The study objective is to assess the outcomes after stereotactic radiosurgical treatment (SRS) in patients with pilocytic astrocytomas after non-radical surgery and after continued tumor growth.Materials and methods. The report includes 56 patients (37 males and 19 females) who have undergone SRS in N. N. Burdenko National Medical Research Center of Neurosurgery from March 2005 to January 2018.Results. The majority of patients (75 %) were children. Almost 43 % of patients underwent SRS as part of the primary treatment after biopsy or incomplete removal, other patients – in the event of continued tumor growth after non-total surgery. Tumors involved the cerebellum (41 %), brainstem (23 %), thalamus (19.6 %) and cerebral hemispheres (16.1 %). The median tumor volume was 1.9 cm3 (0.14–19.00 cm3), 23 % of patients had cysts in the tumor. The prescribed dose was 12 to 22 Gy (median 18 Gy) over 50–80 % isodose line. The follow-up was available for 54 (96.4 %) patients. The median follow-up was 67 months (3–151 months). All patients were alive at the end of the follow-up examination. In 14 (25 %) patients, the development of pseudo-progression (PSP) was noted. The median detection of PSP is 11 months (3–65 months). Of these, in 7 (50 %) patients PSP was accompanied by clinical deterioration: in 5 – an increase in general cerebral symptoms and in 1 patient – an episode and an increase in cerebellar symptoms. Eight (57 %) patients with PSP were reoperated: 4 underwent removal of the tumor, 4 – emptying of the cyst and placement of the Ommaya reservoir, which led to regression of the existing symptoms. No other toxicity was observed in patients. At the time of the completion of the follow-up examination (with a given median follow-up), no relapses were detected. There was no clinical deterioration after SRS.Conclusion. Radiosurgical treatment is an effective and safe method of radiation treatment for patients with primary pilocytic astrocytomas and recurrent pilocytic astrocytomas, providing control over tumor growth in all patients with a low risk of complications.

Choroid plexus carcinomas (CPCs) are rare pediatric tumors with a generally poor prognosis. Currently there is no definite optimal treatment strategy for this neoplasm. This study is supported by the Independent Ethics Committee and approved by the Academic Council of the Dmitry Rogachev National Medical Research Center of Pediatric Hematology, Oncology and Immunology. This study included patients with a verified diagnosis of CPC at the age 0 to 18 years in the period from 01.01.2009 to 31.12.2019. A total of 32 patients were registered. The median age was 2.65 years; 93.5% of CPCs were localized in the lateral ventricles. Initial metastases were found in 21.8% of cases; 5-year event-free survival (EFS) in children with metastases was lower than in those who did not have metastases 29 ± 17% and 49 ± 12%. In our cohort, gross total resection (GTR) was performed in 65.6% of patients. The five-year EFS in patients with complete CPC removal was higher than in patients who underwent subtotal and partial resection (63 ± 13%, 12 ± 11%, and 0%, respectively). In addition, overall survival (OS) was slightly higher in those who underwent GTR compared with subtotal and partial resection (74 ± 12%, 67 ± 16%, and 0%, respectively). Of the 32 children with CPCs, 15 children received programm chemotherapy, 17 non-programm chemotherapy, 5-year EFS in patients who received programm and non-programm chemotherapy was 79 ± 11% and 0%, respectively (p = 0.0006), 5-year OS in patients who received programm and non-programm chemotherapy was 93 ± 7% and 36 ± 14% (p = 0.0054).

OBJECTIVE To evaluate variables that may predict the outcome after hemispherotomy basing on a retrospective study of a large consecutive pediatric cohort of patients from a single institution. MATERIAL AND METHODS One hundred and one patients with refractory seizures and variable decline in development (n=78) underwent hemispherotomy (med. age - 43 months, med. epilepsy history - 30 months). Developmental pathology was the anatomical substrate of disorder in 42 patients, while the infantile post-stroke scarring and gliosis was its origin in the majority of 43 cases with acquired etiology. The progressive pathology (the Rasmussen encephalitis, Sturge-Weber angiomatosis and tuberous sclerosis) was the etiology in 16 children. Left-sided hemisphere was impaired in 54 cases; some contralateral anatomical and potentially epileptogenic MRI-abnormalities were noted also in «healthy» hemisphere in ¼ of all cases. Eight patients needed second surgery to complete sectioning of undercut commissural fibers. FU is known in 91 patients (med. - 1.5 years) and 73 of them were free of seizures (80.2%), but only 30 of 40 patients with FU > 2 years were still SF (75%). All but one of re-do hemispherotomies were successful. AED-treatment was discontinued in 46 cases and tapered in other 27 patients. Up to 90% of kids demonstrated some improvement in behavior and cognition. Surgery is an effective approach for drug-resistant temporal lobe epilepsy following hippocampal sclerosis. There is still no clear and unanimous opinion about advantages and disadvantages of certain surgical technique.There were 103 surgical interventions in 101 patients. Females prevailed (1.45:1). Age of patients ranged from 16 to 56 years (median 28). Anteromedial temporal lobectomy and selective amygdaloghippocampectomy were performed in 49 (47.6%) and 54 (52.4%) patients, respectively. In the latter group, 30 patients were operated via a 14-mm burr hole-subtemporal approach. Postoperative outcomes were assessed using the Engel grading system. The follow-up period ranged from 2 to 8 years (median 4 years).By the 2Selective amygdaloghippocampectomy is not inferior to anteromedial lobectomy. Background: The purpose of this study was to evaluate the effectiveness of multiple hippocampal transections (MHT) in the treatment of drug-resistant mesial temporal lobe epilepsy. Methods: Six patients underwent MHT at Burdenko Neurosurgery Center in 2018. The age of the patients varied from 18 to 43 years. All patients suffered from refractory epilepsy caused by focal lesions of the mesial temporal complex or temporal pole in dominant side. Postoperative pathology revealed neuronal-glial tumors in two patients, focal cortical dysplasia (FCD) of the temporal pole – in two patients, cavernous angioma – in one patient, and encephalocele of the preuncal area – in one patient. Results: All patients underwent surgery satisfactorily. There were no postoperative complications except for homonymous superior quadrantanopia. This kind of visual field loss was noted in four cases out of six. During the follow-up period five patients out of six had Engel Class I outcome (83.3%). In one case, seizures developed after 1 month in a patient with FCD in the uncus (Engel IVA). After surgery, three out of six patients developed significant nominative aphasia. Two patients relative to the preoperative level demonstrated improvement in delayed verbal memory after MHT. Two patients showed a decrease level in delayed verbal memory. In preoperative period, visual memory was below the normal in one patient. Delayed visual memory in two cases impaired compared to the preoperative level. Conclusion: MHT can be considered as an effective method of drug-resistant mesial temporal lobe epilepsy caused by tumors of the medial temporal complex. At the same time, MHT makes it possible to preserve memory in patients with structurally preserved hippocampus. However, MHT do not guarantee the preservation of memory after surgery.

The study objective is to report 2 cases of endoscopic transsphenoidal removal of hypothalamic hamartomas (HH). This surgical method has not been previously considered in both Russian and foreign publications. Materials and methods. A 16-year-old male patient (R.) was admitted to hospital with complaints of epileptic seizures occurring once every 5–6 days; the disease duration was 9 years. A 23-year-old female patient (M.) was also admitted to hospital with complaints of epileptic seizures occurring once a week; the disease duration was 16 years. Results. Magnetic resonance imaging of the brain of patient R. revealed a HH (1.5 cm × 1.4 cm × 1.4 cm) and focal cortical dysplasia in the right occipital lobe. During a 3-day video electroencephalogram monitoring, we registered 3 secondary generalized complex partial seizures. Patient R. underwent endoscopic transsphenoidal removal of the HH. After surgery, he experienced no epileptic seizures during the next 18 months, even though he continued to receive the same doses of antiepileptic drugs. Magnetic resonance imaging of the brain of patient M. demonstrated a HH (2.44 cm × 2.79 cm × 2.68 cm). During a 4-day video electroencephalogram monitoring, we registered 2 secondary generalized complex partial seizures. The patient underwent transsphenoidal removal of the HH. During the next 8 months, the patient experienced no seizures. Conclusion. Our results suggest that transnasal removal of HH is a highly effective and safe procedure for patients with drug-resistant epilepsy. Surgical removal of HHs ensures effective control of the disease, which was previously resistant to pharmacotherapy.

Background Immunohistochemistry is a basic diagnostic technique. Immunohistochemical examination results reflect mainly qualitative and less quantitative characteristics of proteomic status of cells. A combined approach with complex quantitative evaluation of marker expression using colorimetric analysis and computer technologies can expand the diagnostic capabilities of immunohistochemistry. We studied such an approach developed by using expression of the proliferative marker Ki-67 in pituitary adenomas. Methods A retrospective, blind, randomized, comparative study was performed of Ki-67 expression activity in pituitary adenomas using the traditional Ki-67 labeling index and a simple immunohistochemical cytocolorimetric analysis developed by us with immunohistochemical cytocolorimetric index (ICI) estimation as predictors of relapse, assessing the relationships of these indicators with the time before relapse. Results Mean Ki-67 labeling index was 3.87% ± 0.29% in the relapse-free group and 4.01% ± 0.29% in the relapse group; the difference was not statistically significant. The average Ki-67 ICI was 24.16% ± 0.51% in the relapse-free group and 30.68% ± 0.64% in the relapse group; the difference was statistically significant. The correlation coefficient of ICI values and time before relapse was −0.302, indicating the presence of a weak negative correlation. Conclusions We successfully tested an ICI estimation method developed by us to assess Ki-67 expression in pituitary adenomas. The ICI technique can be used both as a prognostic factor for relapse and, in combination with other modern proteomic and genetic methods, as the basis for creation of new multimodal analyzing systems for functional state assessment of cells and tissues.

Chondrosarcoma is a rare malignancy composed of transformed cells of cartilage. This cancer is characterized by slow growth. Almost 75% of intracranial chondrosarcomas are observed on the skull base and grow from bone synchondrosis. Neuroblastoma is a malignancy developing from the embryonic neuroblasts of sympathetic nervous system. Primary sellar neuroblastomas are extremely rare (there are currently only 11 case reports in the literature). Possible development of neuroblastoma in sellar region expands differential diagnosis of local processes due to inclusion of neuroblastoma into the spectrum of suspected tumors. We report a literature review and description of a patient with primary sellar neuroblastoma.Нейробластома является злокачественной опухолью, развивающейся из эмбриональных нейробластов симпатической нервной системы. Первичные нейробластомы селлярной локализации встречаются крайне редко (в настоящее время в литературе представлено лишь 11 случаев). Currently, cabergoline therapy is the main method of treatment with prolactin. The use of the drug in most cases leads to tumor regression, normalization of prolactin levels and restoration of gonadotropic function. The mechanism of its impact on tumor cells in vivo, which is dynamically traced in the same human tumor, is the case of considerable interest. We observed a 30-year-old patient who was operated on twice for a giant prolactinoma before and on treatment by cabergoline. The morphological study after the first surgery (before introducing of cabergoline therapy) revealed a prolactin-positive pituitary tumor with a Ki-67 labeling index of 8% and with strong expression of dopamine type 2 receptors (D2R), CD31 and CD34. After 4 months, during which the patient received cabergoline at a dose starting from 0.5 mg to 1.5 mg per week, a second transsphenoidal surgery was performed with subtotal removal of residual tumor tissue. During the morphological study of the second biopsy sample, the tumor retained a pronounced immunopositivity to prolactin and D2R, with a decrease in the labeling index Ki-67 to 2%, as well as a decrease in the expression of CD31 and CD34. Subsequent cabergoline therapyresulted in persistent normoprolactinemia, restoration of androgen (and reproductive) status, and no tumor recurrence over a 10-year period on cabergoline treatment. Thus, one of the mechanisms of effect of cabergoline that leads to tumor regression is a decrease in the proliferative index and angiogenesis of the tumor.

Brain metastases of various types of cancer are diagnosed in 8-10% of all cancer patients. In the world literature, only 30 cases of cancer metastasis to the pituitary adenoma are described. This article presents yet another observation of a patient with breast cancer metastasis into the hormone-inactive pituitary adenoma at the Burdenko neurosurgical center, Russia The patient underwent endoscopic endonasal transsphenoid removal of the neoplasm. During microscopy and immunohistochemical studies of the biopsy, two types of tissue (pituitary adenoma and cancer metastasis) with different Ki-67 treated surgically (1% and over 40%) were found.Метастазы различных видов рака в головной мозг выявляются у 8-10% от всех онкологических пациентов. В мировой литературе описано всего 30 случаев метастазирования рака в аденому гипофиза. В нашей публикации представлено еще одно наблюдение хирургического лечения пациентки с метастазом рака молочной железы в гормонально-неактивную аденому гипофиза, которое проведено в ФГАУ 'Национальный медицинский исследовательский центр нейрохирургии им. акад. Н.Н. Бурденко' Минздрава России. Пациентка подверглась эндоскопическому эндоназальному транссфеноидальному удалению новообразования. При проведении микроскопии и иммуногистохимического исследования биоптата выявлено два вида ткани (аденома гипофиза и метастаз рака) с разными индексами Ki-67 (1% и свыше 40%).

McCune-Albright Syndrome is a rare genetic disease characterized by the formation of fibrous osteodysplasia foci of various localization, including the bones of skull base. Having a gross lesion of the main bone body and the simultaneous formation of the pituitary adenoma, its transnasal removal becomes very difficult.Two clinical observations are presented where at patients with the classic manifestation of McCune-Albright syndrome we were able to successfully remove somatotropinomas with endoextrasellar growth. In both cases the presence of visual disorder was the indication for surgery.Despite pronounced deviations in the anatomy of the skull base bones in both cases we managed to access the Turkish saddle. Removal of the pituitary tumor did not differ from standard surgery. Improved vision after surgery was observed in two patients. Tumors were removed subtotally and clinical and biochemical remission of acromegaly was not achieved. In both cases, the tumors had morphological signs of atypia. Patients continued to receive therapy with somatostatin analogues and radiation.The possibility of performing transnasal surgery in this category of patients using specific instruments and intraoperative navigation is shown.Синдром МакКьюна-Олбрайта - редкое генетическое заболевание, характеризующееся формированием очагов фиброзной остеодисплазии различной локализации, в том числе в костях основания черепа. При грубом поражении тела основной кости и одновременном формировании аденомы гипофиза ее трансназальное удаление представляется очень сложным. Материал и методы. Приведено два клинических наблюдения, в которых у пациентов с классическим проявлением синдрома МакКьюна-Олбрайта нам удалось успешно удалить соматотропиномы с эндоэкстраселлярным ростом. В обоих случаях показанием к операции было наличие зрительных нарушений. Результаты. Несмотря на выраженные нарушения анатомии костей основания черепа, нам удалось получить доступ к турецкому седлу у обоих пациентов. Удаление опухоли гипофиза ничем не отличалось от стандартной операции. Улучшение зрения после операции отмечено у обоих больных. Опухоли удалены субтотально, клинико-биохимическая ремиссия акромегалии не достигнута. В обоих случаях опухоли имели морфологические признаки атипии. Дальнейшее лечение включало аналоги соматостатина и лучевую терапию. Выводы. Показана возможность выполнения трансназальной операции у данной категории больных с использованием специальных инструментов и интраоперационной навигации.

Although radiosurgery-induced transformation of vestibular schwannoma (VS) into malignant peripheral nerve sheath tumor (MPNST) is being widely discussed, little attention is paid to the spontaneous transition of these tumors. Although the pathogenesis of this phenomenon remains uncertain, growing number of reported cases might call to notice them. We present a case of a 29-year-old woman who suffered right-sided hearing loss which remained untreated for 4.5 years. Magnetic resonance imaging revealed a right cerebellopontine tumor and the patient underwent tumor removal through retrosigmoid approach. Pathologically, the tumor was benign acoustic schwannoma with increased Ki-67 8%–10%. The tumor relapsed only 6 months later and was removed again-this time it was malignant peripheral nerve sheath tumor MPNST. The patient was treated with stereotactic radiotherapy, but despite that tumor growth was observed again and she underwent the third operation. Later on, tumor progression was noted with multiple intraaxial metastases and patient died 17 months after the diagnosis of MPNST had been confirmed. This is a seventh documented case of spontaneous transformation of VS into MPNST, which is clinically important for recording.

The purpose of the study is to present the results of successful treatment of a patient with traumatic arterial aneurysm of the middle cerebral artery (MCA).A clinical case of traumatic arterial aneurysm of the M4 segment of MCA was studied. A patient with a ruptured traumatic aneurysm was transferred to Krasnogorsk City Hospital #1 for further examination and emergency surgery.On the sixth day after surgery, the patient was discharged in a satisfactory condition with almost complete regression of neurological symptoms.Treatment of traumatic arterial aneurysms requires an individual approach taking into account the past medical history, the clinical presentation, aneurysm location and anatomy, as well as the mechanism of injury. It is extremely difficult to suspect an arterial aneurysm after a patient had a traumatic brain injury as the incidence of this condition is very low. An angiographic study of cerebral vessels is needed for reliable diagnosis, not just solely performing standard computed tomography of the brain. Patients with this pathology require surgical intervention.Цель исследования - представить результаты успешного лечения пациента с редкой формой аневризмы - травматической артериальной аневризмой средней мозговой артерии (СМА). Материал и методы. Изучен клинический случай травматической артериальной аневризмы М4 сегмента СМА. Пациент с разрывом аневризмы был переведен в Красногорскую городскую больницу #1 для обследования и выполнения экстренного хирургического вмешательства. Результаты. На 6-е сутки после оперативного вмешательства пациент выписан в удовлетворительном состоянии, с практически полным регрессом неврологической симптоматики. Выводы. Лечение травматических артериальных аневризм требует индивидуального подхода с учетом данных анамнеза, клинической картины, локализации и анатомии аневризмы, а также механизма полученной травмы. Заподозрить артериальную аневризму после получения черепно-мозговой травмы крайне затруднительно ввиду низкой частоты встречаемости данной патологии. Для достоверной постановки диагноза необходимо выполнение ангиографического исследования сосудов головного мозга, а не только стандартной компьютерной томографии. Пациенты с такой патологией требуют хирургического вмешательства.

Oculomotor nerve neurinoma not associated with neurofibromatosis type II is an extremely rare pathology. According to the topography, cisternal, cisternocavernous, cavernous, orbitocavernous, and orbital tumor groups are distinguished. The clinical picture of the disease is characterized mainly by either oculomotor disorders or pyramidal symptoms, depending on the tumor localization. Neurinomas of the oculomotor nerve rarely occur without oculomotor disorders. However, in some patients with these tumors, the third nerve function remains intact. In this paper, we present clinical cases of two patients with oculomotor nerve neurinomas and analyze the relevant literature.Невринома глазодвигательного нерва при отсутствии нейрофиброматоза II типа является крайне редкой патологией. По топографии выделяют цистернальные, цистернально-кавернозные, кавернозные, орбитально-кавернозные и орбитальные опухоли. В клинической картине заболевания преобладают глазодвигательные нарушения, реже они дополняются другой симптоматикой. Невриномы глазодвигательного нерва редко протекают без глазодвигательных нарушений. Однако в некоторых наблюдениях функция III черепного нерва может оставаться сохранной. В нашей работе представлены клинические наблюдения двух пациентов (один из которых оперирован) с невриномами глазодвигательного нерва и проведен анализ литературы по данной проблеме.

OBJECTIVE To evaluate the treatment results among 106 patients of molecular subgroup 3 and to determine the factors affecting the prognosis. PATIENTS AND METHODS In all the patients, initial removal of the tumor was performed. All the patients got chemoradiotherapy according to the HIT protocol. There were 34girls and 72boys. Most patients were over 3 years:74 compared to 32 younger than 3. The majority of the patients had stage M+: 65; in 38 stage M0 was determined; in 3patients, stage was not specified, Mx.MYC amplification was found in 20 patients; MYCN amplification, in 4 patients. Classic medulloblastoma was predominant: 65, and 41 patients had anaplastic/large cell medulloblastoma. RESULTS The five-year progression-free survival was 0.51±0.05, the five-year overall survival was 0.52±0.04.The median survival was 82months,and the median progression-free survival was 37 months. There were no significant variations of PFS depending on the sex and age. The treatment results depended on the histological subtype: for classic medulloblastoma, the five-year PFS was 0.57; for the anaplastic/largecell,0.38(р = 0.02030). The presence of metastases significantly affected the survival: PFS for stage M0 was 0.77; for stage М+,0.35(р = 0.00062). Patients with MYC amplification had a significantly worse survival compared to MYCN patients and those without MYC amplification: 0.1, 0.75, and 0.58, respectively (р = 0.00002). Three patients with MYC amplification are alive: two patients had MGMT methylation detected. CONCLUSIONS The results of treatment among the patients with molecular subgroup 3 depended on the tumor subtype, presence of metastases, MYC amplification and MGMT methylation. In the absence of unfavorable factors, the survival was the same as in molecular subgroup 4.

Surgery is the first-line treatment option in children with FCD and refractory epilepsy, but the rate of success and patient numbers who became free of seizures vary widely from series to series.To elicit variables affecting the outcome and predicting achievement of the long-term seizure-free status.One hundred sixty-nine children with cortical dysplasia and DR-epilepsy underwent surgery Preoperative evaluation included prolonged video-EEG and MRI (in all patients) and neuropsychological testing when possible. Fourteen patients underwent invasive EEG, fMRI and MEG were used also in some cases. Including 27 repeat procedures the list of overall 196 surgeries performed consists of: cortectomy (lesionectomy with or without adjacent epileptogenic cortices) – in 116 cases; lobectomy – in 46; and various disconnective procedures – in 34 patients. Almost routinely employed intraoperative ECOG (134 surgeries) was combined with stimulation and/or SSEP in 47 cases to map eloquent cortex (with CST-tracking in some). A new permanent and not anticipated neurological deficit developed post-surgery in 5 cases (2,5%). Patients were follow-upped using video-EEG and MRI and FU which lasts more than 2 years (median – 3 years) is known in 56 cases. Thirty-two children were free of seizures at the last check (57,2% rate of Engel IA). A list of variables regarding patients’ demography, seizure type, lesion pathology and localization, and those related to surgery and its extent were evaluated to figure out anyone associated with favorable outcome.Both Type II FCDs and their anatomically complete excision are positive predictors for favorable outcome and achievement of SF-status (p0,05). Residual epileptic activity on immediate post-resection ECOG do not affect the outcome.Patients with Type II FCD, particularly with Type IIb malformations are the best candidates for curative surgery, including cases with lesions in brain eloquent areas. Kids with Type I FCD have much less chances to become free of seizures when attempting focal cortectomy. However, some of them with early onset catastrophic epilepsies may benefit from larger surgeries using lobectomy or various disconnections.Хирургическое лечение — метод выбора у детей с кортикальными дисплазиями (КД) и фармакорезистентной симптоматической эпилепсией, однако его результативность и доля благоприятных исходов с полным прекращением приступов и стойкой ремиссией широко варьируют.Выявить критерии, которые влияют на исход лечения и ассоциированы с прекращением приступов.Оперировано 169 детей. Все дети обследованы с помощью видеоэлектроэнцефалографии (ЭЭГ) и магнитно-резонансной томографии (МРТ), 14 — с помощью инвазивной ЭЭГ. Произведено 196 операций, включая 27 повторных вмешательств: 116 кортэктомий, 46 лобэктомий и 34 различных дисконнекции. Рутинно (134 случая) использовали интраоперационную электрокортикографию (ЭКОГ), сочетая ее в 47 случаях с картированием сенсомоторных зон коры, а в части случаев — и кортикоспинального тракта. Новый непредвиденный и стойкий неврологический дефицит отмечен у 5 (2,5%) больных. Катамнез длительностью более 2 лет с повторными видео-ЭЭГ и МРТ известен у 56 больных (медиана — 3 года). Благоприятный результат с полным прекращением всех приступов и стойкой ремиссией (исход IA Engel) достигнут у 32 (57,2%) пациентов. Анализировали связь между различными данными, которые характеризовали пациента (возраст дебюта и длительность эпилепсии, возраст на момент операции, ведущий тип приступов и их электро-клиническая картина, локализация и морфологический тип КД), и тем, как и насколько радикально удалось иссечь (или изолировать) эпилептический фокус.Вероятность благоприятного исхода наиболее высока у больных со II типом фокальной КД (ФКД), которые лучше распознаются и на МРТ, и визуально во время операции, и потому иссекаются радикальнее. Зона инициации приступов при этих мальформациях соответствует и тождественна анатомическим границам ФКД и поэтому разрушается вместе с ней. Остаточная эпилептическая активность на ЭКОГ на результатах не сказывается.Пациенты со II, особенно с IIb-типом фокальной кортикальной дисплазии — наилучшие кандидаты для операции с перспективой на полное прекращение приступов при минимальной морбидности, к тому же если фокальная кортикальная дисплазия расположена вблизи от функционально критических зон коры. У детей с I типом кортикальной дисплазии шансы на прекращение приступов путем кортэктомии невелики. В части подобных случаев, особенно у детей с катастрофическим течением заболевания, несмотря на вынужденный новый неврологический дефицит, возможны расширенные резекции (лобэктомии) или дисконнекции.

Glioneuronal tumors (GNT) are usually found in children (less than 1.5% of all neoplasms of the brain). With rare exceptions, they are benign and usually manifest only by epilepsy, which is quite often resistant to treatment with AE drugs. Tumor removal usually helps to cope with epileptic seizures, however, a number of issues regarding diagnosis and surgical treatment (interpretation of morphological data and classification, epileptogenesis and topography of the epileptogenic zone, the value of intraoperative invasive EEG and the optimal volume of resection) remain debatable.To describe the morphology, electro-clinical picture and MR-semiology in patients with gloneuronal brain tumors, as well as to analyse the results of their surgical treatment and the factors determining its outcome.152 children with a median age of 8 years were treated surgically (There were 64 gangliogliomas, 73 DNT, 15 cases where the tumor classification failed - GNT NOS). In children under 2 years of age, temporal localization of the tumor prevailed. In 81 cases, ECoG was used during the operation. Surgical treatment complications: transient neurological deficit (in 15 cases); hematomas removed without consequences (in 2 cases), infectious (osteomyelitis of bone bone flap in 2 cases). We analyzed: the age of the epilepsy onset (median - 4 years 7 months) and its duration (median - 23.5 months), the type of seizures, as well as the features of MR-semiology and morphology of tumors and adjacent areas of the brain. The volume of tumor resection was verified by MRI (in 101 cases) and CT (in each case). The follow-up was collected through face-to-face meetings, with repeated video EEG and MRI, as well as telephone interviews. We studied the effect of a number of parameters characterizing the patient and features of his/her operation on the outcome of treatmen.Among 102 patients in whom the follow-up history is one year or more (median - 2 years), a favorable outcome (Engel IA) was observed in 86 of them (84%); 55 of them (54%) at the time of the last examination stopped drug AE treatment. Radical tumor removal and younger age at the time of surgery were statistically significantly associated with a favorable result.In children with gloneuronal brain tumors, removal of the tumor is effective and relatively safe in the treatment of symptomatic epilepsy. Radical tumor resection and earlier intervention are the most important prerequisites for a favorable outcome and persistent remission of seizures.Глионейрональные опухоли (ГНО) встречаются обычно у детей (менее 1,5% от всех новообразований мозга). Они, за редким исключением, доброкачественные и, как правило, проявляются только эпилепсией, весьма нередко резистентной к лечению антиэпилептическими (АЭ) препаратами. Удаление опухоли обычно помогает справиться с эпилептическими приступами, однако ряд вопросов, касающихся диагностики и хирургической тактики (трактовка морфологических данных и классификация, эпилептогенез и топография эпилептогенной зоны, значение интраоперационной инвазивной электроэнцефалографии (ЭЭГ) и оптимальный объем резекции) остаются дискуссионными.Описать морфологическую и электро-клиническую картину, а также магнитно-резонансно-томографическую (МРТ) семиологию у больных с ГНО мозга; провести анализ результатов их хирургического лечения и факторов, определяющих его исход.Оперировано 152 ребенка с медианой возраста 8 лет (у 64— ганглиоглиомы, у 73 — дизэмбриопластическая нейроэпителиальная опухоль (DNT), в 15 случаях классификация опухоли не удалась, ГНО NOS). У детей до 2 лет преобладала височная локализация опухоли. В 81 наблюдении в ходе операции использовали электрокортикографию (ЭКоГ). Осложнения операций: преходящий неврологический дефицит (15), гематомы, удаленные без последствий (2), инфекционные (остеомиелит костного лоскута — 2). Анализировали возраст дебюта эпилепсии (медиана — 4 г. 7 мес) и ее длительность (медиана — 23,5 мес), тип приступов, а также особенности МР семиологии и морфологии опухолей и прилежащих участков мозга. Объем резекции опухоли верифицировали по данным МРТ у 101 пациента и по данным компьютерных томограмм (КТ) у всех. Катамнез собирали путем очных встреч, с помощью повторных видео-ЭЭГ и МРТ, а также интервью по телефону. Изучалось влияние ряда параметров, характеризующих пациента и особенности проведенной ему операции, на исход лечения.Среди 102 пациентов, у которых катамнез составляет год и более (медиана — 2 г.), благоприятный исход (класс по шкале исходов Engel — IA) отмечен у 86 (84%); 55 (54%) из них на момент последнего осмотра прекратили лекарственное АЭ лечение. Среди различных факторов с благоприятным результатом статистически значимо ассоциировались радикальность удаления опухоли и младший возраст на момент операции.У детей с глионейрональными опухолями мозга удаление опухоли является эффективным и сравнительно безопасным в лечении симптоматической эпилепсии. Радикальная резекция опухоли и более раннее вмешательство — важнейшие предпосылки благоприятного исхода и стойкой ремиссии приступов.

BACKGROUND: To evaluate the efficacy and outcomes of the complex treatment of primary CNS germinoma in patients in non-child population, evaluate the dynamic of the ophthalmological and endocrine functions. MATERIAL AND METHODS: The retrospective analysis of 91 patients aged above 16 years with histologically verified primary CNS germinoma was performed. All patients underwent biopsy or surgery with subsequent shunting operation, if indicated. From 2 to 6 cycles (mainly 4 cycles, in 81 cases) of cisplatin and etoposide regimens combination was applied. After chemotherapy all patients underwent either whole ventricular (70 cases) or whole brain (19 cases) or craniospinal irradiation (2 cases) with 24 Gy in 12 fractions. The endocrine status of patients before and after treatment was evaluated in 44 cases. The dynamic of ophthalmological deficit was followed in 31 cases. Endocrine status at different stages of treatment was followed in 74 cases. RESULTS: All patients, who underwent combined treatment with chemotherapy and irradiation achieved a complete remission. Median follow-up time was 48 months (1–196 months). 5-year overall survival and progression-free survival rates were 96 and 92 % respectively. 7 patents out of 91 had a relapse within first 5 years of follow-up. The visual disturbances improved in 35%, oculomotor function improved in 5% of patients. Endocrine status improved in 20% of patients with initial deficit. CONCLUSION: Complex treatment of primary CNS germinoma, utilizing 4 cycles of cisplatin + etoposide regimen with subsequent 24 Gy whole ventricular or whole brain irradiation prove to be a good option, alternating craniospinal irradiation alone. Use of this regimen may preserve and improve the endocrine and ophthalmological deficit.

Material and methods In the period from 2010 to 2016. 14 patients with cavernous hemangioma (CH) and 2 patients with capillary hemangioma (CapH) of the orbit were examined. The age of CH patients varied from 17 to 67 years (median, 53 years); 8 females and 6 males. The age of CapH patients was 35 and 54 years. All patients underwent surgery with subsequent histological verification. CT-perfusion was performed in 10 CH patients and 2 CapH patients according to a developed low-dose protocol (80 kV, 200 mAs, tscan=40 s) with allowance for a target localizer (80 kV, 120 mAs) and at a maximum radiation dose of not more than 4.0 mZv. Neoplasm microcirculation was quantitatively assessed by calculating hemodynamic parameters: blood flow velocity (BFV), blood volume (BV), and mean transit time (MTT). MRI without and with contrast enhancement was performed in 11 CH patients and 2 CapH patients according to the ophthalmologic protocol (Signa GE, 3.0 T) accepted at the Institute: without contrast enhancement - T1, T2, and T2-FLAIR modes, T1 and T2 with a Fat Sat technique at a scan thickness of 3 mm, and DWI MRI; contrast enhancement - T1 (three projections) mode, including the Fat Sat technique. SWAN (n=2) and non-contrast MR perfusion ASL (n=3) were also used. Diffusion-weighted images (DWI) were processed with calculation of the apparent diffusion coefficient (ACD). Results In all CH patients, CT-perfusion revealed low perfusion parameters of blood flow: BVCH=0.86±0.37 mL/100 g, BFVCH= 4.89±2.01 mL/100 g/min with a high mean transit time MTTCH=10.13±3.05 s compared to the same parameters of blood flow in the normal white matter: CBVNormWM=1.63±2.22 mL/100 g, CBFVNormWM=9.72±3.13 mL/100 g/min, and MTTNormWM=6.76±2.78 s. In CapH cases, significantly increased blood flow velocity and volume values and a low MTT value in the tumor were observed: BVCapH=10.30±4.10 mL/100 g, BFVCapH=119.72±53.13 mL/100 g/min, and MTTCapH=4.35±1.79 s. In the case of orbital hemangiomas, optimal MRI modes were T1 and T2 with the Fat Sat technique, a scan thickness of 3 mm, and intravenous contrast enhancement. The revealed pattern of contrast agent accumulation by CH, initially in the central part and then in the periphery, may be a useful radiographic sign in the differential diagnosis with other orbital tumors. Conclusion Modern CT- and MRI-based diagnostics of orbital hemangiomas provides not only the exact location, size, and spread of the lesion but also reveals the characteristic structural features of these tumors, and the use of perfusion techniques visualizes hemodynamics of the tumors. CT-perfusion-based hemodynamic parameters of cavernous hemangiomas typical of this type of hemangiomas may be used in the differential diagnosis with other tumors of this location. The use of contrast enhancement and the Fat Sat technique with a scan thickness of not more than 3 mm is optimal for MRI diagnostics of orbital hemangiomas.

The work explores the molecular genetic features of anaplastic astrocytomas and oligodendrogliomas in a series of 43 cases. The mutational status was studied using domestic chemicals and reagent kits. We revealed clear genetic differences between astrocytic and oligodendroglial tumors and proposed an algorithm to study diagnostic and prognostic markers.Работа посвящена изучению молекулярно-генетических особенностей анапластических астроцитом и анапластических олигодендроглиом в серии из 43 наблюдений. Для изучения мутационного статуса использованы в том числе наборы реагентов и реактивов отечественного производства. Выявлены четкие генетические различия между астроцитарными и олигодендроглиальными опухолями и предложен алгоритм исследования диагностических и прогностических маркеров.

Background Angiolipomas are mesenchymal tumors composed of mature adipocytes with abnormal vascular elements. They are most commonly found in subcutaneous tissues of the trunk and are rarely seen at craniospinal axes. Case Description We present an untypical case of frontal lobe angiolipoma manifested with seizures and initially described as an arteriovenous malformation. Conclusion MRI with fat-suppression sequence plays key role in angiolipomas diagnostics.

A combined method of spectroscopic analysis of biochemical and structural markers of tumor changes, including blood volume, hemoglobin oxygen saturation, protoporphyrin IX accumulation, and change in the scattering properties, was developed on the basis of the results of simulation modeling of light propagation in media with optical properties similar to those of biotissues. The method was verified on a series of optical phantoms and applied in a clinical setting for intraoperative navigation with the aim of demarcation of glioblastoma multiforme borders. It was shown that the method developed is superior in sensitivity and specificity to the method of video-fluorescent visualization with a Carl Zeiss OPMI Pentero microscope and can be used for demarcation of the borders of tumors exhibiting infiltrative growth.

Diencephalic cachexia (DС) is progressive weight loss despite a normal caloric intake and a satisfactory state of health, which is caused by hypothalamic lesions. This is a rare (about 100 cases were reported) and potentially fatal disorder of unknown pathogenesis. At present, there is no effective pharmacological therapy for the disorder. Cachexia may regress only if the tumor reduces in size, therefore the timely diagnosis and treatment are of vital importance for the patient. DС is typical of early childhood, and only a few cases have been reported in adults. We present a rare case of DС in a 24-year-old female with papillary craniopharyngioma.Под диэнцефальной кахексией (ДК) понимают прогрессирующее снижение массы тела, обусловленное опухолевым поражением гипоталамуса. Это редкое (в литературе описано около 100 случаев), но потенциально смертельное расстройство с неизвестным до сих пор патогенезом. В настоящее время не существует эффективных фармакологических методов его лечения. Регресс кахексии может отмечаться только на фоне уменьшения объема опухоли, поэтому своевременная диагностика и лечение становятся жизненно важными для пациента. Развитие ДК характерно для раннего детского возраста и лишь единичные случаи зарегистрированы у взрослых. Мы представляем редкое наблюдение - развитие ДК у женщины 24 лет с папиллярной краниофарингиомой.

To identify mutations in cerebral cavernous malformation (CCM) genes in patients with hereditary and sporadic CCMs in the Russian population.Blood samples from 73 randomly selected patients, including 29 MRI-confirmed familial cases, 8 clinically confirmed familial cases and 38 so-called sporadic cases, were examined. A search for large deletions/duplications was performed using multiplex ligation-dependent probe amplification (MPLA). For MLPA-negative samples, the whole genome sequencing was performed to search for single nucleotide polymorphisms (SNP).Deletions in three genes (ССМ1, ССМ2, ССМ3) were identified in 14 patients, including 5 without definitely established familial type, in whom the familial character of disease was not confirmed by clinical and neuroimaging results. SNP mutations were found in 13 patients, CCM gene mutations in 27. Mutations were detected in 91.7% of familial cases. In two patients, new CCM3 deletions were identified. Gene distribution was as follows: 60.7 for CCM1, 32.2 for CCM2 and 7.1% for CCM3. In two members of a family with hereditary CCMs, no high effect mutations in the known CCM genes were found. Patients with mutations had greater severity of disease. Two patients with CCM3 mutations demonstrated the most aggressive clinical course. De novo formation and growth of CCM were observed only in patients with mutations.The distribution of pathogenic mutations in known CCM genes is consistent with other large-scale studies. Familial CCMs are associated with more severe disease course and may be caused by mutations beyond the known CCM genes.Цель исследования. Определение мутаций генов, ассоциированных с формированием церебральных кавернозных мальформаций, у больных с наследственной формой заболевания в российской популяции. Материал и методы. Генетические исследования выполнены у 73 больных: у 29 - с наследственной формой заболевания, у 6 - с условно доказанными семейными каверномами и у 38 - со спорадическими. Во всех случаях выполнен поиск крупных мутаций методом мультиплексной амплификации лигазносвязанных проб. Если крупные изменения генов у больных с клинически доказанными наследственными формами обнаружены не были, выполняли полноэкзомное секвенирование для выявления и анализа точечных мутаций и полиморфизмов. Результаты. В 14 образцах выявлены крупные перестройки в трех известных генах кавернозных мальформаций (ССМ1, ССМ2, ССМ3), в том числе у 5 больных с неустановленным по клинико-нейровизуализационным данным наследственным характером болезни. Всего с однонуклеотидными мутациями было 13 больных, а с мутацией в генах кавернозных мальформаций - 27. Мутации в генах выявлены в 91,7% обследованных семей. В 2 случаях найдены ранее не описанные делеции в гене CCM3. Соотношение генов CCM1, CCM2 и CCM3 составило 60,7, 32,2 и 7,1% соответственно. У 2 человек из семьи с наследственными каверномами в известных генах кавернозных мальформаций мутаций с высоким эффектом не обнаружено. Для больных с мутациями было характерно более тяжелое течение заболевания. Наиболее агрессивное клиническое течение отмечалось при мутациях в гене ССМ3. Случаи роста и новообразования каверном наблюдались только у больных с мутациями в генах кавернозных мальформаций. Заключение. Распределение частоты мутаций в трех генах кавернозных мальформаций в нашей работе и по данным других крупных исследований совпадает. Наследственные каверномы склонны к более тяжелому клиническому течению. Наследственные формы каверном могут быть обусловлены мутациями вне известных генов кавернозных мальформаций.

The median overall survival of glioblastoma patients is about 15 months. Only a small number of patients survive 3 years. The factors of a favorable prognosis for the 'longevity phenomenon' in glioblastoma patients are not fully understood.to determine the occurrence rate of long-living patients with glioblastomas, identify clinical predictors of a favorable prognosis, and identify the presence and prognostic significance of an IDH1 mutation.Among 1494 patients operated on for glioblastoma at the Burdenko Neurosurgical Institute from 2007 to 2012, there were 84 (5.6%) patients who lived more than 3 years after primary surgery. In all the cases, histological specimens were reviewed, and immunohistochemical detection of a mutant IDH1 protein was performed. Overall survival was calculated from the time of first surgery to the date of the last consultation or death, and the recurrence-free period was calculated from the time of first surgery to MRI-verified tumor progression.The median age of long-living patients with glioblastoma was 45 years (19-65 years). All tumors were located supratentorially. The median Karnofsky performance status score at the time of surgery was 80 (range, 70-100). All patients underwent microsurgical resection of the tumor, followed by chemoradiotherapy. The median recurrence-free period was 36 months (5-98 months). Overall survival of 48, 60, and 84 months was achieved in 23, 15 and 6% of patients, respectively. Among 49 specimens available for the IDH1 analysis, 14 (28.6%) specimens had a mutant protein. There was no significant difference in survival rates in patients with positive and negative results for IDH1 (44.1 vs. 40.8 months; p0.05).The significance of various factors that may be predictors of a favorable course of the disease is discussed in the literature. This work is the first part of analysis of prognostically significant factors positively affecting overall survival of glioblastoma patients. In our series, the predictors of a favorable prognosis for long-living patients with the verified diagnosis of glioblastoma were as follows: young age, the supratentorial location of the tumor, a high Karnofsky score before surgery, and tumor resection. In our series, we used immunohistochemical tests and found no prognostic significance of the IDH1 gene mutation; further analysis will require application of direct sequencing. We plan to study other morphological and molecular genetic features of tumors, which explain prolonged survival of glioblastoma patients, as well as the role of various types of combined chemoradiation treatment.Медиана общей выживаемости пациентов с глиобластомами составляет около 15 мес. Лишь небольшое количество пациентов переживают 3 года. Факторы благоприятного прогноза при 'феномене долгожительства' у больных с глиобластомами в настоящее время до конца не изучены. Цель исследования - определить частоту встречаемости пациентов-'долгожителей' с глиобластомами, идентифицировать клинические предикторы благоприятного прогноза, выявить наличие и прогностическую значимость мутации IDH1. Материал и методы. Среди 1494 пациентов, оперированных в Институте нейрохирургии им. Н.Н. Бурденко (ныне - Национальный научно-практический центр нейрохирургии им. акад. Н.Н. Бурденко) с 2007 по 2012 г. по поводу глиобластом, выявлены 84 (5,6%) пациента, проживших более 3 лет после первичной операции. Относительно всех пациентов гистологические препараты были пересмотрены, проведено иммуногистохимическое исследование на наличие мутантного протеина IDH1. Общая выживаемость высчитывалась с момента первой операции до даты последней консультации или смерти, безрецидивный период - с момента первой операции до прогрессии опухоли по данным МРТ-исследования. Результаты. Медиана возраста у пациентов-'долгожителей' с глиобластомой составила 45 лет (в среднем 19-65 лет). Все опухоли были расположены супратенториально. Медиана индекса функционального состояния по шкале Карновского на момент операции составила 80 (диапазон 70-100). Всем пациентам было проведено микрохирургическое удаление опухоли с последующей химиолучевой терапией. Медиана безрецидивного периода составила 36 мес (5-98 мес). Уровень общей выживаемости в 48, 60 и 84 мес был достигнут у 23, 15 и 6% пациентов соответственно. Среди 49 образцов, доступных для анализа на IDH1, 14 (28,6%) имели мутантный протеин. Достоверной разницы показателей выживаемости у пациентов с положительным и отрицательным IDH1 не выявлено (44,1 мес против 40,8 мес; p0,05). Заключение. Значимость различных факторов, которые могут являться предикторами благоприятного течения болезни, обсуждаются в литературе. Данная работа является первым фрагментом анализа прогностически значимых факторов, позитивно влияющих на общую выживаемость больных с глиобластомой. В нашей серии предикторами благоприятного прогноза для пациентов-'долгожителей' с верифицированным диагнозом 'глиобластома' оказались: молодой возраст, супратенториальное расположение опухоли, высокий индекс Карновского до операции, удаление опухоли. Прогностическая значимость мутации в гене IDH1 с использованием иммуногистохимических методов в нашей серии не выявлена; для дальнейшего анализа необходимо использование метода прямого секвенирования. Планируется проведение исследований других морфологических и молекулярно-генетических особенностей опухолей, объясняющих факт 'долгожительства' пациентов с глиобластомой, а также роли различных вариантов комбинированного химиолучевого лечения.

Hippocampal sclerosis, also known as Ammon horn sclerosis or mesial temporal sclerosis, is usually associated with intractable epilepsy and characterized by specific patterns of neuronal loss and gliosis in the medial temporal lobe structures. Hippocampal sclerosis manifests clinically as epilepsy, often intractable epilepsy; in most cases, this condition is surgically treatable. As the most common histological diagnosis in adult patients subjected to epilepsy surgery, hippocampal sclerosis is characterized by complex pathogenesis and requires a multidisciplinary approach to its diagnosis and treatment. This article reviews the pathologic features, natural history, pathogenesis, and electroclinical and MRI signs of hippocampal sclerosis.Склероз гиппокампа, или же склероз Аммонова рога, или же медиальный темпоральный склероз представляет собой совокупность макроскопических и микроскопических изменений, характеризующихся уменьшением объема нервной ткани, выпадением нейронов, пролиферацией глии в медиальных отделах височной доли. Клинически склероз гиппокампа проявляется эпилепсией, часто не контролируемой лекарствами, однако в значительном числе случаев излечиваемой хирургически. Являясь самым частым гистологическим диагнозом в хирургии эпилепсии у взрослых пациентов, склероз гиппокампа имеет сложный патогенез и требует мультидисциплинарного подхода в диагностике и лечении. В обзоре приведены современные данные о патогенезе и клинической картине заболевания, гистологической классификации, электроклинической и МРТ-диагностике симптоматической эпилепсии, связанной с гиппокампальным склерозом.

An increased blood level of the thyroid stimulating hormone (TSH) is usually associated with primary hypothyroidism (PHT) but can also be observed in such rare cases as TSH-secreting pituitary tumor. The article describes four clinical cases of elevated TSH blood levels: 1) TSH-secreting pituitary adenoma with hyperthyroidism; 2) TSH-secreting adenoma with hypothyroidism; 3) hormonally inactive pituitary adenoma combined with primary hypothyroidism; 4) reversible thyrotropic hyperplasia. These clinical situations substantiate the importance of considering different diagnoses in a patient with a pituitary gland tumor associated with an increased TSH blood level.Повышение уровня тиреотропного гормона (ТТГ) в крови, как правило, вызвано первичным гипотиреозом, но может наблюдаться и в таких редких случаях, как ТТГ-секретирующая опухоль гипофиза. В статье рассматриваются четыре клинических случая повышения уровня ТТГ в крови у пациентов: 1) ТТГ-секретирующая аденома гипофиза с гипертиреозом; 2) ТТГ-секретирующая аденома с гипотиреозом; 3) гормонально-неактивная аденома гипофиза в сочетании с первичным гипотиреозом; 4) случай обратимой тиреотрофной гиперплазии. Эти клинические ситуации обосновывают важность рассмотрения различных диагнозов у пациента с опухолью гипофиза, сопровождающейся повышением уровня ТТГ в крови.

An extended endoscopic endonasal approach is increasingly used in surgical treatment of space-occupying skull base lesions. The international literature reports only 20 cases of surgical treatment for fibrous dysplasia (PD) of the skull base using the endoscopic endonasal approach. We present our experience with the endoscopic endonasal approach in surgical treatment for giant fibrous dysplasia of the skull base, spreading to the right orbital cavity and nasopharynx.A 26-year-old male patient presented with cranial pain, Vth nerve dysfunction on the right, right keratopathy. OD=0.2 (near acuity - 0.3), OS=1.0, OD - incomplete eyelid closure of 2 mm, conjunctival injection, mucous discharge, corneal opacity in the lower pole and paracentrally, OS - normal appearance. Severe right-sided exophthalmos (more than 15 mm), impaired nasal breathing on the right, nasal (hemorrhagic) discharge. Magnetic resonance imaging and spiral computed tomography scans revealed a bone density lesion located in the area of the orbit, nasal cavity, maxillary sinus on the right, and labyrinth of the ethmoid bone. The patient underwent endonasal endoscopic resection of the lesion.The lesion was resected totally, which was confirmed by control SCT. Right-sided exophthalmos partially regressed (on the right: exophthalmos of 8 mm; protrusion: OD=23 mm, OS=15 mm; the eyeball was displaced downward and outward). The visual and oculomotor functions did not change. The neurological status remained at the preoperative level.Fibrous dysplasia of the skull base is an extremely rare disease. Modern techniques expand the indications for surgery of giant tumors of the skull base using minimally invasive approaches, in particular the endoscopic endonasal approach.Расширенный эндоскопический эндоназальный доступ находит все большее применение в хирургическом лечении объемных образований основания черепа. Представляем наш опыт оперативного лечения эндоскопическим эндоназальным доступом гигантского патологического очага фиброзной дисплазии основания черепа, распространяющегося в полость правой орбиты и носоглотку. Клинический случай. Мужчина, 26 лет, с краниальным болевым синдромом, недостаточностью V нерва справа, кератопатией справа. OD=0,2 (вблизи — 0,3), OS=1,0, OD — несмыкание век 2 мм, конъюнктивальная инъекция, слизистое отделяемое, в нижнем полюсе роговицы и парацентрально помутнение роговицы, OS — спокоен. Выраженный правосторонний экзофтальм (более 15 мм), нарушение носового дыхания справа, выделения (геморрагического характера) из носа. На магнитно-резонансных томограммах и спиральных компьютерных томограммах выявлено образование костной плотности в области орбиты, полости носа, верхнечелюстной пазухи справа, лабиринта решетчатой кости. Произведено эндоскопическое эндоназальное удаление патологического очага. Результаты. Удаление патологического очага тотальное, что подтверждено контрольной СКТ. Частичный регресс правостороннего экзофтальма (справа — экзофтальм 8 мм, выстояние OD=23 мм, OS=15 мм, глазное яблоко смещено книзу и кнаружи). Зрительные и глазодвигательные функции без динамики. Неврологический статус на дооперационном уровне. Заключение. Фиброзная дисплазия основания черепа является крайне редким заболеванием. Современные технологии позволяют расширять показания к оперативному лечению гигантских новообразований основания черепа с использованием минимально инвазивных доступов, а именно эндоскопического эндоназального доступа.

Thyrotropinomas (TSH-secreting tumors) are a rare type of pituitary adenomas, which account for about 0.5-2.0% of all pituitary tumors. The criterion of thyrotropinoma is visualization of the tumor in the presence of a normal or elevated level of the thyroid-stimulating hormone (TSH) in the blood and elevated concentrations of free T4 (fT4) and free T3 (fT3).To study the clinical, diagnostic, and morphological characteristics and treatment outcomes of TSH-secreting pituitary tumors.The study included 21 patients aged from 15 to 67 years with pituitary adenoma and a normal or elevated blood TSH level combined with elevated fT4 and fT3 levels who were operated on at the Neurosurgical Institute in the period between 2002 and 2015. Before surgery, in the early postoperative period, and 6 months after surgery, the patients were tested for levels of TSH, fT4, fT3, prolactin, cortisol, the luteinizing hormone (LH), the follicle-stimulating hormone (FSH), estradiol/testosterone, and the insulin-like growth factor (IGF-1). The thyroid status was evaluated using the following reference values: TSH, 0.4-4.0 mIU/L; fT4, 11.5-22.7 pmol/L; fT3, 3.5-6.5 pmol/L. An immunohistochemical study of material was performed with antibodies to TSH, PRL, GH, ACTH, LH, FSH, and Ki-67 (MiB-1 clone); in 13 cases, we used tests with antibodies to somatostatin receptors type 2 and 5 and to D2 subtype dopamine receptors.Thyrotropinomas were detected in patients aged from 15 to 67 years (median, 39 years), with an equal rate in males (48%) and females (52%). Before admission to the Neurosurgical Institute, 11 (52%) patients were erroneously diagnosed with primary hyperthyroidism; based on the diagnosis, 7 of these patients underwent surgery on the thyroid gland and/or received thyrostatics (4 cases). Hyperthyroidism symptoms were observed in 16 (76%) patients. The blood level of TSH was 2.47-38.4 mIU/L (median, 6.56); fT4, 22.8-54.8 nmol/L (median, 36); fT3, 4.24-12.9 pmol/L (median, 9.66). Tumors had the endosellar localization in 4 (19%) cases and the endo-extrasellar localization in 17 (91%) cases. Total tumor resection was performed in 7 (33%) patients. All these tumors had the endosellar and endo-suprasellar localization. No total resection was performed in patients with infiltrative growth of adenoma (invading the skull base structures). An immunohistochemical study of tumor resection specimens detected only TSH expression in 3 (14%) cases; 18 (86%) tumors were plurihormonal and secreted TSH and GH and/or PRL. Of 13 tumors, expression of the type 2 dopamine receptor was detected in 9 (69%) cases; expression of somatostatin receptors type 5 and type 2 was found in 6 (46%) and 2 (15%) cases, respectively.The criterion for total tumor resection was a postoperative decrease in the TSH level to 0.1 mIU/L or less. Total resection was performed in 33% of patients with tumors of only the endosellar and endo-suprasellar localization. In most cases, tumors were plurihormonal and secreted TSH and GH and/or PRL.Тиреотропиномы (ТТГ-секретирующие опухоли) - редкий тип аденом гипофиза, на долю которых приходится около 0,5-2,0% всех опухолей гипофиза. The expression profiles of 10 mature microRNA (7, 10a, 17, 20a, 21, 23a, 26a, 137, 222) in biopsy specimens of gliomas of different malignancy were studied by the real time PCR with SYBR Green I fluorescent stain. The expression of microRNA-21 increased significantly, while that of microRNA-137 decreased, depending on the tumor malignancy. The expression of microRNA-9, -17, -20a, -23a, -26a was significantly higher, while that of microRNA-7 lower in the tumor vs. control tissue samples. New data on the molecular pathological mechanisms of gliomas related to their malignancy were obtained. Quantitative analysis of microRNA was suggested as a potential diagnostic marker of brain tumors.

The paper describes a clinical case of atypical teratoid/rhabdoid tumor with preserved INI1 expression and SMARCA4 gene mutations in an 8-month-old girl. Genome-wide DNA methylation, hierarchical clustering, and next-generation sequencing were used to make a tumor diagnosis. However, BRG1 immunohistochemical examination may be recommended in the routine practice of diagnosis and study of childhood CNS malignant tumors.Представлено клиническое наблюдение атипической тератоидно-рабдоидной опухоли с сохранной экспрессией INI1 и мутациями гена SMARCA4 у 8-месячной девочки. Для диагностики опухоли использовали методы выявления структуры метилирования ДНК на уровне генома, иерархическое кластерирование и секвенирование нового поколения. Однако в рутинной практике диагностики и исследования детских злокачественных опухолей ЦНС может быть рекомендовано иммуногистохимическое исследование с BRG1.

Sarcoidosis is a multisystem granulomatous disorder of unknown nature. Patients often present with pulmonary, skin, eye, and orbital lesions. Involvement of the central nervous system (CNS) is accompanied by granulomatous leptomeningitis and damage to the basal brain structures with formation of granulomas near the cranial nerves, hypothalamus, pituitary gland, cavernous sinuses, optic chiasm, and intracranial optic nerves. The optic nerves can be affected independently of the other CNS regions, which may be the first manifestation of the disease. The article presents two clinical cases of sarcoidosis affecting the anterior visual pathway. Diagnosis of the disease was associated with certain difficulties. A biopsy revealed a sarcoidosis lesion.Саркоидоз является мультисистемным гранулематозным заболеванием, природа которого неизвестна. Клинические проявления наблюдаются чаще со стороны легких, кожи, глаз и орбиты. При поражении центральной нервной системы (ЦНС) имеет место гранулематозный лептоменингит, а также поражение структур основания мозга с формированием гранулем около черепно-мозговых нервов, в области гипоталамуса, гипофиза, кавернозных синусов, хиазмы, интракраниальных отделов зрительных нервов. Зрительные нервы могут поражаться независимо от других отделов ЦНС, что может быть первым проявлением заболевания. В статье представлены два клинических наблюдения поражения структур переднего зрительного пути саркоидозом. Диагностика заболевания имела определенные трудности. Биопсия образования позволила уточнить саркоидозную природу поражения.

Basal ganglia germinomas are a specific group of intracranial germinomas. Their early diagnosis is complicated due to their atypical localization and diversity of neuroimaging and clinical signs.We describe 4 cases of basal ganglia germinoma in boys of 13, 14, 15, and 16 years of age. The medical history data, clinical features, neuroimaging and histological characteristics of basal ganglia germonomas, and preliminary results of the treatment are presented.Basal ganglia germinomas are usually verified at the late stage of the disease when patients are detected with extended lesions of the basal ganglia and severe neurological and neuroendocrine deficits. This situation is due to clinical and imaging signs that are untypical of common germinomas.Герминомы подкорковых узлов занимают особое место среди интракраниальных гермином. Их сложно диагностировать на ранней стадии болезни в связи с нетипичной локализацией, разнообразием нейровизуализационных проявлений и клинической картины. Материал и методы. В статье описываются 4 случая герминомы подкорковых узлов. Все пациенты мужского пола в возрасте 13-16 лет. Приводятся данные анамнеза, особенности клинического течения, нейровизуализационные и гистологические характеристики гермином, предварительные результаты лечения. Выводы. Герминомы подкорковых узлов, как правило, верифицируются на поздней стадии заболевания, когда у пациентов выявляется распространенное поражение подкорковых узлов с выраженными неврологическими и нейроэндокринными нарушениями. Это обусловлено нехарактерными для типичной герминомы клинико-визуализационными данными.

Pineoblastoma is a rare malignant tumor of the central nervous system (CNS), which arises from the parenchyma of the pineal gland. It is characterized by aggressive clinical behavior and frequent metastases along the craniospinal axis. Extraneural metastases may occur due to surgical seeding of tumor cells beyond the dura and/or hematogenous spread, ventriculoperitoneal shunting, or through Batson's plexus. To our knowledge, only six documented cases of intraosseous metastases of pineoblastoma are described in the literature. A 23-year-old female patient presented with clinical and radiological symptoms of a pineal tumor causing secondary hydrocephalus. After initial surgical treatment, chemotherapy, and local radiotherapy with craniospinal irradiation, she developed multiple metastases affecting the anterior skull base, intracranial meninges, frontal bone, and finally, the entire vertebral column. The patient received surgical treatment for the anterior skull base metastasis, repeated irradiation of the neuraxis, radiosurgical and radiotherapeutic procedures, and chemotherapy. The patient survived 57 months after the primary disease manifestation and died of multiple metastases. This presented case is the first known description of metastasis of pineoblastoma in the anterior cranial base. Multiple intracranial metastases were suppressed using CyberKnife radiation treatment and chemotherapy until massive involvement of spinal column occurred. Interestingly, no signs of brain radiation necrosis after repeated radiation treatments were observed, and the patient developed only moderate neurocognitive decline.

Fluorescence diagnostics has been extensively applied in surgery of malignant brain gliomas. However, the use of this technique in surgery of intracranial meningiomas has remained controversial.The study objective was to assess the sensitivity of 5-aminolevulinic acid-based (5-ALA) fluorescence diagnostics in surgery of brain meningiomas and to clarify the clinical and biological factors that may influence the fluorescent effect.The study consistently included 101 patients with intracranial meningiomas of various locations who were operated on using 5-ALA. There were 28 (27.72%) males and 73 (72.27%) females (median age, 54 years). In all patients, surgery was performed using an operating microscope equipped with a fluorescent module; in 24 of these, laser spectroscopy was used. For comparison of chances to observe the fluorescent effect of 5-ALA in patients having meningiomas with different WHO histological grades (Grade I vs Grade II-III), we performed a meta-analysis that included 10 studies (the largest series) on outcomes of surgical treatment of meningiomas using intraoperative fluorescence diagnostics.Of 101 patients included in this series, observable fluorescence was detected in 95 (94.1%) patients: weak fluorescence in 12 (11.9%), moderate fluorescence in 23 (22.8%) cases, and strong fluorescence in 60 (59.4%) patients. There was no statistically significant relationship (p0.05) between the rate and intensity of observable fluorescence and the tumor growth pattern (primary/continued), location, WHO grade of malignancy, and histological subtype. In the absence of intraoperative bleeding, tumor fluorescence was statistically significantly brighter (p=0.02). Of 26 patients with hyperostosis, bone fluorescence was observed in 11 (42.3%) cases. There was no statistically significant relationship between administration of dexamethasone, its dose, administration of anticonvulsants, gastrointestinal tract diseases, as well as diabetes mellitus and the fluorescence intensity. There was also no significant relationship between the extent of tumor resection (Simpson scale) and the presence of fluorescence as well as its intensity. Comparison of the observable fluorescence intensity and the laser spectroscopy indicators revealed a significant correlation (r=0.75; p=0.005).Meningioma is a well fluorescent tumor, with the technique sensitivity being 94.1%. In some cases, the use of fluorescence diagnostics in surgery of meningiomas improves identification of residual tumor fragments and enables correction of a surgical approach. To assess the effect of fluorescence diagnostics on the recurrence rate and disease-free duration, further research is required.Использование флуоресцентной диагностики (ФД) нашло широкое применение в хирургии злокачественных глиом головного мозга. Однако использование данного метода в хирургии интракраниальных менингиом остается дискутабельным. Цель исследования - оценить чувствительность метода ФД с использованием 5-аминолевулиновой кислоты (5-АЛК) в хирургии менингиом головного мозга и уточнить клинические и биологические факторы, влияющие на флуоресцентный эффект. Материал и методы. В исследование последовательно включен 101 пациент (28 (27,72%) мужчин и 73 (72,27%) женщины, средний возраст 54 года) с интракраниальными менингиомами различной локализации, которые были оперированы с использованием 5-АЛК. Все операции проводили под операционным микроскопом, оснащенным флуоресцентным модулем, и у 24 из них была использована лазерная спектроскопия. Для сравнения шансов получить флуоресцентный эффект при использовании 5-АЛК у пациентов с разным гистологическим типом менингиом по классификации ВОЗ (Grade I в сравнении с Grade II-III) проведен метаанализ 10 работ (наиболее крупные серии), посвященных хирургическому лечению менингиом с применением интраоперационной ФД. Результаты. Из 101 пациента, включенного в данную серию, видимая флуоресценция наблюдалась у 95 (94,1%) пациентов: слабая - у 12 (11,9%), умеренная - у 23 (22,8%) и выраженной интенсивности - у 60 (59,4%). Статистически значимой взаимосвязи частоты эффекта флуоресценции и степени ее выраженности с характером роста опухоли (первичный/продолженный), локализацией, степенью злокачественности по классификации ВОЗ, гистологическим подтипом не выявлено (p0,05). При отсутствии кровотечения во время оперативного вмешательства флуоресценция опухоли была статистически значимо ярче (p=0,02). Из 26 пациентов с гиперостозом флуоресценция кости наблюдалась у 11 (42,3%). Статистически значимой зависимости между приемом дексаметазона, его дозой, приемом противосудорожных препаратов, наличием заболеваний ЖКТ, СД и степенью флуоресценции не выявлено. Достоверной зависимости между радикальностью удаления опухоли по Simpson и свечением, а также интенсивностью флуоресценции также не выявлено. При сопоставлении визуальной степени флуоресценции с показателями лазерной спектроскопии выявлена достоверная корреляция (r=0,75; р=0,005). Заключение. Менингиома является хорошо флуоресцирующей опухолью c чувствительностью метода 94,1%. Применение ФД в хирургии менингиом позволяет в ряде случаев улучшить выявление остаточных фрагментов опухоли и изменить тактику хирургического вмешательства. Для оценки влияния флуоресцентной диагностики на частоту рецидивов и длительность безрецидивного периода необходимы дальнейшие исследования.

since the 1990s, the literature has described cases of glioblastoma metastases with the development of foci located at a distance from the primary tumor. However, the pathogenesis of this process remains unclear until the end. This focus is believed to result, on the one hand, from tumor metastasis from the primary site and, on the other hand, from multifocal growth. This article presents a literature review and a description of clinical observations of patients with glioblastoma metastases.The study included 6 patients (1 female and 5 males) with brain glioblastomas who received treatment at the Burdenko Neurosurgical Institute (5 patients) and the Department of Neurosurgery of the Research Center of Neurology (1 patient) in the period from 2010 to 2014. Neurophysiological control was used if the tumor was localized near the eloquent cortical areas and pathways; 4 of 6 patients were operated on using the methods of intraoperative fluorescence diagnosis (5-ALA agent--Alasens).Four patients had metastases within one hemisphere, two had metastases in the contralateral hemisphere in the period of 5 to 18 months after the first operation. The primary tumor site was located near the ventricular system in two patients. In one patient, the lateral ventricle was opened during the first operation. In another patient, the prepontine cistern was opened during the first operation. In two patients, the primary tumor site was located at a distance from the lateral ventricles, however, the tumor was located near them during recurrence. Based on metabolic navigation, fluorescence of the tumor was observed in the four patients during both the first and repeated operations.The close relationship between primary glioblastomas and metastases and the cerebrospinal fluid circulation pathways may confirm the fact of dissemination of tumor cells with cerebrospinal fluid flow. In our opinion, there should be an increased suspicion of the possibility for metastases of glioblastomas that are closely associated with the cerebrospinal fluid circulation pathways. Metabolic navigation with 5-ALA is effective both during primary surgery in patients with glioblastomas and during resection of glioblastoma metastases.В литературе с 1990 г. описаны случаи метастазов глиобластом с образованием очагов, расположенных на отдалении от первичной опухоли. Однако остается до конца неясным патогенез данного процесса. Считается, что такой очаг является, с одной стороны, следствием метастазирования опухоли из первичной зоны, с другой - результатом мультифокального роста. В статье приводятся данные обзора литературы и описания клинических наблюдений пациентов с метастазами ГБ. Материал и методы. В исследование были включены 6 пациентов (1 женщина и 5 мужчин) с глиобластомами головного мозга, получивших лечение в НИИ нейрохирургии им. акад. Н.Н. Бурденко (5 пациентов) и нейрохирургическом отделении Научного центра неврологии РАН (1 пациент) в 2010-2014 гг. При локализации опухоли вблизи функционально значимых корковых зон и проводящих путей использовался нейрофизиологический контроль; 4 из 6 пациентов оперированы с использованием методов интраоперационной флуоресцентной диагностики (препарат 5-аминолевулиновой кислоты (5-АЛК) Аласенс, производитель ГНЦ НИОПИК, Россия). Результаты. У 4 пациентов наблюдались метастазы в пределах одного полушария, у 2 - в контралатеральном полушарии в срок от 5 до 18 мес после первой операции. У 2 из них первичный очаг опухоли располагался вблизи желудочковой системы, во время первой операции у 1 из 6 больных был вскрыт боковой желудочек, у другого - околостволовая цистерна. У 2 других из числа прооперированных первичный очаг опухоли находился на удалении от боковых желудочков. Метастазы глиобластом локализовались вблизи путей ликвороциркуляции (боковые желудочки и парастволовые цистерны) во всех наблюдениях. При использовании метаболической навигации с 5-АЛК у 4 пациентов флуоресценция опухоли отмечалась как при первой (удалении первичного очага) операции, так и при повторной (удалении метастаза). Выводы. Тесная связь первичных глиобластом и метастазов с путями ликвороциркуляции может служить подтверждением факту распространения опухолевых клеток с током ликвора. На наш взгляд, необходимо с повышенной осторожностью относиться к возможности развития метастазов глиобластом при их тесной связи с путями ликвороциркуляции. У пациентов с глиобластомами метаболическая навигация с 5-АЛК эффективна при операциях как первичных, так и последующих по удалению их метастазов.

Myxoma of the peripheral nerve sheath is a rare benign tumor with predominant localization in the upper extremities, head, neck, and chest. In this study, we reported a clinical case of a patient with intradural myxoma at the L1 level. Much attention was given to histological characterization of the tumor and differential diagnosis of histological types of benign extramedullary tumors. A conclusion was drawn that patients with myxoma need further thorough examination as there is a risk of generalization of tumor process.Миксома оболочек периферических нервов является редкой доброкачественной опухолью с преимущественной локализацией в верхних конечностях, голове, шее и грудной клетке. Приведен клинический случай интрадуральной миксомы на уровне LI позвонка. Большое внимание уделено гистологической характеристике опухоли и проведению дифференциальной диагностики между гистологическими типами доброкачественных экстрамедуллярных образований. В результате работы был сделан вывод о необходимости тщательного дополнительного обследования пациентов в случае выявления миксомы, так как возможна генерализация процесса.

Thyrotropinomas (TSHomas) are rare pituitary adenomas, particularly in childhood. We present here the case of an 11-year-old boy with type 1 autoimmune polyglandular syndrome (APS1) and TSHoma which was diagnosed by elevated thyroid – stimulating hormone and thyroid hormones levels without evident clinical signs of hyperthyroidism. He was underwent partial resection of the tumor via transsphenoidal approach and subsequently radiation therapy. Consequently, 1 year after radiotherapy, the patient developed growth hormone deficiency, three and half years after radiation became euthyroid, and five and half years after treatment – hypothyroid. This is the first case of the coexistence of these two rare endocrine diseases in one patient.

Context.—Routine pathologic examination cannot distinctively predict the clinical course of meningiomas because even histologically benign tumors may recur after gross total resection. Therefore, numerous efforts have been made to evaluate the meningioma growth fraction and its prognostic value. However, a universally applicable proliferative marker for meningioma outcome is not yet a reality. Objective.—To investigate the prognostic utility of 3 proliferative markers, namely, Ki-67, DNA topoisomerase II-α (topoII), and cyclin A in a representative series of intracranial meningiomas. Design.—Two hundred sixty-three adult patients with intracranial meningiomas (208 benign, 42 atypical, and 13 anaplastic) were studied retrospectively. Tumor specimens were immunohistochemically examined with antibodies to Ki-67 (MM-1), topoII, and cyclin A. A computerized color image analyzer was used to count immunostained nuclei. Results.—The topoII and cyclin A scores exhibited a close correlation with Ki-67 immunostaining. Significant differences between the indices for all 3 markers were noted among the 3 grades of meningiomas. The scores for all 3 markers were significantly different between recurrent and nonrecurrent meningiomas, including benign tumors that were treated with gross total resection. Recurrence-free survival was significantly reduced for cases with a Ki-67 labeling index (LI) of 4.4% or greater, a topoII LI of 3.2% or greater, and a cyclin A LI of 3.1% or greater. Multivariate analysis revealed that the risk of recurrence for the entire meningioma cohort was significantly associated with tumor grade (hazard ratio = 2.7; P = .004), topoII LI of 3.2% or greater (hazard ratio = 5.5; P < .001), and a cyclin A LI of 3.1% or greater (hazard ratio = 2.4; P = .01). Conclusions.—There is a close correlation in the expression of these 3 proliferative markers in meningiomas, and all of the markers showed a significant association with tumor grade, recurrence rate, and recurrence-free survival. Consequently, in addition to Ki-67, immunoexpression of topoII and cyclin A is available for predicting meningioma recurrence. Moreover, the topoII and cyclin A staining scores were found to be more sensitive predictors for meningioma progression than Ki-67 and, therefore, either of these 2 markers may prove to be clinically informative and useful.

Background MicroRNAs (miRNAs) are a class of small non-coding RNA molecules involved in the regulation of key biological processes. Different miRNAs with pro-oncogenic and anti-oncogenic properties have been identified in glioblastomas. We decided to analyze expression profiles of 10 mature miRNAs (miR-7-1, miR-10а, miR-17, miR-20а, miR-21, miR-23а, miR-26а, miR-137, and miR-222) in post-surgery glioma specimens of different grades in order to find whether the expression level correlates with tumor grades. We also measured expression of six key genes such as PTEN, p21/CDKN1A, MDR1, ABCG2, BAX, and BCL-2 involved in the regulation of critical glioma signaling pathways to establish the effect of miRNAs on these signaling mechanisms. Methods Using RT-PCR, we performed expression analysis of 25 tumor fresh samples (grades II-IV). Results We found gradual increase in miR-21 and miR-23a levels in all tumor grades whereas miR-7 and miR-137 were significantly down-regulated depending on the glioma grade. MDR, ABCG2, and p21/CDKN1A levels were significantly up-regulated while expression of PTEN was down-regulated in tumor samples compared to the normal brain tissue. Conclusions These observations provide new insights into molecular pathogenic mechanisms of glioma progression and suggest about a potential value of miRNAs as a putative diagnostic marker of brain tumors.

Hemangioblastoma is a rare CNS vascular tumor that develops sporadically and can also be associated with von Hippel-Lindau disease. Hemangioblastomas account for 2-6% of all spinal cord tumors and are ranked third in the structure of intramedullary space-occupying lesions of the spinal cord. For the first time in our practice, we observed a dumbbell paravertebral hemangioblastoma. The international literature reports only 3 cases of the tumor with this growth type.Гемангиобластома - редко встречающаяся сосудистая опухоль ЦНС, которая возникает спорадически, а также бывает ассоциирована с болезнью Гиппеля-Линдау. Large parasagittal meningiomas, in particular hyperostotic ones, in children are rare and problematic in the differential diagnosis. The literature reports only single clinical cases related to this issue; opinions about the indications, surgical treatment options, and prognosis are contradictory. This paper presents a clinical case of a hyperostotic parasagittal meningioma with intra-extracranial growth in a 10-year-old boy with neurofibromatosis type II significantly worsening the prognosis. We discuss the epidemiological and clinical features of childhood meningiomas and issues of their diagnosis, treatment, and prognosis.В детском возрасте крупные парасагиттальные менингиомы, в том числе сопровождающиеся гиперостозом, являются редким наблюдением и представляют сложность при дифференциальной диагностике. В литературе описаны лишь отдельные клинические случаи, посвященные этой проблеме; мнения о показаниях и видах оперативного лечения, а также прогнозе противоречивы. В данной работе представлено клиническое наблюдение гиперостотической парасагиттальной менингиомы с интра-экстракраниальным ростом у ребенка 10 лет с нейрофиброматозом II типа, который значительно ухудшает прогноз. Objective This study is aimed at revealing the relationship between local interictal epileptic activity, cytoarchitectural disturbances and magnetic resonance imaging (MRI) findings. Material and methods We prospectively investigated a series of 25 patients with symptomatic epilepsy due to isolated forms of focal cortical dysplasia (FCD) or hippocampal sclerosis and low-grade tumours associated with FCD, all of whom underwent tailored surgical procedures under intraoperative electrocorticography; we conducted neuropathological examinations of 92 biopsies taken from different places. We examined the relationship between dysplastic changes in the cortex and the absence or presence of seizure patterns (SPs), including regular spikes/sharp waves, recruiting discharges, paroxysmal fast activity and rhythmic delta–theta activity. Comparisons with MRI findings were also performed. Results Complete removal of the SPs zone was associated with better results of surgical treatment. Areas with isolated architectural abnormalities were associated with SPs significantly more often than those where the cortex contains immature or giant neurons; these areas were associated with SPs more than areas containing dysmorphic neurons. The extent of MRI signs appearance in the neocortex correlated neither with the presence of SPs nor with the types of histological changes. Conclusions We suppose an inverse relationship between the morphological changes in neurons and their ability to generate epileptic activity. Electrocorticography may be used for the identification of the MRI-negative epileptogenic lesions.

INTRODUCTION: Primary CNS lymphoma (PCNSL) is confined to the CNS and/or the eyes at presentation and is usually initially treated with intravenous methotrexate-based chemotherapy and whole-brain radiotherapy (WBRT). However, the intact blood-brain barrier (BBB) can limit diffusion of methotrexate into brain and tumor and so very high doses of methotrexate are needed for intravenous treatment. With BBB disruption (BBBD), enhanced drug delivery to the tumor can be achieved. Both methods are well known and are using in different clinics in the World. PATIENTS AND METHODS: This report summarizes the single institutional (Burdenko Neurosurgical Institute, Moscow, Russia) experience of 104 newly diagnosed patients with PCNSL treated with osmotic BBBD and intra-arterial (IA) methotrexate from 2000 to 2013. There were 57 female and 47 male at age from 25 to 75 (median 57,6 and 40,1% > or = 60). 51.3% had Karnofsky performance score (KPS) less than 70 at diagnosis. Diagnosis was verified by stereotactic biopsy (STB) in 76,7 % of cases and by tumor removing in 21,1% and by open biopsy in 2,2%. RESULTS: The overall response rate was 86.4% (63.8% complete; 21.6% partial). Median overall survival (OS) was 35 months. Median progression-free survival (PFS) was 24 months, with 5-year PFS of 29,4%. In 20 of 104 patients ophthalmological signs of intraocular lymphoma were registered (19.2%). Procedures were generally well tolerated and there were no cases of technique associated deaths in our series. CONCLUSION: Our results of treating large series of PCNSL patients using BBBD/IA methotrexate-based chemotherapy confirmed successful and durable tumor control and outcomes that were reported early. BBBD/IA technique are comparable to other PCNSL treatment regimens.

The relationship between disorders in the cytoarchitectonics (microdysgenesis) and formation of epileptogenic zones was studied on clinical material. Results of surgical treatment of 29 patients with drug-resistant temporal epilepsy are presented. Intraoperative electrocorticography was carried out in all patients. Histological studies of the cortex from the zone of regular epileptic activity registration were carried out in 23 cases. A relationship between completeness of radical removal of the zone of regular epileptic activity and treatment results was detected. Histological studies of resected cortex revealed signs of microdysgenesis in all cases. The presence of similar changes in the zone of regular epileptic activity registration, resection of which is essential for attack control, suggests a pathogenetic relationship between microdysgenesis and generation of epileptic activity.

The aim of the study was to assess the capabilities of diffusion kurtosis imaging (DKI) in diagnosis of the glioma proliferative activity and to evaluate a relationship between the glioma proliferative activity index and diffusion parameters of the contralateral normal appearing white matter (CNAWM).The study included 47 patients with newly diagnosed brain gliomas (23 low grade, 13 grade III, and 11 grade IV gliomas). We determined a relationship between absolute and normalized parameters of the diffusion tensor (mean (MD), axial (AD), and radial (RD) diffusivities; fractional (FA) and relative (RA) anisotropies) and diffusion kurtosis (mean (MK), axial (AK), and radial (RK) kurtosis; kurtosis anisotropy (KA)) and the proliferative activity index in the most malignant glioma parts (p0.05). We also established a relationship between the tensor and kurtosis parameters of CNAWM and the glioma proliferative activity index (p0.05).The correlation between all the absolute and normalized diffusion parameters and the glioma proliferative activity index, except absolute and normalized FA and RA values, was found to be statistically significant (p0.05). Kurtosis (MK, AK, and RK) and anisotropy (KA, FA, RA) values increased, and diffusivity (MD, AD, RD) values decreased as the glioma proliferative activity index increased. A strong correlation between the proliferative activity index and absolute RK (r=0,71; p=0.000001) and normalized values of MK (r=0.8; p=0.000001), AK (r=0.71; p=0.000001), RK (r=0.81; p=0.000001), and RD (r=-0.71; p=0.000001) was found. A weak, but statistically significant correlation between the glioma proliferative activity index and diffusion values RK (r=-0.36; p=0.014), KA (r=-0.39; p=0.007), RD (r=0.35; p=0.017), FA (r=-0.42; p=0.003), and RA (r=-0.41; p=0.004) of CNAWM was found.DKI has good capabilities to detect immunohistochemical changes in gliomas. DKI demonstrated a high sensitivity in detection of microstructural changes in the contralateral normal appearing white matter in patients with brain gliomas.Цель работы - оценить возможности диффузионно-куртозисной МРТ (ДК-МРТ) в диагностике пролиферативной активности глиом головного мозга. Оценить связь индекса пролиферативной активности глиом с диффузионными параметрами контралатерального нормального белого вещества мозга (КНБВ). Материал и методы. В исследование включены 47 пациентов (23 с глиомами низкой, 13 с глиомами III степени и 11 с глиомами IV степени злокачественности). Выявлена связь абсолютных и нормализованных параметров диффузионного тензора (средняя (СД), аксиальная (АД) и радиальная (РД) диффузия, фракционная (ФА) и относительная (ОА) анизотропия) и диффузионного куртозиса (средний (СК), аксиальный (АК) и радиальный (РК) куртозис, куртозисная анизотропия (КА)) с индексом пролиферативной активности глиом в их наиболее злокачественных участках (p0,05). Также была выявлена связь индекса пролиферативной активности глиом с тензорными и куртозисными параметрами КНБВ (p0,05). Результаты. Корреляция индекса пролиферативной активности со всеми абсолютными и нормализованными диффузионными параметрами глиом оказалась статистически значимой (p0,05), за исключением абсолютных значений ФА и ОА. С увеличением пролиферативной активности глиом отмечалось увеличение значений куртозиса (СК, АК и РК) и анизотропии (КА, ФА и ОА) и уменьшение значений диффузии (СД, АД и РД). Сильная корреляция индекса пролиферативной активности была выявлена с абсолютным РК (r=0,71; p=0,000001) и нормализованными значениями СК (r=0,8; p=0,000001), АК (r=0,71; p=0,000001), РК (r=0,81; p=0,000001) и РД (r=–0,71; p=0,000001). Статистически значимая слабая корреляция с индексом пролиферации глиом была найдена для значений РК (r=–0,36; p=0,014), КА (r=–0,39; p=0,007), РД (r=0,35; p=0,017), ФА (r=–0,42; p=0,003) и ОА (r=–0,41; p=0,004) КНБВ. Заключение. ДК-МРТ показала высокую чувствительность в выявлении как иммуногистохимических изменений в глиомах, так и микроструктурных изменений в неизмененных, по данным стандартной МРТ, структурах мозга, которые наблюдаются при повышении малигнизации глиом.

We report a unique case of primary multiple liponeurocytoma. Liponeurocytoma is a rare benign tumor (Grade II) of the posterior cranial fossa with neural- or probably astrocytic-lineage cells; the tumor stroma contains mature adipocytes. This case is the thirty-eighth case of this pathology reported in the world literature and the first case of multiple form of this disease. We have provided a sufficiently thorough neuroradiological and histological picture that allows one to differentiate between a liponeurocytoma and other histological variants. Recommendations for the tactics for managing patients with this rare disease are proposed based on the analysis of treatment of this pathology reported in the world literature.Представлено уникальное наблюдение пациента с первично-множественной формой липонейроцитомы. Липонейроцитома - редкая доброкачественная опухоль (Grade II) задней черепной ямки с нейрональной, возможно, астроцитарной дифференцировкой клеток, в строме которой находятся зрелые адипоциты. Представленное наблюдение является 38-м в мировой литературе случаем данной патологии и первым случаем первично-множественной формы данного заболевания. Нами дана достаточно полная нейрорентгенологическая и гистологическая картина, позволяющая дифференцировать липонейроцитому от других гистологических вариантов. По результатам анализа мирового опыта лечения пациентов с данной патологией были предложены рекомендации по тактике ведения пациентов с этим редким заболеванием.

the objective of the study was to determine significant differences in electrocorticographic patterns for various types of focal cortical dysplasias.42 patients diagnosed with drug-resistant focal epilepsy were operated on at the Burdenko Neurosurgical Institute in the period from 2006 to 2013. Patients who were histologically diagnosed with focal cortical dysplasia (FCD) and underwent video-electroencephalography and electrocorticography were analyzed.The classification of epileptiform patterns proposed by Palmini in 1995 was used. The sporadic epileptiform activity pattern was predominant in electrocorticographic studies. The continued pattern was more frequent in the case of type II focal cortical dysplasias (FCDs), both combined and isolated; burst and sporadic activity patterns prevailed in combinations in the case of type III FCDs. A uniform distribution of all pattern types of the epileptiform activity was observed in type I FCDs. The data are statistically significant for groups with sporadic and continued patterns.The continued epileptiform activity pattern is predominant in type II focal cortical dysplasia that corresponds to the most pronounced epileptogenesis processes of brain tissue with the presence of pathological forms of neurons. A uniform pattern distribution is observed for type I FCD. Patterns of the epileptiform activity and sporadic spike bursts are predominant in the case of type III FCDs. The sporadic activity is likely to be non-specific and almost uniformly distributed in all types of cortical dysplasias.Цель исследования - определение значимых различий электрокортикографических паттернов при различных типах фокальных корковых дисплазий. Материал и методы. 42 пациента с диагнозом 'фармакорезистентная фокальная эпилепсия' прооперированы с 2006 по 2013 г. в НИИ нейрохирургии им. акад. Н.Н. Бурденко. Рассматривались пациенты, которым гистологически был установлен диагноз 'фокальная кортикальная дисплазия' и были проведены видеоэлектроэнцефалография и электрокортикография. Результаты. Использовалась классификация эпилептиформных паттернов, предложенная Palmini в 1995 г. В электрокортикографических исследованиях преобладал паттерн спорадической эпилептиформной активности. Продолженный паттерн встречался чаще при II типе фокальных кортикальных дисплазий (ФКД) как в комбинациях, так и изолированно; паттерны вспышек и спорадической активности преобладали в комбинациях при III типе ФКД. При I типе ФКД отмечалось равномерное распределение всех типов паттернов эпилептиформной активности. Для групп с наличием спорадического и продолженного паттернов данные статистически достоверны. Заключение. При II типе фокальных кортикальных дисплазий преобладает паттерн продолженной эпилептиформной активности, что соответствует наиболее выраженным процессам эпилептогенеза ткани головного мозга c наличием патологических форм нейронов. При I типе ФКД отмечается равномерное распределение паттернов. При III типе ФКД преобладают паттерны спорадической эпилептиформной активности и вспышек спайков. Спорадическая активность, вероятно, является неспецифической и практически равномерно распределяется при всех типах кортикальных дисплазий.
