Background/Aim. Duchenne muscular dystrophy (MD) and Becker MD are caused by mutations in the gene for dystrophin (DMD). They are X chromosome-linked recessive diseases where males are affected, and females are healthy carriers of the mutation in most cases. It is estimated that 2/3 of mothers of Duchenne MD probands are carriers, while 1/3 of probands have de novo mutations. The aim of the study was to confirm the carrier status of female members of the families of Duchenne MD/Becker MD probands using direct genetic testing methods. Methods. The study included 38 females from 31 families of Duchenne MD/Becker MD probands with deletion/duplication in the DMD gene. Moreover, 4 cases of prenatal diagnosis of Duchenne MD/Becker MD were included. The methods of polymerase chain reaction - PCR and the multiplex ligation-dependent probe amplification - MLPA were applied for detecting deletions, i.e., deletion/duplication mutations in the DMD gene. Results. In the total of 31 Duchenne MD/Becker MD probands, 87.1% of deletions and 12.9% of duplications of one or more exons in the DMD gene were detected. Of the 29 tested mothers, mutations were found in 17 of them (14 deletions and 3 duplications). Mutations were detected in 11 (57.9%) out of 19 mothers of probands with the Duchenne MD phenotype and 6 (60%) out of 10 mothers of Becker MD probands. Furthermore, 14 (56%) out of 25 mothers were carriers in probands with deletions, and 3 (75%) out of 4 mothers were carriers in probands with duplications. In the remaining 9 other female relatives of the patients, mutations were found in 4. In prenatal diagnosis, we identified a deletion in one male and one female fetus of one single mother who was confirmed as a carrier. Conclusion. The study showed that mothers were carriers in almost 60% of sporadic cases of Duchenne MD/Becker MD with deletions and duplications. In addition, the carrier frequency tended to be higher in mothers of the probands with duplications (75%) compared to mothers of probands with deletions (56%).

Background/Aim. Despite the availability of safe and effective vaccine since 1982, hepatitis B virus (HBV) infection still remains a major occupational disease among health care workers (HCWs) worldwide. The aim of this study was to estimate the level of knowledge regarding HBV among HCWs in Serbia. Methods. A random sample of 352 HCWs, stratified by occupation, was selected from the list of employees in the Clinical Centre of Serbia during December 2015. Anonymous questionnaire was used in data collection. Data were statistically assessed. Results. The mean HBV knowledge score was 22.9 ± 4.8 (out of maximum 30). Better knowledge score correlated with higher education attainment (ρ = 0.377; p < 0.001), younger age (ρ = -0.113; p = 0.034) and less working experience (ρ=-0.127; p = 0.017). We observed that those HCWs who were previously vaccinated against hepatitis B demonstrated statistically significantly higher knowledge level (t = 5.656, p < 0.001) compared to HCWs who were not vaccinated (23.9 ± 3.7 vs. 20.9 ± 5.8, respectively). Conclusion. We found poor level of knowledge on some aspects of HBV infection among HCWs in Serbia. Tailoring of education campaigns in this high-risk exposure group is necessary. Education intervention should focus on change of attitudes and behaviour modification to ensure safe and responsible health care environment. Background/Aim. Duchenne muscular dystrophy (DMD) and its allelic form Becker muscular dystrophy (BMD) are X-linked diseases that affect males, characterized by progressive muscle and cardiopulmonary weakness, especially in DMD as a severe form of the disease. They result from mutations in the dystrophin gene, and the most common changes are large intragenic deletions and duplications (80%). One third of patients have de novo mutation and 2/3 of the mothers are estimated as carriers. The aim of the study was to analyze the frequency of duplications versus deletions in the dystrophin gene in patients with dystrophinopathies, as well as to analyze the phenotypic effect of large mutations obtained and to determine the carrier status of female relatives in probands with duplications. Methods. We examined 22 DMD and 35 BMD unrelated patients and 6 female relatives of the probands where duplications were found. We used polymerase chain reaction (PCR) and multiplex ligation-dependent probe amplification (MLPA) methods, according to the protocol, to detect or confirm mutations in probands and female carriers. Results. In probands, there were 34 (59.6%) large deletions (mostly affected exons 44–60) and 6 (10.5%) large duplications in 4 DMD and 2 BMD patients. Also, duplications were found in 3 out of 4 (75%) tested mothers. The distribution of duplications was heterogeneous, affecting N-terminal and central rod domain, and included more exons, except for one DMD patient who had duplication of exon 2. An exception from the Monaco rule was present in 9.5% of DMD and 15.8% of BMD probands, i.e. in 12.5% of DMD/BMD cases. Conclusion. In 57 DMD/BMD probands, we found 59.6% of large deletions and 10.5% of large duplications. The most affected region of the DMD gene was the central rod domain. An exception to Monaco''s rule was present in 12.5% of DMD/BMD cases. Three out of 4 examined proband''s mothers were confirmed as carriers. Background/Aim. The assessment of association of depression and diabetes mellitus type 2 using the Patient Health Questionaire (PHQ-9) has not been done in Montenegro. The aim of this study was to assess the prevalence of depression in the patients with type 2 diabetes mellitus, and to identify the risk factors associated with the presence of depression. Methods. A cross-sectional study was conducted at the General Hospital in Bijelo Polje, from July to September, 2015. It included 70 patients over 35 years of age with the diagnosis of diabetes for at least six months. For the assessment of depression presence and intensity PHQ–9 was used. All variables associated with the presence of depression at a significance level of p < 0.05 were included into the final method of the multivariate logistic regression analysis. Results. Comorbidities were statistically significant more frequent among patients with depression (χ2 = 5.40; p = 0.020). Duration of diabetes over five years was significantly associated with depression (χ2 = 12.48; p < 0.001). Depression occurred more frequently among physically inactive subjects (χ2 = 10.74; p = 0.005). The presence of diabetic polyneuropathy (χ2 = 6.04; p = 0.014) and cataract (χ2 = 5.351; p = 0.021) were also significantly associated with depression. A multivariate logistic regression analysis showed that the duration of diabetes over five years and presence of cataract were independently associated with depression. Conclusion. The risk factors for depression among the subjects with diabetes were disease duration more than five years and the presence of cataract. Since depression is a serious disease and can be a risk factor for many chronic diseases, the best way of prevention is its early detection and treatment.

Background/Aim. Acne is well recognized condition that adversely affects quality of life (QoL) of the patients. The aim of this study was to determine the self-reported prevalence of acne and its main characteristics in high school pupils, and to assess their QoL. Methods. The cross-sectional study conducted in May 2011 comprised 440 pupils from Serbia (281 from Belgrade and 159 from Užice). Participation in the study was voluntary and anonymous. Two questionnaires were administered: a short demographic questionnaire, and Children's Dermatology Life Quality Index (CDLQI). Internal consistency was assessed using the Cronbach’s alpha while item-total score correlations were assessed using Spearman's correlation analysis. Results. The majority of the study population (84.3%) were girls, and 15.7% were boys. The total mean age of the pupils was 16.48 ± 0.55 years. A total of 228 (51.8%) pupils self-reported their acne with significantly higher prevalence in Užice (73.6%) than in Belgrade (39.5%). The mean CDLQI score was 3.55 with the similar quality of life impairment in adolescents from the two cities. The mean Cronbach’s alpha was 0.83. There was a statistically significant positive correlation between the mean overall CDLQI score and CDLQI subscale scores that ranged from 0.401 to 0.841. Conclusion. Our study confirmed that acne is associated with impairment in QoL that is in accordance with previous studies performed on teenagers. The CDLQI is a reliable and valid measure, and can be used as a practical tool for measuring the impact of acne on patients’ QoL. Background/Aim. Acne is a common problem in adolescent children with considerable emotional and psychological effects. The aim of this study was to determine the self-reported prevalence of acne and to assess its impact on the quality of life in high school pupils in Serbia. Methods. The cross-sectional study was conducted in May 2011 in two medical high schools in Serbia. Only pupils who gave a written informed consent to participate in the study (n = 440) were asked to fill in two questionnaires: short demographic questionnaire and Cardiff Acne Disability Index (CADI), a disease-specific questionnaire measuring disability induced by acne. Internal consistency (tested by Cronbach’s alpha) and item-total score correlations (Spearman's correlation analysis) were used for reliability analyses. Results. The study population consisted of 440 pupils, 281 from Belgrade and 159 from Užice. Among them 371 (84.3%) were girls and 69 (15.7%) boys, with similar sex distribution in Belgrade and Užice. The total mean age of pupils was 16.48 years (SD = 0.55). Out of 440 pupils 228 (51.8%) self-reported their acne. The acne prevalence was significantly higher in pupils from Užice (73.6%) than in those from Belgrade (39.6%). The overall mean CADI score for the whole sample was 2.87 ± 2.74, with the similar quality of life impairment in adolescents from Belgrade and from Užice. The mean Cronbach’s alpha was 0.82. Conclusion. This study shows that the quality of life impairment due to acne is mild for the majority of the affected pupils. The Serbian version of the CADI is a reliable, valid, and valuable tool for assessing the impact of acne on the quality of life.

Background/Aim. Basal cell carcinoma (BCC) is the most common form of skin cancer in the white population. Increasing incidence of BCC imposes the requirement to identify the risk factors due to eventual preventive action. The aim of this study was to assess the role of some constitutional characteristics in development of BCC among the Montenegrin population. Methods. A case-control study was conducted at the Dermatology Department of Clinical Center of Montenegro in Podgorica from 2002−2003. The study group included 100 histopatologically confirmed BCC cases, while the control group consisted of 100 patients from the same department, who did not present skin cancer and who were individually matched to the cases by sex and age (±5 years). All participants were interviewed using an epidemiological questionnaire. For statistical analysis t test and McNemar χ2 test for matched pairs and multivariate logistic regression analyses were used. Results. The risk for development of BCC was increased among the persons with lighter hair color (t = 4.63; df = 99; p < 0.001), fair skin (t = 2.37; df = 99; p = 0.020), lighter eyes color (t = 2.86; df = 99; p = 0.005), with nevuses (OR = 13.13; p = 0.025; 95% IP = 1.39-12.03), and among those whose skin tone after sun exposure remained light (OR = 3.14; p = 0.001; 95%IP = 1.59-6.18). Conclusion. Our study confirmed the significance of constitutional characteristics such as lighter hair color, fair skin, lighter eyes color, and the presence of nevuses in the development of BCC. .

Backgound/aim: Basal cell carcinoma (BCC) is the most frequent form of carcinomas in the whites. Among the environmental factors, the most important risk factor for its occurrence is the exposure to sun radiation. The aim of this study was to assess the role of the sun radiation in the development of basal cell carcinoma BCC in the Montenegrian population. Methods. A case-control study was conducted in a period from 2002-2003. The study group included 100 histopatologically confirmed cases with BCC, while the control group included 100 patients from the same population, who did not present skin cancer and who were individually matched with the cases from the study group by sex and age (± 5 years). All the participants were interviewed using an epidemiological questionnaire. For statistical analysis, the χ2 test and univariate logistic regression analysis were used. Results. The risk for development of BCC was increased in the persons: that always had burns with no tan during the exposure to sunlight (OR = 1.75; 95% CI = 1.20-2.55; p = 0.003); that developed sunburns after two hours of the exposure to sunlight (OR = 3.72; 95% CI = 2.39-5.79; p < 0.001) that kept light tan or remained without changes in childhood and adolescence after the repeated exposures to sunlight (OR = 2.92; 95% CI = 1.89-4.52; p < 0.001) that often had severe and painful sunburns (OR = 4.48; 95% CI = 2.74-7.33; p < 0.001). Conclusion. Our study confirmed the significance of sunlight exposure for the development of BCC.

Background. Patients admitted to intensive-care units (ICU) are at a high risk of nosocomial infections (NI) due to susceptibility associated with severity of their condition, but also the invasive medical procedures they undergo. Aim. To determine the frequency of NI at the ICU of the General Hospital Užice, and to identify the risk factors for their development. Methods. A prospective surveillance study of NI, conducted between June 27 and December 31 2001, included 914 patients who spent at least 24 hours in the ICU (total of 2 615 days). The surveillance of NI in the ICU was carried out daily. Follow-up period covered the time from the ICU admission to 48 hours after the ICU discharge. To assess risk factors for NI, we performed a case-control study. The variables measuring of extrinsic and intrinsic risk factors for NI were collected. Results. In a six-month prospective surveillance study, the incidence of NI was 16.7% or 58.5 per 1,000 patient-day, respectively. The most frequent were the infections of the surgery wounds (32.6%), urinary tract infections (23.5%), and infections of the blood (7.1%). The identified independent risk factors for NI were: surgical intervention (OR = 5.74; CI = 2.01-16.41), endotracheal tubes (OR = 3.40; CI = 1.07-10.89), cystoscopy (OR = 2.35; CI = 1.38- 4.02), obesity (OR = 1.98; CI = 1.27-3.11), and the duration of the infusion (OR = 1.34; CI = 1.23-1.46). Conclusions. The most important risk factors for NI at ICU were surgical interventions and endotracheal tubes.
