Objectives: New temperature management concepts of moderate and mild hypothermic circulatory arrest during aortic arch surgery have gained weight over profound cooling. Comparisons of all temperature levels have rarely been performed. We performed direct and indirect comparisons of deep hypothermic circulatory arrest (DHCA) (≤20°C), moderate hypothermic circulatory arrest (MHCA) (20.1–25°C), and mild hypothermic circulatory arrest (mild HCA) (≥25.1°C) in a network meta-analysis. Methods: The literature was systematically searched for all papers published through February 2022 reporting on clinical outcomes after aortic arch surgery utilizing DHCA, MHCA and mild HCA. The primary outcome was operative mortality. The secondary outcomes were postoperative stroke and acute kidney failure (AKI). Results: A total of 34 studies were included, with a total of 12,370 patients. DHCA was associated with significantly higher postoperative incidence of stroke when compared with MHCA (odds ratio [OR], 1.46, 95% confidence interval [CI], 1.19–1.78) and mild HCA: (OR, 1.50, 95% CI, 1.14–1.98). Furthermore, DHCA and MHCA were associated with higher operative mortality when compared with mild HCA (OR 1.71, 95% CI, 1.23–2.39 and OR 1.50, 95% CI, 1.12–2.00, respectively). Separate analysis of randomized and propensity score matched studies showed sustained increased risk of stroke with DHCA in contrast to MHCA and mild HCA (OR, 1.61, 95% CI, 1.18–2.20, p value =.0029 and OR, 1.74, 95% CI, 1.09–2.77, p value =.019). Conclusions: In the included studies, the moderate to mild hypothermia strategies were associated with decreased operative mortality and the risk of postoperative stroke. Large-scale prospective studies are warranted to further explore appropriate temperature management for the treatment of aortic arch pathologies.

OBJECTIVES The Berlin Heart EXCOR (BHE) offers circulatory support across all paediatric ages. Clinically, the necessary care and the outcomes differ in various age groups. The EUROMACS database was used to study age- and size-related outcomes for this specific device. METHODS All patients RESULTS In total, 303 patients were included [mean age: 2.0 years (interquartile range: 0.6–8.0, males: 48.5%)]. Age and BSA were not significantly associated with mortality (n = 74, P = 0.684, P = 0.679). Factors associated with a transplant (n = 175) were age (hazard ratio 1.07, P = 0.006) and aetiology other than congenital heart disease (hazard ratio 1.46, P = 0.020). Recovery rates (n = 42) were highest in patients with a BSA of CONCLUSIONS Mortality rates in Berlin Heart-supported patients cannot be predicted by age or BSA. Early post-operative sinus node dysfunction (SND) is common in paediatric patients undergoing surgical correction of congenital heart defects (CHD). At present, the pathophysiology of these arrhythmias is incompletely understood. In this case series, we present three paediatric patients in whom we performed intraoperative epicardial mapping and who developed early post-operative SND. All patients had either an inferior or multiple sinoatrial node (SAN) exit sites, in addition to extensive conduction disorders at superior and inferior right atrium. Our findings contribute to the hypothesis that pre-existing alterations in SAN exit sites in combination with atrial conduction disorders may predispose paediatric patients with CHD for early post-operative SND. Such insights in the development of arrhythmias are crucial as it may be the first step in identifying high-risk patients.

OBJECTIVES When surgical resection is indicated for a congenital lung abnormality (CLA), lobectomy is often preferred over segmentectomy, mostly because the latter is associated with more residual disease. Presumably, this occurs in children because sublobar surgery often does not adhere to anatomical borders (wedge resection instead of segmentectomy), thus increasing the risk of residual disease. This study investigated the feasibility of identifying eligible cases for anatomical segmentectomy by combining virtual reality (VR) and artificial intelligence (AI). METHODS Semi-automated segmentation of bronchovascular structures and lesions were visualized with VR and AI technology. Two specialists independently evaluated via a questionnaire the informative value of regular computed tomography versus three-dimensional (3D) VR images. RESULTS Five asymptomatic, non-operated cases were selected. Bronchovascular segmentation, volume calculation and image visualization in the VR environment were successful in all cases. Based on the computed tomography images, assignment of the CLA lesion to specific lung segments matched between the consulted specialists in only 1 out of the cases. Based on the three 3D VR images, however, the localization matched in 3 of the 5 cases. If the patients would have been operated, adding the 3D VR tool to the preoperative workup would have resulted in changing the surgical strategy (i.e. lobectomy versus segmentectomy) in 4 cases. CONCLUSIONS This study demonstrated the technical feasibility of a hybridized AI–VR visualization of segment-level lung anatomy in patients with CLA. Further exploration of the value of 3D VR in identifying eligible cases for anatomical segmentectomy is therefore warranted.

Summary OBJECTIVES The emergence of big cardio-thoracic surgery datasets that include not only short-term and long-term discrete outcomes but also repeated measurements over time offers the opportunity to apply more advanced modelling of outcomes. This article presents a detailed introduction to developing and interpreting linear mixed-effects models for repeated measurements in the setting of cardiothoracic surgery outcomes research. METHODS A retrospective dataset containing serial echocardiographic measurements in patients undergoing surgical pulmonary valve replacement from 1986 to 2017 in Erasmus MC was used to illustrate the steps of developing a linear mixed-effects model for clinician researchers. RESULTS Essential aspects of constructing the model are illustrated with the dataset including theories of linear mixed-effects models, missing values, collinearity, interaction, nonlinearity, model specification, results interpretation and assumptions evaluation. A comparison between linear regression models and linear mixed-effects models is done to elaborate on the strengths of linear mixed-effects models. An R script is provided for the implementation of the linear mixed-effects model. CONCLUSIONS Linear mixed-effects models can provide evolutional details of repeated measurements and give more valid estimates compared to linear regression models in the setting of cardio-thoracic surgery outcomes research.

OBJECTIVES A third paediatric report has been generated from the European Registry for Patients with Mechanical Circulatory Support (EUROMACS). The purpose of EUROMACS, which is operated by the European Association for Cardio-Thoracic Surgery, is to gather data related to durable mechanical circulatory support for scientific purposes and to publish reports with respect to the course of mechanical circulatory support therapy. Since the first report issued, efforts to increase compliance and participation have been extended. Additionally, the data provided the opportunity to analyse patients of younger age and lower weight. METHODS Participating hospitals contributed pre-, peri- and long-term postoperative data on mechanical circulatory support implants to the registry. Data for all implants in paediatric patients (, European Association for Cardio-Thoracic Surgery (EACTS), The European Association for Cardio-Thoracic Surgery (EACTS) supported this study and made resources available to execute this work.

OBJECTIVES: Ventricular assist device support as a bridge to transplant or recovery is a well-established therapy in children on the cardiac transplant waiting list. The goal of this study was to investigate the incidence of and the associated factors for cerebrovascular accidents in paediatric patients supported by a Berlin Heart EXCOR. METHODS: All patients, European Association for Cardio-Thoracic Surgery, The European Association for Cardio-Thoracic Surgery supported this study and made resources available to execute this work. OBJECTIVES This retrospective study investigated the correlation between the angular position of the left ventricular assist device (LVAD) inflow cannula and relapsing low-flow alarms. METHODS Medical charts were reviewed of all patients with HeartMate 3 LVAD support for relapsing low-flow alarms. A standardized protocol was created to measure the angular position with a contrast-enhanced computed tomography scan. Statistics were done using a gamma frailty model with a constant rate function. RESULTS For this analysis, 48 LVAD-supported patients were included. The majority of the patients were male (79%) with a median age of 57 years and a median follow-up of 30 months (interquartile range: 19–41). Low-flow alarm(s) were experienced in 30 (63%) patients. Angulation towards the septal–lateral plane showed a significant increase in low-flow alarms over time with a constant rate function of 0.031 increase in low-flow alarms per month of follow-up per increasing degree of angulation (P = 0.048). When dividing this group using an optimal cut-off point, a significant increase in low-flow alarms was observed when the septal–lateral angulation was 28° or more (P = 0.001). Anterior–posterior and maximal inflow cannula angulation did not show a significant difference. CONCLUSIONS This study showed an increasing number of low-flow alarms when the degrees of LVAD inflow cannula expand towards the septal–lateral plane. This emphasizes the importance of the LVAD inflow cannula angular position to prevent relapsing low-flow alarms with the risk of diminished quality of life and morbidity.

OBJECTIVES In patients supported by a durable left ventricular assist device (LVAD), infections are a frequently reported adverse event with increased morbidity and mortality. The purpose of this study was to investigate the possible association between infections and thromboembolic events, most notable cerebrovascular accidents (CVAs), in LVAD patients. METHODS An analysis of the multicentre European Registry for Patients Assisted with Mechanical Circulatory Support was performed. Infections were categorized as VAD-specific infections, VAD-related infections and non-VAD-related infections. An extended Kaplan–Meier analysis for the risk of CVA with infection as a time-dependent covariate and a multivariable Cox proportional hazard model were performed. RESULTS For this analysis, 3282 patients with an LVAD were included with the majority of patients being male (83.1%). During follow-up, 1262 patients suffered from infection, and 457 patients had a CVA. Cox regression analysis with first infection as time-dependent covariate revealed a hazard ratio (HR) for CVA of 1.90 [95% confidence interval (CI): 1.55–2.33; P CONCLUSIONS Infection during LVAD support is associated with an increased risk of developing an ischaemic or haemorrhagic CVA, particularly in the setting of VAD-related or VAD-specific infections. This suggests the need of a stringent anticoagulation management and adequate antibiotic treatment during an infection in LVAD-supported patients.

BackgroundTo assess the current state of patient information and decision-making in congenital aortic and pulmonary valve disease, we conducted a survey among patients, parents and physicians.MethodsA questionnaire was sent by ground mail to 157 adults and 32 parents of children who previously underwent surgery for congenital aortic or pulmonary valve disease at 0–40 years of age between January 2005 and February 2014 at the Erasmus University Medical Center and to all paediatric and adult congenital cardiologists and congenital cardiac surgeons in the Netherlands (n=88).Results73 patients/parents (39% response rate, 62 adult patients, 11 parents of paediatric patients) and 35 physicians (40% response rate) responded. Median patient age at the time of surgery was 25.7 years. Basic disease-specific knowledge was adequate in 42% of patients/parents and numeracy was sufficient in 47%. Patients/parents reported that they rely heavily on their physicians for information and often experience difficulty in finding reliable information elsewhere. They lack information on psychosocial aspects of disease (29% of respondents) and risks and benefits of treatment options (26%). They feel less involved in decision-making than they would prefer to be (p=0.014). Decisional conflict at the time of surgery was experienced by 31% of patients/parents. If they had to do it again, 72% of patients/parents would want the same treatment. Quality of life is often impaired due to various valve-related anxieties and lifestyle changes. Physicians reported that they are unable to fully inform and sufficiently involve patients, due to limited patient/parent knowledge and understanding (56%) and limited time during consultations (32%). Patients/parents (98%) and physicians (97%) agree that they should have shared roles in decision-making.ConclusionThe substantial shortcomings in our current practice of patient information and decision-making underline the need for innovative solutions, such as careful implementation of patient information tools and shared decision-making in the care path.

BACKGROUND: Mechanical valves and bioprostheses are widely used for aortic valve replacement. Though previous randomised studies indicate that there is no important difference in outcome after implantation with either type of valve, knowledge of outcomes after aortic valve replacement is incomplete. OBJECTIVE: To predict age and sex specific outcomes of patients after aortic valve replacement with bileaflet mechanical valves and stented porcine bioprostheses, and to provide evidence based support for the choice of prosthesis. METHODS: Meta-analysis of published results of primary aortic valve replacement with bileaflet mechanical prostheses (nine reports, 4274 patients, and 25,726 patient-years) and stented porcine bioprostheses (13 reports, 9007 patients, and 54,151 patient-years) was used to estimate the annual risks of postoperative valve related events and their outcomes. These estimates were entered into a microsimulation model, which was employed to calculate age and sex specific outcomes after aortic valve replacement. RESULTS: Life expectancy (LE) and event-free life expectancy (EFLE) for a 65 year old man after implantation with a mechanical valve or a bioprosthesis were 10.4 and 10.7 years and 7.7 and 8.4 years, respectively. The lifetime risk of at least one valve related event for a mechanical valve was 48%, and for a bioprosthesis, 44%. For LE and EFLE, the age crossover point between the two valve types was 59 and 60 years, respectively. CONCLUSIONS: Meta-analysis based microsimulation provides insight into the long term outcome after aortic valve replacement and suggests that the currently recommended age threshold for implanting a bioprosthesis could be lowered further.

Objective: To investigate the incidence of clinical problems related to a bicuspid valve (aortic stenosis and regurgitation) and the incidence of ascending aorta and aortic arch pathology in combination with coarctation repair. Patients: 124 adult patients after surgical correction of aortic coarctation were studied. The incidence of aortic valve, ascending aorta, and aortic arch pathology was determined using echocardiography and magnetic resonance imaging. The median age at coarctation repair was 9 years and at last follow up 28 years. Results: Three patients died from aorta pathology. Aortic valve disease was found in 63% of the patients, requiring an intervention in 22%, at a median of 13 years after coarctation repair. Ascending aorta dilatation was observed in 28% and aortic arch abnormalities in 23%, among whom kinking of the aortic arch was found in 12%. Antihypertensive medication was used in 24%. In the patients with hypertension the age at operation and age at follow up were significantly higher (p = 0.0001 and p < 0.0001, respectively). Conclusion: In addition to the well known problems of hypertension and recoarctation, aortic valve and aortic arch pathology are commonly encountered in patients with previous coarctation repair. Aortic abnormalities may predispose to dilatation and dissection, thus necessitating careful lifelong attention in all patients with coarctation.

Background Aortic root replacement with cryopreserved allografts is associated with excellent hemodynamics, little endocarditis, low thromboembolic event rates, and no need for anticoagulation. There is, however, concern regarding the long-term durability of this valve substitute, especially in younger patients. Meta-analysis and microsimulation were used to calculate age-specific long-term prognosis after allograft aortic root replacement based on current evidence. Methods Our center's experience with cryopreserved allograft aortic root replacement in 165 adult patients was combined in a meta-analysis with reported and individual results from four other hospitals. Using this information, the microsimulation model predicted age- and gender-specific total and reoperation-free and event-free life expectancy. Results The pooled results comprised 629 patients with a total follow-up of 1860 patient-years (range 0 to 12.8 years). Annual risks were 0.6% for thromboembolism, 0.05% for bleeding, 0.5% for endocarditis, and 0.5% for nonstructural valve failure. Structural allograft failure requiring reoperation occurred in 15 patients, and a patient age–specific Weibull function was constructed accordingly. Calculated total life expectancy varied from 27 years in a 25-year-old to 12 years in a 65-year-old male; corresponding actual lifetime risk of reoperation was 89% and 35%, respectively. Conclusions Cryopreserved aortic allografts have an age-related limited durability. This results in a considerable lifetime risk of reoperation, especially in young patients. The combination of meta-analysis and microsimulation provides an appropriate tool for estimating individualized long-term outcome after aortic valve replacement and can be useful both for patient counseling and prognostic research purposes.

Aims Prognosis after aortic valve replacement (AVR) with mechanical prostheses depends on multiple inter-related factors. Investigation into these factors is complicated by limited knowledge on outcome after AVR and by the varying therapeutic ranges of International Normalized Ratio employed worldwide. Meta-analysis was combined with microsimulation to calculate evidence-based, age-specific outcomes after AVR with St. Jude Medical (SJM) prostheses (St. Jude Medical, Inc., St. Paul, MN, U.S.A.). Method and results Eight studies were included in a meta-analysis of published results of primary isolated AVR with SJM prostheses (2986 patients, 16,163 patient-years) in order to estimate the hazard of postoperative valve-related events. Using microsimulation, calculated life expectancy and event-free life expectancy were 22 and 16 years in males aged 35 years, and 7 and 5 years in males aged 75 years, respectively. Calculated lifetime risks for thromboembolic and bleeding events were 22% and 15% in males aged 35 years, and 7% and 37% in males aged 75 years, respectively. Varying thromboembolic and bleeding hazards resulted in considerable shifts in lifetime risks and deaths associated with these events. Conclusion Meta-analysis combined with microsimulation is a powerful tool with which to calculate reliable estimates of long-term prognosis, and allows detailed insight into the occurrence of valve-related events. Thromboembolism and bleeding occur frequently after AVR with mechanical prostheses and have an important impact on survival. Optimal control of International Normalized Ratio is therefore of utmost importance.

Large sized coronary artery fistulas are rare and diagnosed in only 0.05% of adult catheterized patients. Only a minority of these fistulas are operated upon. We describe a percutaneous technique to close a left coronary artery fistula draining into the right atrium in a 30-yr-old male patient. The fistula was closed by implantation of a trefoil coil, inserted through a catheter selectively advanced into the fistula.

Tetralogy of Fallot with pulmonary atresia (PA) comprises absence of functional continuity between the right ventricle and the pulmonary arterial tree, ventricular septal defect (VSD), and overriding of the aorta and right ventricular hypertrophy. In this setting the pulmonary blood flow may be supplied by the ductus arteriosus or by systemic-pulmonary collateral arteries (SPCA’s) (3). In patients with a well developed pulmonary arterial system, treatment mostly consists of initial palliation with a modified Blalock-Taussig shunt and later complete repair (7). On the other hand, the presence of SPCA’s is often associated with a diminutive central pulmonary arterial system (7). In this setting early and preferably complete unifocalization is necessary to attain an adequate pulmonary arterial system and to avoid imbalances of pulmonary vascular resistance. In both settings, the complete repair concerns closure of the VSD and establishment of the right ventricular-pulmonary arterial continuity with closure of previously constructed shunts. At present, the pulmonary homograft appears to be the conduit of choice for reconstruction of the right ventricular outflow tract (1, 2, 9).

Aortic root replacement with reimplantation of the coronary arteries using the pulmonary autograft (PAG) is an accepted operative technique to treat aortic valve disease in children and adults. However, the technically demanding operative technique and lack of long term results regarding function of the PAG limit widespread use. The behaviour of the PAG in the systemic circulation raises concern because of the thinner wall of the pulmonary root and the lesser amount of collagen as compared to the aortic root.1 Severe PAG dilatation may result in aortic regurgitation (AR) and subsequent reoperation. We reviewed our first cohort of adult patients who underwent this operation.2 From January 1989 until May 1995, 37 adult patients (20 male; mean age 29.1 years, range 19.3–52.1 years) underwent aortic root replacement using the PAG for aortic valve disease. The aetiology was bicuspid valve (n = 28), degenerative valve disease (n = 5), cured endocarditis (n = 3), and annuloaortic ectasia (n = 1). From 1987 to 1992, 22 children (age 0 days-14 years) were operated for congenital bronchopulmonary disease. One patient had a hamartoma. Four children had a bronchogenic cyst, in 1 patient combined with an esophageal duplication. Intrapulmonary sequestration was diagnosed in 3 children; one of whom had an esophageal duplication as well. Four children had localized emphysema. In 1 patient histology showed rhabdomyosarcoma. A lobectomy was performed following chemotherapy but recurrence was not resectable at a third thoracotomy. Ten patients had cystic adenomatoid malformation, 6 were dependent on artificial ventilation before surgery. Three patients with cystic adenomatoid malformation died in hospital: 2 after pneumonectomy who were shown to have bilateral disease and 1 died after bilobectomy and was shown to have a hypoplastic contralateral lung. Hospital morbidity involved 1 patient with recurrent pneumothorax, 2 with atelectasis of the operated lung and 3 with prolonged artificial ventilation. Late mortality involved 1 patient due to pulmonary rhabdomyosarcoma. After 1-26 months of follow-up there were no late complications. Adequate multidisciplinary treatment allows acceptable mortality and low morbidity in surgery for congenital bronchopulmonary disease in children.

A patient is described with chylothorax after harvesting the left internal thoracic artery for coronary artery bypass surgery. Conservative treatment with a medium chain triglycerides diet and tube thoracostomy was not only unsuccessful, but also resulted in the complications of malnutrition and empyema. We recommend early surgical treatment of chylothorax after coronary bypass surgery with the use of an internal thoracic artery.

A patient with Cantrell's syndrome is presented with ectopia cordis, ventricular septal defect and a left ventricular as well as a right ventricular diverticulum. One-stage correction was performed with resection of both diverticula. Closure of the ventricular septal defect was complicated due to exposure problems as a result of the malposition of the heart. Although the presentation of Cantrell's syndrome can be variable, one-stage repair is technically feasible and should be considered as the treatment of choice in this rare syndrome.

To investigate the timing of surgery in active infective endocarditis, the data of 54 patients, consecutively operated for this reason from September 1973 to May 1989, were analysed. Native valves were involved in 31 patients (57%): the aortic valve in 22, the mitral valve in 6, and both valves in 3 cases. Prosthetic valves were involved in 23 patients (43%): the aortic valve in 14, the mitral valve in 7, and both valves in 2 cases. There were no significant differences between involvement of native or prosthetic valves and mortality, morbidity, or consequences of morbidity. No significant correlation was found between causative microorganism and mortality, morbidity, or consequences of morbidity. The indication for operation was cardiac failure in 15 patients (28%), ongoing infection in 24 (44%) or a combination of these in 15 (28%). Major embolization occurred in 12 patients (22%) and affected women more than men (p = 0.05). Hospital mortality was 8 (15%). Morbidity involved 15 more patients; structural deterioration of the valve prosthesis occurred in 1 patient; nonstructural dysfunction of the valve prosthesis occurred 11 times in 10 patients; anticoagulation-related hemorrhage involved 2 patients (1 with nonstructural dysfunction of the valve prosthesis); endocarditis was diagnosed in 3 patients. The consequences of these morbid events concerned 14 patients; reoperations were done 9 times in 8 patients; mortality was valve related in 6 cases. Because 2 more patients died during the course of the study, total late mortality was 8. Probability of survival 5 years after operation was 72% (95% cl 56-83) and at 10 years 47% (95% cl 21-70).(ABSTRACT TRUNCATED AT 250 WORDS)

With great interest, we read the paper on inclusion of the transcervical approach in video-assisted thoracoscopic thymectomy (VATS) by Shigemura et al. [1]. Our recent clinical experience with thoracoscopic thymectomy supports the conclusions of their nicely conducted study. In the first nine patients with generalised myasthenia gravis (MG) that we operated upon with videothoracoscopy, we combined a cervical approach with a left-sided thoracoscopic approach. In the first stage of the operation, through a cervical incision, the cervical part of the thymus was dissected up to the anonymous vein. Thereafter, we dissected the thoracic part of the thymus through a leftsided thoracoscopic approach. The postoperative course was uneventful in all cases, and patients were much more rapidly mobilised and discharged from the hospital than in the days of trans-sternal thymectomy. In all cases, both the patient and the neurologist were satisfied with the operative result with respect to improvement of the clinical symptoms of MG during a follow-up period of two years. Due to the experience and confidence gained, we then changed our policy and in the next two patients we performed a thoracoscopic thymectomy through a right-sided approach without a cervical extension. Both patients were young slender females with generalised MG, and both objected to a cervical scar for cosmetic reasons. Neither of these two patients had a thymoma. The postoperative course was uneventful and both patients were discharged on postoperative day three. Initially the symptoms of MG diminished somewhat, but within three months MG symptoms worsened in both patients to an extent more serious then their preoperative symptoms. In both cases, preand postoperative computed tomography (CT) scans were undertaken. The postoperative CT and magnetic resonance imaging (MRI) scans of the first patient suggested that not all thymic tissue had been removed. In the second patient the postoperative CT scan showed a minimal thymus remnant. Our clinical results confirm the conclusions so elegantly drawn by Shigemura and colleagues. We fully agree with their idea that even a carefully-executed VATS approach alone may not be a sufficient procedure to acquire maximal radicality in thymectomy for MG.
