This paper proposes a new methodology to improve territorial cohesion in clustering processes where many variables from different topics are considered. Clustering techniques provide added value to identify typologies, but there are still unsolved challenges when data contain an unbalanced number of variables from different topics. The territorial feature selection method (TFSM) is presented as a method to select the representative variable of each topic such that the interpretability of resulting clusters is preserved and the geographical cohesion is improved with respect to classical approaches. This paper also introduces the thermometer as a new knowledge acquisition tool that allows experts to transfer semantics to the data mining process. TFSM proposes the index of potential explainability ( E k ) as the criteria to select the most promising variables for clustering. E k is based on the combination of inferential testing and metrics such as support. The proposal is applied with the INSESS-COVID19 database, where territorial groups of vulnerable populations were found. A set of 195 variables with 21 unbalanced thematic blocks is used to compare the results with a traditional multiview clustering analysis with promising results from both the geographical and the thematic point of view and the capacity to support further decision making. The football rivalry between Real Madrid CF and FC Barcelona is one of the most popular at club level globally. In Spain, where it is known as El Clásico (the Classic), it has an unrivalled status in the sports media industry. Its significance relies in part to the historical tension between Catalonia, the region of which Barcelona is the main centre, and Madrid, the capital of Spain. The exaltation of confrontation and partisanship is the central feature of highly popular radio programmes devoted to El Clásico. This study aimed to observe how the media discourse articulated by these programmes was affected by the fact that, during the Covid-19 pandemic, matches were played in empty venues. Through the comparisons of broadcasts of two games, one from 2017 and from 2020, the paper shows that the sensationalistic style is toned down, and a less polarised and partisan sports journalism is possible. Critical geopolitics conceives of the internal organization of the state as a relevant geopolitical subfield. In this context, this paper aims to develop a geopolitical interpretation of the covid-19 crisis management in Catalonia and Spain from March 2020 to mid-April 2020. The paper begins by providing an overview of internal geopolitics literature by Spanish geographers, focusing on the Catalonia/Spain conflict, followed by a thorough analysis of written and oral materials compiled for that purpose. Three discursive periods are identified: up until the declaration of the State of Alarm in mid-March 2020, the first two weeks with this in force, and the first half of April 2020. The paper evinces that the examined materials support contested visions about the distribution of territorial powers within Spain, while also unearthing some specificities that ultimately relate to the Catalan national question. The choices that researchers make while conducting a statistical analysis usually have a notable impact on the results. This fact has become evident in the ongoing research of the association between the environment and the evolution of the coronavirus disease 2019 (COVID-19) pandemic, in light of the hundreds of contradictory studies that have already been published on this issue in just a few months. In this paper, a COVID-19 dataset containing the number of daily cases registered in the regions of Catalonia (Spain) since the start of the pandemic to the end of August 2020 is analysed using statistical models of diverse levels of complexity. Specifically, the possible effect of several environmental variables (solar exposure, mean temperature, and wind speed) on the number of cases is assessed. Thus, the first objective of the paper is to show how the choice of a certain type of statistical model to conduct the analysis can have a severe impact on the associations that are inferred between the covariates and the response variable. Secondly, it is shown how the use of spatio-temporal models accounting for the nature of the data allows understanding the evolution of the pandemic in space and time. The results suggest that even though the models fitted to the data correctly capture the evolution of COVID-19 in space and time, determining whether there is an association between the spread of the pandemic and certain environmental conditions is complex, as it is severely affected by the choice of the model. Featured Application: The results of this research have been delivered to the General Director of Social Services and the General Director of Equity from the Catalan Government and will be used to support the new policies and strategies on Social Services in the post-COVID19 period. In this paper, the results of the project INSESS-COVID19 are presented, as part of a special call owing to help in the COVID19 crisis in Catalonia. The technological infrastructure and methodology developed in this project allows the quick screening of a territory for a quick a reliable diagnosis in front of an unexpected situation by providing relevant decisional information to support informed decision-making and strategy and policy design. One of the challenges of the project was to extract valuable information from direct participatory processes where specific target profiles of citizens are consulted and to distribute the participation along the whole territory. Having a lot of variables with a moderate number of citizens involved (in this case about 1000) implies the risk of violating statistical secrecy when multivariate relationships are analyzed, thus putting in risk the anonymity of the participants as well as their safety when vulnerable populations are involved, as is the case of INSESS-COVID19. In this paper, the entire data-driven methodology developed in the project is presented and the dealing of the small subgroups of population for statistical secrecy preserving described. The impact of the COVID-19 on tourist satisfaction is a particular relevant issue, due to the role that elements such as the prevention measures implemented at the destination might play. For this reason, this article examines tourist satisfaction during the peak tourist season of 2020 in a mature coastal destination in Catalonia in relation to safety and prevention measures imposed due to the COVID-19 pandemic. We use explanatory factor analysis and partial least squares path modelling for comparing the determinants of tourist satisfaction prior and during the pandemic based on surveys conducted in 2019 (N = 1556) and 2020 (N = 2220). The results suggest that the determinants of overall tourist satisfaction in 2019 remained significant in 2020. Moreover, although tourists especially appreciated feeling safe in 2020, our results suggest that such a perception was unrelated to measures to prevent COVID-19. The paper raises concerns towards the management of situations such as the pandemic in tourist destinations, as a proper balance must be found between the need of making visitors feel safe, and avoiding measures that can be felt as invasive or annoying, hampering the tourist experience. The lockdown measures in Spain due to COVID-19 social measures showed a wide decrease in the urban noise levels observed. This paper presents an analysis of the noise levels in Girona, a 100 000 citizen city in the North-East of Catalonia (Spain). We present the LAeq levels in four different locations from January 2020 to June 2020, including all the stages of the lockdown. Several comparisons are conducted with the monitoring data available from the previous years (2019, 2018, and 2017, when available). This analysis is part of the project "Sons al Balcó," which aims to draw the soundscape of Catalonia during the lockdown. The results of the analysis in Girona show drastic LAeq changes especially in nightlife areas of the city, moderate LAeq changes in commercial and restaurants areas, and low LAeq changes in dense traffic areas. Based on a SEIRD model (Susceptible, Exposed, Infective, Recovered and Deceased) for COVID-19 infection with a new parametrization using a high infection rate, and a low fatality, we define the model in System Dynamics, Python, and Specification and Description Language (SDL). The different implementations obtained can be improved depending on the capabilities of the approach and, more interestingly, can be used to improve the Validation and Verification processes. In this paper, we are focused on describing how this comparison with other models' validation processes allows us to find the parameters of the system dynamics model, hence the parameters of the pandemic. This is a crucial element, specifically in this case, because the data are not complete or validated for different reasons. We use using existing data from Korea and Spain and showing that the proposed method and the obtained parametrization for the model fit with the empirical evidence. We discuss some implications of the validation process and the model parametrization. Patients with cancer were excluded from pivotal randomized clinical trials of COVID-19 vaccine products, and available observational evidence on vaccine effectiveness (VE) focused mostly on mild, and not severe COVID-19, which is the ultimate goal of vaccination for high-risk groups. Here, using primary care electronic health records from Catalonia, Spain (SIDIAP), we built two large cohorts of vaccinated and matched control cancer patients with a primary vaccination scheme (n = 184,744) and a booster (n = 108,534). Most patients received a mRNA-based product in primary (76.2%) and booster vaccination (99.9%). Patients had 51.8% (95% CI 40.3%−61.1%) and 58.4% (95% CI 29.3%−75.5%) protection against COVID-19 hospitalization and COVID-19 death respectively after full vaccination (two-doses) and 77.9% (95% CI 69.2%−84.2%) and 80.2% (95% CI 63.0%−89.4%) after booster. Compared to primary vaccination, the booster dose provided higher peak protection during follow-up. Calibration of VE estimates with negative outcomes, and sensitivity analyses with slight different population and COVID-19 outcomes definitions provided similar results. Our results confirm the role of primary and booster COVID-19 vaccination in preventing COVID-19 severe events in patients with cancer and highlight the need for the additional dose in this population. Observational studies are needed to understand the effectiveness of COVID-19 vaccines in groups not included in randomised controlled trials, such as patients with cancer. Background Coronavirus disease 2019 (COVID-19) disproportionately affects migrants and ethnic minorities, including those with human immunodeficiency virus (HIV). Comprehensive studies are needed to understand the impact and risk factors. Methods Using data from the PISCIS cohort of people with HIV (PWH) in Catalonia, Spain, we investigated COVID-19 outcomes and vaccination coverage. Among 10 640 PWH we compared migrants and non-migrants assessing rates of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) testing, diagnosis, and associated clinical outcomes through propensity score matching and multivariable Cox regression. Results The cohort (mean age, 43 years; 83.5% male) included 57.4% (3053) Latin American migrants. Migrants with HIV (MWH) had fewer SARS-CoV-2 tests (67.8% vs 72.1%, P <.0001) but similar COVID-19 diagnoses (29.2% vs 29.4%, P =.847) compared to Spanish natives. Migrants had lower complete vaccination (78.9% vs 85.1%, P <.0001) and booster doses (63.0% vs 65.5%, P =.027). COVID-19 hospitalizations (8.1% vs 5.1%, P <.0001) and intensive care unit (ICU) admissions (2.9% vs 1.2%, P <.0001) were higher among migrants, with similar hospitalization duration (5.5 vs 4.0 days, P =.098) and mortality (3 [0.2%] vs 6 [0.4%], P =.510). Age ≥40 years, CD4 counts <200 cells/μL, ≥2 comorbidities, and incomplete/nonreception of the SARS-CoV-2 vaccine increased the risk of severe COVID-19 among migrants. Conclusions MWH had lower rates of SARS-CoV-2 testing and vaccination coverage, although the rates of COVID-19 diagnosis were similar between migrants and non-migrants. Rates of COVID-19–associated hospitalizations and ICU admissions were higher among migrants in comparison with non-migrants, with similar hospitalization duration and mortality. Persistent symptoms following the acute phase of COVID-19 present a major burden to both the affected and the wider community. We conducted a cohort study including over 856,840 first COVID-19 cases, 72,422 re-infections and more than 3.1 million first negative-test controls from primary care electronic health records from Spain and the UK (Sept 2020 to Jan 2022 (UK)/March 2022 (Spain)). We characterised post-acute COVID-19 symptoms and identified key symptoms associated with persistent disease. We estimated incidence rates of persisting symptoms in the general population and among COVID-19 patients over time. Subsequently, we investigated which WHO-listed symptoms were particularly differential by comparing their frequency in COVID-19 cases vs. matched test-negative controls. Lastly, we compared persistent symptoms after first infections vs. reinfections.Our study shows that the proportion of COVID-19 cases affected by persistent post-acute COVID-19 symptoms declined over the study period. Risk for altered smell/taste was consistently higher in patients with COVID-19 vs test-negative controls. Persistent symptoms were more common after reinfection than following a first infection. More research is needed into the definition of long COVID, and the effect of interventions to minimise the risk and impact of persistent symptoms. Post-acute COVID-19 condition is difficult to quantify because it includes a range of symptoms that may have other causes. In this study, the authors use primary care data from England and Catalonia, Spain, to estimate the incidence of the condition and identify symptoms that occur more frequently following infection than in uninfected controls. Purpose: The aim of this research is to analyze the academic literature on online grocery shopping to identify a set of variables to assess by means of a validated survey the shopping habits of the population of Catalonia, starting from COVID-19, in a representative sample of the population in the 2020-2021 pandemic period. Design/methodology: The methodology used for this research is qualitative and quantitative, since a validated survey is carried out with a representative sample of the population of Catalonia and subsequently, using a quantitative methodology, the answers of the questionnaire are analyzed using descriptive statistics to interpret the results and reach conclusions. Findings: More people are planning to shop online in the future because of COVID-19 than not people who have shopped online for the first time during the pandemic. Users who have increased their online grocery shopping during the pandemic also intend to use online grocery shopping in the future. Because of COVID-19, the adoption of online grocery shopping has advanced by a few years. However, it must be taken into account that the consumer still perceives risks, such as the fact that many people want to see fresh products before buying them. Originality/value: The most relevant contribution of this work is related to show how online food shopping is for customers and to value the shopping experience in times of pandemic. Purpose: The primary aim of this work was to convert the Information System for Research in Primary Care (SIDIAP) from Catalonia, Spain, to the Observational Medical Outcomes Partnership (OMOP) Common Data Model (CDM). Our second aim was to provide a descriptive analysis of COVID-19-related outcomes among the general population.Patients and Methods: We mapped patient-level data from SIDIAP to the OMOP CDM and we performed more than 3,400 data quality checks to assess its readiness for research. We established a general population cohort as of the 1st March 2020 and identified outpatient COVID-19 diagnoses or tested positive for, hospitalised with, admitted to intensive care units (ICU) with, died with, or vaccinated against COVID-19 up to 30th June 2022.Results: After verifying the high quality of the transformed dataset, we included 5,870,274 individuals in the general population cohort. Of those, 604,472 had either an outpatient COVID-19 diagnosis or positive test result, 58,991 had a hospitalisation, 5,642 had an ICU admission, and 11,233 died with COVID-19. A total of 4,584,515 received a COVID-19 vaccine. People who were hospitalised or died were more commonly older, male, and with more comorbidities. Those admitted to ICU with COVID-19 were generally younger and more often male than those hospitalised and those who died.Conclusion: We successfully transformed SIDIAP to the OMOP CDM. From this dataset, a general population cohort of 5.9 million individuals was identified and their COVID-19-related outcomes over time were described. The COVID-19 pandemic focused public attention on youth non-compliance with restrictions, but the social and health factors underpinning this behaviour were overlooked. Hereby, we considered the complex relationships between age (16–29 vs. 30+), non-compliance, socioeconomic vulnerabilities and poor mental health using a gender perspective. Data were derived from the 'Survey on the impact of COVID-19', fielded on 11–15 April 2020 in Catalonia. In a non-probabilistic sample of 14,123 individuals, we performed gender-stratified mediation analyses using Structural Equation Models. The results suggest that youth was related to non-compliance among men, but not among women. Nonetheless, young women were more frequently fired than older women, which enhanced their likelihood of non-compliance. Mental health outcomes were poorer among young people but did not relate to youth non-compliance. Finally, adverse socioeconomic situations related to non-compliance and poor mental health, but this was not an idiosyncrasy of young people. Lessons from the first COVID-19 wave suggest that young age intersects with gender and socioeconomic factors in determining non-compliance. This highlights the need to incorporate a gender and socioeconomic inequalities perspective in the design and evaluation of policies that tackle non-compliance among young people. This research studies the evolution of COVID-19 crude incident rates, effective reproduction number R(t) and their relationship with incidence spatial autocorrelation patterns in the 19 months following the disease outbreak in Catalonia (Spain). A cross-sectional ecological panel design based on n = 371 health-care geographical units is used. Five general outbreaks are described, systematically preceded by generalized values of R(t) > 1 in the two previous weeks. No clear regularities concerning possible initial focus appear when comparing waves. As for autocorrelation, we identify a wave's baseline pattern in which global Moran's I increases rapidly in the first weeks of the outbreak to descend later. However, some waves significantly depart from the baseline. In the simulations, both baseline pattern and departures can be reproduced when measures aimed at reducing mobility and virus transmissibility are introduced. Spatial autocorrelation is inherently contingent on the outbreak phase and is also substantially modified by external interventions affecting human behavior. [ABSTRACT FROM AUTHOR]

The spread of the SARS-CoV-2 modeling is a challenging problem because of its complex nature and lack of information regarding certain aspects. In this paper, we explore a Digital Twin approach to model the pandemic situation in Catalonia. The Digital Twin is composed of three different dynamic models used to perform the validations by a Model Comparison approach. We detail how we use this approach to obtain knowledge regarding the effects of the nonpharmaceutical interventions and the problems we faced during the modeling process. We use Specification and Description Language (SDL) to represent the compartmental forecasting model for the SARS-CoV-2. Its graphical notation simplifies the different specialists' understanding of the model hypotheses, which must be validated continuously following a Solution Validation approach. This model allows the successful forecasting of different scenarios for Catalonia. We present some formalization details, discuss the validation process and present some results obtained from the validation model discussion, which becomes a digital twin of the pandemic in Catalonia. Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) is a strain of coronavirus that causes COVID-19 (coronavirus disease 2019), the respiratory illness responsible for the on-going COVID-19 pandemic. In March 2020, it was declared global pandemic, causing millions of deaths. An evident tendency of global pharmaceutical consumption due to COVID-19 pandemic should be seen worldwide, and this increase might suppose an environmental threat. Pharmaceuticals administrated at home or in pharmacies are excreted by faeces and urine after consumption, and wastewater treatment plants (WWTPs) are not able to remove all pharmaceuticals residues that eventually will end up in the aquatic media (rivers and sea). For this reason, analytical techniques such as liquid chromatography coupled to tandem mass spectrometry (LC–MS/MS) have become prominent to identify and quantify pharmaceuticals residues in aquatic matrices. In view of the scarce data on the occurrence of pharmaceuticals used as COVID-19 treatment, the aim of the present study was to evaluate the presence of these class of pharmaceuticals in river water which were dexamethasone, prednisone, ciprofloxacin, levofloxacin, remdesivir, ritonavir, lopinavir, acetaminophen, hydroxychloroquine, chloroquine and cloperastine, their toxicity in the aquatic environment using D. magna and to perform an exhaustive risk assessment in seven points of the Llobregat river basin. Dexamethasone, cloperastine and acetaminophen were the pharmaceuticals with higher concentrations, showing mean levels between 313 and 859 ng L−1. [ABSTRACT FROM AUTHOR]

Background: Since March of 2020, the scientific community has been engaged a marathon to answer the different questions that COVID-19 pandemic has brought. During this time, Ethics Committees played an important role in reviewing the research protocols, COVID-19 or not, ensuring that the quality of scientific research is not relaxed by the hasty need for answers. Methods: Descriptive study from January 2019 to December 2021, comparing COVID-19 protocols to those not COVID-19 related protocols and comparing the work overload. Variables related to the characteristics of the research protocols (i.e. study design, funding...), the principal investigators (gender, PhD degree, professional role...) and outcomes of the Ethics Committee process (requirements of modifications and time until approval) were analyze. Results: The number of sessions increased during COVID-19 pandemics (12 in 2019, 25 in 2020 and 18 in 2021). In total 751 protocols were evaluated during the study period; 513 (68.3%) had an observational design and 434 (57.8%) had no funding. The principal investigator was a woman in 491 (65.4%) studies and a General Practitioner in 330 (43.9%). The mean of the days until the protocol approval was 42.09 days (SD 60.2) with a decrease of 20.1 days from 2019 to 2021. A total of 614 (81.7%) protocols were approved, 336 (54.7%) within the first month after their initial evaluation. Less than half of the protocols were COVID-19 related (208, 44.3%). The COVID-19 protocols main topics were impact on the population (71, 34.1%); and COVID-19 pharmacological treatments (including vaccines) showed a higher increase in 2021 (37, 30.3%). Conclusions: Despite the work overload during the pandemic due to the increase in the number of meetings and protocols, the IDIAPJGol EC reviewed all of them (COVID-19 or not) adapting to the new situation but according to its criteria of good practices to provide a quick response in the EC opinion. In Primary Health Care the most study designs have been observational studies, many of them with no funding and led by GPs. In 2021 there was an increase in the number of protocols focused on drugs, most likely related to COVID-19 vaccines studies. [ABSTRACT FROM AUTHOR]

This research aims to better understand the circumstances of museums in times of COVID-19 and investigate the reopening of museums as physical spaces for visitors??? experience. The influence of visitors??? sociodemographic and tripographic characteristics on visitor behavior, safety measures implemented by museums, and visitation experience were examined in two major art museums in Catalonia (Spain): the Dal??-Theatre Museum and the Picasso Museum. Participants (879) were selected following systematic random sampling at the museum exit gates. A quantitative analysis of the data was performed using Pearson and ANOVA and the participants??? profiles are presented. Findings indicate significant relationships between visitors??? characteristics and visitor behavior, safety measures, and visitation experience, with place of residence showing a positive influence on all three. Museums complied with specific safety regulations due to COVID-19, and in this context, the findings revealed that visitors slightly changed their visitor behavior in museums. Most visitors positively evaluated the key aspects of museum management relating to health and safety and social distancing. Age, nationality, and personal experience were found to influence visitor behavior in museums. Specific effects of each characteristic are further analyzed. Theoretical and practical implications and suggestions for future research studies are also discussed. Marketing activities will benefit from knowing, in detail, the visitors??? characteristics–information that can be used to target niches. Background: The study of impact of lockdowns on individual health-related behaviors has produced divergent results.Purpose: To identify patterns of change in multiple health-related behaviors analyzed as a whole, and their individual determinants.Methods: Between March and August 2020, we collected data on smoking, alcohol, physical activity, weight, and sleep in a population-based cohort from Catalonia who had available pre-pandemic data. We performed multiple correspondence and cluster analyses to identify patterns of change in health-related behaviors and built multivariable multinomial logistic regressions to identify determinants of behavioral change.Results: In 10,032 participants (59% female, mean (SD) age 55 (8) years), 8,606 individuals (86%) modified their behavior during the lockdown. We identified five patterns of behavioral change that were heterogeneous and directed both towards worsening and improvement in diverse combinations. Patterns ranged from "global worsening" (2,063 participants, 21%) characterized by increases in smoking, alcohol consumption, and weight, and decreases in physical activity levels and sleep time, to "improvement" (2,548 participants, 25%) characterized by increases in physical activity levels, decreases in weight and alcohol consumption, and both increases and decreases in sleep time. Being female, of older age, teleworking, having a higher education level, assuming caregiving responsibilities, and being more exposed to pandemic news were associated with changing behavior (all p < .05), but did not discriminate between favorable or unfavorable changes.Conclusions: Most of the population experienced changes in health-related behavior during lockdowns. Determinants of behavior modification were not explicitly associated with the direction of changes but allowed the identification of older, teleworking, and highly educated women who assumed caregiving responsibilities at home as susceptible population groups more vulnerable to lockdowns. The aim is to analyse that, during the current pandemic situation, the reduction in the number of cases of influenza suggests that the non-pharmaceutical interventions (NPIs) applied to contain the expansion of SARS-CoV-2 also affect the influenza expansion. We analyse the interaction of influenza and SARS-CoV-2 spread based on an extended SEIRD model for the Catalonia region in Spain. We show that the dynamic evolution of the spread of SARS-CoV-2 and influenza generates a small interference. This interference causes a small reduction in the number of cases of seasonal influenza, reducing its expansion over the population. Other elements like the face mask mandates, social distancing and hand cleaning boost the reduction in both expansions. Influenza expansion will present a small reduction in the number of cases due to the interaction with SARS-CoV-2 expansion but mainly because the NPIs applied to the population. Wide tensions regarding the organization of nation-state power have been triggered over the last years in the UK and Spain. By contrast, in the UK, (i) the plebiscite on Scottish Independence has been characterized since 2014 so far by a regular hegemony of the SNP in Scotland, and (ii) more recently, distinct resilient responses to tackle COVID-19 have dramatically shifted perceptions about the potential constitutional arrangements in Wales partially opposing a state-centric vision of the UK. By contrast, the role played by the constitutionally illegal but socially constitutive referendum in Catalonia on 1 October 2017, remarkably provoked the re-emergence of the Spanish far-right narrative through the surge of the new political party called Vox. In both cases, the urban in Glasgow, Cardiff, and Barcelona has been shaping various oppositions to state-centric agendas, and such oppositions have shaped elections in the UK and Spain. This article sheds light on the distinct, emerging, and emancipatory urban citizenship regimes in Catalonia, Scotland, and Wales, particularly illustrating the roles that Barcelona, Glasgow, and Cardiff, respectively, are playing in articulating a counter-reaction by rescaling a state-centric vision. Population-based studies can provide important evidence on the safety of COVID-19 vaccines. Here we compare rates of thrombosis and thrombocytopenia following vaccination against SARS-CoV-2 with the background (expected) rates in the general population. In addition, we compare the rates of the same adverse events among persons infected with SARS-CoV-2 with background rates. Primary care and linked hospital data from Catalonia, Spain informed the study, with participants vaccinated with BNT162b2 or ChAdOx1 (27/12/2020-23/06/2021), COVID-19 cases (01/09/2020-23/06/2021) or present in the database as of 01/01/2017. We included 2,021,366 BNT162b2 (1,327,031 with 2 doses), 592,408 ChAdOx1, 174,556 COVID-19 cases, and 4,573,494 background participants. Standardised incidence ratios for venous thromboembolism were 1.18 (95% CI 1.06-1.32) and 0.92 (0.81-1.05) after first- and second dose BNT162b2, and 0.92 (0.71-1.18) after first dose ChAdOx1. The standardised incidence ratio for venous thromboembolism in COVID-19 was 10.19 (9.43-11.02). Standardised incidence ratios for arterial thromboembolism were 1.02 (0.95-1.09) and 1.04 (0.97-1.12) after first- and second dose BNT162b2, 1.06 (0.91-1.23) after first-dose ChAdOx1 and 4.13 (3.83-4.45) for COVID-19. Standardised incidence ratios for thrombocytopenia were 1.49 (1.43-1.54) and 1.40 (1.35-1.45) after first- and second dose BNT162b2, 1.28 (1.19-1.38) after first-dose ChAdOx1 and 4.59 (4.41- 4.77) for COVID-19. While rates of thrombosis with thrombocytopenia were generally similar to background rates, the standardised incidence ratio for pulmonary embolism with thrombocytopenia after first-dose BNT162b2 was 1.70 (1.11-2.61). These findings suggest that the safety profiles of BNT162b2 and ChAdOx1 are similar, with rates of adverse events seen after vaccination typically similar to background rates. Meanwhile, rates of adverse events are much increased for COVID-19 cases further underlining the importance of vaccination. The Sentinel Schools project was designed to monitor and evaluate the epidemiology of COVID-19 in Catalonia, gathering evidence for health and education policies to inform the development of health protocols and public health interventions to control of SARS-CoV-2 infection in schools. The aim of this study was to estimate the prevalence and incidence of SARS-CoV-2 infections and to identify their determinants among students and staff during February to June in the academic year 2020–2021. We performed two complementary studies, a cross-sectional and a longitudinal component, using a questionnaire to collect nominal data and testing for SARS-CoV-2 detection. We describe the results and perform a univariate and multivariate analysis. The initial crude seroprevalence was 14.8% (95% CI: 13.1–16.5) and 22% (95% CI: 18.3–25.8) for students and staff respectively, and the active infection prevalence was 0.7% (95% CI: 0.3–1) and 1.1% (95% CI: 0.1–2). The overall incidence for persons at risk was 2.73 per 100 person-month and 2.89 and 2.34 per 100 person-month for students and staff, respectively. Socioeconomic, self-reported knowledge, risk perceptions and contact pattern variables were positively associated with the outcome while sanitary measure compliance was negatively associated, the same significance trend was observed in multivariate analysis. In the longitudinal component, epidemiological close contact with SARS-CoV-2 infection was a risk factor for SARS-CoV-2 infection while the highest socioeconomic status level was protective as was compliance with sanitary measures. The small number of active cases detected in these schools suggests a low transmission among children in school and the efficacy of public health measures implemented, at least in the epidemiological scenario of the study period. The major contribution of this study was to provide results and evidence that help analyze the transmission dynamic of SARS-CoV-2 and evaluate the associations between sanitary protocols implemented, and measures to avoid SARS-CoV-2 spread in schools. The effectiveness of noninvasive respiratory support in severe COVID-19 patients is still controversial. We aimed to compare the outcome of patients with COVID-19 pneumonia and hypoxemic respiratory failure treated with high-flow oxygen administered via nasal cannula (HFNC), continuous positive airway pressure (CPAP) or noninvasive ventilation (NIV), initiated outside the intensive care unit (ICU) in 10 university hospitals in Catalonia, Spain. We recruited 367 consecutive patients aged ≥ 18 years who were treated with HFNC (155, 42.2%), CPAP (133, 36.2%) or NIV (79, 21.5%). The main outcome was intubation or death at 28 days after respiratory support initiation. After adjusting for relevant covariates and taking patients treated with HFNC as reference, treatment with NIV showed a higher risk of intubation or death (hazard ratio 2.01; 95% confidence interval 1.32–3.08), while treatment with CPAP did not show differences (0.97; 0.63–1.50). In the context of the pandemic and outside the intensive care unit setting, noninvasive ventilation for the treatment of moderate to severe hypoxemic acute respiratory failure secondary to COVID-19 resulted in higher mortality or intubation rate at 28 days than high-flow oxygen or CPAP. In this study, changes in air quality by NO2, O3, and PM10 in Barcelona metropolitan area and other parts of Catalonia during the COVID-19 lockdown with respect to pre-lockdown and to previous years (2018 and 2019) were evaluated. Selected air monitoring stations included 3 urban (Gràcia, Vall d'Hebron, and Granollers), 1 control site (Fabra Observatory), 1 semi-urban (Manlleu), and 3 rural (Begur, Bellver de Cerdanya, and Juneda). NO2 lockdown levels showed a diminution, which in relative terms was maximum in two rural stations (Bellver de Cerdanya, − 63% and Begur, − 61%), presumably due to lower emissions from the ceasing hotel and ski resort activities during eastern holidays. In absolute terms and from an epidemiologic perspective, decrease in NO2, also reinforced by the high amount of rainfall registered in April 2020, was more relevant in the urban stations around Barcelona. O3 levels increased in the transited urban stations (Gràcia, + 42%, and Granollers, + 64%) due to the lower titration effect by NOx. PM10 lockdown levels decreased, mostly in Gràcia, Vall d'Hebron, and Granollers (− 35, − 39%, and − 39%, respectively) due to traffic depletion (− 90% in Barcelona's transport). Correlation among mobility index in Barcelona (− 100% in retail and recreation) and contamination was positive for NO2 and PM10 and negative for O3 (P < 0.001). Satellite images evidenced two hotspots of NO2 in Spain (Madrid and Barcelona) in April 2018 and 2019 that disappeared in 2020. Overall, the benefits of lockdown on air quality in Catalonia were evidenced with NO2, O3 and PM10 levels below WHOAQG values in most of stations opposed to the excess registered in previous years. In this article we study the discursive construction of the EU managerial role in the early stages of the Covid-19 pandemic in four Spanish newspapers: two published in mainland Spain – El Mundo and El País – and two published in Catalonia – La Vanguardia and ARA. By doing a qualitative study of newspapers, this article aims to identify which discourse strategies are used when informing about the actions and decisions taken in European political and economic fora. The analysis identifies the three main generic frames which are used in news pieces: morality (mostly as a call for solidarity), economic (mostly as preventing possible harm), and conflict (focused on divide(s) within (EU)rope). The activation of these frames in the news pieces contribute to an implicit legitimation of the existence of (EU)rope as based on two core values – solidarity and cooperation – together with a delegitimation of the actions performed by some European institutions. Background: Before the COVID-19 pandemic, Sexually transmitted infections (STIs) were increasing in Europe, and Spain and Catalonia were not an exception. Catalonia has been one of the regions with the highest number of COVID-19 confirmed cases in Spain. The objective of this study was to estimate the magnitude of the decline, due to the COVID-19 pandemic, in the number of STI confirmed cases in Catalonia during the lockdown and de-escalation phases.Methods: Interrupted time series analysis was performed to estimate the magnitude of decline in the number of STI reported confirmed cases - chlamydia, gonorrhoea, syphilis, and lymphogranuloma venereum- in Catalonia since lockdown with historical data, from March 13th to August 1st 2020, comparing the observed with the expected values.Results: We found that since the start of COVID-19 pandemic the number of STI reported cases was 51% less than expected, reaching an average of 56% during lockdown (50% and 45% during de-escalation and new normality) with a maximum decrease of 72% for chlamydia and minimum of 22% for syphilis. Our results indicate that fewer STIs were reported in females, people living in more deprived areas, people with no previous STI episodes during the last three years, and in the HIV negative.Conclusions: The STI notification sharp decline was maintained almost five months after lockdown started, well into the new normality. This fact can hardly be explained without significant underdiagnosis and underreporting. There is an urgent need to strengthen STI/HIV diagnostic programs and services, as well as surveillance, as the pandemic could be concealing the real size of the already described re-emergence of STIs in most of the European countries. The natural history of COVID-19 and predictors of mortality in older adults need to be investigated to inform clinical operations and healthcare policy planning. A retrospective study took place in 80 long-term nursing homes in Catalonia, Spain collecting data from March 1st to May 31st, 2020. Demographic and clinical data from 2,092 RT-PCR confirmed cases of SARS-CoV-2 infection were registered, including structural characteristics of the facilities. Descriptive statistics to describe the demographic, clinical, and molecular characteristics of our sample were prepared, both overall and by their symptomatology was performed and an analysis of statistically significant bivariate differences and constructions of a logistic regression model were carried out to assess the relationship between variables. The incidence of the infection was 28%. 71% of the residents showed symptoms. Five major symptoms included: fever, dyspnea, dry cough, asthenia and diarrhea. Fever and dyspnea were by far the most frequent (50% and 28%, respectively). The presentation was predominantly acute and symptomatology persisted from days to weeks (mean 9.1 days, SD = 10,9). 16% of residents had confirmed pneumonia and 22% required hospitalization. The accumulated mortality rate was 21.75% (86% concentrated during the first 28 days at onset). A multivariate logistic regression analysis showed a positive predictive value for mortality for some variables such as age, pneumonia, fever, dyspnea, stupor refusal to oral intake and dementia (p<0.01 for all variables). Results suggest that density in the nursing homes did not account for differences in the incidence of the infection within the facilities. This study provides insights into the natural history of the disease in older adults with high dependency living in long-term nursing homes during the first pandemic wave of March-May 2020 in the region of Catalonia, and suggests that some comorbidities and symptoms have a strong predictive value for mortality. Background: Pneumonia is one of the complications of COVID-19. Primary care electronic health records (EHR) have shown the utility as a surveillance system. We therefore analyse the trends of pneumonia during two waves of COVID-19 pandemic in order to use it as a clinical surveillance system and an early indicator of severity.Methods: Time series analysis of pneumonia cases, from January 2014 to December 2020. We collected pneumonia diagnoses from primary care EHR, a software system covering > 6 million people in Catalonia (Spain). We compared the trend of pneumonia in the season 2019-2020 with that in the previous years. We estimated the expected pneumonia cases with data from 2014 to 2018 using a time series regression adjusted by seasonality and influenza epidemics.Results: Between 4 March and 5 May 2020, 11,704 excess pneumonia cases (95% CI: 9909 to 13,498) were identified. Previously, we identified an excess from January to March 2020 in the population older than 15 years of 20%. We observed another excess pneumonia period from 22 october to 15 november of 1377 excess cases (95% CI: 665 to 2089). In contrast, we observed two great periods with reductions of pneumonia cases in children, accounting for 131 days and 3534 less pneumonia cases (95% CI, 1005 to 6064) from March to July; and 54 days and 1960 less pneumonia cases (95% CI 917 to 3002) from October to December.Conclusions: Diagnoses of pneumonia from the EHR could be used as an early and low cost surveillance system to monitor the spread of COVID-19. The petrochemical industry has made the economic development of many local communities possible, increasing employment opportunities and generating a complex network of closely-related secondary industries. However, it is known that petrochemical industries emit air pollutants, which have been related to different negative effects on mental health. In addition, many people around the world are being exposed to highly stressful situations deriving from the COVID-19 pandemic and the lockdowns adopted by national and regional governments. The present study aims to analyse the possible differential effects on various psychological outcomes (stress, anxiety, depression and emotional regulation strategies) stemming from the COVID-19 pandemic and consequent lockdown experienced by individuals living near an important petrochemical complex and subjects living in other areas, nonexposed to the characteristic environmental pollutants emitted by these kinds of complex. The sample consisted of 1607 subjects who answered an ad hoc questionnaire on lockdown conditions, the Perceived Stress Scale (PSS), the Hospital Anxiety and Depression Scale (HADS), the Barratt Impulsivity Scale (BIS) and the Emotional Regulation Questionnaire (ERQ). The results indicate that people living closer to petrochemical complexes reported greater risk perception [K = 73.42, p < 0.001, with a medium size effect (η2 = 0.061)]. However, no significant relationship between psychological variables and proximity to the focus was detected when comparing people living near to or far away from a chemical/petrochemical complex. Regarding the adverse psychological effects of the first lockdown due to COVID-19 on the general population in Catalonia, we can conclude that the conditions included in this survey were mainly related to changes in the participants' impulsivity levels, with different total impulsivity scores being obtained if they had minors in their care (p<0.001), if they had lost their jobs, if they were working (p<0.001), if they were not telecommuting (p<0.001), if they went out to work (p<0.001) or if they established routines (p = 0.009). However, we can also be fairly certain that the economic effects are going to be worse than those initially detected in this study. More research will be necessary to corroborate our results. This article is the result of a campaign done during the COVID-19 lockdown in Catalonia. The Television of Catalonia audience was involved in an action to inform about the weather from their own homes by posting Twitter videos. Some of the videos were shown on air in the weather segment of the television station's main news programs. We have correlated participation in the campaign with meteorological and public health data and found that weather is related to the mood of people when using social media platforms such as Twitter. Importance: The rapid pandemic expansion of the disease caused by the new SARS-CoV-2 virus has compromised health systems worldwide. Knowledge of prognostic factors in affected patients can help optimize care. Objective: The objective of this study was to analyze the relationship between the prognosis of COVID-19 and the form of presentation of the disease, the previous pathologies of patients and their chronic treatments. Design, participants and locations: This was an observational study on a cohort of 418 patients admitted to three regional hospitals in Catalonia (Spain). As primary outcomes, severe disease (need for oxygen therapy via nonrebreather mask or mechanical ventilation) and death were studied. Multivariate binary logistic regression models were performed to study the association between the different factors and the results. Results: Advanced age, male sex and obesity were independent markers of poor prognosis. The most frequent presenting symptom was fever, while dyspnea was associated with severe disease and the presence of cough with greater survival. Low oxygen saturation in the emergency room, elevated CRP in the emergency room and initial radiological involvement were all related to worse prognosis. The presence of eosinophilia (% of eosinophils) was an independent marker of less severe disease. Conclusions: This study identified the most robust markers of poor prognosis for COVID-19. These results can help to correctly stratify patients at the beginning of hospitalization based on the risk of developing severe disease. [ABSTRACT FROM AUTHOR]

Background: The rapid spread of the disease caused by the novel SARS-CoV-2 virus has led to the use of multiple therapeutic agents whose efficacy has not been previously demonstrated. The objective of this study was to analyze whether there is an association between the use of azithromycin and the evolution of the pulmonary disease or the time to discharge, in patients hospitalized with COVID-19. Methods: This was an observational study on a cohort of 418 patients admitted to three regional hospitals in Catalonia, Spain. As primary outcomes, we studied the evolution of SAFI ratio (oxygen saturation/fraction of inspired oxygen) in the first 48 hours of treatment and the time to discharge. The results were compared between patients treated and untreated with the study drug through subcohort analyses matched for multiple clinical and prognostic factors, as well as through analysis of non-matched subcohorts, using Cox multivariate models adjusted for prognostic factors. Results: There were 239 patients treated with azithromycin. Of these, 29 patients treated with azithromycin could be matched with an equivalent number of control patients. In the analysis of these matched subcohorts, SAFI at 48h had no significant changes associated to the use of azithromycin, though azithromycin treatment was associated with a longer time to discharge (10.0 days vs 6.7 days; log rank: p = 0.039). However, in the unmatched cohorts, the increased hospital stay associated to azithromycin use, was no significant after adjustment using Multivariate Cox regression models: hazard ratio 1.45 (IC95%: 0.88–2.41; p = 0.150). This study is limited by its small sample size and its observational nature; despite the strong pairing of the matched subcohorts and the adjustment of the Cox regression for multiple factors, the results may be affected by residual confusion. Conclusions: We did not find a clinical benefit associated with the use of azithromycin, in terms of lung function 48 hours after treatment or length of hospital stay. [ABSTRACT FROM AUTHOR]

It is paramount to expand the knowledge base and minimize the consequences of the pandemic caused by the new Coronavirus (SARS-Cov2). Spain is among the most affected countries that declared a countrywide lockdown. An ecological study is presented herein, assessing the trends for incidence, mortality, hospitalizations, Intensive Care Unit admissions, and recoveries per autonomous community in Spain. Trends were evaluated by the Joinpoint software. The timeframe employed was when the lockdown was declared on March 14, 2020. Daily percentage changes were also calculated, with CI = 95% and p<0.05. An increase was detected, followed by reduction, for the evaluated indicators in most of the communities. Approximately 18.33 days were required for the mortality rates to decrease. The highest mortality rate was verified in Madrid (118.89 per 100,000 inhabitants) and the lowest in Melilla (2.31). The highest daily percentage increase in mortality occurred in Catalonia. Decreasing trends were identified after approximately two weeks of the institution of the lockdown by the government. Immediately the lockdown was declared, an increase of up to 33.96% deaths per day was verified in Catalonia. In contrast, Ceuta and Melilla presented significantly lower rates because they were still at the early stages of the pandemic at the moment of lockdown. (1) Background: In epidemiological terms, it has been possible to calculate the savings in health resources and the reduction in the health effects of COVID vaccines. Conducting an economic evaluation, some studies have estimated its cost-effectiveness; the vaccination shows highly favorable results, cost-saving in some cases. (2) Methods: Cost–benefit analysis of the vaccination campaign in the North Metropolitan Health Region (Catalonia). An epidemiological model based on observational data and before and after comparison is used. The information on the doses used and the assigned resources (conventional hospital beds, ICU, number of tests) was extracted from administrative data from the largest primary care provider in the region (Catalan Institute of Health). A distinction was made between the social perspective and the health system. (3) Results: the costs of vaccination are estimated at 137 million euros (€48.05/dose administered). This figure is significantly lower than the positive impacts of the vaccination campaign, which are estimated at 470 million euros (€164/dose administered). Of these, 18% corresponds to the reduction in ICU discharges, 16% to the reduction in conventional hospital discharges, 5% to the reduction in PCR tests and 1% to the reduction in RAT tests. The monetization of deaths and cases that avoid sequelae account for 53% and 5% of total savings, respectively. The benefit/cost ratio is estimated at 3.4 from a social perspective and 1.4 from a health system perspective. The social benefits of vaccination are estimated at €116.67 per vaccine dose (€19.93 from the perspective of the health system). (4) Conclusions: The mass vaccination campaign against COVID is cost-saving. From a social perspective, most of these savings come from the monetization of the reduction in mortality and cases with sequelae, although the intervention is equally widely cost-effective from the health system perspective thanks to the reduction in the use of resources. We examined the correlation between previous antibiotic exposure and COVID-19 severity using a population-based observational matched cohort study with patient level data obtained for more than 5.8 million people registered in SIDIAP in Catalonia, Spain. We included all patients newly diagnosed with COVID-19 from March to June 2020 and identified all their antibiotic prescriptions in the previous two years. We used a composite severity endpoint, including pneumonia, hospital admission and death due to COVID-19. We examined the influence of high antibiotic exposure (>4 regimens), exposure to highest priority critically important antimicrobials (HPCIA) and recent exposure. Potential confounders were adjusted by logistic regression. A total of 280,679 patients were diagnosed with COVID-19, 146,656 of whom were exposed to at least one antibiotic course (52.3%) during the preceding two years. A total of 25,222 presented severe COVID-19 infection (9%), and the risk of severity was highest among those exposed to antibiotics (OR 1.12; 95% CI: 1.04–1.21). Among all individuals exposed to antibiotics, high, recent and exposure to HPCIAs were correlated with increased COVID severity (OR 1.19; 95% CI: 1.14–1.26; 1.41; 95% CI: 1.36–1.46; and 1.35; 95% CI: 1.30–1.40, respectively). COVID-19, the illness caused by infection with the novel coronavirus SARS-CoV-2, is a rapidly spreading global pandemic in urgent need of effective treatments. Here we present a comprehensive examination of the host- and virus-targeted functions of the flavonolignan silibinin, a potential drug candidate against COVID-19/SARS-CoV-2. As a direct inhibitor of STAT3—a master checkpoint regulator of inflammatory cytokine signaling and immune response—silibinin might be expected to phenotypically integrate the mechanisms of action of IL-6-targeted monoclonal antibodies and pan-JAK1/2 inhibitors to limit the cytokine storm and T-cell lymphopenia in the clinical setting of severe COVID-19. As a computationally predicted, remdesivir-like inhibitor of RNA-dependent RNA polymerase (RdRp)—the central component of the replication/transcription machinery of SARS-CoV-2—silibinin is expected to reduce viral load and impede delayed interferon responses. The dual ability of silibinin to target both the host cytokine storm and the virus replication machinery provides a strong rationale for the clinical testing of silibinin against the COVID-19 global public health emergency. A randomized, open-label, phase II multicentric clinical trial (SIL-COVID19) will evaluate the therapeutic efficacy of silibinin in the prevention of acute respiratory distress syndrome in moderate-to-severe COVID-19-positive onco-hematological patients at the Catalan Institute of Oncology in Catalonia, Spain.
