The purpose of this study was to evaluate the risk of malignancy of thyroid incidentalomas detected on (18)F-FDG PET and the diagnostic accuracy of sonography for differentiating benign from malignant focal thyroid incidentalomas that were detected on FDG PET.Retrospective review was performed of a database of 87 focal thyroid lesions seen on FDG PET and sonography. Forty-two focal lesions were malignant. We compared the accuracy of the maximum standard uptake value (SUV) to differentiate benign from malignant thyroid lesions. We classified the thyroid nodules as probably benign or suspicious for malignancy by the sonographic features. Statistical analyses compared two subgroups by sonographic classifications between benign and malignant thyroid lesions.The maximum SUV of the malignant nodules was not significantly higher than that of benign lesions. Thirty-seven (75.5%) of 49 lesions with suspicious sonographic findings revealed malignancy on cytopathology, compared with five (13.2%) of 38 lesions that showed probably benign sonographic findings. These differences were statistically significant using a kappa test (kappa = 0.675, p = 0.001) and logistic regression (odds ratio = 26.2, p = 0.001).The probability (48.3%) of malignancy of focal thyroid incidentalomas seen on FDG PET is high. The maximum SUV of thyroid cancer is not significantly higher than that of benign lesions. The probability (13.2%) of malignancy is much lower when the sonographic findings appear benign, as compared with a significantly higher probability (75.5%) of malignancy when the sonographic findings are suspicious for malignancy.

Purpose To evaluate inter- and intra-observer variabilities in breast sonographic feature analysis and management, using the fourth edition of the Breast Imaging Reporting and Data System (BI-RADS). Materials and methods We included 136 patients with 150 breast lesions who underwent breast ultrasound (US) and ultrasound-guided core needle biopsy. A pathological diagnosis was available for all 150 lesions: 77 (51%) malignant and 73 (49%) benign. Four radiologists retrospectively reviewed sonographic images of lesions twice within an 8-week interval. The observers described each lesion, using BI-RADS descriptors and final assessment. Inter- and intra-observer variabilities were assessed with Cohen's kappa statistic. Positive predictive value and negative predictive value (NPV) for final assessment were also calculated. Results For inter-observer agreements for sonographic descriptors, substantial agreement for lesion calcification and final assessment (κ = 0.61 for both), moderate agreement for lesion shape, orientation, boundary, and posterior acoustic features (κ = 0.49, 0.56, 0.59, and 0.49, respectively), and fair agreement for lesion margin and echo pattern (κ = 0.33 and 0.37, respectively) were obtained. For intra-observer agreement, substantial to perfect agreement was found for almost all lesion descriptors and final assessments. NPV for final assessment category 3 was 95%. Positive predictive value (PPV) for final assessment categorized as 4 or 5 were as follows: category 4a, 26%; category 4b, 89%; category 4c, 90%; and category 5, 97%. Conclusion Because inter- and intra-observer agreement with the BI-RADS lexicon for US is good, the use of BI-RADS lexicon can provide accurate and consistent description and assessment for breast US.

Objective To determine the frequency of imaging-histologic discordance at percutaneous breast biopsy and to evaluate differences in clinical and radiologic findings between pathologically upgraded lesions and non-upgraded lesions. Materials and methods From February 2000 to June 2005, we reviewed 386 cases that had suspicious imaging findings but yielded benign histology at US-core needle biopsy and that underwent subsequent excisional biopsy. In 74 of 386 cases, the benign histology at core needle biopsy could not provide a satisfactory explanation for the radiologically suspicious lesions. The clinical, radiologic and histologic findings were reviewed for those 74 cases that were classified as the upgrade group and the non-upgrade group after excisional biopsy. Results The upgrade rate was 17.6% (95% confidence interval, 10.6–27.8%, 13 of 74 cases) revealed upgraded pathology at the subsequent excisional biopsy. Besides the size of masses, there were no statistically significant differences in imaging findings between the upgrade and non-upgrade groups at excisional biopsy. Conclusion This upgrade rate of 17.6% suggests that excisional biopsy or re-biopsy is warranted in those cases presenting imaging-histologic discordance at US-guided core biopsy.

The purpose of this presentation is to illustrate the sonographic findings of chest wall lesions that were depicted on breast sonography.Chest wall lesions detected during breast sonography were collected and reviewed retrospectively.The sonographic findings of normal chest walls and various pathologic chest wall lesions, including inflammatory lesions, benign neoplasms, and malignant neoplasms, are discussed.Familiarity with normal sonographic anatomy and chest wall lesions could be helpful in differentiating a chest wall lesion from a breast lesion and in showing whether the origin of any palpable breast lump is in the breast parenchyma or the chest wall on breast sonography.

OBJECTIVE. This study was performed to compare the clinical and radiologic findings and pathologic staging between index cancer and contralateral synchronous breast cancers and to determine the roles of mammography and sonography in their detection.CONCLUSION. Additional contralateral breast cancers in bilateral synchronous breast cancers are apt to be small and less palpable and to have less suspicious imaging findings and less advanced cancer staging than the index cancer. Bilateral whole-breast sonography with mammography is useful in the early detection of contralateral synchronous breast cancer.

The purpose of this study was to identify differences in the MRI findings of septic arthritis and transient synovitis in patients with nontraumatic acute hip pain and hip effusion.The MRI findings in nine patients with septic arthritis and 11 with transient synovitis were reviewed retrospectively. This study was approved by our institutional review board. The diagnoses were based on findings at physical examination, laboratory studies, and joint aspiration and bacteriologic study. The MRI findings were analyzed with emphasis on the grade of joint effusion, alterations in signal intensity in the soft tissues and bone marrow, and the presence of decreased perfusion at the femoral head.Low signal intensity on fat-suppressed gadolinium-enhanced T1-weighted coronal MRI suggesting decreased perfusion at the femoral head of the affected hip joint was seen in eight of nine patients with septic arthritis and in two of 11 patients with transient synovitis. Statistically reliable differences (p = 0.005) were found between the two groups. Alterations in signal intensity in the bone marrow were seen in three patients with septic arthritis but in none of the patients with transient synovitis. Decreased perfusion on fat-suppressed gadolinium-enhanced coronal T1-weighted MRI was seen in the six patients with septic arthritis who did not have alterations in signal intensity involving the bone marrow.Decreased perfusion at the femoral epiphysis on fat-suppressed gadolinium-enhanced coronal T1-weighted MRI is useful for differentiating septic arthritis from transient synovitis.

OBJECTIVE We wanted to describe the characteristic ultrasonography (US) features and clinical findings for making the diagnosis of subacute granulomatous thyroiditis. MATERIALS AND METHODS A total of 31 lesions from 27 patients were confirmed as subacute granulomatous thyroiditis by US-guided fine needle aspiration biopsy. We analyzed the ultrasonographic findings such as the lesion's size, margin and shape, the discrepancy between length and breadth and the vascularity. The clinical findings such as acute neck pain or fever were reviewed. The follow-up clinical and ultrasonographic data were reviewed for 15 patients. RESULTS The thyroid gland was found to be enlarged in five patients, it was normal size in 20 patients and it was smaller in two patients. All the lesions had focally ill-defined hypoechogenicity. Hypervascularity was not noted in any of the lesions. Painful neck swelling was present in 18 patients. An accompanying fever was documented in nine of the 18 patients. Twelve patients showed disappearance (n = 3) or a decreased size (n = 9) of their lesions on follow-up US. CONCLUSION The presence of ill-defined hypoechoic thyroid lesions without a discrete round or oval shape is characteristic for subacute granulomatous thyroiditis, and particularly when this is associated with painful neck swelling and/or fever.

Objectives Columnar cell lesions are being encountered with increasing frequency in breast biopsies performed. The purpose of our study was to determine whether columnar cell lesions of the breast have any distinctive imaging characteristics. Materials and methods We retrospectively reviewed our institutional database for all records of breast pathology obtained in a 17-month period. Columnar cell lesion was diagnosed in 53 lesions and 12 of these 53 lesions contained columnar cell lesions as the sole histopathologic findings. These 12 lesions in nine patients made up our study population. They included columnar cell change (n = 4), columnar cell hyperplasia (n = 5), and columnar cell hyperplasia with atypia (n = 3). Results All nine patients underwent mammography and sonography within 1 month of each other. Of the mammograms in nine patients, nine lesions (75%) appeared as clustered amorphous or indistinct (n = 5), fine pleomorphic (n = 3), or round (n = 1) microcalcifications. The tenth lesion showed a focal mass without microcalcifications and the remaining two lesions showed no abnormal findings. At sonography, not-circumscribed masses were depicted in six lesions and microcalcifications were visible in four lesions, of which three lesions were concurrent with masses. There were no sonographically focal lesions in the remaining five. Overall 11 lesions were classified as BI-RADS category 4 (92%) and one as category 3. Of the three lesions with atypia, two were classified as category 4a and one was classified as category 4c, and they showed no distinct imaging appearance from those without atypia. Conclusion Columnar cell lesions usually present as nonpalpable, clustered indeterminate or suspicious microcalcifications on mammography. They are indistinguishable from other causes of suspicious microcalcifications such as atypical ductal hyperplasia or ductal carcinoma in situ and require needle biopsy or excisional biopsy for diagnosis.

Objective. The purpose of our study was to evaluate the utility of the American College of Radiology’s Breast Imaging Reporting and Data System (BI-RADS) sonographic final assessment system and palpation-guided fine-needle aspiration (FNA) for evaluation of palpable breast lesions. Methods. Our computerized database identified 160 palpable lesions of the breast in which follow-up palpation-guided FNA, targeted sonography, and pathologic confirmation were performed. We used BI-RADS sonographic data on all lesions. The sensitivity, specificity, accuracy, positive predictive value, and negative predictive value of malignancy were calculated for sonography and palpation-guided FNA. Two-sample binomial proportion tests were used as the statistical analysis (P < .05). Results. The FNA results were defined as benign, atypical cells, suspicious for malignancy, malignancy, and insufficiency. The sensitivity, specificity, accuracy, positive predictive value, and negative predictive value were 90.9%, 82.7%, 84.3%, 57.7%, and 97.2%, respectively, on sonography and 75.8% to 90.9%, 82.7% to 98.4%, 84.3% to 94.4%, 57.7% to 92.6%, and 93.9% to 97.2% on FNA. There was no statistically significant difference for sensitivity and negative predictive value between the two examinations. Conclusions. The diagnostic accuracy of sonography was similar to that of palpation-guided FNA for not missing the malignancy. Clinical application of FNA results can be difficult, especially when the result is insufficiency or atypical cells. Moreover, FNA is invasive and overlaps other procedures. Therefore, we conclude that sonography can replace palpation-guided FNA for diagnosis of palpable lesions of the breast when the BIRADS sonographic final assessment system is used appropriately. Aim To performed a prospective investigation of the relative merits of rapid cytokeratin immunohistochemical (CK-IHC) staining of the SLN removed during the operation of breast cancer patients. Study design Between December 2002 and March 2004, 62 patients with T1 and T2 breast cancer were enrolled after undergoing successful sentinel lymph node biopsy. Eighty-nine sentinel lymph nodes (mean number, 1.44) were biopsied and first examined by hematoxylin–eosin (H&E) stained frozen section. All the tumour free sentinel lymph nodes by H&E stained frozen section were immunostained for cytokeratin using a rapid immunohistochemical assay (Cytokeratin (PAN), 1:50, Novocastra Lab., Newcastle, UK) during the operations. Results Rapid IHC staining revealed seven positive sentinel lymph nodes that were negative for metastasis by H&E staining. This study showed a sensitivity of 92.86%, a specificity of 100%, an accuracy of 98.9%, and a negative predictive value of 98.7%. Conclusions The intraoperative examination of sentinel lymph nodes is an accurate and effective way of predicting the axillary lymph node status of patients with breast cancers.

Breast edema is defined as a mammographic pattern of skin thickening, increased parenchymal density, and interstitial marking. It can be caused by benign or malignant diseases, as a result of a tumor in the dermal lymphatics of the breast, lymphatic congestion caused by breast, lymphatic drainage obstruction, or by congestive heart failure. Here we describe several conditions, that cause unilateral breast edema with the aim of familiarizing radiologists with these disease entities.

To evaluate the mammographic and sonographic findings of pregnancy-associated breast cancer.A total of 22 consecutive patients with breast cancer pathologically diagnosed during pregnancy (n = 10) or lactation (n = 12) were included in this study. The ages of the patients ranged from 26 to 49 years. Both mammography and sonography were performed on 12 patients; sonography only was performed on 7 patients; and mammography only was performed on 3 patients. Mammographic and sonographic findings were evaluated retrospectively.Mammography revealed positive findings in 13 (86.7%) of 15 patients, even though all 15 patients had dense breasts. Mammographic findings included masses (n = 5), masses with calcifications (n = 2), calcifications with axillary lymphadenopathy (n = 2), a mass with axillary lymphadenopathy (n = 1), calcifications alone (n = 1), asymmetric density alone (n = 1), and diffuse skin and trabecular thickening alone (n = 1). Sonographic findings were positive and showed masses for all 19 patients (100%). The common sonographic findings of masses were irregular shapes (n = 15), irregular margins (n = 16), parallel orientation (n = 11), complex echo patterns (n = 14, including marked cystic [anechoic] components [n = 4]), and posterior acoustic enhancement (n = 12). Surrounding tissue effects could be seen in 5 patients, including ductal changes (n = 2), Cooper ligament thickening (n = 1), edema (n = 3), and skin thickening (n = 3). Calcifications within or outside a mass (n = 7) and axillary lymphadenopathy (n = 8) were also detected.Although a mass could not be discernible by mammography because of increased radiodensity during pregnancy or lactation, calcification, asymmetric density, axillary lymphadenopathy, and skin and trabecular thickening were helpful for diagnosis of pregnancy-associated breast cancer. Sonographic findings of a solid mass with posterior acoustic enhancement and a marked cystic component were somewhat different from the appearance of breast cancer in nonpregnant women, possibly because of the physiologic changes of pregnancy and lactation.

OBJECTIVE. This study was performed to identify the typical imaging features of steatocystoma multiplex on mammography and sonography.CONCLUSION. Steatocystoma multiplex is manifest by multiple clinically palpable skin-colored or yellow nodules scattered on the axillae and the anterior chest wall. Steatocystoma multiplex is included in the differential diagnoses of lipoma, fat necrosis, galactocele, epidermal cyst, and so on, so radiologists need to be aware of its distinguishing features. The findings of a well-circumscribed round fat-density nodule on mammography or an intradermal hypoechoic nodule on sonography, combined with a family history of steatocystoma multiplex (40%) and an extensive bilateral spatial distribution of nodules, confirm the diagnosis of steatocystoma multiplex.

Objective: To compare sonography and mammography in terms of their diagnostic value in breast cancer cases which initially presented as an axillary mass without a palpable mass or other clinical symptoms. Materials and Methods: Seven patients with enlarged axillary lymph nodes who first presented with no evidence of palpable breast lesions and who underwent both mammography and sonography were enrolled in this study. In six of the seven, the presence of metastatic adenocarcinoma was confirmed preoperatively by axillary needle aspiration biopsy; in four, subsequent sonographicallyguided breast core biopsy performed after careful examination of the primary site indicated that primary breast cancer was present. In each case, the radiologic findings were evaluated by both breast sonography and mammography. Results: Breast lesions were detected mammographically in four of seven cases (57%); in three of the four, the lesion presented as a mass, and in one as microcalcification. In three of these four detected cases, fatty or scattered fibroglandular breast parenchyma was present; in one, the parenchyma was dense. In the three cases in which lesions were not detected, mammography revealed the presence of heterogeneously dense parenchyma. Breast sonography showed that lesions were present in six of seven cases (86%); in the remaining patient, malignant microcalcification was detected at mammography. Final pathologic examination indicated that all breast lesions except one, which was a ductal carcinoma in situ, with microinvasion, were infiltrating ductal carcinomas whose size ranged from microscopic to greater than 3 cm. At the time of this study, all seven patients were alive and well, having been disease free for up to 61 months after surgery. Conclusion: In women with a palpable axillary mass confirmed as metastatic adenocarcinoma, breast sonography may be a valuable adjunct to mammography.

We report a case of tuberculous thyroiditis in a woman with Behcet's syndrome. The initial physical examination in May 1998 revealed multiple soft, nontender, mobile lymph nodes, each measuring less than 1 cm, in the left lower internal jugular chain. Sonography performed in February 2000 showed multiple small (< 1 cm), oval lymph nodes, each with an intact fatty hilum, in the left lower internal jugular chain; the thyroid gland appeared normal. Follow-up sonography 6 months later showed multifocal, heterogeneous, hypoechoic lesions with ill-defined margins in both lobes of the thyroid and several small, oval lymph nodes, each with an intact fatty hilum, in the left lower internal jugular chain. Fine-needle aspiration was performed on the largest thyroid lesion, and cytologic analysis of the aspirate revealed a small number of epithelioid histiocytes in a necrotic background, which was suggestive of tuberculosis. Follow-up sonography after 3 months of antituberculosis chemotherapy showed that the thyroid lesions had resolved.

We report a case of breast cystic lymphangioma diagnosed in a 34-year-old woman. It showed a large, circumscribed, lobulated, high-density mass on mammography and a well-defined anechoic cyst without septa and solid nodules located in the right tail of Spence on sonography. This lesion had a location and imaging findings typical of breast cystic lymphangioma.

This study was designed to assess the diagnostic indexes of sonographic surveillance with mammography for the detection of metachronous contralateral breast cancer.Between January 2003 and December 2003, 1,706 breast sonographic examinations were performed by three radiologists in 1,256 Asian women with a history of surgery for breast cancer in one breast as an adjunct screening test to mammography in an academic medical center. We evaluated the biopsy recommendation rate, a diagnostic index, of the combination of whole-breast sonography and mammography for the detection of contralateral metachronous breast cancers and the positive predictive value (PPV) of this biopsy recommendation rate.Based on 1,706 examinations in 1,256 women, the biopsy recommendation rate was 3.5% per patient and 2.6% per examination. The PPV of the biopsy recommendation rate was 41.0% with 18 breast cancers diagnosed (cancer detection rate, 1.4% per patient and 1.1% per examination). Among these cancers, two were detected on sonography alone. One false-negative cancer was found on the next sonographic examination but could not be seen on the next mammographic examination.With a false-negative rate of only 0.06% and a PPV of 41.0% for the biopsy recommendation rate, our results suggest that annual sonography could be a useful adjunctive tool to mammography for the detection of metachronous contralateral cancers.

Objective To evaluate the diagnostic accuracy of the use of an ultrasonography (US)-guided vacuum-assisted biopsy for microcalcifications of breast lesions and to evaluate the efficacy of the use of US-guided vacuum-assisted biopsy with long-term follow-up results. Materials and Methods US-guided vacuum-assisted biopsy cases of breast lesions that were performed between 2002 and 2006 for microcalcifications were retrospectively reviewed. A total of 62 breast lesions were identified where further pathological confirmation was obtained or where at least two years of mammography follow-up was obtained. These lesions were divided into the benign and malignant lesions (benign and malignant group) and were divided into underestimated group and not-underestimated lesions (underestimated and not-underestimated group) according to the diagnosis after a vacuum-assisted biopsy. The total number of specimens that contained microcalcifications was analyzed and the total number of microcalcification flecks as depicted on specimen mammography was analyzed to determine if there was any statistical difference between the groups. Results There were no false negative cases after more than two years of follow-up. Twenty-nine lesions were diagnosed as malignant (two invasive carcinomas and 27 carcinoma in situ lesions). Two of the 27 carcinoma in situ lesions were upgraded to invasive cancers after surgery. Among three patients diagnosed with atypical ductal hyperplasia, the diagnosis was upgraded to a ductal carcinoma in situ after surgery in one patient. There was no statistically significant difference in the number of specimens with microcalcifications and the total number of microcalcification flecks between the benign group and malignant group of patients and between the underestimated group and not-underestimated group of patients. Conclusion US-guided vacuum-assisted biopsy can be an effective alternative to stereotactic-guided vacuum-assisted biopsy in cases where microcalcifications are visible with the use of high-resolution US.

Purpose To evaluate the safety and efficiency of the Ultrasound (US)-guided large needle core biopsy of axilla lymph nodes. Materials and Methods From March 2004 to September 2005, 31 patients underwent the US-guided core biopsy for axilla lymph nodes. Twenty five lesions out of 31 were detected during breast US, and 6 of 31 cases were palpable. Lymph nodes were classified based on their shape and cortical morphology. The core biopsy of axilla lymph nodes was performed on suspicious lymph nodes found during breast ultrasonography to find out whether the patients had a history of breast cancer or not. Among the 31 patients, 16 patients were associated with breast cancer. The lesion sizes varied from 0.6 cm to 3.3 cm (mean = 1.59 ± 0.76 cm). US-guided core biopsies were performed with 14 G needles with an automated biopsy gun. Total 3 or 5 specimens were obtained. Results Among the 31 cases of axilla lymph nodes core biopsies, 11 cases showed malignant pathology. Seven out of 11 cases were metastatic lymph nodes from breast cancer; 2 cases were from primary unknown and 2 cases from lymphomas. On the other hand, 20 histopathologic results of axilla lesions were benign: subacute necrotizing lymphadenitis (n = 2), dermatopathic lymphadenitis (n = 1), reactive hyperplasia (n = 10) and free of carcinoma (n = 7). Conclusion The US-guided large needle core biopsy of axilla lesions is safe and effective for the pathological evaluation. The core biopsy is believed to be easy to perform if suspicious lymph nodes or mass lesions are found in the axilla.

The objective of our study was to report the results of classification of sonographic findings according to BI-RADS and to calculate the positive predictive value (PPV) for each BI-RADS assessment category.We prospectively classified 4,668 breast sonograms according to BI-RADS final assessment category. Suspicious sonographic findings were divided into major and minor suspicious findings. Category 1 was normal and category 2 was a benign finding such as cyst or nodule with uniform and intense hyperechogenicity. A nodule neither category 2 nor category 4 or 5 was defined as category 3. A nodule with one or more suspicious findings, not category 5, was defined as category 4. A nodule with two or more major suspicious findings was defined as category 5.Of the 4,668 cases, 321 cases failed to undergo follow-up of at least 1 year. The PPV was 0.1% in category 1 (3/2,191), 0% in category 2 (0/773), 0.8% in category 3 (6/737), 31.1% in category 4 (161/519), and 96.9% in category 5 (123/127). In palpable lesions (n = 751), the PPV was 2.2% in category 1 (2/93), 0.9% in category 3 (2/217), 54% in category 4 (107/198), and 98% in category 5 (98/100). In nonpalpable lesions (n = 3,596), the PPV was 0.05% in category 1 (1/2,098), 0.8% in category 3 (4/520), 16.8% in category 4 (54/321), and 92.6% in category 5 (25/27).As with mammography, placing sonographic lesions into BI-RADS categories is useful for predicting the presence of malignancy.

Chest wall disorders, especially in a submammary location, occasionally mimic breast tumors. We report a case of a venous angioma between the pectoralis major and minor muscles. It presented as a palpable mass in the left breast. The mass mimicked a breast tumor both clinically and on mammography.

This study was undertaken to retrospectively assess the contribution of sonographic surveillance in the early detection of metachronous contralateral breast cancer.We retrospectively reviewed the pathologic, mammographic, and sonographic records of 51 patients with surgically proven metachronous bilateral breast cancer in 2,498 surgically proven breast cancers during 2000-2006. We first evaluated cancer staging according to the method of detection used to identify metachronous breast cancers. The sensitivity of imaging studies to identify the lesions was also assessed. We compared cancer staging on the basis of whether the patient was included in a screened group, which was one in which a mammogram and sonogram were obtained within 12 months of the pathologic diagnosis of metachronous cancer. Within the screened group, we compared cancer staging on the basis of whether a screening sonogram was obtained within 6 months of the diagnosis of metachronous cancer.The staging of metachronous cancers showed no statistically significant differences related to detection method. The sensitivity of sonography was 94% and of mammography was 80% in the detection of metachronous cancers. The cancer stage in the screened group was 0 or stage I in 81% and that in the unscreened group was stage II or III in 71% (p0.05). Among the screened group, no significant difference was seen in staging regardless of whether a screening sonogram was obtained in the 6 months after diagnosis of metachronous cancer (p = 0.576).Sonography alone detected 14% of metachronous contralateral breast cancers. The results of this study suggest that annual additional sonography with mammography contributes to the early detection of metachronous cancers. However, sonography every 6 months is unlikely to be helpful for the early detection of metachronous cancer.

The objective of our study was to determine the diagnostic accuracy of sonographically guided core needle biopsy for breast masses by evaluating the outcomes of benign biopsies that had at least a 2-year follow-up.In this retrospective study, we included a total of 2,420 lesions from 2,198 women who had undergone sonographically guided 14-gauge core needle biopsy. For evaluating the diagnostic accuracy of this procedure, the pathologic results were reviewed and correlated with rebiopsy or long-term imaging follow-up. Agreement rate, high-risk underestimate rate, ductal carcinoma in situ (DCIS) underestimate rate, and false-negative rate were assessed. The false-negative diagnoses of core needle biopsy were reviewed in detail.The pathologic results for the core needle biopsies were malignant in 52%, high-risk in 4%, and benign in 44%. The agreement rate was 96% (2,328 of 2,420). The underestimate rate was 29% (36 of 126) for DCIS and 27% (25 of 93) for high-risk (52% for 27 atypical ductal hyperplasia (ADH), 17% for 66 non-ADH). Of 1,071 benign lesions, malignancy was found at rebiopsy in 31 lesions (25 immediate and six delayed false-negative diagnoses), and the false-negative rate was 2.4% (31 of 1,312). The frequency of malignancy in lesions that had rebiopsy because of suspicious imaging findings (19.1%, 26 of 136) was significantly higher than that because of suspicious physical findings or request by patient or physician (0.9%, five of 584).Sonographically guided 14-gauge core needle biopsy is an accurate method for evaluating breast masses. Imaging-pathologic correlation and follow-up of benign biopsy are essential for a successful breast biopsy program.

This study was conducted to assess the accuracy of US-guided directional vacuum-assisted removal (US-DVAR) in evaluating nonmalignant papillary breast lesions. This retrospective study was approved by the institutional review board at our institution; patient consent was not required. We reviewed the clinical and pathology findings from a total of 39 papillary lesions diagnosed at vacuum-assisted removal in 37 patients (age range, 26–60 years; mean age, 44.5 years). Over the follow-up period, we evaluated whether any histologic upgrade occurred and whether or not residual lesions were detected on follow-up imaging. US-DVAR of 39 lesions yielded tissue that was classified as benign in 35 and atypical in 4. Of the 35 lesions that were diagnosed as histologically benign at US-DVAR, 2 were surgically excised. Both of them yielded benign results. Of the 33 benign lesions that were not surgically excised, 28 (85%) were not seen at radiographic follow-up. Of the four lesions diagnosed as atypical at US-DVAR that were surgically excised, all the four were benign. None proved to be malignant. The upgrade rate was 0.0% (95% confidence interval, 0–9%). Among our patients, diagnosis by US-DVAR of benign papillary lesions proved to be accurate, and benign papillary lesions at US-DVAR did not need to be surgically excised for accurate diagnosis.

OBJECTIVE. The purpose of this study is to assess the utility of sonographic detection and the role of sonography-guided fine-needle aspiration biopsy (FNAB) in thyroid carcinomas that appear only as microcalcifications without an associated mass.CONCLUSION. This study covers 11 cases in which the only abnormal finding suggesting thyroid cancer was microcalcifications detected at sonography. All cases described were diagnosed as thyroid papillary carcinoma after surgery. Hence, it is shown that real-time high-resolution sonography is effective in the detection of thyroid carcinomas that appear as microcalcifications without an associated mass. The results of sonography-guided FNAB were satisfactory to diagnose these microcalcifications as thyroid carcinomas. Therefore, we believe that thyroid sonography combined with sonography-guided FNAB is a useful approach for diagnosing thyroid microcalcifications.

Breast cancer developing from a surgical scar is rare; this type of malignancy has been reported in only 12 cases to date. Herein, we report on a 52-year-old female who developed infiltrating ductal carcinoma in a surgical scar following excision of a benign mass. Two years previously, the patient underwent surgery and radiotherapy for invasive ductal carcinoma of the contralateral breast. The initial appearance of the scar was similar to fat necrosis; it was observed to be progressively shrinking on follow-up sonography. On the two year follow-up ultrasound, the appearance changed, an angular margin and vascularity at the periphery of the scar were noted. A biopsy and subsequent excision of the scar were performed; the diagnosis of infiltrating ductal carcinoma of the scar was confirmed.

Axanthogranuloma (XG) is a rare benign histiocytic disorder that occurs mostly in infants and young children.' Approximately 15% of XGs occur in adults and usually appear as solitary lesions in the head and the neck region. In most cases, an XG appears as a solitary yellow-red cutaneous nodule in the head and the neck region that regresses spontaneously over a course of months to years. 1,2 An extracutaneous XG is uncommon, and it can occur with or without a cutaneous XG. An extracutaneous XG most commonly occurs in the eye, but findings at other sites such as the central nervous system, lungs, liver, spleen, kidneys, and bone have also been reported. 2-5 The pathogenesis of XGs is unknown. Although physical and infectious factors have been considered, most investigators postulate that they are caused by a reactive response to an unknown stimulus. 3,4 Xanthogranulomas can be divided into juvenile and adult forms. Distinctive from the juvenile form, the adult XG rarely undergoes a spontaneous regression, and concomitant extracutaneous examples are very uncommon. 2-5 Patients with adult XGs are mostly in their late teens to late 30s, although approximately 5% of the patients are in their 60s to 70s. 2-5 We report an exceptional case of an adult breast XG in a 60-year-old woman.

Ultrasonographically (US) guided core needle biopsy is currently recognized as a reliable alternative to surgical biopsy for the histopathologic diagnosis of breast lesions. However, despite advances in biopsy devices and techniques, false-negative diagnoses are unavoidable and may delay the diagnosis and treatment of breast cancer. The most common reasons for false-negative diagnosis are (a) technical or sampling errors, (b) failure to recognize or act on radiologic-histologic discordance, and (c) lack of imaging follow-up after a benign biopsy result. Technical difficulties (eg, poor lesion or needle visualization, deeply located lesions, dense fibrotic tissue) cause inaccurate sampling but can be reduced by using modified standard techniques. Radiologic-histologic correlation is also of critical importance in US-guided core needle biopsy. Radiologic-histologic discordance occurs when the histologic results do not provide a sufficient explanation for the imaging features and indicates that the lesion may not have been sampled adequately, so that repeat biopsy is warranted. Appropriate follow-up imaging is invaluable; even patients with concordant benign findings after US-guided core needle biopsy are directed to undergo follow-up imaging because there may be delays in the recognition of false-negative findings. Optimization of technique, radiologic-histologic correlation, and postbiopsy follow-up protocols are recommended to reduce the occurrence of false-negative diagnosis at US-guided core needle biopsy performed by radiologists.

Aim To evaluate the ultrasound (US) features of the diffuse sclerosing variant of papillary carcinoma (DSVPC) of the thyroid, with histopathological correlations. Materials and methods The US and histopathological findings of six patients with surgically proven DSVPC of the thyroid were retrospectively assessed. Results Most of cases showed diffuse, scattered microcalcifications with or without associated masses and underlying heterogeneous hypoechogenicity at the site of the DSVPC in the thyroid. At ultrasound, scattered microcalcifications and heterogeneous hypoechogenicity seen in the DSVPC correlated to psammoma bodies and lymphocytic infiltration at histopathological review. Conclusion DSVPC of the thyroid usually manifested as diffuse scattered microcalcifications and associated suspicious mass on ultrasound. These findings are not specific for the DSVPC of the thyroid. However, in relatively young patients with suspicious masses associated with underlying diffuse scattered microcalcifications on US, the possibility of DSVPC should be included in differential diagnosis.

The purpose of this study was to determine the frequency of carcinoma at percutaneous directional vacuum-assisted removal (DVAR) in women with imaging-histologic discordance during ultrasound (US)-guided automated core needle biopsy, and to determine the role of DVAR in breast lesions with imaging-histologic discordance. A US-guided 14-gauge automated core needle biopsy was performed on 837 consecutive lesions. Imaging-histologic discordance was prospectively considered in 33 of 634 benign biopsies. DVAR was recommended in those lesions. Among the 33 lesions, 26 lesions that underwent subsequent DVAR or surgical excision made up our study population. Medical records, imaging studies, and histologic findings were reviewed. Among the 26 lesions, 18 lesions underwent subsequent US–guided DVAR, with 8-gauge probes for 15 of the lesions, and 11-gauge for three of the lesions. Two lesions were diagnosed as having carcinoma (2/18, 11.1% of upgrade rate; 3.1–32.8% CI). The remaining eight lesions underwent subsequent surgical excision, and carcinoma was diagnosed in one case (12.5% of upgrade rate; 2.2–47.1% CI). A US-guided DVAR of the breast mass with imaging-histologic discordance during US-guided 14-gauge automated core needle biopsy is a valuable alternative to surgery as a means of obtaining a definitive histological diagnosis.

Objective. The purpose of this study was to evaluate the sonographic appearance of a galactocele that can sonographically mimic a suspicious solid mass and to differentiate between a galactocele and a solid mass. Methods.From September 2002 to February 2004, 33 galactoceles classified as Breast Imaging Reporting and Data System category 4 were included. They were all confirmed by sonographically guided core biopsies. Their sonographic imaging and clinical findings were reviewed retrospectively. Results. The lesions had a round or irregular shape in 26 patients (78.8%), a noncircumscribed margin in 31 (93.9%), a nonparallel orientation in 22 (66%), and posterior shadowing in 13 (39.4%). Twenty-five nodules (75.8%) had internal hypoechogenicity or mixed echogenicity. Twenty-nine (87.9%) of 33 lesions showed a relatively sharp convex echogenic rim on the anterior or posterior wall. Conclusions. Galactoceles have various sonographic findings, many of which are similar to those of suspicious solid breast masses. However, there is a tendency for a galactocele to appear as a small, round hypoechoic nodule with an indistinct or microlobulated margin and mild posterior shadowing. It is helpful to search for a partial anterior or posterior echogenic rim to identify a galactocele.

Objective The purpose of this study was to evaluate the frequency of focal fibrosis of the breast diagnosed by a sonographically guided core biopsy of nonpalpable lesions, to characterize imaging features, and to evaluate their clinical relevance. Methods In a retrospective review of 724 lesions that underwent sonographically guided core biopsy of nonpalpable breast lesions, 62 cases had a diagnosis of focal fibrosis. Two radiologists analyzed the sonographic and mammographic findings according to the Breast Imaging Reporting and Data System. The results were compared with histologic findings at surgery or imaging findings during surveillance. Results The incidence of focal fibrosis was 8.6% (62/724). Sonographic films were available in 56 cases, so 56 cases were reviewed for their sonographic findings. Among the mammograms reviewed, 64.7% (33/51) had negative findings. Among the sonograms reviewed, the most common features were oval shape (32/56, 57.1%), parallel orientation (36/56, 64.3%), microlobulated margin (24/56, 42.9%), abrupt interface (50/56, 89.3%), isoechoic pattern (42/56, 75.0%), and a lack of posterior acoustic features (45/56, 80.4%). The Breast Imaging Reporting and Data System final assessment was category 3 in 27 (48.2%) and category 4 in 29 (51.8%). Most of the category 4 lesions were category 4A (26/29, 89.7%). Surgical excision (n = 7) and follow-up for at least 1 year (n = 49) showed no malignancy. Conclusions Focal fibrosis was found in 8.6% by a sonographically guided core biopsy of nonpalpable breast lesions. Most of the lesions were categorized as probably benign (category 3) or having a low suggestion of malignancy (category 4A). Focal fibrosis diagnosed at core biopsy can be managed with a 6-month follow-up protocol.

Epidermal inclusion cysts rarely occur in the breast, but when they do, the consequences can be severe. Here, we report the case of a 23-year-old woman who presented with a palpable mass in the right breast. The mass had persisted for 4 years and had been causing pain for 1 month at the time of the patient's presentation. We examined the mass by means of mammography and sonography. Sonographic examination revealed a well-circumscribed solid, avascular mass suggestive of an epidermal inclusion cyst. Excision was performed at the patient's request. Histopathologic examination of the mass after excision confirmed the diagnosis of epidermal inclusion cyst and its localization within the breast parenchyma. To our knowledge, reports on epidermal inclusion cysts within the breast parenchyma are rare.

We performed mammography and sonography on a 49-year-old woman who had a mass in her left axilla that had been present for 1 month and who had undergone excision of a primitive neuroectodermal tumor (PNET) on her back 1 year before. Mammography revealed 2 adjacent, dense nodules in the left breast and enlarged lymph nodes, without a fatty hilum or internal microcalcifications, in the left axilla. Sonography showed 2 round to oval, markedly hypoechoic nodules in the left breast and enlarged, markedly hypoechoic lymph nodes, without microcalcifications, in the left axilla. We then performed sonographically guided core biopsy. Histopathologic analysis of the specimens confirmed the presence of PNET in the left breast and axillary lymph nodes. The patient was then treated with chemotherapy. To our knowledge, this report is the first to describe radiologic findings of metastasis of PNET to the breast.

Vascular tumors of the breast are uncommon, and most are angiosarcomas. 1-3 Benign hemangiomas of the breast are rare, and there are few reports. We report a case of a breast subcutaneous hemangioma in which skin redness had recently developed and high vascularity was shown on power Doppler sonography.

The objective was to evaluate the sonographic findings of sclerosing lobular hyperplasia (SLH) of the breast and to correlate the sonographic findings and the pathologic features. This study consisted of 9 patients with pathologically proven sclerosing lobular hyperplasia who had undergone a preoperative imaging study. We retrospectively reviewed 9 ultrasonograms and 6 mammograms. In each patient mammographic findings including, shape and margin, and ultrasonographic findings including the size, length-to-anteroposterior ratio, shape, margin, internal echo pattern, and the presence and location of internal echogenic septum, were evaluated. Histopathologic correlations focused on characteristic imaging findings. Of the 6 mammograms, 4 cases showed a lobular (3 of 6) or an oval shaped mass (1 of 6) with a partly obscured margin (4 of 6). The remaining 2 cases showed heterogeneously dense breast without visible mass. Ultrasonograms showed a lobular (5 of 9) or an oval (4 of 9) shaped mass with a circumscribed margin (9 of 9). The mean length/anteroposterior ratio was 1.98. Intratumoral echogenic septum was present in 8 cases. Six cases had a peripherally arising septum. Histopathologic review revealed that this septum was correlated to interlobular sclerosis. A peripherally arising intratumoral echogenic septum on ultrasonography seen in SLH might be explained by the interlobular sclerosis.

We evaluated mammographic changes to assess the effect of tamoxifen in breast cancer patients who had undergone surgery.We reviewed the mammograms of 102 breast cancer patients treated with tamoxifen (20 mg per day); these patients were subgrouped into tamoxifen-only patients and patients treated with tamoxifen in combination with another therapy (chemotherapy, radiotherapy, or both). The control group was made up of 50 breast cancer patients who did not undergo tamoxifen therapy and 20 healthy women who had an age distribution and menstruation status similar to those of the breast cancer patients. Two radiologists evaluated parenchymal patterns on mammograms of the contralateral breast in the breast cancer patients and of the left breast in the healthy women. Parenchymal changes before and after surgery were also analyzed.Follow-up mammography showed decreased breast parenchyma in 59.8% of the tamoxifen group, 87.0% of the premenopausal tamoxifen-treated women, 36.0% of the nontamoxifen patients, and 10.0% of the healthy women.On follow-up mammography of breast cancer patients, breast parenchyma was shown to be decreased after tamoxifen therapy. Mammography can be a useful method for evaluating breast parenchymal changes after tamoxifen treatment.

The purpose of our study was to analyze the incidence of incidental thyroid cancers which were detected by simultaneous sonographic examination of breast and thyroid glands. Between January 2001 and March 2004, 518 patients were diagnosed with breast cancer after modified radical mastectomy (n = 369) or breast conserving surgery (n = 149). We screened thyroid glands when we examined breast for diagnosis and follow-up after surgery. If we found the sonographic finding of suspicious for malignancy in thyroid, we immediately performed ultrasound-guided fine needle aspiration biopsy (FNAB). Forty-two cases showed suspicious sonographic findings and of those, 18 cases (42.9%) were determined to have suspicious malignant cytology by ultrasound guided FNAB. Among 518 breast cancers, total 13 cases (2.5%) were diagnosed with papillary carcinoma after thyroidectomy. The mean longest diameter of the thyroid masses was 9.9mm (range 1 - 30mm). Six cases (6/13, 46.2%) were diagnosed as simultaneous breast and thyroid cancers, and the rest of the thyroid cancers were detected after 6 to 33 months (mean 16.5 months) after surgery. In conclusion, the patients with breast cancer had a high incidence (2.5%) of thyroid cancer. Sonographic screening is useful for the early detection of thyroid cancer.

Foreign bodies, such as surgical sponges or drains, are sometimes retained after surgical procedures.1 Retention of a drain in the breast tissue postoperatively is an unusual complication. This report describes two cases of characteristic sonographic findings related to retained silastic drains following breast surgery.

In this paper, we evaluate the radiological features of pregnancy-associated breast lesions and discuss the difficulties in diagnosis by imaging. We selected patients who were diagnosed with pregnancy-associated breast lesions during the previous 5 years. All patients complained of palpable lesions in the breast and underwent ultrasonographic (US) examination, the first choice for examination of pregnancy-related breast lesions. Any suspicious lesions found by the US were recommended for a US-guided core biopsy, US-guided fine needle aspiration (FNA), or surgery. Various breast lesions were detected during pregnancy and lactation, including breast cancer, mastitis and abscesses, lactating adenoma, galactoceles, lobular hyperplasia, and fibroadenomas. The imaging features of pregnancy-associated breast lesions did not differ from the features of non-pregnancy-associated breast lesions; however, some pregnancy-associated benign lesions had suspicious sonographic features. A US-guided core biopsy was necessary for differentiating benign from malignant. In patients with breast cancer, the cancer was often advanced at the time of diagnosis. In conclusion, various pregnancy-related breast lesions were detected and the imaging of these lesions had variable findings. Breast ultrasound could be an excellent imaging modality for diagnosis and differentiation between benign and malignant lesions. However, when the imaging results are suspicious, a biopsy should be performed to obtain a pathologic diagnosis.

Purpose: The purpose of this study was to establish a quality standard for mammographic equipment in Korea and to eventually improve mammographic quality in clinics and hospitals throughout Korea by educating technicians and clinic personnel. Materials and Methods: For the phantom test and on site assessment, we visited 37 sites and examined 43 sets of mammographic equipment. Items that were examined include phantom test, radiation dose measurement, developer assessment, etc. The phantom images were assessed visually and by optical density measurements. For the clinical image assessment, clinical images from 371 sites were examined following the new Korean standard for clinical image evaluation. The items examined include labeling, positioning, contrast, exposure, artifacts, collimation among others. Results: Quality standard of mammographic equipment was satisfied in all equipment during on site visits. Average mean glandular dose was 114.9 mRad. All phantom image test scores were over 10 points (average, 10.8 points). However, optical density measurements were below 1.2 in 9 sets of equipment (20.9%). Clinical image evaluation revealed appropriate image quality in 83.5%, while images from non-radiologist clinics were adequate in 74.6% (91/122), which was the lowest score of any group. Images were satisfactory in 59.0% (219/371) based on evaluation by specialists following the new Korean standard for clinical image evaluation. Satisfactory images had a mean score of 81.7 (1 S.D.=8.9) and unsatisfactory images had a mean score of 61.9 (1 S.D=11). The correlation coefficient between the two observers was 0.93 (p

Purpose: Sentinel lymph node (SLN) biopsy has become a new standard procedure in the treatment of patients with early breast cancer. Furthermore, many institutions have begun offering the sentinel lymph node biopsy without simultaneous axillary dissection as a possible standard procedure when the SLN was free from tumors. For appropriate intraoperative decision making on the presence of cancer cells in axillary lymph nodes, a fast and accurate method to assess the SLN is required. The authors performed a prospective investigation of the relative merits of rapid cytokeratin immunohistochemical (IHC) staining of the SLN removed during the operations of breast cancer patients. Methods: Between December 2002 and August 2003, 38 patients with T1and T2 breast cancer were enrolled after undergoing successful sentinel lymph node biopsy. A total of 60 sentinel lymph nodes (mean number, 1.58) were biopsied and first examined by hematoxylin-eosin (HE7:154-160)

The purpose of this study was to evaluate the usefulness of an ultrasound-guided fine-needle aspiration biopsy (US-FNAB) for the evaluation and treatment planning of nonpalpable thyroid lesions, including infracentimetric nodules. One hundred and twenty one patients underwent US-FNAB for 149 non-palpable solid nodules. Sixty-five patients underwent surgery, and 84 were followed up for at least 36 months. The results of the US-FNAB correlated with the pathological findings and clinical follow-up results. The nodules ranged from 0.3 to 2 cm in diameter, with a mean of 0.8 cm. Among the 149 nodules, 115 were infracentimetric and 34 were centimetric or supracentimetric in size. Of the 149 thyroid nodules, US-FNAB was true positive in 43, true negative in 90, false positive in 7 and false negative in 1. In 8 cases, the lesion was inadequately sampled. The sensitivity, specificity, positive predictive value, negative predictive value and accuracy of the US-FNAB for the infracentimetric nodules were 96.9, 93.4, 86.1, 98.6 and 94.4%, and for the centimetric or supracentimetric nodules, were 100, 90.5, 85.7, 100 and 93.9%, respectively. There were no significant differences in the results between the infracentimetric and centimetric or supracentimetric nodules. In conclusion, an US-FNAB is a useful tool for determining the treatment plan of non-palpable solid thyroid nodules, even when less than 1 cm in diameter, and shows high sensitivity, specificity and accuracy.

The characteristic features of hamartoma in terms of discrepancies in mammographic and sonographic shapes of the mass were evaluated. We reviewed 16 pathologically proven breast hamartomas, which had undergone preoperative mammography and ultrasonography. All masses were analyzed according to ACR-BIRADS on mammography. On sonography, each mass was analyzed for size, shape, margin, internal echogenicity, and posterior acoustic enhancement. We also analyzed the echogenicity of halo, and compared the characteristic changes in the shape of hamartomas attributable to compression in mammograms and sonograms. The most common sites were at 12 o'clock in the right breast and 2 o'clock in the left. The most common mammographic findings of the hamartomas were a round shape (11/16), a circumscribed margin (13/16), internal fat densities (D4)(16/16) and radiolucent halos (14/16). The most common sonographic findings of the hamartomas were an oval shape (16/16), circumscribed margins (10/16), heterogeneous internal echogenicity (14/16), echogenic (7/16) or echolucent halos (5/16), and posterior enhancements (12/16). The characteristic feature of hamartomas was a change of the mammographic round shape mass into an elongated oval shape mass by sonography (11/11), suggesting the compressibility of hamartomas. Three of the hamartomas contained a pathologically proven internal calcification. The presence of a hamartoma was suggested by a change in a mammographic round mass with a radiolucent halo into an oval heterogeneous mass surrounded by an echogenic or echolucent halo on the sonogram. This characteristic difference between the mammographic and sonographic findings was attributed to the hamartoma compressibility, and was associated with the over-proliferation of fat containing mature normal breast tissue.

The purpose of this study was to evaluate the thickness of the compressed breast in mediolateral oblique (MLO) and craniocaudal (CC) mammograms, to relate these thickness and breast patterns to mean glandular dose (MGD) in Korean women, and to evaluate the suitability of using the American College of Radiology's Recommendations for Korean women from a quality assurance standpoint. The study population consisted of 92 paired MLO and CC mammograms obtained on one mammographic unit. The digital readouts of compressed breast thickness, applied compression force and tube voltage were recorded. Entrance skin exposure was measured by dosimetry. MGD was calculated by multiplying entrance skin exposure by the exposure-to-absorbed dose conversion factor. The range of breast thickness was 1.3-6.2 cm in CC mammograms with a mean breast thickness of 3.6 cm, and 1.6-6.5 cm in MLO mammograms with a mean breast thickness of 3.9 cm. MGDs in CC and MLO mammograms were 1.77 mSv and 1.88 mSv per view, respectively. Breast composition patterns were divided into 4 groups according to ACR BI-RADS; P1 (n=20), P2 (n=16), P3 (n=48) and P4 (n=8). The MGDs for these groups were: 1.82, 1.84, 1.84, and 1.91 mSv, respectively. When subjects were subdivided by breast thickness into three groups, namely, below 3 cm, 3 cm to 4.2 cm, aaa and above 4.2 cm, the corresponding MGDs were 1.83, 1.86, and 1.91 mSv. According to our initial trial, the mean breast thickness and the MGDs of Korean women are lower than recommended by the American College of Radiology, which are commonly used for quality assurance purposes.

Purpose: To describe the changes in diagnostic methods and reassess the role of core biopsy compared with needle localization biopsy (NLB) in the evaluation of non-palpable breast lesions. Materials and Methods: We retrospectively analyzed 930 non-palpable breast lesions which underwent either core biopsy or NLB between January 1996 and December 2000. NLB involved 511 lesions in 482 patients, and core biopsy, 419 lesions in 365 patients. For a given lesion, NLB was guided by either mammography (n=213) or ultrasonography (US) (n=298), but only US was used to guide a core biopsy. Over the five-year period, we calculated the total number of biopsies per year relating to non-palpable breast lesions, also determining the mammographic findings in cases involving NLB and the percentage of malignancies seen at histopathology. Results: The total number of biopsies increased with time: 1996: n=91, 1997: n=118, 1998: n=144, 1999: n=245, 2000: n=332. The implementation of core biopsy, however, led to a decrease in the proportion of NLB (1996: 97.8%, 1997: 84.7%, 1998: 75.7%, 1999: 47.8%, 2000: 28.9%). Among nonpalpable lesions which underwent mammography-guided NLB, 46.5% (20/43) were a mass or density in 1996, while in 2000 the proportion was 5.7% (2/35). During this period, however, the finding of calcification without a mass increased from 48.8% in 1996 to 94.3% in 2000. The proportion of cases in which NLB demonstrated malignancy increased from 13.5% in 1996 to 49% in 2000; where US-guided NLB was used, the increase was remarkable: from 15.2% in 1996 to 65.6% in 2000. Conclusion: In the evaluation of non-palpable breast lesions, the appropriate use of core biopsy can decrease the need for a more invasive method such as NLB. Furthermore, because most benign lesions diagnosed by core biopsy do not also undergo NLB, the use of the latter increases the yield of malignancies.

Purpose: To evaluate the clinical and imaging findings of tubular carcinoma of the breast. Materials and Methods: We retrospectively assessed the clinical and imaging findings of ten lesions of pathologically proven tubular carcinoma in nine patients, also evaluating the mammographic findings and categorizing the mass according to the ACR BI-RADS classification. The ultrasonographic findings were assessed in terms of shape, echogenicity, margin and posterior shadowing, and in four cases the size of nodules at physical examination was compared with the mammographic, ultrasonographic and pathologic findings. Results: Nine lesions were palpable at physical examination. Bilateral tubular carcinoma of the breast was found in one patient, and unilateral single lesions in the others. There was no metastasis and no death within an average of 666 (range, 163) days of surgery. At mammography, masses were detected in six cases; the features, observed were a lobular or irregular shape (6/6), a spiculated margin (3/6) and high density (5/6). Ultrasonography showed that all unilateral lesions were hypoechoic (8/8), with a lesion height-to-width ratio of greater than 1.0 in seven of these, an ill-defined margin in sis, and posterior acoustic shadowing in seven. Mean nodule diameter was 1.17 cm at physical examination, 1.09 cm at mammography, 0.86 cm at ultrasonography and 0.80 cm at pathological evaluation. Conclusion: Most tubular carcinomas were palpable in spite of their small size, and their postoperative prognosis was good. Ultrasonography is useful in the detection of mammographically occult tubular carcinoma and for measuring the size of lesions.
