Prostat Biyopsisi Yapılacak Hastalarda Prostat Sağlık İndeksinin Araştırılması Ve ÖnemiAmaç : Çalışmamızda, PSA yüksekliği nedeniyle trans rektal ultrasonografi eşliğinde ince iğne prostat biyopsisi (TRUS-Bx) yapılacak hastalarda eş zamanlı prostat sağlık indeksi (PHİ) hesaplanarak PHİ'nin prostat kanseri tanısı konulmasındaki etkinliğin araştırıdı.Materyal ve Metod : Kliniğimizde, Ağustos 2014 ve Mart 2016 tarihleri arasında PSA yüksekliği nedeniyle TRUS-Bx yapılan 258 ardışık hasta çalışmamıza dahil edildi. Parmakla rektal muayenede prostat kanseri şüphesi olan ve/veya serum PSA yüksekliği saptanan olgulardan işlem öncesi PSA, sPSA ve proPSA bakılıp TRUS-Bx uygulandı.Bulgular : Ortalama yaşları 63,5 ± 8 (36-91) yıl olan 258 hastanın ortalama PSA değeri 40,1 ± 194,2 (0,12 – 2170) ng/ml, ortalama PHİ değeri 118 ± 164,5 (0,41 – 1308) ng/ml idi. 96'sının TRUS-Bx patolojisi Adeno kanser, 140'ı BPH, 10'u ASAP, 1'i HGPİN, 10'u kronik prostatit ve 1'i skuamöz hücreli karsinom olarak raporlanmıştır. TRUS-Bx patolojisi Adeno kanser olan hastaların PSA ortalaması 93,9 ± 311,8 (2,2 – 2170) ng/ml, BPH olanların PSA ortalaması 7,5 ± 6,7 (0,3 – 71,1) ng/ml ve diğer hastaların PSA ortalaması 12,7 ± 14,5 (0,12 – 60,4) ng/ml idi. PSA değeri 4'ün altı düşük, 4 ve üstü yüksek olarak kabul edildiğinde, PSA düşük olan grupta 10 (% 31,2) hasta Adeno kanser 22 (% 68,8) hasta BPH, PSA yüksek olan grupta ise 86 (% 42,2) hasta Adeno kanser, 118 (% 57,8) hasta BPH olarak raporlandı. PSA değeri 4'ün altı düşük, 4 ve üstü yüksek olarak kabul edildiğinde, PSA'nın prostat Adeno kanseri yakalamadaki duyarlılığı % 89,6 özgüllüğü % 15,7 olarak hesaplandı. PHİ'nin değeri 55'in altı düşük, 55 ve üstü yüksek olarak kabul edildiğinde , PHİ'nin prostat Adeno kanseri yakalamadaki duyarlılığı % 71,9 özgüllüğü % 67,9 olarak hesaplandı.Tartışma : Bu bulgular, PHİ'nin prostat kanseri yakalamadaki özgüllüğünün PSA'ya göre daha yüksek olduğunu göstermektedir.Anahtar kelimeler: Prostat kanseri, prostat iğne biyopsisi, PSA, proPSA, prostat sağlık indeksi Investigation and Importance of Prostate Health Index for Candidates of Prostate BiopsyObjective: The aim of the present study was to search the efficiency of prostate health index (PHI) which is calculated simultaneously during ultrasound-guided fine needle prostate biopsy (TRUS-Bx) on diagnosis of prostate cancer.Materials and Methods: The present study included 258 subsequent patients who had TRUS-Bx because of elevated blood levels of PSA in our clinic between August 2014 and March 2016. PSA, sPSA and ProPSA were analyzed in the cases who have suspicion of prostate cancer by digital rectal examination and/or those with elevated serum levels of PSA; and TRUS-Bx was performed.Results: Age average of 258 patients was 63,5 ± 8 (36-91) years and mean PSA level and mean PHI value of those patients were 40,1 ± 194,2 (0,12 – 2170) ng/ml and 118 ± 164,5 (0,41 – 1308) ng/ml, respectively. Pathological examination of TRUS-Bx specimens revealed Adeno cancer in 96 patients, BPH in 140 patients, ASAP in 10 patients, HGPIN in 1 patients, chronic prostatitis in 10 patients and squamous cell carcinoma in 1 patient. The mean PSA of the patients whose TRUS-Bx pathology were adeno cancer was 93,9 ± 311,8 (2,2 – 2170) ng/ml whereas mean PSA levels of those with PBH and mean PSA levels of other patients were 7,5 ± 6,7 (0,3 – 71,1) ng/ml and 12,7 ± 14,5 (0,12 – 60.4) ng/ml, respectively.When the PSA level below 4 was accepted low and at and above 4 were accepted as high, the low PSA group included 10 (31,2%) patients with adeno cancer, 22 (68,8%) patients with BPH; and in the high PSA group, 86 (42,2%) patients were reported as adeno cancer and 118 (57,8%) patients were reported as BPH.When the PSA levels below 4 was accepted low and at and above 4 were accepted as high, sensitivity and specificity of PSA to detect prostate adeno cancer were calculated as 89,6% and 15,7%, respectively. Similarly, when a PHI level below 55 was accepted as low and PHI level at and above 55 were accepted as high, sensitivity sensitivity and specificity of PHI to detect prostate adeno cancer were calculated as 71,9% and 67,9%, respectively.Discussion: Overall, the findings above indicate that specificity of PHI is higher than PSA in terms of prostate cancer detection.Key words: Prostate cancer, prostate needle biopsy, PSA, proPSA, prostate health index 60

PubMedID: 24126283 Purpose We used immunohistochemical methods and transmission electron microscopy to investigate the cytokine profiles and ultrastructural changes in the ureterovesical junction of children with primary vesicoureteral reflux. Materials and Methods A total of 39 distal intravesical ureters were obtained from 23 children who underwent ureteroneocystostomy for primary vesicoureteral reflux. Ureteral wall smooth muscle organization and transforming growth factor-ß1, vascular endothelial growth factor and CD34 were evaluated immunohistochemically and compared to controls, which consisted of 10 age matched autopsy specimens. Ultrastructural evaluations and morphological descriptions were performed semiquantitatively and compared to the published data. Results Of the patients 6 (26%) were male and 17 (74%) were female, and mean ± SD age was 73.2 ± 34.3 months (range 12 to 168). There was no correlation between reflux grade and age (p = 0.39). Smooth muscle disorganization score differed significantly between patients with intravesical ureters and controls (p = 0.01). Farklı Yaş Gruplarında Perkütan Nefrolitotomi Sonuçlarının KarşılaştırılmasıAmaç: Bu çalışmada kliniğimizde perkütan nefrolitotomi uygulanan ve ayrıntılı kayıtları tutulan 65 yaş ve üzerindeki 233 hastada uygulanan perkütan nefrolitotomi ameliyatının etkinliği değerlendirildi.Yöntem ve gereçler: Retrospektif dökümante edilmiş hasta verileri analiz edildi.Bulgular: Yaşlı ( ?65yaş ) böbrek taşlı hastalarda uygulanan perkütan nefrolitotomi operasyonunda; operasyon süresi, skopi süresi, nefrostomi çekilme süresi, hastanede yatış süresi, kan transfüzyon oranları değerlendirildi. Ayrıca özgeçmişinde açık cerrahi ya da shockwave lithotripsy geçirmiş olmanın tüm bu parametrelere etkisi değerlendirildi.Sonuçlar: Böbrek taşı tedavisinde altın standart olan perkütan nefrolitotomi operasyonu yaşlı hastalarda da genç hastalar kadar etkili ve güvenilirdir.Anahtar sözcükler: Perkütan nefrolitotomi, yaşlı hasta, kan transfüzyonu gerektiren kanama Objectives: In this study, the efficacy of percutaneous nephrolithotomy operation was assessed on the data of 233 patients older than 65 years old who underwent percutaneous nephrolithotomy in our clinic.Material and Methods: The data of patients were analyzed retrospectively.Results: The operation time, fluoroscopy time, nephrostomy removal time, hospitalisation time and blood transfusion rate were assessed in patients older than 65 years who underwent percutaneous nephrolithotomy with kidney stone. Amaç: Böbrek iskemisi; böbrek transplantasyonu, parsiyel nefrektomi, sepsis, çeşitli ürolojik girişimler ve hidronefroz gibi çeşitli klinik durumlarda sık görülen bir tablodur. Bu yüzden nefron koruyucu cerrahinin önemi zamanla daha da artmaktadır. Böbrek iskemisine bağlı böbrek dokusunda meydana gelen hasarı en aza indirmek için çeşitli farmakolojik alanlar deneysel olarak çalışılmaktadır. Bunlar içinde çeşitli fosfodiesteraz inhibitörleri mevcuttur. Bizde bu çalışmamızda fosfodiesteraz 4 inhibitoru olan Rolipram adlı ajanın renal reperfüzyon hasarı üzerine olan etkisini araştırmayı amaçladık.Yöntem ve Gereçler: Çalışmamızda 30 adet sıçan altışarlı 5 ayrı gruba ayrılması planlandı. 1.gruba herhangi bir şey yapılmadı, 2.gruba 30 dk boyunca renal pediküle klemp konuldu. 3.gruba klemp konulmadan 30 dk önce, 4.gruba klemp konulduğu esnada, 5.gruba klemp açıldıktan 30 dk sonra 1mg/kg intraperitoneal rolipram verildi. Klempleme süresi 30 dakika olarak belirlenmiştir. Bu grupların hepsine 24 saat sonra nefrektomi uygulandı. Alınan her böbrek dokusunun yarısının histopatolojik incelenmesiyle enflamasyon, tubuler nekroz ve apoptosize bakıldı. Diğer yarısının biokimyasal yöntemlerle malondialdehit, süperoksit dismutaz, katalaz seviyeleri ölçüldü.Bulgular: Çalışmada Rolipram verilmesiyle, sıçanlarda renal İ/R hasarı belirgin azaldı. Oksidatif hasarda serbest radikallerin neden olduğu lipit peroksidasyonunun stabil metaboliti olan malondialdehid seviyeleri; 2.(İ/R) grupta özellikle 5 gruptan belirgin olarak daha yüksekti. Aynı zamanda antioksidan enzim olan Superoksit Dismutaz ve Katalaz seviyesi 5.grupta 2.gruptan anlamlı düzeyde yüksek bulunmuştur. Histopatolojik olarak enflamasyon açısından Rolipram verilen gruplarla 1.( kontrol) ve 2.(İ/R ) gruplar arasında istatistiksel çok anlamlı fark olmamasına rağmen,5 grupta tubuler nekroz ve apoptosis 1.( kontrol ) grup hariç diğer gruplardan anlamlı derecede düşük bulunmuşturSonuç: Rolipram, renal İ/R hasarına yol açabilecek cerrahi girişimlerde kullanılarak, oluşabilecek oksidatif renal doku hasarı azaltılabilir ve renal fonksiyonun ileri derecede bozulması engellenebilir kanısındayız.Anahtar Sözcükler: Rolipram, renal iskemi, iskemi reperfuzyon hasarı ( İ /R ) Purpose: Renal ischemia is a common entity that seen in renal transplantation, partial nephrectomy, sepsis, and a variety of urological procedures and various clinical situations, such as hydronephrosis. Therefore, the importance of nephron-sparing surgery is increasing with time. To minimize the damage of renal ischemia in the kidney tissue, various pharmacological agents studied experimentally. These agents include the various phosphodiesterase inhibitors. In this study, we aimed to investigate the effect of the Rolipram as agent of phosphodiesterase 4 inhibitor, on renal reperfusion injury.Method and Materials: The study was planned by separation of 30 rats to 5 different groups containing 6 in each. In the first group nothing was done. In the second group the renal pedicle clamped for 30 minutes. In the third group, 30 min before clamping, in fourth group when clamp is placed, in fifth group after 30 minutes opening the clamp, 1mg/kg rolipram was given intraperitoneally by injection. Clamping time was determined as 30 minutes. Nephrectomy was performed 24 hours after all of these groups. For each kidney, half of the tissues examined histopathologically for inflammation, tubular necrosis and apoptosize views. The other half of the kidneys examined by biochemical methods including malondialdehyde, superoxide dismutase, catalase levels measuring.Findings: In rats with the administration of Rolipram, renal I / R injury significantly decreased. In oxidative damage, malondialdehyde levels, which is stable metabolite of lipid peroxidation caused by free radicals was significantly higher in second (I / R) group than fifth group in particular. At the same time that the antioxidant enzyme superoxide dismutase and catalase levels were significantly higher in Group 2 than group 5. Histopathologically, the inflammation of the groups which administrated Rolipram compared with group 1 (Control) and 2 (I / R), there was not a very significant difference. Amaç: Bu çalışmada sıçanlarda intraperitoneal bakteriyel lipopolisakkarit (LPS) enjeksiyonu ile prostatit oluşturularak inflamasyonlu sıçanların prostat dokuları üzerindeki tonus değişikliklerini farmakolojik yöntemlerle değerlendirmeyi ve güçlü bir antioksidan olan melatonin'in bu inflamasyona bağlı değişiklikler üzerindeki etkisini araştırmayı amaçladık.Gereçler ve Yöntem: Deneylerde Wistar Kyoto cinsi (200-300 g) erkek sıçanlar kullanıldı. Sıçanlar kontrol ve bakteriyel lipopolisakkarid (LPS) (10 mg/kg) uygulanan diye ana gruba ayrıldı. LPS uygulanan gruplar 5 alt gruba ayrıldı; (I) 24 saatlik LPS çalışma grubu, (II) 72 saatlik LPS + intraüretral E. coli çalışma grubu, (III) 72 saatlik LPS + intraüretral E. coli + aminoguanidin çalışma grubu, (IV) 72 saatlik LPS + intraüretral E. coli + melatonin çalışma grubu, (V) 4 haftalık kronik LPS + intraüretral E. coli çalışma grubu. LPS uygulamasından 24 saat, 72 saat veya 4 hafta sonrasında sıçanlar isofloran anestezi altında öldürülerek prostat dokuları alındı. Her sıçanın prostatının bir lobu patolojik inceleme için ayrıldı diğer lobu izole edilerek 37oC Krebs solüsyonu içeren ve %95O2-%5CO2 karışımıyla oksijenlendirilen (pH: 7,4) organ banyoları içerisine 1g tansiyon altında yerleştirildi. Preparattaki tonus değişiklikleri bir data veri toplama sistemi aracılığı ile bilgisayara kaydedildi. Dokulara elektriksel saha stimülasyonu verilerek nörojenik kasılmalar, alfa-1 adrenerjik reseptör agonisti fenilefrin verilerek adrenerjik kasılmalar ve kolinerjik agonist olan karbokol verilerek kolinerjik kasılma cevaplarına bakıldı. Gruplar arası karşılaştırma Student t testi ile istatistiksel olarak değerlendirildi.Bulgular: Bakteriyel LPS veya LPS+intraüretral LPS uygulanan gruplarda nörojenik, adrenerjik ve kolinerjik kasılma cevapları anlamlı olarak azaldı. Bu azalma 72 saatlik grupta daha belirgin olarak ortaya çıktı. 24 saatlik ve 4 haftalık gruplarda ki azalmalar birbirine yakındı ve daha çok küçük frekans/düşük dozlarda belirgindi. Patolojik incelemelerde 24 saat ve 72 saatlik gruplarda bir akut prostatit olayının geliştiğini teyid etti. 4 haftalık gruplarda ise infeksiyonun kronikleştiği görüldü. 72 saatlik gruplara uygulanan melatonin ve bir İNOS inhibitörü olan aminoguanidin kasılma cevaplarındaki bu azalmayı kısmen geri çevirdi.Sonuç: Bakteriyel LPS uygulaması sıçan prostat dokusunda anlamlı olarak adrenerjik, nörojenik ve kolinerjik kasılma cevaplarını azaltmaktadır. Dokunun bu kasılma cevaplarındaki azalmanın başlıca nedeni oksidatif aktivitenin aşırı artması ve dokudaki inflamasyona bağlı patoloji olabilir. Melatonin bu oksidatif hasara bağlı kontraktilite bozukluğunu anlamlı olarak düzeltmektedir. Melatoninin iNOS inhibitör ve antioksidan etkileri ile prostat inflamasyonuna bağlı kontraktilite bozukluğunu ve patolojiyi düzeltebilmesi antioksidan maddelerin bu tür patolojik durumlarda endike olabileceğini göstermektedir.Anahtar Sözcükler: Bakteriyel lipopolisakkarit, sıçan prostat, melatonin Purpose: In this study, we created a prostatitis with intraperitoneal bacterial lipopolysaccharide (LPS) injection in the rats and aimed to assess the tonus changes on the prostatic tissues of rats with inflammation through pharmacologic methods and to investigate the effect of melatonin which is a strong antioxidant on the changes due to inflammation.Materials and method: Male Wistar Kyoto rats (200-300 g) were used in the experiments. The rats were categorized into two main groups as the control and the group which bacterial lipopolysaccharide (LPS) (10 mg/kg) was applied. The group which LPS was applied were divided into 5 sub-groups; (I) 24-hour LPS study group, (II) 72-hour LPS + intraurethral E.coli study group, (III) 72-hour LPS + intraurethral E.coli + aminoguanidin study group, (IV) 72-hour LPS + intraurethral E.coli + melatonin study group, (V) 4-week chronic LPS + intraurethral E.coli study group. The rats were killed under isoflorane anesthesia and their prostatic tissues were taken out 24 hours, 72 hours and 4 weeks after the LPS implementation. One lobe of each rat?s prostate was reserved for pathological examination, and the other lobe was isolated and placed into organ baths under 1 g tension which contained 37oC Krebs solution and was oxygenated with the mixture of 95% O2- 5% CO2 (pH:7,4). The tonus changes in the preparations were recorded into a computer through a data collection system. The responses to neurogenic spasms were recorded by giving electrical domain stimulation to the tissues, the responses to adrenergic spasms were recorded by administering fenilefrin which is an alpha-1 adrenergic receptor agonist and the responses to the cholinergic spasms were recorded by giving carbocol which is a cholinergic agonist. The comparison of the groups was assessed statistically by student t test.Findings: The responses to the neurogenic, adrenergic and cholinergic spasms in the groups to which bacterial LPS or LPS + intraurethral LPS were implemented meaningfully decreased. This decrease was more significant in the 72-hour group. The decreases in the 24-hour and 4-week groups were close to each other and they were prominent mostly in smaller frequencies and lower doses. The pathological examinations also confirmed that an acute prostatitis was developed in the 24-hour and 72-hour groups. It was seen that the infection became chronic in the 4-week groups. Melatonin and aminoguanidin, an INOS inhibitor, which were implemented to the 72-hour groups partially reversed the decrease in the responses of spasms.Results: Bacterial LPS implementation in the rat prostate tissues decreases the responses of adrenergic, neurogenic, and cholinergic spasms meaningfully. The main reason of this decrease in the responses of the spasms of the tissue might be the extreme increase of the oxidative activity and the pathology related with the inflammation in the tissue. Melatonin significantly improves the contractility defect due to oxidative damage.Since the INOS inhibitor and antioxidant effect of melatonin improves the histopathology and the contractility defect due to prostatic inflammation it can be concluded that antioxidants might be used in prostatic inflammations.Key Words: Bacterial Lipopolysaccharide, rats prostate, melatonin 99

Amaç: Bu çalışmada kliniğimizde perkütan nefrolitotomi uygulanan ve ayrıntılı kayıtları tutulan 1905 hastada gelişen komplikasyonlar yıllara göre değerlendirildi. Taş boyutu, giriş sayısı ve yaşın, operasyon süresi ve kan transfüzyonu gerektiren kanamaya etkisi araştırıldı.Yöntem ve gereçler: Retrospektif dökümante edilmiş hasta verileri 11 yıllık alt gruplara ayrılıp SPSS programı kullanılarak regresyon analizi, tek yönlü varyans analizi, eş yapma yöntemi ve ?2 (ki-kare) testi ile analiz edildi.Bulgular: Onbir yıllık süreç içerisinde kan transfüzyonu gerektiren kanamanın istatistiksel olarak anlamlı azaldığı, taş boyutunun ve giriş sayısının artışının operasyon süresini artırdığı ve buna paralel olarak ta kan transfüzyon ihtiyacının arttığı tespit edildi.Sonuçlar: Böbrek taşı tedavisinde altın standart olan perkütan nefrolitotomi operasyonunda klinik deneyim arttıkça komplikasyon oranları azalacaktır. Ancak her hasta kendine göre dikkatli bir şekilde değerlendirilerek operasyon planlanmalıdır. Objectives: In this study, 1905 patients who underwent percutaneous nephrolithotomy procedures in our clinic and have detailed records were evaluated for complications according to operation year. The effects stone burden, Access number and age on operation time and hemorrhage required was studied.Methods: The retrospectively collected data of patients were separated subgroups per yearly and analyzed by using SSSP program analyzing of regression, one way analysis of variant, paired method and k-square test.Results: In this eleven years, hemorrhage required blood transfusion were decreased significantly. The operation time is prolonged due to large stone burden and multi-access number and as a result blood transfusion rates are increased.Conclusion: Complications rates are decreased by gaining experience on percutaneous nephrolithotomy procedures that is the gold standard technique for the treatment of kidney stone disease. The operations should be planned after the meticulously evaluation of all patients. 54

Kadın cinsel fonksiyon bozukluklarıyaşa bağımlıdır ve yaşartıkça % 30-50 oranındagörülmektedir. Çalışmamızda infertilite nedeniyle başvuran kadınların cinsel fonksiyonuüzerine etkili olabilecek demografik özellikler araştırıldı.Çukurova Üniversitesi Tıp Fakültesi Kadın Hastalıklarıve Doğum Anabilim Dalınınİnfertilite polikliniğe başvuran gönüllü 200 infertil hasta çalışmaya alındı. Kadınlarademografik bilgileri içeren genel bilgi formu, dokuz soruluk Kadın cinsel Fonksiyonİndeksi (IFSF) ve 21 soruluk Beck depresyon ölçeği (BDÖ) formlarıdoldurtuldu. Yaş,evlilik süresi, eğitim durumu, mesleği, aylık geliri, evlenme şekli, eşe karşıhissi IFSFskorlarıile arasındaki ilişki regresyon ve korelasyon analizi ile araştırıldı.Çalışmaya alınan bayanların otalama yaşve IFSF skoru sırayla 30.9 ± 6.1 yıl ve 32±7 olarak saptandı. Kadınlar eğitim durumu, mesleği, aylık geliri, evlenme şekli, eşe karşıhissi alt gruplara ayrıldığında ve IFSF skorlarıile karşılaştırıldığında istatistiksel olarakanlamlıfark saptandı(p0.005).Çalışmamızda eğitim durumu yüksek, çalışan, anlaşarak evlilik yapan gelir düzeyiyüksek olan kadınların IFSF skorlarının anlamlıderecede yüksek olarak saptandı. Female sexual dysfunction depends on age and it seems 50 ? 60 % of women withincreased age. In our study we evaluated sexual function and related demographic featuresof women presented with infertility.This study was performed on 200 volunteer infertile women who visited infertilityoutpatient clinic of Department of Obstetrics and Gynecology of Çukurova University. Weadministered female sexual dysfunction index (IFSF) with 9 questions, Beck?s DepressionInventory (BDI) with 21 questions and general information form for searchingdemographic features. The relation between age, marriage time, education, occupation,salary, situation of partner selection and feelings toward husband and IFSF score wasinvestigated with regression and correlation analysis.Mean age and IFSF score were 30.9 ± 6.1 and 32±7 years respectively. Education,occupation, salary, situation of partner selection and feelings toward husband whenseparated sub into groups and compared with IFSF score; the results were statisticallysignificant (p < 0.005). There was no statistically significant correlation between marriagetime and smoking with IFSF score (p > 0.005).In our study, we detected high level of IFSF score among women, who are welleducated, have occupation, high salary and married with agreement. 52

ÖZET SIÇANLARDA BAKTERİ LİPOPOLİSAKKARİTİ İLE OLUŞTURULAN PROSTAT VE VAZ DEFERENS İNFLAMASYONU ÜZERİNE E VİTAMİNİ VE SELENYUMUN ETKİSİ Antioksidanlar özellikle son 10 yılda gerek klinik gerekse deneysel anlamda üzerinde çalışılan maddelerdir. Yapılan çalışmalar göstermektedir ki, bu maddeler ilerleyen zamanlarda modern tıbbın tedavi protokollerindeki yerini alacaktır. Literatürden inflamatuar yanıt üzerindeki düzeltici etkisini bildiğimiz bu maddelerin, prostat ve vaz deferens inflamasyonu üzerindeki etkilerini araştırmayı planladık. Bu çalışmada, kontrol ve lipopolisakkarit (LPS) uygulanan olmak üzere, herbiri 33 Wistar Kyato cinsi ortalama ağırlığı 246 gr sıçanlardan iki ana grup oluşturuldu. LPS grubu (n=15) sıçanlara, intraperitoneal (ip) LPS (10mg/kg) verilmek suretiyle; prostat ve vaz deferensde inflamasyon oluşturuldu. LPS'li alt gruplardan: birine (n=6), tek doz LPS ile eş zamanlı olarak aminoguanidin (ip) (100mg/kg); diğer gruba (n=6), üç gün tek başına ve dördüncü gün tek doz LPS ile eşzamanlı E vitamini intramusküler (im) (0.3 mg/kg/gün); bir diğer gruba da (n=6) üç gün tek başına ve dördüncü gün tek doz LPS ile eşzamanlı sodyum selenat (ip) (16 ug/kg/gün) enjeksiyonları uygulandı. Kontrol grubu (n=15) sıçanlara serum fizyolojik (ip) (0.1cc/100gr) enjekte edildi. Kontrol alt gruplardan: birine (n=6), tek doz aminiguanidin; diğerine (n=6), dört gün tek başına vitamin E; bir diğer gruba da dört gün tek başına sodyum selenat enjeksiyonları uygulandı. Tüm gruplardaki vaz deferens dokuları orta kısımlarından birer cm'lik epididimal ve prostatik iki parçaya ayrıldı. Elektriksel saha stimülasyonuna (ESS; 2, 4, 8, 16, 32 Hz, 50 v, 0.5 ms) maruz bırakıldı. ÖZET Bu çalışmada azoospermik veya ağır oligozoospermik infertil hastalarda ince iğne aspirasyon sitolojisinin açık testis biyopsisine göre tanısal ve testiküler sperm ekstraksiyonu açısından güvenilirliği araştırılmıştır. Azoospermik 34, oligozoospermik 4 ve prostat Ca nedeniyle iki taraflı orşiektomi yapılan 2 olmak üzere toplam 40 hasta çalışmaya alındı. Her hastanın ayrıntılı klinik ve laboratuvar değerlendirilmesi yapıldı, her hastadan 6 hafta aralıklarla 2 semen örneği alındı. İnce iğne aspirasyon sitolojisi için 20 mi lik şırınga 26 gauge 13 mm lik iğne kullanıldı ve sitolojik yaymalar May-Grunwald-Giemsa ve Papanicalou boyası ile boyandı. Daha sonra iğne giriş noktasına 3-4 mm'lik insizyon yapılarak açık biyopsi alınıp, hazırlanan kesitler hemotoksilen ve eozin ile boyandı. Sitolojik yaymalar ile histolojik kesitler ışık mikroskobunda incelenip sonuçları karşılaştırıldı. Sitolojik yaymalar mevcut hücre popülasyonuna ve morfolojisine dayanarak dört grupta sınıflandırıldılar; 1- Normal spermatogenezis (10), 2- Hipospermatogenezis (5), 3- Sadece Sertoli hücreleri (1), 4- Spermatojenik arrest (22). İnce iğne aspirasyon sitolojisi yapılan 40 hastanın 36'sında (%90) sitolojik bulgular histolojik tanı ile doğrulanmıştır. İki hastanın sitolojik değerlendirmesi yetersiz aspirat ve hücre nedeniyle yapılamadı. Azoospermik iki hastanın sitolojik değerlendirmesi spermatojenik arrest iken, histolojik değerlendirmesi sonucu yaygın fibrozis tanısı kondu. Azoospermik hastalardan 5'inde, oligozoospermik hastalardan 3'iinde obstrüksiyon tespit edildi. Normal spermatogenezis ve hipospermatogenezisli 16 hastanın 1 5'inde (%93.7) ince iğne aspirasyon sitolojisi ile testiküler spermatozoa elde edildi. Diğer 24 hastada hem ince iğne aspirasyon sitolojisi hemde açık biyopsi ile testiküler spermatozoa elde edilemedi. Testisin ince iğne aspirasyon sitolojisi basit, ucuz, hızlı minimal invaziv, tecrübeli ellerde yapıldığında testiküler sperm ekstraksiyonu ve tanısal açıdan açık testis biyopsisi kadar etkili ve geliştirilebilir bir yöntemdir. IV ABSTRACT Comparison of Fine Needle Aspiration Cytology and Open Testicular Biopsy in Infertile Males It has beeen compared the diagnostic value of testicular fine needle aspiration (FNA) with open biopsy in infertile males with azoospermia or severe oligozoospermia and to evaluate the reliability of testicular sperm extraction by FNA in infertile males in this study. A total of 40 patients, 34 with azoospermia, 4 oligozoospermia and 2 orchiectomized patients with prostate cancer were included in the study. Detailed clinical and laboratory examinations were perfomed and 2 semen analysis were obtained every 6 weeks from each patient. A 20ml 26 Gauge 13mm needle was used for FNA and smears were stained with May Grunwald Giemsa and Papanicolaou stain. An open biopsy was performed in each patient after FNA and the samples were stained with hematoxilen eosin. Smears and histologic samples were examined and compared under light microscopy. The smears were divided into four groups according to the present cell population : A. Normal spermatogenesis (10), B. Hypospermatogenesis (5), C. Sertoli cell only (1), D. Spermatogenic arrest (20). In 36 of the 40 patients (90%) FNA cytology results showed no difference from the histology. Two cases revealed unsatisfactory aspirate. Two cases with azoospermia revealed spermatocytic arrest cytologically while subsequent biopsies revealed diffuse fibrosis. In 5 of the azoospermic and 3 of the oligozoospermic patients obstruction were detected. In 15 of 16 patients (93.7%) with normal spermatogenesis and hypospermatogenesis, FNA was able to extract testicular spermatozoa. Spermatozoa could not be obtained after neither FNA nor open biopsy in the other 24 patients. Testicular FNA in infertile males is a simple, reliable and minimally invasive diagnostic tool. It is as effective as open biopsy for testicular sperm extraction and better results can be achieved in experienced hands. Keywords: Testis, fine needle aspiration, infertility, testis biopsy. 55
