. Darüber hinaus ist den Klienten wichtig, dass ihnen in den Beratungsgesprächen mit Akzeptanz und Vorurteilsfreiheit begegnet wird. Die Angst der Klienten vor Stigmatisierung, Verurteilung und Ausgrenzung verlangt eine besondere Sensibilität und lebensstilakzeptierende Haltung auf Seiten des Personals. Schlussfolgerung Die Faktoren, die für Klienten bei der Entscheidung über einen HIV-Test und der Auswahl eines Test-Angebots von Bedeutung sind, werden klar abgebildet. Die zentralen Elemente in der Entscheidungsfindung sind die eigene Risikowahrnehmung sowie die Furcht vor Stigmatisierung aufgrund vermeintlich abweichenden Sexualverhaltens oder aufgrund einer befürchteten HIV-Infektion. Die Öffentlichkeitsarbeit sollte Risikofaktoren bzw. -wahrnehmung in Bezug auf HIV sowie Vertraulichkeit und Anonymität des Angebots betonen. Die Nutzung sozialer Netzwerke ist hierbei besonders empfehlenswert. Neue Angebotsformen wie Heimtests könnten die Sorgen hinsichtlich Vertraulichkeit und Anonymität auflösen.

ZusammenfassungDas Prinzip „Reha vor Pflege“ lässt sich als Schwesterprinzip zu „ambulant vor stationär“ verstehen. Gleichwohl weisen die Zahlen aus den Routinedaten als auch die Erfahrungen der Akteure darauf hin, dass das Prinzip nur unzureichend umgesetzt wird. Diese Frage aufgreifend hat sich ein interdisziplinärer Autorenkreis zusammengetan, um auszuloten, welche Bedeutung in der realen Versorgung das Prinzip „Reha vor Pflege“ haben kann und welche Implikationen für eine gesundheitspolitische Weiterentwicklung daraus abzuleiten wären. Diese Ergebnisse dieses Workshops sind im nachfolgenden Papier dargelegt.

Specific immunotherapy is the only causal treatment in respiratory allergy. Due to high treatment cost and possible severe side effects subcutaneous immunotherapy (SCIT) is not indicated in all patients. Nevertheless, reported treatment rates seem to be low. This study aims to analyze the effects of increasing treatment rates of SCIT in respiratory allergy in terms of costs and quality-adjusted life years (QALYs). A state-transition Markov model simulates the course of disease of patients with allergic rhinitis, allergic asthma and both diseases over 10 years including a symptom-free state and death. Treatment comprises symptomatic pharmacotherapy alone or combined with SCIT. The model compares two strategies of increased and status quo treatment rates. Transition probabilities are based on routine data. Costs are calculated from the societal perspective applying German unit costs to literature-derived resource consumption. QALYs are determined by translating the mean change in non-preference-based quality of life scores to a change in utility. Key parameters are subjected to deterministic sensitivity analyses. Increasing treatment rates is a cost-effective strategy with an incremental cost-effectiveness ratio (ICER) of 3484€/QALY compared to the status quo. The most influential parameters are SCIT discontinuation rates, treatment effects on the transition probabilities and cost of SCIT. Across all parameter variations, the best case leads to dominance of increased treatment rates while the worst case ICER is 34,315€/QALY. Excluding indirect cost leads to a twofold increase in the ICER. Measures to increase SCIT initiation rates should be implemented and also address improving adherence.

Zusammenfassung. Fragestellung: Schulvermeidung erhöht ohne frühzeitige Diagnostik und Therapie das Risiko eines vorzeitigen Abbruchs der Bildungskarriere. Ziel dieser Arbeit ist die ökonomische Evaluation eines Manuals zur Behandlung von schulvermeidendem Verhalten im Vergleich zur Behandlung im kinder- und jugendpsychiatrischen Hilfesystem unter Standardbedingungen. Methodik: Im Rahmen der durchgeführten Kosten-Minimierungs-Analyse erfolgt die Erhebung der Ressourcenverbräuche retrospektiv für je 6 Monate mittels adaptiertem CSSRI-Fragebogen. Die Ressourcenverbräuche in den meisten Leistungsbereichen werden mit publizierten Standardpreisen bewertet. Die Darstellung der Kosten erfolgt aus gesellschaftlicher Perspektive für das Jahr 2011. Der Kostenvergleich zwischen den Therapiekonzepten im einjährigen Interventionszeitraum erfolgt anhand eines Differenz-von-Differenzen-Ansatzes. Ergebnisse: Bei den 112 Studienteilnehmern liegen am häufigsten phobische und emotionale Störungen vor. Die durchschnittlichen Gesamtkosten pro Patient im Interventionszeitraum betragen 7197 € (95 %-KI: 4746 € bis 10 079 €) in der Manualgruppe und 9294 € (95 %-KI: 6313 € bis 12 878 €) in der Kontrollgruppe. Die Differenz der bereinigten Kosten beträgt 1453 € zugunsten der Manualgruppe, wobei dieser Unterschied keine statistische Signifikanz erreicht. Schlussfolgerungen: Die manualbasierte Therapie stellt in Hinblick auf andernorts berichtete klinische Endpunkte und Krankheitskosten ein dem bisherigen Vorgehen gleichwertiges, tendenziell vorteilhaftes Therapiekonzept dar.

Systematic screening for chronic hepatitis B and C does not yet exist in Germany. Therefore, the implementation of a screening approach within a preventive medical examination performed by primary care physicians ('Check-Up 35+') was evaluated in a recent prospective multicenter study. The present analysis estimates the financial consequences for the statutory health insurance by budget impact analysis.A Markov cohort model was developed consisting of 21 health states. Four different screening scenarios derived from the previous multicenter study were compared to usual care, a strategy without screening for hepatitis. Actual cost data for Germany were calculated and systematic literature searches for all input parameters were performed.The base case results in incremental costs for the screening strategies compared to no hepatitis screening of 165-227 € per patient in a 20-year horizon. Two main parameters influence the financial consequences: (A) detection and treatment increase the costs in the beginning. (B) Screening avoids hepatitis induced end-stage liver disease. The initial higher costs exceed the later savings. Sensitivity analyses demonstrate a strong impact of medication costs for the treatment of additionally detected hepatitis infections on the outcome. This finding is robust to sensitivity analysis.The screening strategy proposed here implies additional costs for the statutory health insurance, however, a decision regarding its usefulness must consider criteria other than cost. For example, the high burden of disease due to liver cirrhosis and liver carcinoma should be considered. Therefore, an additional cost-effectiveness-analysis should be conducted.

Die evidenzbasierte Sekundarpravention nach akutem Koronarsyndrom mit persistierender ST-Streckenhebung (STEMI) besteht unter anderem aus einer Kombination von Acetylsalicylsaure, Statinen, ACE (Angiotensin-Converting-Enzym)-Hemmern und haufig Betablockern. Eine erfolgreiche Therapie setzt eine konsequente Einnahme voraus. Diese ist jedoch mit zunehmender Komplexitat der Medikamenteneinnahme haufig vermindert. Eine Polypille, bestehend aus Acetylsalicylsaure, Atorvastatin und Ramipril, wurde entwickelt, um die Einnahmekomplexitat zu verringern und die Adharenz zu erhohen. Zur Analyse der Kosteneffektivitat der Polypille im Vergleich zur Standardtherapie („standard of care“ [SOC]) wurde ein fur Grosbritannien entwickeltes Markov-Modell auf den deutschen Kontext angepasst. Die inkrementelle Kosteneffektivitat pro qualitatsadjustiertem Lebensjahr („quality adjusted life year“ [QALY]) der Polypille im Vergleich zum SOC wurde anhand eines Modells aus Perspektive der gesetzlichen Krankenversicherung fur einen lebenslangen Zeithorizont ermittelt. Die Kohorte schliest Erwachsene nach akutem Myokardinfarkt ein. Zur Validierung des Modells wurden multiple Sensitivitatsanalysen durchgefuhrt. Die Polypille verursacht im Vergleich zu SOC je 1000 Patienten Mehrkosten von 203.653 €. Die Differenz liegt in den Arzneimittelkosten begrundet. Die inkrementelle Kosten-Effektivitats-Relation (IKER) betragt 9228 €/QALY. Die Sensitivitatsanalyse weist einen Wertebereich von 5000–30.000 €/QALY aus. Insgesamt erscheint die Therapie mit der Polypille in Deutschland in einem vergleichsweise kosteneffektiven Bereich zu liegen. Bei Zweifeln bezogen auf Datenquellen wurde ein konservativer Ansatz verfolgt, sodass die Folgekosten aufgrund von Ereignissen bei mangelnder Therapieadharenz eher geringer eingeschatzt wurden.

Gentests konnen die Pradisposition fur verschiedene Erkrankungen offenbaren. Die Nachfrage nach Gendiagnostik und korrespondierenden prophylaktischen Masnahmen steigt stetig an. Fur die gesetzliche Krankenversicherung (GKV) bestehen jedoch Rechtsunsicherheiten, ob ein bloses Erkrankungsrisiko („risk of disease“) Grund genug dafur ist, die Kosten fur prophylaktische Masnahmen zu tragen. Stehen bei bestimmten Krebserkrankungen, wie bspw. beim hereditaren Brustkrebs, medizinisch wirkungsvolle prophylaktische Masnahmen zur Verfugung, so erscheint die derzeitige Praxis, im Einzelfall zu entscheiden, insuffizient. Sofern Personen mit einem hochgradigen und stark erhohten Brustkrebsrisiko nicht in die Regelversorgung aufgenommen werden, zeichnet sich eine Ungleichbehandlung beim Zugang zu medizinischen Leistungen innerhalb des GKV-Systems ab, da bestimmte Fallgruppen bereits in geregelter Weise Leistungsanspruche geltend machen konnen (Faktor-V-Leiden-Mutation). Wird aber darum eine allgemeine sozialrechtliche Berucksichtigung im Falle des hereditaren Brustkrebses angestrebt, so ist zum einen aufseiten der Klinik die Frage der adaquaten Risikokommunikation im Entscheidungsfindungsprozess der Betroffenen naher zu beleuchten und ggf. zu optimieren. Zum anderen mussen die budgetaren Auswirkungen fur das Gesundheitssystem so abgewogen werden, dass eventuelle zusatzliche Kosten tragbar sind. Der vorliegende Artikel zeigt auf, dass eine allgemeine sozialrechtliche Berucksichtigung von Personen mit hochgradigen und stark erhohten Erkrankungsrisiken aus medizinischer, ethischer und sozialrechtlicher Perspektive unumganglich ist, weshalb schon vorab die Konsequenzen einer solchen Regelung bedacht werden sollten.

Zusammenfassung Ziel der Arbeit ist die Ermittlung der Kosten der alternativen Implementierungsstrategien „Train-the-Trainer-Seminar“ (TTT) und „schriftlicher Leitfaden“ (LF) einer standardisierten Rückenschulung in der medizinischen Rehabilitation. Die Implementierungsstrategien wurden in 10 Rehabilitationskliniken zufällig zugewiesen und der Zeitaufwand mittels Erhebungsbögen erfasst. Zudem wurden die Kosten für Sachmittel sowie Reisekosten berücksichtigt. Die klinikbezogenen Gesamtkosten der Implementierung liegen beim TTT-Seminar mit 4 582 € um ein Drittel (35%) höher als die Gesamtkosten der LF-Strategie. Die höheren Kosten der Implementierung über ein TTT-Seminar waren insbesondere auf den höheren personellen Ressourcenverbrauch durch das zeitintensive Seminar zurückzuführen. Für die LF-Gruppe zeigt sich allerdings ein wesentlich höherer Kostenanteil für die Nachbereitung der Programminhalte (23,5%).

Eines der moglichen Residuen des Schlaganfalls ist die Spastik der oberen Extremitat mit einer Haufigkeit von 19 % bei Patienten 3 Monate nach einem Schlaganfall bis hin zu 38 % ein Jahr nach einem Schlaganfall. Bei fokaler, multifokaler und segmentaler Spastik kann eine intramuskulare Injektion von Botulinumtoxin indiziert sein. Derzeit wird in Deutschland hinsichtlich der Behandlung der Spastiksymptome nach einem Schlaganfall mit Botulinumtoxin eine Unterversorgung angenommen. Das Ziel der vorliegenden Untersuchung besteht darin, die Versorgungssituation von Patienten mit einer Spastik der oberen Extremitat infolge eines Schlaganfalls unter besonderer Berucksichtigung des Einsatzes von Botulinumtoxin als eine Behandlungsoption zu evaluieren. Mittels eines standardisierten Fragebogens wurde eine postalische Befragung einer reprasentativen bundesweiten Stichprobe von 800 Neurologen und Nervenarzten durchgefuhrt. Die Rucklaufquote der postalischen Befragung betrug 37 %. 59 % der befragten Arzte antworteten, dass sie nie Botulinumtoxin in der Therapie einsetzen. Insgesamt 87 % der Arzte sehen Hurden hinsichtlich des Einsatzes von Botulinumtoxin. Dabei werden die Hohe der Honorierung der arztlichen Leistung mit 40 % und die fehlende Kostenubernahme durch die Gesetzliche Krankenversicherung bei Off-Label-Use mit 60 % sowie das Erlangen einer Fort-/Weiterbildung zur Behandlung mit Botulinumtoxin genannt. Insgesamt kann trotz eines Rucklaufs bei der postalischen Befragung von 37 % ein Selektionsbias nicht ausgeschlossen werden. Viele der teilnehmenden Facharzte setzen Botulinumtoxin nicht in der Therapie ein, obwohl es bei entsprechender Indikation fur die Behandlung in den Leitlinien empfohlen wird. Grunde hierfur lassen sich in den oben dargestellten Hurden erkennen.

Objective: Atrial fibrillation (AF) is a major cause for recurrent stroke, has severe impact on a patient's health and imposes a high economic burden for society. Current guidelines recommend 24 h ECG monitoring (standard-of-care, SoC) to detect AF after stroke to reduce the risk of future events. However, paroxysmal AF (PAF) is difficult to detect within this period as it occurs infrequently and unpredictably. In a randomized controlled trial (Find-AF(RANDOMISED)), prolonged and enhanced Holter ECG monitoring (EPM) revealed a significantly higher detection rate of AF compared to SoC, although its cost-effectiveness has not yet been investigated. Methods: Based on the data of FIND-AF(RANDOMISED), an economic evaluation was conducted. One group received EPM for 10 days after the event, and at 3 and 6 months; the other group received SoC. Healthcare resource use and quality of life (QoL) data were measured at baseline, and after 6 and 12 months. Incremental costs and quality-adjusted life years (QALYs) between both groups were compared. Non-parametric bootstrapping and one-way-sensitivity analyses were performed. Results: A total of 281 patients with healthcare resource use and QoL data for all measurement points were considered in the economic evaluation (complete case analysis, CCA). The CCA yielded nonsignificant 315euro lower mean costs and 0.0013 less QALYs for patients receiving EPM with no statistically significant differences in any cost categories. Sensitivity analyses revealed robust results. Bootstrapping the results indicated moderate probability of cost-effectiveness. Conclusions: EPM yielded reduced not significantly different costs without affecting QoL and may be a useful strategy to detect PAF in acute ischemic stroke patients in time.

Major guidelines for HIV recommend that testing should be provided in a way that is adapted to the lifeworld of groups at risk. However, precise definitions as to what adaption to the lifeworld means are missing. Therefore, this research project evaluated how the lifeworld approach can be operationalized in regard to HIV testing and how voluntary counseling and testing services can use this approach to improve efficacy. A survey for VCT providers in Germany showed a limited number of factors influencing the adaption to the lifeworld of groups at risk. The opening hours, the number of languages used in public relations, and the comfort of the waiting zone are important for all clients. For men who have sex with men, information material for use in social networks and peer volunteers are relevant. Intravenous drug users are concerned with participation, (low) costs, and counseling. For migrants, continuing staff training and anonymity are important.

ZusammenfassungVor dem Hintergrund der anhaltenden Diskussionen um die Sicherstellung der medizinischen Versorgung – insbesondere in ländlichen, strukturschwachen Regionen Deutschlands, ist der Begriff „Landarzt“ ein häufig verwendeter Terminus. Bis dato mangelte es jedoch an einer konkreten Definition des Landarztbegriffes. Dieses Defizit soll durch den Landarzt-Abgrenzungsalgorithmus (La-Abal) behoben werden. Anhand einer einheitlichen Abgrenzungssystematik kann nun die Differenzierung zwischen Land- und städtisch/urban tätigen Ärzten realisiert werden. Der vorliegende Artikel gibt Auskunft darüber, ob der La-Abal generell auf Gesamtdeutschland anwendbar ist, in welchen Regionen sich Landärzte befinden und inwieweit die als Landarztbereiche identifizierten Regionen mit hausärztlich unterversorgten Regionen übereinstimmen. Die Methodik des La-Abals basiert im Kern auf den drei Parametern: „Grundzentrum“, „Ländliche Besiedelung“ und „Räumliche Lage“. Einheitsgemeinden und Gemeindeverbände dienen in diesem Kontext als Analyseebene. Eindeutige, durch den La-Abal vorgegebene Bedingungen ermöglichen es, Einheitsgemeinden/Gemeindeverbände als Landarztbereiche zu definieren und somit zu identifizieren. Als Ergebnis zeigt sich, dass der La-Abal auf Gesamtdeutschland anwendbar ist. Des Weiteren existieren Landarztbereiche, einige davon korrespondieren mit denen als hausärztlich unterversorgt geltenden Regionen. Der La-Abal ist ein ergebnisorientiertes, praktikables und standardisiertes Modell, das es ermöglicht, ein einheitliches Landarztverständnis zu etablieren. Dies ist notwendig, um Antworten auf gesellschaftliche Veränderungen entwickeln und den damit einhergehenden Forderungen nach Sicherstellung der gegenwärtigen und zukünftigen Gesundheitsversorgung gezielt planen und steuern zu können.

Background Pre-exposure prophylaxis (PrEP) is a highly effective HIV prevention strategy for men-who-have-sex-with-men (MSM). The high cost of PrEP has until recently been a primary barrier to its use. In 2017, generic PrEP became available, reducing the costs by 90%. Aim Our objective was to assess cost-effectiveness and costs of introducing PrEP in Germany. Methods We calibrated a deterministic mathematical model to the human immunodeficiency virus (HIV) epidemic among MSM in Germany. PrEP was targeted to 30% of high-risk MSM. It was assumed that PrEP reduces the risk of HIV infection by 85%. Costs were calculated from a healthcare payer perspective using a 40-year time horizon starting in 2018. Results PrEP can avert 21,000 infections (interquartile range (IQR): 16,000–27,000) in the short run (after 2 years scale-up and 10 years full implementation). HIV care is predicted to cost EUR 36.2 billion (IQR: 32.4–40.4 billion) over the coming 40 years. PrEP can increase costs by at most EUR 150 million within the first decade after introduction. Ten years after introduction, PrEP can become cost-saving, accumulating to savings of HIV-related costs of EUR 5.1 billion (IQR: 3.5–6.9 billion) after 40 years. In a sensitivity analysis, PrEP remained cost-saving even at a 70% price reduction of antiretroviral drug treatment and a lower effectiveness of PrEP. Conclusion Introduction of PrEP in Germany is predicted to result in substantial health benefits because of reductions in HIV infections. Short-term financial investments in providing PrEP will result in substantial cost-savings in the long term.

Objectives: Potential opportunities and challenges of predictive genetic risk classification of healthy persons are currently discussed. However, the budgetary impact of rising demand is uncertain. This project aims to evaluate budgetary consequences of predictive genetic risk classification for statutory health insurance in Germany. Methods: A Markov model was developed in the form of a cohort simulation. It analyzes a population of female relatives of hereditary breast cancer patients. Mutation carriers are offered intensified screening, women with a BRCA1 or BRCA2 mutation can decide on prophylactic mastectomy and/or ovarectomy. The model considers the following scenarios: (a) steady demand for predictive genetic testing, and (b) rising demand. Most input parameters are based on data of the German Consortium for Hereditary Breast and Ovarian Cancer. The model contains 49 health states, starts in 2015, and runs for 10 years. Prices were evaluated from the perspective of statutory health insurance. Results: Steady demand leads to an expenditure of €49.8 million during the 10-year period. Rising demands lead to additional expenses of €125.5 million. The model reveals the genetic analysis to be the main cost driver while cost savings in treatment costs of breast and ovarian cancer are indicated. Conclusions: The results contribute to close the knowledge gap concerning the budgetary consequences due to genetic risk classification. A rising demand leads to additional costs especially due to costs for genetic analysis. The model indicates budget shifts with cost savings due to breast and ovarian cancer treatment in the scenario of rising demands.

Hintergrund und Fragestellung: Im Krankenhausmarkt besteht allgemein ein hoher wirtschaftlicher Druck, dem die Krankenhauser sowohl durch Kosteneinsparungen als auch durch Leistungsausweitungen zu begegnen versuchen. Angesichts der schwierigen finanziellen Rahmenbedingungen des DRG-Systems und der unzureichenden Investitionsfinanzierung stellt sich die Frage, inwieweit die Patientenversorgung und die Arbeitsbedingungen der Arzte durch diese Umstande beeinflusst werden. Insbesondere gastroenterologische Leistungen gelten gemeinhin als unzureichend im DRG-System abgebildet. Vor diesem Hintergrund fokussiert sich die Untersuchung auf diesen Fachbereich. Methodik: Basierend auf fruheren Studien sowie mehreren halbstrukturierten Interviews mit stationar tatigen Gastroenterologen wurden fachbereichs- und zielgruppenspezifische Fragebogen fur Chefarzte, Oberarzte und Assistenzarzte entwickelt. Insgesamt wurden 1751 Mitglieder der Deutschen Gesellschaft fur Gastroenterologie, Verdauungs- und Stoffwechselkrankheiten (DGVS) angeschrieben. Von 642 Teilnehmern wurde der Fragebogen beantwortet – der Rucklauf betragt somit 36,7 %. Die Antworten wurden deskriptiv und mittels multivariater Analyseverfahren untersucht. Ergebnisse: Ein erheblicher wirtschaftlicher Druck wird von den befragten Gastroenterologen wahrgenommen. Dieser manifestiert sich primar in Versorgungsdefiziten im Bereich der Zuwendung und Pflege der Patienten – ebenfalls wird die Arbeitszufriedenheit negativ beeintrachtigt. Zudem werden Probleme in der Personalrekrutierung festgestellt, die jedoch nur teilweise auf wirtschaftliche Grunde zuruckzufuhren sind. Rationierung stellt in der Gastroenterologie ein vergleichsweise seltenes Phanomen dar. Auch finanziell motivierte Uberversorgung erscheint eher nachrangig. Tendenziell werden mehrere Problembereiche, die Patientenversorgung betreffend, von den Assistenzarzten insgesamt etwas kritischer gesehen als von den Chefarzten. Schlussfolgerungen: Insgesamt ist die Situation der Gastroenterologie mit anderen Fachbereichen vergleichbar – bei einzelnen Fragestellungen (z. B. nach erhohten Eingriffszahlen) gibt es jedoch Abweichungen. Angesichts einer als unzureichend empfundenen Abbildung gastroenterologischer Leistungen im DRG-System sind weitergehende Projekte zur verbesserten Abbildung anzudenken.

Objective To explore the operational feasibility of using mobile health clinics to reach the chronically underserved population with maternal and child health (MCH) services in Tanzania. Design We conducted fifteen key informant interviews (KIIs) with policy makers and district health officials to explore issues related to mobile health clinic implementation and their perceived impact. Main results Policy makers' perspective indicates that mobile health clinics have improved coverage of essential maternal and child health interventions; however, they face financial, human resource-related and logistic constraints. Reported are the increased engagement of the community and awareness of the importance of MCH services, which is believed to have a positive effect on uptake of services. Key informants (KIs)' perceptions and opinions were generally in favour of the mobile clinics, with few cautioning on their potential to provide care in a manner that promotes a continuum of care. Immunization, antenatal care, postnatal care and growth monitoring all seem to be successfully implemented in this mode of service delivery. Nevertheless, all informants perceive mobile clinics as a resource intensive yet unavoidable mode of service delivery given the current situation of having women and children residing in remote settings. Conclusion While the government shows the clear motive, the need and the willingness to continue providing services in this mode, the plan to sustain them is still a puzzle. We argue that the continuing need for these services should go hand in hand with proper planning and resource mobilization to ensure that they are being implemented holistically and to promote the provision of quality services and continuity of care. Plans to evaluate their costs and effectiveness are crucial, and that will require the collection of relevant health information including outcome data to allow sound evaluations to take place.

In Germany, social health insurance is offered by about 110 sickness funds and covers about 90% of the population. Income-related contributions are the main source of funding, supplemented by payments from the federal government. The resources are collected in a Central Health Fund and distributed through a prospective risk adjustment approach that includes the risk factors: age, gender, reduced earning capacity, and morbidity. Morbidity is operationalized in 50–80 expensive, severe, and/or chronic diseases. Different risk adjustment formulas are used for the “insured residing abroad” and the sick leave payment. A number of ongoing issues, such as using citizens’ region as a risk factor and implementing some risk-sharing elements, are under consideration.

In Germany, a regional social health insurance fund provides an integrated care program for patients with schizophrenia (IVS). Based on routine data of the social health insurance, this evaluation examined the effectiveness and cost-effectiveness of the IVS compared to the standard care (control group, CG). The primary outcome was the reduction of psychiatric inpatient treatment (days in hospital), and secondary outcomes were schizophrenia-related inpatient treatment, readmission rates, and costs. To reduce selection bias, a propensity score matching was performed. The matched sample included 752 patients. Mean number of psychiatric and schizophrenia-related hospital days of patients receiving IVS (2.3 ± 6.5, 1.7 ± 5.0) per quarter was reduced, but did not differ statistically significantly from CG (2.7 ± 7.6, 1.9 ± 6.2; p = 0.772, p = 0.352). Background: Many established products (EPs – marketed for eight years or more) are widely used off-label despite little evidence on benefit–risk ratio. This exposes patients to risks related to safety and lack of efficacy, and healthcare providers to liability. Introducing new indications for EPs may represent a high societal value; however, manufacturers rarely invest in R&D for EPs. The objective of this research was to describe incentives and disincentives for developing new indications for EPs in Europe and to investigate consequences of current policies. Methods: Targeted literature search and expert panel meetings. Results: Within the current European-level and national-level regulatory framework there are limited incentives for development of new indications with EPs. Extension of indication normally does not allow the price to be increased or maintained, the market protection period to be extended, or exclusion from a reference price system. New indication frequently triggers re-evaluation, resulting in price erosion, regardless of the level of added value with the new indication. In consequence, manufacturers are more prone to undertake R&D efforts at early to mid-stage of product life cycle rather than with EPs, or to invest in new chemical entities, even in therapeutic areas with broad off-label use. This represents a potentially missed opportunity as developing new indications for EPs offers an alternative to off-label use or lengthy and expensive R&D for new therapies, opens new opportunities for potentially cost-effective treatment alternatives, as well as greater equity in patients’ access to treatment options. Conclusion: There are potential benefits from the development of new indications for EPs that are currently not being realized due to a lack of regulatory and pricing incentives in Europe. Incentives for orphan or paediatric drugs have proven to be effective in promoting R&D. Similarly, incentives to promote R&D in EPs should be developed, for the benefit of patients and healthcare systems.

Purpose Allergic respiratory diseases represent a global health problem. The two major treatment strategies are symptom treatment and specific immunotherapy (SIT). SIT is considered the only causal treatment option available with the ability to alter the course of the disease. This study aims to describe the course of disease and medication of respiratory allergy across treatment strategies and disease groups. Methods The analysis is based on routine data from a German statutory health insurance. The patient cohort is observed from 2007–2012. For each year based on assured outpatient diagnoses patients are assigned to a disease group: rhinitis, asthma or both diseases. Additionally, prescribed medication is considered. Treatment comparisons are based on matched pairs. Results The study population comprises 165,446 patients with respiratory allergy. In 2007 the most frequent disease group is rhinitis (70%), followed by asthma (16%) and both diseases (14%). During the observation period a second allergic respiratory diagnosis occurs only in about 12% of rhinitis patients and 28% of asthma patients. In about 50% of patients with both diseases one of the diagnoses is omitted. These patients are more likely to no longer report their asthma diagnosis when receiving immunotherapy compared to symptom treatment. Furthermore immunotherapy reduces the frequency of asthma medication use. Conclusions Results of detailed analysis of diagnoses reflect the alternating nature of allergic diseases. Although limited by accuracy of documentation and the lack of clinical information, the comparison of treatment strategies shows some advantages of immunotherapy regarding course of disease and asthma medication use.

Vor mehr als fünf Jahren wurde der Gesundheitsfonds eingeführt, verbunden mit der Erwartung, dass mehr Gerechtigkeit und Wettbewerb in das Krankenversicherungssystem Einzug hielte. Vor allem der erhoffte Übergang vom Preis- zum Qualitätswettbewerb ist aber offenbar nicht gelungen. Der Gesundheitsfonds hat nach der Auffassung einiger Autoren auch die Nachhaltigkeit der Finanzierung nicht verbessert, die vorhandene strukturelle Einnahmenschwäche wurde vielmehr durch die gute Konjunktur und die hohen Beitragssätze überdeckt. Wie sich die neuerliche Umstellung von pauschalen Zusatzbeiträgen, die in den letzten Jahren von keiner Kasse erhoben wurden, auf einkommensabhängige Beiträge auswirkt, wird unterschiedlich beurteilt. In 2009 the German central health fund was implemented. It was the result of a political compromise. One political party intended to equalise the risk structure among 130 different health care funds, and the other wanted to intensify competition and to improve efficiency among the different health insurances. The fund is fl anked by a health-based risk adjustment and is aimed at sustainably ensuring the funding of the statutory health insurance system. It also constitutes the basis for competition among statutory health care funds, securing quality and efficiency in health care provision. To cover additional expenditures, health insurers had to charge flatrate premiums. A new law, which will come into effect in January 2015, will oblige health insurers to charge income related contributions. It is not clear how this will change competition among insurers. The federal government will not need to finance subsidies to low income individuals anymore. Income redistribution will only take place within the public health insurance system, exempting the privately insured and public servants from supporting low income individuals. The authors claim that the health care fund will not succeed in its aim of securing a sustainable financial basis for the statutory health insurance system. Most of the authors argue that there is no evidence of greater efficiency thus far, and they offer proposals on how to achieve improved performance.

Background: Worldwide, the number of patients with allergic diseases is global increasing, especially in the industrialized nations. Experts estimate that only about 10 % of allergic patients are treated according to the latest guidelines. The aim of this study is to map the medical care situation of patients with allergic respiratory diseases under the German statutory health insurance system and to investigate the impact on patient care since January 1, 2009, when ceilings for service volumes in allergology were implemented.

Die Arzneimittelzulassung und der Aufnahmeprozess zur Kostenerstattung sollen die Entwicklung und Vermarktung von pharmazeutischen Innovationen mit Patientennutzen nicht behindern, zugleich aber die Wirtschaftlichkeit der Arzneimittelversorgung fur die Kostentrager nicht gefahrden. Eine Anpassung der Verfahren an die Merkmale personalisierter Arzneimittel erscheint notwendig. Dabei ist allerdings zu fragen, ob eine ungerechtfertigte Privilegierung erfolgt. In den USA und in der EU werden die jeweiligen Zulassungsverfahren fur Arzneimittel und Tests schrittweise angepasst und integriert. Zulassung und Erstattungsentscheidungen sollen koordiniert werden. Eine Privilegierung, wie bei Arzneimitteln fur seltene Indikationen (Orphan Drugs), findet jedoch fur personalisierte Medizin nicht statt. Es bestehen keine unzumutbaren Hurden fur die Hersteller. Zuruckhaltung bei der Entwicklung innovativer Produkte ware deshalb nicht gerechtfertigt.

The aim of this study was to investigate the impact of a transformational leadership style on employee well-being.Data for the study were taken from an on-line survey of all employees from six German companies in the information and communication technology sector. The data obtained were analyzed using multiple logistic regressions.Analysis of data from a total of 318 employees (response rate 58.4%) revealed a significant relationship between transformational leadership and employee well-being (P = 0.00; OR = 1.13; 95% CI = 1.05 to 1.22). A higher degree of perceived transformational leadership is associated with greater well-being.The results of this study suggest that a transformational leadership style, which both conveys a sense of trust and meaningfulness and individually challenges and develops employees, also has a positive effect on employee well-being.
