Background: Currently increasing importance is attributed to the inflammatory process as a crucial factor responsible for the progressive damage to vascular walls and progression of atherosclerosis in obese people. We have studied the relationship between clinical and biochemical parameters and C-peptide and anti-inflammatory IL-10, as well as selected markers of inflammation and endothelial dysfunction such as: CCL2, CRP, sICAM-1, sVCAM-1 and E-selectin in obese women with various degree of glucose metabolism disturbance., Material and Methods: The studied group consisted of 61 obese women, and 20 normal weight, healthy volunteers. Obese patients were spited in subgroups based on the degree of glucose metabolism disorder. Serum samples were analyzed using ELISA kits., Results: Increased concentrations of sICAM-1, sVCAM-1, E-selectin, CCL2 and CRP were found in all obese groups compared to the normal weight subjects. In patients with Type 2 diabetes mellitus (T2DM) parameters characterizing the degree of obesity significantly positively correlated with levels of CRP and CCL2. Significant relationships were found between levels of glucose and sICAM-1and also E-selectin and HOMA-IR. C-peptide levels are positively associated with CCL2, E-selectin, triglycerides levels, and inversely with IL-10 levels in newly diagnosed T2DM group (p<0.05). Concentrations of IL-10 correlated negatively with E-selectin, CCL2, C-peptide levels, and HOMA-IR in T2DM group (p<0.05)., Conclusion: Disturbed lipid and carbohydrate metabolism are manifested by enhanced inflammation and endothelial dysfunction in patients with simply obesity. These disturbances are associates with an increase of adhesion molecules. The results suggest the probable active participation of higher concentrations of C-peptide in the intensification of inflammatory and atherogenic processes in obese patients with type 2 diabetes. In patients with obesity and type 2 diabetes, altered serum concentrations of Il-10 seems to be dependent on the degree of insulin resistance and proinflammatory status.

Current cardiovascular risk scores do not include obesity or fat distribution as independent factors, and may underestimate risk in obese individuals. Assessment of early vascular ageing (EVA) biomarkers including arterial stiffness, central blood pressure, carotid intima-media thickness and flow-mediated vasodilation may help to refine risk assessment in obese individuals in whom traditional cardiovascular risk scores and factors suggest no need for specific medical attention. A number of issues need to be addressed before this approach is ready for translation into routine clinical practice. Methodologies for measurements of vascular markers need to be further standardized and less operator-dependent. The utility of these nontraditional risk factors will also need to be proven in sufficiently large and properly designed interventional studies. Indeed, published studies on vascular markers in obesity and weight loss vary in quality and study design, are sometimes conducted in small populations, use a variety of differing methodologies and study differing vascular beds. Finally, current vascular measurements are still crude and may not be sufficient to cover the different aspects of EVA in obesity.

Obese patients are prone to arterial hypertension, require more antihypertensive medications, and have an increased risk of treatment-resistant arterial hypertension. Obesity-induced neurohumoral activation appears to be involved. The association between obesity and hypertension shows large inter-individual variability, likely through genetic mechanisms. Obesity affects overall cardiovascular and metabolic risk; yet, the relationship between obesity and cardiovascular risk is complex and not sufficiently addressed in clinical guidelines. The epidemiological observation that obesity may be protective in patients with established cardiovascular disease is difficult to translate into clinical experience and practice. Weight loss is often recommended as a means to lower blood pressure. However, current hypertension guidelines do not provide evidence-based guidance on how to institute weight loss. In fact, weight loss influences on blood pressure may be overestimated. Nevertheless, weight loss through bariatric surgery appears to decrease cardiovascular risk in severely obese patients. Eventually, most obese hypertensive patients will require antihypertensive medications. Data from large-scale studies with hard clinical endpoints on antihypertensive medications specifically addressing obese patients are lacking and the morbidity from the growing population of severely obese patients is poorly recognized or addressed. Because of their broad spectrum of beneficial effects, renin-angiotensin system inhibitors are considered to be the most appropriate drugs for antihypertensive treatment of obese patients. Most obese hypertensive patients require two or more antihypertensive drugs. Finally, how to combine weight loss strategies and antihypertensive treatment to achieve an optimal clinical outcome is unresolved.

Introduction: The aim of this study was to evaluate the influence of body mass changes on plasma concentrations of proinflammatory cytokines in obese women after the initially obtained weight reduction in a five-year follow-up period., Material and Methods: Thirty out of 42 women with simple obesity (age 41.8 ± 11.9 years; BMI 36.5 ± 4.6 kg/m2) who achieved a greater than 5% weight loss at the end of a three-month weight loss programme were re-examined after five years. In addition to anthropometric and body composition measurements, plasma concentrations of TNF-alpha, sTNFRs and IL-6 were determined., Results: The mean weight loss after the three-month weight loss programme was 7.9 ± 4.4 kg. After five years, body mass was still lower than initially in 14 women, while in 16 it was higher (the so-called 'yo-yo effect'). A significant decrease of plasma TNF-alpha and IL-6 and increase of sTNFR1 and sTNFR2 levels obtained after weight loss therapy were maintained after five years, including in the subgroup with the yo-yo effect. During the follow-up period, the increase of body fat mass was similar in the subgroup that maintained reduced weight (+4.4 ± 10.7 kg) and in the subgroup with the yo-yo effect (+4.1 ± 7.1 kg), while a significant difference was found in changes of body free fat mass (-7.1 ± 7.1 v. -0.7 ± 4.5 kg, respectively)., Conclusions: The yo-yo effect has a modest influence on systemic microinflammation and seems not to abolish the benefit achieved via a weight loss programme. This may suggest that the persistence of changes in lifestyle implemented during the programme such as regular physical activity and diet composition may have a significant impact on the level of systemic microinflammation in the obese. Introduction: The beneficial effect of obesity on bone mineral density (BMD) has not been definitely established., Objectives: The aim of the study was to evaluate changes in BMD in obese perimenopausal women during a 5-year follow-up., Patients and Methods: The study involved 54 obese women. The group was divided into 2 subgroups according to the menopausal status: postmenopausal women--M (n = 35) and premenopausal women--P (n = 19). Laboratory tests (parathyroid hormone, 25-hydroxyvitamin D₃, C-terminal telopeptide of type I collagen, osteocalcin, and osteoprotegerin), anthropometric measurements, and densitometry were performed twice during the 5-year follow-up. The control group consisted of 19 healthy women of the same age and with normal body weight., Results: Obese postmenopausal women were characterized by lower BMD in the proximal femur and lumbar spine, higher fracture risk, and higher serum osteocalcin levels at baseline. During the 5-year follow-up, there was a 1.52% and 6.86% decrease in proximal femur BMD and 2.34% and 5.17% in lumbar spine BMD (in premenopausal and postmenopausal women, respectively). In postmenopausal controls, BMD reduction was 2.36% and 4.3%, respectively. In the combined analysis including all postmenopausal women, there was an inverse correlation between the initial body mass index and the changes in proximal femur BMD (r = -0.25; P <0.05) and lumbar spine BMD (r = -0.28; P = 0.08) that occurred during the 5-year follow-up., Conclusions: Obesity appears not to protect against bone mineral loss in postmenopausal women.

Background: Experimental studies have shown that tumor necrosis factor-alpha (TNF-α) and interleukin-6 (IL-6) downregulate visfatin gene expression in adipocytes. On the other hand, the induction of cytokine production by visfatin in leucocytes and monocytes has also been described., Aim: To assess the possible interrelation between plasma concentrations of visfatin and TNF-α and TNF soluble receptor in obese women fulfilling, or not, the criteria of metabolic syndrome (MS)., Methods: Ninety two obese women were included in the study. Metabolic syndrome, based on IDF criteria (2005) was diagnosed in 71 subjects (mean age 53 ± 9 years; body mass index 39.1 ± 5.6 kg/m(2), waist circumference 109.6 ± 11.4 cm). The remaining 21 formed the non-MS subgroup (mean age 52 ± 9 years, body mass index 36.3 ± 5.2 kg/m(2), waist circumference 104.7 ± 11.0 cm). Fourteen healthy normal weight women served as controls. In all subjects, body composition was assessed by the bioimpedance method., Results: In the MS subgroup, but not in the non-MS subgroup, visfatin levels were significantly higher than in controls. We did not observe any significant difference in plasma concentrations of visfatin, TNF-α or sTNFRs between the MS subgroup and the non-MS subgroup. Only in the MS subgroup and in the combined analysis of all study subgroups did plasma visfatin concentrations correlate significantly with TNF-α levels (R = 0.31, p = 0.01, R = 0.21, p = 0.03; respectively). Additionally, in the MS subgroup there was a positive correlation between visfatin levels and insulin resistance (R = 0.53, p = 0.01)., Conclusions: Our findings suggest that visfatin in metabolic syndrome should be regarded as a proinflammatory factor indirectly favouring the development of insulin resistance.

The implications of a long-lasting mechanical load on the locomotor activity are poorly understood. The objective of the present studies was to determine an impact of excess body weight on basic spatiotemporal gait measures and to test the hypothesis that leg swing phase may account for a load-related adaptation of the stride characteristics. To this end the basic spatial and temporal stride measures were assessed in 100 obese and 36 lean women (age range between 18 and 67 years) walking with their self-selected pace on a 10-meter long and 1 meter wide instrumented pathway. Among the subjects there were: 44 with class I obesity, 27 with class II obesity, and 29 with class III. Subjects' stance and swing times as well as the stride lengths were recorded by means of contact copper-film electrodes attached to a sole of subject' footwear. The acquired gait measures were used then to compute: a mean velocity of walking, double support times and a mean velocity of a foot during swing phase. Data analysis showed that subjects from every experimental groups walked with a very similar speed (1.08 +/- 0.2 m/s) and cadence (106 +/- 10 steps/min). Their stance time was not affected by body weight and it remained at the mean level of 746 +/- 90 ms for all groups. The temporal stride characteristics and the stance-to-swing ratio were, however, substantially modified in obese individuals due to attenuation of the swing time. As a consequence, the remaining normalized (i.e., expressed as percentage of gait cycle time) phases of stride: the stance and the double support were relatively longer. While the swing time negatively correlated with the body mass index (BMI), the normalized stance and the double support exhibited strong positive correlation (r=0.46) with the BMI. The increase of leg swing velocity seems the main and unique adaptation mechanism that is utilized in the preferred walking gait in obese women.

Obesity is recognised as a global epidemic and the most prevalent metabolic disease world-wide. Specialised obesity services, however, are not widely available in Europe, and obesity care can vary enormously across European regions. The European Association for the Study of Obesity (EASO, www.easo.org) has developed these criteria to form a pan-European network of accredited EASO-Collaborating Centres for Obesity Management (EASO-COMs) in accordance with accepted European and academic guidelines. This network will include university, public and private clinics and will ensure that the obese and overweight patient is managed by a holistic team of specialists and receives comprehensive state-ofthe-art clinical care. Furthermore, the participating centres, under the umbrella of EASO, will work closely for quality control, data collection, and analysis as well as for education and research for the advancement of obesity care and obesity science., (Copyright © 2011 S. Introduction: The protective effect of adipocity on bone metabolism has not been confirmed during long-term follow-up. It is not known whether the rate of bone turnover and changes in mineral metabolism in obese people result from endocrine properties of the adipose tissue or merely the mechanical load., Objectives: The aim of the study was to evaluate bone and calcium-phosphorus metabolism in obese women during a 5-year follow-up., Patients and Methods: The study involved 47 obese women who underwent a 3-month weight loss therapy. We evaluated changes in the serum levels of parathormone (PTH), calcidiol (25(OH)D3), collagen type I crosslinked C-telopeptide (CTx-I), osteocalcin, total calcium, inorganic phosphates, and in bone mineral density. The control group consisted of 17 healthy women with proper body weight., Results: We observed a similar decrease in bone mineral density (BMD) in the lumbar spine and femoral neck, and a comparable decrease in the serum levels of CTx-I and osteocalcin in both groups during the 5-year follow-up. Changes in serum PTH levels were not statistically significant. In obese women, a nonsignificant increase in the serum level of 25(OH)D3 was observed as early as after a 3-month weight loss therapy and during follow-up. In controls, serum 25(OH)D3 levels tended to decrease. During follow-up, the number of obese patients with disturbances in vitamin D metabolism decreased from 78.7% to 53.2% (P = 0.01). Such disturbances were observed in 35.3% of the control group. In obese patients, there was a positive correlation between the change in body mass and BMD in the proximal femur (r = 0.279, P = 0.04). In controls, there was a positive correlation between the change in body mass and BMD in the lumbar spine (r = 0.477, P = 0.05)., Conclusions: In obese women who underwent weight loss therapy, the levels of bone turnover markers decreased and abnormal vitamin D metabolism was still observed during the 5-year follow-up.

Introduction: Recent experimental studies have revealed that increased ghrelin levels protect against depressive symptoms of chronic stress in rats. Moreover it was shown that neuropeptide Y (NPY) has an anxiolytic-like effect mediated through both Y1 and Y5 receptors. On the other hand, peptide YY inhibits, while ghrelin stimulates, the release of NPY. The aim of this study was to assess the association between plasma concentrations of ghrelin and PYY and the level of depression in obese women., Material and Methods: In forty-five obese women (55 +/- 7 years old) without concomitant diseases, body composition was estimated by bioimpedance method, in addition to anthropometric measurements. Beck's Depression Inventory (BDI) was used for the evaluation of depression levels. Plasma concentrations of ghrelin and PYY were measured in collected blood samples., Results: 23 (55.1%) women had severe (22.0 +/- 5.2 pts) and 10 (22.2%) had mild (12.1 +/- 1.9 pts) levels of depression. Obese women with concomitant depression were of similar age and had comparable body mass, BMI, and body composition as those without depression. In addition, there were no differences in plasma ghrelin and PYY levels between the study subgroups. No correlation between BDI and plasma concentrations of ghrelin or PYY was found., Conclusions: Depression in obese women does not seem to be related to the plasma levels of ghrelin and PYY.

Research that evaluated both static and dynamic stability was performed, to clarify the impact of excessive body weight on postural control. The spontaneous center of foot pressure (CP) motion during quiet stance and a range of forward voluntary CP displacements were studied in 100 obese, and 33 lean women. Characteristics of postural sway were acquired while the subjects were standing quiet on a force plate with eyes open (EO) and with eyes closed (EC). Their anterior range of CP voluntary displacements was assessed upon a range of maximal whole body leanings which were directed forward. A substantial reduction of postural sway was observed in all patients which had increased body weight. Main postural sway parameters i.e., the total path length as well as its directional components were negatively correlated with the body mass and body mass index (BMI). The range of a whole body voluntary forward leaning, did not exhibit any significant change in patients with an obesity grade of I and II. Such a deficit was, however, found in subjects with a body mass index above 40. In conclusion, the increased body weight imposed new biomechanical constraints, that resulted in functional adaptation of the control of the erect posture. This functional adaptation was characterized by a reduced postural sway associated with a substantial reduction of the dynamic stability range in subjects with BMI>40.

Obesity is an important public health issue due to its steady increase in the prevalence and the cost of treatment of its complications. Both genetic and environmental factors exert their influence on energy consumption and energy expenditure. The pathogenesis of obesity is complex and not fully understood. One of the mechanisms leading to its development may be disturbances in motor and transport function of the digestive tract. The present paper is a review of the literature concerning connections between function of the digestive tract and the development of obesity.

Adult obesity has been associated with depression, especially in women. Whether depression leads to obesity or obesity causes depression is unclear. Chronic inflammation is observed in obesity and depression. In 63 obese women without additional diseases depression level was assessed with the Beck's questionnaire. After evaluation of depression level study group was divided into groups according to the mood status (A--without depression, B-mild depression, and C--severe depression), and serum concentration of TNF-alpha, sTNFs, leptin, and IL-6 were measured by ELISA. No differences in age, body mass, BMI, and body composition were observed in study groups. We did not observe differences of serum concentrations of TNF-alpha, sTNFRs, leptin, and IL-6 between subgroup A and subgroups B and C. It seems that circulating adipokines did not exert influence on depression levels in obese women.

Introduction: Aim of this study was to check the relation between body image and the effectiveness of weight loss therapy., Material and Methods: Four body image aspects were separated: a declarative, a sensory, an imaginable and a perceived one. Five research methods were used: an objective body weight measurement, the silhouette test, the body mass assessment scale, the body satisfaction scale and a questionnaire. All methods were used: at the beginning of the therapy, after 5 per cent, 10 per cent and 15 per cent reduction in initial body weight., Subjects: 150 overweight women, participants of an outpatient weight loss therapy., Results: The biggest body weight reduction was linked with: an earlier appearance of excess weight (Z=-2.281, P<0.05), bigger planned body mass reduction (Z=-2.472, P<0.05), more overweight declarative self-estimation (Z=-2.136, P<0.05), worse declarative sense of looking nice (Z=-2.126, P<0.05), rare confirmation that once they had looked as an imaginary Self (Z=-1.990, P<0.05), greater general satisfaction with the appearance of their imaginary Self (Z=-2.357, P<0.05), frequent statement that an imaginary Self is attractive (Z=-2.060, P<0.05) and agile (Z=-2.387, P<0.05),worse perceived looking nice (Z=-2.596, P<0.01). A 15% (R2=0.148) reduction in initial body weight was explained with initial values of the position in the silhouette test in a perceptive (Beta=0.256, P<0.01) and an imaginable (Beta=-0.256, P<0.001) body image aspect and the BMI of an imaginary Self (Beta=0.318, P<0.01)., Conclusions: The effectiveness of weight loss therapy is supported by describing and seeing oneself as an obese person as well as imagining oneself as slim but characterized by high body mass index.

Introduction: The aim of the present study was to examine how weight loss treatment modulates plasma concentrations of ghrelin and insulin-like growth factor 1 (IGF-1) in obese women and to determine whether there is any association with possible changes in plasma concentrations of these hormones after weight loss., Material and Methods: The study group consisted of 22 obese women without additional disease (age 40.6 +/- 12.9 years; BMI 37.2 +/- 4.6 kg/m(2)). All subjects participated in a 3-month weight reduction program. The measurements were performed at baseline and after weight loss. Plasma concentration of ghrelin and IGF-1 were measured by enzyme - linked immunosorbent assay (ELISA) kit. Serum concentrations of insulin were measured by radioimmunoassay (RIA). Body composition was determined by bioelectrical impedance analysis using a Bodystat analyser., Results: The mean weight loss was 9.3 +/- 4.1 kg (9.7 +/- 4.3%). Following weight loss, plasma ghrelin and IGF-1 concentrations increased significantly (63.5 +/- 13.0 vs. 72.8 +/- 15.1 pg/ml; p < 0.01; 126.9 +/- 67.0 vs. 170.5 +/- 83.3 ng/ml p < 0.01, respectively) and serum insulin concentrations decreased significantly (17.5 +/- 8.5 vs. 14.8 +/- 10.4 mIU/ml p< 0.05). We observed a significant positive correlation between the increase of ghrelin and decrease of body fat percentage after weight loss (r = 0.44, p = 0.03). There are no correlations between change of ghrelin and IGF-1concentrations and between changes of insulin and IGF 1 concentrations., Conclusion: Plasma concentrations of ghrelin and IGF-1 increased after weight loss. However, it seems there is no association between serum concentrations of ghrelin and IGF-1 in obese women.

Obesity is associated with an increased risk of cardiovascular disorders. The aim of the present study was to compare the indexes of arterial structure and function in women with simple obesity and healthy individuals. Twenty-two women with simple obesity (body mass index [BMI]: 33.6 +/- 2.9 kg/m(2), age: 29.7 +/- 6.2 years), and 34 healthy women were included in the study. Healthy subjects were divided into two subgroups according to their age (<35 and >45 years): Control A-16 young women (age <35 years, BMI: 24.0 +/- 3.0 kg/m(2)), and Control B-18 older women (age >45 years, BMI: 25.8 +/- 2.9 kg/m(2)). Noninvasive, high-resolution, vascular ultrasound was used to evaluate the endothelial-dependent vasodilatation: flow-mediated dilatation of brachial artery (FMD); the arterial structure: intima-media thickness (IMT) of common carotid artery (CCA); and the compliance parameters corresponding to structural changes in large arteries (PWV: pulse wave velocity; PP: pulse pressure; TAC: total arterial compliance; Ao C: aorta compliance, CCA C: CCA compliance, stiffness indexes). Endothelial-dependent vasodilatation as represented by FMD was comparable in the obese group (16.8% +/- 7.9%; median: 15.5%) and healthy subjects (Control A: 14.1% +/- 4.7%; median: 13.6%; Control B: 13.9% +/- 6.5%; median: 13.0%). The mean value of IMT was significantly increased (P < 0.05) in Control B group (0.67 +/- 0.07 mm) in comparison to both obese patients (0.58 +/- 0.09 mm) and Control A group (0.53 +/- 0.05 mm). The compliance parameters (PWV, AoC, CCA C, stiffness indexes) were impaired in obese patients and Control B patients as compared to Control A individuals. PWV and stiffness indexes were significantly increased, and the AoC, CCA C-diameter, CCA C-area were significantly decreased. Simple obesity constitutes an important risk factor accelerating arterial stiffness in women.

Studies of the role of immune system activation in the pathogenesis of obesity and its concomitant diseases have been conducted for some years. Numerous recent studies revealed an association between increased immune activation in obesity and the development of insulin resistance. On the other hand there is the hypothesis that immune activation in obesity is a homeostatic mechanism to protect the organism from reaching the point at which the over-accumulation of fat decreases the possibility to move. The aim of the present study was to review the current literature on immune activation in obesity and the participation of adipokines produced by adipose tissue in the development of insulin resistance. Attention is drawn to the similarities in function and gene expression of adipocytes and macrophages.

Both bone and adipose tissue change their size, shape and distribution during the whole human being's life. Many factors, including genetic factors, hormones and activity of nervous system are responsible for these changes. It is generally accepted that obesity has a protective effect on bone tissue. On the other hand some authors present an opposite results--the lack of beneficial effect of obesity on development of osteoporosis fractures. The aim of this article was to present and discuss the relations between adipose tissue and bone metabolism.

Background: In animal models ghrelin reduces cardiac afterload and increases cardiac output via receptors in the cardiovascular system. The aim of our study was to evaluate a potential relationship between weight loss treatment, blood pressure and serum ghrelin concentrations in obese women., Material and Methods: A group of 37 obese premenopausal women with no previous history of hypertension (BMI: 36.5 +/- 5 kg/m2) were involved in the study. Blood pressure and serum ghrelin levels were assessed before and after a three-month weight reduction treatment, which consisted of a diet of 1000 kcal/day and physical exercise. Body composition was determined by impedance analysis using Bodystat., Results: Following weight loss (mean 8.9 +/- 4.8 kg) SBP decreased (120 +/- 13 vs. 115 +/- 14 mm Hg, p = 0.01) and serum ghrelin levels increased significantly (66.9 +/- 13.7 vs. 73.9 +/- 15.4 pg/ml; p = 0.005). There were significant correlations between values for ghrelin levels after weight loss and SBP (r = -0.45, p = 0.02), DBP (r = -0.41, p < 0.05), and between Deltaghrelin levels and DeltaSBP (r = 0.52, p = 0.006), DeltaDBP (r = 0.53, p = 0.005). There was a positive correlation between an increase in ghrelin and a decrease in percentage body fat during weight loss (r = 0.51; p = 0.002)., Conclusion: The results seem to provide evidence that weight loss may decrease blood pressure in obese patients via a ghrelin-dependent mechanism.

Introduction: Apoptosis can influence both adipose tissue mass and its distribution. The suprafamily of tumour necrosis factor (TNF) receptors stimulate apoptosis. The aim of the study was to assess serum concentrations of tumour necrosis factor alpha (TNF-alpha), TNF soluble receptors (sTNFRs) and FAS in obese subjects and to examine the changes in these parameters after weight loss., Material and Methods: The study group consisted of 23 obese women without additional disease aged 36.6 +/- 10.9 years. These were examined before and after three-month weight reduction treatment consisting of a diet of 1000 kcal/day and physical exercise. The control group comprised 17 lean healthy women aged 40.3 +/- 5.5 years. Blood samples were taken in the morning after an overnight fast. Serum concentrations of TNF-alpha, sTNFRs and FAS were measured by enzyme linked immunosorbent assay (ELISA). Serum concentrations of insulin were measured by RIA. Serum concentrations of glucose, total cholesterol, HDL cholesterol and triglycerides were measured by an enzymatic procedure., Results: The mean weight loss over the three-month treatment was 11.4 +/- 3.1 kg. Following weight loss, serum TNF-alpha concentrations decreased significantly (7.3 +/- 3.0 vs. 5.4 +/- 1.6 pg/ml; p < 0.005) and concentrations of sTNFRs increased significantly (1222.6 +/- 211.8 vs. 1325.6 +/- 261.6 pg/ml; p < 0.05 and 1881.5 +/- 337.2 vs. 2057.4 +/- 358.7 pg/ml; p < 0.05 respectively). However, no changes in serum concentrations of FAS were observed after weight loss., Conclusion: We observed increased serum concentrations of TNF-alpha but not of FAS in obese women. The concentrations of TNF decreased and those of sTNFRs increased after weight loss. However, the weight reduction therapy did not change serum concentrations of FAS.

Background: Dairy products not only reduce the risk of hypertension and cardiovascular diseases but may play a role in the treatment of obesity. As there is some evidence that calcium (Ca) and vitamin D may play a role in effective weight management, we decided to evaluate the influence of Ca and vitamin D supplementation on weight and fat loss in obese women., Material and Methods: Forty obese women were enrolled in this study. Subjects were divided into 2 groups comparable with body mass index (BMI) and age. Group 1 was provided with calcium carbonate and 1-(OH)-vitamin D supplementation. Group 2 was provided with only a diet. Subjects participated in a 3-month weight reduction therapy (balanced diet, modification of life style, and regular physical exercise). Blood samples (serum concentration of Ca, phosphorus (P), parathormone (PTH), 25-(OH)-D3) and clinical characteristics (weight, height, BMI, body composition) were taken at baseline and after the 3-month program., Results: No significant differences of body weight, body fat content, serum parathormone, 25-(OH)-D3 concentration, and plasma total Ca and P concentration were observed between analyzed groups both before and after the treatment. Additionally, we did not observe any significant influence of Ca and vitamin D supplementation on weight and fat loss., Conclusion: Ca plus vitamin D supplementation during a 3-month low caloric diet has no additional effect on weight and fat loss in obese women.

Introduction: The aim of the present study was to evaluate the effect of modest weight reduction on serum concentrations of tumour necrosis factor alpha (TNF-alpha), TNF soluble receptors (sTNFRs) and interleukin-6 (IL-6) in obese women with polycystic ovary syndrome (PCOS)., Material and Methods: The study group consisted of 15 obese women with PCOS (mean age 28.5 +/- 7.7 years). Serum concentrations of TNF-alpha, sTNFRs and IL-6, insulin, FSH, LH, DHEAS, androstendione, total and free testosterone, cortisol, 17OH-progesterone, oestradiol and sex hormone binding globulin (SHBG), glucose, total cholesterol, HDL cholesterol and triglycerides were measured before treatment and after 10% weight loss. All patients were advised to follow a 1000-1200 kcal diet with a limited intake of simple carbohydrate and animal fats and to exercise regularly (30 min, 3 times a week). Body composition was measured by bioimpedance. Serum concentrations of TNF-alpha, sTNFRs and IL-6 were determined by enzyme linked immunosorbent assay (ELISA). Plasma insulin, FSH, LH, DHEAS, androstendione, total and free testosterone, cortisol, 17OH-progesterone, oestradiol and SHBG were measured by a commercial RIA. Blood glucose, total cholesterol, HDL cholesterol and triglycerides were measured by an enzymatic procedure., Results: We observed no differences in serum concentrations of TNF-alpha, sTNFRs or IL-6 after treatment., Conclusions: It seems that more than a modest weight reduction is necessary to obtain a decrease in serum concentrations of proinflammatory cytokines and an improvement in ovarian function in obese women with polycystic ovary syndrome.

The development of consensus guidelines for obesity is complex. It involves recommending both treatment interventions and interventions related to screening and prevention. With so many publications and claims, and with the awareness that success for the individual is short-lived, many find it difficult to know what action is appropriate in the management of obesity. Furthermore, the significant variation in existing service provision both within countries as well as across the regions of Europe makes a standardised approach, even if evidence-based, difficult to implement. In formulating these guidelines, we have attempted to use an evidence-based approach while allowing flexibility for the practicing clinician in domains where evidence is currently lacking and ensuring that in treatment there is recognition of clinical judgment and of regional diversity as well as the necessity of an agreed approach by the individual and family. We conclude that i) physicians have a responsibility to recognise obesity as a disease and help obese patients with appropriate prevention and treatment, ii) treatment should be based on good clinical care and evidence-based interventions and iii) obesity treatment should focus on realistic goals and lifelong management., (Copyright 2008 S. Background: We assessed the impact of a prolonged lipase inhibition upon gastric emptying (GE) and orocecal transit time (OCTT) of a 355-kcal low-fat solid meal., Methods: In double-blind manner, 40 obese women BMI > 30 kg/m2, randomly allocated into two equal groups, took orally t.i.d. 120 mg orlistat or placebo during 8 weeks of a weight-reducing management. At randomization and after 2 months, GE was measured simultaneously with OCTT by means of a 13C-octanoic acid and a hydrogen breath test, respectively. Lipolytic activity was evaluated with a 13C-mixed triglyceride breath test (13C-MTGBT)., Results: A profound lipase inhibition by orlistat was confirmed by a 79.5% +/- 16.9% reduction of the cumulative 6-h 13C recovery with 13CMTGBT. GE remained unchanged either in the orlistat (T1/2, 188 +/- 35 min start versus 198 +/- 36 min end) or the placebo (T1/2, 191 +/- 35 min start versus 180 +/- 39 min end) group. OCTT increased from 208 +/- 54 min to 271 +/- 64 min (P < 0.01) after orlistat treatment and did not change significantly (216 +/- 76 vs. 234 +/- 72 min) in the placebo group., Conclusions: No adverse effect on the GE and a moderate prolongation of the OCTT of a low-fat solid meal is to be expected under a prolonged treatment with orlistat at a typical dosage regimen.

Introduction: The aim of the present study was to examine the response of plasma orexin and ghrelin to the oral glucose tolerance test (OGTT) in obese women without additional disease., Material and Methods: The study group comprised 15 obese women aged 30.4+/-9.7 years of mean BMI 34.7+/-3.8 kg/m(2). The measurements were performed after an overnight fast and 30, 60 and 120 minutes after the oral administration of 75 grams of glucose. Serum concentrations of ghrelin and orexin A were measured by an enzyme-linked immunosorbent assay (ELISA) kit. Serum concentrations of insulin were measured by radioimmunoassay (RIA). Plasma glucose was determined by an enzymatic procedure. Body composition was determined by impedance analysis using Bodystat., Results: We observed no significant differences between serum concentrations of ghrelin and orexin during OGTT. No correlations were found between serum ghrelin and orexin concentrations and serum insulin and glucose concentrations in any of the measurements., Conclusion: Oral glucose administration did not change serum concentrations of ghrelin and orexin A in obese women without additional disease.

The aim of the present study was to determine serum concentrations of visfatin in obese women in comparison to normal-weight controls. Study subjects were 21 obese women without additional disease (age, 29.0+/-4.9 years; body mass index, 37.1+/-6.1 kg/m2) and 16 healthy, normal-weight women (age, 29.9+/-5.4 years; body mass index, 22.5+/-1.7 kg/m2). Body composition was measured by bioimpedance. Serum concentrations of visfatin were assayed with an enzyme-linked immunosorbent assay kit (Phoenix Pharmaceuticals, Burlingame, CA). Insulin was determined by radioimmunoassay and glucose by colorimetric method. Serum concentration of visfatin was significantly higher in obese women when compared to controls. Positive correlations between serum concentrations of visfatin and insulin in the obese group were found. In the control group, we observed positive correlations between serum concentrations of visfatin and glucose. In conclusion, the observed increase of visfatin in obesity may be a counterregulation preventing further glucose increase.

Objective: To assess the influence of weight reduction therapy on serum osteoprotegerin (OPG) concentration in obese patients and compare these results with normal-weight controls., Research Methods and Procedures: Forty-three obese women (BMI, 36.7 +/- 4.1 kg/m2; mean age, 50.1 +/- 4.5 years) were studied. The control group consisted of 19 normal-weight women (BMI, 24.2 +/- 2.1 kg/m2; mean age, 53.8 +/- 5.2 years). In all patients, serum concentrations of OPG, C telopeptide of type I collagen containing the cross-linking site (CTX), osteocalcin, parathormone, 25-(OH)-D3 (vitamin D), and total calcium and phosphorus were assessed before and after a 3-month weight reduction therapy., Results: In obese subjects, serum concentrations of OPG, 25-(OH)-D3, osteocalcin, total calcium, and phosphorus were significantly lower, and serum concentration of parathormone was significantly higher, before weight reduction therapy in comparison with normal-weight controls. After weight reduction, a significantly higher serum concentration of 25-(OH)-D3 and CTX and significantly lower concentration of OPG were found., Discussion: Serum concentration of OPG was significantly lower in obese patients in comparison with normal-weight controls. Weight reduction therapy resulted in further decrease in OPG serum concentrations. Therefore, OPG cannot be treated as a protective factor from bone loss in obese patients.

Introduction: Assessment of serum osteoprotegerin (OPG) concentrations in obese patients in comparison to healthy controls and evaluation of a possible correlation between OPG and other markers of bone turnover or calcitropic hormones., Material and Methods: 50 obese perimenopausal women without concomitant diseases (BMI 36.7 +/- 4.1 kg/m(2), mean age 50.4 +/- 4.9 yrs). The control group consisted of 19 healthy women (BMI 24.2 +/- 2.1 kg/m(2); mean age 53.8 +/- 5.1 yrs). In all patients serum concentration of OPG, C telopeptide of type I collagen containing the crosslinking site (CTX), osteocalcin, parathormone (PTH) and vitamin D (25-OH-D(3)) was assessed. Dual energy x-ray absorptiometry (the DXA method) of the lumbar spine and femoral neck was performed using a Lunar DPXL to measure bone marrow density (BMD)., Results: In obese perimenopausal women serum OPG, osteocalcin and 25-OH-D(3) levels were significantly lower, and the serum PTH level was significantly higher in comparison to healthy controls. A significantly positive correlation was found between serum OPG level and age in both obese and control subjects., Conclusion: The serum OPG level in obese perimenopausal women is significantly lower in comparison to healthy controls and does not correlate significantly with biochemical markers of bone turnover, calcitropic hormones and BMD. It probably cannot play a protective role in the pathogenesis of bone loss in obese perimenopausal women.

The aim of this study was to investigate the possible role of resistin in obese women with and without insulin resistance. We compared serum concentrations of resistin with interleukin 6 (IL-6), tumor necrosis factor alpha (TNF-alpha), soluble TNF receptors 1 and 2, and certain anthropometric and metabolic parameters in 26 obese women (body mass index [BMI], 35.8 +/- 4.12 kg/m2) and 15 healthy control women (BMI, 22.32 +/- 1.89 kg/m2). Fasting serum resistin and inflammatory cytokine levels were measured by enzyme immunoassay. Insulin resistance was measured by the homeostasis model assessment of insulin resistance (HOMA-R) formula. Compared with lean controls, obese women showed higher HOMA-R values and levels of insulin and increased values of TNF-alpha, soluble TNF receptors, and IL-6. There was no significant difference in resistin levels between the investigated groups of obese women and lean subjects. The results showed that serum resistin concentrations did not correlate with BMI, HOMA, fasting plasma glucose level, or fasting plasma insulin level. Serum resistin correlated with fat mass and IL-6 in the group with impaired glucose tolerance (obese group) (r = 0.51, P < .05, and r = 0.37, P < .05, respectively) and with low-density lipoprotein cholesterol (r = -0.39, P < .05) in the same group. The groups we examined are relatively small; it is likely that with a larger number of subjects, the correlation in other obese women groups may achieve statistical significance. It seems that resistin may be linked with inflammation and obesity and, indirectly, with insulin resistance.

Introduction: The aims of the present study were to evaluate the effect of weight-loss treatment on serum concentrations of NO and TNF-alpha and to examine whether there is an association between TNF-system activity and serum concentrations of NO after weight loss., Material and Methods: The study group involved 43 obese women (aged 41.8 +/- 11.9 years, weight 95.2 +/- 15.0 kg, BMI 36.5 +/- 4.6 kg/m(2)). The women were subjected to three-month complex weight-loss treatment. Patients were advised to keep to a 1000-1200 kcal diet and to exercise regularly. Pharmacological treatment was not administered. Serum concentrations of nitric oxide metabolites, TNF-alpha and its soluble receptors (sTNFR1, sTNFR2) were measured by ELISA kits; insulin was measured by RIA and glucose, cholesterol, HDL cholesterol and triglicerydes by an enzymatic procedure before and after weight loss. Body composition was determined by impedance analysis using Bodystat., Results: The mean weight loss during treatment was 8.3 +/- 4.3 kg. The serum concentrations of TNF-alpha decreased significantly (p < 0.000) and both receptors sTNFR1 and sTNFR2 increased significantly (p < 0.000) after weight loss. No significant changes in serum concentrations of NO were observed after weight loss. A multiple regression analysis was performed using DeltaTNF-alpha, DeltasTNFR1, DeltaTNFR2 and DeltaNO as dependent variables. A significant correlation was observed between DNO and initial plasma concentrations of TNF-alpha, sTNFR1 and sTNFR2., Conclusions: This study demonstrates a decrease in serum TNF-alpha concentration as well as an increase in plasma concentration of both TNF receptors but does not show any change in serum concentrations of NO after weight-loss treatment in obese women. It seems that changes in TNF-system activity may be a counter-regulating mechanism, which inhibits further body mass loss. We did not observe any association between changes in TNF-system activity and serum concentrations of NO after weight loss.

Introduction: Interleukine-6 (IL-6) is one of the cytokines, excreting by adipocytes, which increases in obesity. These cytokines participate in very complicated mechanisms of developing insulin resistance that accompany obesity. The aim of the study was to: 1) evaluate the influence of weight loss on insulin resistance and serum concentration of IL-6, 2) evaluate the hypothetical association between serum concentration of IL-6 and the improvement of insulin sensitivity in obese women after weight loss., Material and Methods: The study involved 27 obese women (age 40.3 +/- 11.1 year; BMI 37.4 +/- 5.2 kg/m(2)) with insulin resistance diagnosed using HOMA index, without concomitant diseases and without any medication. All the patients participated in complex weight reduction treatment (diet, physical activity and psychotherapy). Before and after weight reduction therapy weight and height were measured, body composition was determined using bioimpedance analysis. Serum concentration of glucose was determined by enzymatic procedure, serum concentration of insulin was measured by radioimmunoassay, serum concentration of IL-6 was measured by ELISA. HOMA index was calculated with formula., Results: The mean weight loss after 3-month was 9.2 +/- 4.5 kg (approximately 10% of initial weight). After weight reduction significant decreases in HOMA index, insulin and IL-6 concentrations was observed. However, no correlations between changes in insulin concentrations, HOMA index and decrease of IL-6 concentration were showed. We observed significant correlations between DeltaHOMA and DeltaBMI (r = 0.48; p = 0.012) and Delta percentage fat mass (r = 0.39; p < 0.05)., Conclusions: A moderate weight loss improves insulin sensitivity and decreases serum concentrations of IL-6. However improvement of insulin sensitivity is the effect of fat mass reduction and does not change serum concentration of IL-6.

Unlabelled: Characteristic for an individual and a relatively constant level of anxiety and internal or external locus of control are personality dimensions that influence the biopsychosocial functioning of an individual and are reflected in his experience and behaviour., Aim: The aim of this study was to analyse the level of anxiety in women deciding to start a weight loss programme and to find the relationship between the locus of control and the intensity of this anxiety., Method: Sixty seven women participating in a group-based, interdisciplinary educational weight loss programme were examined using the Locus of Control Questionnaire (Drwal) and the State and Trait Anxiety Inventory (Spielberger, Strelau, Tysarczyk & Wrześniewski). The average age of the subjects was 42.88 +/- 10.96 and the average body mass index was 35.98 +/- 5.66., Results: The level of Anxiety as a State averaged at 5 +/- 2, the level of Anxiety as a Trait averaged at 5 +/- 1.55 and the result in the Locus of Control Questionnaire averaged 7 +/- 1.57. There was a correlation between: age and BMI, (r=0.33; p<0.05), age and state of anxiety (r=0.25; p<0.01), state of anxiety and trait of anxiety (r=0.60; p<0.05), state of anxiety and locus of control (r= 0.41; p<0.05), trait of anxiety and locus of control (r=0.35; p<0.05). In the group of women with lowest BMI their age was also significantly lower than in the other groups. There were no other differences correlating with age across the examined variables., Conclusions: The level of Anxiety (as a State and as a Trait) in women deciding to start a weight loss programme did not differ from the general population. 1. The results in the Locus of Control Questionnaire were higher than the general population--that means a more external locus of control. 2. There was no correlation between BMI and the level of anxiety or the locus of control. 3. There was a correlation between the locus of control and the anxiety level (state and trait).

The Aim: of the present study was to evaluate serum concentrations of adrenal and ovarian androgens and sex hormone-binding globulin in obese women without additional diseases and in obese women with polycystic ovary syndrome with and without insulin resistance., Material and Methods: The study group involved 73 obese women (39 with PCOS--A and 34 obese without additional diseases--B). The serum concentration of glucose and insulin were measured and the study group was divided on the basis of HOMA index into two subgroups: A I-PCO without insulin resistance (n=18, mean age 27.2+/-5.9 yr; BMI 33.2+/-3.5 kg/m2); AII-PCO with insulin resistance (n=21, mean age 27.5+/-7.1 yr; BMI 37.6+/-6.5 kg/m2); B I-obese without insulin resistance (n=8, age 33.5+/-7.5 yr; BMI 35.2+/-4.8 kg/m2); B II-obese with insulin resistance (n=24, age 30.3+/-5.2 yr; BMI 36.4+/-5.8 kg/m2). Body mass and height were measured and body mass index was calculated with formula. Body composition was measured using bioimpedance method. The serum concentrations of FSH, LH, total and free testosterone, androstendione, DHEAS, SHBG and insulin were determined by RIA method and glucose was determined by enzymatic procedure., Results: We observed significantly higher body mass, fat mass and BMI in AII subgroup when compared to AI, BI and BII subgroups. Only serum concentration of free testosterone was significantly higher in AII subgroup when compared to AI subgroup. We observed a positive correlation between serum concentrations of insulin and free testosterone in both groups A and B, moreover we observed positive correlations between serum concentrations of insulin and both DHEAS and LH in group B., Conclusions: It seems that insulin resistance plays a key role in the development of hyperandrogenism in obese women. However mechanisms leading to hyperandrogenism in PCOS are still unrevealed and seem to be more complex.

Unlabelled: The aim of the study was to verify, in epidemiological setting, the hypothesis that at a given body mass index the contents of adipose tissue (AT%) varies with age and is larger in older people., Material and Methods: To verify this assumption population-based data were used. Measurements were obtained in 3359 men and women, aged 30-72 and living in two large towns of Upper Silesian Industrial Zone (Poland). During home visits each subject was weighted, their posture was measured and AT% was obtained using bioelectrical impedance method. According to body mass index (BMI in kg/m2) the subjects were divided into eight groups (I - BMI: 20.0-22.5; II - BMI: 22.6-25.0 ... VII - BMI: 35.0-37.5; VIII - BMI: 37.6-40.0). Within each BMI group four classes of ages were identified: 30-39 years; 40-49 years; 50-59 years; 60-72 years., Results: Mean values of AT% increased with age in each BMI group, both in men and women, and the within-BMI group differences were statistically significant (ANOVA). The effect was consistent, however it was more apparent at low BMI values. For consecutive BMI groups (I-VII) the AT% mean values in extreme age groups (30-39 years and 60-72 years) were as follows: I: 21.3+/-5.9% and 28.1+/-7.1%; Il: 22.5+/-6.4% and 29.7+/-6.7%; III: 25.8+/-6.2% and 32.6+/-6.2%; IV: 28.1+/-6.5% and 35.1+/-5.9%; V: 32.3+/-5.8% and 38.4+/-5.6%; VI: 37.2+/-6.5% and 40.9+/-5.5%; VII: 39.2+/-3.9% and 43.5+/-5.7%; VIII: 40.0+/-7.2% and 46.3+/-3.4%, respectively. In subjects with BMI < 25.1 kg/m2 the difference in AT% between the youngest and the oldest group reached seven points, with a relative difference of 30%., Conclusions: The findings confirmed that BMI value is not a perfect surrogate in the assessment of adipose tissue contents and this fact should be taken into account in epidemiological investigations into risk factors of obesity and its health consequences.

Unlabelled: Epidemiological studies suggest a protective influence of obesity against postmenopausal bone loss. Lower risk of osteoporotic fractures was described in obese patients. However there were only a few studies which examined the effect of weight reduction on bone metabolism and results of these studies are controversial. The aim of the study was to evaluate the influence of weight reduction program using Orlistat on bone metabolism in perimenopausal women. Twenty obese women with simple obesity and without concomitant diseases (BMI 37.1 +/- 3.0 kg/m2, mean age 49.8 +/- 4.6 yrs) were enrolled into this study. The control group consisted of 20 healthy women (mean age 53.5 +/- 5.4 yrs, BMI 24.1 +/- 2.2 kg/m2). All patients have participated in a 3-month weight reduction therapy that consisted of: a 1000-1200 kcal/ day balanced diet (daily calcium consumption about 500mg), Orlistat 3 x 120mg a day and regular physical exercises. Before the weight reduction therapy and after 10% reduction of body weight, serum concentrations of PTH, 25-(OH)-D3, total calcium and phosphorus, total cholesterol were assessed. Dual energy x-ray absorptiometry (DEXA method) of lumbar spine and femoral neck, measuring BMD was performed once, after a 3-month weight reduction therapy using Lunar DPXL. All these measurements were performed only once in control subjects. After a 3-month weight reduction program in patients treated with Orlistat the mean weight loss was 11.6 +/- 5.1 kg which is 12.1 +/- 4.78 %. BMI decreased from 37.1 +/- 3.0 kg/m2 at baseline to 32.6 +/- 2.7 kg/m2 post-treatment. The body weight reduction resulted in significant decrease of body fat and total cholesterol concentration. In obese subjects serum concentration of 25-(OH)-D3 was significantly lower and serum concentration of PTH was significantly higher in comparison to healthy controls, both before and after weight reduction therapy. Serum concentration of PTH, 25-(OH)-D3, total calcium and phosphorus did not change significantly after therapy with Orlistat., Conclusion: 3-month weight reduction program using Orlistat did not influence significantly bone metabolism.

Introduction: TNF-alpha is one with mediators insulin resistance. Previous study showed, that in obesity there is an increased synthesis of TNF-alpha by fat cells and serum concentrations of TNF-alpha. The aim of present study was: 1. To assess of serum concentrations of TNF-alpha and TNF soluble receptors sTNFRs in obese women with diabetes type 2 and obese women without additional disease. 2. To assess possible association between of manner treatment of diabetes type 2 and serum concentrations of TNF-alpha and TNF soluble receptors., Material and Methods: The study group's involved 23 obese women with diabetes type 2 - group A (age 63.6 +/- 8.2 lat; BMI 32.7 +/- 3,9 kg/m2) in this 12 treated of derivatives of sulfonylurea (age 65.1 +/- 6.6 lat; BMI 32.0 +/- 3.4 kg/m2) - subgroup AI and 11 insulin treated (age 62.1 +/- 9.7 lat; BMI 33.4 +/- 4.4 kg/m2) - subgroup AII and 23 obese women without additional disease and without any pharmacological treatment - group B (age 36.6 +/- 10.9 lat; BMI 36.6 +/- 5.6 kg/m2). Body weight and height were measured, body mass index was calculated with formula. Serum concentrations of glucose was measured by enzymatic procedure. Serum concentrations of TNF-alpha and it's soluble receptors sTNFR1 and sTNFR2 was measured by ELISA. and sTNFR2 were significant decreased (respectively p <0,005 i p <0,001) in group A when compared to group B. There are not significant differences serum concentration of TNF-alpha and its soluble receptors between subgroups AI and AII., Conclusions: 1. In obese women with diabetes type 2 serum concentration of TNF-alpha increased and concentrations of its soluble receptors decreased when compared to obese without additional disease. 2. The treatment meaner of diabetes type 2 not influence of serum concentration of TNF-alpha and sTNFR1 but application of insulin maybe a cause increase activity sTNFR2.

Adipose tissue generates many bioactive substances, which may exert autocrine, paracrine and endocrine effects. Recent studies have revealed, that one of these substances is nitric oxide. There are observed increased expressions of synthases NO (eNOS and iNOS) in adipose tissue in obesity. It seems that these increased expression NOS is reflected by increased serum concentration of NO in obese subjects. The review of the current literature on role of nitric oxide in physiology and pathology metabolism is presented in this paper.

Unlabelled: In common beliefs the care of obese people for their health and appearance is not sufficient, which is both the cause and the result of their obesity. The aim of the present study was to check whether obese and normal weight persons are different in selected behaviours connected with eating and self-care, and also whether providing them with knowledge about the desired behaviour changes would improve the effects of weight loss. Authors used structured interview of 20 limited questions. The participants were 32 obese women taking part in group weight loss programme organized in "Waga" treatment center in Katowice. The measures were taken before and after the programme. The comparing group constituted women of normal body weight. Obese women before the treatment comparing to normal weight women, presented more disadvantageous behaviours in the range of: fast eating and reaching for the food in time of strain (p < 0.01). As a results of the weight loss programme they achieved a significant weight loss (p < 0.01) and also the frequency of their unprofitable behaviours decreased except for putting other's needs in front of their own, skipping breakfast and eating daily no more than two meals. Statistically significant improvements were observed in: greater physical activity, greater care for appearance and meals aesthetics (p < 0.01) as well as better ability to relax and profitable lengthen of meal time (p < 0.05). After the treatment patients declared more beneficial behaviours than the normal weight group. There were statistically significant differences in: ability to relax and avoiding to combine wrong meal ingredients (p < 0.01). Obese reached for the food in time of strain still more often, but less often than at the beginning of the treatment., Conclusions: The results did not show significant difference between obese and normal weight participants concerning eating habits and self-care behaviour. The exception was that obese women reached for food more often in the situations of strain and stress. The weight loss of the participants cannot be attributed solely to the change in their behaviour, but could be influenced also by group support and/or by emotional, cognitive and social changes. It might serve as a starting point for further investigations. Researching the long term effects of the treatment (how permanent is the alteration in the participants' weight and self-care behaviour) seems to be essential to answer those questions.

The aims of the present study was to examine how overweight and obesity affect serum concentrations nitric oxide (NO) metabolites and to determine whether there is association between serum concentrations tumor necrosis factor (TNF)-alpha and TNF soluble receptors (sTNF-R) in subjects with overweight and obesity. The study groups involved 154 women: 102 obese (81 obese with body mass index [BMI] 30 to 40 kg/m2 and 21 obese with BMI > 40 kg/m2), 24 overweight patients, and 28 lean controls. Serum concentrations of NO metabolites and of TNF-alpha and its soluble receptors (sTNF-R1, sTNFR-2) were measured by enzyme-linked immunosorbent assay (ELISA) kits. Serum concentration of insulin was measured by radioimmunoassay (RIA). Plasma glucose, cholesterol, high-density lipoprotein (HDL)-cholesterol, and triglicerydes were determined by enzymatic procedure. Body composition was determined by impedance analysis using Bodystat (Douglas, British Isles). Serum concentrations of NO in the overweight group (35.1 +/- 12.1 micromol/L) and the obese groups with BMI 30 to 40 kg/m2 (32.8 +/- 9.3 micromol/L) and with BMI greater than 40 kg/m2 (33.3 +/- 8.5 micromol/L) were significantly higher when compared to controls (28.2 +/- 8.1 micromol/L): P < .05; P < .01, and P < .01, respectively. There was no difference in levels of NO between the overweight group and both obese groups. Serum concentration of TNF-alpha was also significantly higher in the group with overweight (6.5 +/- 3.1 pg/mL), in the obese group with BMI 30 to 40 kg/m2 (6.8 +/- 3.1 pg/mL), and in the obese group with BMI greater than 40 kg/m2 (7.4 +/- 2.6 pg/mL) when compared to controls (2.9 +/- 2.2 pg/mL): P < .00005; P < .00005, and P < .0000001, respectively. However, serum concentrations of sTNF-R1 and -R2 did not differ significantly between the overweight group, both obese groups, and controls. In conclusion, we observed increased serum concentrations of TNF-alpha and NO in overweight and obese women. It seems that there is an association between serum concentrations of TNF-alpha and NO; however, this relationship depends on the degree of obesity.

Obesity is considered important factor contributing development of PCOS. Insulin resistance, low level of serum SHBG and activation of pituitary-adrenal axis plays a major role in pathogenesis and symptoms of PCOS. The achievement of weight loss is an important factor in treatment of PCOS, but difficulties with adherences to the dietary regiment discourage physicians to use this method. In our paper we would like to show the effectiveness of such treatment and pay attention to possibilities of drug facilities in dietary approach in treatment of PCOS coexisting with obesity.

Psychological factors may contribute in creating and maintaining of overweight that often results of disturbances of individual's psychosocial functioning. That is why psychological impact should be a significant element of both obesity treatment and prevention of recurrence. The article presents a project implemented in the Obesity Treatment Centre "Waga" in Katowice. The project acts as a part of a group, complex treatment process leading to weight reduction. Design, target group and the achieved results are presented as well a psychological aspects helping the process of reduction of body weight.

Unlabelled: There are conflicting information about differences of gastric emptying rate between lean and obese subjects and lack of information concerning antral motility in both groups. It has been suggested that obesity is associated with an altered rate of gastric emptying, but the results of earlier studies examining gastric emptying rates in obesity have proved inconsistent., Aim: The purpose of this study was to compare gastric emptying rate and antral motility in obese and lean subjects., Materials and Methods: Gastric antral motility and gastric emptying were measured after consumption of sweet omelette in 2 groups of patients: 10 obese (BMI mean = 33.18) and 10 lean subjects (BMI mean = 22.16) using a scintigraphic technique. Lag phase was assessed by physiologic and mathematic approach. Gastric area was calculated using a computer programme (Diacam-Siemens)., Results: There was no difference in gastric emptying profiles; the mean gastric contents after 2 hours of assessment in obese and lean groups was 69% and 71.6%, respectively. Gastric antral motility parameters such as contraction amplitude and frequency, relaxation and contraction time were not differing between the groups. Only in the 90th minute of the assessment a significant difference in contraction time was found. There were also no differences in gastric area and duration of lag phase. No significant correlation between height, BMI, gastric area and body area was found in both groups., Conclusions: There are no differences in the rate of gastric emptying and gastric antral motility between lean and obese subjects. No correlation between BMI, height, body area and gastric area was found. This findings deny the common opinion suggesting that obese people have bigger stomach which empties more rapidly than in lean subjects.

Introduction: An imbalance between the sympathetic and parasympathetic nervous systems has been indicated in obesity. The aim of the present study was to evaluate the results of tilt table testing (TT) in obese patients., Methods: 59 patients with simple obesity (F/M: 50/9; BMI: 36.9+/-3.9 kg/m(2)), and 19 healthy subjects of the control group (F/M: 16/3; BMI: 24.7+/-4.3 kg/m(2)) were examined. Additionally, TT was carried out in 11 patients from the obese group after a 3-month weight reduction treatment. We evaluated HR (heart rate), SBP (systolic blood pressure), DBP (diastolic blood pressure): values at rest, post-inclination values (HRt-1, SBPt-1, DBPt-1), differences between the maximum and minimum values as obtained on TT (HRt-range, SBPt-range, DBPt-range), and % differences of HR (%HRt-increase, %HRt-decrease, %HRt-range)., Results: A positive TT response was observed in 10 obese patients, and in none of the controls. Higher differences of HR values were observed in the obese (P<0.05) in comparison to control group. Patients with a positive TT response presented a significant increase in HRt-1, HRt-range, and %HRt-range when compared to negative TT and controls (P<0.02). A 3-month weight reduction treatment resulted in a decrease in HR at rest, and decrease in SBP and DBP-both rest values and values after table-inclination., Conclusions: A positive TT response is observed more often in patients with simple obesity. HR reaction to tilting varies between the patients and controls. A 3-month weight reduction treatment influences the cardiovascular response to tilting. Our results seem to support the hypothesis of autonomic imbalance in obesity.

Unlabelled: The aim of the present study was to verify the effect of chitosan as a possible adjuvant in the complex management of obesity., Method: Fifty obese women (22-59 years, BMI > 30) participated in the study. A six months program consisted of 2-hour meetings with a physician, psychologist and dietitian, in a group of about 20 persons, every two weeks. Low calorie diet (1000 kcal/day), physical activity and behaviour modification were recommended. Supplementation with chitosan was evaluated in a randomized, placebo-controlled, double-blind study. In the chitosan group, participants received ChitininN (Primex Ingredients ASA, Avaldsnes, Norway) i.e. 750 mg pure chitosan per tablet, two tablets three times daily before each main meal. The placebo group received identical placebo tablets., Results: Significantly higher body weight loss was noted in the chitosan-supplemented group (15.9 kg) than in the placebo group (10.9 kg) Also a greater decrease of systolic and diastolic blood pressure was observed in the chitosan group. There was no difference between the groups in the decrease of LDL and total cholesterol. No adverse effects were noted but the number of drop-outs was higher in the placebo group than in the chitosan group., Conclusion: Chitosan can be used as a valuable and safe adjuvant in long-term dietary treatment of obesity. Chitosan seems to accentuate the reduction of blood pressure associated with weight reduction.

The effectiveness of weight reducing treatment and maintaining of reduced weight depends in great extend on changing patient's food preferences and it can be achieved through nutritional education and through patients self-esteem. Estimation of changes in eating behaviour in obese women during 3 months of weight reducing treatment carried by doctors, dietician and psychotherapist. 3 months of weight reducing treatment resulted in weight loss of 9.1%. Percentage of animal fats in the meals was significantly reduced and the percentage of complex carbohydrates (fruit and vegetables) and low-fat protein products was increased. The change of a diet structure was accompanied by a symptomatic decrease of preferences of simple carbohydrates and animal fats as well as protein products. 3 months of weight reducing group treatment resulted not only in a reduction of weight but also in modification of eating behaviour. The beneficial change of food preferences which influenced the composition of the consumed meals seems to be good predictor of a long-term effect of the treatment. The most favourable results of the weight reducing treatment can be obtained by group therapy using eating education and psychotherapy.

Abstract: Obesity is associated with subclinical, chronic, and systemic immune activation characterized by increased serum concentration of proinflammatory cytokines released by adipose tissue. The aim of the present study was to determine the relationship between stage of development of obesity and changes in activity of tumor necrosis factor (TNF) system during 5-year follow-up observation. One hundred fifty-four women—102 obese, 24 overweight, and 28 lean—without concomitant diseases were examined for the first time from 2000 to 2001. After 5 years, 57 obese, 12 overweight, and 14 lean subjects were reexamined. In addition to anthropometric measurements, body composition was determined by the bioimpedance method; and serum concentrations of glucose, lipids, insulin, TNF-α, and soluble TNF receptors (sTNFRs) were measured. Only reexamined subjects were included in the analysis. After 5 years, fat mass increased significantly in 46 (66.7%) overweight or obese women and in all lean subjects (39.0 ± 12.3 vs 47.3 ± 13.6 kg, P < .001; 14.8 ± 3.7 vs 20.6 ± 5.4 kg, P < .01, respectively), whereas it decreased in 23 (33.3%) overweight or obese subjects (41.3 ± 12.5 vs 37.2 ± 14.0 kg, P < .005). The TNF-α levels increased significantly only in lean women (3.1 ± 3.0 vs 5.6 ± 2.0 pg/mL, P < .005), but remained unchanged in overweight and obese subjects regardless of fat mass changes. Serum concentrations of sTNFR1 and sTNFR2 decreased by 71% and 25% in obese, by 104% and 21% in overweight, and by 31% and 32% in lean group, respectively. The increase of plasma TNF-α level is an early event in abdominal fat accumulation. It seems that further fat mass gain does not enhance circulating TNF-α levels. [Copyright &y& Elsevier]
