Background: Depressive disorders are prevalent mental health conditions that significantly impair individuals' social functioning and communication competence. These challenges can exacerbate depressive symptoms, creating a vicious cycle that hinders recovery and overall well‐being. Despite the growing recognition of the impact of depressive disorders on social functioning and communication competence, there remains a significant gap in effective interventions addressing these specific areas. Aims: This research aims to investigate the effect of interpersonal effectiveness skill training on social functioning and communication competence among clients with depressive disorder. Design: A quasiexperimental research design. Setting: The research procedure was carried out in the outpatient clinic of Alexandria University Hospital. Subjects: The study group, which included 42 males and 18 females, and the control group had the same number of patients. Tools: After the preliminary assessment of depression, three instruments were used to collect data: a structured interview schedule that gathered sociodemographic and clinical information, the Scale of Communication Competence, and the Social Functioning Scale. Results: The study group showed a positive difference in their social functioning and communication competence score compared with the control group. Implication: The findings of this study have significant implications for the treatment and support of clients with depressive disorder by incorporating structured skill‐building programs into therapeutic regimens for individuals with depressive disorders. Background: In this study, we examined the ethical implications of Egypt's new clinical trial law, employing the ethical framework proposed by Emanuel et al. and comparing it to various national and supranational laws. This analysis is crucial as Egypt, considered a high-growth pharmaceutical market, has become an attractive location for clinical trials, offering insights into the ethical implementation of bioethical regulations in a large population country with a robust healthcare infrastructure and predominantly treatment-naïve patients. Methods: We conducted a comparative analysis of Egyptian law with regulations from Sweden and France, including the EU Clinical Trials Regulation, considering ethical human subject research criteria, and used a directed approach to qualitative content analysis to examine the laws and regulations. This study involved extensive peer scrutiny, frequent debriefing sessions, and collaboration with legal experts with relevant international legal expertise to ensure rigorous analysis and interpretation of the laws. Results: On the rating of the seven different principles (social and scientific values, scientific validity, fair selection of participants, risk-benefit ratio, independent review, informed consent and respect for participants) Egypt, France, and EU regulations had comparable scores. Specific principles (Social Value, Scientific Value, and Fair selection of participants) were challenging to directly identify due to certain regulations embodying 'implicit' principles more than explicitly stated ones. Conclusion: The analysis underscores Egypt's alignment with internationally recognized ethical principles, as outlined by Emanuel et al., through its comparison with French, Swedish, and EU regulations, emphasizing the critical need for Egypt to continuously refine its ethical regulations to safeguard participant protection and research integrity. Key issues identified include the necessity to clarify and standardize the concept of social value in research, alongside concerns regarding the expertise and impartiality of ethical review boards, pointing towards a broader agenda for enhancing research ethics in Egypt and beyond. Background: Rheumatic arthritis (RA) is an autoimmune progressive disease affecting one percentage of the general population Aim: This study aimed to evaluate the effect of health teaching for mothers' on quality of life for children with rheumatic arthritis. Design: A quasiexperimental design was used to conduct this study (pre &post-test). Setting: the study was conducted at Pediatric Rheumatic Fever Clinic of Tanta University Hospital. Subject and method: A quasi-experimental research design was used with a convenience sample of 60 mothers and their children with rheumatic arthritis. Data were collected using three tools: tool (I) Structured Interview Schedule sheet: it was be divided into two part: part (A) Socio demographic characteristics of studied mothers and their children, part (B) Mothers' knowledge and practice about rheumatic arthritis (Pre/Post): Tool (II) Quality of life domains for children with rheumatic arthritis (Pre/Post). Tool (III) FLACC behavioral pain assessment scale (Pre/Post). Results: The results revealed that more than a third of the total score of knowledge about rheumatic arthritis at pre-education compared to less than three quarters of them at post education, and one third of them had good total quality of life at preeducation compared to more than half of them at post education. Also, one third of them had mild pain at pre-education compared to that more than half of them at post education. Conclusion and Recommendations: it can be concluded that after health education was implemented, the quality of life for children with rheumatic arthritis improved. Establish continuous educational and training program for children and their mothers about rheumatic arthritis are recommended to increase their knowledge, quality of life, and management of RA and prevent its complication. Background: Dehydration is a serious public health concern occurred due to the imbalance between water intake and loss which increased in the climate change and Egypt's hot weather, lead to the morbidity and mortality rate raise among the older persons. Aim: To evaluate the effect of nursing educational program for improving older persons' hydration status. Subjects and Method: One group pre-post intervention research design. Setting: The study conducted at the (internal medicine and urology) clinics at the Assuit University hospitals. Sample: Total number of 130 older persons was included. Study tools: Contained four tools; Tool (I): Consisted of four parts; personal data, El-Gilany scale for measurement of socioeconomic status, health condition and knowledge of older persons regarding dehydration. Tool (II): Dehydration screening. Tool (III): Hydration knowledge scale and Tool (IV): Hydration facilitators and barriers attitude. Results: Mean ± SD and (range) of participant age: 69.28±7.65 (60-91). It was found that, more than half (56.9% &53.1% respectively) of them were male and had very low socioeconomic status. There was statistically significant effect of the educational program on the older persons' dehydration status; also there was improvement of older person's hydration & dehydration knowledge and attitude of older persons about hydration facilitators and barriers after the application of the educational program. Conclusion and recommendations: These results support that the nursing educational program improved the hydration status, hydration & dehydration knowledge and attitude toward hydration facilitators and barriers among older persons. Background: Constipation is an underestimated but common health problem worldwide, decreasing the quality of life. Functional constipation (FC) is a common pediatric problem, with reported prevalence ranging from 0.7 to 29.6%. In Egypt, there are no established guidelines for the treatment of constipation in children. Objectives: The aim of this study was to investigate the efficacy of polyethylene glycol (PEG) as a maintenance treatment for functional constipation (FC) in comparison with the classic treatment using (lactulose and senna) in children living in Egypt. Patients and method: This is a randomized single-blinded clinical trial study on pediatric patients who presented with functional constipation at the outpatient clinic of Cairo University Specialized Pediatric Hospital. The study was conducted on 80 children with functional constipation, who were divided into 2 groups: group 1 (40 children), who received polyethylene glycol as maintenance treatment; group 2 (40 children), who received classic treatment in the form of osmotic laxative (lactulose) with or without stimulant (senna-sennosides or senna-glycoside) according to the stool consistency. Results: Our data showed significant improvement in the fecal masses and the number of defecation, fecal pseudo incontinence, painful or hard bowel movement, history of retentive posturing or excessive violation, and large fecal masses in the rectum between group 1 and group 2 after treatment. In group 1, there was a significant improvement in anthropometric measures, CBC parameters, abdominal circumference, and anal fissures and piles after treatment; also, there was a significant decrease in the number of patients that need to continue treatment in group 1 in comparison with group 2 and number of patients that had impaction during treatment. However, there was a statistically significant increase in the number of patients who complained of palatability in group 1. Conclusion: PEG has long-term efficacy in the management of pediatric functional constipation. Significant improvement of results in comparison with the classic treatment (lactulose or senna-lax). Purpose. To maintain the quality of patients care, head nurses (HNs) are confronting various contradicting issues that require the ability to manage polarities and concern coaching behaviors as exploring situations, building alliances, and maintaining communication with nurses. This study aimed to explore the effect of HNs' workplace polarity management educational intervention on their coaching behaviors. Design. A quasiexperimental (pre-posttest) research design was used. Method. This study was conducted at Tanta International Teaching Hospital, on 12 HNs and 300 nurses using three tools: (I) a polarity management knowledge questionnaire; (II) a polarity map evaluation observational checklist; and (III) a nurses' perception of the HNs' coaching leadership behaviors questionnaire. The statistical tests used were as follows: the Kolmogorov-Smirnov test, the significance of the obtained results was judged at the 5% level; the marginal homogeneity test, ANOVA with repeated measures, the Wilcoxon signed ranks test, and the Friedman test. The study was conducted between end of 2021 and beginning of 2022. The educational intervention included 3 sessions: polarity management and the keys to manage it, polarity map (stability versus change), and head nurses' role in managing polarity and how it reflects on their coaching leadership behavior. HNs' ability to practice polarity management was classified into levels according to cutoff points: high >75%, moderate 60–75%, and low <60%. Results. Before intervention (66.7%, 100%) of head nurses had poor knowledge and low practice that changed after 3 months of intervention to be 58.3% and 41.7%, respectively. Before and 3 months after the intervention (70.0%, 33.7%) of nurses perceived that head nurses had a low and a moderate level of coaching, respectively. 70.0% of working nurses reported that they noticed that head nurses' coaching behaviors were low before the educational intervention, but the percentage decreased by half three months after the educational intervention. Conclusion. Implementing the educational intervention for head nurses about workplace polarity management significantly correlated with the nurses' perception of their HNs' coaching behaviors. Background: The novel corona virus is a high contagious disease, declared by World Health Organization (WHO) as a global pandemic in 2020 with profound impact on morbidity and mortality, assessment of outcomes in infected patients and knowledge of prediction of mortality and morbidity are important. We aimed to assess the clinical and laboratory findings in predicting COVID-19 severity and outcome in patients admitted to Suez Canal University Teaching Hospital. Methodology: This cross-sectional prospective study included 500 confirmed PCR COVID-19 infected patients, selected through random sampling. A structured checklist was used to collect patient data. Results: Mean age was 61.8 years, 56.2% were males, 74.8% had comorbidities. Lung involvement was evident in more than 75% on CT, 17.2% had leukopenia, 42.2% had lymphocytopenia between 5 - 10% and 93% of the patients had elevated neutrophil-lymphocyte ratio. 65.8% had elevated D-dimer, and elevated liver and kidney functions were found in 40.6% and 25% respectively. The mortality rate in studied population was 30.2% and it was significantly associated with old age, hypoxemia, having high involvement of the lungs on CT. Decreased WBC count, high D-dimer level and high NLR associated with severity and increased death rate of the disease. Conclusion: The study revealed many findings with impact on the patient's severity and outcome old age, laboratory findings, CT imaging and need to antiviral therapy the most predicting factors of the severity and prognosis of the patient. Depressive disorders affect individual's thoughts, feelings, and social interactions. Enhancing emotional competencies of depressed individuals may alleviate their suffering. Purpose : This study aimed to compare depression severity and emotional intelligence before and after emotional intelligence training in patients with depressive disorders. Methods : A nonrandomized trial (one-group pretest–posttest) research design was applied to 69 patients purposively recruited. The patients' sociodemographic and clinical data were collected. The Beck Depression Inventory-II and Trait Emotional Intelligence Questionnaire-Short Form were completed before and immediately after an 8-weeks of focused weekly group training. Results : A significant improvement in the scores of well-being, self-control, emotionality, and sociability; total emotional intelligence scores; and total depression scores was perceived after training (Z = 5.601, 4.398, 5.686, and 3.516; 4.943; and 2.387, respectively). Implications for Nursing Practice : As emotional intelligence can be learned; it may be a target for interventions when dealing with patients with depressive disorders by strengthening their emotional intelligence. Background Stroke care bundles are evidence-based practices in the management of acute ischemic stroke that are rapidly changing. Early detection of stroke symptoms and prompt treatments can result in a positive outcome. Specialized care in the emergency room during the acute phase and the first week in a neurological ICU or stroke unit for follow-up can enhance the overall prognosis. Aim To evaluate the effect of implementing an initial stroke care bundle on clinical outcomes among patients with acute ischemic stroke. Patients and methods A quasi-experimental research design was used. A convenient sample of 60 critically ill adult patients who were admitted to the emergency care unit and neurology ICU was included. The patients were tested using an initial stroke care bundle during 7 days of admission to the neurological ICU and then followed up another week in the medical ward and 2 weeks in the outpatient clinic. The data were collected using three tools developed by the researchers. Results There were highly statistically significant differences between the control and study groups regarding improving vital signs, neurological status, and hydration status on admission, third and seventh days after admission, and during 3-week follow-up. Moreover, patients with acute stroke who received the initial stroke care bundle had a statistically significant reduction in mortality rate, higher independence, and normal prefeeding assessment compared with those who received routine care on admission, 3 and 7 days after admission, and during 3-week follow-up. Conclusion Implementing an initial stroke care bundle can improve the prognosis of patients with acute ischemic stroke and decrease the number of deaths. Abstract: In this paper we outline some of the major pending research questions that lie ahead in hepatitis C intervention research. We argue why these should be answered with randomised clinical trials and we stress how Egypt and other Arab countries can play an important role in this context. We delineate a number of design issues relevant to recent and future hepatitis C randomised clinical trials and provide insights on how and why randomised clinical trial designed in a pragmatic and evidence-based framework will efficiently answer pending research questions. Background Egypt has the highest prevalence of hepatitis C virus (HCV) infection. Anti-HCV antibodies were detectable in 3% of children in Upper Egypt. Our aim was to evaluate the efficacy of ledipasvir/sofosbuvir for chronic HCV genotype 4 in adolescents with/without hematological disorders and to determine the effect of sustained virological response (SVR) on liver stiffness. Methods Sixty-five adolescents were recruited. There were 3 patient groups: group 1, 44 treatment-naive without hematological disorders; group 2, 6 previously treated; and group 3, 15 treatment-naive with hematological disorders. All patients received sofosbuvir 400 mg/ledipasvir 90 mg per day for 12 weeks. Serum HCV RNA levels were measured before treatment, at week 12, and at 12 weeks after the end of treatment (SVR12). Liver stiffness and the aspartate aminotransferase–platelet ratio index (APRI) score were estimated at baseline and at SVR12. Results SVR12 was 100%. At SVR12, there was a significant improvement in liver stiffness in all groups. The APRI score showed significant improvements in groups 1 and 3 (P  < .001 and P  = .004, respectively). The treatment was well tolerated, with minimal and self-limited side effects. Conclusions Treatment of chronic HCV in adolescents using ledipasvir/sofosbuvir was effective, with a cure rate (at SVR12) of 100%. Significant improvement in liver stiffness was found in all groups. Introduction: The rate of secondary attacks of SARS-COV-2 is high among household close contacts. Social distancing, isolation and infection control measures are important for preventing exposure to infection, but insufficient. Aim: The study aimed to evaluate possible role of oral ivermectin as a chemoprophylaxis in asymptomatic family close contacts with COVID-19 patients. Materials and Methods: A prospective interventional randomised open label-controlled study was conducted (registered at clinicaltrials.gov; NCT04422561) during June and July 2020. Two arms were designed according to use of ivermectin. In ivermectin arm, contacts received ivermectin according to Body Weight (BW) on day of the diagnosis of their index case. The nonintervention group received no treatment. Both groups were followed-up for two weeks for development of symptoms suggestive of COVID-19. Results: Ivermectin group included 203 contacts (to 52 index cases) aged 39.75±14.94 years; 52.2% were males. Nonintervention group included 101 contacts (to a total of 24 index cases) aged 37.69±16.96 years, 49.5% were males. Fifteen contacts (7.4%) developed COVID-19 in the ivermectin arm compared to 59 (58.4%) in the nonintervention arm (P <0.001). The protection rate for ivermectin was more prominent in contacts aged less than 60-year-old (6.2% infected compared to 58.7% if no treatment). Ivermectin in the protection against SARS-CoV-2 infection had an OR of 12.533 and 11.445 (compared to nontreatment) in both univariate and multivariate models, respectively. Side effects of ivermectin were reported in 5.4%; they were mild. Conclusion: Ivermectin is suggested to be a promising, effective and safe chemoprophylactic drug in management of COVID-19. Objective: The aim of the study was to assess whether vaginal administration of misoprostol before copper intrauterine device (IUD) insertion increased the success of the procedure among parous women with previous insertion failure. Methods: A single-centre, parallel-group, double-blind, placebo-controlled, randomised clinical trial was conducted at Ain Shams University Maternity Hospital, Cairo, Egypt, between October 2015 and August 2016. The study comprised 90 parous women undergoing TCu380A IUD insertion after a failed attempt. A computer-generated list of random numbers was used to assign participants to receive either misoprostol 200 μg or a placebo tablet, applied vaginally 10 h and 4 h prior to the second attempted IUD insertion, without ultrasound guidance. The primary outcome was the success of IUD insertion. Secondary outcomes were to establish the effect on insertion success of cervical dilation, cervical softening and previous mode of delivery. Results: Forty-two women (93.3%) in the misoprostol group and 24 women (53.3%) in the placebo group had a successful IUD insertion (p < .001). Cervical dilation was required in 24 women in the misoprostol group and 44 women in the placebo group. Misoprostol application significantly increased insertion success in women with previous caesarean delivery (p < .001) but did not affect insertion success in women with previous vaginal delivery (p = .481). Conclusion: Vaginal misoprostol before IUD insertion in parous women with previous insertion failure increased the rate of successful insertion, particularly in women with previous caesarean delivery. This study aimed to evaluate the left ventricular (LV) functions in a cohort of children with Graves' disease (GD). This is a cross-sectional case-control study. It included 36 children with GD and 36 healthy children matched for age and gender. Thyroid hormones (TSH, FT4, and FT3) and anti-thyroid autoantibodies [anti-thyroid peroxidase (anti-TPO), thyrotropin receptor (TRAbs), and thyroglobulin antibodies] were measured. Conventional and tissue Doppler imaging (TDI) echocardiographies were used to assess left ventricular systolic and diastolic functions. LV mass index (LVMI) and myocardial performance index (MPI) were also measured. Compared to healthy children, conventional echocardiography of patients with GD revealed higher LVMI (P = 0.001) indicating LV hypertrophy but normal LV functions while TDI revealed lower Em/Am ratio indicating LV diastolic dysfunction (P = 0.001). Significant correlations were reported between FT4 with LVMI (P = 0.05), Em/Am (P = 0.01), and MPI (P = 0.01). In multivariate analysis, a positive correlation was identified between FT4 with MPI (OR = 1.17; 95% CI = 1.09-1.15; P = 0.001).Conclusions: Children with newly diagnosed GD may have significant subclinical changes in LV structure and function (diastolic and global). TDI is more sensitive than conventional Doppler in detecting LV dysfunction. These findings highlight the importance of early monitoring of children with GD for left ventricular mass index and diastolic function. We analyze the current situation of clinical research regulatory overview in Egypt to acknowledge the strengths and define the frailties, thus inciting relevant stakeholders to find solutions and bring about change. Uniquely, Egypt possesses favorable factors ideal for a dynamic clinical research market: accessible subjects, a mosaic panel of research areas, and incomparably low cost. Yet, with a majority of participant population needy and illiterate, and in absence of robust legislative constraints, concerns are increasingly being raised, whether ethical pitfalls of clinical research are adequately addressed, and whether the safety and the rights of subjects are constantly prioritized and maintained. In a descriptive explicit literature review, we identify, review, and interpret findings from recent publications backed up by existing data derived from current declared regulations and personal experience in the field. Inferences were made that despite an affiliation to international referential guidelines, real settings implementations are far from being consistent or adherently conforming. Background: Safe and radical excision of pediatric fourth ventricular tumors is by far the best line of management. Pediatric fourth ventricular tumor surgery is a challenge for neurosurgeons. The aim of the study is to present the authors' experience and to evaluate the possible benefits of neuro-navigated intraoperative ultrasonography (NIOUS) during the surgery of fourth ventricular tumors in children. Methods: Nonrandomized clinical trial study was conducted on 60 children with fourth ventricular tumors who were treated at Children's Cancer Hospital-Egypt. Mean age was 5.2 (±2.6) years. Thirty cases were operated upon utilizing the conventional microneurosurgical techniques. Another 30 cases were operated upon utilizing the NIOUS technique. Results: Total tumor excision was achieved in 29 cases (96.7 %) of NIOUS group versus 24 cases (80 %) in the conventional group. Mean operative time NIOUS group was 150 min [standard deviation (SD) = 18.28) versus 140.6 min (SD = 18.6) in the conventional group ( p value = 0.055). The mean operative blood loss was 67.5 ml (SD = 17) in NIOUS group versus 71 ml (SD = 15.4) in the conventional group. Postoperative cerebellar mutism occurred in one case (3.3 %) of NIOUS group versus in six cases (20 %) of the conventional group. Conclusions: Integration of navigated intraoperative ultrasonography in surgery of pediatric fourth ventricular tumors is a useful technology. It safely monitors maximum stepwise tumor excision. It is associated with less operative morbidity without significantly added operative time. It is a real-time, cost-effective, easily applicable, and easily interpretable tool that could substitute the use of intraoperative MRI especially in pediatric neurosurgery. [ABSTRACT FROM AUTHOR]

Objectives: (1) To investigate Egyptian obstetricians' views towards cesarean delivery on maternal request, (2) to investigate Egyptian obstetricians' views towards some of the 'potentially neglected' or controversial obstetrical skills or maneuvers as external cephalic version (ECV), fetal scalp pH measurement or tubal ligation during CS and (3) to examine the effect of professional level on the above factors. Study design: This is a descriptive study performed at the 8th annual Obstetrics and Gynecology conference of Suez Canal University held at Ismailia city in Egypt in June 2011 via a structured self administered questionnaire. Questionnaire was distributed to 223 conference attendants from the three professional levels (consultants, specialists and registrars) working at the two major institutions in Egypt: University and Ministry of Health. The structured questionnaire was based on informed opinion and professional guidelines. In total, 167 (75%) completed the questionnaire. Results: Cesarean delivery on maternal request was accepted by 66% of the studied group and acceptance was significantly higher among consultants. There was no difference in all physicians' practices of cesarean section in both private and public settings. Limited access to medical equipment such as cardiotocogram (CTG) was shown in consultant group reflecting improper private sector preparations. The study revealed that 59% of obstetricians accepted vaginal breech delivery, and only 14% would consider ECV. Fetal scalp pH taking in cases of abnormal CTG was accepted by only 16.3% and 49% rejected the practice of instrumental delivery. There were significant differences among the three professional and the two institutional groups regarding these attitudes. There were different views regarding tubal sterilization during CS. Conclusions: Lack of knowledge, the need to improve some clinical skills and some professional attitudes may shed light on rising CS rates in Egypt. [ABSTRACT FROM AUTHOR]

Objective: To assess the impact of the non-pneumatic anti-shock garment (NASG) on maternal outcome following severe obstetric hemorrhage.Methods: A non-randomized pre-intervention/intervention study was conducted in 2 tertiary hospitals in Egypt from June 2006 to May 2008. Women with obstetric hemorrhage (estimated blood loss >or=1000 mL and/or >or=1 sign of shock [systolic blood pressure <100 mm Hg or pulse >100 beats per minute]) were treated with either a standardized protocol (pre-intervention) or a standardized protocol plus the NASG (intervention). The primary outcome was extreme adverse outcome (EAO), combining maternal mortality and severe morbidity (cardiac, respiratory, renal, or cerebral dysfunction). Secondary outcomes were measured blood loss, urine output, emergency hysterectomy, and (individually) mortality or morbidity. Analyses were performed to examine independent association of the NASG with EAO.Results: Mean measured blood loss decreased from 379 mL pre-intervention to 253 mL in the intervention group (P<0.01). In a multiple logistic regression model, the NASG was associated with reduced odds of EAO (odds ratio 0.38; 95% confidence interval, 0.17-0.85).Conclusion: The NASG, in addition to standardized protocols at tertiary facilities for obstetric hemorrhage and shock, resulted in lower measured blood loss and reduced EAO. Background: Sacrococcygeal pilonidal sinus disease (PSD) is an infection of the skin and subcutaneous tissue at the upper part of the natal cleft of the buttocks. Excision and healing by granulation "lay-open" method is still more preferable than other methods of midline closure or using flaps but the healing time is lengthy. The present study was performed to assess the healing promotion effect of platelet-rich plasma (PRP) on the pilonidal sinus wounds treated by the lay-open method.Methods: One hundred patients suffering from PSD were randomly divided into two groups, they were treated by the lay-open method, at General surgery department, Kafr El-Sheik University hospital, Egypt, during the period from December 2018 to December 2019. Group (A) was adopted the regular dressing postoperatively, while group (B) was treated with PRP injection into the wound at 4 and 12 postoperative days.Results: Accelerated rate of wound healing was detected in group (B) in day 10, with a significant difference detected in days 15, 20, 25 and 30 postoperative, with a mean time of complete healing 45 ± 2.6 days in group B, while it was 57 ± 2.4 days in group A with a p-value of 0.001 which indicates considerable effect in the treated group.Conclusions: PRP injection is an effective new technique in accelerating the healing of pilonidal wound after surgery, with a significant decrease in post-operative pain, complications and an early return to work.Trial Registration: retrospectively registered.Trial Registration Number: 12/35/1016 issued on December 2018 from the Institution Review Board at Kafr El Sheikh University.
