Facial nerve stimulation (FNS) is a potential complication which may affect the auditory performance of children with cochlear implants (CIs). We carried out an exploratory prospective observational study to investigate the effects of the electrical stimulation pattern on FNS reduction in young children with CI. Ten ears of seven prelingually deafened children with ages up to 6 years old who undergone a unilateral or bilateral CI surgery were included in this study. Electromyographic (EMG) action potentials from orbicularis oculi muscle were recorded using monopolar biphasic stimulation (ST1) and multi-mode monophasic stimulation with capacitive discharge (ST2). Presence of EMG responses, facial nerve stimulation thresholds (T-FNS) and EMG amplitudes were compared between ST1 and ST2. Intra-cochlear electrodes placement, cochlear-nerve and electrode-nerve distances were also estimated to investigate their effects on EMG responses. The use of ST2 significantly reduced the presence of intraoperative EMG responses compared to ST1. Higher stimulation levels were required to elicit FNS with ST2, with smaller amplitudes, compared to ST1. No and weak correlation was observed between cochlea-nerve and electrode-nerve distances and EMG responses, respectively. ST2 may reduce FNS in young children with CI. Differently from the electrical stimulation pattern, the cochlea-nerve and electrode-nerve distances seem to have limited effects on FNS in this population.

The Oticon Medical Neuro cochlear implant system includes the modes Opti Omni and Speech Omni, the latter providing beamforming (i.e., directional selectivity) in the high frequencies. Two studies compared sentence identification scores of adult cochlear implant users with Opti Omni and Speech Omni. In Study 1, a double-blind longitudinal crossover study, 12 new users trialed Opti Omni or Speech Omni (random allocation) for three months, and their sentence identification in quiet and noise (+10 dB signal-to-noise ratio) with the trialed mode were measured. The same procedure was repeated for the second mode. In Study 2, a single-blind study, 11 experienced users performed a speech identification task in quiet and at relative signal-to-noise ratios ranging from -3 to +18 dB with Opti Omni and Speech Omni. The Study 1 scores in quiet and in noise were significantly better with Speech Omni than with Opti Omni. Study 2 scores were significantly better with Speech Omni than with Opti Omni at +6 and +9 dB signal-to-noise ratios. Beamforming in the high frequencies, as implemented in Speech Omni, leads to improved speech identification in medium levels of background noise, where cochlear implant users spend most of their day. Purpose For cochlear implant users, the ability to use the telephone is often seen as an important landmark during rehabilitation and an indicator of cochlear implant benefit. The goal of this study was to develop a short questionnaire exploring the ability to use the telephone in cochlear implant users, named Telislife, and test it in a group of experienced users. Method This prospective multicenter study was based on the completion of self-administrated questionnaires. The Telislife includes 20 items using a 5-point Likert scale for answers. Speech recognition scores were obtained with monosyllabic word lists at 70 dB HL. Quality of life was evaluated with the Nijmegen Cochlear Implant Questionnaire. This study included 55 adult patients wearing a cochlear implant for over 1 year. Results The Telislife questionnaire showed excellent reliability (Cronbach's α = .91). A significant correlation was found between Telislife scores and Nijmegen Cochlear Implant Questionnaire scores ( r = .69, p < .001) and speech recognition scores ( r = .35, p = .007). Conclusion Given significant correlations between Telislife scores and both speech recognition and quality of life and given its short form, the Telislife questionnaire appears to be a reliable tool to evaluate cochlear implant outcomes in clinical practice. Objective The aim of the present study was to investigate the pupillary response to word identification in cochlear implant (CI) patients. Authors hypothesized that when task difficulty (i.e. addition of background noise) increased, pupil dilation markers such as the peak dilation or the latency of the peak dilation, would increase in CI users, as already observed in normal-hearing and hearing-impaired subjects. Methods Pupillometric measures in ten CI patients were combined to standard speech recognition scores used to evaluate CI outcomes, namely speech audiometry in quiet and in noise at +10 dB signal-to-noise ratio. The main outcome measures of pupillometry were mean pupil dilation, maximal pupil dilation, dilation latency and mean dilation during return to baseline or retention interval. Subjective hearing quality was evaluated by means of one self-reported fatigue questionnaire, and the Speech, Spatial and Qualities (SSQ) of Hearing scale. Results All pupil dilation data were transformed to percent change in event-related pupil dilation (ERPD, %). Analyses show that the peak amplitude for both mean pupil dilation and maximal pupil dilation were higher during the speech-in-noise test. Mean peak dilation was measured at 3.47 +/- 2.29 % noise vs. 2.19 +/- 2.46 in quiet and maximal peak value was detected at 9.17 +/- 3.25 % in noise vs. 8.72 +/- 2.93% in quiet. Concerning the questionnaires, the mean pupil dilation during the retention interval was significantly correlated with the spatial subscale score of the SSQ Hearing scale (r(8) = -.84, p =0023), and with the global score (r(8) = -.78, p=0.0018). Conclusions The analysis of pupillometric traces, obtained during speech audiometry in quiet and in noise in CI users, provided interesting information about the different processes engaged in this task. Pupillometric measures could be indicative of listening difficulty, phoneme intelligibility, and were correlated with general hearing experience as evaluated by the Speech, Spatial and Qualities of Hearing scale. These preliminary results show that pupillometry constitutes a promising tool to improve objective quantification of CI performance in clinical settings.

Objective This prospective longitudinal cohort study at six tertiary referral centers in Canada and Denmark describes the clinical efficiency and surgical safety of cochlear implantation with the Oticon Medical Neuro cochlear implant system, including the Neuro Zti implant, the EVO electrode array, and the Neuro One sound processor. Methods Patients were adult cochlear implant candidates with bilateral sensorineural hearing loss. Results The mean HINT scores in quiet pre-operatively and at 3, 6, and 12 months post-activation were 13%, 58%, 67%, and 72%, respectively, and in noise (+10 dB SNR) 13%, 46%, 53%, and 59%, respectively. The mean improvement from baseline to 6 months post-activation was 54% in quiet and 40% in noise. The surgical major complication incidence rate was 0% and the post-surgical major complication incidence rate (until 12 months post-activation) was 4%. There was no adverse event that was fatal, that required explantation, or that resulted in sound processor nonuse, and no implant failure. Conclusion Cochlear implantation with the Oticon Medical Neuro system enables speech identification both in quiet and in noise and audiologic outcomes continue to improve in the year following activation. Normal-hearing subjects listening to acoustic simulations of cochlear implants (CI) can obtain sentence recognition scores near 100% in quiet and in 10 dB signal-to-noise ratio (SNR) noise with acute exposure. However, average sentence recognition scores for real CI listeners are generally lower, even after months of experience, and there is a high degree of heterogeneity. Our aim was to identify the relative importance and strength of factors that prevent CI listeners from achieving early, 1-mo scores as high as those for normal-hearing-listener acoustic simulations.Sentence recognition scores (100 words/list, 65 dB SPL) using CI alone were collected for all adult unilateral CI listeners implanted in our center over a 5-yr period. Sentence recognition scores in quiet and in 10 dB SNR 8-talker babble, collected from 1 to 12 mo, were reduced to a single dependent variable, the "initial" score, via logarithmic regression. "Initial" scores equated to an improved estimate of 1-mo scores, and integrated the time to rise above zero score for poorer performing subjects. Demographic, device, and medical data were collected for 118 subjects who met standard CI candidacy criteria. Computed tomography of the electrode array allowing determination of the insertion depth as an angle, and the presence or absence of scala dislocation was available for 96 subjects. Predictive factors for initial scores were selected using stepwise multiple linear regression. The relative importance of predictive factors was estimated as partial r with a low bias method, and statistical significance tested with type II analysis of variance.The etiologies chronic otitis and autoimmune disease were associated with lower, widely variable sentence recognition scores in the long-term. More than 60% of CI listeners scored50/100 in quiet at 1 mo. Congenital hearing loss was associated with significantly lower initial scores in quiet (r 0.23, p0.001), as was longer duration of hearing loss (r 0.12, p0.001, -0.76 pts per year). Initial scores were negatively correlated with insertion depth (r 0.09, p0.001, -0.1 pts per degree), with the highest initial scores being obtained for insertion depths of 300° to 400°. A much greater proportion of scala dislocations was found for perimodiolar arrays compared with straight arrays. Scores were negatively correlated with the proportion of the active electrode array found in scala vestibuli for Nucleus perimodiolar devices (r 0.14, p0.01, coefficient -25). Similar overall results were obtained for sentence recognition scores in noise (+10 dB SNR). The intercept value for the obtained regression functions indicated that CI listeners with the least limiting factors generally scored ~95/100 in quiet and ~90/100 in noise. In addition, CI listeners with insertion angles as low as 315° to 360° could obtain sentence recognition scores80/100 even at 1 day after activation. Insertion depths of 360° were estimated to produce frequency-place mismatches of about one octave upward shift.Patient-related factors etiology and duration of deafness together explained ~40% of the variance in early sentence recognition scores, and electrode position factors ~20%. CI listeners with insertion depths of about one turn obtained the highest early sentence recognition scores in quiet and in noise, and these were comparable with those reported in the literature for normal-hearing subjects listening to 8 to 12 channel vocoder simulations. Differences between device brands were largely explained by differences in insertion depths. This indicates that physiological frequency-place mismatches of about one octave are rapidly accommodated by CI users for understanding sentences, between 1 day to 1 mo postactivation, and that channel efficiency may be significantly poorer for more deeply positioned electrode contacts.

International audience; Cochlear implantation in subjects with single-sided deafness (SSD) offers a unique opportunity to directly compare the percepts evoked by a cochlear implant (CI) with those evoked acoustically. Here, nine SSD-CI users performed a forcedchoice task evaluating the similarity of speech processed by their CI with speech processed by several vocoders presented to their healthy ear. In each trial, subjects heard two intervals: their CI followed by a certain vocoder in Interval 1 and their CI followed by a different vocoder in Interval 2. The vocoders differed either (i) in carrier type-(sinusoidal [SINE], bandfiltered noise [NOISE], and pulse-spreading harmonic complex) or (ii) in frequency mismatch between the analysis and synthesis frequency ranges-(no mismatch, and two frequency-mismatched conditions of 2 and 4 equivalent rectangular bandwidths [ERBs]). Subjects had to state in which of the two intervals the CI and vocoder sounds were more similar. Despite a large intersubject variability, the PSHC vocoder was judged significantly more similar to the CI than SINE or NOISE vocoders. Furthermore, the No-mismatch and 2-ERB mismatch vocoders were judged significantly more similar to the CI than the 4-ERB mismatch vocoder. The mismatch data were also interpreted by comparing spiral ganglion characteristic frequencies with electrode contact positions determined from postoperative computed tomography scans. Only one subject demonstrated a pattern of preference consistent with adaptation to the CI sound processor frequency-to-electrode allocation table and two subjects showed possible partial adaptation. Those subjects with adaptation patterns presented overall small and consistent frequency mismatches across their electrode arrays.

We previously established that under certain physiological conditions the sentence recognition scores in noise of cochlear implant (CI) users one month after activation are similar to those for normal listeners listening to 10–12 channel acoustic (vocoder) simulations (James et al., CIAP, Lake Tahoe, July 2017). We further analysed sentence recognition scores in noise over time in order to determine whether short term, 1-month scores predict long-term outcomes for sentence recognition in noise. Long-term sentence-in-noise data for fixed-SNR-level testing (10, 5, 2.5, 0 dB) were reduced to SNR50 values using a validated analytic sigmoid fit.
