Abstract Background The aim of this study was to investigate the microbiome shift of denture biofilm formation, the incidence of Candida and changes of salivary inflammatory markers following insertion of 3-dimensional printed PMMA denture. Methods This was a pilot study of 6 patients provided with 3D printed (test group) or conventionally heat-cured (control group) removable partial acrylic dentures followed up for 6 weeks. 3 denture swabs were collected at week 1, 3 and 6; and saliva samples were collected at baseline, week 1, 3 and 6 following denture insertion. Microbial DNA was isolated, and the 16S rRNA gene was amplified and sequenced to assess the denture microbiota. The presence of Candida was determined using PCR assay. The levels of salivary biomarkers lactoferrin (LTF) and histatin (HTN) were determined using ELISA. Results Denture microbiome of both groups exhibited similar microbial compositions at weeks 1, 3 and 6. Objective: This study evaluated the effects of incorporating 1% and 2% cinnamon oil into a soft liner on the growth of Candida albicans colonies and the material's hardness.Methods: Thirty soft liner specimens were prepared for the disk diffusion and thirty for the hardness test. In each test, the specimens were divided into three subgroups based on the concentration of cinnamon oil: A) 0% (control), B) 1%, and C) 2% by weight. Cinnamon oil was added to the monomer of a heat-polymerized soft liner. C. albicans was cultured on Mueller-Hinton agar, and the diameters of the inhibition zones around the specimens were measured. The Shore A hardness test was conducted using the Shore durometer. Statistical analysis was performed by the independent t-test, one-way ANOVA, and Tukey post-hoc test (α=0.05).Results: The control group showed no inhibition zone. The specimens in Group C exhibited a significantly larger inhibition zone (16.72 ± 0.82 mm) than Group B (12.56 ± 0.82 mm; P < 0.001). The hardness values differed significantly among groups (P < 0.001). Group C demonstrated the highest hardness (48.54 ± 0.95), significantly greater than both Groups B (47.13 ± 0.72; P < 0.05) and A (45.23 ± 0.73; P < 0.05).Conclusions: Adding cinnamon oil improved the antifungal activity and hardness of the soft denture liner. Adding 2% cinnamon oil increased the hardness values to the upper limit of clinical acceptability. Therefore, the 1% concentration is recommended to balance antimicrobial efficacy and maintain optimal mechanical properties of the soft liner.

Abstract Background Hyoid bone is attached to the mandible, tongue, larynx, temporal bone, and cervical spine via different types of muscles or ligaments. The tongue, mandible, and hyoid system play a crucial role in swallowing function. This within subject study aimed to evaluate the impact of mandibular implant overdentures on the displacement of the hyoid bones during the swallowing process. Methods Twenty five healthy edentulous subjects were selected for participating in the study. New complete dentures were constructed for all the participants. Subsequently two dental implants were inserted in the canine regions of the participant’s mandibular arch. In order to retain the mandibular prosthesis in place, ball attachments were incorporated into the mandibular dentures to convert them into implant overdentures. Using 10 ml of thin liquid bolus, videofluoroscopy swallowing examination was performed in three different oral conditions: without complete dentures (WCD), with complete denture (CDs), and with a mandibular implant overdenture (IODs). ANOVA with Bonferroni test was used to analyze the data in order to determine how the hyoid displacement varied throughout different oral conditions. Results Compared to complete dentures, mandibular implant overdentures showed a significant decrease (P 0.05) between the two oral circumstances in terms of superior hyoid displacement or duration for hyoid maximum elevation (DOHME). There is no penetration or aspiration for both complete denture and implant overdenture oral conditions. Conclusion Implant retained overdentures have a positive effect on hyoid displacement during swallowing of thin liquid bolus consistency relative to conventional complete dentures. ABSTRACT Edentulism is the loss of permanent dental structures, which impacts aesthetics and essential functions like chewing and speech. Dental prosthetics, specifically complete dentures, offer a solution. This study provides an overview of the production process using CAD/CAM technology. Digital methods yield monolithic denture bases, enhancing aesthetics and eliminating issues like shrinkage. Digital technologies expedite the fabrication process. The conventional method has shortcomings, leading to poor-fitting dentures, multiple dental visits, higher costs, and limitations. CAD/CAM is poised to replace traditional techniques, but improvements are needed for a seamless transition. Current CAD/CAM systems must enhance digital techniques, ensure fully digital production, and reduce costs for broader consumer accessibility. This evolution holds the key to a brighter and more comfortable future for edentulous patients.

Abstract Background Tissue conditioners are used for treating and improving the tissues supporting complete dentures. On the other hand, recent advances in nanotechnology have revolutionized various fields of science, including dentistry. The present study aimed to investigate novel antimicrobial applications of copper oxide nanoparticle-based tissue conditioner used in complete prostheses. Methods The present experimental study included 126 tissue conditioner samples with different concentrations of copper oxide nanoparticles (20%, 10%, 5%, 2.5%, 1.25%, 0.625%, and 0% w/w). The samples were incubated with Enterococcus faecalis, Pseudomonas aeruginosa, and Candida albicans in 24-well plates for 24 h. Then, samples from the wells were re-incubated for 24 h, and the microorganisms were counted. Purpose: This systematic review and meta-analysis aimed to evaluate the efficacy of two different loading methods in implant-supported removable prostheses (partial dentures and full-maxillary dentures). Methods: As of August 2023, three electronic databases and nine oral implant-related journals had been searched. Only randomised controlled trials (RCTs) that compare immediate loading with conventional loading in implant-supported removable prostheses were included. The primary outcome was the implant survival rate. At the same time, a meta-analysis of bone-level changes was performed. Cochrane risk-of-bias tool for randomised trials (RoB 2) was used to evaluate the risk of bias in the included trials. Results: A total of 16 RCTs were included, including 543 participants with 1595 implants. The included trials compared immediate loading and conventional loading in implant-supported removable prostheses. Regarding implants as the statistical unit, the immediate loading group had a statistically significant lower survival rate (RR = 0.950; 95% confidence interval [CI], (0.926, 0.974); P = 0.027; I² = 47%). When patients were regarded as statistical units, a statistically significant lower survival rate was also observed in the immediate loading group (RR = 0.929; 95% CI, (0.897, 0.961); P = 0.590; I² = 0%). Background: Oral health is essential to total health and a satisfactory quality of life. One of the most contributing factors is how a human being has an understanding of how to maintain oral health to improve the quality and well-being of life. Purpose: to provide and describe the community knowledge about the importance of replacing missing teeth using dentures and maintaining oral health to improve the quality of life. Methods: The event was carried out with an interactive educational concept by giving pre- and post-tests at the beginning and end of the event. There were at least 80 education participants, consisting of the people of Sekarkare village of various ages, from late adolescents to the elderly, along with village officials and guests from the Sekarkare Health Center. The activity was carried out at the village hall of Sekarkare Village, Dringu District, Probolinggo Regency, East Java. Results: The majority of people understand and are enthusiastic about participating in a series of counseling events. There is an increase in scores in the pre-test (59,06%) and post-test (65,58%) results. Conclusion: The existence of counseling events with interactive educational concepts can increase public enthusiasm for participating in events, making it easier for people to gain new knowledge.

Abstract Background The various physical and chemical conditions within the oral cavity are hypothesized to have a significant influence on the behavior of denture adhesives and therefore the overall comfort of denture wearers. As such, this study aims to understand the influence of oral cavity physiological parameters such as temperature (17 to 52 °C), pH (2, 7, 10), and denture adhesive swelling due to saliva (20–120%) on the behavior of denture adhesives. This study further aims to emphasize the need for a collective approach to modelling the in-situ behavior of denture adhesives. Methods Rheological measurements were carried out using the Super Polygrip Ultra fresh brand denture adhesive cream to evaluate its storage modulus (G´) and loss modulus (G´´) values at a range of physiologically relevant temperatures, pH values, and degrees of swelling, to represent and characterize the wide variety of conditions that occur within the oral cavity. Results Rheological data was recorded with respect to variation of temperature, pH, and swelling. Overall, it can be seen that the physiological conditions of the oral cavity have an influence on the rheological properties of the denture adhesive cream. Specifically, our data indicates that the adhesive’s mechanical properties are weakly influenced by pH, but do change with respect to the temperature in the oral cavity and the swelling rate of the adhesive. Conclusions Our results suggest that the collective inter-play of the parameters pH, temperature and swelling ratio have an influence on the behavior of the denture adhesive. The results clearly highlight the need for developing a multi-parameter viscoelastic material model to understand the collective influence of physiological parameters on the performance of denture adhesives. Multi-parameter models can also potentially be utilized in numerically simulating denture adhesives using finite element simulations.

Background: Denture adhesives are alternatives used to improve retention, stability, comfort and satisfaction in patients with complete dentures. Evidence on the effectiveness of denture adhesives on resorbed mandibular ridges is scarce. Among the many commercially available denture adhesives, the ideal material for the severely resorbed mandibular ridge remains in dispute. Objective: The aim of this study was to evaluate the effectiveness of different quantities of four commercially available denture adhesives on the retention of mandibular complete dentures in severely resorbed ridges. Materials and Methods: A resorbed edentulous mandibular ridge model was manufactured in acrylic resin. A denture base was made and three loops were attached to it. Four commercially available denture adhesives (Fixodent, Perlie White, Fiftydent and Polident) were tested in amounts of 0.2 g, 0.4 g, 0.6 g, 0.8 g and 1.0 g. The acrylic resin model was evenly moistened with 1 ml of water and a weighted amount of adhesive material was applied to the denture base. The universal testing machine engaged the loops fixed on the denture base and applied the vertical displacement force. The maximum vertical displacement force values were recorded for each denture adhesive material at different amounts. Statistical calculation was performed using Kruskal Wallis with Bonferroni post hoc correction. Abstract Denture biofilm acts as a potential reservoir for respiratory pathogens, considerably increasing the risk of lung infections, specifically aspiration pneumonia, mainly 48h after hospital admission. The establishment of a straightforward, affordable, and applicable hygiene protocol in a hospital environment for the effective control of denture biofilm can be particularly useful to prevent respiratory infections or reduce the course of established lung disease. Objectives To evaluate the anti-biofilm effectiveness of denture cleaning protocols in hospitalized patients. Methodology The maxillary complete dentures (MCDs) of 340 hospitalized participants were randomly cleaned once using one of the following 17 protocols (n=20): brushing with distilled water, toothpaste, or neutral liquid soap (controls); immersion in chemical solutions (1% sodium hypochlorite, alkaline peroxide, 0.12% or 2% chlorhexidine digluconate), or microwave irradiation (650 W for 3 min) combined or not with brushing. Background: Banana peels are always thrown away because they are considered items that cannot be eaten or reused, but recent study prove that it contains effective ingredients that can be used as antibacterial and antifungal agent. Kepok banana peel (Musa paradisiaca L) can kill and inhibit certain types of bacteria, including Streptococcus mutans and Candida albicans. Objective: To analyze the effectiveness of banana peel extract (Musa paradisiaca L) as a denture cleanser on the removable denture acrylic plate. Methods: This is an experimental laboratory using 48 denture acrylic plates, size 10x10x2 mm. Kepok banana peels were extracted by maceration technique to concentration of 25%, 50% and 75% and used aquadest as control group. Moreover, 24 acrylic plates in each group that cultured by Candida albicans ATCC (American Type Culture Collection) 10231 dan dan Streptococcus mutans ATCC 25175 were soaked into four solutions (6 plates in each treatment), then total colony was counted by the Total Plate Count (TPC) method. The data was analyzed by computerized by one-way Annova with posttest only-control group design. Results: Reducing the amount of Candida albicans was most effective at a concentration of 75% with an average of 62.33 ± 25.91 (p=0.000), meanwhile the 50% concentration was the most effective on Streptococcus mutans with an average of 0.43 ± 0.41 (p=0.035). Conclusion: Kepok banana peel extract had an good effect on reducing the number of Candida albicans and Streptococcus mutans colonies on the removable denture acrylic plates.

Introduction Vascular endothelial growth factor (VEGF) is signal molecule enrolled in diabetes mellitus type 2 (DM type 2) oral complications, but there are no studies showing the relation between VEGF and pressure caused by denture wearing in diabetic conditions. The aim of this study is to compare tissue VEGF levels in patients and animals with/without DM in conditions of chronic and acute pressure. Methods Research was conducted on DM type 2 and healthy partial denture wearers for more than 5 years (78), candidates for teeth extractions and experimental animals of the Wistar rats (40). For chronic conditions, VEGF was measured in 2 mucosal samples covered and not covered by denture in all denture wearers. Demonstrating acute conditions, after 3 days of wearing experimental plate VEGF was measured in 2 gingival samples of palatal mucosa of DM (20) and control rats (20). The concentrations of VEGF (pg/ml) in human and animal tissues were measured by commercially available ELISA kit. Results Tissue VEGF levels in control and diabetic partial denture wearers not covered by denture were without statistical difference. In comparison to noncovered tissue, VEGF decreased in samples covered by denture, being significantly lower in DM type 2, comparing to healthiest. VEGF levels in palatal mucosa without palatal base did not significantly differ in control and DM rats. VEGF levels under palatal base increased being significantly lower in DM rats comparing to controls. Conclusion Both, chronic and acute mechanical stress caused by wearing palatal denture (plate) decreased the VEGF levels in diabetic conditions comparing the health's suggesting the altered homeostasis.

Oral squamous cell carcinoma is the most common type of oral cancer with multi-factorial causes. Chronic irritation of oral mucosa from ill-fitting dentures has often been mentioned as a contributing factor in the development of oral cancer. There is a unique category of non-smoking non-drinking (NSND) patients reported with carcinoma which usually occurs in women. In contrast to this, our report includes a male patient who is an NSND patient. This case report includes a rare occurrence of carcinoma induced by ill-fitting prosthesis.

Aim: Correct orientation of the occlusal plane plays a vital role in achieving the perfect occlusal balance and function of complete dentures. This study aimed to evaluate the most reliable posterior reference point of the ala-tragus line (ATL) concerning occlusal plane (OP) in a sample of the dentate Sudanese population. Materials and Methods: A total of 150 subjects with healthy and well-aligned permanent teeth were randomly selected. Right lateral profile photographs were taken with subjects having a fox plane placed intra-orally, contacting the occlusal plane. Reference points corresponding to inferior, middle, and superior borders of the tragus and inferior border of the ala of the nose were marked on photographs. The angles between the lines were measured using the Auto-CAD software program, and the most parallel relationship was determined. Descriptive statistics in terms of means and standard deviations were presented. Independent t-test and one-way ANOVA tests were used to compare as appropriate. A p-value < 0.05 was considered significant. Results: The mean angle formed by the OP and ATL was 8.5±3.69º for the superior level, 4.68±3.13º for the middle line, and 2.89±2.57º for the inferior line. A significant difference was found between the means of the three angles (p< 0.001), while no significant difference (p> 0.05) was found between both genders regarding the measured angles. Conclusions: The line joining the inferior border of the ala of the nose with the inferior border of the tragus of the ear was the most reliable line in terms of parallelism to determine the occlusal plane orientation.

Background: Successful denture fitting depends on three key factors: retention, stability, and support. While creating a denture for the upper jaw is relatively easy, the lower jaw presents more challenges due to greater issues with retention and stability. This is because dentures tend to move around relative to the underlying bone during use. To address this problem, a customized tool (an impression and cast) was used in a study to measure the depth of the retromylohyoid fossa. Purpose: The purpose of this study was to analyze the type of tongue and depth of the retromylohyoid fossa using an impression and cast. Materials and Methods: This study was conducted in the department of prosthodontics, where all patients received regular check-ups and dental treatments. The retromylohyoid fossa(extension) and tongue were examined in a sample of 500 patients aged between 20 and 50 years, and impressions were taken. Results: The measurements were recorded, and the depth of the retromylohyoid fossa was assessed. The tongue was also studied. Conclusion: This retromylohyoid fossa and tongue are a suspense itself, and our method is just a suggestion for the mandibular denture prosthesis success.

Background. Chemical agents, in combination with mechanical methods, play an important role in reducing microbial plaque on denture surfaces. However, these methods might change the mechanical behavior of acrylic resins, including microhardness and surface roughness. This in vitro study investigated the effect of two disinfectants, i.e., water and sodium hypochlorite, on the microhardness of conventional heat-cured and TiO2 nanoparticle-reinforced acrylic resins. Methods. Sixty acrylic resin specimens were divided into two groups, and the samples in each group were randomly assigned to three subgroups (n=10). Heat-cured specimens and 1 wt% TiO2 acrylic resin were prepared and immersed in three solutions: water, a solution prepared with NatureDent pills, and 1% sodium hypochlorite for 30, 60, and 90 days. Introduction: Ectodermal dysplasia is a rare genetic disorder that affects structures derived from ectoderm such as teeth, hair, nails, and sweat glands. Oligodontia is a common finding that affects the chewing, smiling and self- esteem of these pediatric patients. Case Report: We present a case report of a 7-years-old pediatric patient who consulted with his mother, showing the same condition. The intervention consisted of a removable prosthesis with satisfactory results. Conclusion: The X-linked hypohidrotic ectodermal dysplasia represents a challenge for a pediatric dentist who offers rehabilitation to the patient according to craniofacial development, age, socioeconomic status, and the number of missing teeth.

Introduction: This study aimed to find out how exposure to gaseous ozone affected heat-cured acrylic resin’s transverse and impact strengths. Methods: Sixty samples of heat-cured acrylic resin were prepared and divided into three subgroups, control, microwave radiation (positive control), and gaseous ozone. Transverse strength and impact strength were evaluated using testing machines. At a level of significance of 5%, data were assessed using one-way analysis of variance (ANOVA) and Tukey’s post hoc test. Results: Transverse strength analysis showed a significant difference among groups (P < 0.001). Post hoc test revealed significant differences between all multiple groups (P < 0.001). Impact strength analysis results showed a non-significant difference among groups (P = 0.13). Conclusion: Within the limits of this research, it is possible to conclude that gaseous ozone exposure improves the transverse strength of heat-cure acrylic resin.

Palatal fistula is a major complication following cleft palate repair and in combination with premature primary teeth loss, compromises a child's mastication, nutrition, speech, and aesthetics. Other major symptoms comprise hyper nasality of voice and nasal regurgitation. Therefore, rehabilitation of the palatal defect and the partially edentulous space needs to be performed until the surgical repair of the palatal fistula is achieved. This case report describes the oral prosthetic rehabilitation of a 4-year-old patient with a palatal fistula and prematurely lost primary incisors utilizing a removable partial denture obturator.

Tooth loss may affect food ingestion and, consequently, nutrition intake. The neuroimaging literature using functional magnetic resonance imaging (fMRI) was reviewed to summarize the changes in brain functions in response to denture rehabilitation in patients with partial or complete edentulous dentition. Overall, this review covered nine fMRI studies on denture rehabilitation. Eight recruited complete edentulous patients, whereas one recruited partially edentulous patients. The risk-of-bias assessment revealed concerns regarding all nine studies. Due to the heterogeneity of the studies and the lack of brain coordinates reported, a meta-analysis could not be conducted, and this review could only summarize the findings without statistical validation. The evidence from jaw-clenching studies suggested that implant-supported fixed dentures could be the best option, as compared to implant-supported overdentures and complete dentures, as it was associated with higher brain activity levels in various brain regions, including those corresponding to the primary sensory (postcentral gyrus) and motor cortices (precentral gyrus). Gum-chewing studies indicated that perhaps the medial and middle frontal gyri were associated with food comminuting and food mixing, which could be improved by the full replacement of the dental arch, instead of only partial replacement. All the fMRI studies described the functional neuroplasticity of the patients undergoing denture rehabilitation and suggested that certain rehabilitation options were more beneficial in restoring masticatory functions, as well as their associated brain activity levels. Significant changes in the microstructure of the brain cause dementia and other mental declines associated with aging and disease. Although research has established a connection between oral health and dementia, the underlying pathologic mechanisms are still unknown. Aim: Our aim was to evaluate dentures' impact on the cognitive state of geriatric patients. Material and methods: A total of 108 individuals seeking treatment at the Faculty of Dental Medicine in Iasi, Romania, participated in the study, which ran from May 2022 to October 2022. Cognitive dysfunction was assessed using the Mini-Mental State Examination. The acquired data were analyzed with IBM SPSS 26.0, and the p-value was set at 0.05. Results: The average value of the MMSE score was 21.81 ± 3.872. Differences between groups of wearer/non-wearer subjects were statistically significant for most of the questions in the questionnaire. Linear regression analysis showed that individuals with a high MMSE score have prosthodontic treatment. A decrease in the MMSE score is associated with a decrease in masticatory efficiency (B = 1.513, p = 0.268). Conclusions: This study provides further evidence that tooth loss is associated with worse cognitive performance. It is thus conceivable that the necessary effects can be achieved by increasing the efforts dedicated to preventing tooth loss in the adult population. [ABSTRACT FROM AUTHOR]

Introduction: Tea consumption causes extrinsic stain formation on teeth and denture surfaces, affecting personal appearance. Herbal plants extract such as green tea, bay leaf, and lime peel are potentially active agents for extrinsic stain removal. This study aimed to analyse the effectiveness of green tea, bay leaf, and lime peel extracts as toothpaste active agents for extrinsic stain removal on teeth, artificial teeth, and denture base. Methods: Twenty-five post-extracted human permanent central incisors, 25 artificial teeth, and 25 heat polymerised acrylic resin denture base (20x20x10mm) were divided into three treatment groups and two control groups. Pictures of all specimens were taken before and after stain induction and after brushing. All specimens were immersed in a tea solution made from 2 grams of tea and 100ml of 100°C boiled water to induce extrinsic stain. The immersion process was repeated every 24 hours for seven days. All specimens were then brushed using an automatic toothbrushing machine for 70 seconds on each surface, 200 grams loads, and five speeds/second. Toothpaste used for brushing were three different active agent toothpaste (treatment groups), no active agent toothpaste, and commercial whitening (control groups). Colour index analysis of the specimen pictures computerised by CIELAB method. The data were analysed by one-way ANOVA (α=0.05). Results: Green tea, bay leaf, and lime peel extract toothpaste and positive control had higher L-values of extrinsic stain removal than essential toothpaste on all specimens. There was a significant difference between green tea, bay leaf, and lime peel extract toothpaste and negative control toothpaste (p0.05). Conclusion: Green tea, bay leaf, and lime peel extract toothpaste are effective as extrinsic stain removal on teeth and acrylic resin denture base because of abrasive materials that work together with the active agents such as epigallocatechin-3-gallate (EGCG) in green tea, eugenol in bay leaf, and naringin in lime peel.

Abstract Background Previous research indicates that patients with mild cognitive impairment (MCI) are more likely to have poor oral health and impairments in oral functions, which may be due to few remaining teeth and impaired tongue and lip motor function. However, the oral health of those patients following comprehensive cognitive assessment by a dementia specialist has not been sufficiently investigated. Therefore, this study aimed to clarify the oral function of patients with MCI and the association between oral health and lower cognitive function. Methods This cross-sectional study included 96 participants (men: 35; women: 61; mean age: 73.3 ± 8.5 years) who visited a dementia clinic between December 2017 and January 2020. Participants’ cognitive function was assessed by a dementia specialist using neuropsychological and hematological tests and neuroimaging immediately after enrollment. The participants were divided into the healthy and MCI groups according to comprehensive cognitive assessment. Participants’ age, sex, body mass index, primary disease, education level, drinking habits, smoking habits, living environment, employment status, and exercise habits were evaluated. Moreover, oral outcomes, including the number of existing teeth, number of functional teeth (natural and prosthetic teeth which were occluded with antagonists), denture use, oral dryness, tongue and lip motor function, tongue pressure, occlusal force, masticatory ability, and swallowing ability were recorded. The Mann–Whitney U test, χ2, and Fisher’s exact tests were used for between-group comparisons. Furthermore, logistic regression analysis using MCI diagnosis as the target variable was performed. Results A comprehensive evaluation of the cognitive function of the study participants by the dementia specialist revealed that 48 participants (mean age: 69.8 ± 8.8 years) were healthy and 48 (mean age: 76.9 ± 6.7 years) had MCI. The mylohyoid muscle, one of the suprahyoid group, forms the floor of the mouth. Its main function is swallowing. It is a margin between the sublingual and the submandibular spaces and is important in the pathway of oral and maxillofacial infection. In prosthodontics, it is one of anatomic landmarks that limits the lingual margin of the mandibular denture. Currently, the muscle receives much interest in the fields of maxillofacial reconstruction and rejuvenation. The hemorrhagic issue around the mandibular lingual region is usually involved with the mylohyoid especially in the dental implant installation. Introduction: The objective of a complete denture prosthesis is restoring aesthetics, comfort, and function by the replacement of missing dental and alveolar structures employing a stable prosthesis. Case Report: Many conditions can complicate the treatment plan and fabrication of a complete denture prosthesis. Complete denture fabrication in clinically compromised conditions is a challenging task for the dentist. In this clinical report, we present comprehensive management of a patient with denture-induced hyperplasia, flabby ridge, and severely resorbed edentulous ridge. The three-part strategy for management of the above-mentioned challenges can provide high-quality complete dentures, based on recognized prosthodontic principles. This first part will discuss the management of denture induced hyperplasia by elimination of the inflammation and excision of the lesion. Part two will cover management of the flabby ridge using a modified window technique for the impression of maxillary flabby tissues for an improved and controlled application of the impression material that is usually obtainable in dental practice. Part three highlights the rehabilitation procedure of the resorbed mandibular ridge using a functional impression technique with minimum soft tissue displacement and neutral zone arrangement of teeth to improve stability of the denture. Conclusion: rehabilitation of a patient with denture induced hyperplasia, flabby ridge, and severely resorbed edentulous ridges was successful.

Microstomia refers to any significant decrease in the size of the oral cavity. It can cause difficulty in the fabrication of dentures, and the result of this situation shows a significant decline in quality of life. This study describes a treatment method for preparing a complete denture in an edentulous patient with microstomia due to burns. The present study decided to employ a sectional tray and single unit denture considering the hygienic issue and prolonged durability. The major advantage of this scheme is reducing food impaction between different parts. Eventually, the patient was noticeably satisfied, and aesthetic and functional demands were provided.

Statement of problemIn patients with an altered skeletal maxillomandibular relationship and bone resorption, the rehabilitation of edentulous jaws by combining 4 implants, 2 straight medially and 2 tilted distally, may be preferred to avoid a bone augmentation procedure.PurposeThe purpose of this single cohort 1-year prospective study was to evaluate the clinical performance of a 4-implant overdenture fully supported by a computer-aided designed and computer-aided manufactured (CAD-CAM) titanium bar.Material and methodsThis single cohort prospective study included edentulous participants rehabilitated with a 4-implant overdenture in 1 of the 2 jaws. The outcomes were implant and prosthetic survival and success rates, any biologic and technical complications, periimplant marginal bone loss, changes in the oral health impact profile (OHIP), bleeding on probing, and the plaque index.ResultsEighteen participants received 72 implants. One year after implant placement, no implants or prosthesis had failed, and no biologic or technical complications had been observed. At the 1-year follow-up, the mean marginal bone loss was 0.29 ±0.16 mm. The OHIP summary scores demonstrated a significant improvement in oral health-related quality of life. At the 1-year follow-up, positive bleeding was found in 2 participants (11.1%) around 3 implants (4.1%). Three participants (16.6%), accounting for 5 implants (6.9%), showed a slight amount of plaque.ConclusionsA 4-implant overdenture supported by a CAD-CAM titanium bar may be a reliable option for the treatment of the edentulous mandible and maxilla over a 1-year period. Oral health-related quality of life significantly improved in all treated participants.

Background: Edentulism affects the chewing ability of a person and can engender nutritional deficiencies which can affect the overall quality of life and depression. Aim: Our aim was to evaluate the association between perceived chewing ability, oral health-related quality of life (OHRQoL) and depressive symptoms among completely edentulous patients with and without dentures. Settings and Design: Institutional study and cross-sectional design. Methods: A cross-sectional study among 207 completely edentulous persons was conducted in South India. The subjects completed a self-administered questionnaire on demographics, Beck's depression inventory, oral health impact profile-edentulous (OHIP-EDENT), age when edentate, chewing ability, and denture satisfaction. Further, denture status was clinically evaluated. Statistical Analysis: Data were summarised and analysed using the Chi-square test and multivariate logistic regression. P < 0.05 was considered significant. Results: Persons not using complete dentures (odds ratio [OR] =3.5, P < 0.05), who reported impaired chewing ability (OR = 4.6, P < 0.05), those who became edentate before 55 years (OR = 4.6, P < 0.05) and with poor denture status (OR = 6.2, P < 0.05) were more likely to report depressive symptoms. Lesser impact in relation to OHRQoL was found to be protective against depression among completely edentulous (OR = 0.24, P < 0.05). Conclusion: Depressive symptoms were associated with impaired chewing ability, higher impacts on OHIP-EDENT, and edentulous persons not using complete dentures. High priority must be given to enhance awareness towards oral rehabilitation among completely edentulous to reduce the chance of depression occurring due to impaired chewing ability and poor OHRQoL.

Denture-induced fibrous hyperplasia or epulis fissuratum refers to reactive tissue alteration occurring in response to trauma and chronic irritation due to the badly fitted prosthesis. Pathologic changes observed in the oral mucosa may vary depending on the severity of trauma to the mucosa. Trauma to minor salivary glands caused by the impingement of denture flanges can result in chronic sialadenitis. In this article, we discuss a case of denture-induced fibrous hyperplasia with prominent minor salivary gland changes in a Male patient of age 63 years.

Background and Aims: Different materials have been used to make dentures during different years. Teeth used in making dentures should have appropriate physical and mechanical features to meet the needs of masticatory functions, meanwhile having desired beauty. The aim of the present study was to determine the frequency of previous denture material and f its form and the duration of use in patients referring to the prosthesis department of Tabriz Dental School during 1385 to 1395. Materials and Methods: This cross-sectional descriptive study was performed on the basis of paraclinical examination on all cases of removable prostheses in patients referring to the department of prosthodontics of Tabriz University of Medical Sciences from 1385 to 1395. Based on the files, the percentage of frequency of previous denture type, previous denture tooth material, previous tooth form, previous time of denture using during the day and night were statistically evaluated by SPSS20 software. Results: The results of this study showed that out of 471 patients, 151 people used removable dentures. A study of previous denture material showed that the frequency of using dentures containing resin teeth with 48.3% was much higher than porcelain teeth with 13.8. This article reported the use of a cobalt-chromium-based removable partial denture to replace a missing maxillary lateral incisor and molars. Palatal retentive arms were employed as the retentive components on the premolars to avoid the metal component being seen when speaking and smiling. Due to the constant search for a harmonic smile, the level of demand and the expectations of patients become high, enabling the development of new materials and dental techniques that aim at more conservative procedures and more and more predictable results aesthetically. Techniques with Computer Aided Design and Computer Aided Manufacturing (CAD / CAM) have occupied a prominent place in Dentistry. This is due to the fact that computer aided design or computer aided manufacturing offers a restoration method that lowers the risk of human error and provides highly aesthetic results in a reduced clinical time, albeit at relatively high costs. The objective of the present study is to present, through a clinical case report, an aesthetic rehabilitation through the practical application of the CAD / CAM system. With the completion of the case, it was possible to observe aesthetic improvement through the preparation of the anterior ceramic crowns and improvement of the occlusal condition with the manufacture of a removable upper prosthesis, which enables greater durability of the ceramic crowns, as it better distributes the occlusal loads among the elements, avoiding excessive loads at the site of new indirect restorations. This case report was carried out in two dental sessions, achieving patient aesthetic satisfaction. It has a short-term follow-up, therefore, cases with long-term follow-up are necessary to prove the success of this technique. The high technological cost can be considered a limitation, but there is a notable advance in Dentistry to improve the quality of treatment in extremely short periods.

Abstract Background The associations between the number of natural teeth/denture use and all-cause mortality remain unclear due to lake of investigation for the potential interaction between tooth loss and denture use and for the potential changes in these exposures over time in older adults. We undertake this study to evaluate the associations of the number of natural teeth and/or denture use with mortality in Chinese elderly. Methods This is a prospective cohort study of 36,283 older adults (median age: 90). The number of natural teeth and denture use were collected with structured questionnaire. We evaluated hazard ratios (HRs) and confidence intervals (CIs) using a Cox proportional hazards model adjusting for demographic factors, education, income, lifestyle factors, and comorbidities. Results We documented 25,857 deaths during 145,947 person-years of observation. Compared to those with 20+ teeth, tooth loss was associated with a gradual increase in mortality, with an adjusted HR of 1.14 (95% CI, 1.06 to 1.23) for those with 10–19 teeth, 1.23 (95% CI, 1.15 to 1.31) for those with 1–9 teeth, and 1.35 (95% CI, 1.26 to 1.44) for those without natural teeth. Denture use was associated with lower risk of mortality (adjusted HR 0.81; 95% CI, 0.77 to 0.84). The frequency of the decay of the ceramic materials used was Vita-95 and Noritake, and methods for their recovery have been proposed. In 119 patients with sintered prosthesis, 63 bridges were examined, 34 of which were made of Noritake and 29 from Vita-95. Of the 56 metal-ceramic crowns, 30 were manufactured using Noritake and 26 crowns using Vita-95. The disintegration of prostheses made from the mass of Noritake and Vita-95 was noted in 53,8 ± 6,2% and 46,2 ± 6,7% of cases, respectively. In 19,3 ± 8,2% of cases, ceramic decays were observed in molars and premolars in the chewing plane of the mandible. At Vita-95 after firing of an opaque ceramic layer, microcracks were detected in 8,6 ± 1,4% of cases, when using Noritakе material - in 13,3 ± 1,7% of cases, which is 1,5 times more often (p < 0, 05). According to the comparative analysis, the ceramic material Vita-95 (Germany) was more effective than the ceramic mass of Noritakе (Japan). To restore the prosthesis proposed two options: restoration by laboratory methods and directly in the oral cavity.

The ageing of population is increasing, and a great percentage of these patients wear removable prostheses, and can suffer denture stomatitis, a condition that has been associated with candidiasis. Aims: To evaluate in vitro the effectiveness of Copper Sulfate against Candida albicans in samples of heat-polymerized acrylic resin, compared to nystatin, sodium hypochlorite and chlorhexidine. Materials and Methods: Initially, the minimum inhibitory concentration (MIC) of copper sulfate for Candida albicans was determined by microdilution. Then, 54 resin samples were divided into 6 treatment groups corresponding to Nystatin 100.000 UI, Sodium Hypochlorite 0.5%, chlorhexidine 0.12%, Copper Sulfate 4.7µg/ml, Copper Sulfate 9.4µg/ml and physiological saline solution, in which samples were submerged for 6 hours. Resin samples were then washed and cultured on solid media at 37°C for 72 hours. The number of colony-forming units was determined using a Quebec colony counter. The statistical analysis was performed using the Kruskal-Wallis test and the Mann-Whitney U test. Results: Copper sulfate at a concentration of 9.4μg/ml presented a similar effectiveness as the other control products regarding the reduction in the number of colonies of Candida albicans post-treatment. Conclusion: The effectiveness of copper sulfate against Candida albicans on acrylic resin was similar to that of nystatin, sodium hypochlorite and chlorhexidine.

A total of 383 implants (303 in the maxilla; 80 in the mandible), supporting 81 full or partial fixed dental prostheses (65 in the maxilla; 16 in the mandible), were either cement- or screw-retained. Three years after loading, a total of 219 pontic sites (153 in the maxilla; 66 in the mandible) were measured, and the thickness of the alveolar ridge mucosa between the prosthetic surface and the underlying bone crest were recorded.ResultsThe overall implant and prosthesis survival rates at 3 years were 98.7% and 100%, respectively. No implant complications were reported, scoring a cumulative implant success rate of 100%. In the maxilla, the overall mean thickness of the alveolar ridge mucosa was 2.32 ± 0.57 mm. In the mandible, the overall mean thickness of the alveolar ridge mucosa was 2.20 ± 0.62 mm. There was no statistical difference between the overall mean values in the maxilla and mandible (p = .471).ConclusionThis radiologic retrospective study suggests the existence of a physiological barrier, named prosthetic biological width, underneath a novel pontic-designed restoration.

Since the introduction of the endosseous concept to North America in 1982, there have been new permutations of the original ad modum Branemark design to meet the unique demands of treating the edentulous maxilla with an implant restoration. While there is a growing body of clinical evidence to assist the student, faculty, and private practitioner in the algorithms for design selection, confusion persists because of difficulty in assessing the external and internal validity of the relevant studies. The purpose of this article is to review clinician- and patient-mediated factors for implant restoration of the edentulous maxilla in light of the hierarchical level of available evidence, with the aim of elucidating the benefit/risk calculus of various treatment modalities.

ObjectiveSince dentures can serve as a reservoir for halitosis-causing oral bacteria, halitosis development is a concern for denture wearers. In this study, we surveyed the prevalence of four selected halitosis-related species (Fusobacterium nucleatum, Tannerella forsythia, Veillonella atypica and Klebsiella pneumoniae) in clinical denture plaque samples, and developed denture biofilm models for these species in vitro to facilitate assessment of antimicrobial treatment efficacy. Design : Denture plaque from ten healthy and ten denture stomatitis patients was screened for the presence of aforementioned four species by PCR. Biofilm formation by these halitosis-associated species on the surfaces of denture base resin (DBR) discs was evaluated by crystal violet staining and confocal laser scanning microscopy. The efficacy of denture cleanser treatment on these mono-species biofilms was evaluated by colony counting. Results : 80% of the subjects in the denture stomatitis group and 60% in the healthy group contained at least one of the targeted halitosis-related species in their denture plaque. All halitosis species tested were able to form biofilms on DBR disc surfaces to varying degrees. These in vitro mono-species resin biofilm models were used to evaluate the efficacy of denture cleansers, which exhibited differential efficacies. When forming biofilms on resin surfaces, the halitosis-related species displayed enhanced resistance to denture cleansers compared with their planktonic counterparts. Conclusion : The four selected halitosis-related bacterial species examined in this study are present on the majority of dentures. The mono-species biofilm models established on DBR discs for these species are an efficient screening tool for dental product evaluation.

Immediately loaded implant prostheses have been used to successfully rehabilitate completely edentulous arches. Risk factors for successful treatment have not included involuntary mandibular movements. The treatment was completed on a patient with a history of neuroleptic medications who had remaining mandibular teeth extracted and then developed involuntary mandibular movements. The patient was dissatisfied with a mandibular removable prosthesis and wanted a fixed prosthesis. The immediate implant loading of a complete arch fixed prosthesis was delivered, and the patient lost 3 of the 6 implants. The patient continued to have problems with her definitive prostheses as the symptoms of her involuntary mandibular movements worsened.

Introduction: Immune Thrombocytopenia (ITP)is characterized by its autoimmune response to platelet degradation. The manifestations in symptomatic patients include bruising and multiple bleeding caused by a decrease in the number of platelets. Case report: This case report presented a 75-year-old male patient referred to the Department of Oral and Maxillofacial Diseases affiliated to the School of Dentistry, Mashhad University of Medical Science, Mashhad, Iran, with a chief complaint of ill-fitting dentures in the last 10 days. The oral examination confirmed the presence of purpura and ecchymosis on the palate and two hemorrhagic bullae on the right mandibular ridge. Moreover, bruisesand red macules were observed on his legs during the clinical examination. The patient was prescribed to receive an emergency blood test. He was referred to the hospital after the diagnosis of severe thrombocytopenia and the possible diagnosis of ITP or leukemia. Eventually, the diagnosis of ITP was confirmed, and he was subjected to a high dose corticosteroid therapy and chemotherapy with intravenous rituximab. The patient recovered fully after two follow-ups within one year. Conclusion: It is of utmost importance to do oral examinations in patients who have ill-fitting and old dentures since theseelderly patients can be more prone to different blood dyscrasias. In some cases, oral changes are among the first manifestations of systemic diseases. Early diagnosis has a dramatic effect on the survival rate of the patients along with early referral for the treatment.

A group of 210 patients with total maxillary and mandibular prostheses were examined to identify the main pathological alterations suffered by edentulous patients. The observed results determined that the average age at which the largest edentulous population is found is between 56 and 65 years with a higher prevalence of females. Of these more than 50% have used oral prostheses for more than 12 years with an adequate hygiene quality in 116 patients where the most frequent oral pathology associated with edentulism was fibrous hyperplasia (epulis), found in 19 patients, followed by stomatitis sub-prosthesis.

The hybrid prosthesis is a very predictable treatment option that allows patients to recover their oral functionality. It is a good alternative to conventional treatments and overdentures. The present report describes controlled an 80 year old female patient with controlled hypertension, who came for consultation with a bar overdenture with attachment holders on two external hexagonal dental implants; an immediate loading hybrid prosthesis was installed over four mandibular dental implants.

Objectives: This study aimed to evaluate the in vitro antifungal efficacy of addition of silver nanoparticles (SNPs) to Mucopren® silicone soft liner material. Materials and Methods: Twenty disc samples (8 × 2 mm) of Mucopren® silicone soft liner containing 0wt% (control), 0.5wt%, 1wt%, 2wt%, and 3wt% SNPs were fabricated. Samples were powdered and added to 150 mL of Sabouraud dextrose agar culture medium and placed on separate culture dish plates. Each plate was inoculated with 106 colony forming units per milliliter (CFUs/mL) of Candida albicans (PTCC 5027) according to the CLSI protocol, and incubated at 37℃. The colony count was verified at 24 h, and the antifungal effect of the samples was evaluated according to the percentage of viable cells in the 2 subgroups with/without thermocycling. Aim: This study aims to evaluate and compare the effect of plant extract (thyme essential oil solution) and commercially available denture cleanser on the flexural strength and surface roughness of denture base resin. Settings and Design: Comparative In-vitro study. Chemical denture cleansers play a vital role in maintaining the hygiene and serviceability of the dentures. Bacterial resistance to these chemical agents paved way to plant-extracts as novel denture cleansing agents. However, the effect of these plant-extract denture cleansers on the physical and surface characteristics of denture base resins has not been evaluated. Material and Methods: A total of 90 heat polymerizing denture base material (Trevalon, Dentsply) samples were fabricated and divided into 3 groups with 30 samples each. Samples from each group were immersed in their respective denture cleanser solution (Group A- Distilled water(control); Group B- Fittydent denture cleanser; Group C- Thyme essential oil solution denture cleanser) for a simulated overnight 8hr immersion for 180 days. The samples were evaluated for increase in surface roughness and flexural strength using Tally-surf Surface Profiler and Instron Universal Testing Machine respectively. Results obtained were statistically analyzed using one-way ANOVA. Statistical Analysis Used: Oneway ANOVA , Post hoc Tukey's test. Results: Thyme essential oil solution group showed minimal increase in surface roughness (ΔRa) with values comparable to that of the control group which had the least increase in surface roughness and Fittydent group showed significant increase (P < 0.05) in surface roughness. For flexural strength, statistically significant difference (P < 0.05) was observed among the three groups with Fittydent group showing the highest flexural strength followed by control group and Thyme essential oil solution group. However, the decrease in the flexural strength was not of clinical significance. Conclusion: Plant extract - thyme essential oil denture cleanser was superior in preserving the surface roughness of denture base resins compared to commercially available denture cleanser. Clinically significant difference in flexural strength was not observed between the denture cleanser groups.

Given the recent demographic trends toward a marked increase of the elderly populations – as evidenced globally and also in Romania, an awareness regarding the specific needs of this age group is required regarding both the general health and the prosthodontic treatment. Method. A literature review of studies published after the year 2000 was performed, regarding the association between general and oral health among the elderly, as well as the association with the prosthodontic treatment and quality of life. Results and discussion. Specific elderly groups were first defined: young elderly (60-69 y.o.), middle-aged elderly (70-79 y.o.), and oldest elderly (over 80 y.o.) in order to provide a more clear picture of the transformations occurring among elderly patients. Multiple morbidities and multiple prescription drugs are often associated to oral health symptoms (e.g., the “dry mouth” or “burning mouth” syndromes), to general conditions such as frailty, disability, cognitive impairment, and also to more severe oral health diagnoses. Among the elderly, there is a strong correlation between the general and oral health, such as the one between periodontal disease and systemic inflammatory processes – which are moderated by age and socio-economic status. Thus, edentulation and removable dentures (conventional or implant-sustained) have an impact not only upon oral but also upon systemic health, as well as the overall quality of life. Conclusions. Given the recent trends toward an increase of the elderly populations, a systemic perspective is absolutely necessary for approaching a prosthetic treatment. The type and quality of dentures, as well as their acceptance by the elderly patient are closely related to the overall and oral health, the social-economic status, and the quality of life of the elderly patient.

Background/Aim: The aim of this study was to examine prevalence of different Candida spp. at diabetics and nondiabetics wearing dentures without clinical signs of Denture Stomatitis (DS) and to study if some local and systematic factors are confounders for harboring Candida at these subjects. Material and Methods: Total of 60 subjects wearing partial or complete upper acrylic denture having at least half of palatal mucosa covered by denture were selected and stratified into three experimental groups: systematically health subjects; patients with diagnosed Type 2 Diabetes (T2D) and good glycoregulation; and T2D subjects with poorly regulated blood sugar level. Cotton swab samples were obtained from each patient from hard palate mucosa and denture surface. Swab cultures were made on Sabouraud dextrose agar and ChromAgar Media for distinciton of various Candida spp. Density growth was also measured. Results: Frequency of Candida spp. findings were similar between groups. At healthy subjects, only C.albicans was detected. At diabetics, C.albicans was the most common isolated species, followed by C.glabrata and C.tropicalis. Negative finding of yeasts on palatal mucosa, but positive on denture surface were detected at all groups, with the highest frequency (33.4%) at diabetics with poor glycoregulation. Denture surface was heavier colonized than hard palate mucosa. Duration of diabetes in years were only independent predictors for harboring Candida spp. at denture surface (Exp B=1.186, CI=1.047-1.344, p=0.007). Conclusions: Prosthesis of denture wearers without DS may serve as reservoir of Candida spp. Presence of more pathogenic and resistant non-albicans species are related to diabetics, even without clinical signs of DS.
