Our study examined the occupational health and safety (OHS) issues that impact environmental public health workforces across various health departments in the U.S. The objective was to generate an OHS risk profile for the workforce to inform future preventative strategies. Data for our study were collected via a cross-sectional online survey (N = 158) supplemented by a virtual focus group (N = 7) discussion to confirm and explore some of the themes and data items that emerged from the survey. Descriptive statistics were used to provide information about the variables in the data sets and to identify relationships between them. Nonparametric tests were used to determine statistical significance between two or more variables that were not normally distributed. Focus group data were analyzed thematically. Our study indicates that the environmental health (EH) workforce in the U.S. is exposed to a range of significant workplace hazards. Some of the OHS concerns identified in our study are similar to those observed in Australia and New Zealand. Biological and chemical exposures appear to be important hazards; however, further study is required to determine the nature and mechanism of these hazards within the EH workforce. Purpose: The burden of health and safety related issues emanating from the modern workplace is overwhelming. Although the prevailing statistics found in the literature show that victims of workplace fatalities and injuries are higher in developing countries and transition economies, there is a very limited understanding of the causal factors that may correspond to the continuous increase in the occupational exposure of employees to various fatalities and injuries. The purpose of this study was to review the literature on occupational safety of workers with the objective of highlighting the major causal factors of workplace injuries and fatalities from a multi-regional perspective, particularly on job-contexts with highest safety and health related hazards. Methodology: A systematic review of the literature was conducted on 73 empirical studies in the regions of Europe, Africa, the Middle East, Asia, Australia, and the Americas (the US & Canada). Findings: The results showed that the highest number of injuries and fatalities were mostly found in the construction industry. It further emerged that most of the studies were centered in developing countries within Africa and Asia. Other results and recommendations were discussed for each region. Value: The findings from this study have the potential to expose the regional dynamics in relation to safety in the workplace. Latino day laborers (LDL) are a vulnerable population of workers facing considerable risk for occupational injury. Under the guidance of our Community Advisory Board, we developed and tested the feasibility, acceptability and preliminary effects of Vales+Tú (You Are Worth More), a workplace injury risk-reduction program implemented by promotores on street-corners where LDL seek employment. The program was informed by theoretical perspectives emphasizing individual and group agency and self-determination. A pilot three-arm cluster-randomized community trial was conducted among 75 LDL. The intervention arms consisted of an individualized Brief Motivational Interview, a Group Problem Solving activity and a standard of care control (OSHA safety cards). We met our study goal of 25 LDL per intervention arm, and contacted 88% of participants post intervention. Participants evaluated the interventions favorably. At post-test, the Brief Motivational Interview group reported significant reductions in exposure to workplace hazards and increases in risk-reduction practices. The Group Problem Solving participants showed significant reductions in exposure to hazards (t -test −4.16, p < 0.001). Both intervention groups increased their reliance on corner peers, a measure of social support. Standard of care participants increased in self-efficacy to work safely. Overall, the only significant different between the three study conditions was in self-efficacy. These findings provide evidence of the feasibility and acceptability of Vales+Tú and show preliminary program efficacy. The Occupational Safety and Health Administration (OSHA) issued an emergency temporary standard (ETS) for COVID-19 applicable to private sector employers with 100 or more employees. Workers in the electric power industry commonly perform physically demanding jobs in hot environments, which combined with the protective clothing worn, places them at risk of disease and health problems related to occupational heat stress. With climate change fueling an increase in the occurrence of hot weather, a targeted approach to heat stress management within the industry is needed. To better understand current heat management practices and identify opportunities for refinement, we conducted an exploratory survey among 33 electric utility companies operating in the United States (n = 32) and Canada (n = 1). Forty-six employees responsible for health and safety of company workers completed 26 questions assessing heat stress as a workplace hazard and heat management practices within five categories: (1) use and administration of heat stress management program; (2) surveillance of heat stress and heat strain; (3) job evaluation and heat-mitigation guidance; (4) education and training programs; and (5) treatment of heat-related illness. While a majority of the respondents (87.0%) indicated heat stress is a workplace hazard and their organization has a heat stress management program (78.3%), less than half reported performing real-time monitoring of heat stress in the workplace (47.8%) or tracking worker heat strain (19.6%) (i.e., physiological response to heat stress). However, most organizations indicated they conducted pre-job evaluations for heat stress (71.7%) and implemented an employee training program on managing heat stress (73.9%). The latter included instruction on various short- and long-term heat-mitigation guidance for workers (e.g., use of work exposure limits, hydration protocols) and the prevention (52.2%) and treatment (63.1%) of heat-related illnesses. Altogether, our survey demonstrates that although many companies employ some form of a heat management program, the basic components defining the programs vary and are lacking for some companies. To maximize worker health and safety during work in hot environments, a consensus-based approach, which considers the five basic components of a heat management program, should be employed to formulate effective practices and methodologies for creating an industry-specific heat management strategy. Harvesting timber for lumber produces is among the most dangerous occupations in the United States. While not exhaustive, the literature on these dangers is substantial. However, several other smaller harvesting forest based industries put workers at risk in unique ways. Relatively little research has been published on these activities, but preliminary exploratory research, summarized here, suggests that besides the risks inherent in being in the forest, workers face some unique workplace risks, the frequency and consequences of which are augmented by the social determinants that characterize the workforce. This paper provides a brief overview of the workplace safety risks to forest green and mushroom harvesters and cedar block cutters in the Northwest. Forest workers, including loggers, foresters, and wildland firefighters, are regularly exposed to some of the most fatal occupational environments in the United States. These hazardous work environments may become even more complex and dynamic when subject to bark beetle outbreaks that have resulted in significant tree mortality. The impacts of tree death from bark beetles are significant, with the cumulative 17-year (2000–2016) footprint for bark beetle caused tree mortality estimated at 54 million acres. However, how workers think about and act in these environments is understudied. This study, therefore, approaches the issue of beetle kill and forest worker safety by examining the perspectives or workers themselves. Its contribution is to leverage ethnographic research to provide insights that can generate new research questions, better inform outreach, and ultimately improve worker safety outcomes. The resulting insights show that beetle kill was understood by workers as a hazard that increased the complexity and dynamism of the work environment, making situational awareness both more necessary and more difficult to maintain. While much research about situational awareness focuses on hazardous situations, it is suggested that building adequate situational awareness should also include broader considerations of organizational communication, as well as training and experience considered over the course of entire careers. Background: Masons have the highest rate of overexertion injuries among all construction trades and rank second for occupational back injuries in the United States. Identified ergonomic solutions are the primary method of reducing exposure to risk factors associated with musculoskeletal disorders. However, many construction workers lack knowledge about these solutions, as well as basic ergonomic principles. Construction apprentices, as they embark on their careers, are greatly in need of ergonomics training to minimize the cumulative exposure that leads to musculoskeletal disorders. Apprentices receive safety training; however, ergonomics training is often limited or non-existent. In addition, apprenticeship programs often lack "soft skills" training on how to appropriately respond to work environments and practices that are unsafe. The SAVE program - SAfety Voice for Ergonomics - strives to integrate evidence-based health and safety training strategies into masonry apprenticeship skills training to teach ergonomics, problem solving, and speaking up to communicate solutions that reduce musculoskeletal injury risk. The central hypothesis is that the combination of ergonomics training and safety voice promotion will be more effective than no training or either ergonomics training alone or safety voice training alone.Methods/design: Following the development and pilot testing of the SAVE intervention, SAVE will be evaluated in a cluster-randomized controlled trial at 12 masonry training centers across the U.S. Clusters of apprentices within centers will be assigned at random to one of four intervention groups (n = 24 per group): (1) ergonomics training only, (2) safety voice training only, (3) combined ergonomics and safety voice training, or (4) control group with no additional training intervention. Outcomes assessed at baseline, at the conclusion of training, and then at six and 12 months post training will include: musculoskeletal symptoms, general health perceptions, knowledge of ergonomic and safety voice principles, and perception and attitudes about ergonomic and safety voice issues.Discussion: Masons continue to have a high rate of musculoskeletal disorders. The trade has an expected increase of 40 % in the number of workers by 2020. Background. The US commercial fishing industry is hazardous, as measured by mortality data. However, research on non-fatal injuries is limited. Non-fatal injuries constitute the majority of occupational injuries and can result in workers' lowered productivity and wages, lost quality of life, and disability. In the United States, a Work Process Classification System (WPCS) has previously been applied in Alaskan freezer-trawl and freezer-longline fleets to identify causes of injuries and specific hazards, but not to other fishing fleets. Objectives. This descriptive epidemiologic study aimed to explore the application and modification of the WPCS in multiple Alaskan fleets, characterize non-fatal occupational injuries in these fleets, and identify work processes that could be targeted for further investigation and future injury prevention efforts. Design. Traumatic, non-fatal injuries on-board Alaskan commercial fishing vessels were identified through United States Coast Guard investigative reports. Characteristics of injuries, as well as worker characteristics, were analysed. Injuries were coded using the WPCS. Results. We successfully utilized the WPCS to code non-fatal injury cases (n = 136). The most frequent main work processes associated with non-fatal injuries included: on-board trawlers, handling frozen fish and processing the catch; on-board vessels using pot/trap gear, handling the gear and shooting/setting the gear; onboard longliners, traffic on board and hauling the gear; and on-board processor vessels, processing the catch, other work with the catch, and handling frozen fish. Conclusions. The study confirmed that a WPCS can be applied to multiple Alaskan fleets to identify hazardous tasks. Hazards were unique for each vessel gear type. Future injury prevention efforts should target work processes associated with the most frequent and most severe injuries. Future studies should establish time estimates for work processes in order to determine risk estimates. Efforts to improve non-fatal injury reporting, especially on smaller commercial fishing vessels, should be undertaken. [ABSTRACT FROM AUTHOR]

The number of personnel providing in-home health care services is increasing substantially. The unique configuration of environmental hazards in individual client homes has a significant impact on the safety and health of home health care providers (HHPs). This mixed-methods study used data from a standardized questionnaire, focus groups, and individual interviews to explore environmental health and safety hazards encountered by HHPs in client homes. The participant sample (N = 68) included nurses, aides, therapists, and owners/managers from a variety of geographic locations. The most often-reported hazards were trip/slip/lift hazards, biohazards, and hazards from poor air quality, allergens, pests and rodents, and fire and burns. Frequency of identified key hazards varied by room, that is, kitchen (e.g., throw rugs, water on floor), bathroom (e.g., tight spaces for client handling), bedroom (e.g., bed too low), living room (e.g., animal waste), and hallway (e.g., clutter). Findings indicate the need for broader training to enable HHPs to identify and address hazards they encounter in client homes. [ABSTRACT FROM AUTHOR]

Objectives. Using the social-ecological model, we hypothesized that the home residences of injured workers would be clustered predictably and geographically. Methods. We linked health care and publicly available datasets by home zip code fortraumatically injured workers in Illinois from 2000 to 2009. We calculated numbers and rates of injuries, determined the spatial relationships, and developed 3 models. Results. Among the 23 200 occupational injuries, 80% of cases were located in 20% of zip codes and clustered in 10 locations. After component analysis, numbers and clusters of injuries correlated directly with immigrants; injury rates inversely correlated with urban poverty. Conclusions. Traumatic occupational injuries were clustered spatially by home location of the affected workers and in a predictable way. This put an inequitable burden on communities and provided evidence for the possible value of community-based interventions for prevention of occupational injuries. Work should be included in health disparities research. Stakeholders should determine whether and how to intervene at the community level to prevent occupational injuries. [ABSTRACT FROM AUTHOR]

Objectives: Asbestos has been used in a broad variety of industrial products, including clutch discs of the transmission system of vehicles. Studies conducted in high-income countries that have analyzed personal asbestos exposures of transmission mechanics have concluded that these workers are exposed to asbestos concentrations in compliance with the US Occupational Safety and Health Administration (US OSHA) occupational standards. Clutch facings are the friction component of clutch discs. If clutch facings are sold separated from the support, they require manipulation before installation in the vehicle. The manipulation of asbestos containing clutch facings is performed by a group of mechanics known as riveters, and includes drilling, countersinking, riveting, sanding, and occasionally grinding, tasks that can potentially release asbestos fibers, exposing the mechanics. These manipulation activities are not reported in studies conducted in high-income countries. This study analyzes personal asbestos exposures of transmission mechanics that manipulate clutch facings. Methods: Air sampling campaigns in two transmission repair shops (TRS) were conducted in November 2012 and July 2013 in Bogotá, Colombia. Four workers employed in these TRS were sampled (i.e. three riveters and one supervisor). Personal samples (n = 39), short-term personal samples (n = 49), area samples (n = 52), blank samples (n = 8), and background samples (n = 2) were collected in both TRS during 3-5 consecutive days, following US National Institute for Occupational Safety and Health (US NIOSH) methods 7400 and 7402. Asbestos samples were analyzed by an American Industrial Hygiene Association accredited laboratory. Results: On at least one of the days sampled, all riveters were exposed to asbestos concentrations that exceeded the US OSHA permissible exposure limit or the Colombian permissible limit value. Additionally, from the forty-seven 30-min short-term personal samples collected, two (4.3%) exceeded the US OSHA excursion limit of 1 f cm"3. Conclusions: In this study, we identified that the working conditions and use of asbestos containing transmission products expose transmission mechanics to asbestos concentrations that exceed both the Colombian and OSHA standards. Aim To explore catalysts to, and circumstances surrounding, patient-to-worker violent incidents recorded by employees in a hospital system database. Background Violence by patients towards healthcare workers (Type II workplace violence) is a significant occupational hazard in hospitals worldwide. Studies to date have failed to investigate its root causes due to a lack of empirical research based on documented episodes of patient violence. Design Qualitative content analysis. Methods Content analysis was conducted on the total sample of 214 Type II incidents documented in 2011 by employees of an American hospital system with a centralized reporting system. Findings The majority of incidents were reported by nurses (39·8%), security staff (15·9%) and nurse assistants (14·4%). Three distinct themes were identified from the analysis: Patient Behaviour, Patient Care and Situational Events. Specific causes of violence related to Patient Behaviour were cognitive impairment and demanding to leave. Catalysts related to patient care were the use of needles, patient pain/discomfort and physical transfers of patients. Situational factors included the use/presence of restraints; transitions in the care process; intervening to protect patients and/or staff; and redirecting patients. Conclusions Identifying catalysts and situations involved in patient violence in hospitals informs administrators about potential targets for intervention. Hospital staff can be trained to recognize these specific risk factors for patient violence and can be educated in how to best mitigate or prevent the most common forms of violent behaviour. Background: Patient handling is a major risk factor for musculoskeletal injuries among nurses. Lifting equipment is a main component of safe patient handling programs that aim to prevent musculoskeletal injury. However, the actual levels of lift availability and usage are far from optimal. Objective: To examine the effect of patient lifting equipment on musculoskeletal pain by level of lift availability and lift use among critical-care nurses. Design and participants: A cross-sectional postal survey of a random sample of 361 critical- care nurses in the United States. Methods: The survey collected data on low-back, neck, and shoulder pain, lift availability, lift use, physical and psychosocial job factors, and sociodemographics. Musculoskeletal pain was assessed by three types of measures: any pain, work-related pain, and major pain. Multivariable logistic regressions were used to examine the associations between musculoskeletal pain and lift variables, controlling for demographic and job factors. Results: Less than half (46%) of respondents reported that their employer provided lifts. Of 168 nurses who had lifts in their workplace, the level of lift availability was high for 59.5%, medium for 25.0%, and low for 13.7%; the level of lift use was high for 32.1%, medium for 31.5%, and low for 31.5%. Significant associations were found between lift availability and work-related low-back and shoulder pain. Compared to nurses without lifts, nurses reporting high-level lift availability were half as likely to have work-related low-back pain (OR = 0.50, 95% CI 0.26-0.96) and nurses reporting medium-level lift availability were 3.6 times less likely to have work-related shoulder pain (OR = 0.28, 95% CI 0.09-0.91). With respect to lift use, work-related shoulder pain was three times less common among nurses reporting medium-level use (OR = 0.33, 95% CI 0.12-0.93); any neck pain was three times more common among nurses reporting low-level use (OR = 3.13, 95% CI 1.19-8.28). Conclusions: Greater availability and use of lifts were associated with less musculoskeletal pain among critical-care nurses. These findings suggest that for lift interventions to be effective, lifts must be readily available when needed and barriers against lift use must be removed. [ABSTRACT FROM AUTHOR]

More than 2.2 million people are incarcerated in the United States, and thousands of these men and women will be brought to medical facilities at some point during their arrest or incarceration. Radiologic technologists across the country conduct imaging procedures for correctional facility patients. Some technologists work within prison infirmaries or in mobile units parked on prison grounds. Others work in hospitals that have contracts with penitentiaries. Whether inside or outside prison walls, imaging of correctional patients requires awareness. The purpose of this study was to provide new insight into the etiology of primarily nonfatal, work-related electrical injuries. We developed a multistage, case-selection algorithm to identify electrical-related injuries from workers' compensation claims and a customized coding taxonomy to identify pre-injury circumstances. Workers' compensation claims routinely collected over a 1-year period from a large U.S. insurance provider were used to identify electrical-related injuries using an algorithm that evaluated: coded injury cause information, nature of injury, 'accident' description, and injury description narratives. Concurrently, a customized coding taxonomy for these narratives was developed to abstract the activity, source, initiating process, mechanism, vector, and voltage. Among the 586,567 reported claims during 2002, electrical-related injuries accounted for 1283 (0.22%) of nonfatal claims and 15 fatalities (1.2% of electrical). Most (72.3%) were male, average age of 36, working in services (33.4%), manufacturing (24.7%), retail trade (17.3%), and construction (7.2%). Body part(s) injured most often were the hands, fingers, or wrist (34.9%); multiple body parts/systems (25.0%); lower/upper arm; elbow; shoulder, and upper extremities (19.2%). The leading activities were conducting manual tasks (55.1%); working with machinery, appliances, or equipment; working with electrical wire; and operating powered or nonpowered hand tools. Primary injury sources were appliances and office equipment (24.4%); wires, cables/cords (18.0%); machines and other equipment (11.8%); fixtures, bulbs, and switches (10.4%); and lightning (4.3%). No vector was identified in 85% of cases. and the work process was initiated by others in less than 1% of cases. Injury narratives provide valuable information to overcome some of the limitations of precoded data, more specially for identifying additional injury cases and in supplementing traditional epidemiologic data for further understanding the etiology of work-related electrical injuries that may lead to further prevention opportunities. Published or unpublished data sets on individual occupational exposure situations are frequently combined and used for some wider purpose, either hazard evaluation or standards setting. This paper describes a model by which such individual data sets for occupational aerosol exposures might be evaluated in terms of their usefulness in this regard. For workplace aerosols, the model is centered around the particle size-selective framework for aerosol exposure assessment that has emerged in recent years as a rational basis for standards setting. In this paper, reported occupational exposures to airborne nickel and lead are used as examples. In a comprehensive review, 106 published peer-reviewed sources of potentially useful exposure data were found for nickel and 111 similar reports for lead. In addition, for lead 116 unpublished reports in the form of hazard evaluation reports from the US National Institute for Occupational Safety and Health were also examined. For both nickel and lead, a wide range of industry sectors was represented, and the data sets cover the period from 1930 to the present day. It was found that such published data sets are highly inconsistent in terms of the criteria by which the data were obtained, notably for the paucity of essential details of the methods that were used. In contrast, for the lead exposures, the unpublished government reports are more consistent, since they usually followed recognized exposure assessment standards. However, the latter may be misleading because they may have tended to represent high exposure situations. The evaluation model described in this paper provides not only a basis for the evaluation of historical exposure data but also guidelines for new exposure assessment to be carried out in the future. Meanwhile, although many of the historical exposure data sets were found to be generally quite weak in terms of their immediate usefulness, attention is drawn to some of their statistical properties that might allow their enhancement for the purposes of hazard evaluation and retrospective exposure assessment exercises. The article discusses the need to provide nurses' safety which otherwise can lead to the threat in providing medical care. Topics include focus on the murder of nurses in August 2014 who attended a workshop in the African kingdom of Lesotho, establishment of initiatives involving the Health Care in Danger project and the Safeguarding Health in Conflict Coalition by the International Council of Nurses, and need to establish a system that monitors attacks on patients and medical professionals.

The article discusses various issues concerning the exposure of hospital pharmacists and oncology nurses to chemotherapy drugs used to treat cancer patients. According to the U.S. Centers for Disease Control and Prevention (CDC), only 56 percent of workers in large cancer centers wear gloves despite risks of infertility, congenital malformations and cancers. Also discussed are the failure to implement guidelines on handling hazardous drugs and the need for nurses to demand standards and surveillance.

Abstract: This study compared death certificate data on usual industry for workplace homicide victims in five urban Texas counties, with medical examiners' data on the industries where victims were working when injured. The overall positive predictive value of the death certificate data was 72 per ¢. Death certificate data on usual industry underestimated the number of victims working in high-risk industries when injured, partly because of victims whose usual industry was recorded as student, housewife, or military personnel. The article presents resolutions concerning occupational health and safety (OHS) in medical care settings in the U.S. passed by the American Public Association. The association, recognizing the needs for evaluation standards of OHS services for employees in all health facilities, recommended that the Joint Commission on the Accreditation of Hospitals and other health accrediting bodies create such standards. It also urged federal and other regulatory agencies to develop standards regarding hazards facing hospital employees, carry out inspection procedures for OHS in hospitals, and to pay for appropriate OHS programs for their employees.

The article focuses on federal regulation related to occupational health and safety in the U.S. The federal Occupational Safety and Health Act of 1970 mandates the Occupational Safety and Health Administration (OSHA) to assure so far as possible every working man and woman in the nation safe and healthy working conditions. Although this objective is certainly laudable, it is too broad and general to offer much guidance to OSHA officials as to how best to allocate the agency's resources. If OSHO seeks to maximize the physical and economic well-being of the labor force, it should strive to minimize the occurrence of work connected disabilities subject to whatever constraints the social costs of prevention impose, in terms of both OSHA's budgetary outlays and the costs associated with OSHA-induced inflation and/or unemployment. Formulating OSHA's guiding principle in this fashion allows for continued emphasis on maximizing its impact on worker safety and health; in addition, it offers the advantage of explicitly recognizing important budgetary and social cost constraints on the agency's actions.

The article focuses on the risks facing doctors caring for patients in the outbreak of Ebola virus disease in west Africa. Topics covered include the impact of poor care practices on the doctors, the death of Liberian doctor Samuel Brisbane and Ugandan doctor Samuel Mutoro from the disease and the warning issued by the World Medical Association on the inadequacy of the working conditions of health professionals.

The article reports on the study conducted by Business & Legal Reports (BLR) regarding the reasons behind employees' injuries in the U.S. Result shows that 40% of respondents are injured at work for less than a year because of lack of knowledge and experience in their field of work. New employees do not understand the importance of using personal protective equipment (PPE) which is one way of protecting itself from injury. Some companies did not communicate properly to employees about the importance of safety. Most often, employers assume that new hirees know more than they really do. Employees do not know enough about hazardous substances in their workplace.
