The Australian government has expressed a continuing interest in the assessment of research productivity as a basic element of a 'rationalised' research funding process. Bibliometric analyses are among the techniques that the government uses to evaluate the quality of research performance. Like many other disciplines, sociology is a target of these assessments. This paper presents data to demonstrate how Australian sociology might measure up if subjected to an inquiry using these methods. Data are provided on the standing of Australian sociology internationally and on the research contributions of major Australian universities. Several important methodological issues are reviewed and discussed. It is concluded that Australian sociologists would be well served by becoming active participants in the ongoing national debate over the assessment of research performance and the allocation of funding for tertiary education and science. In this paper, I provide a critical commentary on the current state of drug ethnography in Australia. I identify and discuss funding problems and the appropriation of ethnography by qualitative and quantitative researchers, both of which undermine the credibility and potential contribution of ethnography to the Australian drug field. I also interrogate the much-heralded but little-scrutinised idea of ‘multidisciplinary drug research’, drawing on examples of non-drug multidisciplinary research to argue that, while multidisciplinary drug research is a worthy goal, the research vision offered by advocates of such research is frequently an intellectually impoverished one. I conclude by suggesting possible ways to strengthen Australia''s capacity to conduct both stand-alone ethnographic drug research and high-quality multidisciplinary drug research involving ethnography. Background: The Hospital Anxiety and Depression Scale (HADS) is frequently used in cancer studies, yet its utility for comparing people with cancer with people in the community is uncertain.Methods: HADS scores were obtained from population-based samples of women with (n = 731) and without (n = 158) early-onset breast cancer. Psychometric properties were examined using differential item functioning (DIF) which is the presence of systematic group differences in certain response items independent of the trait being measured.Results: Women with breast cancer scored lower than reference women on anxiety (mean (SD) 7.5 (4.3) vs. 8.2 (4.0); p = 0.06) and depression (3.3 (3.2) vs. 4.2 (3.0); p = 0.003). Group differences remained following adjustment for demographics. Time since diagnosis was not related to anxiety or depression scores. DIF was present in two anxiety and five depression items. Adjustment for DIF did not substantially change the anxiety or depression group differences.Conclusion: Specific sampling or DIF effects do not explain the observation that women with breast cancer have lower levels of anxiety and depression than population controls. Recent experience in Australia has seen the requirement by the federal Department of Finance and Administration to conduct output pricing reviews of government agencies including research organisations. Health and medical research, while generally regarded as an important 'public good', is now pressed by the same demands as other research fields to account for public investments in terms of value of outcomes and value for investment. This paper reports on current trends towards international benchmarking of health and medical research performance. Comparative data from overseas show unique aspects of the Australian health and medical research funding system. Background: In 2002 the Australian National Competitive Grants System was opened to the Institute of Advanced Studies at the Australian National University as part of a commitment to transparency, competitiveness, and collaboration in national research funding. Results: The block grant to the John Curtin School of Medical Research had progressively eroded over many years. Access to the National Competitive Grants Schemes and associated infrastructure (through an agreed 'buy-in' price of 20% of block funding) has succeeded in its aims and in reversing this progressive effective decrease in funding. Conclusion: Access to the National Competitive Grant Scheme has allowed the John Curtin School of Medical Research to contribute more broadly to Australia's health and medical research effort through increased collaboration, in a transparent and competitive funding environment. This paper explores governance and control in operating room nurses' clinical practice. Traditionally, operating room nurses have been portrayed as 'handmaidens' to the surgeons, a position which implies that nurses' bodies and the knowledge they use in practice are sites of discursive control by others. This paper unsettles this understanding by showing how operating room nurses studied ethnographically in an Australian setting are both disciplined by and actively shape practice through knowing surgeons' technical requirements for surgery, through inscribing them in discourses of time, and through having deep knowledge of the surgeons' 'soul'. We argue that as a form of governance, nurses' knowledge of surgeons is a subjugated form of knowledge, located low down on a hierarchy of knowledges. Furthermore, as a form of governance that has previously been unarticulated in the literature, it transcends the traditional lines of authority and control in the nurse-doctor relationship. More than a decade has passed since the beginnings of the 'Media Wars' debate in Australia. This paper revisits the debate to determine whether there has been any significant shift in journalism research in Australia, especially in light of key changes in the Australian higher education environment. A content analysis of six leading Australian journals and a review of Australian Research Council grant applications provide a snapshot of trends in journalism research and research funding. CSIRO, Australia's largest public research agency, has changed appreciably over the past three decades as the social, economic, political and technological environment has changed. In this paper, we address five areas of change, namely the nature of the Organisation's research, its research funding allocation, its patterns of collaboration, the way it transfers technology and its role in the national innovation scene. We look at some of the pressures leading to change and at the implications of our analysis for the future. CSIRO is no longer the dominant player in Australian science and innovation as other players, notably in the higher education sector, have grown and, despite its undoubted importance as a reservoir of scientific talent and its major scientific and commercial achievements, much uncertainty about its national role remains. This paper provides a brief overview of some of the practical issues to consider when establishing a longitudinal study. The discussion draws on the experiences of some of the initial investigators of the Australian Longitudinal Study on Women’s Health and sets the scene for the subsequent papers in this series. The investigators reflect on the processes of establishing the research team, conceptualising and planning the research, and gaining and maintaining the funding for the study. Their discussion considers the many disciplinary perspectives that have been integrated into the study, and how these came together. The shifting funding climate in Australian higher education encourages universities to enter into research partnerships with industry. Yet, studies of industries' experiences of their research links with universities are rare. Consequently, research managers often find themselves attempting to promote and facilitate industry engagement with universities while knowing relatively little about the nature of such relationships. This article, which explores the barriers to sustainable research partnerships between universities and industry, identifies the 'cultural gap' between academia and industry as a significant impediment to successful collaborations. Many of the university researchers in this case study had a fairly accurate understanding of industry motivations, needs and expectations, and valuable insights into how the cultural divide between industry and academia might be bridged. It would be prudent to draw upon their expertise when putting strategies, policies and mechanisms in place to increase the attractiveness to industry of engaging with academia. The author regards development of Australia's ill-fated Research Quality Framework (RQF) as a “live experiment” in determining the most appropriate approach to evaluating the extra-academic returns, or “impact,” of a nation's publicly funded research. The RQF was at the forefront of an international movement toward richer qualitative, contextual approaches that aimed to gauge the wider economic, social, environmental, and cultural benefits of research. Human height is a highly heritable trait, with genetic factors explaining up to 90% of phenotypic variation. Vitamin D levels are known to influence several physiological processes, including skeletal growth. The vitamin D receptor (VDR) gene has been reported as contributing to variation in height. A meta-analysis of 13607 adult individuals found a small but significant association with the rs1544410 (BsmI) polymorphism. In contrast, the meta-analysis found no effect in a sample of 550 children. Two recent studies reported variants with large effect on height elsewhere in VDR (rs10735810 [FokI] and rs7139166 [-1,521] polymorphisms). We genotyped large Caucasian samples from Australia (N = 3,906) and the Netherlands (N = 1,689) for polymorphisms in VDR. The Australian samples were twin families with height measures from 3 time points throughout adolescence. The Dutch samples were adult twins. We use the available family data to perform both within and between family tests of association. We found no significant associations for any of the genotyped variants after multiple testing correction. The (non-significant) effect of rs1544410 in the Australian adolescent cohort was in the same direction and of similar magnitude (additive effect 0.3 cm) to the effect observed in the published adult meta-analysis. An effect of this size explains approximately 0.1% of the phenotypic variance in height - this implies that many, probably hundreds, of such variants are responsible for the observed genetic variation. Our results did not support any role for two other regions (rs10735810, rs7139166) of VDR in explaining variation in height. [ABSTRACT FROM AUTHOR]

Recent clinic-based and population-based studies have shown evidence of association between ADHD and autistic symptoms in children and adolescents as well as evidence for genetic overlap between these disorders. The objective of the current study was to confirm the association between autistic and ADHD symptoms in a young adult twin sample assessed by self-report, and investigate whether shared genetic and/or environmental factors can explain the association. We performed twin-based structural equation modeling using self-report data from 11 Social Responsiveness Scale (SRS) items and 12 DSM-IV ADHD inattentive and impulsive symptom items obtained from 674 young adult Australian twins. Phenotypic correlation between autistic and ADHD symptoms was moderate. The most parsimonious univariate models for SRS and ADHD included additive genetic effects and unique environmental effects, without sex differences. ADHD and autistic traits were both moderately heritable. In a bivariate model, genetic correlation (r(g)) between SRS and ADHD was 0.72. Our results suggest that in young adults, a substantial proportion of the genetic influences on self-reported autistic and ADHD symptoms may be shared between the two disorders. The genetic basis of cardiovascular disease (CVD) is complex and still largely elusive. Plasma lipid concentrations are well-established risk factors for cardiovascular disease (CVD), and have adult heritabilities ranging from 0.48 to 0.87. Estimates for adolescents are slightly higher (range 0.71 to 0.82). To identify loci affecting lipid concentrations across adolescence, we analyzed longitudinal lipid data in a sample of 134 monozygotic and 626 dizygotic twin pairs at ages twelve, fourteen and sixteen, and their siblings, from 760 Australian families. Univariate linkage analysis for each phenotype and time point was supplemented by multivariate analysis across the time points. A genome-wide association scan was also performed on a subset of the subjects (N = 441). The strongest linkage was seen for triglycerides on chromosome 6p24.3 (multivariate -log(10) p = 6.81; equivalent LOD = 6.13; p = 1.55 x 10(-7)). Significant linkage was also found for LDL cholesterol on chromosome 2q35 (multivariate -log(10)p = 5.59; equivalent LOD = 4.53; p = 2.57 x 10(-6)). In the association analysis, rs10503840 on 8p21.1 was significantly associated with total cholesterol levels at age fourteen (p = 8.24 x 10(-7), estimated significance threshold 2.45 x 10(-6)). Association at p < 2.25 x 10(-6) was also found between triglycerides at age 12 and rs10507266, in an intron of THRAP2 (MIM 608771) on 12q24.21; and between HDL-C at age 14 and rs10506325 in an intergenic region of 12q13.13. Suggestive evidence of association at ages twelve and fourteen was found between HDL-C and rs10492859 on 16q23 (p = 2.42 x 10(-5) and 2.77 x 10(-4), respectively). Background: There is considerable potential for health research to contribute to improved health services, programs, and outcomes; the policies of health research funding agencies are critical to achieving health gains from research. The need for research to better address health disparities in Indigenous people has been widely recognised. This paper: (i) describes the policy changes made by the National Health and Medical Research Council (NHMRC) from 1997 to 2002 to improve funding of Aboriginal health research (ii) examines catalysts for the policy changes (iii) describes the extent to which policy changes were followed by new models of research and (iv) outlines issues for Indigenous health policy in the future. Methods: This study had two parts: (i) semi-structured interviews were conducted over a four - month period with seven individuals who played a leading role in the policy changes at NHMRC during the period 1997-2002, to describe policy changes and to examine the catalysts for the changes; (ii) a case study was undertaken to evaluate projects by recipients of NHMRC People Support awards and NHMRC Capacity Building Grants in Population Health Research to examine the types of research being undertaken five years after the policy changes were implemented. The proposals of these researchers were assessed in terms of whether they reported intending to: evaluate interventions; engage Indigenous community members and organisations; and build research capacity among Indigenous people. Results: Seven policy changes over a period of five years were identified, including those to: establish an ethical approach to working with Indigenous people; increase the influence of Indigenous people within NHMRC; encourage priority research directed at improving Indigenous health; and recognise Aboriginal and Torres Strait Islander health research as a priority area including a commitment to an expenditure target of 5% of annual funds. Seven catalysts for this change were identified. These included: a perceived lack of effective response to the health needs of Indigenous people; a changed perception of the role of NHMRC in encouraging research to maximise health gains; and leadership within the organisation. The case study analysis demonstrated that 45% of all People Support recipients intend to engage Indigenous community members and organisations in consultation, 26% included an evaluation of an intervention and two (6.5%) were granted to an individual from an Indigenous background. Six of seven Population Health Capacity Building Grants that were awarded to study Indigenous health between 2004 and 2006 included an intervention component; these grants supported 34 researchers from Indigenous backgrounds. Conclusion: NHMRC made significant policy changes from 1997 to 2002 to better support Indigenous health as a result of external pressure and internal commitment. The policy changes have made some progress in supporting better research models particularly in improving engagement with Indigenous communities. It is common to find images displayed around the bedsides of sick children who are hospitalized for extended periods. We report a qualitative study of bedside displays that is based on photographs taken at the bedside, and interviews with eight female patients and seven mothers of young children in an Australian paediatric hospital. We found that a variety of people contributed to the displays in different ways, and that the displays served a wide variety of purposes. We offer a general explanation of hospital bedside displays based on our analysis of the data collected for this study, and we differentiate three related domains for further research into hospital bedside displays. We conclude that bedside displays accomplish much more than decoration alone, and should be understood as aesthetic interventions that serve a wide range of communicative purposes. Two major challenges to conducting a community-based twin study of pathological gambling (PG) disorder are that: (a) it is relatively rare, and (b) individuals with the disorder in the community may be difficult to locate and recruit. We describe a new study of 4,764 individuals recruited from the Australian Twin Registry in which we attempt to effectively deal with the first challenge and examine the impact of the second challenge. The lifetime prevalence of DSM-IV PG in this Australian twin sample was 2.2%, which is 400-500% higher than has been obtained in prevalence surveys conducted in the United States. A number of predictors of non-participation were identified, including a lifetime PG disorder diagnosis, but these did not have a large net effect on the estimated number of individuals with PG or related characteristics in the twin sample. Results of biometric modeling suggested that the effect of genetic, shared family environmental, and nonshared environmental influences on the propensity to engage in 11 different specific forms of gambling (e.g., playing the lottery, betting on horse or dog races, playing electronic gaming machines) were generally moderate, low, and moderate, respectively, with mean parameter estimates obtained of 43%, 10%, and 46%. An intriguing comparison with results from a 1963 US adolescent twin study conducted by Loehlin and Nichols (1976) suggests that: (a) propensity genes for gambling involvement may be more likely to be expressed in the heavy-gambling Australian culture, or that (b) the family environment has a transient effect on the gambling behavior of young people. The paper describes the processes and outcomes of the ranking of LIS journal titles by Australia's LIS researchers during 2007-8, first through the Australian federal government's Research Quality Framework (RQF) process, and then by its replacement, the Excellence in Research for Australia (ERA) initiative. The requirement to rank the journals' titles used arose from discussions held at the RQF panel meeting held in February 2007 in Canberra, Australia. While it was recognised that the Web of Science's (formerly ISI) journal impact approach of journal acceptance for measures of research quality and influence might not work for LIS, it was apparent that this model would be the default if no alternative appeared. Although LIS and discipline-related journals were increasingly appearing in the Web of Science listed rankings, representation remained small and it was decided by the Australian LIS research community to undertake the ranking exercise. This article looks at the growth of the field of engineering education research in Australia and New Zealand. It identifies the manifestations of this growth, including the increasing number of research groups leading debate and discussion on engineering education at universities and colleges. It highlights the promotion of the field in international journals and conferences on engineering such as the annual conference of the Australasian Association for Engineering Education. In addition, the article recognizes the funding provided to engineering education research.

There is increasing pressure for university researchers to secure 'end-user' support for their research projects. Yet the ways in which this imperative affects humanities researchers, operating in a science-centric funding environment, have not yet been fully explored. This paper presents the findings of an empirical study into the experiences of humanities researchers in securing competitive national funding for research involving collaborations with the private and public sector. It also provides quantitative data as to the funding behaviour of one of Australia's peak research funding bodies, the Australian Research Council (ARC). The findings clearly show that humanities researchers struggle to secure support for their research. There is also evidence to suggest that, despite rhetorical support from the ARC that it values research which provides cultural benefit, it too particularly prioritises research that promises economic advantages for both the project's partners and wider community. The article offers updates from the Australian Wound Management Association (AWMA) in Australia. The AWMA will hold a fortnight wound awareness on March 5-28, 2010 including its annual conference on March 24-27, 2010. The AWMA will partner in the consortium to procure funding from the Collaborative Research Centre (CRC) for research on diagnostics and new therapies. The AWMA is developing guidelines for the therapy of venous leg ulcers.

During an Australian qualitative and empirical study looking at lesbian, gay, bisexual, and transgender client's experiences of counseling, and counselor's experiences of working with minority clients, a large body of unsolicited data emerged related to experiences of religious-based homophobia. Analysis of the data suggests that a lifelong process of posttraumatic recovery for many lesbian, gay, bisexual, and transgender people follows prior experiences of religious-based homophobia. This paper discusses the sociological debate related to how counselors find themselves at the crossroad between a healthy lifestyle model of homosexuality based in well established contemporary professional ethics versus long standing religious-based attitudes and constraints toward homosexuality. This intersection of conflicting beliefs generates a controversial social and political environment in which counselors must make a basic decision to either support minority clients according to ethical guidelines or to side with socially conservative constructs that, rightly or wrongly, rely largely on Western religious traditions. This mixed methods study investigated the impact on Australian families of the decision-making process and the ongoing (re)habilitation demands of their children's cochlear implantation. Quantitative results from a survey of 247 parents found that parents experienced stress related to the decision-making and rehabilitation processes at significantly higher levels than they had expected pre-implant. Multiple regression analysis identified factors that were predictive of parents' experiences of rehabilitation efforts and stress. Qualitative data from interviews with 27 of the survey respondents elaborated on the quantitative findings and provided insights into the impact on families' lives, identifying the greatest problem areas and potential stressors for parents. The use of standardised patients (SPs) and computer based simulations (CBSs) has been suggested as a method of providing students in the health sciences with basic clinical skills without relying on extensive support from external clinics. This study used a questionnaire to determine if 25 first-year audiology students from an audiology program in a large Australian university felt their interactions with SPs and CBSs had improved their ability to interact with clients and perform a basic audiometric assessment. These students reported their interactions with the SPs significantly ( p < .01) improved their performance in 10 out of 10 areas of client interaction and their interactions with the CBS significantly ( p < .01) improved their abilities in 6 out of 8 areas of basic audiometry. They also reported that the SPs' portrayals of the cases and the content of the simulated cases were realistic, although further preparation for interacting with the SPs was desirable. Overall, these results support the continued investigation of SPs and CBSs as potential methods of training and assessing Australian audiology students in the areas of client interaction and basic audiometry. The author reflects on the need to improve research in primary care and general medical practice in Australia. He narrates how general practice research has been given a low priority in terms of funding until a proposal to form a register of general practitioners was championed by the Royal Australian College of General Practitioners (RACGP) in 1989. Yet, there is still a need to improve the quality of research by focusing on the uncertainty surrounding clinical conditions in primary care services and collaboration with basic science researchers.

Objective: To evaluate an online fatigue self-management programme in a sample of adults with chronic neurological conditions. Design: Randomized controlled trial. Setting: Online fatigue self-management programme delivered across Australia. Participants: Ninety-five people with fatigue secondary to multiple sclerosis, Parkinson's disease or post-polio syndrome. Interventions: An online fatigue self-management programme, an information-only fatigue self-management programme and a control group. Main measures: Groups were compared at pre test, post test and at three months on primary outcomes using the Fatigue Impact Scale, Activity Card Sort and Personal Wellbeing Index. Results: With the exception of the Personal Wellbeing Index at post test (F=3.519; P=0.034) and the Physical Subscale of the Fatigue Impact Scale at follow-up (F=3.473; P=0.035) there were no significant differences between the three groups on primary outcomes. Post-hoc testing showed the differences to be between the information-only and control groups (P=0.036 and P=0.030 respectively). Improvement in the information-only group was unexpected but appears to be similar to results of other online interventions. The fatigue self-management and information-only groups performed better than the control on some secondary outcome measures. Low power in the analysis may have contributed to the findings. Repeated-measures ANCOVA showed that the fatigue self-management and the information-only groups improved over time on the Fatigue Impact Scale and the Activity Card Sort (P<0.05). The control group showed no improvements over time. Conclusions: Although the fatigue self-management group improved over time, results did not demonstrate additional benefit in most outcome measures when compared with the control group. The aim of this study was to understand the environment of health care, and how clinicians and managers respond in terms of performance accountability. A qualitative method was used in a tertiary metropolitan teaching intensive care unit (ICU) in Sydney, Australia, including interviews with 15 clinical managers and focus groups with 29 nurses of differing experience. The study found that a managerial focus on abstract goals, such as budgets detracted from managing the core business of clinical work. Fractures were evident within clinical units, between clinical units and between clinical and managerial domains. These fractures reinforced the status quo where seemingly unconnected patient care activities were undertaken by loosely connected individual clinicians with personalized concepts of accountability. Managers must conceptualize health services as an interconnected entity within which self-directed teams negotiate and agree objectives, collect and review performance data and define collective practice. The purpose was to develop a foot image capture and measurement system with web cameras (the 3-FIS) to provide reliable and valid foot anthropometric measures with efficiency comparable to that of the conventional method of using a handheld anthropometer. Eleven foot measures were obtained from 10 subjects using both methods. Reliability of each method was determined over 3 consecutive days using the intraclass correlation coefficient and root mean square error (RMSE). Reliability was excellent for both the 3-FIS and the handheld anthropometer for the same 10 variables, and good for the fifth metatarsophalangeal joint height. The RMSE values over 3 days ranged from 0.9 to 2.2 mm for the handheld anthropometer, and from 0.8 to 3.6 mm for the 3-FIS. The RMSE values between the 3-FIS and the handheld anthropometer were between 2.3 and 7.4 mm. The 3-FIS required less time to collect and obtain the final variables than the handheld anthropometer. The 3-FIS provided accurate and reproducible results for each of the foot variables and in less time than the conventional approach of a handheld anthropometer. [ABSTRACT FROM AUTHOR]

Abstract This study employs multiple regression models based on DeFries and Fulker (1985), and a large sample of twins, to assess heritability in attitudes towards economic risk, and the extent to which this heritability differs between males and females. Consistent with Cesarini et al. (2009), it is found that attitudes towards risk are moderately heritable, with about 20 percent of the variation in these attitudes across individuals being linked to genetic differences. This value is less than one-half the estimates reported by Zyphur et al. (2009) and Zhong et al. (2009). While females are more risk averse than males, there is no evidence that heritability in attitudes towards risk differs between males and females. Even though heritability is shown to be important to economic risk-taking, the analyses suggest that multivariate studies of the determinants of attitudes towards risk which to not take heritability into consideration still provide reliable estimates of the partial effects of other key variables, such as gender and educational attainment. Self-rated health questions have been proven to be a highly reliable and valid measure of overall health as measured by other indicators in many population groups. It also has been shown to be a very good predictor of mortality, chronic or severe diseases, and the need for services, and is positively correlated with clinical assessments. Genetic factors have been estimated to account for 25-64% of the variance in the liability of self-rated health. The aim of the present study was to identify Single Nucleotide Polymorphisms (SNPs) underlying the heritability of self-rated health by conducting a genome-wide association analysis in a large sample of 6,706 Australian individuals aged 18-92. No genome wide significant SNPs associated with self-rated health could be identified, indicating that self-rated health may be influenced by a large number of SNPs with very small effect size. Aim: The aim of this qualitative study was to explore baby boomers' views and plans in the early days of the global financial crisis (GFC) in 2008. Methods: Informants from National Seniors Australia were interviewed in 15 focus groups conducted nationally. Transcripts were analysed by themes from semi-structured questions. Results: The GFC was found to shake the confidence and plans of boomers. Many workers decided to delay retirement and save longer following losses in superannuation. Those retired on market-linked superannuation felt forced to reduce expenses and restrain lifestyles. Those on full pensions were relatively unaffected. Conclusion: The GFC called into question boomers' expectations for retirement. While financial markets are showing signs of recovery, the GFC had precipitated a decision to work longer and to draw conservatively on retirement savings that may take many years to recover. Background: With human papilloma virus (HPV) vaccines now approved for women up to 45 years of age, gynaecologists in Australia will be approached increasingly for advice on whether to have the HPV vaccine. Aim: We seek to establish the attitudes of gynaecologists to HPV vaccination when advising women in various age groups. Methods: Fellows, members and trainees of Royal Australian and New Zealand College of Obstetricians and Gynaecologists were approached in early 2009 to complete an Australian Society for Colposcopy and Cervical Pathology survey, which they could return by post or internet on-line. There were 836 (49.0%) responses, with 35.6% of responders using 'Tuna' on-line. Results: 836 RANZCOG fellows, members and trainees responded to the survey. The gender of responders are 261 (43%) females and 470 (56%) males. Of the responders, 391 (46.8%) usually initiated discussion regarding the role of HPV vaccination with their patients. More than 93% encourage when recommending the HPV vaccines for female responders aged 12-26 years. This proportion decreased to 66% for women aged 27-45 years, with 15% strongly encouraging vaccination for this age group. The attitudes to HPV vaccination for the different age groups of females were similar for trainees, fellows and members, as well as for gender and age groups of the responders. However, those with lower level of knowledge are less likely to encourage HPV vaccination in all vaccination age groups and the association is statistically significant after adjusting for role, gender and age. Objective: Identify the most important factors associated with choosing rural medical practice. Design: Cross-sectional design using a web survey to collect quantitative and qualitative data. Participants: One hundred and ninety junior doctors - 91 interns, 99 PGY2. Main outcome measures: Choice of practice location (urban/rural), reason for choosing location, enticement to a rural location. Results: Twenty-seven per cent of junior doctors preferred a rural practice location. Preference to practice in a rural area was associated with medical placement bonding schemes, rural background, rural placement experience and being older. High levels of professional expectations and prestige were associated with a preference for an urban location. The most important reasons for choosing a practice location included consideration of partner, family and friends (35.3%), preference for a location (20.5%), lifestyle goals (19.5%) and career opportunities, specialty requirements and infrastructure (17.9%). Those who preferred an urban compared with a rural location gave more importance to factors concerning partner, family and friends. The factors that would entice a doctor to a rural location included partner and family considerations (27.0%), professional support (20.3%), and career opportunities, specialty requirements and infrastructure (16.3%). Women gave more importance to partner and family factors than men. Conclusions: Our findings support the continuation of policies that are known to encourage choice of rural practice, but highlight the need for additional strategies that consider the personal and professional needs of this generation of doctors. Interpretation of changes in health and health care utilization patterns across the life span depends on an understanding of the effects of age, period, and cohort. The purpose of this article is to illustrate differences among three generations of women in demographic factors, health risk factors, and health status indicators from 1996 to 2008. The article examines data from the Australian Longitudinal Study on Women’s Health, a broad-ranging project funded by the Australian Government Department of Health and Ageing (DoHA) and involving three age groups of women (born in the periods 1973—1978, 1946—1951, and 1921—1926) who were first surveyed in 1996 and will be surveyed every 3 years until at least 2015. Patterns in selected demographic factors (marital status and level of educational qualification), health risk factors (smoking, alcohol consumption, physical activity, and body mass index [BMI]), and health status indicators (asthma, hypertension, diabetes and depression; physical functioning and mental health scores from the SF-36) were examined to illustrate examples of biological age, generational differences, or period effects that affect all age groups and generations simultaneously. The results can be used to inform the development of responsive and effective models for both prevention and management of chronic disease, including health and aged-care systems that will meet the needs of different generations of women across their life span. Objective To model the population level impact of tai-chi on future rates of falls and fall-related injury in older people as a tool for policy development. Design An epidemiological and economic model for estimating population-level effectiveness of tai-chi. Setting Australia, 2009. Patients or subjects Australian community-dwelling population aged 70+ years, ambulatory and without debilitating conditions or profound visual defects. Intervention Group-based tai-chi, for 1 h twice weekly for 26 weeks, assuming no sustained effect beyond the intervention period. Main outcome measure Total falls and fall-related hospitalisation prevented in 2009. Results Population-wide tai-chi delivery would prevent an estimated 5440 falls and 109 fall-related hospitalisations, resulting in a 0.18% reduction in the fall-related hospital admission rate for community-dwelling older people. The gross costs per fall and per fall-related hospital admission prevented were $A4414 (€3013) and $A220 712 (€150 684), respectively. A total investment of $A24.01 million (€16.39 million), equivalent to 4.2% of the cost of fall-related episodes of hospital care in 2003/4, would be required to provide tai-chi for 31 998 people and achieve this effect. Conclusions Substantial investment in, and high population uptake of, tai-chi would be required to have a large effect on falls and fall-related hospitalisation rates. Although not accounted for in this study, investment in tai-chi is likely to be associated with additional significant health benefits beyond falls prevention. International Journal of Nursing Practice 2010; 16: 517-524 The first Australian nurse practitioner census: A protocol to guide standardized collection of information about an emergent professional group Internationally, collection of reliable data on new and evolving health-care roles is crucial. We describe a protocol for design and administration of a national census of an emergent health-care role, namely nurse practitioners in Australia using databases held by regulatory authorities. A questionnaire was developed to obtain data on the role and scope of practice of Australian nurse practitioners. Our tool comprised five sections and included a total of 56 questions, using 28existing items from the National Nursing and Midwifery Labour Force Census and nine items recommended in the Nurse Practitioner Workforce Planning Minimum Data Set. Australian Nurse Registering Authorities ( n = 6) distributed the survey on our behalf. This paper outlines our instrument and methods. The survey was administered to 238 authorized Australian nurse practitioners (85% response rate). Rigorous collection of standardized items will ensure health policy is informed by reliable and valid data. This paper is a report of a study of variations in the pattern of nurse practitioner work in a range of service fields and geographical locations, across direct patient care, indirect patient care and service-related activities. Background. The nurse practitioner role has been implemented internationally as a service reform model to improve the access and timeliness of health care. There is a substantial body of research into the nurse practitioner role and service outcomes, but scant information on the pattern of nurse practitioner work and how this is influenced by different service models. Methods. We used work sampling methods. Data were collected between July 2008 and January 2009. Observations were recorded from a random sample of 30 nurse practitioners at 10-minute intervals in 2-hour blocks randomly generated to cover 2 weeks of work time from a sampling frame of 6 weeks. Results. A total of 12,189 individual observations were conducted with nurse practitioners across Australia. Thirty individual activities were identified as describing nurse practitioner work, and these were distributed across three categories. Direct care accounted for 36·1% of how nurse practitioners spend their time, indirect care accounted for 32·0% and service-related activities made up 31·9%. Conclusion. These findings provide useful baseline data for evaluation of nurse practitioner positions and the service effect of these positions. However, the study also raises questions about the best use of nurse practitioner time and the influences of barriers to and facilitators of this model of service innovation. [ABSTRACT FROM AUTHOR]

Behavioural and social science theories and models (BSSTM) can enhance efforts to increase health and safety behaviours, such as the uptake and maintenance of injury prevention measures. However, the extent to which they have been used in sports injury research to date is currently unknown. A systematic review of 24 electronic databases was undertaken to identify the extent to which BSSTM have been incorporated into published sports injury prevention research studies and to identify which theories were adopted and how they were used. After assessment against specific inclusion and exclusion criteria, the full text of 100 potentially relevant papers was reviewed in detail. These papers were classified as follows: (i) explicit — the use of BSSTM was a stated key aspect in the design or conduct of the study; or (ii) atheoretical — there was no clear evidence for the use of BSSTM. The studies that explicitly mentioned BSSTM were assessed for how BSSTM were specifically used. Amongst the 100 identified papers, only eleven (11% of the total) explicitly mentioned BSSTM. Of these, BSSTM were most commonly used to guide programme design/implementation (n = 8) and/or to measure a theory/ construct (n = 7). In conclusion, very few studies relating to sport safety behaviours have explicitly used any BSSTM. It is likely that future sports injury prevention efforts will only be enhanced, and achieve successful outcomes, if increased attention is given to fully understanding the behavioural determinants of safety actions. Objectives To investigate how the tobacco industry is adapting to regulatory action in accordance with provisions of the Framework Convention on Tobacco Control that targets misleading packaging and labelling. To relate the packaging and labelling of new cigarette varieties to their construction and performance. Methods The principal design features and tar, nicotine and carbon monoxide yields of the Marlboro 'brand family' in Australia were measured and compared with those of the US equivalents. Results Marlboro Red and Blue/Medium, could not be differentiated in preliminary tests in Australia, but were different in the USA. However, yield testing showed Marlboro Blue/Medium did not have lower tar and nicotine yields in either country, indeed being higher in Australia. Conclusions Colour can be used to market cigarettes as 'milder', independently of ISO yields and 'Light'/'Mild' descriptors. Banning of 'Light' and 'Mild' brand descriptors may be inadequate to end belief in less harmful cigarettes so long as the tobacco industry remains free to engineer 'mildness' and to use colours, other descriptors and design features to characterise varieties it wants to market as 'milder'. Abstract: For young people with refugee backgrounds, establishing a sense of belonging to their family and community, and to their country of resettlement is essential for wellbeing. This paper describes the psychosocial factors associated with subjective health and wellbeing outcomes among a cohort of 97 refugee youth (aged 11–19) during their first three years in Melbourne, Australia. The findings reported here are drawn from the Good Starts Study, a longitudinal investigation of settlement and wellbeing among refugee youth conducted between 2004 and 2008. The overall aim of Good Starts was to identify the psychosocial factors that assist youth with refugee backgrounds in making a good start in their new country. A particular focus was on key transitions: from pre-arrival to Australia, from the language school to mainstream school, and from mainstream school to higher education or to the workforce. Good Starts used a mix of both method and theory from anthropology and social epidemiology. Using standardized measures of wellbeing and generalised estimating equations to model the predictors of wellbeing over time, this paper reports that key factors strongly associated with wellbeing outcomes are those that can be described as indicators of belonging – the most important being subjective social status in the broader Australian community, perceived discrimination and bullying. We argue that settlement specific policies and programs can ultimately be effective if embedded within a broader socially inclusive society – one that offers real opportunities for youth with refugee backgrounds to flourish. Background: the burden of falls and fall-related injuries among older adults is well established. Contention surrounds the effectiveness, and hence value, of multi-component fall prevention interventions delivered in the community.Objective: using consensus-based analytic guidelines rather than time-to-first fall as the primary endpoint, the objective was to examine the effectiveness of the Whitehorse NoFalls trial on all falls, falls resulting in injury and falls requiring medical care to be sought.Design, setting and participants: the study was a community-based randomised controlled trial, with 1,090 participants assigned to one of eight groups, these being a combination of one or more of exercise, vision and or home hazard reduction or alternatively assignment to the control group.Methods: using negative binomial regression, the incidence of all falls, falls resulting in injury and those requiring medical care in the intervention groups were examined. Falls were reported using a monthly return calendar.Results: exercise alone and in combination with vision and/or home hazard reduction was associated with fewer falls. For falls resulting in injury and the subset requiring medical care, the vision plus exercise intervention was associated with fewer falls.Conclusions: the findings confirm the effectiveness of exercise in preventing falls among community-dwelling older adults and supports contention that multi-component interventions do not prevent more falls than a single intervention. Our aim was to develop a tool to identify specific features of the business and financial management of practices that facilitate better quality care for chronic illness in primary care. Domains of management were identified, resulting in the development of a structured interview tool that was administered in 97 primary care practices in Australia. Interview items were screened and subjected to factor analysis, subscales identified and the overall model fit determined. The instrument's validity was assessed against another measure of quality of care. Analysis provided a four-factor solution containing 21 items, which explained 42.5% of the variance in the total scores. The factors related to administrative processes, human resources, marketing analysis and business development. All scores increased significantly with practice size. The business development subscale and total score were higher for rural practices. There was a significant correlation between the business development subscale and quality of care. The indicators of business and financial management in the final tool appear to be useful predictors of the quality of care. The instrument may help inform policy regarding the structure of general practice and implementation of a systems approach to chronic illness care. Psychosocial risks in the workplace have the potential for causing psychological and social harm that contributes toward the mental health disability burden. Psychosocial risks are influenced by macrolevel factors such as the psychosocial safety climate within the organization. This paper concerns the development and evaluation of a short instrument to measure psychosocial safety climate (PSC). PSC is conceived as an up-stream resource, and concerns senior management values and attitudes toward care and practices in relation to employee psycho-social well being. In a pilot sample (N = 78) we used an iterative procedure incorporating regression analysis to reduce 26 items down to a parsimonious 12 item, four-factor scale (PSC-12). The PSC-12 was then assessed using confirmatory factor analysis and the scale validated in a second representative sample of Australian workers (N = 398). The PSC-12 showed expected relationships with psychosocial risk factors (e.g., job demands, job resources), worker engagement and health, and work related outcomes (e.g., job satisfaction). We further confirmed the invariance of the factor coefficients and factor covariance across the two multioccupational samples using multigroup analysis. In a third organizational study (N = 16 teams, 106 health care workers) we found that PSC showed group like psychometric properties, and team level PSC was associated with individual level psychological distress and work engagement. PSC showed incremental value beyond a physical safety measure. The results provide initial indications that the PSC-12 can be used across a range of occupations, and within organizations. Whole body kinematics of the tennis serve have been reported extensively in the literature, yet comparatively less information exists regarding the kinematic characteristics of the swing and toss. In attempting to develop consistency in placement of the toss and racket trajectory, coaches will often decompose the serve and practice it in separate parts. A 22-camera VICON MX motion analysis system, operating at 250 Hz, captured racket, ball and hand kinematics of the serves of five elite junior players under three conditions. The conditions were flat first serves (FS) directed to a 1 × 1 m target bordering the 'T' of the deuce service box, a ball toss (BT) in isolation and a free swing (SW) in isolation. Players were instructed to perform BT and SW as in the FS. Paired t-tests assessed within-group differences in hand, racket and ball kinematics between the discrete skill and the two decomposed conditions. Vertical displacement of the ball at its zenith increased significantly during BT compared with the FS and temporal associations between racket and ball motion during the FS (r = 0.861) were affected during task decomposition. Transition Care (TC) is a new program for older adults in Australia. At present, program quality is assessed using provider reports of compliance with key requirements established by the Australian Government Department of Health and Ageing. As part of the National Evaluation of the Transition Care Program, the authors developed a questionnaire to measure recipient experience of TC. Validity and reliability were assessed via interviews with 582 recipients or proxies 3 months after discharge from TC. Concordance between test-retest observations was high. Principal component analysis suggested three subscales were important: restoration, continuity of care and patient involvement. Recipients of TC in a residential care setting had lower mean scores on the restoration subscale compared to those who received services in the community. This study found that a standardised measure of recipient experience could inform quality improvement in TC and is feasible to administer via questionnaire. [ABSTRACT FROM AUTHOR]

Since the mid-1980s, Australian governments have focused on expanding community and home-based services for older people. This has led to increased levels of dependency, vulnerability, and complexity to be managed in the community. Consequently, aged care services have had to develop mechanisms for regulating and managing these increased risks, and risk management has become more central to the practices of professional workers in this field. This paper reports on some findings from a large-scale study that explores the way risk management policies have been translated into practice by community-based services in Victoria, Australia. Drawing on interviews with 18 frontline and management professionals employed in community aged care, we found that these workers were wrestling with a diverse and new range of institutional risks beyond those encountered in the actual delivery of frontline care. We found that these workers experienced 'risk' in four different contexts, which often created demands for contradictory or conflicting responses. Here we examine these 'contexts of risk practice': professional workers' relationships with their clients, relationships with other service providers, the unregulated nature of the home as a work environment, and community expectations about the management of risk. Despite tensions that frequently arose, workers expressed strong professional commitments to their clients and were motivated to find positive resolutions amid competing interests. We conclude that tensions experienced by workers were embedded in the structural dimensions of institutional relationships and the systematic absence of shared understandings of 'acceptable' risks in the community care of older people, rather than in the failure of professional agency. The provision of Indigenous cultural training for non-Indigenous health workers has been widely promoted as a method of improving health service provision to 'close the gap' in Indigenous health. However, in the absence of strong evidence, the power of Indigenous cultural training to meaningfully contribute to the health of Aboriginal and Torres Strait Islander peoples remains questionable. This research explored how six hospital-based nurses consider the role of Indigenous cultural training and the impact it has had on their practice through individual semi-structured interviews. Thematic analysis revealed the significance of individual professionals' attitudes in determining the impact of Indigenous cultural training, as well as the need for institutional support to assist in translating Indigenous cultural training into practice. Utilising post-colonial theory, two key findings emerge. First, the way in which Indigenous cultural training conceptualises 'identity' and 'culture' is critical to its ultimate outcomes. Second, deficits in institutional support limit the efficacy of Indigenous cultural training by placing the onus for institutional change on the shoulders of individual health workers.
