Background: Ureteral stricture disease is a troubling urologic issue that can be managed with surgical reconstruction or, more conservatively, with chronic nephrostomy tubes or ureteral stents. These indwelling tubes require exchanges and are prone to complications such as encrustation or stent failure. Metallic ureteral stents are designed to be more resistant to extrinsic compression and allow for exchanges at longer intervals. However, encrustation or tissue ingrowth can occur with these stents as well. The removal of encrusted or embedded metallic ureteral stents poses a difficult clinical scenario. We present a case of an encrusted metallic stent embedded in a proximal ureteral stricture requiring percutaneous endoscopic removal with a novel looped-wire technique. Case Presentation: A 50-year-old Caucasian man with bilateral ureteral stricture disease, managed with chronic indwelling metallic stents, failed retrograde removal on the right during routine exchange. Staged procedures with percutaneous nephrostomy, followed by combined percutaneous antegrade and retrograde endoscopy were required to observe and access the embedded stent. The exposed metallic surface was unable to be grasped by available instruments through flexible endoscopy. Under endoscopic control with fluoroscopic guidance, a polytetrafluoroethylene (PTFE)-coated guidewire was looped around the metallic stent. With gentle traction on the wire loop, the embedded stent curl was delivered out of the stricture and into the renal pelvis from where it was extracted carefully with graspers inserted through a rigid nephroscope. Follow-up antegrade fluoroscopic studies with contrast showed no extravasation. Conclusion: Percutaneous removal of metallic stents retained within the ureter has unique challenges. We present a novel method of extraction of a retained metallic stent with a looped PTFE-coated guidewire, which may safely and effectively be used in complex situations.

This chapter summarizes the results of the Lower Pole Study I study in which patients with symptomatic lower pole renal calculi were randomized to receive extracorporeal shock wave lithotripsy (ESWL) versus percutaneous nephrolithotomy (PCNL). The study found that patients receiving ESWL had lower stone-clearance rates and a more frequent need for retreatment but lower morbidity, whereas those receiving PCNL had higher stone-clearance rate and fewer retreatments but greater morbidity.

Background: Periureteral venous rings are a rare congenital anomaly involving the inferior vena cava (IVC) and the right ureter, where the ureter courses through a venous ring made by the duplication of the IVC during embryogenesis. This anatomic anomaly is also referred to as a transcaval ureter. Although most patients are asymptomatic and radiographic findings are incidental, some patients can be symptomatic. We present the first reported case of asymptomatic obstructive ureterolithiasis at the level of a periureteral venous ring that was effectively treated with endoscopic management. Case Presentation: A 47-year-old woman was found to have right hydroureteronephrosis on MRI. Further CT imaging showed an obstructing ureteral stone at the level of a periureteral venous ring. After initial decompression with ureteral stenting, she underwent ureteroscopy that revealed the ureteral stone at the level of the venous anomaly. The stone was fragmented and removed with laser lithotripsy and stone basket manipulation. After a period of ureteral stenting and removal, she had improved hydroureteronephrosis, no symptoms of ureteral obstruction, and stable renal function. Given these findings, she elected for surveillance with imaging in lieu of any reconstructive procedure to transpose the ureter around the venous anomaly. Conclusions: We present the first reported case of obstructive ureterolithiasis at the level of a periureteral venous ring. Our experience suggests that, with preoperative ureteral stenting, obstructing ureteral stones in the setting of an IVC anomaly can be managed with retrograde flexible ureteroscopy. Conservative laser settings and minimal torqueing of the ureteroscope are advised given adjacent vascular anomaly. Cases wherein the affected ureteral segment is too constricted or tortuous to allow for stone passage or for ureteroscopy may require management by percutaneous antegrade intervention. Surgical reconstruction of the ureter should also be considered.

Aspirin is often stopped prior to percutaneous nephrolithotomy due to concern about the surgical bleeding risk. There is evidence that discontinuing aspirin perioperatively increases thromboembolic events and continuing it may be safe. We assessed the effect of continuing low dose aspirin through percutaneous nephrolithotomy and its effect on surgical and safety outcomes.We retrospectively reviewed the records of 285 consecutive percutaneous nephrolithotomies performed between 2012 and 2015 at our institution. We compared outcomes and complications in patients who continued 81 mg aspirin daily to those in patients not receiving aspirin.A total of 67 patients (24.5%) were maintained on low dose aspirin and 207 (75.5%) were not on aspirin. The aspirin group was older (66 vs 52 years), included more tobacco users (58.2% vs 31.4%) and had a higher ASA® (American Society of Anesthesiologists®) score (2.9 vs 2.5, all p0.001). There was no difference in mean S.T.O.N.E. (size, topography [stone location], obstruction, number of stones and evaluation of HU) score (7.6 vs 7.7, p = 0.71) or blood loss (44 vs 54 ml, p = 0.151). There was no difference in residual stone fragment size, including 0 to 2 mm in 65.3% vs 61.4% of aspirin vs no aspirin cases, 3 to 4 mm in 19.4% vs 16.2% and greater than 4 mm in 15.3% vs 22.4% (p = 0.407). Length of stay and the change in hemoglobin, hematocrit and creatinine were similar. There was no difference in the readmission rate (14.9% vs 12.6%, p = 0.618) or the total complication rate (34.4% vs 26.6%, p = 0.221). There was also no difference in the number of major complications (10.4% vs 5.8%, p = 0.193), bleeding complications (3.0% vs 2.9%, p = 0.971) and the transfusion rate (1.5% vs 1.0%, p = 0.57).Percutaneous nephrolithotomy appears effective and safe in patients who continue low dose aspirin perioperatively.

Proper fluoroscopic education and protocols may reduce the patient radiation dose but few prospective studies in urology have been performed. Using optically stimulated luminescent dosimeters we tested whether fluoroscopy time and/or entrance skin dose would decrease after educational and radiation reduction protocols.At default manufacturer settings fluoroscopy time and entrance skin dose were prospectively measured using optically stimulated luminescent dosimeters in patients undergoing ureteroscopy, retrograde pyelogram/stent or percutaneous nephrolithotomy with access for stone disease. A validated radiation safety competency test was administered to urology faculty and residents before and after web based, hands-on fluoroscopy training. Default fluoroscopy settings were changed from continuous to intermittent pulse rate and from standard to half-dose output. Fluoroscopy time and entrance skin dose were then measured again.The cohorts of 44 pre-protocol and 50 post-protocol patients with stones were similarly matched. The change in mean fluoroscopy time and entrance skin dose from pre-protocol to post-protocol was -0.6 minutes and -11.6 mGy (33%) for percutaneous nephrolithotomy (p = 0.62 and0.001), 0.5 minutes and -0.1 mGy (34%) for ureteroscopy (p = 0.42 and 0.31), and 0.1 minute and -0.1 mGy (29%) for retrograde pyelogram/stent (p = 0.85 and 0.49, respectively). Urologist post-training test scores increased 30% from pretraining scores (p = 0.1).Radiation safety training protocols improved clinical knowledge but did not significantly alter fluoroscopy time. Changing equipment default settings to intermittent pulse rate (12 frames per second) and half-dose lowered the entrance skin dose by 30% across all endourology patients but most significantly during percutaneous nephrolithotomy. To limit patient radiation exposure fluoroscopy default settings should be decreased before all endourology procedures and image equipment manufacturers should consider lowering standard default renal settings.

Several scoring systems have recently emerged to predict stone-free rate (SFR) and complications after percutaneous nephrolithotomy (PCNL). We aimed to compare the most commonly used scoring systems (Guy's stone score, S.T.O.N.E. nephrolithometry, and CROES nomogram), assess their predictive accuracy for SFR and other postoperative variables, and develop a risk group stratification based on these scoring systems.We performed a retrospective review of patients who have had a PCNL at four academic institutions between 2006 and 2013. Primary outcome was SFR within 3 weeks of the surgery and secondary outcomes were operative time (OT), complications, and length of stay (LOS). We performed chi-squared, t-test, logistic, linear, and Poisson regressions, as well as receiver operating characteristics curve with area under the curve (AUC) calculation.We identified 586 patients eligible for analysis. Of these, 67.4% were stone free. Guy's, S.T.O.N.E., and CROES score were predictive of SFR on multivariable logistic regression (odds ratio [OR]: 1.398, 95% confidence interval [CI]: 1.056, 1.852, p = 0.019; OR: 1.417, 85% CI: 1.231, 1.631, p 0.001; OR: 0.993, 95% CI: 0.988, 0.998, p = 0.004) and have similar predictive accuracy with AUCs of 0.629, 0.671, and 0.646, respectively. On multivariable linear regression, only S.T.O.N.E. was an independent predictor of longer OT (β = 14.556, 95% CI: 12.453, 16.660, p 0.001). None of the scores were independent predictors of postoperative complications or a longer LOS. Poisson regression allowed for risk group stratification and showed the S.T.O.N.E. score and CROES nomogram to have the most distinct risk groups.The three evaluated scoring systems have similar predictive accuracy of SFR. S.T.O.N.E. has additional value in predicting OT. Risk group stratification can be used for patient counseling. Further research is needed to identify whether or not any is superior to the others with regard to clinical usefulness and predictive accuracy.

In murine and human hyperoxaluric conditions macrophages can be seen surrounding renal calcium oxalate crystal deposits. We hypothesized that macrophages have a role in degrading and destroying these deposits. We investigated the inflammatory response and phagocytic mechanisms when macrophages were exposed to human kidney stones and inorganic crystals.Human monocytes were differentiated into resting, fully differentiated macrophages by treatment with recombinant human macrophage colony-stimulating factor (M-CSF) or GM-CSF (granulocyte M-CSF) for 6 days. After confirming phenotype by flow cytometry the macrophages were exposed for 20 hours to fragments of sterile human calcium oxalate stones or calcium oxalate crystals. Crystal uptake was determined, and supernatant cytokine and chemokine profiles were analyzed using antibody arrays. Quantitative reverse transcriptase-polymerase chain reaction was done to validate mRNA profile expression.Under direct vision fluorescence microscopy activated human macrophages were noted to surround stone fragments and synthesized crystals, and destroy them in a step-by-step process that involved clathrin mediated endocytosis and phagocytosis. An inflammatory cascade was released by macrophages, including the chemokines chemokine ligand (CCL)2, CCL3, interleukin (IL)-1 receptor antagonist (IL-1ra), complement component C5/C5a and IL-8. Response patterns to stone and crystal material depended on macrophage phenotype and activation status.In our in vitro study macrophages differentiated with M-CSF showed greater ability to phagocytize crystal deposits than those treated with GM-CSF. Following clathrin mediated endocytosis macrophages released a number of cytokines that are crucial for the inflammatory immune response. This suggests that tissue macrophages have an important role in preventing kidney stone disease by removing and digesting interstitial renal crystal deposits.

BACKGROUND: Molecular profiling of renal cell carcinomas (RCCs) may improve the distinction between oncocytoma and malignant RCC subtypes and aid in early detection of metastasis. The hyaluronic acid (HA) family includes HA synthases (HAS1, HAS2, HAS3), hyaluronidases (HYAL-1, HYAL-2, HYAL-3, HYAL-4, PH20, HYAL-P1), and HA receptors (CD44s, CD44v, RHAMM). HA family members promote tumor growth and metastasis. The authors evaluated the expression of HA family members in kidney specimens. METHODS: By using quantitative polymerase chain reaction, mRNA levels of 12 HA family members were measured in tumor specimens obtained from 86 consecutive patients undergoing nephrectomy; 80 of them also provided normal specimens. Mean and median follow-up were 15.2 ± 8.8 and 13.8 months. RCC specimens included clear cell RCC: 65; papillary: 10; chromophobe: 5; oncocytoma: 6; metastasis positive: 17. RESULTS: Median HAS1, CD44s, and RHAMM transcript levels were elevated 3- to 25-fold in clear cell RCC and papillary and chromophobe tumors when compared with normal tissues. HYAL-4, CD44s, and RHAMM levels were elevated 4- to 12-fold in clear cell RCC and papillary tumors when compared with oncocytomas; only HYAL-4 levels distinguished between chromophobe and oncocytoma (P = .009). CD44s and RHAMM levels were significantly higher in tumors

The laparoscopic approach to donor nephrectomy is becoming increasingly common. While it is felt that the recovery from laparoscopic nephrectomy is quicker and less painful, a number of complications have been reported. A rarely reported on complication in the literature with significant morbidity is ipsilateral orchalgia. From 1998 to 2008, 257 hand-assisted laparoscopic donor nephrectomies were performed at our institution. Eight of 129 (6.2%) men complained of de novo ipsilateral orchalgia postoperatively. The average duration of pain was 402 days. Patients reported significant morbidity related to this complication. None, however, required further treatment. Three patients reported that they would reconsider organ donation as a result of testicular pain. Our technique originally included dissection and ligation of the gonadal vein en bloc with the ureter at the level of the left common iliac artery. Since recognizing this complication, we have adopted a gonadal vein sparing approach so as not to disturb the vessel below its point of ligation at the renal vein. To date, 50 patients have undergone the modified technique without experiencing orchalgia. In conclusion, ipsilateral testicular pan is a relatively frequent complication of laparoscopic donor nephrectomy and may be a source of significant morbidity. Using a modified surgical technique, this complication can be reduced or eradicated.

Percutaneous nephrostolithotomy (PCNL) is the current standard of care for management of large renal stones (2 cm). Recent studies have evaluated flexible ureterorenoscopy (URS)/holmium laser lithotripsy as an alternative treatment for patients with contraindications to or preference against PCNL. Stones in an intermediate size range (2-3 cm) may be most amenable to URS/laser lithotripsy as definitive treatment in a single stage. We report a multi-institutional series of URS/laser lithotripsy for renal stone burdens that measure 2 to 3 cm.Patients who underwent URS/holmium laser lithotripsy for renal stones that measured 2 to 3 cm were identified retrospectively at three tertiary care centers. Demographic information, disease characteristics, and perioperative and postoperative data were gathered. Patients with renal stone burdens of 2 to 3 cm who were treated by URS/laser lithotripsy and had at least one postoperative visit and imaging study were included. Stone clearance was evaluated using 0-2 mm and4 mm residual stone burden on postoperative imaging.One hundred and twenty patients underwent URS/holmium laser lithotripsy for renal stones of 2 to 3 cm. Mean stone burden was 2.4 cm, and mean body mass index was 29.3 kg/m². Indications for URS/laser lithotripsy vs PCNL included patient preference (57), technical or anatomic factors (24), patient comorbidities (17), failed shockwave lithotripsy (9), patient body habitus (3), solitary kidney (3), chronic renal insufficiency (3), and strict anticoagulation (2). Thirty-one (26%) patients had stent placement preprocedure, and 94 (78%) patients underwent outpatient surgery. A ureteral access sheath was used in 67%. One hundred and one (84%) patients underwent single-stage procedures. There was one intraoperative complication (ureteral perforation), and there were eight minor postoperative complications (6.7%). The reoperation rate through the mean 18-month follow-up was 3/120 or 2.5%. Seventy-six (63%) patients had residual stone burden of 0 to 2 mm, and 100 (83%) patients had residual burden of4 mm.We demonstrate that single-stage URS/holmium laser lithotripsy is effective for management of renal stones that measure 2 to 3 cm through intermediate follow-up. Staged procedures can be used selectively for technical reasons or disease factors. Although PCNL achieves superior stone clearance overall, URS/laser lithotripsy is a viable treatment option for selected patients.

To evaluate LRN as treatment for high risk patients. Laparoscopic radical nephrectomy (LRN) is performed for renal tumors not amenable to nephron-sparing treatments. Indications are increasing to include higher risk patients including those with end-stage renal disease (ESRD) necessitating dialysis.We performed a retrospective analysis of a patient cohort with clinical stage T1 renal tumors undergoing transperitoneal LRN. Parameters examined included patient demographics, medical comorbidities, tumor characteristics, operative outcomes, and complications.One hundred eighty-nine patients underwent 195 LRN. Sixteen patients (8.5%) had preexistent ESRD requiring dialysis. A higher American Society of Anesthiologists score (P.05), higher age-adjusted Charlson comorbidity index (P=.003), higher incidence of previous abdominal surgery (P=.012), and higher incidence of hypertension (P=.025) were found for the ESRD group. Mean blood loss was 153.0 and 132.0 mL (P=.71) in the ESRD patients and non-ESRD patients, respectively. A longer stay (P=.02) was noted for ESRD patients. Mean tumor size in the ESRD patients and non-ESRD patients was 2.6 and 4.2 cm (P.05), respectively. Renal cell carcinoma was the most common pathology in 14 of 20 (70.0%) ESRD patient renal units and 167 of 175 (95.4%) non-ESRD patient renal units (P=.001). Intraoperative and postoperative complication rates were 6.3% and 31.3% respectively for ESRD patients (P=.05), and 8.7% and 21.4% respectively for non-ESRD patients (P=.35). Most postoperative complications were minor.LRN, for the treatment of renal tumors in ESRD patients requiring dialysis, is feasible and safe with acceptable intraoperative and postoperative complication rates. Patients with ESRD may require longer hospital stay after LRN.

Laparoscopic radical nephrectomy is commonly performed for renal tumors that are not amenable to nephron sparing treatment. A number of techniques for intact specimen extraction are used. The development of incisional hernias from the extraction site is a known but infrequent delayed complication. We analyzed different extraction sites and risk factors for such hernias.We retrospectively analyzed a cohort of patients undergoing laparoscopic radical nephrectomy with intact specimen extraction through 3 sites. Patients and operation specific parameters were included with particular attention to factors predisposing patients to incisional hernia, including chronic obstructive pulmonary disease, diabetes mellitus, chronic steroid use and a high body mass index.A total of 181 nephrectomies were performed in 175 patients and 175 kidneys (96.7%) had malignancy. Mean tumor size was 4.9 cm. Mean followup was 28.8 months. Extraction was done from a lower quadrant site in 55 patients (31.4%), from the umbilical site in 58 (33.2%) and from a paramedian site in 62 (35.4%). Patients with paramedian and lower quadrant extraction sites were older (p = 0.016), and had a higher body mass index (p = 0.001) and greater specimen weight (p = 0.003). In 4 patients an incisional hernia developed. An incisional hernia was significantly associated with the paramedian extraction site (p = 0.015).Incisional hernias may occur as a delayed complication of laparoscopic radical nephrectomy. This complication most commonly develops at the extraction site. In patients with a high body mass index using a paramedian extraction site is a significant risk factor for incisional hernia formation.

Laparoscopic-guided radiofrequency ablation (LRFA) has been introduced as a minimally invasive nephron-sparing management option for renal tumors. Many patients who desire treatment present with multiple comorbidities, which poses a therapeutic challenge. Our purpose is to determine if multipass LRFA is comparable, in terms of surgical risk and immediate postoperative outcomes, to laparoscopic partial nephrectomy (LPN).A retrospective study identified 36 and 33 patients who underwent LRFA and LPN, respectively. Perioperative demographic data, tumor characteristics, and follow-up data were evaluated. Statistical analysis was performed using the Student t test and chi-square analysis.Age, American Society of Anesthesiology score, and Charlson Comorbidity Index were significantly higher in the LRFA group than the LPN group (P0.001). Average tumor size was 2.8 cm and 3.1 cm for the LRFA and LPN groups, respectively. There were no significant differences in change between the preoperative and postoperative creatinine/glomerular filtration rate values or perioperative complication rates for the groups. Estimated blood loss and length of stay were significantly lower for the LRFA group than the LPN group (P0.05). Follow-up ranged 6 to 23 months and 6 to 58 months for the LRFA and the LPN groups, respectively. There has been no evidence of tumor recurrence in the follow-up period.We present our initial report comparing patients undergoing LRFA v LPN for the management of renal tumors. Our preliminary results with our experience with multipass laparoscopic-guided RFA demonstrate that this technique can be safely used in an elderly, higher risk population. Long-term follow-up is needed to determine oncologic efficacy.

A number of clinical variables are believed to be risk factors for complications of laparoscopic renal surgery. We reviewed our experience with laparoscopic surgery specifically for renal cancers to better clarify which clinical variables were significant risk factors.Our laparoscopic experience with 210 cases of renal cancer from April 1999 through August 2004 was reviewed. Preoperative clinical characteristics were recorded. Complete information was available for 134 patients: 54 radical nephrectomies, 41 nephroureterectomies, 19 radiofrequency ablations, and 20 partial nephrectomies. Outcomes monitored included blood loss, length of hospital stay, conversion, blood transfusion, and intraoperative, minor postoperative, and major postoperative complications. Multivariate analysis was performed to determine whether any variable was a significant risk factor for adverse outcomes during or after laparoscopic surgery.The numbers of patients requiring operative conversion or blood transfusions were 6 (4.5%) and 20 (14.9%), respectively. Intraoperative, minor postoperative, and major postoperative complication occurred in 9 (6.7%), 22 (16.4%), and 11 (8.2%) patients, respectively. The year surgery was performed was inversely proportional to the incidence of minor postoperative complications, implying a protective association with the experience of the surgeon. On multivariate analysis, only body mass index (BMI) was found to be a significant risk factor for major postoperative complications with an odds ratio of 1.14 (P = 0.03).Laparoscopic surgery is safe, but with every unit increase in the BMI, the risk of a major complication increases by 14%.

A duplex kidney associated with a poorly functioning upper-pole segment is commonly associated with incontinence, voiding dysfunction, and urinary tract infections. A standard treatment option for this condition is upper-pole heminephrectomy. With the continued development of minimally invasive urology, this technique can now be safely performed laparoscopically. This report details step by step our technique of laparoscopic upper-pole heminephrectomy. Key points include placement of a catheter in the normal ureter at the start of the case, full mobilization of the upper-pole ureter away from the renal hilum, and precise identification of the vasculature supplying the upper pole. Laparoscopic upper-pole heminephrectomy for ectopic ureter is safe and reproducible and offers the patient the typical postoperative benefits of laparoscopic surgery.

To determine the current practice patterns of a large group of urologists in the treatment of large renal stones.A survey was sent to all actively practicing members of the North Central Section of the American Urological Association. The questions pertained to age, time in practice, type of practice, time devoted to treating stones, residency training, case scenarios with treatment options, and whether they or a radiologist performed percutaneous access. The data were statistically analyzed.The response rate was 51% (564/1102 surveys returned). Three quarters (73%) of the urologists were comfortable performing percutaneous nephrolithotomy (PCNL), and 35% gave reasons they do not perform PCNL. Only 11% of those performing PCNL routinely obtained the percutaneous access themselves. Trends in the analysis included: (1) those trained to perform PCNL during residency were more often comfortable with this procedure; (2) younger urologists were more comfortable performing PCNL, even if they had been in practice for only a short time; (3) urologists in private practice were nearly as comfortable performing PCNL as were academic urologists; (4) urologists not comfortable with PCNL more often recommended SWL over PCNL as a primary treatment for moderate/large renal stones; and (5) few urologists routinely obtained percutaneous access themselves.Many urologists trained in recent years are comfortable performing PCNL. The type of training received influences treatment recommendations, and percutaneous access is most often obtained by/in conjunction with radiologists. This information may be useful in guiding residency training programs in the preparation of residents for the treatment of large renal stones.

Purpose: We assessed unenhanced helical computerized tomography (CT) secondary findings as predictors of renal obstruction as determined by diuretic scintirenography, and determined their reproducibility.Materials and Methods: We performed a retrospective review of the records of 77 consecutive patients with unenhanced helical CT findings (stones and secondary findings, including renal parenchymal edema, hydronephrosis, hydroureter, perinephric fat stranding, periureteral fat stranding and extravasation) of urinary lithiasis who had also undergone concomitant diuretic scintirenography during the initial emergency room evaluation during a 1-year period. Unenhanced helical CT films were independently reviewed by 2 attending radiologists (blinded to clinical outcome) to determine interobserver variability. In this chapter we discuss the surgical therapy of renal stones. There are four main modalities of surgical management for renal stones: extracorporeal shockwave lithotripsy (SWL), rigid/flexible retrograde ureteroscopy, percutaneous nephrolithotomy (PCNL), and laparoscopic/open surgery. These four modalities have significant advantages and disadvantages and are chosen based on a number of factors pertaining to the individual stone or stones, including stone size, location, substance, as well as patient factors such as body habitus, medical comorbidities, and anatomy. While the success rates vary with each modality, they all show relatively high stone clearance rates along with low-associated morbidity and mortality when appropriately selected and utilized. Advancing technology is constantly improving and being applied to the treatment of renal stones, and as a consequence stone-free rates, morbidity, and mortality rates will continue to improve.

Purpose: Contemporary predictive tools for percutaneous nephrolithotomy outcomes include the Guy stone score, S.T.O.N.E. nephrolithometry and the CROES nephrolithometric nomogram. We compared each scoring system in the same cohort to determine which was most predictive of surgical outcomes. Methods: We retrospectively reviewed the records of patients who underwent percutaneous nephrolithotomy between 2009 and 2012 at a total of 3 academic institutions. We calculated the Guy stone score, the S.T.O.N.E. nephrolithometry score and the CROES nephrolithometric nomogram score based on preoperative computerized tomography images. A single observer at each institution reviewed all images and assigned scores. Univariate and multivariate analysis was done to determine the most predictive scoring system. Results: We enrolled 246 patients in study. Objective To evaluate the S.T.O.N.E. nephrolithometry scoring system for percutaneous nephrolithotomy using computerized tomography (CT) imaging with strict criteria for stone clearance. Materials and Methods We analyzed a cohort of 122 patients who consecutively underwent primary percutaneous nephrolithotomy from July 2010 to March 2012 at our university-based referral hospital. All patients routinely have preoperative and postoperative CT imaging for stone burden determination. Primary outcome (residual stone) was scored as 0-2, 3-4, and >4 mm. All S.T.O.N.E. nephrolithometry parameters were recorded and scored as per published definition. The t test was used for continuous variables, and the chi-square testing or the Fisher exact test (when counts were small) was used for categorical covariates. S.T.O.N.E. score correlation with stone-free status was analyzed by logistic regression. Results Nephrolithometry score ranged from 5 to 13 with a mean of 9.5. Postoperative CT for residual stone showed 67 (54.9%), 26 (21.3%), and 29 (23.8%) patients had 0-2, 3-4, and >4 mm residual stone, respectively. Mean nephrolithometry scores for residual stone of 0-2, 3-4, and >4 mm were 8.87, 9.73, and 10.79 respectively (P Conclusion With use of strict CT imaging criteria for assessment of residual stone status, the S.T.O.N.E. scoring system is reproducible and predictive of treatment success. Further investigation is required to both validate this model and to determine if other predictive parameters will improve it as a predictive model.

Balloon dilation is an effective method of percutaneous renal access, but it can be impossible in patients with perirenal scarring. We report the use of an atherotomy cutting balloon for dilation of the chronic nephrostomy tract. The ease of use makes this device a useful alternative to standard dilation techniques.

We assessed the methodological and reporting quality of randomized, controlled trials of stone disease management and determined whether the reporting quality of randomized, controlled trials improved with time.We systematically searched the literature for randomized, controlled trials of urolithiasis treatment. We developed and pilot tested a data extraction checklist based on CONSORT (Consolidated Standards of Reporting Trials) criteria as well as a clinical checklist relevant to urolithiasis, each scored as 0 to 25. Our primary outcome measures were the mean differences in CONSORT and clinical summary scores with time. We performed statistical hypothesis testing using the Student t-test with 2-sided α = 0.05 to compare scores between 2002 to 2006 and 2007 to 2011.A total of 104 randomized, controlled trials met study inclusion criteria. The most common procedure types studied were percutaneous nephrolithotomy (41.3%), ureteral stenting (28.8%) and shock wave lithotripsy (25.0%). Mean ± SE CONSORT summary scores were 11.4 ± 0.4 and 12.1 ± 0.3 in 2002 to 2006 and 2007 to 2011, respectively, with a mean difference of 0.7 (95% CI -0.3-1.6, p = 0.167). Mean clinical summary scores were 7.4 ± 0.5 and 9.3 ± 0.4 in 2002 to 2006 and 2007 to 2011, respectively, with a mean difference of 1.8 (95% CI 0.6-3.1, p = 0.004).While the number of randomized, controlled trials of urological devices used to treat stone disease substantially increased with time, methodological and clinical reporting quality remains suboptimal. This compromises their credibility and warrants efforts to promote appropriate performance of future endourological studies.

Background and Purpose: Surgical management of a renal neoplasm in a solitary kidney is a balance between oncologic control and preservation of renal function. We analyzed patients with a renal mass in a solitary kidney undergoing nephron-sparing procedures to determine perioperative, oncologic, and renal functional outcomes. Patients and Methods: A multicenter study was performed from 12 institutions. All patients with a functional or anatomic solitary kidney who underwent nephron-sparing surgery for one or more renal masses were included. Tumor size, complications, and recurrence rates were recorded. Renal function was assessed with serum creatinine level and estimated glomerular filtration rate. Results: Ninety-eight patients underwent 105 ablations, and 100 patients underwent partial nephrectomy (PN). Preoperative estimated glomerular filtration rate (eGFR) was similar between the groups. Tumorsmanaged with PN were significantly larger than those managed with ablation (P = 60 mL/min/1.73m(2). Among these patients, 19 (23%) fell below this threshold after 3 months and 15 (18%) at 12 months. Postoperatively, there was no significant difference in eGFR between the groups. Conclusions: Extirpation and ablation are both reasonable options for treatment. Ablation is more minimally invasive, albeit with higher recurrence rates compared with PN. Postoperative renal function is similar in both groups and is not affected by surgical approach

Gender, smoking history, patient age, and tumor size have been found to impact the likelihood of benign histology at the time of nephron-sparing surgery (NSS). Providing external validation of these variables and evaluating the relationship between body mass index (BMI) and tumor location on the likelihood of benign histology during NSS for T1 tumors were the objectives of this study.Data were analyzed for consecutive patients undergoing NSS for T1 disease. Central tumors either were completely encircled by renal parenchyma, descended below the cortico-medullary junction, or were in direct opposition to the collecting system, renal sinus, or the hilar structures. Categorical variables were evaluated with chi-square test, and continuous variables were analyzed with independent sample t test. Logistic regression identified independent predictors of final pathology.NSS was performed in 316 patients, of whom 79 (24 %) had benign tumors. Patients with benign tumors were more likely to be female, to have a lower BMI, and to have peripheral tumors. On multivariate analysis, female gender (hazard ratio, 3.97; 95 % CI, 2.92-4.53, p0.001), peripheral tumor location (hazard ratio, 2.27; 95 % CI, 1.73-3.21, p = 0.014), and lower BMI (hazard ratio, 1.5; 95 % CI, 1.12-1.94, p = 0.015) were independently associated with benign histopathology at the time of surgical resection.Prospectively identifying which T1 tumors are benign would have tremendous implications for the patient. Ours is the first study that has identified the impact of tumor location and BMI on the risk of benign histology. Additional studies are needed to corroborate these findings and incorporate these data into future nomograms.

We compared laparoscopic and robotic pyeloplasty to identify factors associated with procedural efficacy.We conducted a retrospective multicenter trial incorporating 865 cases from 15 centers. We collected perioperative data including anatomical and procedural factors. Failure was defined subjectively as pain that was unchanged or worse per medical records after surgery. Radiographic failure was defined as unchanged or worsening drainage on renal scans or worsening hydronephrosis on computerized tomography. Bivariate analyses were performed on all outcomes and multivariate analysis was used to assess factors associated with decreased freedom from secondary procedures.Of the cases 759 (274 laparoscopic pyeloplasties with a mean followup of 15 months and 465 robotic pyeloplasties with a mean followup of 11 months, p0.001) had sufficient data. Laparoscopic pyeloplasty, previous endopyelotomy and intraoperative crossing vessels were associated with decreased freedom from secondary procedures on bivariate analysis, with a 2-year freedom from secondary procedures of 87% for laparoscopic pyeloplasty vs 95% for robotic pyeloplasty, 81% vs 93% for patients with vs without previous endopyelotomy and 88% vs 95% for patients with vs without intraoperative crossing vessels, respectively. However, on multivariate analysis only previous endopyelotomy (HR 4.35) and intraoperative crossing vessels (HR 2.73) significantly impacted freedom from secondary procedures.Laparoscopic and robotic pyeloplasty are highly effective in treating ureteropelvic junction obstruction. There was no difference in their abilities to render the patient free from secondary procedures on multivariate analysis. Previous endopyelotomy and intraoperative crossing vessels reduced freedom from secondary procedures.

To report a 6-year multi-institutional experience and outcomes with robot-assisted laparoscopic pyeloplasty (RLP) for the repair of ureteropelvic junction obstruction (UPJO).Between June 2002 and October 2008, 168 adult patients from 3 institutions underwent RLP for UPJO. A retrospective analysis of prospectively collected data were performed after institutional review board approval. Diagnosis was by intravenous urogram or computed tomography scan and diuretic renogram. All patients underwent RLP through a 4-port laparoscopic technique. Demographic, preoperative, operative, and postoperative endpoints for primary and secondary repair of UPJO were measured. Success was defined as a T½ of20 minutes on diuretic renogram and symptom resolution. Pain resolution was assessed by subjective patient reports.Of 168 patients, 147 (87.5%) had primary repairs and 21 (12.5%) had secondary repairs. Of the secondary repairs, 57% had a crossing vessel etiology. Mean operative time was 134.9 minutes, estimated blood loss was 49 mL, and length of stay was 1.5 days. Mean follow-up was 39 months. Overall, 97.6% of patients had a successful outcome, with a 6.6% overall complication rate.To our knowledge, this review represents the largest multi-institutional experience of RLP with intermediate-term follow-up. RLP is a safe, efficacious, and viable option for either primary or secondary repair of UPJO with reproducible outcomes, a high success rate, and a low incidence of complications.

OBJECTIVES To compare conventional laparoscopic pyeloplasty (C-LPP) and robotic-assisted laparoscopic pyeloplasty (RA-LPP), which are both used for correction of ureteropelvic junction obstruction. Robotic assistance may further expedite dissection and reconstruction; however it is unclear whether this has an impact on results. METHODS Between 1999 and 2009, 172 conventional or robotic-assisted transperitoneal laparoscopic pyeloplasties were performed by 2 surgeons. Data were obtained from our prospective database, patient charts, and radiographic reports. Statistical analysis was performed for the groups. RESULTS A total of 98 patients underwent R-LPP, and 74 underwent C-LPP. Mean age, body mass index, and gender distribution were similar for the groups. Of the patients, 22 (12.8%) had secondary ureteropelvic junction obstruction. Operative time in minutes was 189.3 ± 62 for RA-LPP, and 186.6 ± 69 for C-LPP (P = .69) respectively. Intraoperative and postoperative complication rates for RA-LPP and C-LPP were 1%, 5.1% and 0, 2.7% (P = .83 and .85) respectively. There was no significant difference in mean suturing time: 48.3 ± 30 and 60 ± 46 (P = .30) for RA-LPP and C-LPP, respectively. Long-term follow up (minimum 6 months; available for 136 patients) showed 93.4% and 95% radiographic success rate based upon diuretic scintirenography for RA-LPP and C-LPP respectively. CONCLUSIONS Operative time, perioperative outcome and success rates are similar for C-LPP and RA-LPP. Mean suturing time for RA-LPP was shorter; however, there was no significant time difference in total operative time. Complications for both procedures are infrequent. Success rates, as measured by diuretic scintirenography, are high for the 2 procedures.

Objetivo: La nefrectomía del donante vivo ha aumentado significativamente el fondo de donantes para trasplante renal, permitiendo un incremento marcado de los trasplantes. Las mejorías de la medicación contra el rechazo y el refinamiento de los criterios de selección del donante han permitido unas tasas de supervivencia del injerto extremadamente favorables. Más recientemente, la nefrectomía laparoscópica del donante vivo (NLDV) ha reducido significativamente la morbilidad en la población de donantes. La Universidad de Miami/Centro de Trasplantes del Hospital Jackson Memorial ha realizado un gran número de nefrectomías del donante vivo, con un aumento de la utilización de la nefrectomía laparoscópica del donante vivo y en el presente artículo comunicamos nuestra experiencia acumulada. Métodos: Realizamos una revisión retrospectiva de todas las nefrectomías del donante vivo realiza las durante los últimos diez años, incluyendo las nefrectomías laparoscópicas. Se recogieron las complicaciones quirúrgicas, tanto menores como graves. También se registraron las conversiones de laparoscopia cirugía abierta. Se revisó el seguimiento, incluyendo las creatininas al año del trasplante en los grupos de nefrectomía abierta del donante y laparoscópica. Resultados: Se han realizado 413 nefrectomías del donante vivo durante diez años. De éstas, 257 fueron laparoscópicas y 156 abiertas. En dos casos, la nefrectomía laparoscópica fue convertida a abierta. Tres donantes necesitaron de intervención después de la nefrectomía. No hubo ninguna muerte perioperatoria ni trombosis venosa profundas. Las complicaciones menores, incluyendo hernia, fiebre y diarrea por C.difficile fueron muy raras, siendo la más frecuente el dolor testicular en ocho casos. Conclusiones: Nuestra amplia experiencia en nefrectomía del donante vivo, con 413 casos a lo largo de diez años, ha sido muy favorable. El riesgo de complicaciones graves fue extremadamente bajo, con seis casos comunicados en esta serie. Las complicaciones menores también fueron raras. La nefrectomía del donante vivo es un método seguro y factible de aumentar el número de donantes para trasplante renal con mínima morbilidad.

Thermal ablative therapy with cryotherapy and radiofrequency ablation is currently being utilized for treatment of small renal masses. Recently, microwave ablation (MWA) has been investigated for soft tissue destruction, with limited clinical experience for renal masses. This study was designed to evaluate the effectiveness of a new microwave system in the porcine kidney.MWAs were performed in vivo on six pigs using the Evident Microwave Ablation System (Valleylab, Boulder, CO) with a 3.7 cm active tip percutaneous antenna. Ablations were performed in both upper and lower poles at 45 W for 3, 5, and 10 minutes. Three pigs died; three pigs survived for 7 days. After necropsy the kidneys were harvested, ablation zones were measured, and histological analysis was performed.The ablated zones at each time interval were inconsistent. The gross appearance of the lesions in both groups was similar. The antenna tract was charred, the collecting system was damaged, and there was asymmetry of the zones of ablation. Histological analysis revealed coagulative necrosis in the area of the ablation with sloughed and denuded urothelium.MWA of the kidney utilizing this model yields inconsistent geometrical lesions when applied near the renal collecting system. Additionally, microwave energy may be preferentially absorbed by the increased water content in the collecting system resulting in damage to it. Further study is needed to assess the attenuation of the microwave field in the kidney at different locations due to the kidneys heterogeneity.

In 1991, Schuessler et al. first reported the application of laparoscopic pelvic lymph node dissection (L-PLND) for staging of adenocarcinoma of the prostate (1). Subsequently, laparoscopic-limited obturator PLND, and in select cases, extended obturator and iliopsoas node dissection, has become the primary initial application of urologic laparoscopic surgery as a diagnostic and therapeutic technique (2). Follow-up studies on L-PLND clearly indicated that this procedure was comparable in accuracy and significantly less morbid than open PLND (Table 1) (3–6). While early studies showed a longer operative time for L-PLND, they also demonstrated a significant reduction in postoperative pain, hospitalization, and convalescence compared with open PLND (3–5). As expected, L-PLND initially showed an increased overall cost. However, with gradual refinements of technique, reusable equipment, and a reduced need for specialized instrumentation, overall cost is now likely to be equivalent to or less than open PLND (7–9). More recently, robot-assisted L-PLND has been performed in conjunction with robot-assisted laparoscopic radical prostatectomy, both by the transperitoneal and extraperitoneal routes. The cost effectiveness of this procedure is unclear in light of the need for significant capital investment in the robotic device, its maintenance, and the ongoing need for limited reusable accessories.

Renal artery aneurysm (RAA) is an infrequently seen disease entity but one with the potential for significant morbidity and mortality. Complications related to RAA include pain, hematuria, hypertension, and, rarely, rupture. Management is often based on symptomatology or, if symptoms are not present, the potential for rupture with increased size. Treatment options include observation, endovascular methods, or open surgical approaches, including aneurysm repair or nephrectomy. Complex cases often preclude endovascular approaches. We report our initial experience with laparoscopic nephrectomy, ex vivo back-table repair of the aneurysm, and iliac fossa autotransplant in cases of complex RAA.Two patients underwent laparoscopic nephrectomy with RAA repair and heterotopic autotransplant from May 2006 to November 2008. Etiology of the RAA was atherosclerosis in one patient and idiopathic in the second. Laparoscopic nephrectomy and back-table arterial reconstruction was performed, including aneurysmectomy and ostial closure. Patient 2 needed a gonadal vein graft for arterial reconstruction. Both patients had autotransplant into the right iliac fossa.Both patients were women. Mean age was 52.5 years (range 39-66 yrs). Mean operative blood loss was 550 mL (range 350-750 mL). Mean length of stay was 5.5 days (range 5-6 d). Mean discharge creatinine level was 0.8 mg/dL (range 0.5-1.1 mg/dL). No perioperative complications were reported. At last follow-up, all patients are alive with functioning autotransplant and no evidence of functional impairment.Management of complex RAA with laparoscopic nephrectomy, extracorporeal repair, and autotransplant is a feasible and successful method with minimal morbidity.

Ureteral stents, when left in situ in renal transplant patients, are a potential iatrogenic cause of graft compromise and graft failure. Such patients may present with acute renal failure, recurrent urinary tract infections, hematuria, and dysuria. We present a case report of a renal transplant patient with a heavily encrusted forgotten stent. We employed a simultaneous approach, using percutaneous nephrostolithotomy and cystolithalapaxy, for complete removal of the encrusted stent and associated stones. A MEDLINE literature review was then performed to identify and analyze similar cases in which a forgotten stent in a renal allograft was removed. Our experience and that found in the medical literature suggest that removal of forgotten stents can be achieved safely and effectively with proper endourological techniques. We also reviewed the current status of ureteral stent design in terms of attempts to preclude this problem. Ureteral stent design is still in a state of evolution with a focus on creating stents of new materials, and stents with new coatings, that may prevent encrustation.

To review trends in open partial nephrectomy (OPN) at our center, concentrating on patient selection, technique and perioperative complications.A comprehensive database was developed by chart review of consecutive patients undergoing OPN for renal masses at our center. Patient selection, technical modifications, perioperative morbidity, and histopathology were compared in patients undergoing OPN between 1992-1999, 2000-2003, and 2004-2008. Complications were divided into procedure-specific (PSCs) and nonspecific medical complications (NMCs). They were graded using the Common Terminology Criteria for Adverse Events (CTCAE), version 3.0.One hundred and sixty-three OPNs were performed. Temporal trends identified include: an increase in the mean patient BMI (p = 0.04), an increase in the percentage of patients with central tumors (p0.001), decrease in cold ischemia time (p = 0.045), increasing use of a sequential renal vein clamp (p = 0.03), increasing utilization of tissue sealants (p0.001), reduced EBL (p = 0.05), reduced length of stay (p = 0.005), and a decline in PSCs from 16 to 7% (p = 0.002). The incidence of histologically benign tumors declined from 34 to 10% (p = 0.001). Thirty-three (20.2%) patients experienced perioperative complications: 70% were CTCAE grade 1 or 2 adverse events. BMI was the only factor that was found to be associated with the risk of complications on multivariate analysis [odds ratio 1.067, CI 95% (1.002-1.136); p = 0.031].Increasingly, OPN is being utilized for a cohort of challenging patients who are overweight and have centrally located tumors. Despite this, the risk of PSCs is low. Patients who are overweight are at increased risk for perioperative NMCs.

Sparse published data exist on the impact of preexistent ureteral stents on the success of ureteroscopic stone surgery. We investigated the impact of a preexistent ureteral stent in relation to a number of parameters and outcomes of ureteroscopic management for urinary lithiasis.We retrospectively evaluated a cohort of patients undergoing ureteroscopy for renal and ureteral calculi. Data were abstracted on stone side, size, number and site, patient demographics, total stone burden, ureteral access sheath use, preoperative ureteral stent, ureteroscope type and outcome. Statistical analysis was done.The success rate of 1 and 2 ureteroscopic procedures was 86.9% and 97.3%, respectively. Primary analysis included data on 221 initial procedures. The single procedure success rate for stone site was 91.9% for the distal ureter, 89.7% for the proximal ureter, 83.3% for the renal pelvis, 80.5% for the lower pole and 82.4% for the interpolar/upper pole. Success was negatively associated with primary stone size (p = 0.020), total stone number (p = 0.001) and cumulative stone burden (p0.001). Stone site was not a predictor of success (p = 0.394). A preexistent stent was positively associated with success but it was not statistically significant (adjusted OR 2.22; 95% CI 0.88, 5.63; p = 0.254). Secondary analysis in patients who initially underwent flexible ureteroscopy yielded results consistent with those of primary analysis.Results show that ureteropyeloscopic lithotripsy and stone extraction may be performed with a high success rate. Success was significantly inversely related to stone size, cumulative stone burden and number of stones. Success was positively related to a preexisting ureteral stent but not significantly so.

Current clinical requirements mandate the existence of a renal diuretic protocol, which is fast and easy, applicable in all ages and for all indications, convenient for both the patient and the technologist, and provides diagnostic as well as prognostic information. Seventeen years ago a 25-minute protocol, after oral hydration, with no bladder catheterization, and simultaneous injection of mercapto-acetyl-triglycine (MAG(3)) and furosemide (MAG(3)-F(0)), was initiated. It initially was used for the evaluation of drainage and emerged as a protocol to also evaluate the renal parenchyma. Results of this protocol have been published individually, per clinical application. MAG(3)-F(0) was instrumental in the evaluation and prognosis of congenital disorders. For obstruction, in the newborn, an increasing renogram mandates intervention, whereas a downsloping one predicts spontaneous resolution. In children or adults, preoperatively or postoperatively, when the cortex was visualized and drained normally, there was no obstruction, even if urine was retained within a dilated collecting system or an extrarenal pelvis. For diseases of the renal parenchyma, the protocol enabled the diagnosis of acute pyelonephritis (APN) revealing the "regional parenchymal dysfunction," diagnostic of APN. Diffuse parenchymal diseases were characterized by increased residual cortical activity (RCA), and their progression was manifested as a deterioration of RCA. End-stage renal disease was characterized by lack of accumulation and retention. Trauma and leaks were identified with specific patterns. In renovascular hypertension (RVH), an increase in RCA after angiotension-converting enzyme inhibitors is diagnostic of RVH and prognostic of the beneficial effect of angioplasty on hypertension. In renal colic, stratification was possible into (1) complete or severe obstruction requiring immediate intervention, (2) mild obstruction allowing waiting, (3) spontaneous decompression (stunned kidney), and (4) no recent obstruction. In transplants, it enabled differentiation of acute tubular necrosis, acute or chronic rejection and nephrotoxicity, and identified infarcts, RVH, leaks and obstruction. Finally, this method allows for a quick semiquantification of renal function. The clinical usefulness of the MAG(3)-F(0) protocol in most congenital or acquired renal problems is proven through long-term clinical experience and has resulted in a substantial utilization of the test at our Center.

Ablative therapy is increasing for the management of small renal masses. Laparoscopic as well as percutaneous cryotherapy and radiofrequency ablation (RFA) have been utilized. Herein we review our experience with renal biopsy immediately prior to laparoscopic RFA.A prospectively collected database containing all patients who underwent laparoscopic RFA by three different surgeons at two different institutions was reviewed. Renal biopsies were performed in each patient during transperitoneal laparoscopy after mobilization of the kidney and prior to RFA. The biopsy needle was passed percutaneously via a sheath through the abdominal wall. Multiple core biopsies (3-5) were taken under visual and ultrasonic guidance. All were submitted for permanent pathologic sectioning.138 patients underwent renal biopsy prior to RFA. Mean tumor size was 3.0 cm (range 1.0-6.9). The mean age was 72 years (range 39-90). There were 42 females and 96 males. Mean blood loss was 28 ml (0-400 ml). Only 5 patients lost more than 50 ml, and in each case the bleeding was associated with complicated renal mobilization and dissection prior to biopsy. Final pathology revealed renal cell carcinoma in 95, oncocytic neoplasm in 26, and angiomyolipoma in 9.8 patients were considered to have nondiagnostic biopsies. In this group, final pathology revealed benign cysts in 3, inconclusive specimens in 3, fibrosis in 1, and normal tissue in 1. Hence, a clear diagnosis was possible in 130 of 138 patients, which is 94.2%. RCC was diagnosed in 68.8% of the patients, and in 73.1% of the conclusive biopsies. Eight patients had perioperative complications, including low-grade fevers (2) perirenal/retroperitonal hematoma (2), pleural tear/pneumothorax (2), CHF exacerbation, and wound infection.In our multicenter experience, renal biopsy of 138 renal lesions at the time of laparoscopic RFA had a diagnostic yield of 94.2%. RCC was diagnosed in 68.8% of the patients, and in 73.1% of the conclusive biopsies.

We report our experience with laparoscopic pyeloplasty using the daVinci surgical platform.We routinely performed laparoscopic pyeloplasties prior to acquiring the daVinci system. We prospectively evaluated 26 computer-assisted laparoscopic pyeloplasties (CLP) performed since acquiring the device in March 2003. There were 15 male and 11 female patients with a mean age of 34.5 years, who underwent right-sided procedures in 11 cases and left-sided procedures in 15 cases. Four patients (15%) had secondary ureteropelvic junction obstruction. All procedures were performed through a transperitoneal approach over stents placed preoperatively. The operative time excluded the time needed for stent insertion. Radiographic objective success was defined as adequate cortical drainage (t (1/2)or =15 minutes) and preserved or improved renal function on MAG-3 diuretic renography.A total of 23 dismembered pyeloplasties and 3 Y-V plasties were performed. In five patients, nephroscopy was performed for stone removal. The mean operative time and blood loss were 245 minutes and 69 mL, respectively. There were no intraoperative complications or open conversions. Three minor postoperative complications were noted. The mean length of hospital stay was 2 days (range 1-5 days). In 19 evaluable patients, at a median follow-up of 6 months (range 2-10 days), 15 (79%) had complete symptom resolution, while 3 (16%) had marked symptom improvement. The overall subjective improvement rate thus was 95%. The objective success rate based on our strict diuretic renography criteria was 100%. The overall clinical success rate was 95% (18/19).Robot-assisted laparoscopic pyeloplasty is a feasible alternative to laparoscopic or open pyeloplasty with excellent short-term subjective and objective success rates.

Introduction Fibroepithelial polyps of the ureter, usually of congenital or inflammatory origin, are rare entities. These polyps are often solitary, but cases of multiple polyps have also been reported. Treatment of these entities has evolved with emerging surgical techniques and technologies. Surgical Technique We describe our surgical technique for minimally invasive treatment of multiple polyps arising in a single ureter. The base of each polyp is ablated with a 365-μm holmium laser fiber operating at 10 W, and each polyp is removed from the ureteral wall with 3F grasping forceps. Hemostasis is achieved with laser coagulation. Ureteral stones are removed concurrently with a 2.4F basket. A 12/14F ureteral access sheath is used to facilitate the multiple passes of the ureteroscope and the removal of the polyps and stones from the proximal ureter. All 10 polyps were removed from the proximal portion of a single ureter. The ureteral stent was removed at 6 weeks, and follow-up intravenous urography at 6 months and ureteroscopy at 11 months showed no residual polyps or stones. The wall of the ureter was well healed, with no evidence of stricture. Conclusions We have demonstrated a specific minimally invasive technique that can be used to treat even highly complex cases involving upper urinary tract fibroepithelial polyps.

Contrast-enhanced three-dimensional magnetic resonance angiography (3D-MRA) with image reconstruction has important applications in laparoscopic urologic surgery. We now use 3D-MRA as part of our preoperative evaluation in selected patients undergoing laparoscopic donor nephrectomy, pyeloplasty, radical nephrectomy, and partial nephrectomy.From June 2001 to December 2002, 50 patients underwent preoperative 3D-MRA at 1.5 T prior to laparoscopic renal surgery. In general, preoperative 3D-MRA was obtained for donor nephrectomies and pyeloplasties and for cases where prior imaging suggested a possible vascular anomaly. Patients who underwent preoperative imaging included those having donor nephrectomy (N = 28), pyeloplasty (N = 12), radical nephrectomy (N = 5), partial nephrectomy (N = 3), and other laparoscopic renal procedures (N = 2). The 3D-MRA studies were interpreted by one radiologist, and all laparoscopic cases were performed by one of two surgeons. The findings of 3D-MRA were correlated with the intraoperative findings with special attention to aberrant vasculature, including duplicated renal arteries or veins, accessory vessels, or crossing vessels.Among patients undergoing laparoscopic donor nephrectomy, 3D-MRA correctly predicted the number of renal vessels in 27 of 28 cases (96%), including all 3 cases of left retroaortic renal vein. Also, 3DMRA correctly predicted the presence or absence of a crossing vessel in 10 of 12 cases (83%) of laparoscopic pyeloplasty. The imaging study also correctly predicted the number of hilar vessels in all five cases of radical nephrectomy, all three cases of partial nephrectomy, and both cases of other renal operations. Overall, 3D-MRA correctly defined the renal hilar anatomy in 48 of 50 patients, for an overall accuracy of 96%.Three-dimensional MRA findings correlate well (96%) with intraoperative findings in laparoscopic renal surgery. The imaging study provides exquisite vascular detail and is highly accurate, making it sufficient imaging prior to laparoscopic donor nephrectomy and useful for pyeloplasty and other complex renal operations.

Bipolar electrocautery has recently been introduced as a modality for transurethral resection of bladder tumors (TURBT). The primary benefits of bipolar TURBT stem from the use of saline irrigant rather than glycine or water. TURBT should be conducted in a fashion such that the resected tissue can be used for proper grading and staging, so excessive cauterization of the tissue should be avoided. In this study, we compared the pathologic characteristics of bladder tumor specimens resected with bipolar versus standard monopolar energy to determine specimen quality.Bipolar TURBT (Gyrus Medical Inc., Maple Grove, MN) was performed in 11 patients. Pathologic specimens were compared with the specimens from 11 patients who had previously undergone standard monopolar TURBT. Resected tissue was examined by a pathologist who recorded tumor size, grade, location, presence of muscularis propria, presence of muscle invasion, and final diagnosis. The pathologist also determined the degree of cautery artifact in each specimen. The pathologist was blinded to the form of electrocautery used and the clinical diagnosis.Transurethral resection with bipolar electrocautery was carried out without difficulty or complication in all cases. Similarly, there were no complications in resection by standard monopolar electrocautery. The bladder tumor chips obtained with bipolar TURBT were smaller because of the smaller size of the bipolar loop. However, this did not interfere with the pathologic assessment. There were no significant pathologic differences between specimens according to the type of cautery used. A large degree of cautery artifact was noted in the tissue of larger tumors resected using both monopolar and bipolar electrocautery. However, the incidence and degree of cautery artifact were similar in the two groups. No trends between tumor location and degree of cautery effect were noted. The pathologist had no difficulty reaching a full and proper diagnosis in all cases involving either form of electrocautery.Bipolar electrocautery is well suited for TURBT. Bladder tissue obtained from bipolar TURBT is of the same histologic quality as that obtained from standard monopolar TURBT and provides the urologist with a reliable and complete diagnosis.

Many centers have adopted laparoscopic and hand-assisted laparoscopic (HAL) techniques for live donor nephrectomy. Currently, the majority of laparoscopic living donor kidneys are procured from the left side because of the longer renal vein and improved transplantation. However, indications exist for right donor nephrectomy. We present our technique of HAL right-sided donor nephrectomy. A key feature of our dissection is wide mobilization of adjacent structures to achieve good exposure of the right kidney and inferior vena cava. In addition, the use of the hand permits optimal positioning of the kidney for division of the renal vessels with the vascular stapler. At the time of division of the renal vein, the stapler is placed on the wall of the inferior vena cava in order to gain maximal length. This technique has allowed HAL right-sided donor nephrectomy to be performed safely when indicated.

Schuessler et al. first reported in 1999 the application of laparoscopic pelvic lymph node dissection (L-PLND) for staging of adenocarcinoma of the prostate (1). Subsequently, the performance of laparoscopic limited obturator pelvic lymph node dissection and in select cases, extended obturator and iliopsoas node dissection, for staging of adenocarcinoma of the prostate, became the primary initial application of urologic laparoscopic surgery as a diagnostic and therapeutic technique (2). Follow-up studies on L-PLND clearly indicated that this procedure was comparable in accuracy and significantly less morbid than open pelvic lymphadenectomy (see Table 1) (3–6). In addition, earlier studies showed a longer operative time for L-PLND when compared with open PLND, but also demonstrated a significant reduction in postoperative pain, hospitalization, and time of convalescence (3–5). As expected, L-PLND has been shown to have increased overall cost when compared with open PLND. However, with gradual refinements of laparoscopic technique, a reduction in the need for expensive specialized laparoscopic instrumentation, and the use of reusable equipment, overall total cost is now likely to be equivalent to open PLND (7–9).

Throughout medicine, there is a recurrent paradigm of development of noninvasive/less invasive testing used to identify patients who may then be able to forgo relatively more invasive diagnostic testing and treatment. In the realm of cardiology, reconstructed advanced imaging has been extensively investigated as a substitute for cardiac catheterization [1]. In the realm of urology, we have long pursued use of PSA testing to identify men at risk for the prostate cancer and have used this to aid in the selection of patients to undergo prostate biopsy. More recently, further refinements in overall care to those patients diagnosed with prostate cancer, in response to potentially excessive diagnosis and treatment related morbidity, have been introduced with the development of active surveillance protocols to aid in the identification of patients with prostate cancer that may potentially be spared treatment-associated morbidity [2]. In the case of renal tumors, to date, only limited advancements have taken place in terms of preoperative characterization of renal masses. Due to lack of such advancements, notably in the case of small renal masses, we as urologists, in the majority of cases, essentially resort to excisional biopsy of these poorly characterized lesions, for lack of better preoperative characterization. Though partial nephrectomy has certainly undergone a number of refinements and technical advances, this procedure, even in centers of excellence, has well defined risk for morbidity [3]. The effects and nature of renal ischemia are also currently under investigation at a large number of institutions due to concerns over what long-term impact this may have for the patient [4, 5].
