Purpose: To investigate the esthetic outcomes based on the color differences in zirconia (Zr) of varying thickness, resin cement color, and types of titanium (Ti) implant surface treatments. Materials and Methods: Overall, 28 high-translucency monolithic zirconia (HTMZ) specimens were arranged into four groups based on Zr thickness: 1.0, 1.5, 2.0, and 2.5 mm (n = 7 per thickness). Each group was tested using two resin cement colors (clear and opaque) in combination with six surface-treated Ti groups (n = 7), including untreated titanium (UT), anodization (AN), 50-µm alumina airborne-particle abrasion followed by AN (SBAN), AN followed by 50-µm alumina airborne-particle abrasion (ANSB), 9.5% hydrofluoric acid followed by AN (HFAN), and AN followed by 9.5% hydrofluoric acid (ANHF). This created a total of 48 experimental groups, including the use of composite resin (n = 7, shade A2D) for four control groups. All specimens were measured using a spectrophotometer and subsequently compared with composite resin (control) with the corresponding Zr thickness to establish color differences. A color difference of < 2.7 was considered clinically acceptable. The data obtained were statistically analyzed using ANOVA and post hoc test (P = .05). Results: Zr thickness, resin cement color, and type of Ti implant surface treatment significantly affected the observed color differences (P < .05). When using 2.5-mm HTMZ with clear resin cement on AN, UT, SBAN, HFAN, and ANSB, the mean color differences were below the clinically acceptable values, and the 95% CIs of color differences were below the clinically acceptable values for AN, UT, and SBAN groups. Conclusions: HTMZ with a minimum thickness of 2.5 mm and clear resin cement on AN, UT, and SBAN groups potentially result in acceptable color matching with 95% CIs. [ABSTRACT FROM AUTHOR]

Purpose: To compare the effect of taper, length, angle, and number of vertical axial walls on the retentive strength of a cemented crown on a partially customized hybrid abutment in the esthetic zone. Materials and Methods: A total of 35 metal copings were used and divided into two groups. One group had 30 copings cemented to their corresponding 8-degree tapered abutment with lengths from 3 to 8 mm, increasing in 1-mm increments (5 copings per length). The other group had the remaining 5 copings and consisted of a standardized metal coping that matched a hexagonal abutment with 3-mm vertical axial walls and the sequential removal of 1, 2, and 3 contiguous vertical axial walls. Dislodgment tests were performed for all copings in both groups. Maximum retentive forces were measured in retentive strength (kgF), with a conversion factor of 9.807 N = 1 kgF. Results: At each tapered abutment length, the retentive strength increased proportionally and was significantly different, ranging from 31.67 ± 4.10 kgF to 67.68 ± 11.22 kgF (F [5,24] = 20.46, P < .001). An unmodified hexagonal abutment demonstrated the highest retentive strength (70.15 ± 12.97 kgF). The sequential removal of 1, 2, and 3 contiguous vertical axial walls of the hexagonal abutment resulted in retentive strength values of 59.89 ± 10.06 kgF, 57.01 ± 9.62 kgF, and 55.99 ± 9.35 kgF, respectively, with no significant difference (P > .05) in strength. Conclusions: A partially customized abutment with vertical axial walls on one side and a profile reduction on the opposite side can provide comparable retention to cemented copings at one-third the length and at one-sixth the surface area of an 8-mm abutment with an 8-degree taper. Introduction: The esthetics of immediate implant dentistry can be challenging. Clinical considerations for immediate implant placement have demonstrated long‐term success rates comparable to traditional delayed implant protocols. However, it is a sensitive technique that requires proper treatment planning as well as meticulous execution to be predictable and successful in the long term. Methods: This clinical situation demands extensive knowledge of soft and hard tissue management for atraumatic tooth extraction followed by implant therapy, along with an understanding of the available materials to meet esthetic needs. This case report features a female patient with the chief complaint of needing to replace an anterior crown due to loss of retention. After a comprehensive oral assessment and cone beam computed tomography (CBCT) radiographic examination, it was determined that the crown on Tooth #9 was fractured at the subgingival level. Furthermore, gingival zenith positions displayed differences at the keratinized mucosa level in Teeth #7, #8, and #9, and the metal marginal areas of the porcelain‐fused‐to‐metal (PFM) crowns in the anterior area of Teeth #5, #7, #8, #9, and #10 were apparent. The procedure involved atraumatic extraction of Tooth #9, followed by immediate implant placement. Crowns on Teeth #5, #7, #8, and #10 were replaced, and veneers on Teeth #6 and #11 were fabricated using press lithium disilicate‐reinforced ceramic. Connective tissue graft (CTG) was contoured before final implant restorations. Results: The final implant crown was restored using a prefabricated abutment with a titanium base and lithium disilicate ceramic dental material. A well‐planned combined treatment, including atraumatic tooth extractions for immediate implants and ideal contouring of soft tissues, can significantly impact the outcome of esthetic restorations. Background: Children with dental caries are treated with stainless steel metal crowns (SSC), but the aesthetics and precision still need to be improved. Currently, both 3D‐printed resin crowns (PRC) and computer‐aided design/computer‐aided manufacture (CAD/CAM) resin crowns (CRC) meet the clinical requirements for crown applications in terms of strength, production time, cost, and aesthetics. Aim: This study replaced SSC with customized resin crowns by 3D printing and CAD/CAM. Design: In this study, PRC, CRC, and SSC were used for incisor and molar restorations, and 60 crowns were made with 10 for each group. The fabrication efficiency, surface characteristics, marginal fit, and stability of the two different crowns were evaluated. Results: PRC and CRC show superior color and surface characteristics, though production times are longer (5.3–12.4 times and 3.3–9.1 times, respectively) than for SSC (p <.05). They, however, can be completed within 80 min. Edge gaps for PRC and CRC are significantly lower (13.0–19.2 times and 13.0–13.7 times) than for SSC (p <.05). All materials exhibit good stability. Objective: Esthetic dentistry is an important factor in increasing patients' quality of life. This study aimed to investigate the impact of laser use on bleaching procedures for natural teeth and dental restorative materials. Methods: In January 2024, an electronic search was conducted using PubMed, Web of Science (WoS), and Scopus databases with the keywords (tooth) AND (laser) AND (bleaching), following PRISMA guidelines and the PICO framework. The initial search yielded 852 articles, of which 441 were screened. After applying inclusion criteria, 376 articles were excluded as they did not focus on the use of lasers in bleaching natural teeth and restorative materials. Consequently, 40 articles were included in the final review. Results: Of the 40 qualified publications, 29 utilized a diode laser, of which 10 authors concluded that it increases the whitening effect comparing classical methods. Three of included publications investigated the whitening of dental materials, while another three focused on endodontically treated teeth. Whitening procedures on ceramics effectively removed discoloration, but the resulting color did not significantly differ from the initial shade. Conversely, composite materials not only failed to bleach but also exhibited altered physical properties, thereby increasing their susceptibility to further discoloration. The KTP laser demonstrated promising outcomes on specific stains. The Er,Cr:YSGG and Er:YAG lasers also showed beneficial effects, although there were variations in their efficacy and required activation times. Conclusions: The findings partially indicate that laser-assisted bleaching improves the whitening of natural teeth. Objective: This article describes a novel 3D‐printed template armed with interproximal matrices to isolate interproximal contact areas and guide injectable resin composite for consecutive closure of multiple diastema. Clinical Considerations: Among several treatment options proposed for diastema closure, direct resin composite is noninvasive and easy to repair. The "composite injection technique" has been introduced to improve time efficiency and reduce technique sensitivity for clinicians. However, in the case of multiple diastema, the overflow of excess resin materials onto the adjacent teeth during injection poses challenges for recontouring the interproximal anatomy. A 3D‐printed template with special‐designed gaps at interproximal areas was designed and fabricated based on a virtual diagnostic wax‐up. Flowable resin composite was then consecutively injected through the template to close diastemata at multiple adjacent teeth. Conclusion: This technique using a 3D‐printed template with interproximal isolation design contributed to an efficient and accurate operation for multiple anterior diastema closure. Clinical Significance: Efficient and accurate freehand buildups of composite restoration for multiple diastema are challenging in operative dentistry. The study aimed to investigate the influence of H2O2‐based and H2O2‐free in‐office bleaching on the surface and mechanical attributes of CAD/CAM composite blocks. CAD/CAM composite blocks from five different composite materials (CC1, CC2, CC3, CC4, and CC5) were randomly divided into two groups according to bleaching application (H2O2‐based and H2O2‐free). The surface topography, morphology, nanohardness, elastic modulus, flexural strength, and fracture toughness were measured. A paired and unpaired sample t‐tests gauged the effect of pre‐ and post‐bleaching on the substrates. The estimated mean differences (before‐after bleaching) suggested an increase in surface roughness for two materials CC2 and CC4, and a significant decrease in nanohardness for material CC4 and in elastic modulus for materials CC2 and CC4 with H2O2‐based bleaching, whereas H2O2‐free bleaching resulted in changes compatible with no change in these properties. Flexural strength and fracture toughness showed no evidence of changes, irrespective of the bleaching gel used. Scanning electron microscopic analysis revealed erosive effects and micropore formation due to H2O2‐based bleaching. H2O2‐based bleaching deteriorates the surface of CAD/CAM composite materials while H2O2‐free bleaching gel had an insignificant effect on both surface and bulk properties. Zirconia crowns are capping materials used in dentistry for tooth capping and are very popular due to their optical properties and natural-looking visuals. In vitro aging protocols measure zirconia's optical properties, which are vital in order for it to look natural. This study aims to conduct a systematic review to explore the effect of in vitro aging protocols on the optical properties and crystalline structure of high-translucency (HT) zirconia. A correlation matrix was obtained using Microsoft Excel, which was later transferred into SPSS for confirmatory factor analysis (CFA) and hierarchal clustering and to obtain a dendrogram in order to display the distribution of clusters for each key term relevant to the study. Further, for qualitative analysis, 17 studies were screened and reviewed. The result demonstrates that high translucency has been observed in the crystalline structure of zirconia capping. However, quantitative and qualitative results did not demonstrate the in vitro protocol; instead, neglect of the in vitro protocol's vitality in zirconia crown capping was alarming. Overall, zirconia has good optical properties when combined with catalysts such as aluminum and lithium to obtain a more sustainable crystalline structure. [ABSTRACT FROM AUTHOR]

Background: Complicated crown-root fractures are a type of tooth fracture that involves the enamel, dentin, and cementum and accompanied by pulp exposure. The treatment of a complicated crown-root fracture is always challenging due to the difficulties in achieving a hermetic seal and a stable restoration with a fracture level close to the crestale bone level. This study aimed to evaluate and compare the efficacy of single-visit and multi-visit approaches for fragment reattachment in complicated crown-root fractures of anterior teeth. Methods: Two cohort consist of 10 adolescent patients in each group at both genders, who suffered from permanent anterior tooth complicated crown-root fracture were included. Fragment reattachment with root canal treatment was performed with either single or multiple-visit approach. Single visit fragment attachement combined with root cannel therapy was conducted in single-visit approach group immediately after injury. Fragment attachment, root cananel therapy and post resoration were performed during three times ' clinical visit in multi-visit approach group. Results: All the patients in both groups achieved satisfactory aesthetic results one year after fragment reattachment. Patients who underwent a multi-visit approach had a significantly shorter operative duration, less intra-operative pain and fatigue, slightly better periodontal health at an early stage, and a decreased incidence of temporomandibular joint disorders compared to those who underwent a single-visit approach. However, multiple visits approach may increase the risk of fragment detachment postoperatively. Conclusion: Fragment reattachment a reliable but temporary technique for adolescent patients who have suffered from complicated crown-root fractures. Multi-visit approach showed similar effecacy to single-visit approach but with slightly less complications. The choose of these two merhos should depend on the specific patient situation and patient compliance. Objective: This randomized controlled clinical trial compared the clinical efficacy of self‐adhesive bulk‐fill Surefil One with a traditional bulk‐fill composite in class II restorations. Materials and Methods: Sixty‐four direct class II composite restorations were categorized into two groups. Group I, control group (n = 32): cavities were restored by Filtek One bulk‐fill composite with Scotchbond Universal (SBU) adhesive in self‐etch mode, Group II, test group (n = 32): cavities were restored by Surefil One self‐adhesive bulk‐fill composite. The study involved a follow‐up period of 1 year, during which restorations were assessed at baseline (BL), 6 months, and 12 months using Federation Dentaire Internationale (FDI) criteria. Data analysis was performed using nonparametric tests. A comparison of restoration characteristics was performed utilizing the chi‐square test (X2). The significance level was set at 0.05. Results: Filtek One and Surefil One bulk‐fill composites revealed clinically acceptable FDI scores over 12‐month recalls. Thirty‐two patients (64 restorations) were available for all follow‐up visits; 100% of the restorations survived. For esthetic properties, Filtek One was far better than Surefil One at all time points. However, in terms of functional and biological properties, both restorations demonstrated comparable performances. Conclusions: Filtek One bulk‐fill restorations were superior in terms of surface luster, surface staining, color match, and translucency, but Surefil One restorations performed well and were similar to Filtek One restorations; however, additional advancements and research are needed to obtain better esthetics. Furthermore, longitudinal studies with extended follow‐up periods are needed to assess the clinical potential of both materials. Clinical Significance: Both Filtek One and Surefil One met the FDI criteria, with Filtek One demonstrating superior esthetic and functional qualities and similar performance regarding biological criteria. Objective: To describe a shift from injectable resin composite technique to composite resin computer‐aided design and computer‐aided manufacturing (CAD/CAM) veneers in addressing esthetic concerns associated with diastemas between anterior lower teeth in a clinical case. Clinical Considerations: Among several techniques proposed for direct resin composite restoration, the "Injectable resin composite technique" has gained popularity for its time‐efficiency, reduced technique sensitivity, and diminished reliance on clinician skills. However, challenges such as staining and the need for frequent polishing follow‐ups may prompt the consideration of more stable alternatives such as indirect veneer restorations. While ceramic veneers offer superior mechanical and optical properties, resin ceramic veneers, especially those milled from CAD/CAM resin ceramic blocks, offer advantages such as rapid, cost‐effective production, simplified intra‐oral repairs, less susceptibility to fracture, superior stress absorption, and requires minimal tooth preparation, making them an appealing option for many patients. Moreover, a fully‐digital approach not only streamlines the process but also saves time and labor while ensuring the delivery of high‐quality restorations to patients. Conclusion: In addressing a patient's dissatisfaction with constant polishing of direct resin composite restorations, a shift to resin composite CAD/CAM veneers was implemented. Utilizing a fully‐digital approach with CAD/CAM resin ceramic restorations successfully restored both esthetics and function. Clinical Significance: While the injectable resin composite technique achieves immediate esthetic results, its low color stability necessitates frequent polishing sessions. Aim: The objective of the present study was to evaluate the influence of different adhesive strategies regarding shear bond strength (SBS) of provisional resinbased materials bonded to the enamel surface as well as on the enamel surface roughness. Materials and methods: bovine incisors were randomly divided into six groups (n = 10) according to the adhesive strategy used: BRcontrol (bis-acrylic resin); Spot-etch+BR (spot-etch + bis-acrylic resin); Spot-etchSB2+BR (spotetch + adhesive + bis-acrylic resin); Spot-etchz350Flow+BR (spot-etch + flowable composite resin + bis-acrylic resin); SBU+BR (universal adhesive + bis-acrylic resin); Spot-etchsBMP+Z350 (spot-etch + adhesive + composite resin). The enamel surface roughness was determined by a surface profilometer. An SBS test was performed in a universal testing machine, and failure modes were classified under magnification. The SBS data were analyzed by one-way analysis of variance (ANOVA). A paired t test was used for enamel surface roughness intragroup comparisons, and the Friedman one-way repeated measures analysis of variance by ranks was used for differences in enamel surface roughness between groups, with the Tukey post hoc test (a = 0.05). Results: BRControl had the lowest SBS values (MPa), with a significant difference (P < 0.001) from the other groups. Spot-etch+BR had the highest SBS values but with no significant differences from the other groups in which the spot-etch technique was also used. Adhesive failure mode was predominant for all groups. BRControl had the lowest surface roughness difference, significantly different (P = 0.001) from all the other groups. Conclusions: Spot-etch and other adhesive strategies could be applied to increase the SBS values of provisional restorations to enamel compared with no surface pretreatment. Maintaining facial soft tissue contour and inter-implant papilla are challenging for implants in the esthetic zone. To counteract the inevitable hard and soft tissue changes after tooth extraction, the socket shield technique (SST) has been advocated as means to maintain the facial and/ or interproximal osseous and gingival architecture. Because SST is a technique-sensitive procedure, various complications related to SST have been reported. This study explores the effect of using dental brushes with or without metacrylate-based modeling resins on long-term color stability and surface topographies of resin-based composites. This study examined the effects of two variables: (1) the type of brush used (Art brush, Micro-brush, or Mylar strip) and (2) the application of a modeling resin (applied or not applied). The specimens were artificially aged through 10,000 cycles of thermocycling and subsequently immersed in coffee for 30 days. Measurements of color and surface roughness were taken at baseline and after the aging, using a non-contact profilometer for surface roughness and a spectrophotometer for color. Data were analyzed using paired t-tests and one-way ANOVA. Resin-based composites smoothed with dental brushes or micro brushes without modeling resins exhibited lower color change (ΔE) than other groups. Paired t-tests revealed significant differences in average surface roughness (Ra) and valley depth (Rv) for each surfacing technique before and after aging (p ⩽ 0.01). The root means square average of the profile heights (Rq) significantly increased in the control and micro-brush groups (p ⩽ 0.01). In conclusion, the use of brushes in resin-based composites placement does not increase the susceptibility to staining. Dental restorations must replicate the natural appearance of teeth while ensuring biocompatibility and durability. This study evaluated the surface characteristics and color stability of three dental composites—Herculite Ultra XRV, G-ænial A'CHORD, and Omnichroma—exposed to acidic beverages (red wine, black coffee, and Coca-Cola). Sixty disk-shaped specimens were prepared, polished, and immersed in these beverages. Surface roughness was assessed using profilometry and SEM analysis, hydrophobicity via contact angle analysis, and surface charge through streaming potential measurements. Color stability was evaluated using a spectrophotometer, and the pH levels of the solutions were recorded. Results showed that Herculite Ultra XRV had the highest mean contact angle (79.46° ± 6.52), indicating superior hydrophobicity, while Omnichroma had the lowest (64.94° ± 3.08), indicating more hydrophilicity. Significant color changes were observed, especially in black coffee, with ∆E values indicating notable discoloration. The acidic pH of the solutions increased surface roughness and color changes. Statistical analyses confirmed significant increases in surface roughness and color change for all composites, with the nanohybrid resin composite showing the greatest variability. Background: Recent studies have reported the inhomogeneity in the light emitted by dental light-curing units (LCUs). It is essential to understand how this uneven light distribution affects the physical properties of resin-based composites (RBCs) at various points across their surfaces. This study aimed to evaluate the effect of LCU beam's inhomogeneity on the microhardness of RBCs with different shades and thicknesses. Methods: Four body (A1B, A2B, A3B, and A4B), one dentin (A3D), and one enamel shade (A3E) of RBC (Filtek Z350 XT) were examined. The specimens were fabricated in four thicknesses (1, 2, 3, and 4 mm) and subjected to a 40-second light-curing. Vickers microhardness testing was performed at the center point, and 3 mm left and right from the center at the bottom surface of each sample. The LCU beam profile was characterized using a beam profiler, while irradiance after specimen passage was measured using a spectrometer. One-way analysis of variance (ANOVA) and Tukey's post-hoc tests were used to analyze the effects of shades and thicknesses on irradiance and microhardness, respectively. One-way repeated-measures ANOVA was used to compare the microhardness across different points. Pearson's correlation analysis examined the relationship between irradiance and microhardness. Results: The beam profile of LCU revealed inhomogeneous light distribution. Light irradiance was decreased with both the increase in thickness and darker shade of the specimens (p < 0.05). Microhardness was found to decline with an increase in sample thickness (p < 0.05), and was consistently higher at the center point compared to the periphery, particularly in thicker (3 and 4 mm) and darker shades (A3B, A4B, and A3D). A positive correlation was found between the irradiance and microhardness across all evaluated points (p < 0.05). Conclusions: Inhomogeneous light emission from LCU significantly influences the microhardness of RBC samples, depending on the thicknesses and shades. Dental composites are gaining great popularity in restorative dentistry because of their aesthetic appeal and capacity to replicate the natural color of teeth. Nevertheless, their lifespan and durability rely on various factors, such as the polishing technique and the environmental conditions they are exposed to. The study aimed to assess the influence of the method of final polishing of dental composite on the surface roughness and microhardness of materials also considering the environment of different pHs. Disc-shaped samples (5 mm diameter and 2 mm thickness) have been prepared for microhardness and roughness tests from two dental composites: A2 Clearfil Majesty ES 2 Classic and A2D Clearfil Majesty Premium. One-third of samples were polished with polishing discs, OptiDisc, another one-third of samples were polished with Eve Diacomp Twist rubbers and polishing brush with diamond particles, and rest of the samples were stored without any polishing (the control group). Tested materials were incubated in distilled water or acidic buffer (pH = 2) for 3 weeks at a temperature of 37 °C. No statistically significant differences were found for roughness for the two materials tested after incubation in liquids. A decrease in Vicker microhardness was found for Clearfil Majesty ES 2 Classic after soaking in a low pH liquid, and no such relationship was found for Clearfil Majesty Premium. The improved resistance of these materials to the negative oral environment may result in the longer survival of composite restorations in patients with poor diet or diseases, causing a decrease in oral pH. Objective: The aim of our study was to measure the color changes in one-shade composite resins when exposed to common drinks, such as tea, cola and coffee. Materials/Methods: In our study, Omnichroma, Vitrra APS Unique, GC A'chord and Charisma Diamond One composite resins were used. Composite resins were placed in stainless steel molds with depths of 2 mm and diameters of 5 mm. Ten specimens were immersed in tea, 10 specimens were immersed in coffee, 10 specimens were immersed in cola and 10 specimens were immersed in distilled water in an incubator at 37 °C for 14 days. Color measurements were performed at the beginning of the study and after 24 h and 14 days. Color values ​​were measured using a CIE L*a*b* system with a spectrophotometer device. Color and translucency changes were calculated and data analyzed using one-way ANOVA, two-way ANOVA, and post-hoc Tukey test (p < 0.05). Results: The greatest color changes occurred in the tea and coffee groups; the smallest color change occurred in the control group. After 14 days, the greatest color change was observed in the Charisma + Coffee group; the smallest color change was observed in the Omnichroma + Water group. The transparency and contrast ratios changed in all groups, and the smallest change occurred in the Omnichroma control group. Conclusion: Significant differences were found in the composite color changes after immersion in beverages. The color variations significantly differed depending on the beverage in which the specimens were immersed. The initial contrast ratio was markedly different from the examined materials. Clinical significance: The study emphasizes the significance of common beverages on the color stability of one-shade composite resins, underlining the need of appropriate material selection for long-term aesthetic effects in one-shade composite resin restorations. Statement of Problem: Limited evidence is available for the effect of chairside adjustment using rotary cutting instruments on the surface roughness and optical properties of different zirconia types. Purpose: To evaluate the effect of simulated adjustments on surface roughness and optical properties of different zirconia types. Materials and Methods: Three Partially Stabilized Zirconia (PSZ) types based on mole percent yttria (Y) concentration from the same manufacturer (Katana; Kuraray) were used: 3Y‐PSZ, 4Y‐PSZ, and 5Y‐PSZ. Thirty disk‐shaped specimens (Ø14 × 1.2 mm) from different zirconia types (N = 90) were prepared. Specimens were either left without adjustment (NA), adjusted with Dialite ZR finishing and polishing system (Brasseler) (APol), or adjusted with course diamond instruments only (ADia). The specimens were distributed into 9 groups (n = 10): Group 3Y‐PSZ/NA, Group 3Y‐PSZ/APol, Group 3Y‐PSZ/ADia, Group 4Y‐PSZ/NA, Group 4Y‐PSZ/APol, Group 4Y‐PSZ/ADia, Group 5Y‐PSZ/NA, Group 5Y‐PSZ/APol, and Group 5Y‐PSZ/ADia. The surface roughness of specimen was analyzed using an Atomic Force Microscope (AFM) (Bruker's Dimension Icon, Bruker) and Root Means Square (RMS) were recorded (nm). Surface Gloss (SG), Translucency Parameter (TP), and Contrast Ratio (CR) values of all groups were recorded using an integrating sphere spectrophotometer. Statistical analysis was performed using analysis of variance (ANOVA) and Tukey's multiple comparison tests for pairwise comparisons at p < 0.05 and 95% confidence interval. Results: APol had no effect on the surface roughness (p = 0.88) while ADia had a significant negative effect (p < 0.05) despite the type of zirconia. Out of the three testes optical properties, only SG was negatively affected by ADia for all types of zirconia (p < 0.05). The two adjustment types did not affect the TP of all the tested zirconia (p = 0.91). The CR was not affected by the tested adjustments for all zirconia types (p = 0.726). Conclusion: Proper zirconia adjustment following a sequence of burs and polishers can maintain acceptable roughness and optical properties. Adjustment of zirconia with rough diamond can lead to deleterious effects and should be avoided. Clinical Significance: Chairside adjustment of zirconia could lead to rougher surface and unpredictable changes of surface gloss. The color adjustment potential of resin composites depends on the specific characteristics of the surrounding structures. The authors evaluated the influence of cavity wall thickness on CIEDE2000 color adjustment potential (CAP 00) of single-shade composites using the CIEDE2000 color difference (ΔE 00) formula. Two types of specimens were produced: single, using Vittra APS color DA3 (FGM Dental), Vittra APS Unique (VU) (FGM Dental), and Omnichroma (Tokuyama Dental) composites; and dual, using Vittra APS color DA3, but with cavity wall thicknesses of 3, 2, or 1 mm, restored with VU or Omnichroma. CIEL∗a∗b∗ color coordinates were obtained from digital photographs of the specimens at different locations, and ΔE 00 and CAP 00 were calculated. Statistical analysis included 2-way analysis of variance, Kruskal-Wallis, Holm-Sidak, Student-Newman-Keuls, Wilcoxon signed rank, and paired t tests (α = 5%). Cavity walls 1-mm-thick had higher ΔE 00 values and lower CAP 00 values, particularly for VU (P <.05). Restorations matched better with the control at the periphery than the central area (P <.001). Overall ΔE 00 means exceeded the color perceptibility threshold (ie, 0.8) and the acceptability threshold (ie, 1.8). Color differences (between center of restoration and control composite) were moderately unacceptable for 3-mm and 2-mm thicknesses and extremely unacceptable for 1-mm thickness. Both composites had a ΔE 00 color shift predominately determined by means of the lightness difference. Positive CAP 00 values were found for VU and Omnichroma. Wall thickness, especially at 1 mm, negatively impacted the color of single- shade composites. This effect was more pronounced for VU, affecting mainly the restoration's central area. Comparative studies of interim veneer restorations crafted using subtractive computer-aided manufacturing (s-CAM) milling technology and traditional direct hand-made approaches are needed. This comparative in vitro study evaluated the fracture resistance of two types of provisional veneer restorations for maxillary central incisors: milled (s-CAM) and traditional direct hand-made bis-acryl veneers. Fifty maxillary right central incisor veneers (25 specimens per group) were fabricated and divided according to the fabrication method: (1) s-CAM milled (Structure CAD, VOCO Dental); and (2) hand-made (Protemp Plus, 3M). The restorations were cemented onto 3D-printed resin dies using temporary cement and subjected to 1000 cycles of thermal cycling between 5° and 55 °C. These restorations subsequently were subjected to compressive loading until fracture occurred. Images of the fractured samples were captured using a scanning electron microscope (SEM). Statistical analysis was performed using the one-way ANOVA test and the Mann-Whitney U test. Significant differences (p < 0.001) in the fracture resistance were observed between the two groups. s-CAM milled interim veneers displayed higher fracture resistance values (439.60 ± 26 N) compared to the traditional method (149.15 ± 10 N). The manufacturing method significantly influences the fracture resistance of interim veneer restorations. s-CAM interim laminate veneer restorations for maxillary central incisors exhibit a fracture resistance superior to that of the traditional method using bis-acryl. Purpose: To observe the effect of different prosthetic materials (polymethyl methacrylate [PMMA] and flowable composite [FC]) on marginal bone loss, pink esthetic score (PES), and cytokine levels (receptor activator of nuclear factor kappa-B ligand [RANKL] and osteoprotegerin [OPG]). Materials and Methods: A total of 46 patients (31 women, 15 men) were treated with immediate implant therapy after tooth extraction. For standardization, only the maxillary premolar tooth of each patient was restored, and also, both of the adjacent teeth were present in the dental arch. Provisional crowns were prepared before the surgery on patient models with two different materials (24 PMMA, 22 FC). Following the surgical procedure, provisional crowns were adjusted on polyetheretherketone (PEEK) abutments as nonfunctional in centric and eccentric movements. After the surgery, patients were evaluated monthly for 3 months. At each follow-up, periapical radiography was obtained by the parallel technique, and peri-implant crevicular fluid (PICF) was collected. Pink esthetic scoring was done. MBL was calculated for the mesial and distal sides separately. Cytokine levels were analyzed from PICF. Statistical analyses (Shapiro-Wilk, Mann-Whitney U, independent-samples t test, Wilcoxon t test, paired-samples t test, and Friedman two-way analysis of variance with Bonferroni correction) were completed (α = .05). Results: PES was increased significantly within groups (P < .01). However, there was no statistically significant difference between groups. According to the Mann-Whitney U test, no significant difference was found for the MBL (P > .05). When the RANKL/OPG values were evaluated within the group by using the Friedmann two-way analysis of variance test, no significant difference was found (P > .05). Conclusion: Similar pink esthetics, cytokine levels, and bone loss can be achieved using a protocol including immediate implants, particle grafts, soft tissue graft, PEEK abutments, and provisional restorations fabricated using PMMA and FC. Background: The rising demand for improved aesthetics has driven the utilization of recently introduced aesthetic materials for creating custom post and core restorations. However, information regarding the fracture resistance of these materials remains unclear, which limits their practical use as custom post and core restorations in clinical applications. Aim of the study: This study aimed to evaluate the fracture resistance of three non-metallic esthetic post and core restorations and their modes of failure. Materials and methods: Thirty-nine single-rooted human maxillary central incisors were endodontically treated. A standardized post space preparation of 9mm length was performed to all teeth to receive custom-made post and core restorations. The prepared teeth were randomly allocated to receive a post and core restoration made of one of the following materials (n=13): glass fiber-reinforced composite (FRC), polyetheretherketone (PEEK) and polymer-infiltrated ceramic-network (PICN). An intraoral scanner was used to scan all teeth including the post spaces. Computer-aided design and computer-aided manufacturing (CAD-CAM) was used to fabricate post and core restorations. Post and core restorations were cemented using self-adhesive resin cement. All specimens were subjected to fracture resistance testing using a universal testing machine. Failure mode analysis was assessed using a stereomicroscope and SEM. The data was statistically analyzed using One-Way ANOVA test followed by multiple pairwise comparisons using Bonferroni adjusted significance level. Results: Custom PEEK post and core restorations displayed the least fracture load values at 286.16 ± 67.09 N. In contrast, FRC exhibited the highest average fracture load at 452.60 ± 105.90 N, closely followed by PICN at 426.76 ± 77.99 N. In terms of failure modes, 46.2% of specimens with PICN were deemed non-restorable, while for PEEK and FRC, these percentages were 58.8% and 61.5%, respectively. Conclusions: Within the limitation of this study, both FRC and PICN demonstrated good performance regarding fracture resistance, surpassing that of PEEK. [ABSTRACT FROM AUTHOR]

Background: The establishment of good facial esthetics is one of the main objectives of complete denture construction. Unfortunately, it may be the caused issue for patients having a prominent premaxilla due to excessive lip support by the labial flange of the maxillary denture. Open-face dentures (OFD) may suggest suitable prosthetic management for these patients. However, clinical evidence regarding the efficiency of OFD is scarce. Methods: A total of 38 completely edentulous participants having prominent premaxilla and skeletal class I Angle's classification were enrolled in this study. Each participant received a mandibular complete denture and 2 opposing maxillary dentures; conventional (CD) and open-face (OFD). On the day of denture insertion, the participants were divided into 2 groups; CD-OFD and OFD-CD where CD-OFD group was instructed to use the mandibular denture and the maxillary CD for 3 months and then to use the maxillary OFD for another 3 months after a wash-out period of 2 weeks. While group OFD-CD was instructed to use the mandibular denture and the maxillary OFD for 3 months then to use the maxillary CD for another 3 months after a wash-out period of 2 weeks. The dislodging force of the maxillary dentures was evaluated using the universal testing machine and the patient perception of retention, esthetics, and comfort was evaluated using the Visual Analogue Scale (VAS). Evaluation was carried out 1 day, 1 month, and 3 months after denture insertion. The Student t-test was used to compare the 2 maxillary dentures and the intervals for each denture were compared by using the ANOVA test with repeated measures followed by a Post Hoc test (adjusted Bonferroni) for pairwise comparison. Results: The significance of the obtained results was judged at the 5% level (P value). The dislodging force and patient perception of retention did not show significant differences between the 2 dentures, while the perception of esthetics showed significant differences throughout the follow-up period. Perception of comfort showed an insignificant difference only at the 3-month interval. Conclusions: Open-face maxillary dentures can be a suitable alternative for patients with prominent premaxilla to achieve satisfactory retention, aesthetics, and comfort. [ABSTRACT FROM AUTHOR]

Purpose: The aim of the current study was to evaluate the effect of simulated gastric acid on the color and translucency of different indirect restorative materials. Materials and methods: A total of 36 disc-shaped samples were cut by using an isomet saw and divided into four equal groups (n = 9) according to the material type: Group Z: translucent zirconia (Ceramill® Zolid ht.+ preshade, Amann Girrbach, Koblach, Austria); Group E: lithium disilicate (IPS e.max CAD, Ivoclar Vivadent AG, Schaan, Liechtenstein); Group C: resin nanoceramic (Cerasmart, GC, Tokyo, Japan); Group P: polyether ether ketone (PEEK) (Bettin Zirconia Dentale Italy) veneered with indirect high impact polymer composite (HIPC) (breCAM HIPC, Bredent GmbH & Co. KG, Germany). The samples were immersed in simulated gastric acid (HCl, pH 1.2) for 96 hours at 37 °C in an incubator. The color change (ΔE00) and translucency (RTP00) were measured every 9.6 hours (one-year clinical simulation) of immersion in simulated gastric acid. Results: For color change (∆E00) and translucency (RTP00) among the tested materials, there was a highly statistically significant difference (P < 0.001) after every year of follow-up. The color change in both Z and G groups was the lowest after 1 year of acid immersion, followed by that in group H, and the highest change in color was recorded in group P. Background: Treating white spot lesions (WSLs) with resin infiltration alone may not be sufficient, raising questions about its compatibility with other treatments amid controversial or incomplete data. Therefore, this study aimed to assess the aesthetic feasibility of resin infiltration combined with bleaching, as well as its potential mechanical effect on ceramic bonding to WSLs. Methods: One hundred and fifty flat enamel surfaces of bovine incisors were prepared. Ninety specimens were deminerailized and randomly assigned to three groups(n = 30): post-bleaching resin infiltration (Bl-R), pre-bleaching resin infiltration (R-Bl), and only resin infiltration (R). Color, surface roughness and microhardness were assessed in immediate, thermocycling and pigmentation tests. The remaining sixty samples were randomly assigned to three groups (n = 20): control (Ctrl), bonding (Bo), pre-bonding resin infiltration (R-Bo). Shear bonding strength, failure mode, micro-leakage depth and interface morphology were evaluated after ceramic bonding. The Tukey test and analysis of variance (ANOVA) were used for statistical analysis. Results: For the effect of resin infiltration and bleaching on WSLs, the R-Bl group showed the worst chromic masking ability, with the highest |ΔL|, |Δa|, |Δb|, and ΔE values after treatment. Compared with those in the Bl-R group, the R-Bl and R groups showed significant time-dependent staining, which is possibly attributed to their surface roughness. For the effect of resin infiltration on the adhesive properties of WSLs, resin infiltration reduced the staining penetration depth of WSLs from 2393.54 ± 1118.86 μm to 188.46 ± 89.96 μm (P < 0.05) while reducing WSLs porosity in SEM observation. Conclusions: Post-bleaching resin infiltration proved to be advantageous in the aesthetic treatment of WSLs. Resin infiltration did not compromise bonding strength but it did reduce microleakage and enhance marginal sealing. Overall, resin infiltration can effectively enhance the chromatic results of treated WSLs and prevent long-term bonding failure between ceramics and enamel. Based on these findings, the use of post-bleaching resin infiltration is recommended, and resin infiltration before ceramic bonding is deemed viable in clinical practice. [ABSTRACT FROM AUTHOR]

It has been observed in recent years that zirconia (Zr) is being increasingly used for a wide range of clinical applications. There are several reasons for this, but the most significant one is its excellent mechanical properties, specifically its transformation toughening properties compared to other dental ceramics and its improved natural appearance when compared to ceramometal restorations. As a result of the advancement of chairside milling and developments in rapid-sintering technology, the fabrication of dental restorations has become more computerized, time-saving, and accurate over the past few decades. However, a main disadvantage of conventional Zr restorations is that they lack the translucency of glass–ceramics, although they are extremely strong. Recently, by increasing the yttrium %, changing the grain size, and reducing the impurities, the ultra-translucent monolithic zirconia "5-mol%-yttria-stabilized tetragonal zirconia polycrystals" has been introduced, with successful attempts to make translucent Zr an aesthetically attractive option for minimally invasive veneer restorations. It is important to note that veneer restorations do not possess the mechanical retentive features of the tooth preparations and rely primarily on bonding to resin cement. This presents a great challenge for the inert Zr since it does not bond chemically with resin cement, unlike glass–ceramic materials that establish chemical adhesion with resin cement, favoring their use for indirect veneer restorations. Taking this into account, this article aims to review the progressive development of ultra-translucent monolithic Zr materials as they are available today and, in the future, represents a concerted drive toward maximum translucency and strength, which renders them a viable treatment option for esthetic veneer restorations. This study evaluated the radiographic alterations that occur in immediate postextraction implants and in delayed implants inserted in a preserved ridge. In group A, an implant was placed immediately after tooth extraction, grafting the bone-to-implant gap. In group B, alveolar ridge preservation was performed after tooth extraction, and delayed implant placement was performed 4 months later. The final follow-up was 1 year after prosthetic loading in both groups. The marginal bone level (MBL) was measured at implant insertion (T1), at loading (T2), and at the final follow-up, 12 months after prosthetic loading (T3). At tooth extraction (T0) and T3, the horizontal ridge width (HW) was measured on CBCT scans at three different levels. No statistically significant differences in MBL or HW were found between the two study groups. The outcomes suggest that it is possible to maintain MBL and alveolar bone volumes regardless of whether the procedure is performed through immediate postextraction implant placement or through delayed implant placement in a preserved ridge. Soft tissue changes were evaluated over a period of 1 year in 48 patients who required extraction of a single tooth in the anterior maxillary arch (premolar to premolar) and its replacement with an implant. The patients were randomly divided into two groups: In group A, an immediate postextraction implant was placed, and the bone-to-implant gap was filled with bovine bone mineral; in group B, the alveolar ridge preservation technique was performed after extraction, and the implant was placed 4 months later. On the day of tooth extraction (T0) and 1 year after tooth extraction (T1), the soft tissue horizontal width, mesial and distal papillary levels, midfacial gingival level, and Pink Esthetic Score were evaluated in both groups. No significant differences were observed between the groups in any of the considered parameters. Statistically significant differences were found in the soft tissue horizontal width between T0 and T1. The clinical results of the two procedures were similar and comparable over time. When evaluating the stability of the soft tissue contour, and considering the specific indications of the two techniques, it is possible to choose either an immediate implant or an alveolar ridge preservation technique with staged placement. Alveolar ridge preservation (ARP) is indicated to attenuate anatomic and physiologic changes following tooth extraction. A properly contoured ovate pontic placed immediately into an extraction socket may be adequate to maintain alveolar ridge architecture for improved esthetic results. This prospective clinical study evaluated the ability of immediately placed ovate pontics in conjunction with ARP to attenuate postextraction tissue dimensional changes in the esthetic zone and maintain alveolar ridge contour. Ten patients (11 sites) completed the study. All subjects received a combination of socket grafting with allogeneic particulate graft material and socket sealing with an ovate pontic provisional restoration. A set of clinical linear and volumetric outcomes were assessed after a 6-month healing period. At 6 months postoperative, the linear measurements for the mean ridge dimensional loss were 0.9 ± 0.6 mm (range: 0.2 to 1.8 mm) in height and 1.4 ± 0.6 mm (range: 0.1 to 2.4 mm) in width. The mean volumetric tissue loss observed was 24.4 ± 15.4 mm³ (range: 2.6 to 50.1 mm³) at 3 months postoperative and 32.2 ± 14.2 mm³ (range: 3.8 to 50.5 mm³) at 6 months postoperative. Resorption pattern assessment showed the overall cervical area to have less resorption than the apical areas at 6 months postoperative, with the least amount of resorption in the midbuccal cervical section. When compared to the data of a previous pilot study, no statistically significant difference was seen between the dimensional losses when using ovate pontics with and without ARP. This may be evidence that the use of an ovate pontic provisional restoration immediately after extraction effectively attenuates postextraction dimensional changes. [ABSTRACT FROM AUTHOR]

Objectives: This study will evaluate the effects of anti-epileptic drugs and brushing used in children on the color change of three restorative materials by creating an in vitro study model. Methods: Forty samples of polyacid-modified composite resin (compomer), glass ionomer cement (GIC), and composite resin (CR) were prepared. Samples were split into four groups (n = 10) and soaked in three anti-epileptic drugs (Tegretol, Depakine, Keppra) and distilled water. For each group (n = 5), two subgroups (brushing and non-brushing) were created. Discolorations [CIEDE2000 (ΔE00)] were determined initially and on days 7 and 14. The data were analyzed with a four-factor repeated measures ANOVA analysis, and a post hoc analysis Bonferroni test was used. Results: After the second week, the highest ΔE00 value was seen in the non-brushed compomer material in the Tegretol drug group (8.59 ± 0.43). In contrast, the lowest value was seen in GIC filling material-brushing-Depakine drug (3.45 ± 2.14). ΔE00 values in the brushing groups were statistically significantly lower than those in the no brushing groups (p < 0.05). Conclusions: It has been determined that the color stability of aesthetic restorative dental materials used in pediatric dentistry is affected by antiepileptic drugs. In addition, it has been determined that tooth brushing positively affects the color stability of restorative materials. Clear aligner therapy (CAT) has gained popularity in recent years. As technological advancements increase within the field of dentistry, clinicians have opted to produce orthodontic appliances within their own offices or clinics, often referred to as in-house CAT. Construction of in-house aligners utilizes 3-dimensional printing devices, potentially offering practitioners enhanced control and improving patient comfort. This article provides an overview of the available evidence on the performance, with particular consideration of the longevity of restorative materials used in the management of tooth wear. Different materials can be employed depending on whether a subtractive or additive strategy is used. Preference should be given to the latter to help preserve remaining tooth tissue. The use of both direct and indirect materials may yield acceptable survival results when considering data with at least 5 years of follow-up. Patients should be informed that because tooth wear is a continuous process, a certain level of maintenance of the restorations will be necessary and that this may impact on the success of the treatment. Application of spectral analysis for measuring the optical parameters of new and promising dental materials used in esthetics and restorations is considered. Modern spectrometers have high measurement accuracy, are small in size and easy to operate, and fully meet the requirements for analysis in various fields of science and technology, so the study of dental materials with these devices is relevant and has potential. This article defines research objectives and identifies tasks, study objects, and measurement methods. An external view of the u‑Violet DB spectrophotometer is presented, along with the optical scheme of the device. Spectral relationships between the absorption coefficient K(λ) and the radiation wavelength of study specimens were obtained. The spectral curves of materials studied here were found to have identical distributions, though Omnichroma Blocker had the greatest absorption in the wavelength range λ = 250–360 nm. Objective: This clinical case describes a completely digital workflow using current digital technologies for a full‐mouth adhesive rehabilitation with ultra‐translucent multilayer zirconia restorations. Clinical Considerations: A healthy 60‐year‐old man with abfractions on all upper and lower molars and severe tooth wear underwent a full‐mouth rehabilitation with laminate veneers and partial adhesive restorations. A proper zirconia bonding protocol was realized to provide a durable bond between the ultra‐translucent zirconia and the resin cement. Furthermore, the implementation of a digital workflow enables the clinician to have an effective communication during treatment planning and simplify the clinical and laboratory procedures, providing the patient with long‐term esthetic treatment and functional results. Conclusion: The implementation of a completely digital workflow and the use of ultra‐translucent multilayer zirconia for indirect adhesive restorations can be an alternative with simplified and predictable procedures for patients with dental wear and teeth discolorations. Clinical Significance: The digital workflow described is intended to facilitate the planning and execution of a full‐mouth adhesive rehabilitation and demonstrates to clinicians a reliable zirconia bonding concept for minimally invasive anterior and posterior restorations. Objective: To describe the clinical use of nitride‐coated titanium CAD/CAM implant abutments in the maxillary esthetic zone in two patients with high esthetic and functional demands and, to highlight the advantages of nitride‐coated milled titanium abutments when compared to stock/custom titanium, one‐piece monolithic zirconia, and hybrid metal‐zirconia implant abutments. Clinical Considerations: Due to the inherent mechanical and esthetic clinical challenges, single implant‐supported reconstructions in the maxillary esthetic zone are a complex restorative treatment. While CAD/CAM technology has been suggested to enhance and ease implant abutment design and manufacturing, implant abutment material selection remains as a critical decision affecting restoration's long‐term clinical outcomes. To date, considering the esthetic disadvantages of conventional titanium implant abutments, the mechanical limitations of one‐piece zirconia abutments and the manufacturing time and costs associated with hybrid metal‐zirconia abutments, no abutment material can be considered "ideal" for all clinical scenarios. Due to their biocompatibility, biomechanical characteristics (hardness and wear resistance), optical properties (yellow color), and peri‐implant soft tissue esthetic integration, the use of CAD/CAM titanium nitride‐coated implant abutments has been suggested as a predictable implant abutment material in mechanically challenging but esthetically demanding clinical situations, as the maxillary esthetic zone. Conclusions: Two patients requiring a combined tooth‐implant restorative treatment in the maxillary esthetic zone were treated using CAD/CAM nitride coated titanium implant abutments. The principal advantages of TiN coated abutments include comparable clinical outcomes to stock abutments, optimal biocompatibility, adequate fracture, wear, and corrosion resistance, reduced bacterial adhesion, and excellent esthetic integration with adjacent soft tissues. Clinical Significance: Clinical reports and short term mechanical, biological and esthetic clinical outcomes indicate that CAD/CAM nitride coated titanium implant abutments can represent a predictable restorative alternative to stock/custom and metal/zirconia implant abutments and be considered a clinical relevant option in mechanically challenging but esthetically demanding situations, as often found in the maxillary esthetic zone. Objectives: Natural restorations combine digital workflow and shell technique to create CAD/CAM restorations with the form and texture of natural teeth. This case report describes an interdisciplinary digital workflow combined with CAD/CAM natural restorations to achieve the naturalness of an anterior rehabilitation. Clinical considerations: A 38‐year‐old patient attended to the office with esthetic issues. An interdisciplinary treatment plan was conducted, which included periodontal surgery to recreate the gingival contour, associated with bleaching and manufacturing CAD/CAM ceramic veneers to return an optimal teeth surface texture and shape. Conclusions: The design and manufacturing of CAD/CAM natural restorations using a digital workflow allowed a predictable result and overcame the limitations of conventional shell technique. Clinical Significance: Conventional shell technique is used to create restorations with the form and texture of natural teeth. Full-coverage restorations are indicated for posterior primary teeth as definitive treatments for high caries-risk children and in patients whose treatment is being performed under general anesthesia. Advances in biomaterials and parental preferences have driven interest in esthetic full coverage treatment options for primary posterior teeth. The prefabricated resin crown offers a 'flex fit' adaptation over the anatomic cervical convexity of the primary clinical crown, similar in functionality to a stainless steel crown. A new commercial product, BioFlx, hopes to address the ductility, color stability and durability concerns that glass fiber-reinforced composite crowns have while maintaining the enticing clinical aspects, namely the flex fit, esthetic appearance and more conservative tooth preparation when compared to pediatric zirconia crowns. The purpose of this case report is to describe an alternative restorative material that can be employed to deliver an esthetic full coverage solution in a child with a potential nickel allergy. Patients with a complex problem set involving multiple levels of altered structure challenge the clinician to develop an individualized, appropriate treatment plan. Dentofacial deficiency, occlusal problems, and loss of tooth structure require intervention to establish stability and regain function, speech, esthetics, and masticatory muscle comfort. The comprehensive examination must quantify each problem to specify the diagnosis for realistic treatment planning. The clinical case of a patient with Costello syndrome is presented to illustrate essential concepts in diagnosis and treatment of complex cases, including (1) Global Diagnosis of anterior esthetic relationships, (2) occlusal analysis with diagnostic casts verified in centric relation, (3) comprehensive restoration previewed with a diagnostic wax-up and removable acrylic resin overlay, (4) adhesive monobody composite resin onlays that preserve tooth structure, and (5) programmed occlusion, quantified with digital occlusal analysis, to ensure stability and comfort. Costello syndrome is a neurodevelopmental syndrome causing multisystem effects, including a distinctive craniofacial phenotype, cardiovascular disease, intellectual disability, growth hormone deficiency, and dental abnormalities such as delayed dental development, bruxism, and demineralized enamel lesions. Background: This study compared the effectiveness of three bone grafting materials used for treating class II fresh extraction sockets in the esthetic zone with immediate implant placement using Vestibular Socket Therapy (VST) to evaluate the pink esthetic score (PES), peri-implant mucosal levels (PML), and facial bone thickness (FBT). Methods: Twenty-four surgical sites in the maxillary anterior region presented with type II socket defects received immediate implants and simultaneous bone grafting with either a collagen plug soaked in blood, demineralized bone matrix Grafton, or a particulate mixture of 2/3 autogenous bone chips and 1/3 deproteinized bovine bone mineral MinerOss X. The outcome measures were evaluated at 6 and 12 months. The study was registered on www.clinicaltrial.gov (12/07/2021 - ID: NCT04957654). Results: Twenty-two cases (91.6%) showed a total PES score of > 10, without a significant difference between all groups. The vertical height soft tissue changes showed significant improvement in the Collagen plug and Grafton groups at 6 and 12 months, while MinerOss X showed no significant difference at 6 and 12 months compared to baseline. Radiographically, FBT was 0.72 ± 0.20, 0.44 ± 0.12, and 0.95 ± 0.37 at baseline, which significantly increased to 1.61 ± 0.88, 1.48 ± 1.20 and 2.31 ± 0.86 at 12 months for all three groups, respectively. Conclusion: The use of a particulate bone graft mixture significantly increases the FBT compared to collagen plugs and DBM-Grafton when performing VST during immediate implant placement in compromised Class II extraction sockets. Background: Computer-aided design and computer-aided manufacturing (CAD-CAM) materials for prosthetic is gaining popularity in dentistry. However, limited information exists regarding the impact of thickness and roughening treatment on the optical properties of contemporary CAD-CAM restorative materials. This study aimed to quantitatively evaluate the translucency and opalescence of six dental CAD-CAM materials in response to different thicknesses and roughening treatments. Methods: Six dental CAD-CAM materials, lithium disilicate glass–ceramic (IPS e.max CAD, LS), polymer-infiltrated ceramic (VITA Enamic, VE), resin-nano ceramic glass–ceramic (LAVA Ultimate, LU), polymethyl methacrylate (Telio CAD, TE), and two zirconia reinforced lithium silicate (VITA Suprinity, VS, and Celtra Duo, CD), in shade A2 were prepared as 12 × 12mm2 specimens of four thicknesses (0.5mm, 1.0mm, 1.5mm, and 2.0mm) (N = 240, n = 10). After three different treatments (polished, roughened by SiC P800-grit, and SiC P300-grit), the translucency parameter (TP00) and opalescence parameter (OP) were measured with a spectrophotometer (VITA Easyshade V). The surface roughness was analyzed with a shape measurement laser microscope. The data were analyzed using a MANOVA, post hoc Tukey–Kramer test, the t test, and regression analysis (α =.05). Results: The TP00 and OP were significantly influenced by material type, thickness and roughening treatment (P <.05). TP00 showed a continues decline with increasing thicknesses, while the variations of OP were material-dependent. TP00 ranged from 37.80 (LS in 0.5mm) to 5.66 (VS in 2.0mm), and OP ranged from 5.66 (LU in 0.5mm) to 9.55 (VS in 0.5mm). The variations in TP00 of all materials between adjacent thicknesses ranged from 2.10 to 15.29, exceeding the acceptable translucency threshold except for LU. Quadratic and logarithmic regression curves exhibited the best fit for TP00 among the materials. Compared to polished specimens, rougher specimens exhibited lower TP00 and higher OP in all materials except for LS (P < 0.05). Roughening with P300-grit decreased TP00 and OP by an average of 2.59 and 0.43 for 0.5mm specimens, and 1.26 and 0.25 for 2.0mm specimens, respectively. Conclusions: Variations in translucency caused by thickness and roughening treatment were perceptible and may be clinically unacceptable. Background: One of the crucial factors influencing the longevity of anterior aesthetic dental restorations is the colour stability of resin composites. Bleaching and whitening have become popular methods for enhancing dental aesthetics. Home whitening techniques, such as special pens, are widely available commercially. This in vitro study aimed to determine the efficiency of a whitening pen in removing tea stains from resin composite by measuring colour differences (ΔE00). Additionally, the study aimed to evaluate the variations in colour parameters measured by extra-oral and intra-oral spectrophotometers. Methods: A total of 45 disc-shaped resin composite specimens were randomly divided into three groups; Group 1: stored in artificial saliva (control), Group 2: stored in artificial saliva followed by a whitening pen application, and Group 3: stored in tea followed by a whitening pen application. Colour measurements were taken three times for each specimen using two spectrophotometers (extra-oral and intra-oral devices); T1: before storage (baseline), T2: after storage in artificial saliva or tea for 6 days; and T3: after one week of whitening pen application in groups 2 and 3. The data were statistically analyzed using one-way ANOVA followed by the Tukey post hoc test (p ≤ 0.05). The independent sample t-test was also employed. The equation of CIEDE2000 (ΔE00) was used to calculate the colour difference between the dry, as-prepared specimens (baseline), and those after storage or bleaching. The colour changes exceeding the acceptability threshold (∆E00 = 1.8) were considered unacceptable. Results: After whitening, the colour of the specimens stored in brewed tea (Group 3) remained unacceptable, as indicated by both the extra-oral and intra-oral spectrophotometers (ΔE00 = 4 and 2.9, respectively). Groups 1 and 2 exhibited lower ΔE00 values than Group 3 (p = 0.01 *). No significant difference was observed between Group 1 (stored in artificial saliva) and Group 2 (stored in artificial saliva and then bleached) (p = 0.3). Significant differences were consistently observed between the data obtained from the extra-oral spectrophotometer and the intra-oral one. Conclusions: The whitening pen proved ineffective in removing tea stains from resin composites. Although significant differences were found between the values obtained by the two spectrophotometers (extra-oral and intra-oral), both devices confirmed the unacceptable colour of the tea-stained resin composites after whitening. Aim: To evaluate the influence of the abutment material (zirconia vs. titanium) on the long‐term aesthetic and clinical outcomes of implant‐supported restorations. Materials and Methods: In 30 patients, a single implant‐supported restoration with either a zirconia or a titanium abutment was placed in the anterior maxilla (incisors, canines, and bicuspids). Aesthetic (Implant Crown Aesthetic Index or ICAI), clinical, radiographic, and patient‐centred outcomes were recorded at baseline (1 month after final restoration), 1 year, and 5 years of follow‐up. This study was registered in ClinicalTrials.gov (NCT02315794). Results: Twenty‐five subjects completed the follow‐up visits at 1 and 5 years. ICAI values showed statistically significant better aesthetic outcomes when zirconia abutments were used compared to titanium abutments. Between 1 and 5 years, the aesthetic sub‐analysis of the crown component worsened but the mucosal sub‐analysis improved. There were no significant changes in bone levels, but the plaque index, bleeding on probing, and probing depths worsened in both groups.
