Background: Stability of a dental implant is very important when planning immediate loading and design of a final restoration. The aim of this study was to compare the primary and the secondary stability of dental implants inserted by three different surgical techniques: conventional (standard) technique using a sequence of drills for implant bed preparation, osteotome technique using tapered hand instruments for creating implant sites by condensing the bone and guided flapless implant surgery with surgical templates. Material and methods: The study included 150 patients (80 males and 70 females), 46–71 years old, who required implant supported fixed partial dentures in the posterior maxilla of D3 or D4 bone density. Patients were randomly assigned into one of the three surgical insertion technique groups. All patients received tapered implants of the same manufacturer of the same length and two different widths (3.3 11.5 mm or 4.2 11.5 mm). Primary and secondary implant stability were measured by means of resonance frequency analysis (RFA) at the time of implant placement and 5 months after surgery using the Ostell ISQ device (Osstell AB, Sampgatan, Goteborg, Sweden). Statistical analysis included onesample Kolmogorov Smirnov test, descriptive statistics, multivariate analysis (Bonferoni post-hoc tests) and paired t-tests. Results: Patients in the osteotome group exhibited higher primary stability (P < 0.01) than in the conventional and surgically guided flapless groups. Objective The aim of this study was to report the clinicopathologic features of 105 new cases of adenomatoid odontogenic tumor (AOT) from Brazil, Guatemala, Mexico, and South Africa. Study Design Clinical and radiographic data were collected from records of 5 oral pathology laboratories, and all cases were microscopically reviewed. Results This series of cases included 36 (34.2%) from South Africa; 33 (31.4%) from Brazil; 23 (21.9%) from Mexico; and 13 (12.3%) from Guatemala. Seventy-two patients (68.5%) were females and 33 patients (31.4%) were males, with an average age of 19.2 years. The tumors predominantly affected the anterior maxilla (48.5%), followed by the anterior mandible (29.5%); posterior mandible (15.2%); and posterior maxilla (6.6%). Sixty-three tumors (63.6%) were follicular, 34 (34.3%) were extrafollicular, and 2 (2%) were peripheral. In most cases, the tumors caused considerable cortical expansion (average size 3.4 cm). Microscopically, most cases showed rosette-like and duct-like structures. Conclusions This is one of the largest case series of AOTs reported in the literature and the first multicenter study involving populations from Latin America and South Africa. In a fifth of the present cases, the posterior regions of the gnathic bones were affected and the tumors demonstrated larger sizes compared with cases occurring in other populations.

Purpose Bone augmentation and implant placement often lead to a coronal shift of the soft tissue. Therefore replacement of the mucogingival border during second-stage surgery is required. Different techniques have been described in the literature. Material and methods In this case report a new technique for second-stage surgery from a combination of a modified roll flap, an apically repositioned split thickness flap and thick free gingival grafts between the implants will be described for predictable gain of keratinized mucosa around and between dental implants. Results Stable soft tissue conditions between the implants and buccal in the posterior maxilla could be gained showing almost no post-surgical resorption. Conclusion With this modified technique for second-stage surgery functional and esthetic results can be achieved.

Purpose: The aim of this study was to evaluate the survival and success rates of oral rehabilitations in a shortened maxillary dental arch and expanded maxillary sinus with 4-mm extrashort implants splinted to longer ones. Materials and methods: One 10-mm and one/two extrashort 4-mm titanium-zirconium SLActive tissue-level dental implants were inserted into 11 patients with limited vertical bone availability due to an expanded maxillary sinus antrum. Immediately and 6 months after insertion, implant stability resonance frequency analysis (RFA) values were assessed with an Osstell device. Splinted crowns combining 4- and 10-mm implants were supplied to all 11 cases. Results: In 10 cases, the bone quality was type III, and in one case, type IV. Among 17 4-mm and 11 10-mm implants, the median RFA values were 61 (interquartile ranges [IQR]: 59 to 64) and 66 (IQR: 64 to 72). One 4-mm implant failed to osseointegrate and was removed. After 6 months of healing, secondary-stability measurements of 16 of the remaining 4-mm implants increased to 68 (IQR: 62 to 72) and of 10-mm implants to 78 (IQR: 77 to 80). After 1 year, all (11/11) oral rehabilitations supported by 10-mm (11/11) and 4-mm (16/16) implants were functional. The medians and IQRs of the probing depths (median: 2.8 mm, IQR: 2.3 to 3.1 mm vs median: 2.9 mm, IQR: 2.4 to 3.1 mm) and the crestal bone loss (median: 0.75 mm, IQR: 0 to 0.9 mm vs median: 0.22 mm, IQR: 0 to 0.4 mm) for the 10-mm and 4-mm implants, respectively, were similar. Conclusion: Rehabilitations with splinted crowns combining 4- and 10-mm implants demonstrated a favorable 1-year performance in a shortened maxillary dental arch.

This study aimed to rehabilitate shortened maxillary dental arch with splinted crowns by connecting ultra-short implants with longer ones. In the posterior maxilla of 11 patients, one 10-mm (n = 11) and one or two ultra-short 4-mm (n = 17) dental implants were inserted. The insertion torque was lower than 20 Ncm in 55% of the 10-mm implants and in 94% of the 4-mm implants (P > .05). Median (range) implant stability quotients at the time of insertion and after 6 months were 61 (14 to 72) and 68 (51 to 79), respectively, for 4-mm implants, and 66 (52 to 78) and 78 (60 to 83), respectively, for 10-mm implants (P .05). Splinted crowns combining 4- and 10-mm implants may contribute to a better force distribution in the treatment with ultra-short implant-supported prosthesis in the posterior maxilla.

Background: Augmentation of the maxillary sinus increases the lost height of ridge in the posterior maxilla. Bony convolutions from the floor of the sinus called septa may be encountered during the procedure and significantly impact the outcome of the procedure. Materials and Methods: For this retrospective study, 120 cone-beam computed tomography (CBCT) scans were collected from the archives of the department of oral radiology in the time period between January 2016 and August 2018. Planmeca Romexis® software version 3.2.7 was used to check for the incidence, and then among the septa, plane of orientation, location with respect to the wall of the sinus, and in terms of relation to tooth were examined. The average height and average width of all the septa were measured using a measuring tool in the software. Results: Thirty-five of the total 88 patients showed septa in 40 sinuses, in which 36 sinuses showed one septum and 4 sinuses showed 2 septa. Forty-one septa were located on the floor of the sinus in the frontal orientation and 3 septa were located on the lateral wall of the sinus in the axial orientation. The mean height and mean width of the 41 septa on the floor of the sinus were 5.72 mm and 3.47 mm, respectively. The mean height of septa found on the lateral wall of the sinus was 5.59 mm. Conclusion: Septa had a higher tendency to be in bucco-palatal direction and on the floor of the sinus and frequent occurrence in the region of molars. A thorough prediagnostic evaluation using CBCT is necessary prior to sinus augmentation procedures.

The aim of this case series was to assess, over a period of 24 months, the clinical and radiographic outcomes in partially edentulous patients receiving bone-level tapered implants. In total, 33 partially edentulous patients and 50 implants were evaluated. Patients received single or multiple implants in the posterior maxilla. Clinical and radiographic measurements of vertical bone levels were assessed at surgery, at loading, and 6, 12, and 24 months after surgery. The success and survival rates of the implants were also evaluated. Within the 24-month follow-up, only 1 implant failed (2.0%). Other biological or technical complications were not observed. The mean insertion torque was 34 ± 5.3 Ncm. Bone-level changes of 0.35 ± 0.23 mm were found between surgery and 12 months after surgery, and changes of 0.03 ± 0.05 mm were found between 12 months and 24 months after surgery. The overall change from surgery to 24 months after implant placement was 0.38 ± 0.24 mm. Most of the bone loss occurred between surgery and 3 months (0.28 ± 0.19 mm; P < .001); thereafter, the loss was minimal and statistically nonsignificant. Bone-level tapered implants yielded a high survival and success rate with minimal bone-level changes. Tapered implants could be considered as a predictable treatment option for partially edentulous patients with different types of bone qualities and loading protocols.

Adenoid ameloblastoma is a rare benign odontogenic tumor that favors a slight predilection for male patients, fourth and fifth decades of life, and posterior regions of the jaws. To date, less than 40 cases have been reported in the English language literature. The radiographic aspects of adenoid ameloblastoma vary from unilocular and well-defined lesions to diffuse and multilocular lesions. Most of the lesions exhibit a radiolucent image and are usually large, with a mean size of 3.5 cm. Microscopically, pseudoductal structures composed of columnar cells in a palisaded arrangement formed from the parenchyma of the tumor were observed. We describe a case of adenoid ameloblastoma in a 54-year-old woman, who presented with no symptoms. Panoramic radiography showed a well-circumscribed, unilocular radiolucency in the left posterior maxilla. As odontogenic tumors are rare, some entities are infrequently encountered, making the diagnosis more difficult. Clinicians, oral and maxillofacial surgeons and oral pathologists should be familiar with the adenoid ameloblastoma and its differential diagnosis for accurate diagnosis and management.

BACKGROUND There is limited information about inferior meatus pneumatization in terms of implant treatment for the posterior maxilla. PURPOSE To demonstrate the clinical, radiologic, and nasal endoscopic outcomes of implants accidentally protruding into the nasal cavities with inferior meatus pneumatization in the posterior maxilla. MATERIALS AND METHODS A total of 560 implants (324 patients) with transcrestal sinus augmentation in the posterior maxilla were screened. On panoramic radiographs, 132 implants (78 patients) appear to penetrate the sinus floor, but cone-beam computed tomography demonstrated that 26 implants (13 patients) penetrated the nasal floor due to inferior meatus pneumatization. Width of nasal and sinus cavities and extent of penetration were radiographically measured. Nasal endoscopy was performed. RESULTS Two implants were lost during 13.81 ± 5.40 years due to peri-implantitis. However, no nasosinusal problem occurred in any patient. The nasal cavity was significantly larger in patients with implants penetrating the pneumatized inferior meatus than in those with implants penetrating the sinus floor (P< .05). Nasal endoscopy demonstrated that the apical portion of the implants was covered with thin mucosa without inflammation. CONCLUSION Implants accidentally penetrating the pneumatized inferior meatus demonstrated high survival with no specific nasosinusal complications. Inferior meatus pneumatization should be considered for implant treatment in the posterior maxilla.

METHODS: A total of 310 implants inserted in 113 patients were evaluated, 87 of which were inserted in association with grafted maxillary sinuses (56 morse taper (MT) implants and 31 external hexagon (EH) implants) in 37 patients, and 223 implants were inserted in native bone areas (112 MT and 111 EH implants) in 76 patients. Peri-implant clinical analyses were performed (bleeding on probing, probing depth, clinical insertion level, peri-implant marginal level, and the presence of mobility or suppuration), and the radiographic bone level was evaluated. Objective To compare the early changes in implant stability of implants with different neck design during the first 3 months of healing in the posterior maxilla. Materials and methods Patients were randomized to receive triangular neck implant (test), or round neck implant (control). Resonance frequency analysis (ISQ) measurements were obtained at surgery and at 2, 4, 7, 14, 21, 28, 45, 60, and 90 days following implant placement. Non-parametric statistic was used for data analysis. Results Thirty-two patients were included (17 test and 15 controls). Initial ISQ values of the test implants were high (mean: 68.4, SD = 8.4) and increased over time (mean: 74.4, SD = 6.0). Control implants presented a statistically significant higher initial ISQ value at implant placement (mean: 76.9, SD = 8.7), which was maintained over the healing period (mean: 77.6, SD = 3.6) with no significant changes between time intervals. After 6 weeks of healing, both implants displayed comparable ISQ values with no differences between the groups. All implants exhibited a decrease in stability on days 2 and 21 post-placement. All roundneck implants used, and 82% of the triangularneck implants showed initial ISQ values above the suggested threshold for immediate loading (>60). Conclusions Implant neck design plays a role in implant primary stability in the posterior maxilla. Both implants show high primary stability, with significantly higher values for the round neck. However, these differences disappeared after 6 weeks of healing. While primary implant stability is partially governed by implant neck design, the role of this result is negligible for the achievement of secondary stability.

The edentulous posterior maxilla poses challenges to reconstruction. Posterior forces are great, yet bone quality and quantity are diminished. There is frequent loss of bone resulting from ridge resorption as well as sinus pneumatization. There are also advantages of placing implants in the posterior maxilla compared with the anterior maxilla, such as improved hygiene, esthetics and phonetics, and biomechanical load distribution. This article will present strategies and techniques for improving implant support in the posterior maxilla through various grafting techniques.

The aim of this retrospective study was to investigate the accuracy of dynamic navigation for the placement of intentionally tilted implants in the posterior maxilla. The study included 12 patients with edentulism or continuous multiple tooth loss, who had 48 implants inserted under dynamic navigation guidance in the posterior maxilla. Twenty-four implants near maxillary sinuses were intentionally tilted. The average platform deviation was 1.3 ± 0.4 mm (range 0.8–2.3 mm), apex deviation was 1.1 ± 0.5 mm (range 0.2–2.3 mm), and axis deviation was 3.1 ± 1.0° (range 1.8–6.7°). The other 24 implants were axially positioned. The average platform deviation was 1.5 ± 0.5 mm (range 0.7–3.1 mm), apex deviation was 1.3 ± 0.7 mm (range 0.5–3.1 mm), and axis deviation was 3.2 ± 1.5° (range 1.5–7.7°). There was no significant difference in platform deviation, apex deviation, or axis deviation between the tilted implants and implants in the axial position (P > 0.05). This analysis indicates that a dynamic navigation system can be used as a method of guidance to place intentionally tilted implants as accurately as axially positioned implants in the posterior maxilla, thereby preventing damage to the maxillary sinuses and the need to graft bone.

Pterygoid implant placement has not been a common treatment modality to manage the atrophic posterior maxilla. This randomized, controlled clinical trial evaluated the accuracy of dynamic navigation using trace registration (TR) technology in pterygoid implant placement when compared to free-hand surgery. Partially edentulous patients requiring at least one pterygoid implant to rehabilitate the atrophic posterior maxilla were included. Implant accuracy (in a prosthetically directed context) and the relation of the placed implants to the greater palatine canal (GPC) were evaluated using EvaluNav to compare the preoperative CBCT plan with the postoperative CBCT implant location. Osseointegration success, mucosal thickness, implant length, time spent for surgical placement, and ease of prosthetic restorability via degree of multi-unit abutment angulation were assessed. A total of 63 pterygoid implants were placed (31 using TR, 32 using free-hand) in 39 partially edentulous patients. Mean deviations between the planned and actual position for TR-placed implants were 0.66 mm at the coronal level, 1.13 mm at the apical level, 0.67 mm in depth, and 2.64 degrees of angular deviation, compared to 1.54 mm, 2.73 mm, 1.17 mm, and 12.49 degrees, respectively, for free-hand implants. In relation to the GPC, TR implants were more accurate when compared to the presurgical plan and took less surgical time. The mean mucosal thickness measured for all implants was 5.41 mm. Most implants were 15 to 18 mm long, and most prostheses (92%) could be accommodated by a 17- or 30-degree multi-unit screw-retained abutment. TR implants had greater short-term osseointegration success rates than free-hand implants (100% vs 93.75%). Pterygoid implant surgery can be a predictable and successful modality for prosthetically directed implant rehabilitation in the atrophic posterior maxilla, is more accurate than free-hand surgery, and takes less time when using dynamic navigation via TR.

This study aimed to compare the developmental characteristics of bony nasopharyngeal (NP) in children with cleft palate (CP) and non-cleft subjects.This study was conducted on 64 non-syndromic CP patients who underwent repair surgery and 23 non-cleft subjects as controls. Lateral cephalograms were performed on participants at rest in the natural head position. The X-coordinate and the Y-coordinate of three points on cephalograms were determined as Hormion (Ho), posterior maxillary point (PMP) and anterior point of the atlas (At) representing the anterior-posterior (AP) and vertical dimension of the NP. The linear dimension of the NP (i.e. Ho-At, Ho-PMP, At-PMP) and its area was also calculated.The bony structures of NP in the clef-affected subjects in isolated CP subgroup, had a significantly greater downward development in the maxillary region (PMP) (both with p = 0.001), more linear growth in cranial-maxillary (Ho-PMP) dimension (p = 0.017 and 0.004, respectively), and larger area (p = 0.017 and0.001, respectively), when compared to normal subjects. There was no significant difference between either the unilateral cleft lip and palate (CLP) or bilateral CLP group with the control group regarding AP, vertical, and linear growth of the NP and its area (P 0.05).Patients with repaired CP had downward deviated posterior maxilla, more linear growth in cranial-maxillary dimension and larger area compared to normal subjects. Among CP subtypes, a balanced growth was observed among repaired UCLP and BCLP patients, suggesting that with appropriate repaired surgery, normal development of the NP region could be expected in these subtypes.

This study aimed to compare the crestal bone loss between a 2-implant–supported, noncantilevered 3-unit fixed partial prosthesis (TUFPP) with sinus augmentation and a 2-implant–supported, distal cantilevered TUFPP without sinus augmentation in the posterior maxilla having insufficient crestal bone. The study subjects were enrolled in 2 groups. Group 1 included patients with 2 implants: an anterior implant placed in the native bone and a posterior implant inserted with simultaneous sinus augmentation using a xenogenic bone graft to support a TUFPP. Group 2 included patients with 2 implants inserted in the native bone to support a distal cantilevered TUFPP. The crestal bone levels at the distal and mesial aspects of each implant were measured at baseline and 6-, 12-, and 24-month follow-up controls on panoramic radiographs. Fifty-two patients and 104 implants were included. There was a significant difference in distal crestal bone loss between anterior and posterior implants in group 1 at 6 months (P < .05) but not at 12 and 24 months (P > .05). Distal crestal bone loss was significantly increased in group 1 posterior implants compared to the group 2 posterior implants at 6 months (P < .05). There was no significant difference in mesial bone loss between the anterior and posterior implants in both groups at all follow-up controls (P > .05). There was also no significant mesial crestal bone loss in relation to the anterior and posterior implants of both groups at all follow-up controls (P > .05). Noncantilevered 2-implant–supported TUFPP with sinus augmentation may have similar medium-term crestal bone loss when compared to cantilevered 2-implant–supported TUFPP without sinus augmentation. Further prospective studies should be designed to compare the performance of the 2-implant–supported cantilevered TUFPP and 2-implant–supported TUFPP with sinus augmentation.

Background:Zygomatic implant surgery is considered as a safe and successful alternative to the conventional implant surgery with bone grafts for patients with severe atrophic maxilla. Purpose:The aim of this retrospective clinical case series was to report clinical outcome of zygomatic implants with a follow-up between 6 months and 7 years. Materials and methods:A total of 110 patients with 302 zygomatic implants were included in this study. The intra and postoperative complications and survival rate of zygomatic implants were evaluated. Results:The study included 110 consecutively treated patients with an age range of 21 to 76 years (mean 57.35 years, SD 10.42). The overall zygomatic implant survival rate was 98.34%. There were five implant failures in four patients. One intraoperative and 17 postoperative complications developed in 18 patients. There were no dropouts and the median follow-up of the patients was 41.75 months (with a range of 6-89 months). Conclusions:According to the results, in cases of severely atrophic posterior maxilla, zygomatic implant surgery can be considered as an effective and safe alternative to conventional implants and bone grafting procedures.

Background The aims of this study were to enumerate the primary implant stability quotient (ISQ) value of self-tapping dual etched implants and to explore the influence of parameters such as implant length, implant diameter, age, gender, implant location and osteotomy preparation on the ISQ value. Methods Retrospective data from clinical worksheets given to participants during two implant courses held between the periods of 2013 to 2014 were evaluated. A total of 61 implants were considered based on the inclusion criteria. The effects of parameters such as implant diameter, implant length, age, gender, implant location and osteotomy protocol on ISQ values were analyzed. Results Mean ISQ value for all implants was 67.21±9.13. Age of patients (P=0.016) and location of implants (P=0.041) had a significant linear relationship with the ISQ values. Within the age limit of the patients in this study, it was found that an increase in one year of patient's age results in 0.20 decrease in ISQ value (95% CI: -0.36, -0.04). However, placing an implant in the posterior maxilla may negatively affect the ISQ with a likely decrease in primary stability by 6.76 ISQ value (95% CI: -13.22, -0.30). Conclusions The results suggest that the mean ISQ achieved by the participants were comparable with the range reported for this particular type of implants. The patient's age and location of implants were elucidated as the determinant factors of primary implant stability.

Objectives High crown‐to‐implant ratios may lead to complications due to unfavorable occlusal forces, including nonaxial forces, on the bone surrounding the neck of the implant and within the connection of the crown and implant itself. The aim of this study was to perform a systematic review on the influence of crown‐to‐implant ratio of single‐tooth, nonsplinted, implants on biological and technical complications. Materials and Methods MEDLINE (1950–January 2018), EMBASE (1966–January 2018), and Cochrane Central Register of Controlled Trials database (1800–January 2018) were searched to identify eligible studies. Inclusion criteria were as follows: crown‐to‐implant ratio of single‐tooth, nonsplinted, implant‐supported restorations in the posterior maxilla or mandible and follow‐up of at least 1 year. Main outcome measures were as follows: implant survival rate, marginal bone level changes, biological complications, and technical complications. Two reviewers independently assessed the articles. A meta‐analysis was carried out for implant survival rate and peri‐implant bone changes. Results Of 154 primarily selected articles, eight studies fulfilled the inclusion criteria. Study groups presented a mean crown‐to‐implant ratio varying from 0.86 (with 10‐mm implants) to 2.14 (with 6‐mm implants). Congenital maxillomandibular fusion or syngnathia is a rare craniofacial disorder with only 26 cases reported in the literature. We present a case of a congenital complex zygomatico-mandibular syngnathia associated with a palatal cleft, posterior maxilla and turbinate agenesia, mild hemifacial microsomia, and a disordered dental eruption. The patient has the highest age (15 years) at diagnosis described in the literature. 3D planning of the surgery was performed to study the patient's anatomy and design the necessary osteotomies to separate the jaws. En bloc removal of the fused fragment with bilateral coronoidectomy and aggressive long-term physiotherapy for 3 months led to a stable increase in mouth opening from 0 to 21 mm inter-incisor distance. The patient reported an improvement in speech, was able to eat without restriction regarding food consistency, and could maintain a good oral hygiene.

Purpose Imbalance of bone remodeling causes osteoporosis. The aim of this study was to compare marginal bone loss (MBL) in patients with osteoporosis and those without. Methods This was a prospective cohort study. Females who needed dental implant restoration at the posterior maxilla were studied. Dual-energy x-ray absorptiometry (DXA) was performed, and the T scores were documented. MBL was measured at 12 months after loading. Patients were assigned to 2 groups: group 1 (osteoporosis group: T score ≥ 2.5) and group 2 (non-osteoporosis group: T score Results Ninety females (44 in group 1 and 46 in group 2) were studied. The mean MBL for group 1 was 1.20 ± 0.29 mm and in group 2, it was 0.87 ± 0.15 .There was a significant difference in the means of the MBL in the 2 groups (P = .001) there was a correlation between T scores and MBL (P = .001). Conclusions It appears that MBL in females with osteoporosis is more significant than that in females without osteoporosis. There was a correlation between T scores and MBL in the posterior maxilla.

Background Implant failures are more common when multiple missing posterior teeth need lateral sinus floor elevation owing to inadequate tissue maturation after grafting. Effects of lateral window dimensions on vital bone formation have rarely been compared. Purpose To compare endo-sinus bone formation between two- and solo-window techniques to rehabilitate multiple missing posterior teeth that need substantial augmentation. Methods and materials Patients with severely atrophic posterior maxilla were randomized to receive lateral sinus floor elevation via solo or two bony windows. Bone core specimens harvested from lateral aspect of the augmentation sites were histomorphometrically analyzed. Proportions of mineralized bone (MB), bone substitute materials (BS), and nonmineralized tissue (NMT) were quantified. Results Twenty-one patients underwent 23 maxillary sinus augmentations. One patient in each group dropped out during the follow-up period. Lateral window dimensions were 81.65 ± 4.59 and 118.04 ± 19.53 mm2 in the test and control groups, respectively. Histomorphometric analysis revealed mean MB of 42.32% ± 13.07% and 26.00% ± 15.23%, BS of 40.34% ± 9.52% and 60.03% ± 10.13%, and NMT of 18.14% ± 14.24% and 14.75% ± 10.38% in test and control groups, respectively, with significant differences. Conclusion The two-window technique could facilitate faster maturation and consolidation of the grafted volume and is an effective alternative for rehabilitation of severely atrophic posterior maxilla with multiple missing posterior teeth.

The aim of this narrative review is to describe treatment options for the posterior regions of the mandible and the maxilla, comparing short implants vs. longer implants in an augmented bone. The dental literature was screened for treatment options enabling the placement of dental implants in posterior sites with a reduced vertical bone height in the maxilla and the mandible. Short dental implants have been increasingly used recently, providing a number of advantages including reduced patient morbidity, shorter treatment time, and lower costs. In the posterior maxilla, sinus elevation procedures were for long considered to be the gold standard using various bone substitute materials and rendering high implant survival rates. More recently, implants were even placed without any further use of bone substitute materials, but the long-term outcomes have yet to be documented. Vertical bone augmentation procedures in the mandible require a relatively high level of surgical skill and allow the placement of standard-length dental implants by the use of autogenous bone blocks. Both treatment options, short dental implants, and standard-length implants in combination with vertical bone augmentation procedures, appear to result in predictable outcomes in terms of implant survival rates. According to recent clinical studies comparing the therapeutic options of short implants vs. long implants in augmented bone, the use of short dental implants leads to a number of advantages for the patients and the clinician., Graphical Abstract

Sinus augmentation with a lateral window technique and delayed implant placement are the traditional treatment schedules when the residual alveolar bone height in atrophic posterior maxilla is less than 5 mm. This article presents a transcrestal window approach (TWA) to replace a missing first molar in an atrophic posterior maxillary ridge with 1 to 2 mm of residual bone height (RBH). Transcrestal sinus augmentation and delayed implant placement were performed. The antral pseudocyst that was located in the nasal wall of the maxillary sinus disappeared in the cone-beam computer tomography images taken after sinus elevation surgery. The 1-year follow-up visit showed that the implant was successful and the Periotest value was -4.8. This approach reduces discomfort to the patient. The TWA with an RBH of 1 to 2 mm is a minimally invasive method for maxillary sinus augmentation.

The primary objective of the present in vitro study was to evaluate the influence of implant site preparation technique (drills vs ultrasonic instrumentation) on the primary stability of short dental implants with two different designs inserted in simulated low-quality cancellous bone. Eighty implant sites were prepared in custom-made solid rigid polyurethane blocks with two different low cancellous bone densities (5 or 15 pounds per cubic foot [PCF]), equally distributed between piezoelectric (Surgysonic Moto, Esacrom, Italy) and conventional drilling techniques. Two short implant systems (Prama and Syra, Sweden & Martina) were tested by inserting 40 fixtures of each system (both 6.0 mm length and 5.0 mm diameter), divided in the four subgroups (drills/5 PCF density; drills/15 PCF density; piezo/5 PCF density; piezo/15 PCF density). Insertion torque (Ncm), implant stability quotient values, removal torque (Ncm), and surgical time were recorded. Data were analyzed by 3-way ANOVA and Scheffé's test (α = 0.05). With slight variations among the considered dependent variables, overall high primary implant stability was observed across all subgroups. Piezoelectric instrumentation allowed for comparable or slightly superior primary stability in comparison with the drilling procedures in both implant systems. The Prama implants group showed the highest mean reverse torque and Syra implants the highest implant stability quotient values. Piezoelectric implant site preparation took prolonged operative time compared to conventional preparation with drills; among the drilling procedures, Syra system required fewer surgical steps and shorter operative time.

Purpose: The purpose of this prospective study is to evaluate the relationship between implant diameter, and primary and secondary stability. Materials and methods: Five hundred fifty-nine implants with diameters of 3.7, 4.0, and 4.3 mm and lengths of 10 and 11.5 mm were placed in 195 patients. The resonance frequency was measured during surgery and at 3, 6, and 12 months. Results: Related average implant stability quotient (ISQ) values were 69.62 for 3.7-mm implants, 72.02 for 4.0-mm implants, and 69.67 for 4.3-mm implants. Values in men were greater than values in women. Values were greater for the mandible than for the maxilla. There are significant differences between 4.0-mm implants, and 3.7 anterior maxilla and 4.3 posterior maxilla. Conclusions: There is no relationship between increased ISQ values and increased diameters. We observed a preference regarding sex, with men having significantly greater values for 3.7- and 4.3-mm diameters. The mandible obtained the greatest ISQ values, with significant differences for diameters 3.7 and 4.3 mm. Objective The aim of this study was to evaluate the efficacy of simultaneous endoscopic endonasal sinus surgery and sinus augmentation with immediate implant placement. Patients and methods The study patients (n = 23) were partially or completely edentulous in the posterior maxilla and required maxillary sinus augmentation. All included patients had a sinus pathology confirmed clinically and radiographically. The technique of simultaneous endoscopic endonasal sinus surgery and sinus augmentation was used in 15 patients, with eight endonasal sinus surgery procedures being performed 2–3 months before sinus augmentation. Where possible, implants were placed during the same surgical procedure (with a ridge bone height of at least 4 mm). Results There were no any major intraoperative complications. Implants placed in the reconstructed areas were shown to integrate normally, and postoperative occlusal function and aesthetics were favorable. Of the 95 implants placed in these 23 patients, two failed to osseointegrate. Conclusion The method of simultaneous endoscopic endonasal sinus surgery and sinus augmentation with immediate implant placement leads to a reduction in postoperative complications, significantly shortening the rehabilitation period for patients with maxillary sinus diseases and insufficient bone tissue.

The use of the crestal approach to augment vertical height of the posterior maxilla for dental implant placement is reviewed in this article. The review of the literature documents the success of using a procedure that has minimal complications associated with it when performed correctly. An algorithmic approach is presented to guide surgeons on using the crestal osteotome approach for posterior maxillary augmentation.

Introduction Sinonasal anatomical anomalies and inflammatory conditions may reduce success rates of maxillary sinus elevation (MSE) procedures used to allow implant placement in the atrophic posterior maxilla. Approaches combining endoscopic sinus surgery (ESS) and MSE were firstly proposed by our group and have already been described in the literature. This article aims to re-evaluate the procedure in a larger sample of patients comparing results and indications with the pertaining literature. Materials and methods Thirty-three patients (19 men and 14 women, mean age 52.79 ± 11.95 years) underwent combined ESS/MSE with 48 MSE procedures performed. Results No intraoperative complications were reported. An uneventful and complete graft integration was obtained after 6 months in all but one patient. All patients completed prosthetic rehabilitation within 9 to 12 months. Conclusion Combined ESS and MSE has proven to be a safe and reliable procedure, which can be proposed to selected patients presenting with reversible sinonasal contraindications to MSE and should be no more considered an experimental procedure.

Purpose: The aim of this review was to systematically appraise survival rates of implants placed using the osteotome technique with and without grafting in the published literature. Materials and Methods: An electronic search was conducted to identify prospective and retrospective studies on osteotome sinus floor elevation published between January 1, 2000 and October 30, 2015. Studies were included that (1) involved use of the osteotome technique with or without grafting; (2) provided data regarding the implant survival rates, residual bone height (RBH), and grafting materials; and (3) reported mean follow-up of at least 1 year after functional loading and included a minimum of 10 patients. The mean weighted cumulative implant survival rates were used to compare the two treatment strategies-grafted or nongrafted. The influence of RBH and implant length on weighted cumulative implant survival was also evaluated. Results: After search and evaluation of the literature according to the inclusion criteria, 34 studies involving 1,977 patients and 3,119 implants were included. Eighty-four out of 102 implant failures documented in the studies occurred within 1 year of functional loading. Statistically significant differences in the cumulative survival rates were found in the graft and nongraft groups (95.89% and 97.30%, respectively; P = .05). In the nongraft group, no statistically significant difference in the cumulative survival rate was found when implants were placed at RBH < 5 mm or ≥ 5 mm (95.04% and 97.63%, respectively; P = .12). In the graft group, however, a statistically significant difference was found when implants were placed at RBH < 5 mm or ≥ 5 mm (92.19% and 97.59%, respectively; P < .01). Significantly lower weighted mean cumulative implant survival rates were found in the shorter (< 8 mm) implant group than in the longer (≥ 8 mm) implant group (83.33% and 96.28%, respectively; P < .01). Conclusion: The cumulative survival rates were significantly higher in the nongraft group than in the graft group. Early failures (< 1 year functional loading) accounted for the vast majority of the implant failures. The cumulative survival rates in the graft group were significantly lower when the RBH was < 5 mm, while the cumulative survival rates in the nongraft group demonstrated no statistically significant difference based on RBH. Shorter (< 8 mm) implants demonstrated significantly lower cumulative survival rates than longer implants.

Purpose The aim of this study was to determine the influence of anatomical conditions on primary stability in the models simulating posterior maxilla. Methods Polyurethane blocks were designed to simulate monocortical (M) and bicortical (B) conditions. Each condition had four subgroups measuring 3 mm (M3, B3), 5 mm (M5, B5), 8 mm (M8, B8), and 12 mm (M12, B12) in residual bone height (RBH). After implant placement, the implant stability quotient (ISQ), Periotest value (PTV), insertion torque (IT), and reverse torque (RT) were measured. Two-factor ANOVA (two cortical conditions×four RBHs) and additional analyses for simple main effects were performed. Results A significant interaction between cortical condition and RBH was demonstrated for all methods measuring stability with two-factor ANOVA. In the analyses for simple main effects, ISQ and PTV were statistically higher in the bicortical groups than the corresponding monocortical groups, respectively. In the monocortical group, ISQ and PTV showed a statistically significant rise with increasing RBH. Measurements of IT and RT showed a similar tendency, measuring highest in the M3 group, followed by the M8, the M5, and the M12 groups. In the bicortical group, all variables showed a similar tendency, with different degrees of rise and decline. The B8 group showed the highest values, followed by the B12, the B5, and the B3 groups. The highest coefficient was demonstrated between ISQ and PTV. Conclusions Primary stability was enhanced by the presence of bicortex and increased RBH, which may be better demonstrated by ISQ and PTV than by IT and RT., Graphical Abstract

Background Very few controlled studies have compared short and long implants placed with appropriate sinus floor elevation techniques. Purpose To compare the 2-year outcomes of 6.5-mm hydrophilic implants placed with osteotome sinus floor elevation (OSFE) and standard implants placed with lateral sinus floor elevation in patients with a severely atrophic posterior maxilla. Materials and Methods Thirty-eight patients with a residual bone height of 4–5 mm were randomized to receive one of the two above-mentioned treatments. Intra- and postoperative complications were recorded. The implant survival rate, peri-implant bone level, and periapical endosinus bone gain were assessed. Results Of the 80 inserted implants, one in the long implant group failed because of abscess formation. The peri-implant bone level change (0.35 ± 0.60 mm vs 0.40 ± 0.71 mm) was not significantly different between the two groups. The endosinus bone gain was 2.94 ± 0.81 mm and 10.19 ± 0.95 mm in the short and long implant groups, respectively. No serious adverse events related to implant surgery were recorded. Conclusions The results suggest that the placement of 6.5-mm short implants with OSFE is an effective alternative for the rehabilitation of a severely atrophic posterior maxilla.

Objectives The aim of this study was to evaluate the hybrid technique, which was modified from the lateral approach in sinus elevation by combining transalveolar access. Materials and methods Sixty patients (mean age of 53.3 ± 11.6 years) with 105 implants placed in the posterior maxilla with the average remaining bone height (RBH) of 4.3 ± 1.8 mm (range of 1.2–9.2 mm) were enrolled in the hybrid group and followed up for 96 months. The control group included 36 patients with 44 implants placed in the corresponding site with sufficient RBH. Cumulative survival rate (CSR), radiographic measurements in RBH, augmented bone height (ABH), and marginal bone loss (MBL) were analyzed during the follow-up period. Results Cumulative survival rate in the hybrid group was 98.1%, which was comparable to the control group (97.7%). MBL of the mesial and distal aspects in the hybrid group (0.28 ± 0.66 mm and 0.41 ± 0.93 mm, respectively) was comparable to the control group (0.84 ± 1.11 mm and 0.76 ± 0.88 mm, respectively). In the hybrid group, the membrane perforation rate during the surgery was 4.5%. ABH was 8.3 ± 1.8 mm after the surgery and maintained consistently during the follow-up period (7.4 ± 1.9 mm after 4 years). Conclusion The hybrid technique is a simple and predictable surgical method for simultaneous placement of a dental implant with sinus floor elevation regardless of the RBH.

Chondrosarcoma is a malignant neoplasm that is rare in the jaws, accounting for approximately 0.1% of all neoplasms in this region. This report describes the case of a 37-year-old man who presented a swelling of firm consistency in the right maxilla region, which was asymptomatic with 11 months of evolution. The orthopantomography findings showed a discrete radiopaque mass with poorly defined limits involving the molar teeth. The incisional biopsy results revealed the presence of a lobular neoplasm containing cartilaginous matrix and atypical chondrocytes with different sizes, rare atypical mitoses, and sometimes binucleation and hyperchromatism. Invasion of alveolar bone and absence of necrosis was also observed. The final diagnosis was chondrosarcoma of histopathologic grade I with grade II areas. The patient underwent surgical resection with safety margins and was without signs of relapse.

Objectives The purpose of present study was to compare short (6 mm) with longer implants with the same surface use in the posterior maxilla and/or mandible. Material and Methods A total of 110 implants of 6 or 10 mm in length were placed with an internal hex (n = 60) and with a conical connection (n = 50) but the same material, surface and design, supporting single crowns in the posterior maxilla and/or mandible. Outcomes measured were implant survival and marginal bone level changes up to 24 months after loading. Results Final group consisted of 105 implants: 6 mm (n = 58) and 10 mm (n = 47). Success rate after 24 months was similar between treatment groups (98.3% vs. 100%; P = 0.361). Failure rates of the short implants in mandible (1/18, 5.6%) and in maxilla (0/40, 0%) were also not significantly different (P = 0.133). Success rate after 2 years was similar between internal hex vs. conical connection implants (100% vs. 97.7%; P = 0.233). Subjects lost statistically significant marginal peri-implant bone in both groups, but without differences (6 mm group: 0.38 mm [95% CI = 0.09 to 0.67] vs. 10 mm group: 0.43 mm [95% CI = 0.15 to 0.61]; P = 0.465 at 24 months), in relation also to type of implant (internal hex vs. conical, P = 0.428 at 24 months) or operator (P = 0.875 at 24 months). Conclusions Short implants may be successful in the posterior areas during the first 24 months of loading, with similar outcomes to 10 mm long implants, supporting their use as a valid option in selected cases. However, larger and longer follow-ups of 5 years or more are needed.

Certain patients with facial deformities require superior repositioning of the maxilla via Le Fort I osteotomy; however, the magnitude of superior repositioning of the maxilla is often less than expected. In this study, the correlation between the accuracy of superior repositioning of the maxilla and the anatomical form of the maxillary posterior region was examined. Seventy-five patients who underwent Le Fort I osteotomy without forward movement of the maxilla but with superior repositioning of the maxilla were included in this study. The bone volume around the descending palatine artery (DPA), the angle of the junction between the pterygoid process and the tuberosity, and the distance between the upper second molar and the pterygoid process were measured via three-dimensional analysis. A significant negative correlation (r = −0.566) was found between the bone volume around the DPA and the ratio of repositioning (actual movement divided by expected movement). It is possible that the superior repositioning of the maxilla expected prior to surgery was not sufficiently attained because of the large volume of bone around the DPA. The results of this study show that in some patients, superior repositioning was not achieved at the expected level because of bone interference attributable to the anatomical form of the maxillary posterior region.

Background To compare the radiological parameters and success of posterior maxillary direct sinus lift with simultaneous or delayed implant placement, or implant placement in native bone, after a minimum follow-up period of 5 years. Material and Methods A retrospective cohort study was carried out in a university clinic, selecting patients subjected to implant treatment in the posterior maxilla between the years 2005 and 2011. The patients were divided into three groups: 1) implants placed in native bone; 2) direct sinus lift with simultaneous implant placement; and 3) direct sinus lift with delayed implant placement. Bone crest level, bone loss, vertical bone gain, and implant success and survival after a minimum follow-up period of 5 years after prosthetic loading were analyzed. Results A total of 163 patients and 329 implants were included in the study. The mean duration of follow-up was 7.0 ± 1.9 years. Bone loss and implant success and survival were very similar in all three groups, with no significant differences among them. Graft reabsorption was greatest during the first 12 months, though graft stabilization was confirmed after 5 years of follow-up. Conclusions Bone loss and percentage success and survival proved very similar for the implants placed in native bone and for sinus lift with simultaneous or delayed implant placement. The height of the graft material decreased mainly in the first 12 months, and continued until stabilization after 5 years, with no significant variations thereafter.
