Aesthetic procedures are widely performed by dermatologists and plastic surgeons, often with subjective results. In cosmetic dermatology, there is a lack of studies that use accurate instruments to assess result outcomes. Quality of life (QoL) questionnaires can objectively assess results.To assess the impact of nonsurgical facial cosmetic procedures on QoL.This was a longitudinal observational study performed with 81 patients who underwent facial cosmetic procedures at a University Hospital in Rio de Janeiro, Brazil. The Skin Rejuvenation Outcome Evaluation questionnaire, previously adapted and translated to the Portuguese language spoken in Brazil, was selected, and the results were compared with the Dermatology Life Quality Index. The impact on QoL was statistically evaluated after the interventions.The Skin Rejuvenation Outcome Evaluation scores after the procedures were significantly higher than before ( p.001), independent of sociodemographic characteristics. Patients treated with botulinum toxin and dermal fillers showed greater satisfaction than those treated with laser, chemical peels or microneedling.Minimally invasive cosmetic procedures result in improvements in QoL. The use of translated, adapted, and validated questionnaires favors the comparison of results among different populations and reinforces the confidence on the results of established cosmetic treatments.

Introdução: Com frequência, pacientes solicitam avaliação e tratamento para o rejuvenescimento das regiões perioral e periorbitária. Na maioria das vezes, é preciso usar uma combinação de procedimentos para que o resultado atenda às expectativas. Objetivo: Por meio de um estudo prospectivo, unicêntrico e comparativo, comparar o rejuvenescimento das regiões perioral e periorbital, após tratamento com peeling químico médio com fenol 88% com ou sem a aplicação prévia de toxina botulínica. Métodos: O peeling regional de fenol 88% foi utilizado em 16 pacientes; em oito delas houve aplicação de toxina botulínica duas semanas antes. A avaliação foi feita por meio de fotografias padronizadas e escalas de avaliação por médicos e pacientes. Resultados: A avaliação global por médicos e pacientes no 45º dia foi considerada excelente ou boa para todas as pacientes, com pequena vantagem para o tratamento combinado na região periorbital. Conclusão: A combinação desses dois procedimentos de custo relativamente baixo, muitas vezes negligenciado pelos dermatologistas, resultou na melhora significativa das alterações do envelhecimento dessas áreas.

Esta carta traz uma reflexão surgida a partir da palestra da professora Fatimata Ly, da University Cheikh Diop de Dakar (África), no último Congresso da Academia Europeia de Dermatologia, em Paris. A professora F. Ly proferiu a palestra Depigmentation: when, where and how. O tom de pele uniforme é um dos critérios de beleza. Esta carta não diz respeito aos pacientes que chegam ao consultório médico e que são cuidadosamente acompanhados e monitorados por dermatologistas criteriosos. Ela quer chamar a atenção para os indivíduos que não chegam aos consultórios, que repetem prescrições de conhecidos ou que mantêm por tempo indefinido uma prescrição realizada por dermatologista numa consulta pontual. Recentemente, participei, de forma anônima, de fóruns de “discussão” na internet de pessoas leigas sobre melasma. A intensidade e a velocidade do compartilhamento em relação àquilo que eles aconselham são expressivas. A criatividade é enorme ao sugerirem usar produtos que podem causar algum dano. Entre os clareadores mais utilizados estão esteroides, hidroquinona, mercúrio e ácidos. Há que se realizar uma campanha de conscientização da população em relação aos perigos de se utilizarem despigmentantes cutâneos sem indicação precisa do médico dermatologista.

A aplicação de toxina botulínica nos pacientes com sequela de paralisia de Bell é uma terapia auxiliar de extrema valia para a redução da sincinesia e de assimetrias faciais. A paralisia de Bell é a causa mais comum de paralisia do neurônio motor facial. Após a fase de paralisia dos músculos da face, o quadro pode evoluir com assimetria facial e sincinesia. No tratamento da sincinesia, as injeções pontuais no músculo orbicular e platisma aliviam os espasmos. A aplicação seletiva na hemiface não acometida objetiva reduzir a assimetria facial e seu impacto social negativo, com melhora da qualidade de vida.

Background: Scientific literature estimates that around 18 to 40 % of asymptomatic third molars are extracted. The aims of the present study were to determine the indications for extraction of asymptomatic lower third molars in a sample of Spanish and Portuguese dentists, and to relate these indications to the clinicians’ training and professional experience. Material and Methods: A survey consisting of 15 cases of asymptomatic lower third molars was emailed to Portuguese and Spanish dentists. The clinicians were asked to assess the level of difficulty of the extractions and to make a reasoned recommendation based on the panoramic radiographs, gender and age of the patients. Results: 381 clinicians filled in the questionnaires. Most of the professionals had over 13 years of clinical experience. The number of Spanish clinicians with postgraduate degrees in Oral Surgery was significantly higher. On average, 42% of respondents recommended extraction of asymptomatic third molars. Objectives: The purpose of this study was to determine the incidence and clinical symptoms associated with sharp mandibular bone irregularities (SMBI) after lower third molar extraction and to identify possible risk factors for this complication. Study Design: A mixed study design was used. A retrospective cohort study of 1432 lower third molar extractions was done to determine the incidence of SMBI and a retrospective case-control study was done to determine potential demographic and etiologic factors by comparing those patients with postoperative SMBI with controls. Results: Twelve SMBI were found (0.84%). Age was the most important risk factor for this complication. The operated side and the presence of an associated radiolucent image were also significantly related to the development of mandibular bone irregularities. The depth of impaction of the tooth might also be an important factor since erupted or nearly erupted third molars were more frequent in the SMBI group. Conclusions: SMBI are a rare postoperative complication after lower third molar removal. Older patients having left side lower third molars removed are more likely to develop this problem. The treatment should be the removal of the irregularity when the patient is symptomatic. Key words:Third molar, postoperative complication, bone irregularities, age.
