This study aimed to assess the trends of different smoking indicators among Peruvian adolescents. Additionally, we evaluated whether such trends were different by sex or among those without previous smoking history.We analyzed the Global Youth Tobacco Survey (Global Youth Tobacco Survey 2007, 2014, 2019). Active smoking was defined according to smoking in the last 30 days. Passive smoking was assessed based on exposure to tobacco inside and outside the household, both overall and daily. Analyses considered the sample design.A total of 17,047 records (9,869 in 2007, 3,424 in 2014, and 3,754 in 2019) were analyzed; the mean age was 14 years, and 49.9% were women. Previous smoking history was reported in 26.6% of the records; such prevalence fell from 45.2% (2007), to 25.3% (2014), and to 19.4% (2019, p-value for trend0.001), whereas active smoking fell from 17.1% (2007) to 8.7% (2014) and to 5.7% (2019). The overall prevalence of passive smoking inside the household fell from 24.7% to 12.9% and 10.4% (p-value0.001), whereas the overall prevalence of passive smoking outside the household dropped from 46.3% to 39.4% and 36.3% (p-value0.001) during the same period. The reduction of the smoking indicators was observed mainly among women than in men.There is evidence of a sustained reduction in smoking indicators in Peruvian adolescents. Passive smoking outside the household continues to be common, calling for strengthening current tobacco control policies.El objetivo del presente estudio fue evaluar las tendencias ocurridas en diferentes indicadores de tabaquismo en adolescentes peruanos. Asimismo, se evaluó las tendencias en dichos indicadores de acuerdo a sexo y en aquellos sin antecedente de tabaquismo previo.El presente estudio usa la Encuesta Mundial sobre Tabaquismo en Jóvenes (Global Youth Tobacco Survey 2007, 2014, 2019). Tabaquismo activo se definió según el consumo de tabaco en los últimos 30 días. Tabaquismo pasivo se definió en función a exposición a tabaco dentro y fuera del hogar, tanto global como en forma diaria. Los análisis consideraron el diseño muestral.Un total de 17 047 registros (9869 en el 2007, 3424 en el 2014, y 3754 en el 2019) fueron analizados; edad media 14 años y 49,9% mujeres. El 26,6% reportó antecedente de tabaquismo previo; dicha prevalencia cayó de 45,2% (2007), a 25,3% (2014), y a 19,4% (2019, p de tendencias0,001), mientras que el tabaquismo activo cayó de 17,1% (2007) a 8,7% (2014) y a 5,7% (2019). La prevalencia global de tabaquismo pasivo en el hogar cayó de 24,7%, a 12,9% y a 10,4% (p de tendencias0,001), mientras que la prevalencia global de tabaquismo pasivo fuera del hogar se redujo de 46,3%, a 39,4%, y a 36,3% (p0,001) en ese lapso. La caída en los indicadores de tabaquismo se vio principalmente en mujeres que en varones.Se evidencia una reducción sostenida en los indicadores de tabaquismo en adolescentes peruanos. El tabaquismo pasivo fuera del hogar continúa siendo frecuente requiriendo el fortalecimiento de las actuales políticas de control de tabaco.

To determine if there is an association between type 2 diabetes mellitus (T2DM) and some sleep characteristics: duration, sleep difficulties and quality, in a population aged between 30 and 69 years in Tumbes.Cross-sectional study. The outcomes were sleep difficulty (sometimes/ almost never vs. frequently), sleep duration (normal, short, and long), and sleep quality (good and bad). The exposure of interest was the presence of T2DM assessed using the glucose tolerance test (without T2DM, with T2DM but without previous diagnosis, and with T2DM and with previous diagnosis). Poisson regression models were used to report prevalence ratio (PR) and 95% confidence intervals (95%CI).A total of 1604 subjects were analyzed and the mean age was 48.2 years; 50.3% were women, 71 (4.4%) had T2DM without a previous diagnosis, and 105 (6.5%) had T2DM with a previous diagnosis. Regarding sleep characteristics, 12.0% had short sleep, 8.2% had long sleep, 3.7% had sleep difficulties, and 19.5% presented poor sleep quality. In a multivariable model, T2DM with previous diagnosis was associated with sleep difficulty (PR= 2.20; 95%CI: 1.13 - 4.27) and bad sleep quality (PR= 1.40; 95%CI: 1.05 - 1.92) compared to those without T2DM.Individuals with previous diagnosis of T2DM had greater probability of presenting sleep difficulties and poor sleep quality. These results suggest the need to evaluate periodically the sleep characteristics of patients with T2DM.Determinar si existe asociación entre la presencia de diabetes mellitus tipo 2 (DM2) y la duración, dificultad para dormir, y calidad de sueño, en sujetos de 30 a 69 años en Tumbes.Estudio transversal analítico. Las variables dependientes fueron dificultad para dormir (a veces/casi nunca y frecuentemente), duración del sueño (normal, corto y prolongado) y calidad del sueño (buena y mala). La variable independiente fue la presencia de DM2 evaluada usando la prueba de tolerancia oral a la glucosa (no DM2, con DM2 y sin diagnóstico previo, y con DM2 y diagnóstico previo). Se usó modelos de regresión de Poisson para reportar razones de prevalencia (RP) e intervalos de confianza al 95% (IC 95%).Se analizaron 1604 individuos, con una edad media de 48,2 años; 50,3% fueron mujeres; 71 (4,4%) tenían DM2 sin diagnóstico previo y 105 (6,5%) tenían DM2 con diagnóstico previo. Según las características del sueño, 12,0% presentó sueño corto y 8,2% presentó sueño prolongado; 3,7% reportó dificultad para dormir, y 19,5% tuvo mala calidad de sueño. En el modelo multivariable, tener DM2 con diagnóstico previo estuvo asociado con dificultad para dormir (RP = 2,20; IC 95%: 1,13-4,27) y mala calidad de sueño (RP = 1,40; IC 95%: 1,05-1,92) comparado con aquellos sin DM2.Los individuos con DM2 que tenían diagnóstico previo tuvieron mayor probabilidad de presentar dificultad para dormir y mala calidad de sueño. Nuestros resultados sugieren la necesidad de evaluar en forma periódica las características del sueño en pacientes con DM2.

OBJECTIVE To assess the longitudinal effect of double burden of malnutrition (DBM) on the risk of developing child and adolescent overweight or obesity. METHODS Analysis of the Peruvian Young Lives Study, younger cohort: baseline (2002) and 4 follow-ups (2006–2007, 2009–2010, 2013–2014, and 2016–2017). Outcomes were the incidence of overweight and obesity as defined by the World Health Organization standards. The exposure comprised a variable with 4 categories: non-stunted child with a non-overweight mother (reference group), non-stunted child with an overweight mother, stunted child with a non-overweight mother, and stunted child with an overweight mother (i.e., DBM). Poisson regression models were built to assess the association of interest, and relative risks (RR) and 95%CI were reported. RESULTS Data from 2,034 children; 50.0% were girls and the mean age was 12.0 (3.6) months at baseline. Non-stunted children with an overweight mother had greater risk (RR = 1.64; 95%CI: 1.35–1.99) of developing overweight, compared with the risk for stunted children with a non-overweight mother (RR = 1.38; 95%CI: 1.10–1.72), and for those with DBM (RR = 1.28; 95%CI: 1.02–1.61). When compared with the reference group, obesity risk was greater among non-stunted children with an overweight mother (RR = 2.33; 95%CI: 1.68–3.22), greater among stunted children with a non-overweight mother (RR = 2.59; 95%CI: 1.75–3.84), and greater among those with DBM (RR = 2.14; 95%CI: 1.39–3.28). CONCLUSIONS DBM is a risk factor for childhood overweight and obesity in Peru. Dual-duty policies tackling both undernutrition in children and overweight in mothers are needed to reduce DBM and its future effects in Peru.

To determine the association between socioeconomic level and the presence of obesity, hypertension and type 2 diabetes mellitus in the Peruvian population.Secondary analysis of data from the National Demographic and Family Health Survey ( Encuesta Nacional Demográfica y de Salud Familiar , Endes) from 2018 to 2020. The outcomes were obesity, hypertension, and type 2 diabetes mellitus. The exposure variables were two indicators of socioeconomic status: educational level (7 years, 7-11 years, and 12+ years) and wealth index (in tertiles). Models were created using Poisson regression, reporting prevalence ratios (PR) and 95% confidence intervals (95%CI).Data from 98,846 subjects were analyzed. Mean age: 45.3 (SD: 16.0) years, and 55.5% were women. The prevalence of obesity was 26.0% (95%CI: 25.4-26.6); of hypertension, 24.9% (95%CI: 24.3-25.5); and of type 2 diabetes mellitus, 4.8% (95%CI: 4.5-5.1). In multivariate model, and compared with those with a low wealth index, those with a high wealth index had a higher prevalence of obesity (PR = 1.49; 95%CI: 1.38-1.62), hypertension (PR = 1.09; 95%CI: 1.02-1.17) and type 2 diabetes mellitus (PR = 1.72; 95%CI: 1.29-2.29). On the other hand, higher educational level was only associated with a reduction in the prevalence of obesity (PR = 0.89; 95%CI: 0.84-0.95).There is a differential association between the wealth index, educational level and markers of noncommunicable diseases. There is evidence of a positive association between wealth index and obesity, hypertension and type 2 diabetes mellitus, whereas educational level was only negatively associated with obesity.

To determine the prevalence and incidence of arterial hypertension, as well as the prevalence of previous diagnosis of arterial hypertension (self-reported) among the adult population of Peru.Systematic review and meta-analysis of epidemiological studies available in LILACS, EMBASE, MEDLINE and Global Health. Studies were included if they followed a random sampling approach in adult population. Screening and assessment of manuscripts was carried out independently by two researchers. A random-effects meta-analysis was conducted to quantify the overall prevalence and incidence of hypertension. The Newcastle-Ottawa scale was used to assess the risk of bias in the manuscripts.A total of 903 papers were screened, and only 15 were included in the estimation of hypertension prevalence, 8 in the assessment of previous hypertension diagnosis, and 4 for incidence estimations. The pooled prevalence of hypertension was 22.0% (95% CI: 20.0% - 25.0%; I2=99.2%). This estimate was lower in national studies [20.0% (95% CI: 17.0% - 22.0%; I2=99.4%] than in sub-national studies [24.0% (95% CI: 17.0% - 30.0%; I2=99.2%]. The pooled prevalence of previous hypertension diagnosis was 51.0% (95% CI: 43.0% - 59.0%; I2=99.9%). The pooled incidence of hypertension was 4.2 (95% CI: 2.0 - 6.4; I2=98.6%) per 100 person-years. The included studies did not present high risk of bias.Our findings show that one in five Peruvians has hypertension, and that four new cases appear per 100 persons per year; in addition, only half of the subjects with hypertension are previously diagnosed.Determinar la prevalencia e incidencia de hipertensión arterial, y la prevalencia de diagnóstico previo de hipertensión arterial (autorreportado) en población general adulta del Perú.Revisión sistemática y metaanálisis de estudios epidemiológicos disponibles en LILACS, EMBASE, MEDLINE y Global Health. Se incluyeron estudios que siguieron un muestreo aleatorio de la población general adulta. El tamizaje y estudio de los manuscritos fue realizado por dos investigadores de forma independientemente. Se hizo metaanálisis de efectos aleatorios para cuantificar la prevalencia e incidencia global. Los manuscritos fueron evaluados con la escala Newcastle-Ottawa para evaluar el riesgo de sesgo.Se tamizaron 903 artículos, y se incluyeron 15 manuscritos para prevalencia, 8 para prevalencia de diagnóstico previo, y 4 para incidencia de hipertensión. El metaanálisis mostró una prevalencia agregada de hipertensión de 22,0% (IC 95%: 20,0% - 25,0%; I 2=99,2%). Esta prevalencia fue menor en estudios nacionales [20,0% (IC 95%: 17,0% - 22,0%; I 2 =99,4%] que en subnacionales [24,0% (IC 95%: 17,0% - 30,0%; I 2 =99,2%]. La prevalencia global de diagnóstico previo de hipertensión fue 51,0% (IC 95%: 43,0% - 59,0%; I 2=99,9%). La incidencia global fue de 4,2 (IC 95%: 2,0 - 6,4; I 2=98,6%) por cada 100 personas-año. Los manuscritos estudiados no presentaron alto riesgo de sesgo.Nuestros hallazgos muestran que uno de cada cinco peruanos tiene hipertensión, y que aparecen cuatro nuevos casos por 100 personas en un año, además solo la mitad de los pacientes hipertensos tienen el diagnóstico previo de su condición.

To determine the incidence of stroke, overall and by sub-types, in Peru between 2017 and 2018.Analysis of hospital morbidity data obtained from SUSALUD (open data). Using the ICD-10 codes, the following were studied: subarachnoid hemorrhage (I60), atraumatic intracerebral hemorrhage (I61), cerebral infarction (I63), and unspecified stroke (I64). The crude and age-standardized incidence was calculated according to the population of the World Health Organization and using the national projected population number of people according to year, age and sex as the denominator.In 2017, a total of 10,570 stroke cases were recorded, whereas, in 2018, there were 12,835 cases. Ischemic events were more frequent in both years. Regardless of stroke subtype and year, men were more affected than women. In the 35+ year-old population, an increase in the crude incidence of total stroke was observed between 2017 and 2018, from 80.9 to 96.7 per 100,000 person-years. The age-standardized incidence showed the same trend, but in a greater magnitude: from 93.9 to 109.8 per 100,000 person-years. Ischemic stroke was the one that increased the most, with an age-standardized rate in people aged 35+ years of 35.2 in 2017 and 46.3 per 100,000 person-years in 2018.The incidence of stroke is high in Peru. Ischemic cases are the most frequent and they disproportionately affect men. Our results suggest the need for a surveillance system to robustly quantify the incidence of these cases and understand their determinants.Determinar la tasa de incidencia del accidente cerebro vascular (ACV) total y por subtipos en el Perú entre 2017 y 2018.Análisis de datos de morbilidad hospitalaria obtenidos de la Superintendencia Nacional de Salud (datos abiertos). A partir de los códigos CIE-10 se estudiaron la hemorragia subaracnoidea (I60), la hemorragia intracerebral atraumática (I61), el infarto cerebral (I63) y el ACV no especificado (I64). Se estimó la tasa de incidencia cruda y estandarizada por edad (comparando contra la población de la Organización Mundial de la Salud) y se usó como denominador el número de personas según año, edad y sexo de acuerdo con proyecciones nacionales.En 2017, se registró un total de 10 570 casos de ACV, mientras que en 2018 hubo 12 835. Los eventos isquémicos fueron más frecuentes en ambos años. Independientemente del subtipo de ACV y del año, los varones fueron más afectados que las mujeres. En la población de 35 años a más, se evidenció un aumento en la incidencia cruda de ACV total entre 2017 y 2018, de 80,9 a 96,7 por 100 000 personas-año. La incidencia estandarizada mostró la misma tendencia, pero en mayor magnitud: de 93,9 a 109,8 por 100 000 personas-año. El ACV isquémico fue el que más aumentó, con una tasa estandarizada en mayores de 35 años de 35,2 en 2017 y de 46,3 en 2018 por 100 000 personas-año.La incidencia de ACV es elevada en el Perú, con predominio de casos isquémicos y afectando desproporcionalmente a los varones. Estos resultados sugieren la necesidad de tener un sistema de vigilancia para cuantificar la incidencia de estos casos y entender sus determinantes.

BackgroundDisability rates increase with age. In 2012, Peruvian older adults (≥ 65 years) represented 9% of the population. Additionally, older population reported disabilities at about 5 times the rate of Peruvians between 36 and 64 years old, and 30% of older population lived in poverty. Peruvian seniors living in extreme poverty experience disabilities and the extent of their access to healthcare is unknown.ObjectiveThis study assesses associations between disability and access to healthcare among Peruvians older individuals living in extreme poverty.MethodsSecondary analysis of a national representative population based survey that utilizes information from Peru's 2012 survey Health and Wellbeing in Older Adults (ESBAM), which includes older adults living in extreme poverty. We define disability in terms of the Activities of Daily Living (ADL disability) framework. Healthcare access was assessed as having any of Peru's available health insurance schemes combined with preventive health services (vision assessment, influenza vaccination, blood pressure assessment, diabetes screening, and cholesterol assessment). Poisson robust regression models were used to evaluate the associations among relevant variables. Prevalence Ratios and 95% confidence intervals (95%CI) were reported.ResultsData from 3869 individuals (65 to 80 years old), of whom 1760 (45.5%) were females, were analyzed. The prevalence of ADL disability was 17.3% (95%CI: 16.0%-18.4%). In addition, more than 60% had never received any of the preventive measures evaluated, except for the blood pressure assessment. In the adjusted model, people with ADL disability had 63% less probability of having extensive insurance, compared to those without disability (pConclusionsThis study shows that this Peruvian older population living in extreme poverty has limited access to healthcare services. Although there was no consistent association between ADL disability and the healthcare access, there is an urgent need to reduce the inequitable access to healthcare of this poor Peruvian older population.

Family support is crucial for managing chronic conditions but it is often overlooked when designing behavioral interventions in type 2 diabetes mellitus (T2DM). As part of the formative phase of a feasibility randomized control trial (RCT), we conducted 20 semistructured interviews with people with T2DM from Lima, Peru. Based on such results, we describe the support people with T2DM receive from their families and the role that such support has in their efforts to implement diabetes management practices. We learned that participants receive support from family members, but mostly from their spouses and children. Their relatives encourage them and motivate them to fight for their health, they also provide instrumental support by preparing healthy meals, reminding them to take medications, and sharing physical activity. Participants also reported controlling actions which were not always “well received.” Thus, any intervention supporting self-management practices need to work with key family members. We support the literature that suggests that interventions should target family members to ensure improved T2DM self-management practices.

To describe the mortality trend due to chronic kidney disease (CKD) in Peru in the period 2003-2015.Analysis of secondary data and ecological design. National mortality registries based on death certificates were analyzed. ERC was defined as CIE code 10: N18 in the basic cause. Absolute and relative frequencies of death by ERC are described. To estimate the proportion of deaths by CKD in each region of Peru, a generalized mixed linear model was used. The outcome variable was the proportion of deaths, the independent variables were each region of the country and the year.The analysis included 1,086,778 deaths, of which 25,091 (2.0 % age- and sex-adjusted) were by CKD. During the study period, the average age at death increased by 2.6 years (p0.001); in addition, the proportion of age- and sex-adjusted CKD deaths was always higher in women. In the observation period, the region with the highest mortality from CKD was Puno (4.1%), and with the lowest mortality was Amazonas (1.1%). Those regions that showed a significantly greater increase than the others were Tacna, La Libertad, Tumbes, Apurímac, Cusco, Ica, Moquegua, Ayacucho, Huancavelica and Puno.In the 2003-2015 period, mortality from CKD in Peru has increased; this trend is observed in several regions of the country. Preventive measures, early identification, and access to treatment must be implemented to control this trend.Describir la tendencia de mortalidad por enfermedad renal crónica (ERC) en el Perú en el periodo 2003-2015.Análisis de datos secundarios y de diseño ecológico. Se analizaron registros nacionales de mortalidad basados en certificados de defunción. ERC fue definida como código CIE 10: N18 en la causa básica. Se describen frecuencias absolutas y relativas de muerte por ERC. Para estimar la proporción de muertes por ERC en cada región del Perú, se utilizó un modelo lineal mixto generalizado. La variable desenlace fue la proporción de muertes, las variables independientes fueron cada región del país y el año.El análisis incluyó 1 086 778 defunciones, de los cuales 25 091 (2,0 % ajustado por edad y sexo) fueron por ERC. Durante el periodo de estudio la edad promedio, al momento del fallecimiento, aumentó en 2,6 años (p0,001); además, la proporción de muertes por ERC ajustada por edad y sexo fue siempre mayor en mujeres. En el periodo de observación la región con mayor mortalidad por ERC fue Puno (4,1 %), y con menor mortalidad fue Amazonas (1,1 %). Aquellas regiones que mostraron un aumento significativamente mayor que las demás fueron Tacna, La Libertad, Tumbes, Apurímac, Cusco, Ica, Moquegua, Ayacucho, Huancavelica y Puno.En el periodo 2003-2015, la mortalidad por ERC en el Perú ha aumentado; esta tendencia se observa en varias regiones del país. Es necesario implementar medidas de prevención, temprana identificación, y acceso al tratamiento, para mejorar esta tendencia.

Objective To determine the association between consumption of snacks and sweetened beverages and risk of overweight among children. Design Secondary analysis of the Young Lives cohort study in Peru. Setting Twenty sentinel sites from a total of 1818 districts available in Peru. Subjects Children in the younger cohort of the Young Lives study in Peru, specifically those included in the third (2009) and the fourth (2013) rounds. Results A total of 1813 children were evaluated at baseline; 49·2 % girls and mean age 8·0 (sd 0·3) years. At baseline, 3·3 (95 % CI 2·5, 4·2) % reported daily sweetened beverage consumption, while this proportion was 3·9 (95 % CI 3·1, 4·9) % for snacks. Baseline prevalence of overweight was 22·0 (95 % CI 20·1, 23·9) %. Only 1414 children were followed for 4·0 (sd 0·1) years, with an overweight incidence of 3·6 (95 % CI 3·1, 4·1) per 100 person-years. In multivariable analysis, children who consumed sweetened beverages and snacks daily had an average weight increase of 2·29 (95 % CI 0·62, 3·96) and 2·04 (95 % CI 0·48, 3·60) kg more, respectively, than those who never consumed these products, in approximately 4 years of follow-up. Moreover, there was evidence of an association between daily consumption of sweetened beverages and risk of overweight (relative risk=2·12; 95 % CI 1·05, 4·28). Conclusions Daily consumption of sweetened beverages and snacks was associated with increased weight gain v. never consuming these products; and in the case of sweetened beverages, with higher risk of developing overweight.

Aim Limited information exists regarding the implementation of salt reduction strategies on collective food services, such as restaurants and food concessionaires. The present study aimed to assess the effect of a salt reduction strategy on blood pressure levels and food acceptability among customers of a food concessionaire. Methods A quasi‐experimental study with two phases was conducted. In the pre‐intervention phase, the amount of salt used in food preparation was determined. In the intervention phase, a reduction of 20% in salt added to food preparations was implemented. Four hedonic tests and two blood pressure measurements were performed before and after the intervention implementation using standardised techniques. In addition, an evaluation of uneaten food was conducted daily on all customers’ plates. Mixed linear regression models were generated to assess the effect of the intervention on blood pressure and acceptability. Results A total of 71 workers were evaluated, mean age of 37.5 years, 57.8% females, who consumed the food of the concessionaire, on average, 4.4 (SD: 0.7) days per week. The aim of this study was to estimate the incidence of T2DM in three population groups: rural, rural-to-urban migrants and urban dwellers. Data from the PERU MIGRANT Study was analysed. The baseline assessment was conducted in 2007–2008 using a single-stage random sample and further follow-up was undertaken in 2015–16. T2DM was defined based on fasting glucose and self-reported diagnosis. Poisson regression models and robust variance to account for cluster effects were used for reporting risk ratios (RR) and 95%CI. At baseline, T2DM prevalence was 8% in urban, 3.6% in rural-to-urban migrants and 1.5% in rural dwellers. After 7.7 (SD: 1.1) years, 6,076 person-years of follow-up, 61 new cases were identified. The incidence rates in the urban, migrant and rural groups were 1.6, 0.9 and 0.5 per 100 person-years, respectively. Relative to rural dwellers, a 4.3-fold higher risk (95%CI: 1.6–11.9) for developing T2DM was found in urban dwellers and 2.7-fold higher (95%CI: 1.1–6.8) in migrants with ≥30 years of urban exposure. Migration and urban exposure were found as significant risk factors for developing T2DM. Within-country migration is a sociodemographic phenomenon occurring worldwide; thus, it is necessary to disentangle the effect of urban exposure on non-healthy habits and T2DM development.

Background Studies have reported the incidence/risk of becoming obese, but few have described the trajectories of body mass index (BMI) and waist circumference (WC) over time, especially in low/middle-income countries. We assessed the trajectories of BMI and WC according to sex in four sites in Peru. Methods Data from the population-based CRONICAS Cohort Study were analysed. We fitted a population-averaged model by using generalised estimating equations. The outcomes of interest, with three data points over time, were BMI and WC. The exposure variable was the factorial interaction between time and study site. Results At baseline mean age was 55.7 years (SD: 12.7) and 51.6% were women. Mean follow-up time was 2.5 years (SD: 0.4). Over time and across sites, BMI and WC increased linearly. The less urbanised sites showed a faster increase than more urbanised sites, and this was also observed after sex stratification. Overall, the fastest increase was found for WC compared with BMI. Background:Physical inactivity and sedentary behaviors have been linked with impaired health outcomes. Establishing the physical inactivity profiles of a given population is needed to establish program targets and to contribute to international monitoring efforts. We report the prevalence of, and explore sociodemographical and built environment factors associated with physical inactivity in 4 resource-limited settings in Peru: rural Puno, urban Puno, Pampas de San Juan de Miraflores (urban), and Tumbes (semiurban).Methods:Cross-sectional analysis of the CRONICAS Cohort Study’s baseline assessment. Outcomes of interest were physical inactivity of leisure time (Results:Data from 3593 individuals were included: 48.5% males, mean age 55.1 (SD: 12.7) years. Physical inactivity was present at rates of 93.7% (95% CI 93.0%–94.5%) and 9.3% (95% CI 8.3%–10.2%) within the leisure time and transport domains, respectively. In addition, 41.7% (95% CI 40.1%–43.3%) of participants reported watching TV for more than 2 hours per day. Rates varied according to study settings (P < .001). In multivariable analysis, being from rural settings was associated with 3% higher prevalence of leisure time physical inactivity relative to highly urban Lima. The pattern was different for transport-related physical inactivity: both Puno sites had around 75% to 50% lower prevalence of physical inactivity. Too much traffic was associated with higher levels of transport-related physical inactivity (PR = 1.24; 95% CI 1.01–1.54).Conclusions:Our study showed high levels of inactivity and marked contrasting patterns by rural/urban sites. These findings highlight the need to generate synergies to expand nationwide physical activity surveillance systems.

Background: Over one-quarter of the world’s adult population has hypertension, yet achieving adequate treatment or control targets remains a challenge.Objective: This study sought to identify, longitudinally, characteristics associated with antihypertensive treatment and blood pressure (BP) control among individuals with hypertension.Methods: Data from individuals enrolled in the population-based CRONICAS Cohort Study (adults ≥35 years, living in 4 different rural/urban and coastal/high-altitude Peruvian settings) with hypertension at baseline were used. Antihypertensive treatment and BP control were assessed at baseline and at 15 months. Multinomial logistic regressions were used to estimate relative risk ratios (RRR) and 95% confidence intervals (95% CI) of factors associated with antihypertensive treatment and BP control at follow-up.Results: At baseline, among 717 individuals with hypertension (53% women, mean age 61.5 ± 12.4 years), 28% were unaware of their hypertension status, 30% were aware but untreated, 16% were treated but uncontrolled, and 26% were treated and controlled. At follow-up, 89% of unaware and 82% of untreated individuals persisted untreated, and only 58% of controlled individuals remained controlled. Positive predictors of receiving treatment and being controlled at follow-up included age (RRR: 0.81; 95% CI: 0.73 to 0.91 for every 5 years) and family history of a chronic disease (RRR: 0.53; 95% CI: 0.31 to 0.92 vs. no history); whereas Puno rural site (RRR: 16.51; 95% CI: 1.90 to 143.56 vs. Lima) and male sex (RRR: 2.59; 95% CI: 1.54 to 4.36) were risk factors. Systolic BP at baseline (RRR: 1.27; 95% CI: 1.16 to 1.39 for every 5 mm Hg) and male sex (RRR: 1.75, 95% CI: 1.02 to 2.98) were risk factors for being treated but uncontrolled at follow-up.Conclusions: Large gaps in treatment of hypertension were observed. Targeting specific populations such as men, younger individuals, or those without family history of disease may increase coverage of antihypertensive treatment. Also, targeting male individuals or those with higher systolic BP could yield better rates of BP control in the short term.HighlightsThe majority of subjects with hypertension correspond to the unaware (28%) or aware but untreated (30%) categories.Between of 80% and 90% of patients in these 2 groups remain untreated at 15 months.Forty-two percent of individuals whose blood pressure was controlled at baseline no longer do so at follow-up.Implementation designs tailored to groups according to their risk profile together with patients' challenges and needs are required.

BACKGROUND: Urbanisation is an important contributor to the prevalence of asthma worldwide, and the burden of this effect in low-income and middle-income countries undergoing rapid industrialisation appears to be growing. We sought to characterise adult asthma prevalence across four geographically diverse settings in Peru and identify both individual and environmental risk factors associated with adult asthma. METHODS: We collected sociodemographics, clinical history and spirometry in adults aged >/=35 years. We defined asthma as meeting one of the three criteria: physician diagnosis, self-report of wheezing attack or use of asthma medications. We used multivariable logistic regression to assess individual and environmental factors associated with adult asthma. RESULTS: We analysed data from 2953 participants (mean age 55 years; 49% male). Overall asthma prevalence was 7.1%, which varied with urbanisation: highest in Lima (14.5%), followed by urban Puno (4.0%), semiurban Tumbes (3.8%) and rural Puno (1.8%). In multivariable analysis, being male (OR=0.60, 95% CI 0.39 to 0.93) and living at high altitude (OR=0.26, 95% CI 0.16 to 0.42) were associated with lower odds of having asthma, whereas living in an urban setting (OR=4.72, 95% CI 3.15 to 7.23) and family history of asthma (OR=1.83, 95% CI 1.19 to 2.73) were associated with higher odds. Current daily exposure to biomass fuel smoke (OR=1.18, 95% CI 0.70 to 1.91) and smoking (OR=0.99, 95% CI 0.73 to 1.22) were not associated with asthma. CONCLUSIONS: These findings confirm that urbanisation is an environmental risk factor of asthma, questions biomass fuel smoke exposure as an important risk factor and proposes high altitude as possibly protective against the development of asthma.

15 p., Background The Global Alliance for Chronic Diseases comprises the majority of the world’s public research funding agencies. It is focussed on implementation research to tackle the burden of chronic diseases in low- and middle-income countries and amongst vulnerable populations in high-income countries. In its inaugural research call, 15 projects were funded, focussing on lowering blood pressure-related disease burden. In this study, we describe a reflexive mapping exercise to identify the behaviour change strategies undertaken in each of these projects. Methods Using the Behaviour Change Wheel framework, each team rated the capability, opportunity and motivation of the various actors who were integral to each project (e.g. community members, non-physician health workers and doctors in projects focussed on service delivery). Teams then mapped the interventions they were implementing and determined the principal policy categories in which those interventions were operating. Guidance was provided on the use of Behaviour Change Wheel to support consistency in responses across teams. Ratings were iteratively discussed and refined at several group meetings. Results There was marked variation in the perceived capabilities, opportunities and motivation of the various actors who were being targeted for behaviour change strategies. Despite this variation, there was a high degree of synergy in interventions functions with most teams utilising complex interventions involving education, training, enablement, environmental restructuring and persuasion oriented strategies. Similar policy categories were also targeted across teams particularly in the areas of guidelines, communication/marketing and service provision with few teams focussing on fiscal measures, regulation and legislation. Conclusions The large variation in preparedness to change behaviour amongst the principal actors across these projects suggests that the interventions themselves will be variably taken up, despite the similarity in approaches taken. The findings highlight the importance of contextual factors in driving success and failure of research programmes. Forthcoming outcome and process evaluations from each project will build on this exploratory work and provide a greater understanding of factors that might influence scale-up of intervention strategies.

Study objectives Sleep disordered breathing (SDB) is a highly prevalent condition in high-income countries, with major consequences for cardiopulmonary health, public safety, healthcare utilization, and mortality. However, its prevalence and effect in low- and middle-income countries are less well known. We sought to determine the prevalence, risk factors, and comorbidities of SDB symptoms in four resource-limited settings. Design Cross-sectional analysis of the CRONICAS Cohort, a population-based age- and sex-stratified sample. Setting Four resource-limited settings in Peru varying in altitude, urbanization, and air pollution. Participants There were 2,682 adults aged 35 to 92 y. Measurements and results Self-reported SDB symptoms (habitual snoring, observed apneas, Epworth Sleepiness Scale), sociodemographics, medical history, anthropometrics, spirometry, blood biomarkers were reported. We found a high prevalence of habitual snoring (30.2%, 95% confidence interval [CI] 28.5-32.0%), observed apneas (20.9%, 95% CI 19.4-22.5%) and excessive daytime sleepiness (18.6%, 95% CI 17.1-20.1%). SDB symptoms varied across sites; prevalence and adjusted odds for habitual snoring were greatest at sea level, whereas those for observed apneas were greatest at high altitude. In multivariable analysis, habitual snoring was associated with older age, male sex, body mass index (BMI), and higher socioeconomic status; observed apneas were associated with BMI; and excessive daytime sleepiness was associated with older age, female sex, and medium socioeconomic status. Adjusted odds of cardiovascular disease, depression, and hypertension and total chronic disease burden increased progressively with the number of SDB symptoms. A threefold increase in the odds of having an additional chronic comorbid disease (adjusted odds ratio 3.57, 95% CI 2.18-5.84) was observed in those with all three versus no SDB symptoms. Conclusions Sleep disordered breathing symptoms were highly prevalent, varied widely across four resource-limited settings in Peru, and exhibited strong independent associations with chronic diseases.

Supplemental Digital Content is available in the text., It is unclear how well currently available risk scores predict cardiovascular disease (CVD) risk in low-income and middle-income countries. We aim to compare the American College of Cardiology/American Heart Association (ACC/AHA) Pooled Cohort risk equations (ACC/AHA model) with 6 other CVD risk tools to assess the concordance of predicted CVD risk in a random sample from 5 geographically diverse Peruvian populations. We used data from 2 Peruvian, age and sex-matched, population-based studies across 5 geographical sites. The ACC/AHA model were compared with 6 other CVD risk prediction tools: laboratory Framingham risk score for CVD, non-laboratory Framingham risk score for CVD, Reynolds risk score, systematic coronary risk evaluation, World Health Organization risk charts, and the Lancet chronic diseases risk charts. Main outcome was in agreement with predicted CVD risk using Lin’s concordance correlation coefficient. Two thousand one hundred and eighty-three subjects, mean age 54.3 (SD ± 5.6) years, were included in the analysis. Overall, we found poor agreement between different scores when compared with ACC/AHA model. When each of the risk scores was used with cut-offs specified in guidelines, ACC/AHA model depicted the highest proportion of people at high CVD risk predicted at 10 years, with a prevalence of 29.0% (95% confidence interval, 26.9–31.0%), whereas prevalence with World Health Organization risk charts was 0.6% (95% confidence interval, 0.2–8.6%). In conclusion, poor concordance between current CVD risk scores demonstrates the uncertainty of choosing any of them for public health and clinical interventions in Latin American populations. There is a need to improve the evidence base of risk scores for CVD in low-income and middle-income countries.

BACKGROUND: It is important to understand the local burden of non-communicable diseases including within-country heterogeneity. The aim of this study was to characterise hypertension and type-2 diabetes profiles across different Peruvian geographical settings emphasising the assessment of modifiable risk factors. METHODS: Analysis of the CRONICAS Cohort Study baseline assessment was conducted. Cardiometabolic outcomes were blood pressure categories (hypertension, prehypertension, normal) and glucose metabolism disorder status (diabetes, prediabetes, normal). Exposures were study setting and six modifiable factors (smoking, alcohol drinking, leisure time and transport-related physical activity levels, TV watching, fruit/vegetables intake and obesity). Poisson regression models were used to report prevalence ratios (PR). Population attributable risks (PAR) were also estimated. RESULTS: Data from 3238 participants, 48.3% male, mean age 45.3 years, were analysed. Age-standardised (WHO population) prevalence of prehypertension and hypertension was 24% and 16%, whereas for prediabetes and type-2 diabetes it was 18% and 6%, respectively. Introduction: One third of the world population is affected by latent tuberculosis, with 9.4 million new cases; medical students have 2 to 50 times the probability of acquiring the infection. Objectives: Establish the baseline prevalence of positive tuberculin skin test (TST) at the beginning of medical studies and determine the incidence and variables associated with TST conversion in medical students. Materials and Methods: Secondary analysis of a historical cohort (2007-2010) of medical students in a private Peruvian university. The TST conversion was evaluated. A binomial regression analysis was applied for each associated variable. Results: 707 medical students were included, of whom 219 (31%) were male. The basal prevalence of reactive TST was 14.4%. Signifi cant associations were found with the year of university entry of 2007-08 (p = 0.007) and a history of tuberculosis (p = 0.02). With a total of 822 person-years, the incidence of conversion was 2.92 cases per 100 person-years (CI95%: 1.96-4.36). The TST conversion was associated with the year of university entry (RR = 2,55; IC95%: 1,06-6,30) and a body mass index ≥ 25 kg/m2 (RR = 0,16; IC95%: 0,01-0,97). No association was detected with gender, tobacco or alcohol use. Conclusions: There is evidence of a high basal prevalence of latent tuberculosis infection in medical students. The incidence rate is within expected values and high in comparison with the general population. People with BMI ≥ 25 kg/m2 have protection against a latent infection. In medical students, more attention should be paid to biosecurity.; Introducción: Un tercio de la población mundial presenta una infección tuberculosa latente, con 9,4 millones de casos nuevos reportados en el 2009; los estudiantes de medicina tienen de 2 a 50 veces más probabilidad de adquirir la infección. Objetivos: Establecer la prevalencia de PPD positivo basal al inicio de la carrera médica y determinar la incidencia y los factores asociados a la conversión de PPD en alumnos de medicina. Materiales y Métodos: Análisis secundario de datos de una cohorte histórica (2007-2010) involucrando alumnos de medicina de una universidad privada en Perú. Se evaluó la conversión de PPD. Se aplicó un análisis de regresión binomial para cada variable de interés. Resultados: 707 estudiantes fueron seguidos, de ellos 219 (31%) fueron hombres. La prevalencia basal de PPD positivo fue 14,4%. Se encontró asociación significativa con el año de ingreso 2007-08 (p = 0,007) y antecedente de tuberculosis anterior (p = 0,02). Con un total de 822 personas-año, la incidencia de conversión fue de 2,92 por 100 personas-año (IC95%: 1,96-4,36). En el modelo bivariado, el año de ingreso a la carrera y el IMC > 25 kg/m2 estuvieron asociados con conversión de PPD. Sin embargo, en el modelo multivariado, sólo el año de ingreso mostró asociación estadísticamente significativa (RR = 2,53; IC95%: 1,11-5,76). Conclusiones: Existe una prevalencia basal elevada de infección latente en alumnos de medicina. La tasa de incidencia está dentro de los valores esperados y previamente reportados. Se recomienda prestar más atención a las medidas de bioseguridad y prevención en estudiantes de medicina.

To assess the association between frequency of television viewing, overweight and obesity in a nationally representative sample of Peruvian women.Secondary analysis of the Demographic and Health Survey 2008 including women aged from 15 to 49 years old. The outcome variables were obesity (body mass index30 kg/m²) and overweight (body mass index25 but30 kg/m²) whereas the exposure variable was frequency of television viewing (never, occasionally, almost every day). Logistic regression taking into account the multistage study design and adjusting for potential confounders was used. Results were presented as adjusted odds ratios (aOR) with 95% confidence intervals (95%CI).A total of 21,712 women were included in the analysis. The prevalence of overweight was 34.7% (95%CI 33.8%;35.7%), and obesity prevalence was 14.3% (95%CI 13.6%;15.1%). Compared to women who never watched television, those who reported watching television occasionally and almost daily were more likely to be obese: aOR 1.7 (95%CI 1.3;-2.3) and aOR 2.6 (95%CI 2.0;3.5), respectively. The magnitude of this association was lower for overweight: aOR 1.2 (95CI 1.3;2.3) and aOR 1.6 (95%CI 1.1;1.4), respectively. The strength of the association was greater in urban areas.Frequency of television viewing was associated with overweight and obesity in Peruvian women and the strength of this association varied by area of residence. These findings can provide input to strategies for obesity prevention in the Peruvian context.

To explore if there is a difference in the perception and self reported quality of life between rural-to-urban migrants and urban groups.Cross-sectional study, secondary analysis of the PERU-MIGRANT study (PEru's Rural to Urban MIGRANTs Study). WHOQOL-Brief survey' s global scores and per specific domains obtained in the survey were compared using Kruskall-Wallis' test and assessing size effect.A total of 307 subjects (62.2% migrants, 57% female, means age 47 years-old) were surveyed. Compared with the urban group, migrants reported lower quality of life both on the global scores as well as in psychological health and the living environment domains. Migrants reported a higher score on the physical health’s domain.The impact of rural-to-urban migration on quality of life suggests a differential effect within its specific domains.

To evaluate the frequency of intestinal parasites and risk factors associated with coccidian infection in patients cared for at a public hospital in Lima-Peru.A descriptive, cross-sectional study was conducted using reports of the parasitology laboratory. Patients included were18 years, who gave at least one faecal sample for parasitologic evaluation. Logistic regression was used to calculate Odd Ratios (OR) and 95%) confidence intervals.2.056 patients were included in the analysis, 55.2% of them were males and 334 (16.3%) were HIV positive. Overall, Blastocystis hominis was the most frequent parasite (35.4%). The multivariate model adjusted for sex shows that HIV infection (OR = 4.53; 95% CI: 3.03-6.77), being hospitalized (OR = 2.42; 95% CI: 1.06-5.52), and ageor = 40 years (OR = 0.57; 95% IC: 0.37-0.86) were associated with coccidian infection.Blastocystis hominis was the most frequent parasite in HIV positive and negative patients. Being hospitalized and HIV infection were risk factors for coccidian infection, but ageor = 40 years was a protective factor.

Although in Peru, Control Program of Tuberculosis has been successful in the reduction of incidence, morbidity and mortality, fatal cases are still observed. A longitudinal retrospective study was undertaken in a population on the outskirts of Lima between January 2000 and December 2005. Kaplan-Meier and Cox Regression analysis were used in this study. A total of 425 patients commencing tuberculosis treatment were included, with a case fatality ratio of 4.5%. Factors associated with mortality were: body mass index18 kg/ m(2) at treatment beginning, HR = 4.89 (95%CI: 1.49 - 16.03) and HIV infection, HR=5.78 (95% CI: 1.11-29.99); whilst education level, HR = 0.28 (95% CI: 0.10 - 0.83) was associated with survival. Amongst this population commencing treatment for tuberculosis, body mass index and HIV infection were associated with an increased risk of death; in contrast, the education level was associated with improving in the survival.

OBJECTIVE: We aimed to characterize metabolic status by body mass index (BMI) status. METHODS: The CRONICAS longitudinal study was performed in an age-and-sex stratified random sample of participants aged 35 years or older in four Peruvian settings: Lima (Peru's capital, costal urban, highly urbanized), urban and rural Puno (both high-altitude), and Tumbes (costal semirural). Data from the baseline study, conducted in 2010, was used. Individuals were classified by BMI as normal weight (18.5-24.9 kg/m2), overweight (25.0-29.9 kg/m2), and obese (>/=30 kg/m2), and as metabolically healthy (0-1 metabolic abnormality) or metabolically unhealthy (>/=2 abnormalities). Abnormalities included individual components of the metabolic syndrome, high-sensitivity C-reactive protein, and insulin resistance. RESULTS: A total of 3088 (age 55.6+/-12.6 years, 51.3% females) had all measurements. Of these, 890 (28.8%), 1361 (44.1%) and 837 (27.1%) were normal weight, overweight and obese, respectively. Overall, 19.0% of normal weight in contrast to 54.9% of overweight and 77.7% of obese individuals had >/=3 risk factors (p/=65 years, female, and highest socioeconomic groups were more likely to have this pattern. In contrast, only 16.4% of overweight and 3.9% of obese individuals were metabolically healthy and, compared to Lima, the rural and urban sites in Puno were more likely to have a metabolically healthier profile. CONCLUSIONS: Most Peruvians with overweight and obesity have additional risk factors for cardiovascular disease, as well as a majority of those with a healthy weight. Prevention programs aimed at individuals with a normal BMI, and those who are overweight and obese, are urgently needed, such as screening for elevated fasting cholesterol and glucose.
