Introduction Coronavirus disease 2019 (COVID-19) has spread rapidly across the world. Given the sharply increased infection rate, the number of pregnant women and children with COVID-19 is correspondingly on the rise. SARS-CoV-2 infection is transmitted through droplets; though hypothesized, other transmission routes have not been confirmed. As of now, it remains unclear whether and how SARS-CoV-2 can possibly be transmitted from the mother to the fetus. Method This study examines the medical records of 30 neonates born to women with COVID-19, the objective being to provide documented information on maternal-child transmission and infant outcomes. Results Out of the 30 newborns, 28 had negative PCR test results for SARS-CoV-2; among their mothers, fifteen had fever, nine had cough and twenty had delivered by cesarean section. The median birth term was 37wk2dy, and twenty of the neonates were male. Most of them were asymptomatic, except for the three who presented with shortness of breath. Two of them were intubated and both died, the first because of severe sepsis and the second due to severe hyaline membrane disease. As regards the two infected neonates, the first represents a probable case of congenital SARS-CoV-2 infection, which appears unlikely in the second case. The outcome for both of them was good, without any complications. Conclusion Maternal-fetal transmission of the SARS- CoV-2 virus was not detected in the majority of the reported cases, although two of 30 neonates had positive qRT-PCR test results. Our study supports the hypothesis that though it seldom actually occurs, in utero SARS-CoV-2 vertical transmission is possible.

Resume Introduction Le Streptococcus pneumoniae est un agent rare d’infection maternofœtale. Malgre son incidence faible, il est responsable d’une mortalite et d’une morbidite neonatales elevees. L’objectif de notre etude est de determiner le profil anamnestique, clinique, biologique, therapeutique et evolutif de l’infection maternofœtale a S. pneumoniae a partir de 6 observations colligees sur une periode de 15 ans au service de neonatalogie et reanimation neonatale de l’hopital Abderrahim-Harrouchi du CHU Ibn-Rochd de Casablanca au Maroc. Resultats La mediane d’âge d’apparition des signes cliniques etait de 30 heures avec des extremes de h0–h72 et la mediane d’âge d’admission de 54 heures avec des extremes de h18–j4. La grossesse etait mal suivie chez tous les patients, dont 4 nouveau-nes a terme et 2 prematures. L’accouchement etait par voie basse dans 4 cas. L’anamnese infectieuse etait positive chez 4 nouveau-nes. La symptomatologie clinique etait dominee par signes respiratoires, les troubles neurologiques et un cas fievre associee a un ictere precoce. Le S. pneumoniae a ete isole dans le sang chez 2 patients, dans le liquide cephalorachidien chez 2 autres et dans les 2 sites (sang et liquide cephalorachidien) chez un patient. Il a ete resistant a l’ampicilline chez deux malades et sensible aux cephalosporines de troisieme generation et a la vancomycine chez 3 autres. L’antibiogramme n’a pas ete realise chez un patient ou le S. pneumoniae a ete detecte apres la recherche des antigenes sensibles. L’antibiotherapie de premiere intention a ete basee sur les cephalosporines de troisieme generation associees a un aminoside en attendant l’obtention de l’antibiogramme. La vancomycine a ete instauree secondairement chez 2 nouveau-nes. L’evolution a ete bonne chez un seul patient, 4 patients decedes entre j1 et j8, et persistance de sequelles neurologiques post-meningitiques chez un autre. Conclusion Notre travail illustre la tres faible frequence des infections maternofœtales a S. pneumoniae ainsi que leur gravite (pouvant etre liee a l’augmentation des resistances aux antibiotiques ou au retard diagnostic). D’ou l’interet d’un diagnostic precoce necessitant une collaboration entre pediatres, obstetriciens et bacteriologistes.

An increased incidence and severity of invasive group A streptococcus (GAS) infections over the past decade have been reported by several authors, but GAS remains an uncommon cause of bacterial meningitis. The aim of this study was to describe and analyze the clinical and biological data of GAS meningitis by reporting 10 new cases of pediatric GAS meningitis and making a literature review. The mean age of patients, seven girls and three boys, was 3 years. There was a history of preexisting or concomitant community-acquired infection in five patients over 10. The outcome was fatal in two cases. All patients received an initial empirical antimicrobial therapy with a third generation cephalosporin switched in six cases to amoxicillin. The prognosis for this type of streptococcal meningitis is usually good, but death may occur even in children without any identified risk factor for severe infection.

Gastric pneumatosis is a rare pathology. Its occurrence in the neonatal period requires looking for ulcerative-necrotizing enterocolitis in a context of prematurity or an underlying surgical obstacle. We report a case of gastric pneumatosis at a newborn child born at term, admitted on the third day of life for neonatal occlusion with a flat stomach. The abdomen without preparation showed substantial gastric distension with aspects of gastric pneumatosis and embellishes with images showing a double gastric bubble. The surgical exploration showed gastric pneumatosis, complete duodenal atresia located at the level of the second duodenal portion, and an annular pancreas. Progression was favorable after duodenostomy.

Objectifs Identifier les caracteristiques des infections a Serratia Marcescens. Proposer une strategie de gestion d’epidemie. Methodes Etude retrospective d’infections nosocomiales a Serratia Marcescens de septembre a Octobre 2013 au servicede neonatologie de l’hopital d’enfant de Casablanca. Des prelevements dans l’environnement hospitalier et chez le personnel ont ete realises. Resultats Neuf cas ont ete enregistres,7 inborn et 2 outborn. Quatre nouveaux- nes etaient des prematures dont trois avaient un poids inferieur a 2000g. Sept nouveaux-nes etaient sous antibiotherapie. L’hemoculture initiale etait sterile chez tous les cas. Le delai moyen d’apparition de l’infection nosocomiale etait de 5 jours. Le mode de revelation etait un choc septique dans 5cas, des convulsions chez 2 cas, et un echec de deventilation chez deux malades. Neuf cas de bacteriemies et deux cas de meningites etaient rapportes. Toutes les souches ont presente le meme antibiotype. La duree moyenne d’hospitalisation etait de 20 jours. Le taux de mortalite etait de 54%. Aucune source de contamination n’a ete retrouvee. Conclusions L’isolement technique et geographique des cas infectes, la limitation du nombre d’hospitalisation, la sensibilisation du personnel soignant ont permis la maitrise de diffusion du germe.
