Introduction: Vascular surgery through a groin incision may be associated with severe wound healing disorders in this sensitive area. There are many options to reconstruct the defect surgically. The choice of surgical reconstruction depends mainly on the individual status of vasculature, which is most often compromised in these patients. There are random pattern flaps, as well as perforator, pedicled flaps or microvascular flaps to choose from., Aim: We give an overview of plastic surgical solutions for groin defects, with a special focus on complex wounds after vascular surgical complications. We discuss advantages and disadvantages of different flaps with two case reports and also show alternatives., Patients and Methods: We demonstrate in two cases how the reconstruction of the groin defect was planned, taking into account the vascular status, and why we chose an innovative and seldom-used option in each case., Results: The selected flaps, a pedicled fasciocutaneous ALT propeller flap and a perforator-based, pedicled abdominal advancement flap reconstructed the defects successfully., Discussion: The surgical therapy for the reconstruction of groin defects should be chosen according to the individual vascular status to ensure safe and reliable blood supply. To guarantee the best possible reconstruction and avoid postoperative healing disorders and infections, less common flaps should also be considered.

Background: Skin grafting for syndactyly repair is time-consuming, and it has the potential risks of scar contracture and web creep. Several techniques have been described for web reconstruction without skin grafts in simple syndactyly. Based on the hexagonal advancement flap, we designed a "plane-shaped" advancement flap on the dorsal hand or foot for a web reconstruction with primary closure., Methods: Between June 2010 and August 2013, a "plane-shaped" advancement flap was used for web reconstruction in 24 syndactylies of 20 pediatric patients. The patients' age ranged from 1.5 to 8 years (mean age: 2.5 years). The patients were followed up for 15-48 months (mean: 28 months)., Results: The mean operation time was 60 min (range: 50-75 min). All incisions were closed primarily without skin grafting. All the flaps survived completely without early complications such as infection, vascular compromise or delayed healing. The mean hospitalization time was 3.5 days. There was no occurrence of long-term complications including web creep, flexion contractures, angulation deformity, or obvious keloid formation during the 15-48 months follow-up. Cosmetic and functional satisfactory outcomes were achieved in all cases without secondary revision., Conclusion: The dorsal plane-shaped advancement flap without skin graft is a solution for web space reconstruction in simple syndactyly, especially in the foot., (Copyright © 2015 British Association of Plastic, Reconstructive and Aesthetic Surgeons. Despite profound expertise and advanced surgical techniques, ischemia-induced complications ranging from wound breakdown to extensive tissue necrosis are still occurring, particularly in reconstructive flap surgery. Multiple experimental flap models have been developed to analyze underlying causes and mechanisms and to investigate treatment strategies to prevent ischemic complications. The limiting factor of most models is the lacking possibility to directly and repetitively visualize microvascular architecture and hemodynamics. The goal of the protocol was to present a well-established mouse model affiliating these before mentioned lacking elements. Harder et al. have developed a model of a musculocutaneous flap with a random perfusion pattern that undergoes acute persistent ischemia and results in ~50% necrosis after 10 days if kept untreated. With the aid of intravital epi-fluorescence microscopy, this chamber model allows repetitive visualization of morphology and hemodynamics in different regions of interest over time. Associated processes such as apoptosis, inflammation, microvascular leakage and angiogenesis can be investigated and correlated to immunohistochemical and molecular protein assays. To date, the model has proven feasibility and reproducibility in several published experimental studies investigating the effect of pre-, peri- and postconditioning of ischemically challenged tissue.

Background: Shockwave (SW) application has been shown to limit flap necrosis. However, the underlying microhemodynamic mechanisms remain unclear. Therefore, the objective of this study was to analyze the effect of SW application on a microcirculatory level., Methods: We treated 12 C57BL/6 mice with local SW application (500 shockwave impulses at 0.15 mJ/mm(2)) either 24 h before (preconditioning [PRE]) or 30 min after (postconditioning [POST]) flap elevation. Animals with an untreated flap (CON) or without a flap served as controls. We applied dorsal skinfold chambers to the animals and performed epifluorescence microscopy over a 10-d period to assess microcirculatory parameters (arteriolar diameter, red blood cell velocity, blood flow, functional capillary density, and intercapillary distance) as well as inflammation, apoptotic cell death, and necrosis., Results: SW application significantly decreased tissue necrosis independently of the application time point (PRE: 29% ± 7%; POST: 25% ± 7% versus CON: 47% ± 2%; day 10, P < 0.05). Arteriolar diameter, red blood cell velocity, and blood flow were not statistically significantly different among the 3 flap groups. However, SW (PRE and POST) resulted in an early and persistent increase in functional capillary density and consequently decreased intercapillary distance compared with CON and the group without a flap (P < 0.05). Also, SW resulted in a significantly decreased inflammatory response (P < 0.05) and induced an angiogenic response, as indicated by new functional microvessel formation observed 5 d after therapy., Conclusions: Local SW application improved tissue survival by recruitment of sleeping capillaries within the non ischemic tissue and maintenance of capillary perfusion within the critically perfused tissue after induction of ischemia, which was independent of the application time point. Neoangiogenesis occurred beyond the ischemic tolerance of the tissue, and therefore does not seem to contribute to improved tissue survival., (Copyright © 2013 Elsevier Inc. All rights reserved.)

Phalloplasty in female-to-male transsexuals is a very demanding procedure concerning preoperative planning and surgical implementation and many operative techniques have been described in the past. Here we illustrate the phalloplasty by means of a pedicled anterolateral-thigh-flap ("ALT-flap") in a patient who underwent unsuccessful groin-flap-phalloplasty alio loco. The sensible innervation was constituted via coaptation to a branch of the pudendal nerve. The technique presented here shows an aesthetically appealing result 6 months postoperatively with a 2-point discrimination of 2.5 cm and ongoing reinnervation. The operative time was reasonable with 360 min. The benefit of a pedicled transposition vs. a free transplantation becomes obvious especially as a salvage procedure in a preoperated situs with altered vascular anatomy. Additionally the shortened operating time and the lack of possible complications of microvascular anastomoses bear advantages. In conclusion we consider this technique for phalloplasty concerning operating time and effort, complications, donor site morbidity and aesthetic result as an appropriate alternative to established methods in selected patients., (© Georg Thieme Verlag KG Stuttgart · New York.)

Preoperative Doppler ultrasonography for planning free perforator flaps is widely established to identify preoperatively perforators. The method allows one to localise the penetrating point of the perforator through the abdominal fascia. By this means it is not possible to see the intramuscular course or the position of the perforator in relation to the inferior epigastric artery. Lately the technique of computed tomographic angiography provides an opportunity for visualising the course of perforator vessels in these tissues. This paper summarises our experience with the preoperative CT angiography in our breast centre. Since spring 2009 we have reconstructed the breasts of 44 female patients by using free flaps from the lower abdominal wall. 6 of these were bilateral. In a total number of 50 breast reconstructions we used 23 deep inferior epigastric perforator (DIEP) flaps and 27 muscle-sparing transverse rectus abdominis muscle (TRAM) flaps. In addition to the preoperative ultrasonography, a CT angiography of the lower abdomen was conducted in 29 patients. On average they showed at least 2 perforators on the left as well as right abdominal sides, which could be used as flap vessels based on their signal intensity. Based on their estimated microsurgical dissection complexity, the perforator vessels could be classified into 3 groups: 1) direct perforators of category A with short intramuscular course (39%), 2) perforators with long intramuscular course of category B (50%) and 3) "turn around" perforators of category C, which pass medially around the rectus abdominis (11%). The technique of CT angiography permits a reliable preoperative visualisation of perforators in their entire course and facilitates the selection of the supplying perforator as well as the intraoperative procedure for the surgeon. The suggested classification of perforators into 3 groups simplifies the preoperative assessment of the microsurgical dissection effort. Compared to the commonly used Doppler ultrasonography there are disadvantages like the additional cost factor and the radiation exposure. However, in our experience the more detailed planning increases the safety of flap raising and reduces surgery time, so that we consider CT angiography a positive tool to facilitate free perforator flaps., (© Georg Thieme Verlag KG Stuttgart · New York.)

The prevention of ischaemia and the adequate restitution of blood flow to ischaemic tissue are pivotal to halt the progression of cellular injury associated with decreased oxygen and nutrient supply. Accordingly, the search for novel strategies which aim at preventing ischaemia-reperfusion-induced tissue damage is still of major interest in flap surgery. Preconditioning represents an elegant approach to render the tissue more resistant against deleterious ischaemic insults. For many decades, 'surgical delay' has been the standard method of tissue preconditioning. During the last 10 years, ischaemic preconditioning was added to the repertoire of plastic surgeons to protect flaps from ischaemic necrosis. The invasiveness and expenditure of time of these procedures, however, have always been major drawbacks, hindering a wide distribution in clinical practice. Consequently, the motivation has all along been to further refine and simplify protective strategies. Recent experimental studies have now shown that efficient protection from ischaemic necrosis can also be achieved by remote preconditioning or pretreatment with chemical agents and growth factors, which mimic the action of surgical delay and ischaemic preconditioning. In addition, the local application of unspecific stressors, including both heating and cooling, have been shown to effectively improve flap microcirculation and, thus, tissue survival. In view of successful translational research, it is now time that the efficacy of these novel preconditioning procedures is proven in prospective randomised clinical trials.

Background: The aim of this study was to examine the impact of our multidisciplinary therapeutical concept (MTC) on the clinical outcome in posttraumatic defect injuries of the lower extremity., Patients and Methods: Since 1999, we have performed a free tissue transfer in 37 patients. In group 1 (n = 21), our MTC was started more than two weeks (range: 17 to 83 days) after trauma and in group 2 (n = 16) within two weeks (range: four to twelve days) after trauma. Both groups were comparable with respect to trauma severity., Results: 100 % patients of group 1 and 25 % patients of group 2 had clinical and microbiological manifestations of wound infection after admission in our clinic. Significantly more operations were necessary in group 1 compared to group 2 (7.3 +/- 2.6 versus 2.9 +/- 0.8; p < 0.05), until definitive wound closure could be achieved. The postoperative course was uneventful in 35 cases. All injured extremities could be salvaged. Multistaged bone reconstructions by means of spongious or tricortical bone from the iliac crest were successful in all cases. Hospital stay after admission to our clinic was significantly longer in group 1 compared to group 2 (57 +/- 12 days versus 31 +/- 7 days, p < 0.05)., Conclusions: In our study, the early start of MTC significantly reduced the risk for wound infection, the number of surgical revisions before definite wound closure and time until bone and wound healing was completed.

Plastic Surgery has been focussing on wound healing and therefore also on angiogenic regulatory mechanisms since many years. Nowadays most of the pioneering work including different plastic surgical techniques like tissue transfer and microsurgery are surgical standard. But only since 10 to 15 years we have tools and methods to investigate the molecular mechanisms of functional angiogenesis in vivo. This article presents some mechanisms and therapeutic strategies for angiogenesis, which can be applied both clinically and experimentally in plastic surgery.

The authors' experience indicates that good functional results are achievable in cases with wide resection of the knee extensor mechanism. They report a low-grade myxoid fibrosarcoma patient in whom a one-stage functional reconstruction was carried out by transfer of a free latissimus dorsi flap without secondary surgical procedures.

Background: Injections of single-dose vascular endothelial growth factor (VEGF)(165) have been advocated as a therapeutic tool for angiogenesis in ischemic flaps. We challenged this thesis by employing both VEGF(165) and vascular endothelial growth factor receptor-1 (VEGFR-1) (for competitive inhibition of VEGF signal transduction) in different experimental settings of an ischemic rat flap model., Material and Methods: 80 isogenic rats were divided in two groups of 40 animals (groups 1A-1D and 2A-2D). The ischemic target was a 7 x 7-cm epigastric island flap, based on the right inferior epigastric pedicle. Group 1 received flap treatment 1 week prior to flap elevation by test substance injection into its flap panniculus carnosus: 1 ml NaCl 0.9% (1A), 1 ml Dulbecco's modified Eagle's medium (1B), 1.0 microg VEGF(165) (1C), and 10 microg sFLT-1 with 1.0 microg VEGF(165) (1D). sFLT-1 is a soluble receptor for VEGF and is able to prevent VEGF signaling through the cell surface receptor. Group 2 had the same flap treatment at the day of flap elevation., Results: In group 1C we found the most vital flap tissue, without reaching significance. Compared with group 1D, however, significantly more flap tissue maintained vital. In groups 2A-2D, no significant results were found with respect to flap survival., Conclusions: Local application of single-dose VEGF(165) 1 week prior to ischemia dose not have significant clinical angiogenic effects. In this experimental setting, VEGF(165)-induced angiogenic effects can be significantly inhibited by adding sFLT1 in vivo. A single-dose of VEGF(165) under ischemic conditions causes no significantly better flap survival in this model.

Background: The aim of this study was to induce therapeutic angiogenesis in ischemically challenged flap tissue by means of gene transfer., Methods: Isogenic rat fibroblasts were retrovirally transfected to produce platelet-derived growth factor (PDGF)-AA. Stable gene expression was monitored by PDGF-AA enzyme-linked immunosorbent assay. Eighty animals were divided into 2 groups (1 and 2), each with 4 subgroups. The angiogenic target was a 7 x 7-cm epigastric island flap used as a necrosis model. Group 1 received flap treatment 1 week before flap elevation: 10(7) genetically modified fibroblasts, expressing PDGF-AA (genetically modified fibroblasts) plus 1 mL of Dulbecco's modified Eagle's medium (DMEM) (1A), 10(7) nonmodified fibroblasts (NMFB) plus 1 mL of DMEM (1B), 1 mL of DMEM (1C), and 1 mL of sodium chloride 0.9% (1D). All substances were injected at evenly distributed spots into the panniculus carnosus of the entire flap. Group 2 had the same flap treatment at the day of flap elevation. All flaps were sutured back. Seven days later, the flaps were harvested and examined both clinically, histologically, and immunohistochemically., Results: In vitro, the GMFB produced up to 117.9 +/- 57.2 ng of PDGF-AA/mL medium during a 4-day period, compared with 0.7 +/- 0.6 ng of PDGF-AA/mL medium produced by NMFB in the same time period. In vivo production of PDGF-AA in flaps amounted to 1.3 +/- 0.7 ng of PDGF-AA/1 microL flap tissue for group 1A and 1.7 +/- 1.1 ng of PDGF-AA/1 microL flap tissue for group 2A seven days after cell transplantation. Fibroblasts persisted in all flaps from groups 1A, 1B, 2A, and 2B without major inflammatory reaction. Clinically, group 2A developed significantly less flap necrosis compared with all other groups, including group 1A. Accordingly, only group 2A gave significant histologic and immunohistochemical evidence for enhanced angiogenesis within the flap tissue., Conclusions: After retroviral gene transfer, isogenic rat fibroblasts produce high amounts of PDGF-AA in vitro. In vivo, PDGF-AA can be detected in flaps receiving genetically modified fibroblasts, which suggests survival of the implanted fibroblasts in this model. PDGF-AA produced by GMFB can induce flap angiogenesis only under ischemic conditions in this model. Transplantation of PDGF-AA-overexpressing fibroblasts results in higher flap survival in this model.

In a previous study, the authors found persistence of pedicle blood flow up to 10 years after uncomplicated free latissimus dorsi transfer. In this study, the impact of postoperative complications (hematoma, thrombosis, infection) and successful surgical revision was tested. Since 1982, more than 1200 free tissue transfers have been performed at the authors' institution (Hannover Medical School). Of these, the authors selected two groups of 30 patients each who had received a free latissimus dorsi transfer to the lower leg without microsurgical nerve coaptation for wound coverage. All patients included in this study were carefully selected for clinical homogeneity, with one difference: group I comprised patients who had no postoperative complications after free latissimus dorsi transfer. Group II included only patients with major postoperative complications after the procedure. All flaps in group II survived after successful surgical revision. The arteries, which nourished the lower leg, were visualized and documented by means of a duplex scanner in both groups. Three different time intervals were chosen for measurements of blood flow: 4 to 6 months (groups I.I and II.I), 4 to 6 years (groups I.II and II.II), and 8 to 10 years (groups I.III and II.III). Quantitative measurements of local flap perfusion in milliliters per minute per 100 g tissue were performed by means of the hydrogen clearance technique. In each patient, a total of nine measurements was performed in three phases: phase A, before closing the vascular pedicle by manual compression (n = 3); phase B, with a closed pedicle (n = 3); and phase C, after releasing the vascular pedicle from manual compression (n = 3). Each measurement took approximately 10 minutes. One hundred percent closure of each pedicle in phase B was confirmed by the duplex scanner. Furthermore, all patients were monitored both clinically and by means of the hydrogen clearance technique during phase B for adequate blood supply to the lower leg. Lower leg perfusion showed no statistical differences for phases A, B, and C in all groups of patients. In group I, no statistical differences in local flap perfusion were encountered for phases A and C. In phase B, however, a statistically significant (p < 0.01) complete extinction of local flap perfusion was registered in all patients of group I at the site of the flap's skin paddle. In group II, however, persistent flap perfusion was registered during phase B in up to 50 percent of cases in one subgroup (II.III). No statistically significant alterations of local blood flow were registered in the surrounding tissue of group II during phases A, B, and C. Patients with thrombosis of the venous anastomosis (n = 7) seemed to have the highest incidence of loss of autonomous blood supply through the vascular pedicle (5 out of 11 cases). No inconstant results were found during the repetitive measurements (n = 3) for each patient in each phase. After uncomplicated free tissue transfer, the flap's intact vascular pedicle seems to play an important role in permanent flap survival up to 10 years after the procedure. Postoperative complications after free tissue transfer with successful surgical revision, especially venous thrombosis of the vascular anastomosis, may lead to loss of vascular flap autonomy over time.

In a retrospective study, including 60 patients, after free latissimus dorsi transplantation (FLDT) to the lower leg, we found persistent pedicle blood flow up to ten years postoperatively. After uncomplicated FLDT, the pedicle supported the flap in all cases. After complicated FLDT (hematoma, thrombosis, infection) only 50% of all flaps after ten years were perfused by the vascular pedicle.

The necessity for routine angiography in evaluation of the vasculature of recipient legs prior to microsurgical free-tissue reconstruction still remains controversial. This prospective study was designed to determine the indications and limitations of angiography pertinent to this issue. The protocol consisted of palpation of dorsalis pedis and posterior tibial artery pulsation and lower-limb angiography prior to surgery, as well as intraoperative observation of the posterior tibial, anterior tibial, and peroneal arteries. Thirty-six patients were included who were treated from November, 1993 to December 1998. In five patients either the posterior tibial pulse (1), the dorsalis pedis pulse (3), or both pedal pulses, including the popliteal pulse (1) were not palpable preoperatively. These clinical findings correlated with the vascular lesion images on angiography. In two patients, pedal pulse palpation could not clearly be evaluated because of the injury. Among the 29 patients with both pedal pulses palpable, three patients angiographically presented an injury of the peroneal artery, and one patient a pseudoaneurysm of the anterior tibial artery. In none of the cases with at least one palpable pedal pulse (33), did preoperative angiography add relevant information which led to a plan change in the free-flap transfer. However, in two cases, severe scarring and fibrosis required an intraoperative change of the recipient vessel in one case, and a change of the anastomosis level and use of a vein graft, in the other case, although the angiography had demonstrated normal vascularity in both. The authors conclude that preoperative angiography is indicated only when both pedal pulses are not palpable, and that normal preoperative angiography does not guarantee the presence of vessels suitable for anastomosis.

A 36-year-old woman sustained an amputation of her right leg at the thigh level and a degloving injury of her left foot and ankle region in an accident during a suicide attempt. Primarily, her left foot was covered with a split skin graft, resulting in a soft-tissue defect at the medial malleolus and at the calcaneus bone. Reconstruction was planned with a free latissimus dorsi muscle flap. Preoperative examinations revealed an arteria peronea magna with a hyperplastic peroneal artery solely providing arterial blood supply to the foot. The arteria peronea magna divided into two branches proximal to the upper ankle joint, replacing the dorsal pedis artery and the medial plantar artery. Tibial posterior and tibial anterior arteries were hypoplastic-aplastic. Microvascular end-to-end anastomoses of the flap vessels to the medial branch ("medial plantar artery") of the arteria peronea magna and its concomitant vein at the medial malleolar bone level were successfully performed. The postoperative course was uneventful. Four weeks postoperatively, the patient started walking assisted by a prosthesis on her right thigh stump. This experience demonstrates that even in a case of arteria peronea magna, free flap surgery for lower limb salvage is a reliable and worthwhile method.

Success rates after free tissue transplantation (FTT) have greatly improved over the last 20 years, partly due to improved technical performance of microvascular anastomoses with better optical and instrumental aids. However, flap failure is still a clinical problem and occurs in 5 to 10%, mainly due to blood vessel thrombosis within the first 24 postoperative hours. The clinical results after FTT can be optimized by in-time diagnosis of irreversibly compromised tissue blood flow and immediate operative reexploration. Therefore, there is a special demand for adequate and reliable postoperative monitoring techniques. This article reviews all monitoring techniques which have been performed in the experimental-clinical setting after FTT thus far.

The hypothesis of whether or not flap perfusion remains persistent through its vascular pedicle up to 10 years after free tissue transfer was tested. Since 1982, more than 1,000 free tissue transfers have been performed at this institution. Of these, 40 patients were selected with comparable posttraumatic soft-tissue defects of the lower leg and surgical repair by a latissimus dorsi myocutaneous free flap. All patients had a postoperative course free of complications. Measurements of flap perfusion were started in groups 1 through 4 (each 10 patients) 3 to 5 weeks, 5 to 7 months, 4 to 6 years, and 8 to 10 years after free tissue transfer, respectively. Quantitative measurements of local flap perfusion were performed by means of the hydrogen clearance technique (Ameda, Switzerland) at definite sites intracutaneously and subcutaneously within the flap's skin paddle as well as in the adjacent intracutaneous and subcutaneous skin of the surrounding soft tissue. Simultaneously, the vascular pedicle of the flap was visualized by a duplex scanner (Toshiba, Japan). In each group nine measurements were performed before (phase A), during (phase B), and after closing the pedicle (phase C) by manual compression. Each measurement took about 10 minutes. Statistical evaluation of the obtained values was achieved by the Mann-Whitney U test and the Wilcoxon signed rank test. Local flap perfusion showed no statistical differences for phase A and C in all four groups of patients. In phase B, however, a statistically highly significant (p < 0.01) absence of local flap perfusion was registered in all four groups at the site of the flap's skin paddle. No statistically significant alterations of intracutaneous and subcutaneous blood flow was found in the surrounding soft tissue. In our clinical-experimental setting, flap perfusion persisted by means of its vascular pedicle even 10 years after free tissue transfer. Our findings support the importance of an intact vascular pedicle for permanent flap survival after free tissue transfer.

In a prospective study including 60 patients after free latissimus dorsi transfer (FLDT) to the lower leg, we found persistence of pedicle blood flow up to 10 years postoperatively. After uncomplicated FLDT the pedicle supported the flap in all cases, whereas after complicated FLDT (hematoma, thrombosis, infection) we found only 50% of all flaps autonomously perfused by the vascular pedicle.

Gene therapy was tested for inducing functional angiogenesis in the superficial rat epigastric island flap to allow earlier pedicle division. Autologous rat fibroblasts were grown, harvested, cultured and retrovirally transfected to produce PDGF-AA, an angiogenetically active protein. Stable gene expression was monitored by PDGF-AA ELISA. 180 animals were divided into 3 groups (I-III) and a bilateral flap created in each animal. In all experiments, the rightsided flap was subjected to experimental treatment and the left-sided flap served as control (1 ml saline 0.9%). During flap elevation, group I received 5 x 10(6) GMFB (genetically modified fibroblasts) plus 1 ml DMEM as medium. Group II was treated with 5 x 10(6) NMFB (non modified fibroblasts) plus 1 ml medium and group III received 1 ml medium alone. The flaps were sutured back and the vascular pedicle was bilaterally ligated and divided in each 10 animals during the following 6 days. 7 days later, the flaps were harvested, the amount of necrosis measured and histologically examined. The GMFB produced up the 560-times more PDGF-AA than the NMFB, measured by ELISA. The GMFB-treated flaps tolerated surgical division of the vascular pedicle significantly earlier than groups II and III. Histologically, fibroblasts persisted in all flaps of groups I and II without major inflammatory reaction. In all GMFB-treated flaps, massive angiogenesis could be demonstrated. By means of retroviral gene transfer autologous rat fibroblasts can be genetically modified for stable expression of the PDGF-A gene to produce high amounts of PDGF-AA, which is angiogenetically active. After injection into the panniculus carnosus, these cells induce functional angiogenesis to permit earlier division of the vascular pedicle in this flap model.

Mentosternal contractures represent a surgical challenge to the plastic and reconstructive surgeon. We add the supraclavicular artery island flap to the armamentarium of surgical procedures to improve the function and cosmesis of disfigured patients. Since July of 1994, the supraclavicular artery island flap has been used at our institution for releasing postburn mentosternal contractures in eight patients. The flap was planned to be 4 to 10 cm in width and 20 to 30 cm in length with the supraclavicular vessels running axially. All donor defects could be closed primarily without significant postoperative complications in seven of the eight patients. All flaps healed primarily, achieving a good functional result by complete removal of contracting scar tissue for all patients; one donor site healed by secondary intention. We found the supraclavicular artery island flap both reliable and safe for immediate resurfacing after resection of cervical scars. The anatomy, operative procedure, and postoperative results of the supraclavicular artery island flap are outlined in this paper.

The hydrogen clearance technique was introduced for monitoring postoperative blood flow after free-tissue transfer in this prospective clinical study. This technique allows unlimited repeatable quantitative measurements of tissue blood flow in milliliters per minute per 100 gm of tissue at any site including buried flaps. In this study a real-time blood flow measuring system (Ameflow, Ameda, Switzerland) was employed. Two thousand eight hundred and twenty-three blood flow measurements were carried out on 72 free-tissue transfers, which were performed on 71 patients. Nine of these 72 flaps showed vascular complications (12.5 percent), including arterial thrombosis in 6.9 percent (n = 5), hematoma in 4.2 percent (n = 3), and venous thrombosis in 1.4 percent (n = 1). Complications as well as uneventful postoperative cases were monitored correctly by the hydrogen clearance technique in all cases, reaching sensitivity and specificity values of 1.0 for this technique in our study. Furthermore, all complications could be detected earlier by the hydrogen clearance technique than by clinical monitoring alone, which allowed flap salvation in 7 of 9 cases and a resulting permanent failure rate of free-tissue transfer of 2.8 percent (n = 2). From our data we conclude that the hydrogen clearance technique is a promising tool for postoperative blood flow monitoring after free-tissue transfer. For experimental pathophysiologic and pharmacologic studies of tissue blood flow in flaps, further evaluation of our measuring device including comparative studies with other established techniques is highly recommended.

Mentosternal contractures still represent a surgical challenge due to their exposed location. They require early operative treatment for both functional as well as aesthetic reasons. Careful clinical examination of scar location and traction forces both in the resting and functional, moving state including proper evaluation of the surrounding soft tissue is mandatory for exact preoperative planning of the reconstructive surgical procedure required. In general, the technically most feasible operation is favoured, if functional and aesthetic results are good and the postoperative risk of recurrent mentosternal contractures is low. Between 1987 and 1994, 21 patients with cervical, mentosternal and mentothoracic contractures underwent operative procedures at our clinic. Eight patients underwent a surgical reconstruction with local flaps and 13 patients received a free microvascularized flap.

The case of a four-year old girl with arthrogryposis multiplex congenita is reported. Primary clinical examination revealed bilateral passive elbow flexion only 0-20-70 without active elbow flexion. A two-stage operative procedure was chosen for both sides. At first an elbow arthrolysis was performed with subsequent intensive physiotherapy. In a second session we performed a bipolar transposition of the latissimus dorsi as a myocutaneous flap. The patient was first operated on the left and afterwards on the right side. By this procedure, the patient achieved a significant improvement of both passive and active elbow flexion mobility. The range of active motion was 0-30-90 and 0-40-90 degrees.

Success rates for free tissue transfer (FTT) have greatly improved over the last 20 years, partly due to improved technical performance of microvascular anastomoses with better optical and instrumental aids. However, flap failure is still a clinical problem and occurs in 5-10%, mainly due to blood vessel thrombosis within the first 24 postoperative hours. Salvation rates of failing FTT can be optimized by the prompt diagnosis of irreversibly compromised tissue blood flow and immediate operative reexploration. Therefore, there is a special demand for adequate and reliable postoperative monitoring techniques. This article gives an overview of all monitoring techniques, which have been performed both in the experimental and clinical setting after FTT thus far.

The purpose of this study was to compare the hydrogen clearance technique (HCT) with two different, well-established techniques, i.e., the laser Doppler flowmetry (LDF) and the Erlangen micro-lightguide spectrophotometer (EMPHO), for tissue blood flow measurements in an experimental setting. For the animal experiments, we chose a rat model for arterial and venous flap thrombosis, using the epigastric groin flap. Forty male dark Aguty rats were included in the study. The animals were divided into eight groups, each with a different vascular thrombotic model. HCT was used to collect 1,467 measurements, and 2,934 graphs were recorded; 27 measurements (54 graphs) had to be discarded due to faulty electrode placements and electronic noise. In 19 of the 27 discarded measurements the cause of failure was moving of the awaking animal with disruption of the decay signal. The LDF and EMPHO measurements were performed continuously during each measuring phase. Simple and multiple linear regression and paired t-tests were used to compare the three techniques. The reproducibility of registered blood flow values in phases 1, 3, 6 and 7 varied between 7.8% and 13.6% which is in about the same range as LDF and EMPHO. We could not find a significant difference between the three techniques in this regard. The correlation coefficient for HCT and LDF was r = 0.89. For HCT and EMPHO we found r = 0.67. Sensitivity and specificity values for HCT were both 1.0 in detecting arterial and venous flap thrombosis; for LDF we found values of 0.89 and 0.92, respectively. Sensitivity and specificity values for EMPHO were 0.92 and 0.95, respectively. After careful evaluation of three different techniques (HCT, LDF, and EMPHO) for measurements of local tissue blood flow we came to the conclusion that HCT must be favoured as a reliable tool for quantitative measurement of local tissue blood flow and early diagnosis of arterial and venous flap thrombosis.

Between 1981 and 1992, 631 cases of free tissue transfer were operated on at our clinic. Of these cases, 489 were reviewed for vascular complications. The aim of the study was to evaluate retrospectively possible correlations between complications and operative faults. Vascular complications occurred in 16%. By immediate operative revision 50% of the cases with a vascular complication could be salvaged. In general, earlier diagnosis and treatment of vascular complications after free tissue transfer prompted a higher success rate of the surgical revision.

The use of laser Doppler flowmetry (LDF) was tested in 108 consecutive patients, who underwent free tissue transfer at our institution between December, 1988, and June, 1990. Sixteen patients (14.8%) suffered vascular complications, which could be detected in all cases earlier by LDF than by clinical examination alone. LDF therefore prompted faster surgical intervention in these cases, allowing flap salvation in 9.3% (n = 10). Difficulties had to be noted in diagnosing venous thrombosis by means of the LDF due to sometimes unspecific alterations in LDF signals. Technical improvement of LDF might lead to a higher diagnostic accuracy if venous thrombosis occurs.
