Achieving safe endoscopic skills is quintessential for a pediatric gastroenterologist to effectively manage various gastrointestinal conditions. This has become more important as the latest international guidelines stress on the importance of complete bowel workup for surveillance and reassessment of pediatric inflammatory bowel disease. A pediatric endoscopist needs to have more refined and age-appropriate diagnostic and therapeutic endoscopic skills as compared to their adult counterpart. This chapter details the essential principles required to achieve safe and effective endoscopic skills including patient preparation, update on use of prophylactic antibiotic, equipment required, and step-by-step approach to achieve full ileocolonoscopy and enteroscopy, including double balloon enteroscopy. A section is dedicated to pediatric inflammatory bowel disease including endoscopic findings, classification, treatment targets, and new scoring system for assessing disease severity. New advances in endo-diagnostic gastrointestinal procedures including wireless capsule endoscopy, colon capsule endoscopy, high magnification chromoscopic colonoscopy, confocal LASER endoscopy, and endosonography are also discussed.
