Purpose: To investigate axial elongation (AE) and changes in relative peripheral refraction (RPR) in anisomyopic children undergoing orthokeratology (ortho-k)., Methods: Bilateral anisomyopic children, 7-12 years of age, were treated with ortho-k. Axial length (AL) and RPR, from 30° nasal (N30°) to 30° temporal (T30°), were measured at baseline and every 6 months over the study period. AE, changes in RPR and changes in the interocular AL difference were determined over time., Results: Twenty-six of the 33 subjects completed the 2-year study. The AE of the higher myopic (HM) eyes (at least 1.50 D more myopia than the other eye) (0.26 ± 0.29 mm) was significantly smaller than for the less myopic (LM) eyes (0.50 ± 0.27 mm; p = 0.003), leading to a reduction in the interocular difference in AL (p = 0.001). Baseline RPR measurements in the HM eyes were relatively more hyperopic at T30°, N20° and N30° (p ≤ 0.02) and greater myopic shifts were observed at T20° (p < 0.001), T30° (p < 0.001), N20° (p = 0.02) and N30° (p = 0.01) after lens wear. After 2 years of ortho-k lens wear, temporal-nasal asymmetry increased significantly, being more myopic at the temporal locations in both eyes (p < 0.001), while AE was associated with the change in RPR at N20° (β = 0.134, p = 0.01). The interocular difference in AE was also positively associated with the interocular difference in RPR change at N30° (β = 0.111, p = 0.02)., Conclusions: Ortho-k slowed AE in bilateral anisomyopia, with slower growth in the HM eyes leading to a reduction in interocular AL differences. After ortho-k, RPR changed from hyperopia to myopia, with greater changes induced in the HM eyes, and slower AE was associated with a more myopic shift in RPR, especially in the nasal field of both eyes., (© 2024 College of Optometrists.)

Objective: To compare the effectiveness of myopia control in Chinese children, who had been wearing either orthokeratology (ortho-k) lenses or myopia control spectacles (MCS) for one year., Methods: Relevant data were retrieved from 212 patients, with baseline myopia of -5.00 to -0.75 D, astigmatism ≤ 1.50 D, who had been undergoing myopia control treatment with either ortho-k (Euclid (OK1) or CRT (OK2)) or MCS (Stellest (MCS1) or DIMS (MCS2)) for at least one year. Myopia control effectiveness among the four groups was compared based on the change in spherical equivalent refraction (SER) (for the spectacle groups) and axial length (AL) (for all groups). Data retrieved, from the right eye only, included best corrected visual acuity (BCVA), SER, and AL at both the initial clinic visit (baseline) and the one-year visit was analysed., Results: Although no significant differences in gender, cylindrical power, or BCVA were observed at the baseline visit (P > 0.05), there were significant differences in baseline age, myopia, and AL among the four groups (P < 0.05). No significant difference in axial elongation (AE) was found among the four groups after one year of treatment (P = 0.49). AE, adjusted for baseline age and spherical power, were 0.19 ± 0.15, 0.18 ± 0.14, 0.19 ± 0.19, 0.20 ± 0.18 mm for OK1, OK2, MCS1, MCS2, respectively. Only age was a significantly factor associated with AE and SER increase (P < 0.05), with negative associations between AE and age in all groups and positive association between SER increase and baseline age in the spectacle groups observed., Conclusion: Both brands of ortho-k lenses and MCS had similar myopia control effectiveness for mild to moderate myopic children treated in a real-world setting in China. Clinical Relevance: Identification of the baseline chracteristics for children undergoing orthokeratology with relatively fast myopia progression can allow a more accurate determination of the risk/benefit ratio., Background: This study aimed to investigate if baseline corneal biomechanics can classify relatively slow and fast myopia progression in children., Methods: Children aged six to 12 years with low myopia (0.50 to 4.00 D) and astigmatism (less than or equal to 1.25 D), were recruited. Participants were randomised to be fitted with orthokeratology contact lenses with a conventional compression factor (0.75 D, n  = 29) or an increased compression factor (1.75 D, n  = 33). Relatively fast progressors were defined as participants who had axial elongation of 0.34 mm or above per 2 years. A binomial logistic regression analysis and a classification and regression tree model were used in the data analysis. The corneal biomechanics were measured with a bidirectional applanation device. The axial length was measured by a masked examiner., Results: As there were no significant between-group differences in the baseline data (all p  > 0.05), data were combined for analysis. The mean ± SD axial elongation for relatively slow ( n  = 27) and fast ( n  = 35) progressors were 0.18 ± 0.14 mm and 0.64 ± 0.23 mm per 2 years, respectively. The area under the curve (p2area1) was significantly higher in relatively fast progressors ( p  = 0.018). The binomial logistic regression and classification and regression tree model analysis showed that baseline age and p2area1 could differentiate between slow and fast progressors over 2 years., Conclusions: Corneal biomechanics could be a potential predictor of axial elongation in orthokeratology contact lens-wearing children.

Purpose: To report the characteristics (prevalence, severity, and location) of corneal epithelial microcysts and investigate associated risk factors in children wearing orthokeratology (ortho-k) lenses., Method: Ninety-five myopic children wearing ortho-k lenses (examined by one of three independent investigators from March to September 2020) were included in this retrospective cross-sectional study. Pertinent data at baseline before ortho-k treatment and at the aftercare visits (the first visit when the microcysts were observed for children with microcysts, and the last visit before October 2020 for children without microcysts) were retrieved and analysed., Results: A microcystic response was observed in 52.6% of children wearing ortho-k lenses. Children with high myopia (≥ 5.00 D) had a higher prevalence (100.0%, 23/23) and severity (69.5% (16/23) > grade 2 Efron scale) compared to children with low myopia (≤ 4.00 D) (prevalence of 37.5% (27/72) and 7.0% (5/72) > grade 2, p < 0.001). Microcysts were predominantly (86.0%) observed in the region of the inferior pigmented arc, typically originating in the inferior mid-peripheral cornea, and expanding over time into a semi- or whole annulus. Baseline myopia and topographical change at the treatment zone centre were significantly greater (p < 0.05) in low myopic children with microcysts (univariate analyses)., Conclusions: During the COVID-19 pandemic, probably due to lifestyle changes, microcysts were frequently observed in children wearing ortho-k lenses and were associated with higher baseline myopia. Practitioners should examine ortho-k wearers with caution using a slit lamp with high magnification and illumination, especially the mid-peripheral cornea. Objective: To determine changes in various ocular parameters of adults wearing orthokeratology (ortho-k) lenses and their levels of satisfaction and quality of life (QoL) after commencing treatment., Methods: Adults aged 18-38 years, with mild to moderate myopia and astigmatism < 1.50D, were wearing ortho-k lenses for one year. Data collection, which included history taking, refraction, axial length (AL), corneal topography, corneal biomechanics, and biomicroscopy examination, was performed at baseline and every 6 months during the study period. The level of satisfaction with the treatment and QoL was determined via questionnaires., Results: Forty-four subjects completed the study. AL was shortened significantly at the 12-month visit: AL -0.03 (-0.45 to 0.13) mm compared to baseline (p < 0.05). A significant number of subjects in both groups presented with overall and central corneal staining, but the majority were mild (Grade 1). Central endothelial cell density was reduced by 40/mm 2 (loss rate 1.4 %) (p < 0.05). High scores were obtained in the satisfaction questionnaire, with no significant differences between visits. At the 12-month visit, NEI-RQL-42 total score, dependence on correction, activity limitation, appearance, and satisfaction with the treatment all significantly increased compared to baseline values., Conclusions: Results suggest that ortho-k can be an effective and safe myopia correction technique for adults with low to moderate myopia, improving daytime vision without serious adverse events. Myopia is a dynamic and rapidly moving field, with ongoing research providing a better understanding of the etiology leading to novel myopia control strategies. In 2019, the International Myopia Institute (IMI) assembled and published a series of white papers across relevant topics and updated the evidence with a digest in 2021. Here, we summarize findings across key topics from the previous 2 years. Studies in animal models have continued to explore how wavelength and intensity of light influence eye growth and have examined new pharmacologic agents and scleral cross-linking as potential strategies for slowing myopia. In children, the term premyopia is gaining interest with increased attention to early implementation of myopia control. Most studies use the IMI definitions of ≤-0.5 diopters (D) for myopia and ≤-6.0 D for high myopia, although categorization and definitions for structural consequences of high myopia remain an issue. Clinical trials have demonstrated that newer spectacle lens designs incorporating multiple segments, lenslets, or diffusion optics exhibit good efficacy. Clinical considerations and factors influencing efficacy for soft multifocal contact lenses and orthokeratology are discussed. Topical atropine remains the only widely accessible pharmacologic treatment. Rebound observed with higher concentration of atropine is not evident with lower concentrations or optical interventions. Overall, myopia control treatments show little adverse effect on visual function and appear generally safe, with longer wear times and combination therapies maximizing outcomes. An emerging category of light-based therapies for children requires comprehensive safety data to enable risk versus benefit analysis. Given the success of myopia control strategies, the ethics of including a control arm in clinical trials is heavily debated. IMI recommendations for clinical trial protocols are discussed.

Background: To investigate whether combining 0.01% atropine with orthokeratology (AOK) has a better effect in retarding axial elongation, compared with orthokeratology alone (OK) over two years., Methods: A total of 96 Chinese children aged six to < 11 years with myopia (1.00 - 4.00 D, inclusive) were randomized into either the AOK or OK group in a 1:1 ratio. Axial length (the primary outcome), and secondary outcomes (e.g. pupil size and choroidal thickness) were measured at 1-month and at 6-monthly intervals after commencement of treatment., Results: Both intention-to-treat and per-protocol analyses showed significantly slower axial elongation in the AOK group than OK group over two years (P = 0.008, P < 0.001, respectively). AOK subjects had statistically slower axial elongation (adjusted mean [standard error], 0.17 [0.03] mm vs 0.34 [0.03] mm, P < 0.001), larger increase in mesopic (0.70 [0.09] mm vs 0.31 [0.09] mm, P = 0.003) and photopic pupil size (0.78 [0.07] mm vs 0.23 [0.07] mm, P < 0.001), and greater thickening of the choroid (22.6 [3.5] µm vs -9.0 [3.5] µm, P < 0.001) than OK subjects over two years. Except for a higher incidence of photophobia in the AOK group (P = 0.006), there were no differences in the incidence of any other symptom or adverse events between the two groups. Purpose: To determine the frequency and severity of ocular symptoms and signs in new orthokeratology (ortho-k) subjects using a povidone iodine (PI) disinfecting system compared to those present before lens wear, and whether these were associated with cleaning regimes., Methods: This study recruited 80 subjects from two myopia control studies, who used a PI disinfecting solution for routine use. Ocular symptoms and signs at baseline, one- and six-month after lens wear were reported via questionnaires and ocular examination, respectively. To determine if rates of occurrence were attributable to differences in cleaning regime, subjects were randomly assigned into four groups with respect to routine care procedures, which involved various combinations of rubbing and use of a daily and/or enzymatic cleaner., Results: Sixty-eight subjects completed all aspects of the study. As there were no significant differences in incidence of signs and symptoms between the four groups before and after lens wear (Friedman tests, p > 0.07), data were combined for further analysis. Prior to lens wear, itchiness (69 %) and dryness (53 %) were the most commonly reported symptoms. The frequency and severity of all symptoms remained similar after lens wear (p > 0.10). Presence of follicles in the lower tarsal conjunctiva (22 %) and conjunctival injection (15 %) was frequently observed, but reduced significantly after lens wear (p < 0.01). Mild corneal staining, noted in 13 % of subjects at baseline, did not change significantly over time (p = 0.17). Ocular signs were not necessarily reflected in symptoms and vice versa., Conclusion: Use of a PI-based solution did not increase the frequency or severity of ocular signs and symptoms observed before lens wear. Purpose: To investigate the effectiveness of orthokeratology (ortho-k) lenses and corneal changes with increased compression factor for myopia control over a 2-year period., Methods: Young participants (age: 6-<12 years), with low myopia (0.50-4.00 D) and low astigmatism (≤1.25 D), were recruited and allowed to choose to wear either single-vision spectacles or ortho-k lenses (randomly assigned to compression factor of either 0.75 or 1.75 D). Axial length and cycloplegic refraction were measured at six monthly intervals for two years by a masked examiner. The myopia control effectiveness was determined by axial elongation., Results: A significant number of control (63 %) dropped out, mainly due to concern about myopia progression (58 %). A total of 75 participants (mean age: 9.3 ± 1.0 years; control: n = 11, ortho-k [0.75 D]: n = 29, ortho-k [1.75 D]: n = 35) completed the study. Considering ortho-k groups only, the mean axial elongation of participants wearing ortho-k lenses of conventional compression factor (0.75 D) and increased compression factor (1.75 D) were 0.53 ± 0.29 and 0.35 ± 0.29 mm, respectively, over the 2-year study period. The between-group differences in corneal health were not significant at all visits., Conclusion: Participants wearing ortho-k lenses of increased compression factor further slowed axial elongation by 34%, when compared with the conventional compression factor without compromising corneal health. Purpose: To investigate the repeatability and reproducibility of choroidal thickness measurements using Lenstar images in young myopic children before and after one-month orthokeratology (ortho-k) treatment., Method: Ocular biometry of 39 subjects were performed using the Lenstar 900. The first five measurements with maximum differences of 0.02 mm in axial length in the right eyes were saved and used for measurement of choroidal thickness. Subfoveal choroidal thickness were manually measured by identifying the signals from the retinal pigmented epithelium layer and chorioscleral interface. Repeatability was determined by comparing measurements of the same images made by the same observer on two separate occasions (four weeks apart), while reproducibility was calculated by comparing measurements of the same images made by two independent observers. Data was analysed using intra-class correlation coefficients (ICC) and non-parametric Bland and Altman plots., Results: The choroidal peaks could not be identified in all five measurements in all subjects. On average, only 71% subjects had at least four definable images. Compared with the use of fewer than four images, reliability using an average of four definable images improved statistically, but remained clinically unacceptable (>10 µm), although pre- and post-ortho-k ICC values were good to excellent for repeatability (0.867 and 0.975, respectively) and excellent and good for reproducibility (0.959 and 0.868, respectively). Non-parametric pre- and post-ortho-k limits of agreement (2.5% and 97.5% percentiles) obtained were -45.8 to 79.3 µm and -30.3 to 9.5 µm, respectively for repeatability, and -29.0 to 33.5 µm and -21.8 to 70.0 µm, respectively for reproducibility., Conclusion: Choroidal thickness measurements using the Lenstar did not show good reliability, despite the high ICC values, non-parametric Bland and Altman plots demonstrated a wide variability of measurement errors. Purpose: To compare the treatment zone (TZ) measurements obtained using manual and software-based methods in orthokeratology (ortho-k) subjects and explore the TZ characteristics of children with slow and fast axial elongation after ortho-k., Methods: Data from 69 subjects (aged 7 to <13 years old), who participated in three 24-month longitudinal orthokeratology studies, showing fast (>0.27 mm, n = 38) and slow (<0.09 mm, n = 31) axial elongation, were retrieved. The TZ after ortho-k was defined as the central flattened area enclosed by points with no refractive power change. TZ parameters, including decentration, size, width of the peripheral steepened zone (PSZ), central and peripheral refractive power changes and peripheral rate of power change, were determined manually and using python-based software. TZ parameters were compared between measurement methods and between groups., Results: Almost all TZ parameters measured manually and with the aid of software were significantly different (p < 0.05). Differences in decentration, size and the PSZ width were not clinically significant, but differences (0.45 to 0.92 D) in refractive power change in the PSZ were significant, although intraclass coefficients (0.95 to 0.98) indicated excellent agreement between methods. Significantly greater TZ decentration, smaller TZ size and greater inferior rate of power change (relative to the TZ centre) were observed in slow progressors using both methods, suggesting a potential role of TZ in regulating myopia progression in ortho-k., Conclusion: TZ measurements using manual and software-based methods differed significantly and cannot be used interchangeably. Aim: To understand the views of contact lens (CL) practitioners across the globe regarding what they perceive as opportunities and threats in CL practice., Methods: A self-administered anonymised questionnaire, constructed in English and translated in six more languages, was distributed through reputed international professional bodies and academic institutions worldwide. The questionnaire included items on demographic characteristics, type of practice, and questions designed to explore practitioners' perspective on the future of their CL practice over the next five years., Results: A total of 2408 valid responses were analysed. Multifocal CLs for presbyopia, CLs for myopia control, use of daily disposable (DD) CLs for occasional wear, and biocompatible materials to improve comfort were identified as promising areas of opportunities by practitioners (all 8/10). Respondents from North America, and Europe valued DDCLs for occasional wear moderately more favourable (Median: 9/10 for all) as compared to colleagues in Asia (Median: 8/10, p < 0.001), South America (Median: 8/10, p < 0.01), and Africa (Median: 8/10p < 0.01). Multifocal CLs for presbyopia was perceived as a better opportunity by practitioners in North America and Europe (Median: 9/10 for both), as well as in Australasia (Median: 8/10), in comparison to Asia, Africa, and Middle East (for all Median: 6/10, p < 0.001). Practitioners expressed concerns about the availability of CLs and CL prescriptions online without direct professional involvement (both 9/10)., Conclusions: Overall, the most appealing opportunities for CL practice growth were identified in occasional use of DD CLs, biocompatible materials to reduce CL discomfort, multifocal CLs for presbyopia correction and management of myopia control with CLs. Lack of regulation in CL sales, especially online, seemed to be a constant threat. Purpose: To compare myopia progression estimated by the Brien Holden Vision Institute (BHVI) Myopia Calculator with cycloplegic measures in Hong Kong children wearing single-vision distance spectacles over a 1- and 2-year period., Methods: Baseline age, spherical equivalent refraction (SER) and ethnicity of control participants from previous longitudinal myopia studies were input into the BHVI Myopia Calculator to generate an estimate of the SER at 1 and 2 years. Differences between the measured and estimated SER (116 and 100 participants with 1- and 2-year subjective refraction data, respectively, and 111 and 95 participants with 1- and 2-year objective refraction, respectively) were analysed, and the measured SER compared with the 95% confidence interval (CI) of the estimated SER., Results: In children aged 7-13 years, 36% progressed within the 95% CI of the Myopia Calculator's estimate, whereas 33% became less myopic than predicted (range 0.31 to 1.92 D less at 2 years) and 31% became more myopic than predicted (range 0.25 to 2.33 D more myopic at 2 years). The average difference between the estimated and measured subjective or objective SER at 1 and 2 years of follow-up was not clinically significant (<0.25 D)., Conclusions: On average, the BHVI Myopia Calculator estimated SER was in close agreement with measured cycloplegic SER after 1 and 2 years of follow-up (mean differences < 0.25 D). However, the measured myopia progression only fell within the 95% CI of the estimated SER for 32%-38% of children, suggesting that the BHVI 'without management' progression data should be interpreted with caution. The inclusion of additional data, modified to include axial elongation, from longitudinal studies of longer duration with larger sample sizes and a range of racial backgrounds may improve the Calculator's ability to predict future myopia progression for individual children., (© 2021 The Authors. Purpose: To investigate antibiotic eye drops prescribing patterns of contact lens practitioners in Mainland China for orthokeratology (ortho-k) patients and to develop guidelines for their use., Methods: A questionnaire on their antibiotic prescribing habits was administered to eye care practitioners (ECP) involved in ortho-k lens fitting nationwide via an official online account. Multiple logistic regression models were used to assess the relationships between prescribing habits and demographics., Results: A total of 555 ECP completed the survey, of whom 50.5 % were optometrists (non-medical degree). About 30 % of the respondents routinely prescribed antibiotic eye drops for prophylactic use before and/or after commencement of ortho-k treatment. Forty one percent of the respondents dispensed antibiotics to patients for emergency use without giving written instructions, 48.5 % used antibiotic eye drops to wet fluorescein strips during ortho-k lens fitting, and 44.9 % considered treating corneal infiltrate(s) with antibiotics to be appropriate. A set of guidelines, including when to use antibiotic eye drops during ortho-k lens treatment and their proper use, stratified by different corneal conditions, was developed., Conclusions: Inappropriate use of antibiotic eye drops appeared to be common among ortho-k practitioners in Mainland China. Objective: To measure and compare the vision-related quality of life between Chinese children wearing orthokeratology (ortho-k) lenses and single vision spectacles, to understand acceptance of ortho-k treatment by children in China., Methods: Subjects of Chinese origin, with myopia of -5.00 to -0.75 D, astigmatism < 1.50 D were recruited. All subjects had been wearing optical correction - ortho-k lenses or single vision spectacles (SVS), for the past 12-18 months and were aged between 8-12 years. The Pediatric Refractive Error Profile (PREP) questionnaire, translated to Chinese, was used to evaluate the perceptions of children wearing spectacles in overall vision, near vision, far vision, symptoms, appearance, satisfaction, activities, academic performance, handling of optical corrections, and peer perceptions. PREP questions, rephrased to address the same issues for ortho-k subjects who did not wear spectacles in the daytime, were used for ortho-k wearers (PREP-OK). The mean score of all items was calculated as the overall score. For ortho-k wearers, four additional questions on experience and frequency of symptoms: experiencing difficulty in falling asleep, ocular discomfort, itchy/burning/dry eyes, and foreign body sensation during ortho-k lens wear at night were asked and reported separately., Results: Forty subjects (20 ortho-k, 20 SVS) completed the study. Overall vision, far vision, appearance, satisfaction, activities, and peer perception scores in the ortho-k group were significantly better than the SVS group (all P < 0.05). Handling of optical correction score in the ortho-k group was significantly worse than the SVS group (P = 0.04). No significant differences in near vision, symptoms in the daytime and academic performance were found between two groups (P > 0.05). With respect to symptoms during ortho-k lens wear at night, none of the subjects reported difficulty in falling asleep, but 30-40 % of subjects reported occasional ocular discomfort, itchy/burning/dry eyes, and foreign body sensation after lens insertion., Conclusion: Although ortho-k may induce some ocular discomfort with lens wear during the night, these were infrequent and the benefits from ortho-k can compensate for the discomfort, leading to better vision-related quality of life in Chinese children, compared with those wearing SVS., (Copyright © 2020 British Contact Lens Association. Purpose: To evaluate thickness changes in the central and mid-peripheral cornea (CCT and mPCT), corneal epithelium (CET and mPET) and stroma (CST and mPST) of myopic children during and after short-term orthokeratology (ortho-k) lens wear, with conventional (CCF, 0.75 D) and increased compression factors (ICF, 1.75 D)., Methods: This was a self-controlled case series study. Subjects wore a CCF lens in one eye and an ICF lens in the other. Anterior segment optical coherence tomography images were captured weekly for 1 month during lens wear and for 2 weeks after discontinuing lens wear. CCT and CET (central 3-mm cornea) and mPCT and mPET (within a 4-6 mm diameter annulus) were measured. Stromal thickness (ST) was determined by subtracting epithelium thickness (ET) from corneal thickness (CT). The repeatability of the analytical software was also investigated on age-matched spectacle-wearing subjects (n = 98)., Results: Excluding three outliers (>3 S.D.s), the coefficient of repeatability and intraclass correlation coefficients of 98 spectacle-wearing subjects ranged from 2.63 to 4.64 μm and from 0.90 to 0.99, respectively. For the weekly-change study, CCT and CET in both eyes were significantly thinner after lens wear (p < 0.001) and CET thinning in the ICF eyes were significantly higher (p < 0.02). CCT changes were mainly contributed by CET. CST, mPCT, mPET and mPST changes were not significant (p > 0.20) in either eye. CT (all sublayers) rebounded to baseline values 2 weeks after discontinuing lens wear (0.99 > p > 0.12)., Conclusions: Significant reductions in CT and ET, but not ST, were observed within 1 month of ortho-k lens wear. Wearing ICF lenses resulted in a higher reduction in CET. Purpose: To present the 1-year results of the Variation of Orthokeratology Lens Treatment Zone (VOLTZ) Study, which aims to investigate the myopia control effect of orthokeratology (ortho-k) lenses with different back optic zone diameters (BOZD)., Method: Children, aged 6 to <11 years, having myopia -4.00 D to -0.75 D, were randomly assigned to wear ortho-k lenses with 6 mm (6-MM group) or 5 mm (5-MM group) BOZD. Data collection included changes in refraction, vision, lens performance and binding, ocular health conditions, axial length and characteristics of the treatment zone (TZ) area., Results: The 1-year results of 34 and 36 subjects (right eye only) in the 6-MM and 5-MM groups, respectively, are presented. No significant differences in baseline demographics were found between the groups (p > 0.05). The first-fit success rates, based on satisfactory centration at the 1-month visit, were 100% and 94% respectively. Horizontal TZ size was 0.92 mm and 0.72 mm smaller in the 5-MM group at the 6-month and 12-month visits, respectively (p < 0.05). At the 12-month visit, no significant between-group differences were found in the incidence of corneal staining (low grade only), lens binding and visual performance (all p > 0.05). Axial elongation was slower in the 5-MM group (0.04 ± 0.15 mm) than the 6-MM group (0.17 ± 0.13 mm) (p = 0.001). A significant positive correlation was observed between the horizontal TZ size and axial elongation (r = 0.36, p = 0.006)., Conclusion: Clinical performance of the two ortho-k lenses was similar, indicating that a smaller BOZD (5 mm) did not affect lens performance or ocular integrity. Purpose: The International Myopia Institute (IMI) Yearly Digest highlights new research considered to be of importance since the publication of the first series of IMI white papers., Methods: A literature search was conducted for articles on myopia between 2019 and mid-2020 to inform definitions and classifications, experimental models, genetics, interventions, clinical trials, and clinical management. Conference abstracts from key meetings in the same period were also considered., Results: One thousand articles on myopia have been published between 2019 and mid-2020. Key advances include the use of the definition of premyopia in studies currently under way to test interventions in myopia, new definitions in the field of pathologic myopia, the role of new pharmacologic treatments in experimental models such as intraocular pressure-lowering latanoprost, a large meta-analysis of refractive error identifying 336 new genetic loci, new clinical interventions such as the defocus incorporated multisegment spectacles and combination therapy with low-dose atropine and orthokeratology (OK), normative standards in refractive error, the ethical dilemma of a placebo control group when myopia control treatments are established, reporting the physical metric of myopia reduction versus a percentage reduction, comparison of the risk of pediatric OK wear with risk of vision impairment in myopia, the justification of preventing myopic and axial length increase versus quality of life, and future vision loss., Conclusions: Large amounts of research in myopia have been published since the IMI 2019 white papers were released. The yearly digest serves to highlight the latest research and advances in myopia.

The prevalence of myopia has markedly increased in East and Southeast Asia, and pathologic consequences of myopia, including myopic maculopathy and high myopia-associated optic neuropathy, are now some of the most common causes of irreversible blindness. Hence, strategies are warranted to reduce the prevalence of myopia and the progression to high myopia because this is the main modifiable risk factor for pathologic myopia. On the basis of published population-based and interventional studies, an important strategy to reduce the development of myopia is encouraging schoolchildren to spend more time outdoors. As compared with other measures, spending more time outdoors is the safest strategy and aligns with other existing health initiatives, such as obesity prevention, by promoting a healthier lifestyle for children and adolescents. Useful clinical measures to reduce or slow the progression of myopia include the daily application of low-dose atropine eye drops, in concentrations ranging between 0.01% and 0.05%, despite the side effects of a slightly reduced amplitude of accommodation, slight mydriasis, and risk of an allergic reaction; multifocal spectacle design; contact lenses that have power profiles that produce peripheral myopic defocus; and orthokeratology using corneal gas-permeable contact lenses that are designed to flatten the central cornea, leading to midperipheral steeping and peripheral myopic defocus, during overnight wear to eliminate daytime myopia. The risk-to-benefit ratio needs to be weighed up for the individual on the basis of their age, health, and lifestyle. The measures listed above are not mutually exclusive and are beginning to be examined in combination.

Orthokeratology (ortho-k) is the process of deliberately reshaping the anterior cornea by utilising specialty contact lenses to temporarily and reversibly reduce refractive error after lens removal. Modern ortho-k utilises reverse geometry lens designs, made with highly oxygen permeable rigid materials, worn overnight to reshape the anterior cornea and provide temporary correction of refractive error. More recently, ortho-k has been extensively used to slow the progression of myopia in children. This report reviews the practice of ortho-k, including its history, mechanisms of refractive and ocular changes, current use in the correction of myopia, astigmatism, hyperopia, and presbyopia, and standard of care. Suitable candidates for ortho-k are described, along with the fitting process, factors impacting success, and the potential options for using newer lens designs. Ocular changes associated with ortho-k, such as alterations in corneal thickness, development of microcysts, pigmented arcs, and fibrillary lines are reviewed. The safety of ortho-k is extensively reviewed, along with an overview of non-compliant behaviours and appropriate disinfection regimens. Finally, the role of ortho-k in myopia management for children is discussed in terms of efficacy, safety, and potential mechanisms of myopia control, including the impact of factors such as initial fitting age, baseline refractive error, the role of peripheral defocus, higher order aberrations, pupil size, and treatment zone size., (Copyright © 2021. Objective: To investigate the repeatability of pupil size measurements, determined from the NIDEK OPD-Scan III, of myopic eyes in children wearing single-vision spectacles (SVS), undergoing orthokeratology (OK) and receiving combined treatment of 0.01% atropine and orthokeratology (AOK)., Methods: A single examiner took two sets of mesopic pupil size measurements from the right eyes of 80 children (SVS:16, OK: 34, AOK: 30) (mesopic 1 and mesopic 2) and two sets of photopic (photopic 1 and photopic 2) pupil size measurements (internal light source) using the NIDEK OPD-Scan III. Each set was taken at a single visit, 15 min apart. Subjects in the OK and AOK groups had undergone treatment for at least 6 months, and the SVS subjects had no previous myopia treatment other than wearing spectacles. Repeatability of measurements was analysed using intraclass correlation coefficient (ICC), Bland-Altman (BA) plot, and coefficient of repeatability (CoR)., Results: Mesopic 1 and 2 values were not significantly different among the three groups (p = 0.56, 0.77), hence the data were pooled for each set of measurements (mean [±S.D.] values were 7.09 ± 0.61 and 7.12 ± 0.61 mm, respectively) for further analyses. ICC for repeated mesopic pupil size measurements was 0.98 (95% CI, 0.98-0.99). No significant correlation was found between the means of the mesopic 1 and 2 measurements and their differences (Pearson's r = -0.02, p = 0.83). A BA plot also demonstrated narrow 95% limits of agreement, with a CoR of 0.28 mm. Mean photopic 1 and 2 measurements for the AOK group (3.74 ± 0.46; 3.73 ± 0.43 mm, respectively) were significantly larger (p = 0.01; 0.009) than those of the SVS and OK groups, but no significant difference was found between the latter two groups (p > 0.05). Hence, photopic 1 and 2 measurements for the SVS and OK groups were pooled (SVS-OK) for further analyses. ICC for repeatability of the photopic measurements was 0.98 (95% CI, 0.96 to 0.99) for the SVS-OK and AOK groups. The differences between photopic 1 and 2 measurements were not significantly correlated with their means, either in the AOK group (Pearson's r = -0.25, p = 0.19), or in the pooled SVS-OK group (Pearson's r = -0.04, p = 0.78). Purpose: To investigate factors that influence parental decisions to use orthokeratology (ortho-k) as a myopia control method for their children, and parents' perspectives during children's ortho-k treatment in a hospital setting., Methods: Parents/guardians of children wearing ortho-k lenses for at least six months were required to complete a face-to-face survey to investigate the reasons for choosing ortho-k and their experience of the treatment., Results: Parents/guardians of 128 ortho-k wearers were recruited from the West China Hospital, Sichuan, China. The majority of the participants (74%) were mothers. Wearers' mean age at the time of the survey was 12.0 ± 2.0 years, with a mean history of ortho-k lens wear of 20.3 ± 8.0 months. Most participants had learned about ortho-k through word of mouth (56%) or from ophthalmologists (40%); in China, only ophthalmologists may prescribe ortho-k. The highest ranked motivation was the rapid progression of their children's myopia (54%), followed by ophthalmologists' advice (17%) or participants' who felt they had obtained enough knowledge (17%) of ortho-k (Kruskal-Wallis test, p = 0.002). 'Effectiveness' was the most common reason that participants decided to use ortho-k (81%), and potential injury was their major concern (75%). Itchy/sore eye was the most common initial adverse effect experienced with lens wear (22%), while a lost or broken ortho-k lens was the most frequently reported problem (34%). Parents of older lens wearers were significantly less involved in all procedures (Spearman rank correlation test, all p < 0.001). Most wearers, according to either the participants or the wearers themselves, were willing to wear ortho-k lenses at treatment commencement (80%), and the most common reason was 'to avoid having to wear spectacles in the daytime'., Conclusions: The most powerful motivation for parents to enrol their children in ortho-k treatment was the rapid progression of myopia. The main reason why participants chose ortho-k, which most participants learned of by word of mouth, was the effectiveness of the treatment. During use, the most common symptom reported was itchy or sore eyes, but only initially. Children's willingness to participate is very important for successful ortho-k treatment; however, their motivation can be quite different from that of their parents. Aims: To compare the value of pre-treatment axial elongation (AE) and changes in refractive sphere (M change) for predicting the success in orthokeratology (ortho-k), in order to better identify suitable candidates for myopia control., Methods: This study further analysed the data of 66 subjects receiving 7-month ortho-k treatment, following a 7-month observation period, during which single-vision spectacles were worn. Rate of myopia progression was determined by AE and M change and subjects categorised as slow, moderate, or rapid progressors based on these changes. Outcomes of myopia control, based on the AE reduction after ortho-k, were classified as 'ineffectual', 'clinically insignificant', or 'beneficial'., Results: Of the 20 subjects, initially categorised as slow by AE and, of whom 95% were similarly categorised by M change, none benefitted from ortho-k. In contrast, of the 22 subjects with moderate AE, 77% and 23% displaying slow and moderate M change, respectively, the majority (73%) benefitted from ortho-k lens wear. The 24 subjects with rapid AE were poorly identified by M change, with only 21% correctly categorised. The vast majority of rapid progressors showed significant benefit after ortho-k., Conclusion: Progression of AE is a good indicator of subsequent success of ortho-k treatment. Delaying commencement of therapy is prudent for children with slow progression as results indicate that they would be unlikely to benefit from this intervention. Objective: To report the study design and one month's preliminary results of a randomized, single-masked, one-year prospective study of orthokeratology (ortho-k) in adults wearing lenses of different compression factors., Methods: Adults aged 18-38 years, with myopia of -0.75 to -5.00 D and astigmatism < 1.50 D, were recruited and randomly assigned into two groups: a conventional compression factor or Jessen Factor (CCF) group (compression factor = 0.75 D) and an increased (extra 1.00D) compression factor (ICF) group. Clinical outcomes, including spherical equivalent refraction (SER) reduction, visual acuity, corneal hysteresis (CH) and corneal resistance factor (CRF), and signs and symptoms were collected at the one-month follow-up visit. Indicators of the level of satisfaction and quality of life after commencing treatment were determined via a satisfaction questionnaire and the NEI-RQL-42 questionnaire., Results: Baseline data from 26 CCF and 24 ICF participants were analysed and no significant differences were observed between the two groups (p > 0.05). The first fit success rates were 90 % for CCF group and 83 % for the ICF group, SER reductions were 97 % and 95 % for the CCF and ICF group, respectively, with uncorrected high-contrast visual acuity of -0.06 (-0.18 to 0.42) and 0.00 (-0.16 to 0.52), respectively (p > 0.05) at the 1-month visit. Overall, the incidence of corneal staining was 77 % in the CCF and 79 % in ICF group; central corneal staining was 15 % and 33 %, respectively. However, the differences of corneal staining between the groups did not reach significance in any visit (p > 0.05). The main complaint from participants was glare (both groups). No significant differences in CRF and CH were found in the first month (p > 0.05). Both groups recorded high scores in the level of satisfaction questionnaire, with no significant differences between groups (p > 0.05). Compared with baseline scores, 1-month NEI-RQL-42 subscales of dependence on correction, appearance, and satisfaction with correction significantly increased, and the glare score significantly decreased in both groups (all p < 0.05)., Conclusions: The majority of participants were satisfied with the treatment and no serious corneal adverse effects were observed. Purpose: To compare the changes in higher order aberrations (HOA's) for photopic and mesopic pupil diameters in children undergoing orthokeratology treatment (OK) or combined 0.01% atropine with orthokeratology treatment (AOK), and their association with axial elongation., Methods: Children aged 6 to <11 years with 1.00-4.00 D of myopia were randomly assigned to each treatment group. Photopic and mesopic pupil diameters were quantified using automated pupillometry and HOA's were measured with a Hartmann-Shack aberrometer and Badal system to control for accommodation. HOA's were rescaled to photopic and mesopic pupil diameters and fitted with a 6 th order Zernike polynomial expansion. Axial length was measured using an optical biometer under cycloplegia., Results: Baseline and six-month data from 25 AOK and 28 OK participants were analysed. At the six-month visit, pupil diameter was larger in the AOK group under photopic conditions (3.70 ± 0.42 vs 3.12 ± 0.33 mm, p < 0.001), along with a range of HOA metrics [3 rd to 6 th order and higher order root mean square error values (HO RMS), all p ≤ 0.003] and individual Zernike terms (primary spherical aberration, and oblique quadrafoil, both p ≤ 0.03). Axial elongation was greater in the OK treatment group (0.05 ± 0.08 vs -0.01 ± 0.12 mm, p = 0.02). In the AOK group, axial elongation was correlated with the increase in photopic pupil diameter (r = -0.45, p = 0.02) and with several HOA metrics; however, these associations were not observed in the OK group., Conclusion: AOK treatment resulted in increased photopic pupil size and HOA's, and significantly less axial elongation over a six-month period compared to OK treatment alone. Purpose: To report the 1-year results of an investigation into whether there is an additive effect between 0.01% atropine and orthokeratology (ortho-k), in a single-masked, two-arm, randomised controlled trial: Combined Atropine with Orthokeratology (AOK) for myopia control study (ClinicalTrials.gov number: NCT02955927)., Methods: Chinese children aged between 6 and 11 years with 1.00-4.00 D of myopia, astigmatism <2.50 D, and no more than 1.00 D anisometropia, were randomly assigned either to an AOK group or ortho-k only (OK) group at a 1:1 ratio. Subjects in the AOK group instilled one drop of 0.01% atropine into each eye, 10 min before nightly wear of ortho-k lenses. The primary outcome, axial elongation, was examined at 6-monthly intervals, along with secondary outcomes including best-corrected visual acuity (BCVA), manifest refraction, accommodation, pupil size, and corneal topography., Results: 29 AOK and 30 OK subjects completed the 1-year visit. The overall axial elongation rate was significantly slower in the AOK group than in the OK group (mean (S.D.), 0.07 (0.16) mm vs 0.16 (0.15) mm, respectively; p = 0.03). A significant between-group difference in axial elongation was observed over the first 6-month period only (p < 0.001), but not over the second period (p = 0.818). At the 1-year visit, increases in mean (S.D.) mesopic and photopic pupil sizes in the AOK group were 0.64 (0.48) mm and 0.36 (0.34) mm, respectively, which were significantly higher than 0.10 (0.50) mm and 0.02 (0.28) mm in the OK group (p < 0.001). At the 6-month visit, a significant moderate negative correlation was found between axial elongation and the increase in photopic pupil size (r = -0.42, p = 0.02) in the AOK group., Conclusions: There is an additive effect between 0.01% atropine and ortho-k over one year, with mean axial elongation in the AOK group 0.09 mm slower than that in the OK group. Purpose: This retrospective longitudinal study aimed to examine the relationship between ocular higher-order aberrations (HOA) and axial eye growth in young myopic children undergoing orthokeratology (ortho-k) treatment., Methods: Axial length and ocular HOA, measured under cycloplegia annually over a 2-year period from the right eyes of myopic children, who previously completed ortho-k clinical trials, were retrieved. Linear mixed model analyses were applied to determine the association between ocular HOA, other known confounding variables (age, sex, and refractive error), and axial eye growth., Results: Data from 103 subjects were analyzed. The root-mean square (RMS) values of total ocular HOA (third to sixth orders combined), spherical (\({\rm{Z}}&#95;4^0\) and \({\rm{Z}}&#95;6^0\) combined), and comatic (\({\rm{Z}}&#95;3^{ - 1}\), \({\rm{Z}}&#95;3^1\), \({\rm{Z}}&#95;5^{ - 1}\), and \({\rm{Z}}&#95;5^1\) combined) aberrations increased by approximately 3, 9, and 2 times, respectively, after 2 years of ortho-k treatment. After adjusting for age, sex, and refractive error, higher RMS values of total HOA and spherical aberrations were associated with both longer axial length and slower axial elongation (all P < 0.01). For individual Zernike term coefficients, a higher level of positive spherical aberration (\({\rm{Z}}&#95;4^0\)) was also associated with longer axial length and slower axial elongation (both P < 0.01), after adjusting for baseline HOA., Conclusions: Ortho-k for myopia control significantly increases the Zernike coefficients and therefore the RMS values for a range of total ocular HOA terms or metrics in children. These findings suggest the potential role of HOA, particularly spherical aberration, as the possible mechanism of slowing axial elongation in ortho-k treatment.

Purpose: A survey in 2015 identified a high level of eye care practitioner concern about myopia with a reported moderately high level of activity, but the vast majority still prescribed single vision interventions to young myopes. This research aimed to update these findings 4 years later., Methods: A self-administrated, internet-based questionnaire was distributed in eight languages, through professional bodies to eye care practitioners globally. The questions examined: awareness of increasing myopia prevalence, perceived efficacy of available strategies and adoption levels of such strategies, and reasons for not adopting specific strategies., Results: Of the 1336 respondents, concern was highest (9.0 ± 1.6; p < 0.001) in Asia and lowest (7.6 ± 2.2; p < 0.001) in Australasia. Practitioners from Asia also considered their clinical practice of myopia control to be the most active (7.7 ± 2.3; p < 0.001), the North American practitioners being the least active (6.3 ± 2.9; p < 0.001). Orthokeratology was perceived to be the most effective method of myopia control, followed by pharmaceutical approaches and approved myopia control soft contact lenses (p < 0.001). Although significant intra-regional differences existed, overall, most practitioners did not consider single-vision distance under-correction to be an effective strategy for attenuating myopia progression (79.6 %), but prescribed single vision spectacles or contact lenses as the primary mode of correction for myopic patients (63.6 ± 21.8 %). The main justifications for their reluctance to prescribe alternatives to single vision refractive corrections were increased cost (20.6 %) and inadequate information (17.6 %)., Conclusions: While practitioner concern about myopia and the reported level of activity have increased over the last 4 years, the vast majority of eye care clinicians still prescribe single vision interventions to young myopes. Purpose: To evaluate changes in refractive and corneal responses in myopic children wearing orthokeratology (ortho-k) lenses with conventional compression factor (CCF, 0.75 D) and increased compression factor (ICF, 1.75 D)., Methods: This was a double-blind self-controlled study. Subjects were randomly fitted with CCF in one eye and ICF in the fellow eye. Weekly monitoring of refraction, visual acuity (VA), external ocular health, and corneal responses were performed over one month of lens wear and after discontinuation of lens wear until stabilization was achieved., Results: Twenty-five subjects, aged 9.4 ± 1.0 years, completed one-month lens wear, of whom 23 completed the washout period. The first fit success rates for CCF and ICF were 93% and 96%, respectively. Myopia, unaided VA, central corneal thickness, anterior corneal curvatures (K), apical power (AP), and corneal resistance factor (CRF) all changed significantly during the study period (P <  0.003). Between-eye difference was significant only for myopia reduction (P =  0.001). About 4% and 8% of ICF and CCF eyes were under-corrected. The attempted target was achieved in about 80% of CCF and 60% of ICF eyes at week 4. At the end of the washout period (3 weeks), myopia, K, and AP were significantly different compared to baseline (P <  0.001)., Conclusion: Increasing the compression factor did not affect the first fit success rate and external ocular health, but did allow faster correction of refractive error within one month of lens wear. Background: To investigate the influence of compression factor upon changes in ocular higher-order aberrations (HOAs) in young myopic children undergoing orthokeratology treatment., Methods: Subjects aged between six and < 11 years, with low myopia (0.50-4.00 D inclusive), low astigmatism (≤ 1.25 D), and anisometropia (≤ 1.00 D), were randomly assigned to wear orthokeratology lenses of different compression factors in each eye (one eye 0.75 D and the fellow eye 1.75 D). HOAs were measured weekly over one month of lens wear. Wavefront analysis was conducted over a 5-mm pupil using a sixth order Zernike polynomial expansion. Linear mixed models were used to examine the individual Zernike co-efficients and specific root-mean-square (RMS) error (spherical, comatic, total HOAs) metrics and their changes between the two eyes during the study period., Results: Twenty-eight myopic (mean manifest spherical equivalent refraction: -2.10 ± 0.58 D) children (median [range] age: 9.3 [7.8-11.0] years) were analysed. Significant interocular differences in HOAs at baseline were observed for Z 6 - 6 and Z 6 - 4 only (both p < 0.05). During the lens wear period, eyes fitted with the increased compression factor showed greater changes in primary spherical aberration ( Z 4 0 , p = 0.04) and RMS values for spherical and total HOAs (both p < 0.01). Considering data from both eyes together, after adjusting for the paired nature of the data, some other Zernike terms ( Z 3 1 and Z 6 0 , both p < 0.01) and the RMS value of comatic aberrations (p < 0.001) significantly increased after one month of orthokeratology treatment. The increase in primary spherical aberration ( Z 4 0 ) was positively correlated with the reduction in spherical equivalent refractive error, but only in eyes fitted with the increased compression factor (r = 0.69, p < 0.001)., Conclusions: Increasing the orthokeratology compression factor by 1.00 D significantly altered some HOAs, particularly spherical aberration. Given the association between positive spherical aberration and eye growth in children, further research investigating the influence of orthokeratology compression factor on axial eye growth is warranted., (© 2019 Optometry Australia.)

Purpose: This study aimed to develop evidence-based guidelines for identifying suitable subjects for myopia control using orthokeratology (ortho-k)., Methods: Changes in axial elongation (AE) in the worse eyes of 66 myopic children (myopia between -0.75 and -4.50 D and astigmatism <2.00D, and aged 6 to <16 years old) who wore single-vision spectacles for seven months before switching to orthokeratology treatment for another seven months were observed., Results: AE during ortho-k lens wear was affected by age and rate of progression during spectacle wear. The percentage of subjects with rapid, moderate, and slow AE (equivalent annual myopia progression; rapid: ≥1.00D; moderate: 0.50D to <1.00D; slow: <0.50D) during spectacle wear was 36.3%, 33.3% and 30.3%, respectively. Rapid progression was most common in subjects aged less than nine, but 25% of subjects aged 9 to <13 and 12.5% aged 13 to <16 also demonstrated rapid progression. All subjects with rapid AE during spectacle wear achieved a significant reduction in eye elongation with ortho-k lens wear. Guidelines for patient selection was proposed based on the initial age and history of myopia progression., Conclusions: Myopia control is indicated for children at risk of developing high myopia. The prevalence of myopia in children is increasing worldwide and is viewed as a major public health concern. This increase has driven interest in research into myopia prevention and control in children. Although there is still uncertainty in the risk factors underlying differences in myopia prevalence between ethnic groups, rates in children of East Asian descent are typically higher regardless of where they live. Mounting evidence also suggests that myopia prevalence in children increases with age. Earlier commencement and more rigorous education systems in these countries, resulting in more time spent on near-work activities and less time on outdoor activities, may be responsible for the earlier age of myopia onset. However, to date, the mechanisms regulating myopia onset and progression are still poorly understood. Findings from several studies have shown orthokeratology to be effective in slowing axial elongation and it is a well-accepted treatment, particularly in East Asian regions. While our understanding of this treatment has increased in the last decade, more work is required to answer questions, including: How long should the treatment be continued? Is there a rebound effect? Should the amount of myopia control be increased? To whom and when should the treatment be offered? Practitioners are now faced with the need to carefully guide and advise parents on whether and when to undertake a long somewhat complex intervention, which is costly, both in time and money. In the near future, a greater demand for effective prophylaxis against childhood myopia is envisaged. Other than orthokeratology, atropine therapy has been shown to be effective in slowing myopia progression. While its mechanism of control is also not fully understood, it is likely that it acts via a different mechanism from orthokeratology. Purpose : The purpose of this study is to present the study design and one month's preliminary results of a 2-year randomized trial, Combined Atropine with Orthokeratology (AOK), for myopia control study. Methods : Children aged 6 to <11 years and with 1.00-4.00 D myopia were randomly assigned to AOK group or ortho-k alone (OK) group. Subjects are required to attend routine ortho-k aftercare visits (first-overnight, 1 week, 2 weeks, 3 weeks, 1 month, and every 3 months after commencement of lens wear). Clinical outcomes, including lens performance, changes in refractive error, unaided vision, ocular adverse events, corneal staining, lens binding and centration, and axial length, are also assessed at 6-monthly data collection visit. Results : Data of 30 AOK and 34 OK subjects who had completed the 1-month visit were analyzed. No significant differences in baseline data were found between the two groups ( P > 0.05). At the 1-month visit, first-fit success rate was 95%, with full myopia reduction. Mild corneal staining was observed in 23.3% and 30.9% and mild self-reported lens binding in 50% and 41% in AOK and OK groups, respectively, after 1-month lens wear. Mean (±standard deviation) change in axial length was significantly higher in AOK than OK subjects (AOK: -0.05 ± 0.05 mm; OK: -0.02 ± 0.03 mm) ( P = 0.003). Conclusions : After 1 month of treatment, first-fit success rate of ortho-k lenses was high in both groups of subjects; addition of 0.01% atropine in AOK subjects did not affect the lens performance or clinical responses. These results provide the assurance that it is acceptable to continue this longitudinal study, as longer study duration, usually 2 years, is required to determine the effectiveness of treatment for myopia control.

Myopia has been predicted to affect approximately 50% of the world's population based on trending myopia prevalence figures. Critical to minimizing the associated adverse visual consequences of complicating ocular pathologies are interventions to prevent or delay the onset of myopia, slow its progression, and to address the problem of mechanical instability of highly myopic eyes. Although treatment approaches are growing in number, evidence of treatment efficacy is variable. This article reviews research behind such interventions under four categories: optical, pharmacological, environmental (behavioral), and surgical. In summarizing the evidence of efficacy, results from randomized controlled trials have been given most weight, although such data are very limited for some treatments. The overall conclusion of this review is that there are multiple avenues for intervention worthy of exploration in all categories, although in the case of optical, pharmacological, and behavioral interventions for preventing or slowing progression of myopia, treatment efficacy at an individual level appears quite variable, with no one treatment being 100% effective in all patients. Further research is critical to understanding the factors underlying such variability and underlying mechanisms, to guide recommendations for combined treatments. There is also room for research into novel treatment options.

Purpose: To compare changes in endothelial morphology in the central and superior cornea in subjects wearing single-vision spectacles and orthokeratology lenses over two years., Methods: Endothelial images of the two locations of 99 subjects (6-12 years) from completed myopia control studies were analysed. Endothelial cell density (ECD), coefficient of variation in cell size (CV), and hexagonality (HEX) before and two years after treatment were compared between the two groups of subjects., Results: Baseline ECD and CV in the central cornea were slightly lower than those in the superior cornea, but no significant difference in HEX was found in the two corneal locations. After two years, reduction in ECD and increase in CV were only significant in the central cornea, but not in the superior cornea. Reduction in HEX was significant in both corneal locations. Subjects receiving orthokeratology had smaller reduction in ECD in the central cornea compared to the controls (orthokeratology: 56±94 cells/mm 2 ; control: 98±91 cells/mm 2 , p=0.024), otherwise, there were no significant differences in the changes in endothelial morphology in the two corneal locations between the two groups of subjects., Conclusions: The current study confirmed that there were differences in endothelial morphology of central and superior cornea of Chinese children aged 6-12 years. The morphological response to normal ageing differed between the two corneal locations as reduction in cell density and polymegathism were found only in the central cornea whilst pleomorphism was found in both locations. Purpose: To determine the relative risk of rapid progression and number needed to treat (NNT) in younger and older children using combined data from the retardation of myopia in orthokeratology (ROMIO) and toric orthokeratology-slowing eye elongation (TO-SEE) studies., Methods: Data from 136 subjects of two studies, ROMIO and TO-SEE, were retrieved (72 orthokeratology [ortho-k]: 37 ROMIO, 35 TO-SEE; 64 control: 41 ROMIO, 23 TO-SEE) and the myopia control effect on younger (6-8 years) and older (9-12 years) subjects evaluated. The rate of axial elongation was classified as not rapid (axial elongation = <0.36 mm/year) or rapid (axial elongation >0.36 mm/year)., Results: Cumulative frequency curves showed that the younger subjects in the control group had the greatest and most rapid axial elongation at the end of 24 months. In the younger subjects, ortho-k lens wear significantly reduced the risk of rapid progression by 88.8% (P = 0.002). The 2-year NNT for the younger ortho-k subgroup was 1.8, suggesting that treating just two younger subjects with ortho-k would prevent one subject from experiencing rapid progression over a 2-year period of treatment. The 2-year NNT for the older ortho-k subgroup was 11.8, which was statistically insignificant (P = 0.197)., Conclusions: Orthokeratology significantly reduced risk of rapid progression in younger subjects. Treating just two 6- to 8-year-old subjects with ortho-k instead of single-vision spectacles could prevent one subject from developing rapidly progressing axial elongation during this critical 2-year period.

Purpose: To compare the effects of normal growth and longer term use of orthokeratology (ortho-k) on ocular biometric parameters in the anterior segment, including central corneal thickness (CCT), anterior chamber depth (ACD), crystalline lens thickness (CLT), and anterior segment length (ASL)., Methods: Baseline and six monthly data were retrieved from 78 subjects (aged 7-10 years, with myopia≤4.00D and astigmatism≤1.25D) who had completed a two-year randomized clinical trial using ortho-k for myopia control. They were randomly assigned to wear ortho-k lenses or single-vision spectacles (control). Anterior segment parameters were measured with the Pentacam after cycloplegia., Results: No significant changes in CLT and ASL over time were observed in either group of subjects (37 ortho-k; 38 control). In the control group, CCT remained unchanged during the study period but in the ortho-k group, it was significantly reduced by an average of 0.009mm by the 6-month visit (p<0.001) and remained unchanged thereafter. No significant changes in ACD was found in the ortho-k group but it was significantly increased by an average of 0.04mm (p=0.001) in the control group., Conclusion: CLT nor ASL did not change over time in either control or ortho-k subjects. Although ACD significantly increased in the control subjects and CCT significantly reduced in the first six months of ortho-k lens wear, these changes were small and did not affect the overall ASL., (Copyright © 2016 British Contact Lens Association. Purpose: Myopia is a global public health issue; however, no information exists as to how potential myopia retardation strategies are being adopted globally., Methods: A self-administrated, internet-based questionnaire was distributed in six languages, through professional bodies to eye care practitioners globally. The questions examined: awareness of increasing myopia prevalence, perceived efficacy and adoption of available strategies, and reasons for not adopting specific strategies., Results: Of the 971 respondents, concern was higher (median 9/10) in Asia than in any other continent (7/10, p<0.001) and they considered themselves more active in implementing myopia control strategies (8/10) than Australasia and Europe (7/10), with North (4/10) and South America (5/10) being least proactive (p<0.001). Orthokeratology was perceived to be the most effective method of myopia control, followed by increased time outdoors and pharmaceutical approaches, with under-correction and single vision spectacles felt to be the least effective (p<0.05). Although significant intra-regional differences existed, overall most practitioners 67.5 (±37.8)% prescribed single vision spectacles or contact lenses as the primary mode of correction for myopic patients. The main justifications for their reluctance to prescribe alternatives to single vision refractive corrections were increased cost (35.6%), inadequate information (33.3%) and the unpredictability of outcomes (28.2%)., Conclusions: Regardless of practitioners' awareness of the efficacy of myopia control techniques, the vast majority still prescribe single vision interventions to young myopes. In view of the increasing prevalence of myopia and existing evidence for interventions to slow myopia progression, clear guidelines for myopia management need to be established., (Copyright © 2016 Elsevier Ltd. Purpose: To solicit parents' knowledge on myopia control strategies available; to investigate their attitudes toward the use of orthokeratology (ortho-k), daily wear soft lenses, and spectacles for myopia control, assuming all three optical strategies were equally effective for myopia control; and to collect their opinion on the age at which children can commence contact lens wear for vision correction., Methods: Telephone interviews were conducted on parents who responded to the advertisement for two myopia control studies, one on ortho-k and one on soft lenses., Results: Most of the 196 respondents (ortho-k group, 56%; soft lens group, 44%) were mothers, aged 36 to 45 years. Ortho-k was the most commonly known myopia control strategy (86%). Parents in the ortho-k group selected ortho-k (49%) and spectacles (45%) (p < 0.001) as their preferred myopia control strategy, whereas parents in the soft lens group had no preference (p = 0.57). Decision was affected by prior knowledge of myopia control strategy. Major considerations were convenience and safety among parents selecting contact lenses and spectacles, respectively, as their preferred options. Overall, because of the lack of confidence in safety in contact lens wear, most parents (75%) were of the opinion that contact lenses for vision correction were only for children older than 14 years (53%) and not indicated for children younger than 8 years., Conclusions: Parents in Hong Kong were open to the use of contact lenses for myopia control in young children and they appeared to regard myopia control contact lenses as "treatment" and not as visual aids. They were more conservative in the use of contact lenses for vision correction in children. Parents' decision in selecting a myopia control strategy, assuming all three strategies to be equally effective, was affected by an individual's concerns of safety and additional benefits as well as influenced by positive evidence on myopia control effects.

Purpose: To compare the axial length elongation and change in refractive errors in a pair of identical twins wearing orthokeratology (ortho-k) and single vision lenses (SVLs), respectively., Case Report: Identical Twin A and B, who were 8 years of age, with the same amount of near activities, were assigned to wear ortho-k and SVLs randomly and they were monitored for two years for myopic progression. Twin A and B were assigned to wear ortho-k and SVLs, respectively. Myopic progression was evaluated by the change in axial length and in refractive errors. A faster axial length elongation was observed in each eye of Twin B during the two-year study period. The overall change in axial length was 0.52mm (OD) and 0.70 (OS) in Twin A and 0.77mm (OD) and 0.82mm (OS) in Twin B. In terms of cycloplegic refractive errors (SER), one month after ceasing lens wear (after completion of the two-year study), the increase (from baseline) were 11% (OD) and 48% (OS) in Twin A and 87% (OD) and 67% (OS) in Twin B., Conclusions: Ortho-k is more effective in controlling myopic progression in terms of axial elongation than wearing SVLs in this pair of identical twins., (Copyright © 2013 British Contact Lens Association. Purpose: This nonrandomized clinical study aimed to investigate the effectiveness of toric orthokeratology (ortho-k) for myopia control in myopic children with moderate-to-high astigmatism, Methods: We enrolled 80 subjects (aged 6-12 years; ortho-k, 43; control, 37) with myopia of 0.50 to 5.00 diopters (D), and with-the-rule astigmatism of -1.25 to -3.50 D, and unremarkable ocular and general conditions. Data collection, including visual acuity, subjective and objective refraction, axial length, corneal topography, and biomicroscopy examination, was performed every 6 months during the 24-month study period. Results from the right eye or the eye with higher astigmatism were reported., Results: A total of 35 ortho-k and 23 control subjects completed the study successfully. Subjects in both groups demonstrated axial elongation (P < 0.001). The average axial elongation at the end of study was 0.31 ± 0.27 and 0.64 ± 0.31 mm in the ortho-k and control groups, respectively (P < 0.001). At the end of 24 months, axial elongation in ortho-k subjects was 52% slower than that in the control group. Axial elongation was correlated significantly with the initial age of the subjects (P = 0.02) and treatment assigned (P = 0.04), but not with sex, initial myopia, initial refractive cylinder, or initial corneal toricity (P > 0.08)., Conclusions: Toric ortho-k lenses can slow axial elongation effectively in myopic children with moderate-to-high astigmatism. Purpose: To report the study design and preliminary results of a pilot study, High Myopia-Partial Reduction Orthokeratology study., Methods: Children with myopia of 6.00D or above and who satisfied the recruitment criteria were randomly assigned to partial reduction orthokeratology (PR ortho-k) and spectacle-wearing control groups. The myopia of the PR ortho-k children were partially reduced using custom made 4-zone ortho-k lenses of target 4.00D. Residual refractive errors were corrected with single vision spectacles. Control subjects were fully corrected with single vision spectacles. PR ortho-k subjects were also required to return for assessment after the first overnight lens wear, and one week and one month after lens wear., Results: Fifty-two eligible subjects were randomly assigned to PR ortho-k group (n=26) and control group (n=26). The median age of each group was 10.00 years. The median (range) subjective myopia of the right eye at baseline was 6.41D (5.00-8.00D) and 6.22D (6.00-8.00D) for PR ortho-k and spectacle groups, respectively (p>0.05). Nineteen (79%) PR ortho-k subjects achieved successful lens fit at the one month visit and the median myopic reduction was 3.75D in the right eye. The incidence of (mild) corneal staining in PR ortho-k subjects reduced from 30% at the first overnight lens wear to 16% at the 1-month visit. Corneal pigmented arc was observed in 32% of PR ortho-k subjects at the 1-month visit., Conclusions: PR ortho-k was successfully applied to high myopic children with no significant changes in ocular health/best corrected visual acuity after one month of lens wear., (Copyright © 2013 British Contact Lens Association. Purpose: To determine the peripheral refraction of children with different types of ametropias and to evaluate the relationship between central refractive changes, baseline relative peripheral refraction (RPR) and changes in RPR over a 12-month monitoring period., Methods: Cycloplegic central and peripheral refraction were performed biannually on the right eyes of children aged 6-9 for 12 months, using an open-view autorefractor. Peripheral refraction were measured along 10°, 20° and 30° from central fixation in both nasal and temporal fields. Refractive data were transposed into M, J0 and J45 vectors for analyses. RPR was determined by subtracting the central measurement from each peripheral measurement., Results: Hyperopic eyes showed relative peripheral myopia while myopic eyes had relative hyperopia across the central 60° horizontal field at baseline. Emmetropic eyes had relative myopia within but showed relative hyperopia beyond the central 30° field. However, there was no significant correlation between central refractive changes and baseline RPR or between changes in central refraction and RPR over twelve months in any refractive groups. Correlations between changes in PR and central myopic shift were found mainly in the nasal field in different groups. In the subgroup analysis on the initially emmetropic and the initially myopic groups, the subgroups with faster myopic progression did not have significantly different RPR from the subgroups with slower progression. The RPR pattern of the initially emmetropic and the initially myopic groups became more asymmetric at the end of the study period with a larger increase in relative hyperopia in the temporal field., Conclusions: RPR patterns were different among hyperopic, emmetropic and myopic eyes. However, baseline RPR and changes in RPR cannot predict changes in central refraction over time. Purpose: To investigate the short-term effect of orthokeratology (ortho-k) lens wear on the anterior segment length for validating the use of axial length for monitoring myopic progression after ortho-k treatment., Methods: THIRTY-SEVEN AND 39 SUBJECTS (AGES: 7-10 years) were randomly assigned to wear ortho-k and single-vision spectacles, respectively. Central corneal thickness (CCT), anterior chamber depth (ACD), crystalline lens thickness (LT), and anterior segment length (ASL: summation of CCT, ACD, and LT) were measured before and 6 months after the treatment under cycloplegia. Changes in these parameters were evaluated and compared between the two groups of subjects., Results: There were no significant between-group differences in the baseline data (P > 0.37). After 6 months of lens wear, in the ortho-k group, CCT was significantly reduced by 0.009 ± 0.009 mm (P < 0.001), whereas ACD and LT remained unchanged (P > 0.15). In the spectacle group, ACD was significantly increased by 0.01 ± 0.03 mm (P = 0.008), whereas CCT and LT remained unchanged (P > 0.06). In both groups of subjects, ASL did not appreciably change but axial length was significantly increased by 0.10 ± 0.10 mm and 0.20 ± 0.11 mm in the ortho-k and the spectacle groups, respectively (P < 0.001)., Conclusions: Eyeball elongation occurred in children wearing both ortho-k and single-vision spectacles. Since ASL was not affected by ortho-k treatment, axial length measured reflects the true growth of the eyeball and is a valid parameter for monitoring myopic progression in ortho-k treated eyes.

Purpose: This single-masked randomized clinical trial aimed to evaluate the effectiveness of orthokeratology (ortho-k) for myopic control., Methods: A total of 102 eligible subjects, ranging in age from 6 to 10 years, with myopia between 0.50 and 4.00 diopters (D) and astigmatism not more than 1.25D, were randomly assigned to wear ortho-k lenses or single-vision glasses for a period of 2 years. Axial length was measured by intraocular lens calculation by a masked examiner and was performed at the baseline and every 6 months. This study was registered at ClinicalTrials.gov, number NCT00962208., Results: In all, 78 subjects (37 in ortho-k group and 41 in control group) completed the study. The average axial elongation, at the end of 2 years, were 0.36 ± 0.24 and 0.63 ± 0.26 mm in the ortho-k and control groups, respectively, and were significantly slower in the ortho-k group (P < 0.01). Axial elongation was not correlated with the initial myopia (P > 0.54) but was correlated with the initial age of the subjects (P < 0.001). The percentages of subjects with fast myopic progression (>1.00D per year) were 65% and 13% in younger (age range: 7-8 years) and older (age range: 9-10 years) children, respectively, in the control group and were 20% and 9%, respectively, in the ortho-k group. Five subjects discontinued ortho-k treatment due to adverse events., Conclusions: On average, subjects wearing ortho-k lenses had a slower increase in axial elongation by 43% compared with that of subjects wearing single-vision glasses. Purpose: To report the clinical performance of the orthokeratology (ortho-k) lens fitted with computer assisted system after 1-month of lens wear, in a group of children undergoing ortho-k treatment in a 2-year randomized myopic control study., Method: Children aged 6-11 years old were fitted with the ortho-k lenses using computerized fitting. The initial myopia was 4.00-0.50 D and the initial refractive astigmatism was within 1.25 D. Lens performance, in terms of centration, myopic reduction, vision, ocular health status and lens binding incidence, was evaluated at one night, one week and one month after lens wear. Only data from the right eye was presented., Results: The initial spherical equivalent refraction (SER) for the 51 subjects was -2.29 ± 0.81 D. The first fit success rate was 90%. The reduction of SER after one night and one week aftercare visit were 57% and 81%, respectively. At the one month visit, the mean reduction in SER was 89% with unaided logMAR visual acuity of 0.03 ± 0.11. Mild central corneal staining was found in 9-20% of the subjects at the aftercare visits. The incidences of lens binding at one night, one week and one month aftercare visits were 17%, 39% and 30%, respectively., Conclusions: Computer assisted system for Menicon Z Night lens fitting gave a high first fit success rate. Menicon Z Night lens was effective in myopic reduction and provided stable vision after one week of lens wear. Purpose: To determine the repeatability of corneal hysteresis (CH) and corneal resistance factor (CRF) measurements in children wearing spectacles and children under orthokeratology (ortho-k) therapy using the Ocular Response Analyzer (ORA)., Methods: CH and CRF were measured twice at a 10-min interval using ORA on the same day in both eyes of 25 children (mean age = 10.6 ± 1.2 years) wearing spectacles and 34 children (mean age = 10.9 ± 1.0 years) wearing ortho-k lenses. Four measurements were obtained from each eye in each set of measurements. Only data from the right eyes were analyzed., Results: No significant between-measurement differences in CH and CRF were found in either group of subjects (paired t-tests, p > 0.05) and no correlations were found between the mean differences and their means (Pearson's correlations, -0.09 < r < 0.21, 0.24 < p < 0.85). The 95% limits of agreement (LA) were -1.87 mmHg to +2.59 mmHg and -1.53 mmHg to +1.41 mmHg for CH and -1.86 mmHg to +2.22 mmHg and -1.45 mmHg to +1.57 mmHg for CRF in the spectacle and ortho-k groups, respectively. A significant between-group difference in CRF was found (unpaired t-tests, p = 0.02)., Conclusions: Repeatability of CH and CRF measurements using the ORA is within ± 2 mmHg in children wearing spectacles or ortho-k lenses. Purpose: To investigate the effect of lens fenestrations on the performance of orthokeratology lenses., Methods: Twenty-two subjects (aged 11 to 31 years) were fitted with identical reverse geometry orthokeratology lenses in the two eyes. One eye was randomly designated to wear a lens with three 0.20 mm fenestrations at 120° intervals placed at the junction of the reverse and alignment curves. The lens for the other eye was not fenestrated. Subjects were reviewed at 1 week, 1 month, 3 months, 6 months, and 1 year after fitting. Data were collected on refraction, visual performance, incidences and severity of corneal staining, lens binding, and corneal pigmented arc., Results: Fifteen subjects achieved full correction in OU. There were no statistically significant differences in refractive and corneal changes, visual performance, incidences and severity of corneal staining and corneal pigmented arc formation between the two eyes. The incidence of lens binding was consistently higher in eyes wearing non-fenestrated lenses but was not statistically significant. Severity of lens binding, however, was statistically significant at the 1-, 3-, 6- and 12-month visits, with the non-fenestrated lenses causing more severe binding., Conclusions: The addition of fenestrations to orthokeratology lenses has no effect on the efficacy of the treatment or outcomes in low to moderate myopic subjects. However, the severity of lens binding is reduced. Fenestrating reverse geometry lenses may be of assistance in cases where lens binding is a problem.

Purpose: The aim was to determine the corneal parameters in low to moderate myopic Chinese children and to investigate the differences in these corneal parameters between male and female subjects., Methods: Refractive errors and corneal parameters were retrieved from subjects who had participated in studies on myopia and astigmatism in 2008 and 2009. Corneal parameters including simulated K (Sim K) and asphericity (Q) at 9.0 mm chord and horizontal visible iris diameter (HVID) were determined for 217 children (112 males and 105 females aged from six to 12 years) using the Medmont E300 topographer., Results: The Q of the corneas was -0.44 ± 0.12 along the flat meridian and -0.22 (-0.70 to 0.63) along the steep meridian. The HVID and Sim K of male subjects were larger and flatter than female subjects. The mean HVID of male and female subjects was 11.3 ± 0.3 mm and 11.1 ± 0.30 mm, respectively. The mean Sim K value in male and female subjects was 7.91 ± 0.24 mm and 7.79 ± 0.21 mm, respectively, along the flat meridian, and was 7.65 ± 0.26 mm and 7.53 ± 0.24 mm, respectively, along the steep meridian., Conclusions: All corneas were prolate elliptical in shape along the flat meridian and the peripheral flattening rate was found to be greater along the flat meridian. A greater flattening rate was observed in the corneas of Chinese children when compared to that of Caucasian children. The Q between male and female subjects was not significantly different.
